Abstract

Importance Anti–vascular endothelial growth factor (VEGF) therapies are a novel treatment option in retinopathy of prematurity (ROP). Data on dosing, efficacy, and safety are insufficient. Objective To investigate lower doses of anti-VEGF therapy with ranibizumab, a substance with a significantly shorter systemic half-life than the standard treatment, bevacizumab. Design, Setting, and Participants This randomized, multicenter, double-blind, investigator-initiated trial at 9 academic medical centers in Germany compared ranibizumab doses of 0.12 mg vs 0.20 mg in infants with bilateral aggressive posterior ROP; ROP stage 1 with plus disease, 2 with plus disease, or 3 with or without plus disease in zone I; or ROP stage 3 with plus disease in posterior zone II. Patients were recruited between September 2014 and August 2016. Twenty infants were screened and 19 were randomized. Interventions All infants received 1 baseline ranibizumab injection per eye. Reinjections were allowed in case of ROP recurrence after at least 28 days. Main Outcomes and Measures The primary end point was the number of infants who did not require rescue therapy at 24 weeks. Key secondary end points included time-to-event analyses, progression of physiologic vascularization, and plasma VEGF levels. Stages of ROP were photodocumented and reviewed by an expert committee. Results Nineteen infants with ROP were enrolled (9 [47.4%] female; median [range] postmenstrual age at first treatment, 36.4 [34.7-39.7] weeks), 3 of whom died during the study (1 in the 0.12-mg group and 2 in the 0.20-mg group). Of the surviving infants, 8 (88.9%) (17 eyes [94.4%]) in the 0.12-mg group and 6 (85.7%) (13 eyes [92.9%]) in the 0.20-mg group did not require rescue therapy. Both ranibizumab doses were equally successful in controlling acute ROP (Cochran-Mantel-Haenszel analysis; odds ratio, 1.88; 95% CI, 0.26-13.49;P = .53). Physiologic intraretinal vascularization was superior in the 0.12-mg group. The VEGF plasma levels were not systematically altered in either group. Conclusions and Relevance This pilot study demonstrates that ranibizumab is effective in controlling acute ROP and that 24% of the standard adult dose (0.12 mg) appears equally effective as 40% (0.20 mg). Superior vascularization of the peripheral retina with 0.12 mg of ranibizumab indicates that the lower dose may be favorable. Unchanged plasma VEGF levels point toward a limited systemic drug exposure after ranibizumab. Abstract

Abstract: Importance: Pre‐treatment symptoms longer than 12 months and foveal cystoid changes are indicators for poor anatomical and functional outcome after photodynamic therapy (PDT). Background: To evaluate the prognostic factors on the effectiveness of PDT with double duration for treatment of exudative circumscribed choroidal haemangioma. Design: Retrospective study. Participants: Twenty‐seven patients with symptomatic exudative circumscribed choroidal haemangioma treated with PDT. Methods: Clinical charts of patients with exudative circumscribed choroidal haemangioma treated with PDT were analysed with regard to visual acuity, duration of symptoms, subfoveal fluid, foveal cystoid changes and foveal thickness in optical coherence tomography. Main Outcome Measures: Change of best‐corrected visual acuity, sub‐ and intrafoveal fluid and foveal thickness measured with optical coherence tomography from baseline to last follow‐up. Results: Mean visual acuity improved by 0.05 from 0.42 logMAR (standard deviation [SD] 0.34) to 0.37 logMAR (SD 0.47). In 70% of the patients, PDT stopped exudation and revealed a dry fovea. The recurrence or persistence of sub‐ or intrafoveal fluid was significantly associated with pre‐therapeutic symptoms existing for more than 12 months (P = 0.046). Mean foveal thickness in optical coherence tomography decreased from 324 μm (SD 223 μm) to 209 μm (SD 109 μm). Conclusions and Relevance: PDT proved to be a safe and effective treatment procedure for exudative circumscribed choroidal haemangioma in our series, with few side effects. We observed a more successful treatment with regard to anatomical and functional results in cases with pre‐existing symptoms less than 12 months and in cases without pre‐therapeutic foveal cystoid changes. Abstract

Acute macular neuroretinopathy (AMNR) is a rare disease entity, the diagnosis of which is frequently complicated by the subtlety of biomicroscopic findings. Two cases of AMNR are presented, in which the diagnosis and follow-up was enabled using the Spectralis® HRA+OCT in the absence of clear biomicroscopic findings. The typical lesions were visualized by hyporeflexion during infrared imaging and faded over time. With spectral domain optical coherence tomography, changes in the outer retina in the affected regions were documented, with no change over time. 44. Indocyanine Green Angiography in Classic Choroidal Neovascularization

Abstract

The indocyanine green (ICG) angiographic features of classic choroidal neovascularization (CNV) were evaluated in the 66 consecutive patients (70 eyes) by ICG angiography using the scanning laser ophthalmoscope. All patients had classic CNV documented by fluorescein angiography. Indocyanine green angiographic findings of classic CNV were as follows: Vessel architecture in 66% (46 of 70) of eyes, feeding vessels in 29% (20 of 70), and late hyperfluorescence in 93% (65 of 70) of eyes. Borders of classic CNV were found well-defined in 47% (33 of 70), and ill-defined in 49% (34 of 70) of eyes. In the remaining 4% (3 of 70) of eyes ICG angiography did not detect CNV. Our study indicates that fluorescein angiography remains the method of choice in the diagnosis of classic CNV. Indocyanine green angiography provides more information in the detection of feeding vessels of classic CNV.
