This mixed methods study investigated the association between multiple bed moves and falls during hospitalisation of older patients identified as a fall risk, as well as safety of ward environments, and staff person-centredness and level of inter-professional collaboration.Methods: Patients aged ≥70 years, admitted through the Emergency Department (ED) and identified at high fall risk, who were admitted to four target medical wards, were followed until discharge or transfer to a non-study ward. Hospital administrative data (falls, length of stay [LoS], and bed moves) were collected. Ward environmental safety audits were conducted on the four wards, and staff completed person-centredness of care, and interprofessional collaboration surveys. Staff focus groups and patient interviews provided additional qualitative data about bed moves.Results: From 486 ED tracked admissions, 397 patient records were included in comparisons between those who fell and those who did not [27 fallers/370 non-fallers (mean 84.8 years, SD 7.2; 57.4% female)]. During hospitalisation, patients experienced one to eight bed moves (mean 2.0, SD 1.2). After adjusting for LoS, the number of bed moves after the move to the initial admitting ward was significantly associated with experiencing a fall (OR 1.56, 95% CI 1.11–2.18). Ward environments had relatively few falls hazards identified, and staff surveys indicated components of person-centredness of care and interprofessional collaboration were rated as good overall, and comparable to other reported hospital data. Staff focus groups identified poor communication between discharging and admitting wards, and staff time pressures around bed moves as factors potentially increasing falls risk for involved patients. Patients reported bed moves increased their stress during an already challenging time.Conclusion: Patients who are at high risk for falls admitted to hospital have an increased risk of falling associated with every additional bed move. 2. What Factors Affect the Way Teams Interact in an On-Line Graduate Course?

Abstract

This study examined how adult graduate students made use of technology to mediate group interactions and processes in an online learning environment, also noting why individual groups of students chose particular tools and methods. The online course was delivered using a third-party course management tool. The structure of the course and the use of computer-mediated communication tools to promote interaction were somewhat controlled by the course management tool's pedagogical assumptions of design and the resulting technological and structural capabilities. Data were collected via conversations with a student, surveys that examined students' beliefs about their group processes, and a focus group interview. Results indicated that time was a major factor in determining how one group structured their use of the available technology tools. The course tool limited some of the design decisions about interaction. However, based on the interactions within the three groups, the personalities and experiences of individual members also played a role in their interaction. The need and ability to trust was a common theme among respondents. For at least two of the groups, students' level of technological experience factored in at the beginning of the group project. Abstract

Objective: To (a) compare characteristics of patients who fall with those of patients who did not fall; and (b) characterise falls (time, injury severity and location) through three fall reporting methods (incident system reports, medical notes and clinician reports). Methods: A substudy design within a stepped‐wedge clinical trial was used: 3239 trial participants were recruited from two inpatient Geriatric Evaluation and Management Units and one general medicine ward in two Australian states. To compare the characteristics of patients who had fallen with those who had not, descriptive tests were used. To characterise falls through three reporting methods, bivariate logistic regressions were used. Results: Patients who had fallen were more likely than patients who had not fallen to be cognitively impaired (51% vs. 29%, p < 0.01), admitted with falls (38% vs. 28%, p = 0.01) and have poor health outcomes such as prolonged length of stay (24 [16–34] vs. 12 [8–19] days [IQR], p < 0.01) and less likely to be discharged directly to the community (62% vs. 47%, p < 0.01). Most falls were captured from medical notes (93%), with clinician (71%) and incident reports (68%) missing 21%–25% of falls. The proportion of injurious falls identified through incident reports was higher than medical records or clinician reports (40% vs. 34% vs. 37%). Conclusions: This study reaffirms the need to improve reporting falls in incident systems and at clinical handover to the team leader. Research should continue to use more than one method of identifying falls, but include data from medical records. Many falls cause injury, resulting in poor health outcomes. [ABSTRACT FROM AUTHOR]

4. Evaluation of the Empower model of care for partial denture clients in a public oral health care setting.

Abstract

Objective: To evaluate the effectiveness of the Empower model of care (EMC) on reducing the addition of teeth to removable partial dentures (RPD). Methods: Data reports were generated through the Titanium electronic database to retrieve all clients at Monash Dental Services (MHDS) who participated in the EMC or received an RPD prior to the implementation of EMC was assessed over a two‐year period to determine if they had additional teeth added to their partial dentures (PD). A logistic regression was performed testing whether a combination of socio‐demographic and clinical variables, provided a multivariate explanation of the EMC outcome. Results: Prior to EMC, 2034 patients attended MHDS requiring RPDs with 363 returning to have teeth added. From 2018 to 2020, 38 of 584 participants in EMC returned to have teeth added to their PDs. Those in the 'High' risk group were two times more likely to return requiring additional teeth (OR = 1.99; 95%CI:1.31–3.02); each additional year of age increased the odds of requiring additional teeth (OR = 1.03; 95%CI:1.02–1.04). Participants of the EMC were more than two times less likely to require additional teeth (OR = 0.42; 95%CI:0.29–0.59). The variance in requiring additional teeth accounted for using the full model was 9.1% (η2 = 0.091). Conclusions: This analysis confirms the effectiveness of the EMC. This approach assists denture wearers to maintain good oral health and provides an effective way of managing public health funds by reducing denture repairs and additions. © 2023 Australian Dental Association. 6. Computational studies of molecular actinide and lanthanide complexes

Abstract

This thesis reports computational DFT studies of three families of complexes (see below). Before presenting the results from these studies, the first chapter introduces the lanthanides and actinides and investigates their similarities and differences through a discussion of selected compounds of each of the series of metals. The second chapter introduces the electronic structure methods that were used in this research and mentions some of the relevant computational utilities. Chapter 3 discusses DFT studies of mixed water/hydroxide uranyl systems, U02(H20)a-:r- OH)x (2-x)+ for x = 0 -> 5 with a = 5, and some a = 3, 4. The reasons for the observed lengthening, and resultant weakening, of U-Oy/ as x increases are investigated. These studies show that this lengthening appears to be predominantly due to a reduction in ionic character stemming from charge build up on the U centre, and not from hydroxide / Oyi competition for U 6d, as has been previously suggested. Time-Dependent DFT is used in chapter 4 to simulate the electronic spectra of U02(NCN)2 and U02(NPN)2 in order to investigate why these complexes are coloured red rather than the normally observed green/yellow of uranyl systems. This chapter involves some preliminary benchmarking calculations on three other uranyl complexes, U02Cl2(TBP)2 , UC>2Cl2(THF)2 , U02(NC>3)2(TBP)2 , as there is as yet no literature suggesting suitable exchange potentials and/or basis sets for TD-DFT calculations on actinides. The conclusion was reached that the unusual colour of the nitrogen donor complexes is due to a small HOMO-LUMO gap resulting from the relatively low energy nitrogen based ligand MOs compared with usual uranyl ligands such as O and CI. Chapter 5 systematically investigates M(N(EPR.2)2)3 , M = Ln (Ln = La, Ce, Pr, Pm, Eu), An (An = U, Np, Pu, Am, Cm) E = O, S, Se, Te R = H to investigate the ligands' suitability for extraction of An(III) from Ln(III), and also to test the suitability of La and U as models for Eu and Am/Cm respectively. The results lead me to conclude that chalcogen donor ligands are extremely promising for successful separation of Cm from Am and Eu, but good separation factors of Am from Eu seem unlikely. Furthermore I conclude that La and U are not suitable models for Eu and Am/Cm as the 2+ capability of Eu seems present in with S, Se, Te U shows considerable covalency in M-E with the heavier chalcogens and there is no evidence of covalency in any Am-E or Cm-E.

Abstract

Abstract Background In 2018, the Australian Government updated the Australian Physical Activity and Sedentary Behaviour Guidelines for Children and Young People. A requirement of this update was the incorporation of a 24-hour approach to movement, recognising the importance of adequate sleep. The purpose of this paper was to describe how the updated Australian 24-Hour Movement Guidelines for Children and Young People (5 to 17 years): an integration of physical activity, sedentary behaviour and sleep were developed and the outcomes from this process. Methods The GRADE-ADOLOPMENT approach was used to develop the guidelines. A Leadership Group was formed, who identified existing credible guidelines. The Canadian 24-Hour Movement Guidelines for Children and Youth best met the criteria established by the Leadership Group. These guidelines were evaluated based on the evidence in the GRADE tables, summaries of findings tables and recommendations from the Canadian Guidelines. We conducted updates to each of the Canadian systematic reviews. A Guideline Development Group reviewed, separately and in combination, the evidence for each behaviour. A choice was then made to adopt or adapt the Canadian recommendations for each behaviour or create de novo recommendations. We then conducted an online survey (n=237) along with three focus groups (n=11 in total) and 13 key informant interviews. Stakeholders used these to provide feedback on the draft guidelines. Abstract

Abusive language is a massive problem in online social platforms. Existing abusive language detection techniques are particularly ill-suited to comments containing heterogeneous abusive language patterns, i.e., both abusive and non-abusive parts. This is due in part to the lack of datasets that explicitly annotate heterogeneity in abusive language. We tackle this challenge by providing an annotated dataset of abusive language in over 11,000 comments from YouTube. We account for heterogeneity in this dataset by separately annotating both the comment as a whole and the individual sentences that comprise each comment. We then propose an algorithm that uses a supervised attention mechanism to detect and categorize abusive content using multi-task learning. We empirically demonstrate the challenges of using traditional techniques on heterogeneous content and the comparative gains in performance of the proposed approach over state-of-the-art methods., AAAI 2021 (AI for Social Impact track)

41. Bed moves, ward environment, staff perspectives and falls for older people with high falls risk in an acute hospital: A mixed methods study

Abstract

Background: Falls remain an important problem for older people in hospital, particularly those with high falls risk. This mixed methods study investigated the association between multiple bed moves and falls during hospitalisation of older patients identified as a fall risk, as well as safety of ward environments, and staff person-centredness and level of inter-professional collaboration. Methods: Patients aged ≥70 years, admitted through the Emergency Department (ED) and identified at high fall risk, who were admitted to four target medical wards, were followed until discharge or transfer to a non-study ward. Hospital administrative data (falls, length of stay [LoS], and bed moves) were collected. Ward environmental safety audits were conducted on the four wards, and staff completed person-centredness of care, and interprofessional collaboration surveys. Staff focus groups and patient interviews provided additional qualitative data about bed moves. Results: From 486 ED tracked admissions, 397 patient records were included in comparisons between those who fell and those who did not [27 fallers/370 non-fallers (mean 84.8 years, SD 7.2; 57.4% female)]. During hospitalisation, patients experienced one to eight bed moves (mean 2.0, SD 1.2). After adjusting for LoS, the number of bed moves after the move to the initial admitting ward was significantly associated with experiencing a fall (OR 1.56, 95% CI 1.11–2.18). Ward environments had relatively few falls hazards identified, and staff surveys indicated components of person-centredness of care and interprofessional collaboration were rated as good overall, and comparable to other reported hospital data. Staff focus groups identified poor communication between discharging and admitting wards, and staff time pressures around bed moves as factors potentially increasing falls risk for involved patients. 42. Leadership and career development curriculum in Australian dental and oral health programmes

Abstract

INTRODUCTION: There is very little evidence of dental schools offering leadership or career development training for students. Given that tertiary students come from diverse backgrounds with differing experiences, it is important that all students learn skills that improve their employability by preparing them for a range of career paths and equipping them with the confidence to lead their profession into the future. This study mapped current leadership and career development training in dental education programmes throughout Australia, focusing on the capacity, barriers and enablers. METHODS: In 2018, an electronic survey was sent to the Program Convenors of all nineteen dental programmes in Australia. Survey questions were sourced from existing literature and the current regulatory competencies. RESULTS: Whilst the majority of respondents (n = 17, 89% response rate) indicated that their programmes included both leadership training (72%) and career development activities (88%), it is clear that the content focus, hours allocated and assessment items vary widely across the country. Further, curriculum appeared to focus almost exclusively on clinical work, which does not provide an avenue for students to explore alternative, non-clinical career options or pathways or develop their skills beyond treating individual patients. Time constraints and ideological differences were reported as the biggest barriers to implementing leadership and career development curricula. CONCLUSION: Despite an inconsistent approach nationally, it appears there is an opportunity for faculty across institutions to collaborate, potentially to develop a set of leadership and career development competencies and best practice teaching and learning activities.

Abstract

Purpose. To analyze the current state of innovation and environmental projects, to develop management strategies and algorithms for project environmental and economic management in subsoil use, to propose a mathematical model of the system of perception and support of solutions for effective environmental and economic management. Methodology. Different methods of cognition were used for scientific research – both general and special. Analytical study on the state and realities of innovation in Ukraine in general and in particular in the mining industry, identification of trends in world practice were conducted based on using the methods of content analysis, quantitative and qualitative comparison. Methods of scientific abstraction and systematization were used to develop strategies and basic algorithm of project environmental and economic management in subsoil use. Mathematical methods were used to develop a mathematical model and coordinate it with the developed algorithm of project environmental and economic management for subsequent use in the system of perception and support of decisions. Findings. A systematic approach to the definition of management strategies and an algorithm of project environmental and economic management for the implementation of effective economic and environmental management of mining enterprises is proposed. A mathematical model designed for use in the system of perception and support of decisions for effective environmental and economic management has been developed. An analytical study on the state of innovation in Ukraine in general and in particular in the extractive industry has been conducted. This provided an opportunity to identify trends in the dynamics of change regarding the introduction of innovations and identify the causes of existing trends. Originality. The study identified threatening trends in the practice of introducing innovation. To solve production and scientific problems in the presence of significant challenges, new tools for finding optimal solutions in the environmental and economic management of subsoil use were proposed. Using a systematic approach, a mechanism for selecting strategies, an algorithm of project environmental and economic management and an original mathematical model have been proposed.
