An intestinal stoma creation is one of the most common surgical procedures. Despite benefits, a stoma can have serious effects on a patient's quality of life. Multiple dimensions of everyday life can be affected such as social life, body image, as well as ability to participate in some religious practices, with some faith groups being disproportionately affected. This study sought to understand the extent to which faith is addressed during these sessions given the integral role it plays for some patients. A survey was developed based on literature review and expert consultation. This was distributed to ostomy certified nursing staff, inflammatory bowel disease gastroenterologists and colorectal surgeons caring for patients requiring a permanent stoma at a high-volume academic institution. Follow-up semistructured interviews were conducted to delve deeper into themes identified in the surveys. The overall survey response rate was 57%. Only 35% reported training on how stomas interface with religious practices. Religious services were either rarely or never present during preoperative counseling discussions. During interviews, it was noted that religious beliefs often impact after care but are not always discussed during preoperative sessions. Interviewees found these conversations to be difficult with minimal support or direction on how to navigate them. Although very important, health-care providers are not including faith-based issues surrounding stomas in perioperative counseling partly due to lack of training or awareness of existing support systems. BACKGROUND: Intestinal stomas are commonly constructed in an emergency as well as elective setting for a variety of indications. Historically associated with a high morbidity, evolution of skills on the part of the surgeon has lead to better understanding of the indications, technique of construction and management of a stoma. This study aims to evaluate the above-mentioned parameters and hence improve the outcome of patients undergoing a stoma. AIM AND OBJECTIVE: 1. To study the various indications of intestinal stomas. 2. To study the techniques of intestinal stomas. 3. To study the complications of intestinal stomas and their management. 4. To study the overall compliance of patients in whom a stoma was constructed. PATIENTS AND METHODS: 50 patients admitted in M K C G Medical college and Hospital Berhampur and later operated and managed with a stoma were closely followed up from the date of admission to the date of discharge and the various parameters were studied. RESULTS: A total of 50 patients were included in the study who underwent stoma formation at this hospital from November 2022 to October 2023. A total of 17 patients, for whom the stoma was constructed, had hollow viscous perforation. Although the site of the perforation varied, the intraoperative findings warranted a stoma in these patients. Loop ileostomy was the most commonly performed surgery (n=16) amongst patients who presented with hollow viscous perforation as the perforation was most commonly located in the ileum or the base of appendix. Only one patient (n=1) underwent a transverse loop colostomy for a perforation in the sigmoid colon. Malignant bowel obstruction was the second most common cause (n=8). Patients with malignant bowel obstruction needed a stoma either for decompression or diversion (n=4 each). All patients with decompression were done a transverse loop colostomy whereas patients requiring diversion were done a loop ileostomy, end ileostomy or an end colostomy according to the level and severity of obstruction. Benign cause of intestinal obstruction was the third most common etiological factor associated with construction of a stoma. However, it is worth noting that benign diseases accounted for 78% (n=39) of all causes of stoma placement. CONCLUSION: Construction and management of stoma was associated with a few complications. Most patients however tolerated the procedure well and the overall compliance was satisfactory. Loop ileostomy was the commonly constructed stoma and the one associated with most complications. This is a retrospective study of 264 intestinal stomas performed in the surgical unit of the Emergency Clinical Hospital "Bagdasar Arseni", Bucharest, within a 7-year period (2015-2021) aiming to evaluate their evolutive complications, risk factors, management and prevention strategies. Material and method. Colostomies: 218 (82.57%) cases, ileostomies 46 (17.43%) cases. Temporary stomas (103 cases or 39.02%) were isolated stomas of discharge in 45 cases and associated with other colorectal procedures in 58 cases. Postoperative complications included general systemic complications in 60 (22.72%) cases and local complications specific to stomas in 84 (31.81%) cases and common to abdominal surgery in 94 (35.60%) cases, which were solved by reoperation in 51 cases, with a reintervention rate of 19.31%. Stoma closures were performed in 34 (33.0%) of the 103 patients with temporary stomas. Of these, 26 (25.24%) patients died in the early postoperative period (< 30 days), the remaining 60 patients refused reintervention or were lost to follow-up. Conclusions. Faecal diversion still represents a therapeutic option for a wide range of benign or malignant digestive or extra-digestive abdominal diseases performed in emergency or scheduled surgeries, mostly for colorectal cancer and its complications. Objection: This study was conducted to examine the effect of self-efficacy levels on stoma adaptation in patients with intestinal stoma. Methods: The study was planned as descriptive and analytical and was carried out in the university adult hospital general surgery stoma and wound care unit. Sixty-two patients with intestinal stoma who met the admission criteria were included in the study. Ethics committee approval, institutional permission and patient consent were obtained for the study. Data were collected from June to September 2021 using the Descriptive Feature Information Form, the Self Efficacy Scale for Individuals with Ostomy, and the Ostomy Adjustment Inventory (OAI-23). Data were analyzed by number, percentage, mean, standard deviation, Mann Whitney U, t test, ANOVA, Pearson's correlation, and linear regression analysis tests. Results: The mean age of the patients with intestinal stoma who participated in our study was 53.12 ± 12.30 years; 71% of them were women, 83.9% were married, and 32.2% were primary and secondary school graduates. The duration of stoma was 8.45 ± 4.69 months, 80.6% of them were opened due to cancer, and 54.8% of them had temporary colostomy. There was a weak and significant correlation between the duration of stoma of the patients and their self-efficacy and stoma adaptation, and as their self-efficacy levels increased, their stoma adaptation increased (p <.05). Conclusions: Self-efficacy is one of the important factors affecting patients' adaptation with the stoma. For this reason, it is recommended to plan trainings to support the self-efficacy levels of patients and to conduct interventional studies in this direction. [ABSTRACT FROM AUTHOR]

Our objective was to examine oncologic outcome in women with advanced epithelial ovarian cancer (aEOC) receiving an intestinal stoma (IS) at the time of cytoreductive surgery (CRS), probability of stoma reversal (SR) and variables affecting odds of SR. This population-based cohort study included all women diagnosed with aEOC between 2009 and 2018 in the Stockholm/Gotland Region of Sweden. The association between IS formation at CRS and survival was analyzed with proportional hazards regression yielding hazard ratios (HR) with 95% confidence intervals (CI), adjusted for predefined confounders. Cumulative incidence functions, with death or recurrence as competing risk, were used to estimate chance of SR. The association between clinical factors and SR was analyzed with logistic regression yielding odds ratios (OR) with 95% CI. The final analysis included 888 women undergoing CRS for aEOC. Of these, 129 (14,5%) received an IS of which 74% (n = 95) were defunctioning and 26% (n = 34) permanent. IS was associated with an increased hazard of death (HR 1.30, CI 95%, 1.05–1.61; p = 0.02) in the univariate analysis, however not in the adjusted analysis. The probability of SR of defunctioning IS within 2 years was 48% (95% CI, 38–58). Median time to SR was 10 months. High surgical complexity score (SCS) was associated with increased odds of reversal (OR 3.43, 95% CI, 1.06–11.05; p = 0.04). IS formation does not seem to affect prognosis in women with aEOC. Aims and Objectives: The aim of the study was to study various type of stoma and their indication and to identify the various complication encountered that occurs after the construction of intestinal stomas to assess the ways in which these complication can be minimized and managed in a better way. Materials and Methods: This was a prospective and observational study conducted in the Department of General Surgery, R.G.KAR Medical College and Hospital both the emergency and elective department. The study was approved by the Institutional Ethics Committee. Results: From the study, it was observed that among the study population, most were male 32 (80%) than female 8 (20%). There were two peak age group found to undergo stoma surgery most of which is 31--40 years consisting of 25 (27.5%) and 51--60 years consisting of 25 (27.5%). Most of the stoma surgery was found to be done in emergency 31 (77.5%) than in elective 9 (22.5%) setting. The most common cause of stoma formation was found to be hollow viscous perforation 12 (30%). Among the type of stoma ileostomy is more common than colostomy. Loop stoma formations are more common than end stoma and double barrel stoma. The most common complication following stoma surgery was found to be skin excoriation 30%. Conclusion: Stoma surgery was associated with a high rate of complication irrespective of the surgical expertise, surgical setup, and per operative planning resulting in higher morbidity. With meticulous dissection, post-operative stoma care, early detection, and management of complications, also patient education helps to achieve good outcomes following stoma surgery. Intestinal stoma site selection is one of the key elements in preoperative planning and nursing care of patients with intestinal diseases. It is a medical procedure that should be performed prior to any scheduled surgery potentially resulting in a stoma. In emergency surgeries this could be difficult, but an attempt should always be made. Preoperative stoma site selection allows the optimal creation and location of the stoma. The priority is that the stoma should be easily accessible and visible to the patient. A correctly located stoma reduces postoperative complications and improves patients’ quality of life. This work discusses the process and principles of selecting the intestinal stoma site in adult patients prior to scheduled or emergency surgeries. It includes a review of the current literature on this subject. The analysis involved scientific publications from the period between 1 January 2000 and 31 August 2021. Literature searches were conducted in the PubMed and Google Scholar databases, using a mixed combination of the following English and Polish terms: “ostomies”, “intestinal stoma”, “education”, “stoma site selection”, “stoma site marking”, “stoma siting”, “stoma complications”, “peristomal skin complications”, “stomia jelitowa”, “model opieki”, “edukacja”, “wyznaczanie miejsca stomii”, and “powikłania stomijne”. Sixty scientific publications were used to analyse the subject matter of this work.

Background: This systematic review was performed to characterize the landscape of research conducted in patients with intestinal stoma (IS) and highlight unmet needs for clinical research in Crohn's disease (CD) and IS., Methods: We searched ClinicalTrials.gov from inception to May 25, 2022, to identify clinical trials assessing interventions in patients with an IS, as well as those with an IS and CD. Studies were grouped according to type of intervention. We excluded observational studies with no treatment arm., Results: A total of 253 studies were included in the final analysis. Most studies investigated devices (n = 122 [48.2%]), or surgical procedures (n = 63 [24.9%]), followed by behavioral interventions (n = 30 [11.8%]), drugs (n = 20 [7.9%]), dietary interventions (n = 2 [0.8%]), skin care products (n = 2 0.8%]), and others (n = 14 [5.5%]). A total of 50.9% (n = 129) of studies had completed recruitment, enrolling 11 116 participants. Only 6 studies (surgery: n = 3; physiological studies: n = 2; drugs: n = 1) exclusively included patients with inflammatory bowel disease (IBD), and 16 studies commented that patients with IBD were excluded in their eligibility criteria. No study assessed efficacy of drugs in patients with CD and IS. Approximately one-quarter of studies (n = 65 of 253) included quality of life as an outcome measure., Conclusion: There is a paucity of research in IBD patients with IS, with the majority focusing on devices and surgical procedures. There have been no drug trials evaluating efficacy in patients with CD and IS. Self‐esteem is one of the most important factors affecting the patient's mental health. It is suggested that factors such as emotional intelligence could be related to self‐esteem. This study seeks to determine the correlation between emotional intelligence and self‐esteem in patients with an ostomy. Design This was a descriptive‐correlational study. Methods This study was conducted on 155 patients with intestinal stoma referring to the selected hospitals affiliated to Iran University of Medical Sciences and Iranian Ostomy Association in 2018. The patients were selected using convenience sampling method. The study tools included demographic characteristics form, Rosenberg self‐esteem scale and Schutte emotional intelligence questionnaire. Data analysis was performed in SPSS v.16 using descriptive and inferential statistics, including variance analysis and independent t test. Introduction: Intestinal stoma following emergent abdominal surgery is a common surgical necessity. Caring about stoma creation is greatly affecting patient physical and psychological health. Many risk factors are contributing incidence of local stoma complications. Patients and Methods: 138 eligible patients were enrolled in this retrospective study. Patients characteristics and surgical variables were studies and the co-incidence of different early local stoma complication were reported to detect risk factors for developing stoma complications. Results: superficial mucocutaneous separation was the commonest complication (11.59%). Significant higher incidence of complications rate were found in patients with colorectal malignancies (40.57%). Loop colostomy has higher complications rate (55.17%) when compared with Hartmann's colostomy or ileostomy. Background: Intestinal stoma is an opening for fecal diversion. The purpose of the present study was to identify indications for commonly performed intestinal stomas and to study complications related to it. Methods: This is a retrospective study was carried out in surgical unit of Guru Gobind singh medical college, Faridkot from April 2019 to March 2021. All patients were admitted through emergency and OPD basis and underwent surgery for various reasons and were followed up to note any complication which resulted in the creation of intestinal stomas. The results were collected, analyzed and compared with other studies. Results: Out of 100 patients, 73were males and 27 were females. The mean age was 39.4 years with a range of 16-75 years. 96 stomas were made in emergency and only 4 in routine. There were 72 cases of ileostomy. Out of these, 64 (84%) were loop ileostomy. The most common type of stoma made was loop ileostomy (64%) followed by sigmoid colostomy (14%). Main indication for a stoma formation was enteric perforation (40%) followed by Koch’s abdomen (16%). Of the various complications encountered with intestinal stoma, peristomal excoriation (32%). Conclusions: Inspite of vast exposure of general surgeons towards stoma formation the complications are inevitable. Early detection of complication and its timely management is the keystone. The creation of an intestinal stoma remains an integral part of many surgical procedures, performed mainly in colorectal cancer treatment. The appropriate surgical technique and the placing of a stoma in the correct anatomical area increase the chance of therapeutic effects and good quality of life. However, the occurrence of skin lesions around the stoma is relatively frequent. Such lesions can occur for mechanical, chemical, allergic, infectious, or genetic reasons. They require immediate diagnosis and implementation of an appropriate treatment plan of the underlying disease. It is particularly important to select appropriate and non-standard stoma devices and to take controlled care and self-care over a stoma. The article provides a comprehensive description of the observed diseases and skin lesions around an intestinal stoma included in clinical classification and presentation. Aims and Objectives: The aim and objective of the study was to identify, compare, and synthesize published qualitative evidence to have in-depth understanding of experiences of patients living with intestinal ostomy. Background: Over the past two decades, there have been numerous qualitative studies published depicting intense experiences of stoma patients brought about by the need to adapt with changing conditions following stoma creation. Synthesizing the findings of these studies can improve the understanding among health-care providers of needed support and care for ostomates. Design: This was a qualitative meta-synthesis. Methods: Published articles were identified from the Medline, CINAHL, SciELO, PsycINFO, PubMed, WOS, Google Scholar and a hand search through selected journals published since 2000, and from references lists. Thirteen articles were selected using the predefined criteria. Results: A total of 222 patients, aged between 14 and 83 years from 13 studies, were identified for data collection. Ninety-seven were male and the rest of them were female. The themes identified were physical problems, psychological issues, social relations, environmental impact, and coping and adaptation to stoma. Conclusions: The findings of the study identified numerous challenges and limitations in the life of patients with stoma. However, improving their skills of using problem-solving coping strategies and their interactions with other ostomates can help them to live a better and well-adjusted life. [ABSTRACT FROM AUTHOR]

Aim: This study aimed to determine the correlation between self-efficacy and self-esteem in patients with an intestinal stoma. Methods: A descriptive-correlational study was conducted with 155 patients with an intestinal stoma. Participants were selected using convenience sampling. Data were collected using demographic questions, the stoma self-efficacy scale and the Rosenberg self-esteem scale, and analysed using descriptive and analytical statistics. Findings: Seventy-nine men and 76 women took part; the 50–70 years age group had the highest proportion of participants. Regarding diagnosis, 52.26% had cancer and 45.81% had inflammatory bowel disease or related conditions. Pearson's correlation coefficient indicated a positive, significant correlation between total self-efficacy and its dimensions with self-esteem (P<0.001; r=0.54). Conclusion: Self-efficacy is positively correlated with self-esteem in patients with an intestinal stoma. Introduction: Cancer is perceived as one of the most stressful diseases, which leads to cognitive, emotional, and behavioural changes. Adapting to the disease is a process of adopting a strategy and style of coping with the disease that mobilises patients to struggle with the disease, and this affects their quality of life. Aim of the study: Assessment of the psychological functioning of people diagnosed with cancer and their adaptation to functioning in society. Material and methods: The study involved 100 patients treated surgically, including colorectal cancer (50%) and breast cancer (50%), in four clinical hospitals in Łódź. The research tools were as follows: the authors' own demographic survey on sociodemographic variables and the Mini-MAC scale. Statistical analysis was performed using the U Mann-Whitney, Kruskal-Wallis, and ² tests. The relationships were considered statistically significant when p < 0.05. Results: The study involved 28 men and 72 women, of whom 50 were women after mastectomy and 22 were women with a stoma. Over half (56%) were professionally active. Most respondents (56%) were diagnosed a year ago. When examining demographic data, no significant correlations were found. A statistically significant difference was shown only between the place of residence and level of education, and coping strategy. Demographic factors also had a modest impact on coping styles. A significant correlation was observed when examining the place of residence and level of education. Conclusions: In patients with intestinal stoma, as well as women after mastectomy, in their strategy of coping with cancer, they prefer a fighting spirit and positive re-evaluation. Most of the respondents had a constructive style. A destructive style was slightly more common in people living in rural areas. Background and Objectives: Intestinal stomas are commonly constructed in an emergency as well as elective setting for a variety of indications. Historically associated with a high morbidity, evolution of skills on the part of the surgeon has lead to better understanding of the indications, complications and management of a stoma. This study aims to evaluate the above mentioned parameters and hence improve the outcome of patients undergoing a stoma. Method 50 patients admitted in MMIMSR, Mullana (Ambala) and later operated and managed with a stoma were closely followed up from the date of admission to the date of discharge and the various parameters were studied. Results The indications, technique, complications and its management were studied in detail by following patients in person or through phone and the results were analyzed in detail. Interpretation and Conclusion Construction and management of stoma was associated with a few complications. Most patients however tolerated the procedure well and the overall compliance was satisfactory. Loop ileostomy was the commonly constructed stoma and the one associated with most complications. Aim of the study: Analysis of the impact of systematic physical activity on the level and quality of life of people with intestinal stoma. Material and methods: Eighty people with an established intestinal stoma associated with the Pomeranian Stoma Association were examined. To assess the quality of life, the Abbreviated Version of the World Health Organisation Quality of Life Best Available Techniques Reference Document (WHOQOL- BREF) was used. Assessment of physical activity was based on the International Physical Activity Questionnaire (IPAQ) - Polish version. In order to collect socio-demographic data, the author's own questionnaire was used. The statistical package IBM SPSS 23 was used for statistical analysis of the collected data. In the obtained calculations, p < 0.005 was used as the significance level. Results: After the emergence of the stoma, only half of the respondents (50%) continued their physical activity from the period before the emergence, and 46% indicated that they did not feel at ease while exercising. On the basis of the results obtained, the physical activity of the respondents was sufficient, and the respondents most often defined their quality of life as average. The collected data showed that people who were not economically active were not sufficiently physically active (p = 0.021). The study did not show a significant relationship between the level of physical activity and quality of life. Conclusions: Physical activity is significantly conditioned by the professional activity of people with intestinal stoma. Socio-demographic factors do not differentiate the level of physical activity of people with intestinal fistula. Physical activity does not significantly affect the quality of their lives. However, HSCT increases the risk of infections, in particular during the aplasia of mobilization and conditioning. Moreover, intestinal stomas in CD augment the risk of morbidity in immunocompromised patients and under aplastic conditions. The objective of this report was to describe the results of the first year after HSCT in a CD patient with an intestinal stoma. Methods: The patient was assessed in respect to disease symptoms and endoscopic findings before the procedure and 30, 90, 180, and 365 days after HSCT.Results: No complications were observed during mobilization and conditioning with sufficient CD34+ cells being harvested in just one apheresis session. Toxicity was restricted to the hematological series. Scores of all the CD indexes and the quality of life of the patient improved. However, two of three endoscopic scores remained unchanged even though improvements were found in the appearance of the lesions.Conclusion: HSCT may be an alternative treatment for refractory CD in patients with an intestinal stoma, and a priori, carefully selected patients with stomas should not be excluded as candidates for this procedure. We present the case of a newborn patient with diagnosis of proximal jejunal atresia type IIIa, complicated with volvulus due to a congenital band. An innovative and alternative procedure is proposed compared to other decompressive and functionalizing stoma techniques utilized for diversion of fecal stream. In our case a T-tube enterostomy presented multiple benefits for the patient. The advantages of the procedure used are discussed.

Objective: The objective of this study is to review the clinicoradiological profile, scheme of management and the outcome in cases of colonic atresia (CA), and ascertain an optimal approach for the treatment of CA to minimize morbidity and mortality. Design and Setting: This was a retrospective observational study carried out at a tertiary health‑care center. Duration: Total of 6 years duration (January 2011–December 2016). Materials and Methods: A retrospective analysis of 10 patients of CA managed over a 6- year period. Data related to demographics, clinical presentation, associated anomalies, radiologic, intraoperative findings, postoperative stay, complications, and outcome were analyzed. Results: There were three cases of Type II atresia involving terminal ileum, cecum, and adjacent colon. Three cases had proximal ascending colon atresia (Type IIIa [n = 2]; Type I [n = 1]) Type I [n=1], and two cases of type IIIa atresia of the hepatic flexure. Two babies had atresia involving the sigmoid colon; one had Type II atresia, while we were unable to assign a type to the other within the prevailing classification. Seven babies were initially treated with a stoma either in the ileum (n = 3), hepatic flexure (n = 2), and sigmoid colon (n = 2), whereas three were treated with a primary anastomoses. Cases treated with a primary anastomoses had lesser morbidity and a better outcome than those with an ileal or ascending colon stoma. Conclusion: Contrary to the theory of an acute antenatal vascular accident, CA may rarely result from a gradual, sequential obliteration of mesenteric vasculature. Primary anastomosis should be contemplated in proximal CA wherever possible as stomal complications, especially high stoma output can result in considerable morbidity.

This is a retrospective study of 264 intestinal stomas performed in the surgical unit of the Emergency Clinical Hospital "Bagdasar Arseni", Bucharest, within a 7-year period (2015-2021) aiming to evaluate their evolutive complications, risk factors, management and prevention strategies. Material and method. Colostomies: 218 (82.57%) cases, ileostomies 46 (17.43%) cases. Temporary stomas (103 cases or 39.02%) were isolated stomas of discharge in 45 cases and associated with other colorectal procedures in 58 cases. Postoperative complications included general systemic complications in 60 (22.72%) cases and local complications specific to stomas in 84 (31.81%) cases and common to abdominal surgery in 94 (35.60%) cases, which were solved by reoperation in 51 cases, with a reintervention rate of 19.31%. Stoma closures were performed in 34 (33.0%) of the 103 patients with temporary stomas. Of these, 26 (25.24%) patients died in the early postoperative period (< 30 days), the remaining 60 patients refused reintervention or were lost to follow-up. Conclusions. Faecal diversion still represents a therapeutic option for a wide range of benign or malignant digestive or extra-digestive abdominal diseases performed in emergency or scheduled surgeries, mostly for colorectal cancer and its complications., Competing Interests: None to declare., (Copyright © 2023, Medical University Publishing House Craiova.)
