INTRODUCTION: Many studies are examining the relationship between cardiovascular diseases and triglyceride levels, and it has been shown that hypertriglyceridemia significantly increases the risk independently. In this study, we wanted to investigate whether there is a similar relationship for stroke. METHODS: A total of 619 patients hospitalized with a diagnosis of acute ischemic stroke in our stroke center were included in the study. Demographic information, accompanying risk factors, hemoglobin level, thrombocyte count, triglyceride level, and C reactive protein level at the time of admission, and the National Institutes of Health Stroke Scale (NIHSS) scores in the first evaluation were recorded. Also, information about mortality in hospital, recurrent stroke, and the modified Rankin Scale (mRS) scores in the follow-up period was entered in our local database. RESULTS: While 182 patients(32.3%) [78 females (42.9%) and 104 males (57.1%); mean age 64.5 +- 12.01 years] were diagnosed with hypertriglyceridemia, this number was 25(4.4%) patients (13 females (52%) and 12 males (48%); mean age 70.9 +- 17.4 years) for hypotriglyceridemia. Epilepsi HastalarındaRutin, Bir Saatlik Ve Tüm Gece EEEG’nin İED Saptamaya KatkısıAmaç: Rutinelektroensefalogram (EEG) incelemelerinde interiktal epileptiform İED saptamaoranı %12-53 arasında değişmektedir. İnteriktal epileptiform deşarj yakalamakiçin tekrarlayan rutin EEG, daha uzun süreli uyku aktivasyonlu veya tüm geceEEG’ler kulanılabilir. Yöntem: Buçalışmada Ocak 2002-Haziran 2022 tarihleri arasında epilepsi tanısı ileizlenen, hem rutin EEG’si, hem bir saatten uzun vEEG’si, hem de tüm gece vEEG’siolan 73 hastayı retrospektif olarak inceledik. EEG’lerinde İED olup olmadığınıkontrol ettik. İED varsa fokal veya jenaralize oluşu, fokalse İED’ninlokalizasyonunu not ettik. Bulgular: İnteriktalepileptiform deşarj rutin EEG’de 30 (%41,1), bir saatten uzun vEEG’de 38(%52,1), tüm gece EEG’de 41 (%56,2) hastada izlendi. Tüm gece EEG ile birsaatten uzun vEEG, İED yakalamada rutin EEG’den istatiksel olarak anlamlıderecede üstün bulundu (p=0,047,p=0,010). Tüm gece vEEG ile bir saatten uzun vEEG karşılaştırıldığında, tümgece vEEG’de İED daha fazla saptanmasına rağmen istatiksel anlamlı farksaptanmadı (p=0,084).Sonuç:Çalışmamızda İED yakalamada hem tüm gece hem de bir saatten uzun vEEG’nin,rutin EEG çekimlerine göre daha üstün olduğunu bulduk. Tüm gece ile bir saatlik vEEG ise İED saptamaaçısından anlamlı fark izlenmedi. Rutin EEG incelemelerine göre daha rahatortamda çekilen 1-3 saatlik vEEG, hastanın uyku evrelerini içerdiği için İEDyakalamada tüm gece EEG ile benzer bulunmuştur. VEEG laboratuvarlarının işyükünü azaltmak için tüm gece yerine gündüz 1-3 saatlik çekimlerdenyararlanılabilir.&nbsp

BİR MOTOR NÖRON HASTALIĞI NEDENİ KALSİYUM KANALOPATİSİ:OLGU SUNUMUGİRİŞ VE AMAÇ Motor nöron hastalığı (MNH), motor nöronların ilerleyici dejenerasyonu ile karakterize sporadik ve herediter olabilen nörolojik bir hastalıktır. MNHnin sporadik amyotrofik lateral skleroz (ALS) için başlangıç yaşı ortalama 58 ila 63, ailesel ALS için ise 40-60 yaşları olan orta yaş bir hastalık olduğu gösterilmiştir. Bu yazıda aile öyküsü olmayan erken başlangıçlı motor nöron hastalığı ile izlediğimiz ve genetik değerlendirmede kalsiyum kanalopatisi izlenen olguyu sunmaktayız.OLGU 33 yaşında erkek hasta yaklaşık 7-8 yıl önce tüm vücutta seyirmeler sonrası 2 yıl önce eklenen dengesizlik, konuşmada bozulma ve mevcut şikayetlerinin son 6 ayda artış göstermesi üzerine tarafımıza başvurdu. Özgeçmisinde bilinen hastalık öyküsü ve ailede genetik hastalık öyküsü yoktu. Anne baba arasında akrabalık yoktu. Başvurusundaki muayenesinde bilinç açık, koopere, oryante izlenen hastanın kraniyal alan muayenesinde glob hareketleri dört yöne doğal, fasial asimetrisi izlenmedi. Uvula orta hatta ve palatal arklar eşit havalanıyordu. Dil hareketleri normal, ancak dilde fasikülasyonlar izlendi. Konuşması ılımlı spastik dizartrik, konuşma yavaşlamış kelimeler anlaşılırdı. Üst ekstremitede omuz kuşağında atrofisi mevcuttu. Deltoid, biceps ve triceps kas gücü 4/5, distalde elin intrensek kaslarında ve başparmak oppozisyonunda zaaf izlendi. Bilateral tricepslerde fasikülasyonları mevcuttu. Alt ekstremitede kas gücü muayenesi tam, topukta ve parmak ucunda yürüyebilen hastanın aşil klonusu ve alt ekstremitede spastisitesi mevcuttu, romberg negatifdi. Bilateral hoffman bulgusu izlendi, altta belirgin olmak üzere dört yanlı derin tendon refleksleri artmıştı. Sfinkter kusuru yoktu. Hastanın yapılan kraniyal MRG ‘si normal sınırlarda, servikal MRG sinde sağ C3-4 de ekstrüde disk hernisi ve C5-6-7 de sinir basısı olmayan bulgingleri izlendi. Yapılan EMG sinde sinir iletim çalışmaları normal, iğne EMG sinde kraniyal, servikal, torakal alanda yaygın denervasyon ve fasikülasyon potansiyelleri ile beraber nörojenik nitelikli motor ünite aksiyon potansiyelleri (MÜAP) saptanmıştı. Klinik bulguları ve EMG bulguları ile beraber hasta amiyotrofik lateral skleroz ile uyumlu değerlendirildi. Ayırıcı tanılara yönelik hastanın değerlendirmesi yapıldı. Laboratuar tetkiklerinde hemogram normal, biyokimyasal parametrelerinde ılımlı CK artışı mevcuttu. B12 ve hormon testleri, vaskülit markerları normal sınırlardaydı. İmmünfiksasyon elektroforezinde yoğun poliklonal artış izlendi. Viral serolojisinde anlamlı patoloji saptanmadı. Paraneoplazi taramasında anlamlı patoloji saptanmadı. Hastanın başlangıç yaşı motor nöron hastalığı için nonspesifik olduğundan tarafımıza refere edilen merkezde genetik açıdan yapılan değerlendirmesinde CACNA1G geninde (EKZON 1-38). HETEROZİGOT c.3197C>T (p.Ser1066Leu) değişiklik saptandı. Kasılma ve seyirme tarifleyen hastaya kademeli artışla semptomatolojisine yönelik lamotrijin başlandı. 8 aydır takibimizde olan hastanın lamotrijin doz artışıyla beraber kasılmalarında bir miktar azalma oldu ve hasta kendini daha iyi hissettiğini ifade etti. Hastanın klinik bulguları stabil seyretmektedir.TARTIŞMA ve SONUÇ Motor nöron hastalığı (MNH), motor nöronların ilerleyici dejenerasyonu ile karakterize sporadik ve herediter olabilen nörolojik bir hastalıktır. Motor nöron hastalığı insidansının 100.000 popülasyonda 2 ila 3e yaklaştığı gösterilirken %10-15 civarı aileseldir. Üst motor nöronları, alt motor nöronları veya her ikisini de etkileyebilir. Motor nöron hastalığının prognozu, başlangıç yaşına ve etkilenen merkezi sinir sisteminin alanına bağlıdır. Motor nöron hastalığının köken yerine ve nörolojik tutulumun ciddiyetine bağlı olarak dört ana fenotipi vardır: Amyotrofik lateral skleroz, progresif bulbar palsi, progresif musküler atrofi ve primer lateral skleroz (1).ALS tanısı diğer etiyolojilerin ekarte edilmesine ve hastalığın ilerlemesinin doğrulanmasına dayanır. Çeşitli tanı kriterleri arasında El Escorial, Airliehouse, Awaji ve ALS tanı indeksi yer alır. Bu kriterler klinik, elektrofizyolojik veya nöropatolojik alt motor nöron dejenerasyonunun gösterilmesi, üst motor nöronların klinik tutulumunun kanıtı ve hastalığın ilerlemesidir (2). ALS, ailesel vakaların yaklaşık %5-10unda Mendel kalıtım modeline sahip karmaşık bir genetik bozukluk olarak kabul edilir, ancak çoğu hastada, daha sonra sporadik veya izole (sALS) olarak adlandırılan hastalıkla ilgili fark edilebilir bir aile öyküsü yoktur (3). Bozulmuş protein homeostazı, anormal RNA metabolizması, mitokondriyal disfonksiyona yol açan genetik mutasyonlara atfedilen, birbiriyle bağlantılı çoklu hücre içi süreçlerin düzensizliğinin bir sonucu olarak ortaya çıkan motor nöron hasarı , bozulmuş DNA onarımı, oksidatif stres bunların tümü nörotoksisite gelişiminde rol oynar. Bunlar dışında etiyopatogenezde nöronal hipereksitabilite ve aksonal disfonksiyon da rol oynamaktadır (3). Nöronal uyarılabilirlikde rolü olan ve pek çok nörolojik hastalıkla ilişkilendirilen kanalopatilerin motor nöron hastalıkların etyopatogenezinde rol alabileceği bildirilmiştir. BunlardanT-tipi kalsiyum kanalları, nöronal uyarılabilirlik, hormon ve nörotransmitter salınımının yanı sıra çeşitli hücresel ve fizyolojik fonksiyonlara katkıda bulunan düşük voltajla aktive olan kalsiyum kanallarıdır.Epilepsi, otizm spektrum bozuklukları, şizofreni, motor nöron hastalıkları, ataksi ve aldosteronizm dahil olmak üzere çeşitli hastalıklar T-tipi kalsiyum kanallarını kodlayan genlerdeki mutasyonlarda izlenmiştir (3). T tipi kalsiyum kanalları elektrofizyolojik ve farmakolojik özelliklerinde de çeşitli farklılıklar gösterirler.Bu heterojenlik kısmen insanlarda sırasıyla CACNA1G,CACNA1HveCACNA1Igenleri tarafından kodlanan kanallar olmak üzere üç T tipi kanal izoformunun varlığıyla açıklanır. CACNA1H kanalopatilerinin idiyopatik jeneralize epilepsi, otizm spektrum bozuklukları, amiyotrofik lateral skleroz, kronik ağrı ve primer aldosteronizmde rolü olduğu gösterilmiştir.Son çalışmalar motor nöronlarda T-tipi kanalların ekspresyonunu bildirmiş olsa da, bu nöronlarda T-tipi kanalların fonksiyonel anlamı henüz analiz edilmemiştir.Artan nöronal uyarılabilirlik, sodyum iletkenliğinde bir artış ve aksonal potasyum akımlarında bir azalmanın gözlendiği ALSde ayırt edici bir özellik olarak rapor edilmiştir.Kalsiyumla aktive olan potasyum kanallarının kontrolünde T-tipi kanalların rolü göz önüne alındığında, ALS ile ilişkili mutasyonların neden olduğu T-tipi kanal aktivitesinin azalmasının potasyum akımlarının değişmesine katkıda bulunma olasılığı vardır.Ek olarak, yakın tarihli bir bulgu, nöronal progenitör hücre canlılığının korunmasında T-tipi kanalların rolünü göstermiştir (3).Yakın zamandaiki kalıtsal heterozigot CACNA1Hmutasyonutaşıyan şiddetli infantil başlangıçlı amiyotrofisi olan olgu bildirilmiştir (4). Bizim hastamızda CACNA1G geninde heterozigot mutasyon izlendi. Literatürde CACNA1G ile bildirilmiş ALS vakasına rastlamadık. TÜM GECE EEG’NİN İEDSAPTAMAYA KATKISIAmaç: Epilepsitanısı ve sınıflandırılmasında interiktal epileptiform (İED) deşarjlar sıklıklakullanılır. Rutin elektroensefalogram (EEG) incelemelerinde İED saptama oranı%12-53 arasında değişmektedir. İnteriktal epileptiform deşarj yakalamak içinkullanılan diğer yol ise tüm geceyi içeren uzun EEG çekimidir. Yöntem: Bu çalışmada Şubat 2019-Şubat2022 tarihleri arasında videoEEG monitorizasyon merkezimizde bir saatten uzun (>1 sa) ve/veya tümgece(ortalama 12 sa) EEG çekilen 257 hastayı retrospektif olarak inceledik. Buhastaların bir saatten kısa (Sonuç: Tüm geceEEG çekilen 127 hasta, bir saatten uzun EEG çekilen 104 hasta ve hem bir saattenuzun hem tüm gece EEG’si olan 26 hasta vardı. İnteriktal epileptiform deşarj, 1sa EEG’de46(%35,1), tüm gece EEG’de 75( %49) hastada izlendi. Tüm gece EEG ile 1 sa hem tüm gece EEG çekimi olan 26 hastadeğerlendirildiğinde, tüm gece EEG’de İED daha fazla saptanmasına rağmenistatiksel anlamlı fark bulunmadı (p=0,97). Trametes versicolor (L.) Lloyd known as turkey tail, is a medicinal mushroom belonging to the Polyporaceae. Although the consumption and commercial sale of this mushroom in our country is new, it has been used for centuries as a medicine in some countries, especially in China. In this study, it was aimed to determine the antimicrobial and antioxidant effects of ethanol and methanol extracts of T. versicolor. Its antimicrobial effects were determined by disk diffusion and microdilution method using pathogenic microorganisms such as Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Bacillus megaterium, Staphylococcus aureus, Candida albicans and Trichophyton sp. Total antioxidant level, total oxidant level and DPPH radical scavenging capacity were detected for the antioxidant activity of the mushroom. According to the results obtained, it was seen that these extracts inhibit the growth of microorganisms at different rates (10-21 mm) according to the disk diffusion method. The minimal inhibitory concentrations of T. versicolor against microorganisms used were determined to be between 62.5-250 µg/mL. The TAS and TOS values of the methanol extract were 0.72 mmol Trolox Equiv./L and 18.39, respectively, the TAS and TOS values of the ethanol extract were detected 0.88 mmol Trolox Equiv./L and 16.71 μmol H2O2 Equiv./L, respectively.

INTRODUCTION[|]Despite the lower frequency in the young than the older population, ischemic stroke has a significant impact on the patients and their families, and the society. In this study, ischemic stroke patients under the age of 50 years registered in our stroke database were compared with elderly patients in terms of risk factors and etiologic classification. Furthermore, ischemic stroke etiology with emphasis on uncommon causes in the young population was evaluated.[¤]METHODS[|]In this study, we retrospectively reviewed the data of 619 patients who were admitted with acute ischemic stroke to our hospital between January 2011 and November 2014. The patients were divided into two groups as young ischemic stroke group (patients under the age of 50) and the others with the age of ≥50 years. All registered demographic, clinical and paraclinical data of the patients were evaluated. [¤]RESULTS[|]Thirty-two (5.2%) patients were under 50 years of age and 587 (94.8%) were ≥50 years of age. As vascular risk factors, hypertension, diabetes mellitus, atrial fibrillation, and coronary artery disease were found to be significantly higher in the ≥50 age group (p˂0.05). Patent foramen ovale as a risk factor of stroke was significantly more frequent in the young group. In terms of stroke etiology, cardioembolic causes were the most common in both age groups, but the etiologic subgroup of uncommon causes was found to be more frequent in the younger population.[¤]DISCUSSION AND CONCLUSION[|]In our study, the risk factors and etiologic subgroups of stroke in patients younger than fifty years were found different from those in older patients. The relatively higher frequency of uncommon causes of ischemic stroke in the young population should be considered in etiology research and treatment strategies.[¤]

To assess whether trainees can learn and implement the operational definition of interictal epileptiform discharges (IEDs) of the International Federation of Clinical Neurophysiology (IFCN), based on six morphological criteria, and whether its implementation improves their diagnostic performance and inter-rater agreement (IRA). Seven trainees evaluated a balanced dataset of 70 EEG samples containing sharp transients (35 from patients with epilepsy and 35 from patients with non-epileptic paroxysmal events). The gold standard was derived from video-EEG recordings of the habitual clinical episodes. The trainees individually reviewed the EEGs, blinded to all other data, in two successive training sessions, three months apart. The second session was preceded by a teaching module about the IFCN criteria, and the trainees implemented them during the second reading session. By implementing the IFCN criteria, trainees significantly improved their specificity (94.29% vs. 77.14%; p=0.01) and overall accuracy (81.43% vs. 64.29%; p=0.01) for identifying IEDs. Sensitivity also improved but did not reach the level of statistical significance (77.14% vs. 60%; p=0.07). IRA improved significantly from fair (k=0.31; 95% CI: 0.22-0.40) to high-moderate (k=0.56; 95% CI:0.46-0.67) beyond-chance agreement. Implementing the IFCN criteria significantly improves the diagnostic performance and IRA of trainees in identifying IEDs. Teaching the IFCN criteria for IEDs will increase specificity in clinical EEG and avoid over-reading, the most common cause of misdiagnosing epilepsy.

In this study, the effects of different cellulosic wastes on the nutritional values of Pleurotus ostreatus (Jacq.) P. Kumm. were investigated. Dry matter, moisture, protein, fat and ash contents of P. ostreatus were analyzed according to AOAC methods. Crude protein and fat contents were determined by the Kjeldahl method and Soxhlet extraction, using ether as a solvent, respectively. Chemical composition of P. ostreatus such as dry matter, moisture, crude protein, fat and organic matter were 93.5-93.7%, 6.3-6.5%, 25.2-33.5%, 1.7-2.5%, 5.3-9.7% and 84.0-88.2% of dry weight, respectvely. Onabotulinum toxin A (ONA, Botox®) and abobotulinum toxin A (ABO, Dysport®) are most frequently used in the treatment of movement disorders. The aim of this study was to identify the dose conversion ratio (ABO dose:ONA dose), comparative efficacy, and adverse events in patients who switched from ONA to ABO.There were 64 patients with cervical dystonia (39), hemifacial spasm (16), oromandibular dystonia (5), blepharospasm (3), and extremity dystonia (1) who switched from ONA to ABO. The efficacy, adverse events, duration of action, and severity of the adverse events after the final dose of ONA, initial dose of ABO, and second dose of ABO were investigated in these patients.The mean dose conversion ratio was 4.70 (2.27-9.62). The mean efficacy of the final ONA injection was 70.62%; initial ABO injection, 72.27%; and second ABO injection, 73.52%, which showed improvement on a visual analog scale (p = 0.71, p = 0.5). Incidence of adverse events after the final ONA injection was 18.8%; this increased to 39.1% after the initial ABO injection (p 0.001) and decreased to 14.1% after the second ABO injection (p = 0.77). After the initial ABO injection, 20% of the adverse events were trivial, 36% were mild, and 32% were severe. After the second ABO injection, 7.8% of the adverse events were mild and 6.3% were severe.Although the mean dose conversion ratio was 4.70, the range was very wide (approximately 2-9). Therefore, we conclude that after the switch from Botox to Dysport, the doses should be tailored to the patients' clinical situation at treatment initiation, without using a dose conversion ratio.

Ficaria verna Huds. is a plant belonging to the Ranunculaceae family, known as mole grass and celandine among the people. It is known to have anti-inflammatory and anti-haemorrhagic pharmaceutical effects. In this study, it was aimed to determine the antimicrobial effect of different concentrations of F. verna extracts obtained from methanol, ethanol and chloroform and the antioxidant activity of different concentrations of the extract obtained from methanol. In the results obtained, the best antimicrobial effect (17-20 mm) against Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, Bacillus megaterium, Salmonella thypii and Candida albicans was determined in the methanol extract of F. verna at a concentration of 1000 µg. It was observed that the scavenging effect of the DPPH radical of F. verna increased depending on increasing concentrations., Ficaria verna Huds. halk arasında köstebek otu ve kırlangıç otu adıyla bilinen ve Ranunculaceae familyasına ait bir bitkidir. Elde edilen sonuçlarda F. verna’nın metanol ekstresi 1000 µg konsantrasonda Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, Bacillus megaterium, Salmonella thypii ve Candida albicans’a karşı en iyi antimikrobiyal etki (17-20mm) göstermiştir. F. verna’nın DPPH radikalinin süpürücü etkisinin artan konsantrasyonlara bağlı arttığı gözlemlenmiştir.

Background and Aims: This study aims to investigate the cytotoxicity and DNA protective effects of semi-arid, arid, or desert truffles such as the Terfezia and Picoa species. Methods: The DNA protective effects of the Terfezia and Picoa species was evaluated using plasmid pBR322 DNA treated with UV and H2O2. The cytotoxic effects of water and methanol extracts of the Terfezia and Picoa species on H1299 and HUVEC cells were evaluated using the MTT assay. Results: Higher concentrations of T. olbiensis and P. lefebvrei extracts have demonstrated a DNA protective effect. The water extract of P. juniperi only at a 40 mg/mL concentration demonstrated a DNA-protective effect, whereas T. boudieri extract did not show any DNA-protective effect at all concentrations tested. H1299 cells showed more sensitivity to the water extract of T. olbiensis (4.66%), P. juniperi (12.04%), P. lefebvrei (21.93%), and the methanol extract of T. boudieri (20.93%). In general, the water and methanol extracts of the Terfezia and Picoa species demonstrated the least cytotoxic effects on HUVEC cells, except for the water extract of P. juniperi (38.46%). Conclusions: In conclusion, results obtained from this study show that the Terfezia and Picoa truffle species have potential DNA protective and cancer prevention properties.
