The needs of their medical care are understudied, which can cause low patient satisfaction and lead to poor clinical outcomes for the clients. To meet those international clients’ medical care needs, hospitals in China are seeking strategies to improve services.Purpose: The aim of this study was to explore international clients’ medical care experiences in China, and their perceptions of the quality of these international healthcare services.Patients and Methods: In May 2020, focus group interviews with 24 clients and four healthcare professionals were conducted in the international clinic at Sir Run Run Shaw Hospital (SRRSH). In the client group, 24 representatives of international clients from nine countries were invited and divided into three groups to discuss healthcare needs of international clients who seek healthcare in China. Four healthcare providers, including two nurses and two physicians who usually serve in the international clinic, were also interviewed. Data were analyzed using hybrid inductive/deductive thematic analysis.Results: Six major healthcare needs of international clients were identified, namely: needs for privacy and confidentiality; effective communication; multicultural sensitive care; pleasant environments; qualified care and procedures; and respect. International healthcare is a complex process for both international clients and healthcare professionals.Conclusion: The government and institutional administrators around the world should construct the policies and protocols and integrate cultural competence, communication skills, and privacy and confidentiality protection into health professionals training program to ensure the quality services in the international clinics.Keywords: assessment of healthcare needs, international aspects, qualitative research, focus groups

BACKGROUND: Literature shows that healthcare and medical education are associated with the countries/institutes' developments, and reviewing the global patterns can shed light on various aspects of this competition. The study aims to investigate the current research aspects on the internationalization of healthcare and medical education and assess its trends worldwide. MATERIALS AND METHODS: In this descriptive study, based on documents in Web of Science (n = 609), PubMed (n = 395), and Scopus (n = 6598), scientometric methods and knowledge visualization techniques were applied to evaluate global scientific production and trends from 1900 to 2020. Results were preprocessed in BibExcel (version 2016-02-20) to create a mapping file, and word co-occurrence analysis was applied to evaluate the publications. Scientific maps, authors' networks, and countries'/institutes' contributions were depicted using VOS (Visualization of Similarity) viewer and NetDraw. For trend analysis, highly frequent words were examined at intervals before 2000, 2000–2004, 2005–2009, 2010–2014, and 2015–2019. RESULTS: Altogether, 129 countries and 14536 (co)authors with 7007 affiliations contributed to the production of 6895 documents. Of the 2378 journals, the top 20 journals published 1298 (18.83%) articles. The top 20 authors together contributed to 247 papers (3.58%). High-income countries made up the most prolific nations. Also, institutes from North America, Europe, and Oceania pioneered production and cooperation. Highly co-occurring words were international cooperation, medical education, medical school, curriculum, medical research, and medical students. CONCLUSION: The countries/institutions' efforts in internationalizing healthcare and medical education are reflected in the publications in the databases of Web of Science, PubMed, and Scopus. Thus, the findings offer evidence-based information about past and present trends and help researchers and policymakers understand their global impact.

This paper discusses a number of interrelated topics relating to the economics of innovation diffusion that merit further research and study. These topics encompass: how innovations are not just technological, may be horizontal or vertical, and may develop over time; the role in the diffusion process of complementarities across and substitutability between different technologies and innovations; how the many different sources of innovation other than R&D are given insufficient attention in the literature; the international dimensions of the diffusion process with emphasis on cross-country effects; the role played by different market structures in the industries supplying goods embodying new technologies; the imbalance in the relative emphases upon intra- and intra-firm diffusion in the existing diffusion literature; the need for more research on the relationship between firm performance and the diffusion of innovations, and government policy on diffusion. The global debt crisis will keep the world economy in a straitjacket for some time and reinforce the shift from established economic powers to Asia. Debt needs to be repaid in the form of lower consumption and the deleveraging process for the G-3 (the United States, the eurozone, and Japan, who have reached an unprecedented level of debt), still has a long way to go. The United States opted for fiscal and monetary stimulus, the eurozone chose austerity, and Japan after some hesitation started to pump money into the economy. It remains to be seen whether the divergence in policies will constitute a barrier for a global recovery. Southeast Asia has so far managed quite well because the lesson was learned in 1997-98 and signs of strong borrowing have been detected only recently. The main problem seems to be how to plug into a new and amended global financial system that has emerged in response to flaws that were identified when the crisis struck. In 1863 the Colonial Office reviewed colonial hospitals and lunatic asylums in those parts of the British Empire it administered – probably the first and widest international comparative study up to that date. This article outlines the background, process and scope of the review of asylums, and considers its significance. The resulting ‘digest’ is an important source to explain how, why, when and by whom metropolitan ideas acquired official endorsement and spread throughout the British world. Using the review’s general findings and suggestions, a tool is provided for comparing inter-colonial achievements. With New Zealand as a case study, the article concludes that, relative to other influences, the digest played a limited and largely indirect part in shaping New Zealand’s mental health policy before 1876. Objective – To evaluate the quality of reference service provided to non‐native, English‐speaking international students in academic libraries.Design – Field stimulation (unobtrusive testing).Setting – Eleven college and university libraries in the lower mainland of British Columbia, Canada, in the fall of 2003.Subjects – Library staff offering reference service at one of the participating libraries.Methods – The study utilized field stimulation, whereby an individual, or “proxy,” posed as a library user and initiated a reference encounter with library staff at each institution. In each case the proxy asked the same question to the library staff member. After the interaction was completed the proxy recorded all observed behaviours. Data were collected using a checklist of actions; a narrative record written by the proxy; and several evaluative questions. Each library was visited by the same proxy on two separate occasions for a total of 22 visits, of which 20 instances resulted in usable data. The narrative recordings of the reference encounters were analyzed using an open coding process.Main results – In 75% of the cases, the proxy was “‘satisfied” or “very satisfied” with help received from the library staff member and was “likely to” or “definitely would return to the staff member” in the future. The reference encounters lasted between a few minutes to half an hour in length, with most lasting between 5 and 15 minutes. Encounters that were brief (less than 5 minutes) resulted in an evaluation of “dissatisfied” or “very dissatisfied” and “not likely to” or “definitely would not return.” Encounters where the library staff member extended an invitation to the proxy to return in the future were all rated with “high satisfaction” and “willingness to return.” The following reference service actions were observed in at least half of the encounters:• Asked questions for clarification (20)• Avoided overwhelming the user with information (19)• Provided instruction on how to use information sources (18)• Explained what he / she was doing at every stage (17)• Demonstrated awareness of language barriers and modified his /her behaviour accordingly (16)• Had a respectful attitude toward the user and her question (16)• Looked approachable (15)• Used library jargon (12)The remaining reference service actions from the checklist were observed in less than half the encounters:• Accompanied the user to information sources (9)• Invited the user to return if she needed more help (6)• Asked the user if she had found what she needed (4)• Referred the user to someone else (2)The data collected from the narrative recordings of the reference encounters resulted in the identification of 17 themes. Most of these themes corresponded with the literature reviewed as important qualities for positive reference interactions. Nine of these themes were found to correlate with the proxy’s positive evaluation of the reference encounter: approachability, awareness of language barriers, asking questions, rephrasing, explanation, library jargon, instruction, early termination of interview, patience, and follow up.Conclusion – This preliminary study documents the actions of reference staff in academic libraries when answering a question from an international student. The researchers found a relationship between some library staff behaviours and the user’s level of satisfaction and likelihood to return to the staff member in the future. The research suggests that reference staff pay special attention to the needs of non‐native English speakers in order to provide a positive reference encounter.

This article focuses on a little-known episode: the American Jewish Joint Distribution Committee's (JDC) package program on behalf of Soviet Jewry in the post-Stalinist Soviet Union, from its inception through the Six Day War in 1967. The article reveals JDC clandestine activity on behalf of Soviet Jewry, in cooperation with the Israeli government, when the JDC was officially banned from working there. This package program was an expression of Western Cold War policy to support discriminated-against Soviet ethnic and religious minorities. I explore the dimensions and dynamics of the package program, its share of the total JDC East European aid and its unique style of administration, its outreach and impact, and the response of the beneficiaries. Soviet policy regarding Western aid to its citizens was often guided by pragmatic rather than ideological considerations; therefore, the stream of packages was never entirely stopped. This article reveals that both Israeli and "establishment" American Jewish organizations' activities on behalf of Soviet Jewry were considerably greater than American grassroots organizations claim. The story is also an encouraging example of mutual involvement of the three largest Jewish communities in the world, even when one of them was behind the Iron Curtain.
