Intraosseous ganglion is a benign cystic bony lesion containing viscous and mucinous gelatinous material, which is usually seen in middle-aged individuals. It is similar to soft-tissue ganglion cysts present in other parts of the body. We, hereby, report a rare case of idiopathic intraosseous ganglion of the tibia in a young adult who presented with chronic pain in his lower leg. The patient was subjected to X-ray and MRI investigations and managed by excisional curettage of the lesion and histopathological examination. On histopathology, the diagnosis of intraosseous ganglion was confirmed. On 2-year follow-up, the patient was doing well without any complication and recurrence. This report describes the diagnosis and successful management of intraosseous ganglion at a very rare site in a young individual. More in-depth studies are required to understand the predisposing factors, underlying pathology, early diagnosis, and management of this lesion.

Fibro-osseous and cystic tumors are some of the more common tumors encountered in bone pathology. Although there is significant histologic overlap, fibro-osseous tumors of bone have distinct clinical, radiologic and pathogenetic features. Similarly, cystic lesions have characteristic radiologic and pathologic features that help distinguish them from tumor of bone that can have secondarily cystic components. More recently, specific genetic abnormalities have been identified in a subset of fibro-osseous and cystic tumors, such as fibrous dysplasia and aneurysmal bone cyst. Intraosseous ganglions (IOGs) are actually quite common but one spanning two adjacent carpal bones is uncommon. We report a case with an IOG spanning the scaphoid and lunate, which was treated surgically. A 16-year-old right-handed female noticed left wrist pain that started spontaneously five years previously. Physical findings indicated carpal instability in the left wrist. Posteroanterior radiographs of the left wrist showed small cysts in the lunate and scaphoid, while the lateral radiograph revealed volar flexion of the lunate. Bone curettage was performed using sharp curettes, and due to the physical findings of carpal instability, temporary scapho-trapezoidal joint fixation was done using two Kirchner wires. Two years post-surgery, wrist pain had significantly improved and carpal instability findings disappeared. Computed tomography revealed no obvious collapse of carpal bones and expansion of bone defects in the lunate and scaphoid. Background: Cystic lesions of the carpal bones are rare entities that are infrequently reported in the literature. Scaphoid intraosseous cystic lesions represent a rare subset of carpal bone cysts. This review aims to summarize the available evidence on the evaluation and treatment of scaphoid cystic lesions to help guide clinical management. Methods: Systematic electronic searches were performed using PubMed, Ovid, and Embase databases. Studies included were graded for their risk of bias. Pooled descriptive statistics were performed on incidence, etiology, physical exam findings, treatment, and follow-up. Results: A total of 38 patients representing 41 scaphoid cystic lesions were pooled from 27 articles. Patients presented with wrist pain without fracture (n = 27), pathological fracture (n = 9), swelling only (n = 1), or were asymptomatic (n = 4). Cystic lesions of the scaphoid were initially revealed on imaging with radiographs alone (n = 22), radiographs in combination with computed tomography (CT) (n = 10) or magnetic resonance imaging (n = 6), CT alone (n = 1), or using all 3 modalities (n = 2). Intraosseous ganglia were identified most frequently (n = 31), followed by "bone cyst-like pathological change" (n = 3), unicameral bone cysts (n = 2), aneurysmal bone cysts (n = 2), primary hydatid cysts (n = 2), and cystic like changes post fall (n = 1). Treatment modalities included curettage and bone graft (n = 39) or below-elbow cast (n = 2). On follow-up (average of 21.3 months; n = 40), all patients improved clinically after treatment and were found to have full wrist range of motion without pain (n = 31), slightly reduced grip strength (n = 3), limited range of motion (n = 2), or persistent mild discomfort (n = 2). Conclusions: Scaphoid cystic lesions are most commonly intraosseous ganglia, but can include other etiologies as well. The main presenting symptom is radial wrist pain that usually resolves after treatment. The presence of intracarpal cystic lesions should be considered in the differential diagnosis of wrist pain. Intraosseous ganglion cyst is a rare bone tumor and the lesion could often be missed. The diagnosis could be delayed so proper radiologic investigation and index of suspicion is necessary .Differential diagnoses of painful cystic radiolucent carpal lesion are osteoid osteoma, osteoblastoma and intraosseous ganglion. Curettage of the scaphoid lesion and filling of void with bone graft provides good functional outcomes. The cyst contains mucoid viscous material without epithelial or synovial lining. We present a case of 30 years old male with intraosseous ganglion cyst of scaphoid which was treated with curettage and bone grafting. The case of a 25-year-old man with a ganglion cyst of the medial aspect of the ankle has been presented. Biopsy was taken during the surgery and sent for histopathology, which showed cystic area lacking epithelial lining filled with mucoid material. Benign intraosseous osteolytic lesions of the glenoid are very rare. The present study reports on three cases of symptomatic intraosseous osteolytic lesions of the glenoid in which surgical interventions were made. Of the three, two cases presented with intraosseous ganglion and one case with fibrous dysplasia. In all the cases, the lesion was located at the posteroinferior portion of the glenoid, and it seems to be related to posterior shoulder pain. If intraosseous osteolytic lesions have symptoms or the risk for chondral defects or cortical breakage, surgical intervention is needed and bone curettage with or without bone grafting will be a useful treatment option. Purpose: To describe the technique and clinical outcome of percutaneous injection of bone cement in the treatment of symptomatic para-articular intraosseous cysts. Materials and methods: Five patients (three men, two women; mean age 35 years) with painful para-articular intraosseous cysts were treated by percutaneous injection of bone cement under combined fluoroscopic and computed tomography (CT) guidance. The lesions were all located in weight-bearing bones, involving the acetabulum, proximal tibia, distal tibia, talus, and calcaneus, respectively. Results: The average amount of bone cement injected was 2.1 ml (range, 0.6-3.5 ml). Calcium phosphate cement was used in four cases and acrylic cement in one case. There were no immediate or delayed complications. Full pain relief was obtained between 1 and 4 weeks after treatment. All patients made a complete recovery and were pain-free at their last visit. Conclusions: Percutaneous injection of bone cement was a safe and efficient technique in the management of symptomatic para-articular intraosseous cysts in our population. [ABSTRACT FROM AUTHOR]

Abstract: We report 5 cases of a localized lesion (4 with osteomyelitis and 1 with an intraosseous ganglion cyst) that were treated successfully by medulloscopy. Medulloscopy is a standard arthroscopic technique for visualizing the intramedullary canal of the tibia. Two portals were created to allow easy access and triangulation to the lesion, one for the 5-mm arthroscope and the other for the instrument. Debridement, irrigation, and resection of the sequestrum were performed for the cases with osteomyelitis, and the intraosseous ganglion cyst was treated with biopsy, debridement, and autogenous bone grafting. All cases were treated successfully with medulloscopy and did not show recurrence at the last follow-up. In addition, there were no complications related to the procedure. [Copyright &y& Elsevier]

Abstract: Intraosseous ganglions occurring in the hand are rare. Most reported cases involve the lunate or the scaphoid. This report presents a very extremely rare case of intraosseous ganglion arising in the middle phalanx of an index finger. The aim of this study is to report the spectrum of imaging findings of intraosseous ganglia (IG) with particular emphasis on the radiographic and magnetic resonance (MR) features. Forty-five patients with a final diagnosis of IG were referred to a specialist orthopaedic oncology service with the presumptive diagnosis of either a primary or secondary bone tumour. The diagnosis was established by histology in 25 cases. In the remainder, the imaging features were considered characteristic and the lesion was stable on follow-up radiographic examination. Radiographs were available for retrospective review in all cases and MR imaging in 29. There was a minor male preponderance with a wide adult age range. Three quarters were found in relation to the weight-bearing long bones of the lower limb, particularly round the knee. On radiographs all were juxta-articular and osteolytic; 74% were eccentric in location, 80% had a sclerotic endosteal margin and 60% of cases showed a degree of trabeculation. Periosteal new bone formation and matrix mineralization were not present. Of the 29 cases that underwent MR imaging, 66% were multiloculated. On T1-weighted images the IG contents were isointense or mildly hypointense in 90% cases. Forty-one per cent of the cases showed a slightly hyperintense rim lining that enhanced with a gadolinium chelate. Thirty-eight per cent were associated with soft tissue extension and 17% with a defect of the adjacent articular cortex. Fifty-five per cent showed surrounding marrow oedema on T2-weighted or STIR images and two cases (7%) a fluid-fluid level prior to any surgical intervention. The authors contend that it is semantics to differentiate between an IG and a degenerate subchondral cyst as, while the initial pathogenesis may vary, the histological endpoint is identical, as are the imaging features apart from the degree of associated degenerative joint disease. IGs, particularly when large, may be mistaken for a bone tumour. Correlation of the typical radiographic and MR imaging features will indicate the correct diagnosis and obviate the need for biopsy. Three cases of periosteal ganglia of long bones are presented. These lesions are produced by mucoid degeneration and cyst formation of the periosteum to produce external cortical erosion and reactive periosteal new bone. They are not associated with a soft tissue ganglion or an intraosseous lesion. They may radiologically mimic other periosteal lesions or soft tissue neoplasms which erode bone. We report a rare occurrence of attritional rupture of both flexor tendons of the right index finger in a 71 year-old man with a pathological fracture of the lunate arising from an intraosseous ganglion. [Copyright &y& Elsevier]

Abstract: Intraosseous ganglion cysts around the wrist joint are more common in lunate and scaphoid. We report a rare ganglion cyst of the radial styloid process in a professional horse rider. The cyst was probably a consequence of an old injury to the scaphoid. The patient underwent arthroscopic debridement of the lesion and was asymptomatic at 2 year follow up. No similar constellation of intraosseous ganglion of the radial styloid process and scaphoid non-union were found in the English literature. [Copyright &y& Elsevier]
