Abstract Background Artificial intelligence (AI) is revolutionizing cephalometric diagnosis in orthodontics, streamlining the patient assessments. This study aimed to assess the reliability, accuracy, and time consumption of artificial intelligence (AI)-based software compared to a conventional digital cephalometric analysis method on 2D lateral cephalogram. Methods 408 lateral cephalometries were analysed using three methods: manual landmark localization, automatic localization, and semi-automatic localization with AI-based software. On each lateral cephalogram, 15 variables were selected, including skeletal, dental, and soft tissue measurements. The difference between the two AI-based software options (automatic and semi-automatic) was compared with the conventional digital technique. The time required to produce a complete cephalometric tracing was evaluated for each method using Student’s t-test. 7. Seabird nutrients increase coral calcification rates and boost reef carbonate production

Abstract While excessive anthropogenic nutrient loads are harmful to coral reefs, natural nutrient flows can boost coral growth and reef functions. Here we investigate if seabird-derived nutrient subsidies benefit the growth of two dominant corals on lagoonal reefs, submassive Isopora palifera and corymbose Acropora vermiculata, and if enhanced colony-level calcification rates can increase reef-scale carbonate production. I. palifera and A. vermiculata colonies close to an island with high seabird densities displayed 1.4 and 3.2-times higher linear extension rates, 1.8 and 3.9-times faster planar area increase, and 1.6 and 2.7-times higher calcification rates compared to colonies close to a nearby island with low seabird densities, respectively. While benthic ReefBudget surveys in combination with average coral growth rates did not indicate differences in reef-scale carbonate production across sites, coral carbonate production was 2.2-times higher at the seabird-rich island when using site-specific linear growth rates and skeletal densities. This study shows that seabird-derived nutrients benefit fast-growing branching as well as previously unstudied submassive coral taxa. It also demonstrates that nutrient subsidies benefit colony-scale and reef-scale calcification rates, which underpin important geo-ecological reef functions. Restoring natural nutrient pathways should thus be a priority for island and reef management.

Aim: The present study aimed to determine the profile of micronutrients and inorganic contaminants in the plant-based and dairy food products most consumed by the Portuguese population. Methods: The sampling plan followed the Total Diet Studies (TDS) methodology and included representative samples of the Portuguese vegetarian diet, chosen based on the National Food and Physical Activity Survey. Five main food groups were selected: Grains and grain-based products (n = 48); Dairy products (n = 60); Products for non-standard diets (n = 72); Pulses, dried fruits, and oilseeds (n = 132); and Fruiting vegetables (n = 12). The sampling plan included 324 individual samples, prepared as 27 pooled samples for laboratory analyses. Each pooled sample was analyzed for 13 elements: As, Cd, Co, Cr, Cu, I, Li, Mn, Mo, Pb, Se, Sr, and Zn. The methodology used for the analysis was ICP-MS with samples previously subjected to acid digestion through closed vessel microwave-assisted, except for I, which required an alkaline extraction. Results: The results of this study indicate that food samples from the group of Pulses, dried fruits, and oilseeds had the highest content of Mn, Zn, Mo, and Se. On the other hand, Products for non-standard diets and Dairy products had the highest content of Cu and I, respectively. The As, Cd, and Pb levels were below the limit of quantification for most of the analyzed samples from every food group. Conclusions: This research enabled updating the Portuguese Food Composition Database (FCD), underscoring the importance of regular dietary assessments and the role of FCDs and TDS in safeguarding public health by ensuring nutritional adequacy and safety in the food supply.

10. Prescription of psychotropic drugs by non-psychiatrist specialists in Morocco: current practices and educational needs

Abstract Introduction Mental health is considered a major public health issue. Non-psychiatric physicians often engage in the treatment of mental disorders. The aim of this study was to describe psychiatric drug prescription knowledge and practices among non-psychiatric specialists and evaluate their training needs. Methods A descriptive cross-sectional study was conducted from September 1st to October 15th, 2021, in 3 Moroccan healthcare facilities and among private practitioners in Kenitra. We asked non-psychiatric specialists about their knowledge and current practices regarding psychotropic drugs, and their needs in psychiatric training. Results The study included a total of 150 participants. The majority of participants demonstrated insufficient knowledge regarding the selection of psychotropic drugs and the duration of pharmacotherapy. Specifically, 61.3% were unaware of the average duration of treatment for depression. 22.7% of participants did not feel comfortable when prescribing psychotropic drugs. Anxiolytics were the most commonly prescribed class of psychotropic drugs, accounting for 30.7% of prescriptions. The most common indications for psychotropic drugs prescription were anxiety (35.3%), followed by insomnia (34.7%) and depression (31.3%). The majority of participants (72%) reported receiving clinical training in psychiatry, with 74.7% expressing varying levels of satisfaction with their undergraduate psychiatry training, while 7.3% expressed dissatisfaction. Regarding CME, only 11.3% of participants engaged in at least one psychiatry-related CME session in the past two years. 54.7% of participants expressed interest in expanding their knowledge of prescribing psychotropic drugs. Around 40% of participants preferred trainings in psychotropic drugs prescription related to their specialty, while 34% were not interested in receiving further training. Conclusions Our study shows gaps in knowledge of non-psychiatric specialists, which raises concern regarding their ability to care for mental disorders. Educational efforts should be made to improve teaching of psychiatry from the undergraduate level. Continuing Medical Education should be tailored to the specific needs and preferred learning methods of non-psychiatric physicians.

This study aimed to assess the preference and use of an artificial shelter in dairy cows managed outdoors at calving during winter. We also evaluated whether this preference would be influenced by weather conditions (rain, air temperature, or wind) or the time of the day at calving. Two weeks before their expected calving date, 18 cows were paired based on body weight, body condition score, parity, and expected calving date. Each pair was assigned to an open paddock (42 m2/cow) with a bare soil surface, high mud content, and access to an artificial shelter until calving. The shelter consisted of a 6 × 3 m metal structure, with 3 sides covered with zinc sheets, a polycarbonate roof, and a thick layer of dry sawdust covering the ground surface. Shelter use and cow behavior inside it (lying and sleeping) were continuously recorded via video and analyzed using continuous observation. Observations were divided into 2 periods: the day before calving (24 to 48 h before calving) and the day of calving (24 h before calving until calving). Ninety-four percent (15/16) of the cows preferred to calve inside the artificial shelter, and this preference was not affected by the time of day or weather conditions. Regardless of the day of study (the day before calving or the day of calving), cows spent approximately 64% of their daily time inside the shelter, and once inside, they spent most of the time lying down. Our findings indicate dairy cows prefer using an artificial shelter as a calving and lying place, suggesting that opportunities for protection should be provided when they are managed outdoors in muddy paddock conditions.

Presentamos a un hombre de 26 años que acude al servicio de urgencias por fiebre, mialgias, hipersudoración y pérdida de peso. En el análisis se observa pancitopenia, velocidad de sedimentación aumentada y enzima convertidora de angiotensina >150 U/L; esplenomegalia, adenopatías retroperitoneales peri aórtico-cava confirmadas por imagenología. Tras excluir las causas más frecuentes, hizo un lavado bronco alveolar con una proporción elevada de CD4/CD8 y linfocitosis superior al 50%, así como granulomas epitelioides confluentes no caseosos en la biopsia de adenopatía, estableciendo el diagnóstico de sarcoidosis en estadio III.

The incidence of primary hypertension is on the rise in the pediatric population, with an approximate prevalence ranging from 3% to 5%. The most significant risk factors for the onset and development of primary hypertension are well-known, and some of them are preventable, including increased salt consumption and obesity. Connected with metabolic risk factors, elevated blood pressure in childhood is carried into adulthood. Primary hypertension is associated with attenuated vascular responses to various physiological stimuli in both peripheral microcirculation and systemic macrocirculation in adults and children. Endothelial dysfunction is one of the most important features of arterial hypertension, together with an increased level of oxidative stress – both determinants significantly contribute to all the pathophysiological changes observed in hypertension. Systemic arterial hypertension has emerged as a prevalent cardiovascular risk factor associated with substantial morbidity and mortality. Hence, the timely identification of individuals with elevated blood pressure and early-life blood pressure management could serve as a crucial strategy to mitigate the risk of cardiovascular disease and mortality in adulthood.

Abstract Enhancing the performance of noisy quantum processors requires improving our understanding of error mechanisms and the ways to overcome them. A judicious selection of qubit design parameters plays a pivotal role in improving the performance of quantum processors. In this study, we identify optimal ranges for qubit design parameters, grounded in comprehensive noise modeling. To this end, we also analyze the effect of a charge-parity switch caused by quasiparticles on a two-qubit gate. Due to the utilization of the second excited state of a transmon, where the charge dispersion is significantly larger, a charge-parity switch will affect the conditional phase of the two-qubit gate. We derive an analytical expression for the infidelity of a diabatic controlled-Z gate and see effects of similar magnitude in adiabatic controlled-phase gates in the tunable coupler architecture. Moreover, we show that the effect of a charge-parity switch can be the dominant quasiparticle-related error source of a two-qubit gate. We also demonstrate that charge-parity switches induce a residual longitudinal interaction between qubits in a tunable-coupler circuit. Furthermore, we introduce a performance metric for quantum circuit execution, encompassing the fidelity and number of single- and two-qubit gates in an algorithm, as well as the state preparation fidelity. This comprehensive metric, coupled with a detailed noise model, enables us to determine an optimal range for the qubit design parameters, as confirmed by numerical simulation. Our systematic analysis offers insights and serves as a guiding framework for the development of the next generation of transmon-based quantum processors.

Abstract Introduction Certolizumab pegol (CZP) is an anti-tumor necrosis factor alpha (TNFα) approved for the treatment of moderate to severe plaque psoriasis (PSO). However, data on its real-world use is currently limited. The objective of this study was to describe the 1-year real-world effectiveness of CZP, its impact on health-related quality of life (HRQoL), and safety outcomes in patients with moderate to severe PSO in multi-country settings. Methods CIMREAL, a prospective, noninterventional study, was conducted across Europe and Canada from August 2019 to December 2022. Patients were followed for 1-year, receiving CZP 400 mg initial doses at weeks 0, 2, and 4, followed by CZP 200 mg every 2 weeks (Q2W) or CZP 400 mg Q2W maintenance dosing. Effectiveness was assessed using the Psoriasis Area and Severity Index (PASI) and Dermatology Life Quality Index (DLQI). Safety was also evaluated. Results Overall, 399 patients with moderate to severe PSO were included. Of these, 93.7% (374/399) and 77.9% (311/399) completed months 3 and 12, respectively. Mean age (± standard deviation) was 42.9 ± 13.5 years and body mass index was 28.5 ± 6.8 kg/m2, with the majority of patients being female (68.2%). At 12 months, CZP showed substantial effectiveness, achieving PASI 75 and PASI 90 response rates (≥ 75% and ≥ 90% improvement from baseline, respectively) of 77% and 56.5%, respectively. Patients with PASI score of ≤ 3 and ≤ 2 experienced improvement from 3 months (49.8% and 41.1%, respectively) to 12 months (82.0% and 75.3%, respectively). HRQoL considerably improved, with mean DLQI scores decreasing from 12.4 to 2.3 after 12 months of treatment, and the proportion of patients with DLQI 0/1 increased from 28.6% at 3 months to 59.4% at 12 months. The 1-year probability of persistence was approximately 85%. Overall, 30.6% of the patients experienced any adverse events and 9.3% had serious adverse events. Conclusion In routine clinical practice, CZP exhibited consistent effectiveness, positively impacting both skin psoriasis activity and HRQoL. The 1-year persistence of CZP was high, and no new safety signals were identified. Abstract Mitochondria require an extensive proteome to maintain a variety of metabolic reactions, and changes in cellular demand depend on rapid adaptation of the mitochondrial protein composition. The TOM complex, the organellar entry gate for mitochondrial precursors in the outer membrane, is a target for cytosolic kinases to modulate protein influx. DYRK1A phosphorylation of the carrier import receptor TOM70 at Ser91 enables its efficient docking and thus transfer of precursor proteins to the TOM complex. Here, we probe TOM70 phosphorylation in molecular detail and find that TOM70 is not a CK2 target nor import receptor for MIC19 as previously suggested. Instead, we identify TOM20 as a MIC19 import receptor and show off-target inhibition of the DYRK1A-TOM70 axis with the clinically used CK2 inhibitor CX4945 which activates TOM20-dependent import pathways. Taken together, modulation of DYRK1A signalling adapts the central mitochondrial protein entry gate via synchronization of TOM70- and TOM20-dependent import pathways for metabolic rewiring. Thus, DYRK1A emerges as a cytosolic surveillance kinase to regulate and fine-tune mitochondrial protein biogenesis.

Objective: This study investigated the weight of different cognitive disorders on patient behaviour influencing the risk of falls after recent stroke. Design: Survey and retrospective monocentric study. Subjects/patients: 74 professionals/108 patients. Methods: Survey of professionals to ask for their thoughts concerning the weight of different cognitive disorders on the risk of falls and a retrospective study of patients post-stroke to determine whether these cognitive deficits could distinguish fallers from non-fallers. Univariate and multivariate logistic regression analyses were conducted. Results: In part 1, major cognitive disorders identified were anosognosia, confusion, inattention, precipitation, and unilateral spatial neglect. In part 2, 25 patients (23%) were fallers. After adjustment for length of rehabilitation stay and disease severity, on multivariate analysis, the cognitive disorders significantly associated with risk of falls were anosognosia (odds ratio 16), precipitation (13.3), inattention (8.3), and perseveration (4.9). Unilateral spatial neglect was not independently associated. Aphasia did not play a role. Conclusion: Some cognitive disorders, easily identified before any neuropsychological assessment, strongly modify patient behaviour in terms of risk of falls. It is proposed that these disorders should not be considered as an additional factor along with physical and general factors but rather as a multiplying factor applied to the others.

ABSTRACT OBJECTIVE: Viral hepatitis and sexually transmitted infections disproportionally affect men who have sex with men (MSM) and transgender women (TGW). However, only a few studies have evaluated the prevalence of hepatitis in these populations, especially in youths and adolescents. This study aimed to estimate the prevalence of biomarkers for hepatitis A, B, and C among young and adolescent MSM and TGW in three Brazilian municipalities. METHODS: Baseline data were collected from a combination of HIV prevention cohort of young and adolescent MSM (AMSM) and TGW (ATGW) aged 15-19 years in three Brazilian municipalities. A social behavioral questionnaire was applied, and immunoassays were performed to detect antibodies against hepatitis A (anti- HAV IgG and IgM), hepatitis B (anti-HBc and anti-HBs), and hepatitis C virus (anti-HCV); testing for the active hepatitis B marker, HBsAg, was also performed. The prevalence of reactive tests and 95% confidence interval (CI) for proportions were measured. RESULTS: The prevalence of naturally or artificially acquired immunity for hepatitis A totaled 17.7% (95%CI: 15.4-20.4), whereas that of acute infection, 0.4% (95%CI: 0.2-1.2). For hepatitis B and C, prevalence rates totaled 2.8% (95%CI: 1.8-4.4) and 0.2% (95%CI: 0.1-1.1), respectively. About 25.7% (95%CI: 22.4-29.4) of participants were non-reactive for anti-HBc and reactive for anti-HBs, the latter being a vaccine marker for hepatitis B. CONCLUSIONS: The investigation of viral hepatitis biomarkers among vulnerable populations enables the early identification of infections, the provision of timely treatment, and an opportunity to point out the need to expand vaccination coverage.

RESUMO OBJETIVO: Avaliar se adolescentes de minorias sexuais que iniciaram a profilaxia pré-exposição sexual (PrEP) em organizações comunitárias (OC) apresentam maior vulnerabilidade social e ao HIV em comparação com adolescentes em PrEP de um serviço de saúde convencional. Além disso, avaliar se esses adolescentes tiveram um acesso mais oportuno à profilaxia. MÉTODOS: Estudo demonstrativo da efetividade de PrEP, realizado na cidade de São Paulo, em duas OC, localizadas no centro (OC-centro) e na periferia (OC-periferia), e em um serviço convencional de testagem para o HIV (CTA-centro). Foram elegíveis para PrEP, entre 2020 e 2022, adolescentes homens cisgêneros que fazem sexo com homens (aHSH), travestis, mulheres transexuais e pessoas transfemininas (aTTrans), de 15 a 19 anos, HIV-negativos e com práticas de maior risco para o HIV. Indicadores de acesso oportuno e de vulnerabilidades dos adolescentes iniciando PrEP nas OC foram analisados, tendo por referência o CTA-centro e empregando regressão logística multinomial. RESULTADOS: 608 adolescentes iniciaram PrEP nas OC e CTA-Centro. Adolescentes das OC estiveram associados a um menor tempo de início de PrEP (1–7 dias; OC-periferia: ORa = 2,91; IC95% 1,22–6,92; OC-centro: ORa = 1,91; IC95% 1,10–3,31); e a um menor IDH de moradia (OC-centro: ORa = 0,97; IC95% 0,94–1,00; OC-periferia: ORa = 0,82; IC95% 0,78–0,86). Na OC-periferia houve aumento na chance de os adolescentes serem mais jovens (ORa = 3,06; IC95% 1,63–5,75) e morarem mais próximos ao serviço (ORa = 0,82; IC95% 0,78–0,86, média 7,8 km). Enquanto adolescentes da OC-centro estiveram associados ao maior conhecimento prévio de PrEP (ORa = 2,01; IC95% 1,10–3,91) e a alta percepção de risco (ORa = 2,02; IC95% 1,18–3,44). Não estiveram associadas aos adolescentes das OC as práticas sexuais de maior risco e as situações de vulnerabilidade ao HIV. CONCLUSÕES: A oferta de PrEP nas OC facilitou o acesso de adolescentes vulnerabilizados e pode contribuir para reduzir inequidades.

ABSTRACT OBJECTIVE: To identify the factors associated with HIV self-testing (HIVST) uptake among adolescent men who have sex with men (AMSM) and adolescent transgender women (ATGW) before and during the COVID-19 pandemic. METHODS: A cross-sectional HIVST uptake study was conducted among AMSM and ATGW. Peer educators and health professionals began providing HIVST in February 2019. The outcome was the HIVST uptake before and during the COVID-19 pandemic. The association between each predictor and outcome in each period was analyzed using simple and multiple logistic regressions, estimating odds ratios, and their respective 95% confidence intervals. RESULTS: The uptake was 229/510 (44.9%) and 382/1,075 (35.5%) before and during the pandemic. During the pre-pandemic period, HIVST uptake was higher in participants who reported receptive anal sex. During the pandemic, uptake was lower in participants with a steady sexual partner and higher in those with frequent oral sex with a steady partner in the previous three months. Before and during the pandemic, HIVST uptake was lower in ATGW and higher in those aged 18–19 years and in participants who lived alone. CONCLUSIONS: Uptake decreased during the pandemic. Sexual behavioral factors associated with HIVST uptake changed during the COVID-19 pandemic, showing the fluid dynamics of sexuality in AMSM and ATGW during this period. HIV programs can optimize the implementation of HIVST among adolescents and young people by incorporating effective and differentiated service delivery models to increase HIV testing uptake and to reach undiagnosed individuals effectively.

RESUMO OBJETIVO: O objetivo deste estudo é analisar o conhecimento sobre o esquema de Profilaxia Pré-Exposição ao HIV sob demanda (PrEP-SD) e a percepção do seu uso potencial entre jovens homens que fazem sexo com homens (HSH), travestis e mulheres trans (TrMT) em acompanhamento na coorte. MÉTODOS: O estudo qualitativo incluiu 50 entrevistas com participantes das cidades de Salvador e São Paulo, entre 15 e 19 anos, que estavam usando PrEP diária ou outros métodos preventivos. Eles foram abordados por meio de diferentes estratégias de criação de demanda. As entrevistas em profundidade abordaram temas como práticas sexuais, conhecimento sobre PrEP-SD, aceitabilidade e motivações para o uso. Foi realizada análise temática em duas fases usando o software Nvivo versão 12. RESULTADOS: A maioria dos participantes desconhecia a PrEP-SD e muitos questionaram a sua efetividade e segurança ao receberem informações sobre ela. No entanto, ao conhecer o esquema, muitos jovens perceberam vantagens, como a não obrigatoriedade diária de medicamentos e a possibilidade de uso apenas em momentos de maior risco. Barreiras para o uso da PrEP-SD também foram identificadas pelos participantes, como a imprevisibilidade das relações sexuais e a dificuldade em administrar as dosagens nessa modalidade. CONCLUSÕES: O conhecimento limitado e as experiências com a PrEP oral diária influenciaram o interesse por ela, o que aponta a necessidade de estratégias informativas que permitam o letramento dos jovens HSH e TrMT na PrEP-SD. Os jovens valorizaram a autonomia e a gestão da prevenção mais alinhada à dinâmica de sua vida sexual propiciada pela nova modalidade, mas enfrentam desafios na gestão da PrEP-SD.

RESUMO OBJETIVO: Analisar os fatores que aumentam a prática de sexo anal sem preservativo (SASP) entre adolescentes homens que fazem sexo com homens (aHSH) e travestis mulheres trans (aTrMT) em três capitais brasileiras. MÉTODOS: PrEP1519 é uma coorte prospectiva, multicêntrica, de demonstração da efetividade da profilaxia pré-exposição (PrEP) ao HIV entre aHSH e aTrMT, com idade entre 15 e 19 anos, em três capitais brasileiras. As análises foram realizadas com dados da linha de base da coorte, com a inclusão de 1.418 adolescentes inscritos de 2019 a 2021. O desfecho estudado foi SASP nos últimos seis meses, e os potenciais fatores associados foram sociodemográficos, comportamentais, de assistência à saúde e histórico de violência e discriminação. Foi realizada análise descritiva, bivariada e multivariada. Estimaram-se razões de prevalência ajustadas (RPa) e intervalos de confiança de 95% (IC95%). RESULTADOS: A maioria dos participantes era aHSH (91,5%), com idade de 18 a 19 anos (75,9%), pretos (40,5%), ensino médio e superior em andamento (92,7%), com relato de SASP na primeira relação sexual (54,2%), início da vida sexual antes dos 14 anos (43,4%) e história de sexo em grupo (24,6%) e de sexo transacional (14,6%). A prevalência de SASP nos últimos seis meses foi de 80,6% (IC95% 78,5%-82,6%). Adolescentes que relataram primeira relação sexual sem preservativo (RPa: 1,18; IC95% 1,10-1,25), consumo de substâncias psicoativas (RPa: 1,09; IC95% 1,03-1,16) e sexo transacional (RPa: 1,11; IC95% 1,04-1,20) tiveram maior prevalência de SASP nos últimos seis meses. Verificou-se também que aqueles com idade de 15 a 17 anos tiveram maior prevalência de SASP em comparação àqueles de 18 a 19 anos (RPa: 1,07; IC95% 0,99-1,13). CONCLUSÕES: A prevalência de SASP foi alta entre aHSH e aTrMT, bem como esteve associada a práticas que podem aumentar o risco de infecções sexualmente transmissíveis (IST). Assim, recomenda-se o fortalecimento de programas de saúde sexual para jovens que abordem o tema da sexualidade e prevenção de IST, tal como a ampliação do acesso a métodos preventivos, como preservativo e PrEP.

ABSTRACT OBJECTIVE: To identify recent HIV-1 infection and estimate HIV incidence among adolescent men who have sex with men (AMSM) and transgender women (ATGW) in Brazil. METHODS: From January to December 2020, a cross-sectional analysis was conducted with baseline data from the PrEP1519 study, an HIV pre-exposure prophylaxis (PrEP) demonstration cohort in Brazil among sexually active AMSM/ATGW aged 15-19. For enrollment, participants were screened with a fourth-generation HIV rapid test. The recent infection testing algorithm (RITA) included a recency assay in blood specimens, viral load, and CD4 cell count prior to antiretroviral treatment use. Among these participants, RITA-based HIV incidence was estimated using a mean duration of recency infection of 214 days and a false-recent rate of 0.02. RESULTS: Out of the 494 participants screened, 21 tested positive for HIV. Following RITA, five adolescents had a recent HIV infection, 14 had long-term infections, and two did not have blood specimens available. We classified these two participants as long-term infection cases due to CD4 cell counts and previous use of antiretroviral treatment. Among those who tested positive, all but one were AMSM (94.7%), 73.6% were aged 18-19, and 76.2% were non-White. The HIV prevalence was 4.2%, and the estimated HIV incidence was 1.7%. CONCLUSIONS: The estimated incidence highlights the need for targeted HIV prevention interventions, such as PrEP, for sexual minority adolescents. Integrating RITA into routine HIV testing services for this population provides valuable information on the current HIV epidemic. This strategy can aid in monitoring the effectiveness of prevention efforts and improving early entry to HIV care.

RESUMO OBJETIVO: Neste artigo, discutimos como a profilaxia pré-exposição (PrEP) e a carga viral indetectável=intransmissível (CVI=I) têm produzido reconfigurações nos contextos de encontros afetivo-sexuais de jovens gays/ homens que fazem sexo com homens (HSH) vivendo com HIV (JVHIV). MÉTODOS: Conduzimos entrevistas em profundidade com nove JVHIV, de 18 a 29 anos, oriundos de duas pesquisas realizadas em Salvador/Bahia, em 2019 e 2021. Focalizamos narrativas sobre acontecimentos inéditos na prevenção e no tratamento do HIV/aids, que têm permitido experiências de maior intimidade e segurança, mas também desafios e tensionamentos às relações afetivo-sexuais. RESULTADOS: Momentos distintos da experiência de viver com HIV revelam diferentes narrativas dos JVHIV com relação às novas biotecnologias de PrEP e CVI. Preocupações em torno de uma possível transmissão do HIV ou da obrigação de revelar a sorologia ficam mais destacadas entre jovens com o diagnóstico mais recente, enquanto aqueles com maior tempo de sorologia se colocam de forma mais confortável e confiante frente às novas tecnologias e seus efeitos significativos nos encontros sexuais, ainda que permaneçam controvérsias a respeito das consequências morais e comportamentais do uso delas. Alguns JVHIV reatualizam preocupações e trazem relatos sobre a continuidade do estigma em relação às pessoas vivendo com HIV. Outros JVHIV enfatizam os benefícios dos avanços biomédicos, com abertura para novas possibilidades interativas, inclusive sem uso da camisinha, sublinhando a existência de outras práticas, saberes, dinâmicas e formas de negociação do risco/cuidado, com tensionamentos no campo da própria sexualidade. CONCLUSÕES: Reiteramos a necessidade de retomada de políticas públicas no campo do HIV/aids para além das estratégias biomédicas, dando destaque às vulnerabilidades, à disseminação de informações sobre as novas tecnologias de prevenção e tratamento do HIV, ao respeito à autonomia das pessoas em suas escolhas preventivas e ao desenvolvimento de estratégias de enfrentamento aos estigmas associados ao HIV/aids.

Introduction and objectives: To evaluate compliance with per capita food supply at lunch in daycare centers. Methods: Visits were made on five consecutive days at lunchtime to six daycare centers enrolled in the “Creche com Sabor e Saúde” project (C2S), meaning “Daycare Centers with Taste and Health”. To quantify food portions for different age groups (6-8 months, 9-11 months, 12-23 months, and 24-35 months), each meal component (carbohydrate sources, protein sources, and vegetables) and dessert (fruit) underwent random selection and was weighed three times. The averages obtained were compared to C2S project guidelines for per capita food supply, in line with national and international recommendations. Results: One hundred and forty-five dishes and 244 desserts were analyzed. No dishes were served to infants under nine months. Per capita amounts for protein-supplying foods were fulfilled (9-11 months: 14.2 g; 12-23 months: 20.3 g; 24-35 months: 23.9 g). Carbohydrate-rich foods were provided in quantities exceeding the recommendations, particularly for the 12-23 month age group (62.2 g vs. 40 g). For infants under 12 months, no vegetables were offered, and above this age, the average amount provided fell below the minimum guideline of 20 g (12-23 months: 9.4 g; 24-35 months: 10.8 g). The quantity of fruit offered exceeded the recommendations for the 6-8 month (84.6 g > 30-50 g) and 9-11 month (133.3 g > 40-70 g) age groups, aligning with guidelines for the 12-35 months. Discussion: Daycare centers inconsistently followed per capita food supply guidelines for lunch, with deficits in vegetable intake and excesses in carbohydrate-rich foods and fruit that surpassed the recommended quantities.

Metastasizing cancer cells encounter a multitude of stresses throughout the metastatic cascade. Oxidative stress is known to be a major barrier for metastatic colonization, such that metastasizing cancer cells must rewire their metabolic pathways to increase their antioxidant capacity. NADPH is essential for regeneration of cellular antioxidants and several NADPH-regenerating pathways have been shown to play a role in metastasis. We have found that metastatic melanoma cells have increased levels of both NADPH and NADP+ suggesting increased de novo biosynthesis of NADP+. De novo biosynthesis of NADP+ occurs through a single enzymatic reaction catalyzed by NAD+ kinase (NADK). Here we show that different NADK isoforms are differentially expressed in metastatic melanoma cells, with Isoform 3 being specifically upregulated in metastasis. We find that Isoform 3 is more potent in expanding the NADP(H) pools, increasing oxidative stress resistance and promoting metastatic colonization compared to Isoform 1. We have found that Isoform 3 is transcriptionally upregulated by oxidative stress through the action of NRF2. Together, our work presents a previously uncharacterized role of NADK isoforms in oxidative stress resistance and metastasis and suggests that NADK Isoform 3 is a potential therapeutic target in metastatic disease.

Abstract The global economic downturn due to the COVID-19 pandemic, war in Ukraine, and worldwide inflation surge may have a profound impact on poverty-related infectious diseases, especially in low-and middle-income countries (LMICs). In this work, we developed mathematical models for HIV/AIDS and Tuberculosis (TB) in Brazil, one of the largest and most unequal LMICs, incorporating poverty rates and temporal dynamics to evaluate and forecast the impact of the increase in poverty due to the economic crisis, and estimate the mitigation effects of alternative poverty-reduction policies on the incidence and mortality from AIDS and TB up to 2030. Three main intervention scenarios were simulated—an economic crisis followed by the implementation of social protection policies with none, moderate, or strong coverage—evaluating the incidence and mortality from AIDS and TB. Without social protection policies to mitigate the impact of the economic crisis, the burden of HIV/AIDS and TB would be significantly larger over the next decade, being responsible in 2030 for an incidence 13% (95% CI 4–31%) and mortality 21% (95% CI 12–34%) higher for HIV/AIDS, and an incidence 16% (95% CI 10–25%) and mortality 22% (95% CI 15–31%) higher for TB, if compared with a scenario of moderate social protection. These differences would be significantly larger if compared with a scenario of strong social protection, resulting in more than 230,000 cases and 34,000 deaths from AIDS and TB averted over the next decade in Brazil. Using a comprehensive approach, that integrated economic forecasting with mathematical and epidemiological models, we were able to show the importance of implementing robust social protection policies to avert a significant increase in incidence and mortality from AIDS and TB during the current global economic downturn.

In the diagnosis process, radiologists play an important role by examining numerous radiology exams to identify different types of nodules. To aid the clinicians’ analytical efforts, computer-aided diagnosis can streamline the process of identifying pulmonary nodules. For this purpose, medical reports can serve as valuable sources for automatically retrieving image annotations. Our study focused on converting medical reports into nodule annotations, matching textual information with manually annotated data from the Lung Nodule Database (LNDb)—a comprehensive repository of lung scans and nodule annotations. As a result of this study, we have released a tabular data file containing information from 292 medical reports in the LNDb, along with files detailing nodule characteristics and corresponding matches to the manually annotated data. The objective is to enable further research studies in lung cancer by bridging the gap between existing reports and additional manual annotations that may be collected, thereby fostering discussions about the advantages and disadvantages between these two data types.

Abstract Background Lung cancer is the leading cause of cancer-related death in the world. In contrast to many other cancers, a direct connection to modifiable lifestyle risk in the form of tobacco smoke has long been established. More than 50% of all smoking-related lung cancers occur in former smokers, 40% of which occur more than 15 years after smoking cessation. Despite extensive research, the molecular processes for persistent lung cancer risk remain unclear. We thus set out to examine whether risk stratification in the clinic and in the general population can be improved upon by the addition of genetic data and to explore the mechanisms of the persisting risk in former smokers. Methods We analysed transcriptomic data from accessible airway tissues of 487 subjects, including healthy volunteers and clinic patients of different smoking statuses. We developed a computational model to assess smoking-associated gene expression changes and their reversibility after smoking is stopped, comparing healthy subjects to clinic patients with and without lung cancer. Results We find persistent smoking-associated immune alterations to be a hallmark of the clinic patients. Integrating previous GWAS data using a transcriptional network approach, we demonstrate that the same immune- and interferon-related pathways are strongly enriched for genes linked to known genetic risk factors, demonstrating a causal relationship between immune alteration and lung cancer risk. Finally, we used accessible airway transcriptomic data to derive a non-invasive lung cancer risk classifier. Conclusions Our results provide initial evidence for germline-mediated personalized smoke injury response and risk in the general population, with potential implications for managing long-term lung cancer incidence and mortality.

Abstract Introduction There are enough cases of colorectal cancer with liver metastasis, but inferior vena cava infiltraion with dissemination to the right atrium is an infrequent event. Presentation of case This is the first case of surgical treatment of recurrent liver metastasis with the infiltration to the inferior vena cava and to the right atrium of the heart, using a cryopreserved pulmonary homograft. Discussion The choice of a cryopreserved pulmonary homograft was preferred by the need for a radical and wide resection of tissues involved in the metastasis, as well as to potentially reduce the risk of thrombosis in the short- and long-term postoperative period. Conclusion The use of a cryopreserved homograft in operation undergoing cardiopulmonary bypass allowed us to perform the required volume of radical resection and to replace an extended section of the inferior vena cava.

46. Investigating Statistical Features of the FX Bid Ask Series in a Small Economy with a Sizeable Informal Economy

The assessment of the effect of the informal use of a foreign currency on the corresponding FX rates toward the national currency is a very difficult task, requiring direct calculation and modeling. It comes mostly because of the unknown quantity of foreign money used in the informal sector, but also because of the lack of quantitative calculation models. To overcome this gap and to realize a qualitative description of this effect in a concrete economic environment, we propose herein a comparative analysis between the behavior of two typical FX rate series recorded in the Albanian currency market, the Euro-ALL and USD-ALL, providing that the Euro is used commonly as a national currency substitute in the informal economy and the USD is not. So, we have evidenced that the un-stationarity degree of the Bid and Ask spread distribution for the Euro-ALL FX series is higher than for the corresponding USD-ALL case, but with a lower variance. Those features occurring simultaneously can be explained by assuming that informal use of the Euro acts as an additional perturbation on the FX system, imposing high nonstationary, but at the same time it provides reservoir or source features for the money disbalances, reducing the average fluctuations. Next, the depth of the market measured by the average Bid-Ask Spread has resulted in a smaller price for the Euro currency, indicating a lower cost of the transactions and reinforcing the assumption regarding the distribution’s in-stationarity features. Based on those indicatory findings, we propose to realize indirect evidence for our assumptions by comparing the reproduction of the corresponding distribution using autoregressive models. In this case, we have evidenced that the distribution of the FX Euro-ALL spread can be reproduced better if we include in standard ARCHX (m, n, p) models a term that mimics the informality measure. When applying the same procedure for the USD-ALL spread, the resulting distribution has not matched equally well with the original ones. Those findings have been discussed in the framework of an alternative description of the effect of the informal use of foreign currency in a small economy with a sizeable informal sector, which convene our current system under analysis, but we believe that they can be applicable for similar economic environments. 47. Prevalence of HIV infection among transgender women and travestis in Brazil: data from the TransOdara study

ABSTRACT Objective The aim of this study was to investigate the prevalence of human immunodeficiency virus (HIV) infection among transgender women and travestis and to analyze factors associated with HIV infection in Brazil. Methods TransOdara was a cross-sectional study on sexually transmitted infections among transgender women and travestis in five Brazilian cities between 2019 and 2021. Self-identified transgender women and travestis aged ≥18 years were recruited using respondent-driven sampling, completed an interviewer-led questionnaire, and provided samples to detect HIV. The outcome was the result of the rapid antigen testing for HIV. Adjusted prevalence ratios (aPR) and 95% confidence intervals (95% CI) were obtained using Poisson regression with robust variance. Results Overall, this population was found to be especially vulnerable, with high levels of unstable housing and engagement in informal work. They usually resort to transactional sex as their main working activity. Half of them earned less than the Brazilian minimum wage, which characterizes a poor population living in dire conditions. The overall HIV prevalence was 34.40%. In the final model, the variables associated with the HIV prevalence were as follows: to be 31 years old or older, not studying at the moment they were interviewed, to be unemployed, and engaged in lifetime transactional sex. Conclusion We found disproportionately high HIV prevalence among transgender women and travestis, compared with a low prevalence among respective segments of Brazil’s general population, which highlights the context of vulnerability in this population. The data point to the urgency for intensification and expansion of access to HIV prevention and strategies to stop discrimination in health care (among other services and contexts) and provide comprehensive services for this population.

ABSTRACT Objective: To investigate the prior testing for HIV, syphilis, hepatitis B (HBV), and hepatitis C (HCV) among transgender women and travestis (TGW) in five Brazilian cities and identify factors associated with each of these previous tests. Methods: This is a cross-sectional study with the recruitment of TGW through respondent-driven sampling (TransOdara Study). The investigated outcome variable was prior testing for HIV, syphilis, HBV, and HCV in the last 12 months. The association between sociodemographic and behavioral factors with the outcome was analyzed using a binomial logistic regression with mixed effects. Adjusted odds ratios (aOR) and 95% confidence intervals (CI95%) were estimated. Results: The proportions of individuals with prior testing in the past year were as follows: 56.3% for HIV, 58.0% for syphilis, 42.1% for HBV, and 44.7% for HCV. Negative associations with prior testing were observed for individuals aged 35 years or older, whereas positive associations were found for those with high school education, those who experienced verbal or psychological violence in the last 12 months, and those who had commercial or casual partners in the last 6 months. Conclusion: There was low frequency of testing in the 12 months preceding the study for HIV, syphilis, HBV, and HCV compared to the guidelines established by the Ministry of Health. Expanding access to and engagement with healthcare and prevention services for TGW is an essential strategy in reducing the transmission chain of HIV and other sexually transmitted infections (STIs).

ABSTRACT Objective To identify groups of transgender women and travestis (TGW) with specific patterns of gender-based discrimination (GBD) and analyze the factors associated with GBD. Methods A cross-sectional study was conducted with TGW recruited through respondent-driven sampling in five Brazilian cities (2019-2021). Latent class analysis was used to characterize GBD (low, medium, and high) using 14 observable variables. Descriptive analysis was performed, and associations between predictor variables and GBD were estimated by adjusted odds ratios (aOR) using ordinal logistic regression. Results Out of a total of 1,317 TGW, 906 (68.8%) answered questions about GBD. Most were under 34 years old, single, and had a Brown race/skin color. GBD was classified as “low,” “medium,” and “high,” with estimates of 41.7, 44.5, and 13.8%, respectively. Variables positively associated with higher intensity of GBD included living in Manaus compared to São Paulo, being ≤34 years old compared to >34, being homeless compared to living in one’s own house or rented apartment, not having legally changed one’s name compared to those who had, and reporting physical or sexual violence compared to those who did not report. Variables negatively associated with higher intensity of GBD included having a Brown or Asian race/skin color compared to White and a monthly income ≥1 minimum wage compared to ³1. Conclusion A high proportion of GBD was observed in Brazilian TGW, with this outcome associated with more vulnerable sociodemographic characteristics and a history of violence.
