- Abstract
Background: Several studies have compared surface electrocardiographic changes following different self-expandable (SE) (Evolut (Medtronic, USA); Acurate (Boston Scientific, USA); Portico (Abbott, USA); and Allegra (NVT, Germany)) and balloon-expandable (BE) Sapien-3 (Edwards Lifesciences, USA) transcatheter heart valves. We aimed to compare these prosthesis with the novel Myval BE prosthesis (Meril Life, India). Methods: Academic European registry of consecutive patients with severe aortic stenosis who received any of the 6 aforementioned valves. Baseline, post-procedural, and discharge 12‐leads electrocardiograms (ECG) were centrally analyzed and compared. Results: A total of 1131 patients were included: 135 Myval (11.9%), 290 Sapien-3 (25.6%), 298 Evolut (26.3%), 180 Acurate (15.9%), 125 Portico (11.1%), and 103 Allegra (9.1%). There were no baseline differences in intraventricular conduction disturbances rate. Compared to the novel BE Myval, there were similar procedural and in-hospital outcomes. Similar rates of early new permanent pacemaker implant (PPI) were observed amongst Myval (7.4%), Sapien-3 (13.4%), and Acurate (9.1%), but Evolut, Portico, and Allegra presented significantly higher rates (18.5%, p = 0.003; 29.5% p < 0.001 and 22%, p = 0.001, respectively). Central analysis of ECGs, unraveled significant prolongation of the PR segment with Evolut, Portico and Allegra whereas Evolut, Acurate, and Portico showed significant QRS widening compared to Myval. However, at discharge no differences in PR segment duration were observed while, Evolut, and Portico– but not Acurate, Allegra or Sapien-3 – still presented significant widening of QRS segment compared to Myval. Conclusions: After blinded central ECG analysis, the novel Myval balloon-expandable prosthesis was associated with a low rate of early conduction disturbances.

Highlights • Takotsubo Syndrome (TTS) is increasingly reported in COVID-19 patients • The pathogenic mechanisms underlying TTS in this particular setting are still unknown • Autopsy findings in fatal cases may contribute to understand the possible role of SARS-CoV-2 in the onset of TTS, COVID-19 can involve several organs and systems, often with indirect and poorly clarified mechanisms. Different presentations of myocardial injury have been reported, with variable degrees of severity, often impacting on the prognosis of COVID-19 patients. The pathogenic mechanisms underlying cardiac damage in SARS-CoV-2 infection are under active investigation. We report the clinical and autopsy findings of a fatal case of Takotsubo Syndrome occurring in an 83-year-old patient with COVID-19 pneumonia. The patient was admitted to Emergency Department with dyspnea, fever and diarrhea. A naso-pharyngeal swab test for SARS-CoV-2 was positive. In the following week his conditions worsened, requiring intubation and deep sedation. While in the ICU, the patient suddenly showed ST segment elevation. Left ventricular angiography showed decreased with hypercontractile ventricular bases and mid-apical ballooning, consistent with diagnosis of Takotsubo syndrome (TTS). Shortly after the patient was pulseless. After extensive resuscitation maneuvers, the patient was declared dead. Autopsy revealed a subepicardial hematoma, in absence of myocardial rupture. On histology, the myocardium showed diffuse edema, multiple foci of contraction band necrosis in both ventricles and occasional coagulative necrosis of single cardiac myocytes. Abundant macrophages CD68+ were detected in the myocardial interstitium. The finding of diffuse contraction band necrosis supports the pathogenic role of increased catecholamine levels; the presence of a significant interstitial inflammatory infiltrate, made up by macrophages, remains of uncertain significance.

Background: Experimental and clinical data suggest that reducing inflammation without affecting lipid levels may reduce the risk of cardiovascular disease. Yet, the inflammatory hypothesis of atherothrombosis has remained unproved. Methods: We conducted a randomized, double-blind trial of canakinumab, a therapeutic monoclonal antibody targeting interleukin-1β, involving 10,061 patients with previous myocardial infarction and a high-sensitivity C-reactive protein level of 2 mg or more per liter. The trial compared three doses of canakinumab (50 mg, 150 mg, and 300 mg, administered subcutaneously every 3 months) with placebo. The primary efficacy end point was nonfatal myocardial infarction, nonfatal stroke, or cardiovascular death. RESULTS: At 48 months, the median reduction from baseline in the high-sensitivity C-reactive protein level was 26 percentage points greater in the group that received the 50-mg dose of canakinumab, 37 percentage points greater in the 150-mg group, and 41 percentage points greater in the 300-mg group than in the placebo group. Canakinumab did not reduce lipid levels from baseline. At a median follow-up of 3.7 years, the incidence rate for the primary end point was 4.50 events per 100 person-years in the placebo group, 4.11 events per 100 person-years in the 50-mg group, 3.86 events per 100 person-years in the 150-mg group, and 3.90 events per 100 person-years in the 300-mg group. The hazard ratios as compared with placebo were as follows: in the 50-mg group, 0.93 (95% confidence interval [CI], 0.80 to 1.07; P = 0.30); in the 150-mg group, 0.85 (95% CI, 0.74 to 0.98; P = 0.021); and in the 300-mg group, 0.86 (95% CI, 0.75 to 0.99; P = 0.031). The 150-mg dose, but not the other doses, met the prespecified multiplicity-adjusted threshold for statistical significance for the primary end point and the secondary end point that additionally included hospitalization for unstable angina that led to urgent revascularization (hazard ratio vs. placebo, 0.83; 95% CI, 0.73 to 0.95; P = 0.005). Canakinumab was associated with a higher incidence of fatal infection than was placebo. There was no significant difference in all-cause mortality (hazard ratio for all canakinumab doses vs. placebo, 0.94; 95% CI, 0.83 to 1.06; P = 0.31). Conclusions: Antiinflammatory therapy targeting the interleukin-1β innate immunity pathway with canakinumab at a dose of 150 mg every 3 months led to a significantly lower rate of recurrent cardiovascular events than placebo, independent of lipid-level lowering. - Abstract
We report the case of a 65-year-old man referred to our department because of a complex coronary aneurysmal disease presenting as subacute ST-segment elevation myocardial infarction due to intraluminal thromboembolism. The patient's past medical history (recurrent episodes of lymphadenopathies, parotid swelling, prostatitis, and dacryoadenitis) raised the suspicion of a unifying systemic disorder with coronary involvement. An extensive infectious and autoimmune screening was performed, leading to the final diagnosis of IgG4-related disease. Our case highlights the importance to include this rare and recently described disease in the diagnostic workup of acquired coronary aneurysms., Nous rapportons le cas d'un homme âgé de 65 ans qui a été orienté vers notre service en raison d'une maladie coronarienne anévrismale complexe caractérisée par un infarctus subaigu du myocarde avec élévation du segment ST imputable à une thrombose intraluminale. Les antécédents médicaux de ce patient (épisodes récurrents de lymphadénopathie, d'enflure de la glande parotide, de prostatite et de dacryoadénite) ont amené à soupçonner une maladie systémique unificatrice avec atteinte des coronaires. Un dépistage exhaustif des maladies infectieuses et auto-immunes a été effectué et a conduit au diagnostic final de maladie associée aux IgG4. Le cas que nous rapportons met en lumière l'importance d'inclure cette maladie rare et de description récente dans l'analyse diagnostique des anévrismes acquis des coronaires.

- Abstract
In rough set theory (RST), and more generally in granular computing on information tables (GRC-IT), a central tool is the Pawlak's indiscernibility relation between objects of a universe set with respect to a fixed attribute subset. Let us observe that Pawlak's relation induces in a natural way an equivalence relation ≈ on the attribute power set that identifies two attribute subsets yielding the same indiscernibility partition. We call indistinguishability relation of a given information table I the equivalence relation ≈, that can be considered as a kind of global indiscernibility. In this paper we investigate the mathematical foundations of indistinguishability relation through the introduction of two new structures that are, respectively, a complete lattice and an abstract simplicial complex. We show that these structures can be studied at both a micro granular and a macro granular level and that are naturally related to the core and the reducts of I. We first discuss the role of these structures in GrC-IT by providing some interpretations, then we prove several mathematical results concerning the fundamental properties of such structures.

Simplicial complexes (here briefly complexes) are set systems on an arbitrary set which are object of study in many areas of both mathematics and theoretical computer science. Usually, they are investigated over finite sets. However, in general, when we consider an arbitrary set Ω (not necessarily finite) and a complex on Ω , the most natural property related to finiteness is the following: for any subset X of Ω , if F ∈ for all finite subsets F of X , then X ∈ . We call locally finite any complex having such a property. Bearing in mind some motivations and constructions derived from the analysis of information systems in rough set theory, in this paper we associate with any locally finite complex a corresponding pre-closure operator σ and, through it, we establish several properties of . Next, we investigate the main features of the specific sub-class of locally finite complexes for which σ is a closure operator. We call these complexes closable and exhibit a particular family of closable locally finite complexes using left-modules on rings with identity. Magnesium is an essential mineral naturally present in the human body, where it acts as cofactor in several enzymatic reactions. Magnesium is a key cardiovascular regulator, which maintains electrical, metabolic, and vascular homeostasis. Moreover, magnesium participates in inflammation and oxidative processes. In fact, magnesium deficiency is involved in the pathophysiology of arterial hypertension, diabetes mellitus, dyslipidemia, metabolic syndrome, endothelial dysfunction, coronary artery disease, cardiac arrhythmias, and sudden cardiac death. In consideration of the great public-health impact of cardiovascular disease, the recognition of the negative effects of magnesium deficiency suggests the possible role of hypomagnesaemia as cardiovascular risk factor and the use of serum magnesium level for the screening and prevention of cardiovascular risk factors and cardiovascular diseases. Moreover, it might help with the identification of new therapeutical strategies for the management of cardiovascular disease through magnesium supplementation.

- Abstract
In this paper we study the lattice of all indiscernibility partitions induced from attribute subsets of a knowledge representation system (information table in the finite case). This lattice, that we here call granular partition lattice, is a very well studied order structure in granular computing and data base theory and it provides a complete hierarchical classification of the knowledge obtained from all possible choices of attribute subsets. We show that it has a lattice structure also in the infinite case and we provide several isomorphic characterizations for this lattice. We discuss the potentiality of this order structure from both a micro-granular and a macro-granular perspective. Furthermore, the sub-poset of all the indiscernibility closures needed to determine when an arbitrary partition is an indiscernibility one is studied. Finally, we show the monotonic behaviour of the granular partition lattice with respect to entropy of partitions and attribute dependency in decision tables.

- Abstract
We introduce and study new generalizations of some rough set tools. Namely, the extended core, the generalized discernibility function, the discernibility space and the maximum partitioner. All these concepts where firstly introduced during the application of rough set theory to graphs, here we show that they have an interesting and useful interpretation also in the general setting. Indeed, among other results, we prove that reducts can be computed in incremental polynomial time, we give some conditions in order that a partition coincides with an indiscernibility partition of a given information table and we give the conditions such that a discernibility matrix corresponds to an information table.

The adjacency relation of a simple undirected graph is a preclusive (irreflexive and symmetric) relation. Hence, it originates a preclusive space enabling us to define the lower and upper preclusive approximations of graphs and two orthogonality graphs. Further, the possibility of defining the similarity lower and upper approximations and the sufficiency operator on graphs will be investigated, with particular attention to complete and bipartite graphs. All these mappings will be put in relation with Formal Concept Analysis and the theory of opposition.

Specifically, we use the adjacency matrix of a digraph as a particular type of information table. In this way, we are able to explore on digraphs the notions of indiscernibility partitions, lower and upper approximations, generalized core, reducts and discernibility matrix. All these ideas will be exemplified on standard digraph families as well on examples from social networks and patterns of flight routes between airports.

In this paper, we introduce a symmetry geometry for all those mathematical structures which can be characterized by means of a generalization (which we call pairing) of a finite rectangular table. In more detail, let $\unicode[STIX]{x1D6FA}$ be a given set. A pairing $\mathfrak{P}$ on $\unicode[STIX]{x1D6FA}$ is a triple $\mathfrak{P}:=(U,F,\unicode[STIX]{x1D6EC})$ , where $U$ and $\unicode[STIX]{x1D6EC}$ are nonempty sets and $F:U\times \unicode[STIX]{x1D6FA}\rightarrow \unicode[STIX]{x1D6EC}$ is a map having domain $U\times \unicode[STIX]{x1D6FA}$ and codomain $\unicode[STIX]{x1D6EC}$. Through this notion, we introduce a local symmetry relation on $U$ and a global symmetry relation on the power set ${\mathcal{P}}(\unicode[STIX]{x1D6FA})$. Based on these two relations, we establish the basic properties of our symmetry geometry induced by $\mathfrak{P}$. The basic tool of our study is a closure operator $M_{\mathfrak{P}}$ , by means of which (in the finite case) we can represent any closure operator. We relate the study of such a closure operator to several types of others set operators and set systems which refine the notion of an abstract simplicial complex. [ABSTRACT FROM AUTHOR]

- Abstract
We investigated whether children with a previous Kawasaki disease (KD) have evidence of abnormal vascular and/or platelet function. We included 14 patients with previous KD and 14 matched controls. We assessed endothelial function by flow-mediated dilation (FMD), carotid intima-media thickness (cIMT), coronary microvascular function by coronary blood flow response (CBFR) to cold pressor test, and platelet reactivity by measuring monocyte-platelet aggregates (MPAs) and CD41-platelet expression by flow cytometry. No differences were found between the groups in FMD, cIMT, or CBFR to cold pressor test. The MPAs were similar in patients with KD and controls. CD41-platelet expression, however, was significantly increased in patients with KD compared with controls, both at rest (14.3 ± 1.9 vs 12.4 ± 1.9 mean fluorescence intensity [mfi], P = .01) and after adenosine diphosphate stimulation (19.3 ± 1.3 vs 17 ± 1.7 mfi, P < .001). In conclusion, children with a previous episode of KD showed increased platelet activation, compared with healthy participants despite no apparent vascular abnormality at follow-up.
