2. Interprofessional Active Learning for Chronic Pain: Transforming Student Learning From Recall to Application.

Abstract

Chronic pain (CP) affects over 50 million Americans daily and represents a unique challenge for healthcare professionals due to its complexity. Across all health professions, only a small percentage of the curriculum is devoted to treating patients with CP. Unfortunately, much of the content is delivered passively via lecture without giving students an opportunity to practice the communication skills to effectively treat patients in the clinic. An interprofessional team of health educators identified 5 essential messages that students frequently struggle to convey to patients with CP. Those messages were based on interprofessional and profession-specific competencies to treat patients with CP from the International Association for the Study of Pain. The 5 messages highlighted the importance of (1) therapeutic alliance, (2) consistent interdisciplinary language, (3) patient prognosis, (4) evidence for pain medicine, surgery, and imaging, and (5) early referral to the interprofessional team. For each message, the team summarized relevant research supporting the importance of each individual message that could serve as a foundation for didactic content. The team then developed active learning educational activities that educators could use to have students practice the skills tied to each message. Abstract

The publication collects and systematizes the main definitions and concepts related to pain sensations of various genesis and their changes in pathological conditions. The work includes terminology with appropriate changes and notes, which was prepared by the working group of the International Association for the Study of Pain (IASP) and is used in numerous reports and scientific publications. The glossary deals mainly with terms and does not contain the classification of pain, which is sufficiently presented in other modern scientific publications. The presented terms and their interpretation will be useful not only to scientists, but also to clinicians and students of medical universities of Ukraine.

Abstract

The purpose of this study was to investigate the Neonatal Pain, Agitation, and Sedation Scale (N-PASS) tool for evidence of validity and reliability in infants and children aged 1 to 36 months in the postoperative recovery period. The design was a prospective, nonexperimental psychometric evaluation. The sample analyzed was composed of 40 subjects aged 1 to 36 months. Data were collected by evaluating participants every 5 minutes, scoring the N-PASS sedation subscale along with the University of Michigan Sedation Scale (UMSS) and the N-PASS pain/agitation subscale along with the Face, Legs, Arms, Cry, Consolability (FLACC) tool. The results showed that the Cronbach's alphas of the N-PASS sedation scale ranged from 0.853 to 0.923, and from 0.935 to 0.971 for the N-PASS pain/agitation scale. Correlations between the N-PASS sedation scale and the UMSS tool ranged from 0.847 to 0.967. Correlations between the N-PASS pain/agitation scale and the FLACC tool ranged from 0.980 to 0.996. Repeated measures ANOVA analysis revealed that the N-PASS sedation score decreased linearly over time, and that N-PASS pain/agitation scores changed erratically over time. Regression analysis revealed a statistically nonsignificant linear trend for the prediction of amount of time spent in the recovery area. Principal axis factor analysis found that two factors accounted for 80.46% to 87.77% of the variance. One factor represented pain/agitation, the other sedation, confirming the two subscales of the N-PASS tool. All items had high factor loadings (> 0.60). Factor structure remained similar over time. The implications for this research indicate sufficient evidence for the use of the N-PASS tool for sedation and pain/agitation assessment in infants and children age 1 to 36 months., Managing pain and alleviating suffering are ethical mandates for nursing practice (American Nurses Association [ANA], 2001). Little data exist regarding pain after a pediatric cardiac catheterization and whether there is a requirement for opioids. As part of an improvement activity, a prospective cohort study was performed to assess pain related to cardiac catheterization and subsequent usage of pain medication following a cardiac catheterization. Of 745 patients studied, the majority had either no pain or pain controlled by acetaminophen. There were 14 (1.9%) patients who had pain unrelieved by acetaminophen, with 7 of these patients having pain at their vascular access site with associated complications. Findings indicate a limited number of pediatric patients require opioid pain relief following cardiac catheterization. This study changed practice surrounding post-procedural pain related to cardiac catheterization because opioids were no longer prescribed. Nurse follow up occurred within 48 hours of each procedure, and any patient experiencing pain was immediately triaged and evaluated by the healthcare team. The study showed persistent pain following a cardiac catheterization is an important sign of potential complication. Although pain control is important, it is also important to target pain relief to the appropriate level required for comfort. A Disease Itself: The Transformation of Pain After 1945By Stephen Beitler A Disease Itself examines a fundamental shift in the understanding and clinical status of pain in the United States after 1945. Pain was transformed from a simple, rote, well understood, and largely treatable event into a clinical syndrome defined by variability, chronicity, and complexity. Pain became subjective, rooted in individual experience, beliefs, and circumstances as much as in organic illness and injury. An insurgent model of pain refashioned the primary clinical challenge as chronic pain, asserted dense interconnections among pain’s psychological and physical dimensions, and advocated multidisciplinary treatment. Together, these changes refashioned how people experienced pain, how clinicians treated it, legislative actions, and the fortunes of dozens of pharmaceutical companies. They reflected as well substantial cultural shifts in ideas and practices of identity, selfhood, authority, and autonomy. A Disease Itself explores the history of how pain has been measured, managed, and organized professionally in order to track the origins and contours of this fundamental change. The dissertation begins with a historical review of the development of scientific, medical, and cultural understandings of pain from 1800 to 1945. It then examines the measurement, management, and professionalization of pain through three in-depth case studies. The first case study examines how a new model of pain was operationalized clinically in an assessment instrument, the McGill Pain Questionnaire (MPQ), published in 1975. The second explores how wider debates over risk and reward in treatment were reflected in enduring clinical ambiguities surrounding a highly successful pain reliever, Darvon, between its debut in 1957 and its removal from the market by the Food and Drug Administration (FDA) beginning in 2010. The third case study depicts how a new pain paradigm was central to the launch of International Association for the Study of Pain (IASP) after 1973. A Disease Itself draws on an extensive array of research studies on the MPQ, government and industry documents on Darvon, and the published works of early leaders of the IASP in its portrayal of a fundamental shift in the understanding and clinical status of pain.

Abstract

Background. Fibromyalgia is a common chronic pain condition that has a significant impact on quality of life and often leads to disability. To date, there have been few well-controlled trials assessing the utility of nonpharmacological treatment modalities such as transcutaneous electrical nerve stimulation (TENS) in the management of pain and improvement in function in individuals with fibromyalgia. Objectives. The purpose of this study will be to complete a long-term, multicenter study to assess the effects of TENS in women with fibromyalgia. Design. This will be a phase II randomized, double-blind, placebo-controlled, multicenter clinical trial. Participants. Three hundred forty-three participants with fibromyalgia will be recruited for this study. Intervention. Participants will be randomly assigned to 1 of 3 groups: the intervention (TENS), placebo, or no treatment. After completing the randomized period, all participants will receive the intervention for 1 month. The participants will be asked to use TENS at the highest tolerable level for at least 2 hours daily during physical activity. Measurements. The primary outcome will be pain with movement, with secondary outcomes assessing functional abilities, patient-reported outcomes, and quantitative sensory testing. Limitations. Because having participants refrain from their typical medications is not practical, their usage and any change in medication use will be recorded. Conclusions. Purpose Estradiol is a female hormone required for maintaining pregnancy and developing follicles in the ovary. Estradiol has been shown to perform a variety of physiological activities, including pain reduction. In this study, we tested the hypothesis that estradiol exerts antinociceptive effects in a rat model of inflammatory hyperalgesia. Methods We established a subacute hyperalgesia model using plantar injection of Freund's complete adjuvant (FCA) in Sprague-Dawley rats. We administered estradiol every 24 h, beginning 12 h after FCA administration, and used the plantar test to determine its effect on hyperalgesia. To determine the mechanism of action of estradiol, we evaluated the role of the opioid antinociceptive system using naloxone and the role of the descending pain inhibitory system using the α-2-receptor antagonist yohimbine and the serotonin receptor antagonist methysergide. Results Administration of FCA induced hyperalgesia, which was significantly reduced by estradiol treatment compared to controls. Moreover, this effect was not antagonized by naloxone, but was attenuated by α-2-receptor and serotonin-receptor antagonists. Conclusion Estradiol is known to perform a variety of physiological functions. Our findings suggest that one such function is antinociception via an interaction with α-2 receptors and serotonin receptors. At the conclusion of the 13th World Congress on Pain in Montreal, Quebec, Canada, the International Association for the Study of Pain (IASP) hosted an International Pain Summit on September 3, 2010, to address the tragedy of unrelieved pain in the world.
