Abstract

The idea that dark matter can be made of intermediate-mass primordial black holes in the $10M_\odot \lesssim M \lesssim 200M_\odot$ range has recently been reconsidered, particularly in the light of the detection of gravitational waves by the LIGO experiment. The existence of even a small fraction of dark matter in black holes should nevertheless result in noticeable quasar gravitational microlensing. Quasar microlensing is sensitive to any type of compact objects in the lens galaxy, to their abundance, and to their mass. We have analyzed optical and X-ray microlensing data from 24 gravitationally lensed quasars to estimate the abundance of compact objects in a very wide range of masses. We conclude that the fraction of mass in black holes or any type of compact objects is negligible outside of the $0.05 M_\odot \lesssim M \lesssim 0.45 M_\odot$ mass range and that it amounts to $20 \pm5$% of the total matter, in agreement with the expected masses and abundances of the stellar component. Consequently, the existence of a significant population of intermediate-mass primordial black holes appears to be inconsistent with current microlensing observations. Abstract

Animal‐source foods (ASFs), namely, meat, milk, eggs, and derived products, are crucial components of a well‐balanced diet owing to their contribution with multiple essential nutrients. The benefits of the consumption of ASFs in terms of hedonic responses, emotional well‐being, and mood are also widely documented. However, an increasing share of consumers decide to exclude ASFs from their diets. Some of these vegan consumers are inclined to consume so‐called "meat" and/or "dairy analogs," which are produced from plant materials (soy, wheat, and oat, among others). In order to simulate appearance, texture, and flavor of ASFs, these industrial vegan foods are designed using an intricate formulation and industrial processing, which justifies their identification as ultraprocessed foods (UPFs). While the introduction of these processed vegan products is becoming popular in developed countries, the consequences of the sustained intake of these products on human health are mostly ignored. Contrarily to common belief, which emphasizes their role as "healthy" alternatives to ASFs, these plant‐based UPFs may enclose certain threats, which are reviewed in the present paper. The remarkable differences between vegan UPFs and the genuine ASFs (meat/dairy products) from sensory, nutritional, hedonic, or health perspectives precludes the designation of the former as analogs of the latter. Understanding the basis of these differences would contribute to (i) providing consumers with grounds to make reasoned decisions to consume meat/dairy products and/or the vegan alternatives and (ii) providing food companies with strategies to produce more appealing, nutritive, and healthy industrially processed vegan products. 01 februari 2023, Item does not contain fulltext, BACKGROUND: Non-motor symptoms (NMS) are a substantial burden for patients with SCA3. There are limited data on their frequency, and their relation with disease severity and activities of daily living is not clear. In addition, lifestyle may either influence or be affected by the occurrence of NMS. OBJECTIVE: To characterize NMS in SCA3 and investigate possible associations with disease severity and lifestyle factors. METHODS: In a prospective cohort study, we performed a cross-sectional analysis of NMS in 227 SCA3 patients, 42 pre-ataxic mutation carriers, and 112 controls and tested for associations with SARA score, activities of daily living, and the lifestyle factors alcohol consumption, smoking and physical activity. RESULTS: Sleep disturbance, restless legs syndrome, mild cognitive impairment, depression, bladder dysfunction and pallhypesthesia were frequent among SCA3 patients, while mainly absent in pre-ataxic mutation carriers. Except for restless legs syndrome, NMS correlated significantly with disease severity and activities of daily living. Alcohol abstinence was associated with bladder dysfunction. Patients with higher physical activity showed less cognitive impairment and fewer depressive symptoms, but these differences were not significant. CONCLUSION: This study revealed a clear association between disease severity and NMS, likely driven by the progression of the widespread neurodegenerative process. Associations between lifestyle and NMS can probably be attributed to the influence of NMS on lifestyle.

01 april 2023, Item does not contain fulltext, The European Spinocerebellar Ataxia Type 3/Machado-Joseph Disease Initiative (ESMI) is a consortium established with the ambition to set up the largest European longitudinal trial-ready cohort of Spinocerebellar Ataxia Type 3/Machado-Joseph Disease (SCA3/MJD), the most common autosomal dominantly inherited ataxia worldwide. A major focus of ESMI has been the identification of SCA3/MJD biomarkers to enable future interventional studies. As biosample collection and processing variables significantly impact the outcomes of biomarkers studies, biosampling procedures standardisation was done previously to study visit initiation. Here, we describe the ESMI consensus biosampling protocol, developed within the scope of ESMI, that ultimately might be translated to other neurodegenerative disorders, particularly ataxias, being the first step to protocol harmonisation in the field.

Abstract

Zr-containing UiO-66 and UiO-66-NH are good heterogeneous catalysts for the acetalization of phenylacetaldehyde with glycerol, producing the corresponding hyacinth fragrance in high yields after short (2 h) reaction times. Mixtures of 1,3-dioxolanes and 1,3-dioxanes are obtained, whose ratios can be modified between 2.8 and 4.6 depending on the catalyst used, the amount of missing linker defects of the solid, and the reaction time. The catalysts are stable under the reaction conditions used, and they can be reused without loss of activity or selectivity. The scope of UiO-66 materials is demonstrated for the formation of other glyceryl acetals of interest for the flavoring industry, which represents an interesting route for glycerol valorization.

Abstract

Introdução – O melanoma maligno cutâneo (MMC) é considerado uma das mais letais neoplasias e no seu seguimento recorre-se, para além dos exames clínicos e da análise de marcadores tumorais, a diversos métodos imagiológicos, como é o exame Tomografia por Emissão de Positrões/Tomografia Computorizada (PET/CT, do acrónimo inglês Positron Emission Tomography/Computed Tomography) com 18fluor-fluorodeoxiglucose (18F-FDG). O presente estudo tem como objectivo avaliar a utilidade da PET/CT relativamente à análise da extensão e à suspeita de recidiva do MMC, comparando os achados imagiológicos com os descritos em estudos CT. Metodologia – Estudo retrospectivo de 62 estudos PET/CT realizados em 50 pacientes diagnosticados com MMC. Excluiu-se um estudo cujo resultado era duvidoso (nódulo pulmonar). As informações relativas aos resultados dos estudos anatomopatológicos e dos exames imagiológicos foram obtidas através da história clínica e dos relatórios médicos dos estudos CT e PET/CT. Foi criada uma base de dados com os dados recolhidos através do software Excel e foi efectuada uma análise estatística descritiva. Resultados – Dos estudos PET/CT analisados, 31 foram considerados verdadeiros positivos (VP), 28 verdadeiros negativos (VN), um falso positivo (FP) e um falso negativo (FN). A sensibilidade, especificidade, o valor preditivo positivo (VPP), o valor preditivo negativo (VPN) e a exactidão da PET/CT para o estadiamento e avaliação de suspeita de recidiva no MMC são, respectivamente, 96,9%, 96,6%, 96,9%, 96,6% e 96,7%. Dos resultados da CT considerados na análise estatística, 14 corresponderam a VP, 12 a VN, três a FP e cinco a FN. A sensibilidade, especificidade, o VPP e o VPN e a exactidão da CT para o estadiamento e avaliação de suspeita de recidiva no MMC são, respectivamente, 73,7%, 80,0%, 82,4%, 70,6% e 76,5%. Comparativamente aos resultados CT, a PET/CT permitiu uma mudança na atitude terapêutica em 23% dos estudos. Abstract

Statins could increase the effectiveness of Helicobacter pylori eradication therapies due to their anti-inflammatory effect. The aim of this study was to analyze the impact of this therapeutic association in real life. This is a multicenter, prospective, non-interventional study aimed at evaluating the management of H. pylori by European gastroenterologists. Patients were registered in an e-CRF by AEG-REDCap from 2013 to 2020. The association between statin use and H. pylori eradication effectiveness was evaluated through multivariate analysis. Overall, 9988 and 705 patients received empirical and culture-guided treatment, respectively. Overall, statin use was associated with higher effectiveness in the empirical group (OR = 1.3, 95%CI = 1.1–1.5), but no association was found with first-line treatment effectiveness (N = 7738), as an exception, statin use was specifically associated with lower effectiveness of standard triple therapy (OR = 0.76, 95%CI = 0.59–0.99). In the rescue therapy empirical group (N = 2228), statins were associated with higher overall effectiveness (OR = 1.9, 95%CI = 1.4–2.6). However, sub-analyses by treatment schemes only confirmed this association for the single-capsule bismuth quadruple therapy (OR = 2.8, 95%CI = 1.3–5.7). No consistent association was found between statin use and H. pylori therapy effectiveness. Therefore, the addition of statins to the usual H. pylori treatment cannot be currently recommended to improve cure rates.

Abstract

ObjectiveThe best approach for Helicobacter pylori management remains unclear. An audit process is essential to ensure clinical practice is aligned with best standards of care.DesignInternational multicentre prospective non-interventional registry starting in 2013 aimed to evaluate the decisions and outcomes in H. pylori management by European gastroenterologists. Patients were registered in an e-CRF by AEG-REDCap. Variables included demographics, previous eradication attempts, prescribed treatment, adverse events and outcomes. Data monitoring was performed to ensure data quality. Time-trend and geographical analyses were performed.Results30 394 patients from 27 European countries were evaluated and 21 533 (78%) first-line empirical H. pylori treatments were included for analysis. Pretreatment resistance rates were 23% to clarithromycin, 32% to metronidazole and 13% to both. Triple therapy with amoxicillin and clarithromycin was most commonly prescribed (39%), achieving 81.5% modified intention-to-treat eradication rate. Over 90% eradication was obtained only with 10-day bismuth quadruple or 14-day concomitant treatments. Longer treatment duration, higher acid inhibition and compliance were associated with higher eradication rates. Time-trend analysis showed a region-dependent shift in prescriptions including abandoning triple therapies, using higher acid-inhibition and longer treatments, which was associated with an overall effectiveness increase (84%–90%).ConclusionManagement of H. pylori infection by European gastroenterologists is heterogeneous, suboptimal and discrepant with current recommendations. Only quadruple therapies lasting at least 10 days are able to achieve over 90% eradication rates. European recommendations are being slowly and heterogeneously incorporated into routine clinical practice, which was associated with a corresponding increase in effectiveness.

Item does not contain fulltext, BACKGROUND: Lifestyle could influence the course of hereditary ataxias, but representative data are missing. OBJECTIVE: The objective of this study was to characterize lifestyle in spinocerebellar ataxia type 3 (SCA3) and investigate possible associations with disease parameters. METHODS: In a prospective cohort study, data on smoking, alcohol consumption, physical activity, physiotherapy, and body mass index (BMI) were collected from 243 patients with SCA3 and 119 controls and tested for associations with age of onset, disease severity, and progression. RESULTS: Compared with controls, patients with SCA3 were less active and consumed less alcohol. Less physical activity and alcohol abstinence were associated with more severe disease, but not with progression rates or age of onset. Smoking, BMI, or physiotherapy did not correlate with disease parameters. CONCLUSION: Differences in lifestyle factors of patients with SCA3 and controls as well as associations of lifestyle factors with disease severity are likely driven by the influence of symptoms on behavior. No association between lifestyle and disease progression was detected. © 2021 The Authors. OBJECTIVES: To investigate the temporal dynamics of SARA item scores in SCA3 patients and evaluate if clinical and demographic factors are differentially associated with evolution of axial and appendicular ataxia. METHODS: In a prospective, multinational cohort study involving 11 European and 2 US sites, SARA scores were determined longitudinally in 223 SCA3 patients with a follow-up assessment after 1 year. RESULTS: An increase in SARA score from 10 to 20 points was mainly driven by axial and speech items, with a markedly smaller contribution of appendicular items. Finger chase and nose-finger test scores not only showed the lowest variability at baseline, but also the least deterioration at follow-up. Compared with the full set of SARA items, omission of both tests would result in lower sample size requirements for therapeutic trials. Sex was associated with change in SARA sum score and appendicular, but not axial, subscore, with a significantly faster progression in men. Despite considerable interindividual variability, the average annual progression rate of SARA score was approximately three times higher in subjects with a disease duration over 10 years than in those within 10 years from onset. CONCLUSION: Our findings provide evidence for a difference in temporal dynamics between axial and appendicular ataxia in SCA3 patients, which will help inform the design of clinical trials and development of new (etiology-specific) outcome measures. © 2022 The Authors. Abstract

Objective: The present study aimed to verify if young adult males who aimed to improve their strength or attain muscle hypertrophy trained in agreement with the current strength and conditioning guidelines. Methods: Four hundred and fourteen subjects aged 18-25 with less than one year of strength training experience and whose training goal was to increase their strength or develop muscle hypertrophy were finally included in this research. They were surveyed through an 18-item self-administered questionnaire to verify whether their strength training practices met the current strength training guidelines. Results: Overall, more than 50% of the respondents did not follow the current strength and conditioning recommendations related to muscle action, number of sets, training structure, type of resistance, training to failure, and training supervision. Only slightly more than 50% of them did not train in agreement with the recommendations regarding exercise selection, number of repetitions, and weekly training frequency. The level of compliance with the training objectives of the respondents who followed the current strength and conditioning guidelines related to exercise selection, rest, speed, type of resistance, and training supervision was higher than specific cohorts who did not. Conclusion: Training in agreement with the current strength and conditioning guidelines would ensure that young adult males with less than one year of strength training experience attain their training objectives to a greater extend. Subjects who practice strength training should follow those strength and conditioning guidelines to avoid limiting future adaptations and increase the variability of their training programs, thus preventing workout plateau and fostering motivation., Objetivo:El propósito del estudio fue verificar si adultos jóvenes de sexo masculino cuyo objetivo era mejorar su fuerza o la hipertrofia muscular entrenaban de acuerdo con las actuales recomendaciones para el entrenamiento de fuerza. Métodos: Un total de 414 hombres con edades comprendidas entre los 18 y 25 años con menos de un año de experiencia en entrenamientos de fuerza fueron finalmente incluidos en el estudio. Fueron encuestados a través de un cuestionario autoadministrado de 18 ítems para verificar si sus entrenamientos de fuerza cumplían con las recomendaciones actuales en el desarrollo de esta capacidad. Resultados: Más del 50% de los encuestados no entrenaba de acuerdo con con las recomendaciones actuales de fuerza relativas al tipo de acción muscular, número de series, estructura del entrenamiento, tipo de resistencia, carácter del esfuerzo y supervisión del entrenamiento. Apenas la mitad de los sujetos respetaron las recomendaciones relativas a la selección de ejercicios, número de repeticiones y frecuencia semanal de entrenamiento. El nivel de cumplimiento con los objetivos de entrenamiento fue más elevado entre los encuestados que siguieron las recomendaciones actuales de fuerza relativas a selección de ejercicios, descanso, velocidad de ejecución, tipo de resistencia y supervisión del entrenamiento. Conclusión: Entrenar de acuerdo a las actuales recomendaciones de entrenamiento de fuerza permitiría un mayor cumplimiento de los objetivos de los encuestados. Los trabajos de fuerza deberían realizarse de acuerdo con las actuales recomendaciones para evitar así el agotamiento prematuro de la reserva de adaptación, aumentar la variabilidad del entrenamiento, evitar estancamientos y fomentar la motivación.

33. Phylogenetic analysis of spike gene of porcine epidemic diarrhea virus in Colombia, 2014-2015

Abstract

Outbreaks of porcine epidemic diarrhea were reported in Colombia during 2014-2015. In the present analysis, complete nucleotide sequences were downloaded from the Genbank for the Spike gene of porcine epidemic diarrhea virus (PEDV) strains reported in Colombia. In the phylogenetic tree based on spike proteins of PEDV, the strains from Colombia were clustered into genotypes G2a and S-INDEL, the strains showed high homology (> 99.9%) with strains from the United States (Colorado / 2013, OH851), as well as insertions and deletions in the S1 domain of the Spike gene present both in Colombian strains and reference strains. Several amino acid substitutions have been identified in the neutralizing epitopes for the Colombian strains compared to the CV777 reference strain. These studies could help to trace the possible routes of introduction of PEDV in Colombia, recognize the phylogenetic relationships of the strains circulating in the country, and implementing control and prevention strategies., Se reportaron brotes de diarrea epidémica porcina en Colombia durante 2014-2015. En el presente análisis, se descargaron secuencias de nucleótidos completas del Genbank para el gen Spike de las cepas del virus de la diarrea epidémica porcina (PEDV) reportadas en Colombia. En el árbol filogenético basado en proteínas de espiga de PEDV, las cepas de Colombia se agruparon en los genotipos G2a y S-INDEL, las cepas mostraron alta homología (> 99,9%) con cepas de Estados Unidos (Colorado / 2013, OH851), como así como inserciones y deleciones en el dominio S1 del gen Spike presente tanto en cepas colombianas como en cepas de referencia. Se han identificado varias sustituciones de aminoácidos en los epítopos neutralizantes de las cepas colombianas en comparación con la cepa de referencia CV777. Estos estudios podrían ayudar a rastrear las posibles rutas de introducción del PEDV en Colombia, reconocer las relaciones filogenéticas de las cepas que circulan en el país e implementar estrategias de control y prevención.

Abstract

BACKGROUND & AIMS: After a first Helicobacter pylori eradication attempt, approximately 20% of patients will remain infected. The aim of the current study was to assess the effectiveness and safety of second-line empiric treatment in Europe. METHODS: This international, multicenter, prospective, noninterventional registry aimed to evaluate the decisions and outcomes of H pylori management by European gastroenterologists. All infected adult cases with a previous eradication treatment attempt were registered with the Spanish Association of Gastroenterology-Research Electronic Data Capture until February 2021. Patients allergic to penicillin and those who received susceptibility-guided therapy were excluded. Data monitoring was performed to ensure data quality. RESULTS: Overall, 5055 patients received empiric second-line treatment. Triple therapy with amoxicillin and levofloxacin was prescribed most commonly (33%). The overall effectiveness was 82% by modified intention-to-treat analysis and 83% in the per-protocol population. After failure of first-line clarithromycin-containing treatment, optimal eradication (>90%) was obtained with moxifloxacin-containing triple therapy or levofloxacin-containing quadruple therapy (with bismuth). In patients receiving triple therapy containing levofloxacin or moxifloxacin, and levofloxacin-bismuth quadruple treatment, cure rates were optimized with 14-day regimens using high doses of proton pump inhibitors. However, 3-in-1 single capsule or levofloxacin-bismuth quadruple therapy produced reliable eradication rates regardless of proton pump inhibitor dose, duration of therapy, or previous first-line treatment. The overall incidence of adverse events was 28%, and most (85%) were mild. Three patients developed serious adverse events (0.3%) requiring hospitalization. CONCLUSIONS: Empiric second-line regimens including 14-day quinolone triple therapies, 14-day levofloxacin-bismuth quadruple therapy, 14-day tetracycline-bismuth classic quadruple therapy, and 10-day bismuth quadruple therapy (as a single capsule) provided optimal effectiveness. Abstract

Parkinson’s disease is a neurodegenerative movement disorder that currently has no disease-modifying treatment, partly owing to inefficiencies in drug target identification and validation. We use Mendelian randomization to investigate over 3,000 genes that encode druggable proteins and predict their efficacy as drug targets for Parkinson’s disease. We use expression and protein quantitative trait loci to mimic exposure to medications, and we examine the causal effect on Parkinson’s disease risk (in two large cohorts), age at onset and progression. We propose 23 drug-targeting mechanisms for Parkinson’s disease, including four possible drug repurposing opportunities and two drugs which may increase Parkinson’s disease risk. Of these, we put forward six drug targets with the strongest Mendelian randomization evidence. There is remarkably little overlap between our drug targets to reduce Parkinson’s disease risk versus progression, suggesting different molecular mechanisms. Drugs with genetic support are considerably more likely to succeed in clinical trials, and we provide compelling genetic evidence and an analysis pipeline to prioritise Parkinson’s disease drug development., There is currently no disease-modifying treatment for Parkinson’s disease, a common neurodegenerative disorder. Here, the authors use genetic variation associated with gene and protein expression to find putative drug targets for Parkinson’s disease using Mendelian randomization of the druggable genome.
