Abstract

Arbuscular mycorrhizal fungi (AMF) are found to be causing the most abundant symbioses between fungi and roots of terrestrial plants. AMF act as a biofertiliser that stimulate plant growth and increase plant productivity under poor soil fertility. In addition, unbalanced application of zinc (Zn) and the antagonistic relationship with phosphorus (P) also play an imperative role in decreasing crop productivity. It is necessary to synchronise Zn application rate with applied inorganic P and AMF to achieve optimum crop yield. For tha purpose, a pot trial was conducted on Zn-deficient soil with five application rates of Zn, i.e. 0, 30, 60, 90, 120 and 150 mg kg−1. Two levels of inorganic P [0 (P0) and 21 (P1) mg kg−1] were applied with and without AMF inoculation. Results showed that more AMF colonisation was observed under deficient Zn and P conditions. Higher soil Zn (Zn120 and Zn150) significantly decreased the germination rate and plant growth. However, a significant improvement in germination, plant height, biomass, transpiration rate and 100-grain weight validated the productive functioning of AMF over no AMF. AMF inoculation alleviated P-induced Zn deficiency and Zn-induced P deficiency. Application of P0Zn60 and P0Zn30 with and without AMF is a better treatment to maximise wheat growth, yield and gas-exchange attributes in Zn-deficient conditions. It is also recommended to apply low Zn, (30 or 60 mg kg−1 Zn) when AMF is used, with 21 mg kg−1 P, or half of the recommended dose of P. Imbalanced application of P and Zn usually disturbs the uptake of both nutrients in plants. Both show antagonistic relation with each other when applied in soil as inorganic fertilisers. However, inoculation of arbuscular mycorrhizae fungi (AMF) can improve the uptake of P. For development of a balance between P and Zn in soil, current study focussed on the combined use of an inorganic P and Zn fertiliser with AMF. 38. Serial Magnetic Resonance Imaging in Creutzfeldt-Jakob Disease: a Case Report and Literature Review

Abstract

Creutzfeldt-Jakob disease (CJD) is a rare, degenerative, invariably fatal brain disorder. CJD usually appears in later life and runs a rapid course. Typically, the onset of symptoms occurs about age 60 and about 90% of individuals die within one year. We report a case of 67-year-old male presented with progressive aphasia, confusion, dysphagia and inability to carry out activities of daily life (ADLs) over a period of three to four weeks. The patient had past medical history of chronic atrial fibrillation and hypertension. Prior to admission, the patient was treated for ischemic stroke of left basal ganglia but continued to have worsening encephalopathy. The spinal tap revealed a 14-3-3 protein level of thirteen times the upper limit of normal; electroencephalogram (EEG) showed a diffuse slowing of the background and periodic sharp waves with greater involvement of the left hemisphere. Diffusion-weighted imaging (DWI) magnetic resonance imaging (MRI) at the time of admission showed extensive signal abnormality in the basal ganglia bilaterally and in the cerebral cortex bilaterally, particularly over the left cerebral hemisphere. The persistence of the MRI findings over several weeks was concerning for spongiform encephalopathy. The probable diagnosis of Creutzfeldt-Jakob disease was made based on these imaging findings taken together with the patient’s clinical signs and symptoms of a rapidly progressive encephalopathy. The patient was able to have some quality time with his family as the diagnosis was made earlier than perhaps otherwise and expired peacefully after comfort care measures were chosen. Serial MRI may serve as a clue to the early diagnosis of CJD and potentially provide a better quality of life for the patients.

39. Abstract 13632: Increase in Long-Term Major Adverse Cardiovascular Events in Patients ?Appropriately? Tested for Troponin Assays in the Emergency Department

Abstract

Introduction:Lack of criteria for ordering troponin in the ED has led to inappropriate ordering and overdiagnosis. We developed an appropriateness criteria and examined long-term cardiovascular outcomes of patients tested with troponin in the ED.Methods:A single center retrospective cohort study with a prospective follow up was conducted (n=407). Criteria for appropriateness included chest pain, breathlessness, anginal "equivalent" (breathlessness, nausea, emesis, back pain, palpitations, unexplained hypotension or syncope with new ECG changes), new onset atrial fibrillation/flutter (AF) or prior MI and PCI/CABG. Patients were then followed over 540 days. Primary outcome was long-term major adverse cardiac events (MACE) [cardiovascular mortality, new incidence of MI, PCI/CABG, CHF, stroke, AF and/or cardiac related re-admissions]. Results were adjusted for patient age, sex, smoking and comorbidities in a Cox regression model.Results:Majority (n=252, 62%) of troponin testing was appropriate, with the remainder deemed inappropriate. Of the appropriately ordered troponins, 34% were positive compared to 28% of the inappropriately ordered troponins (range 0.04-0.10 ng/dl). The appropriateness criteria had a sensitivity of 77% in predicting MACE. There were no significant differences in age, sex, comorbidities, echocardiogram and ECG parameters between the two groups. MACE occurred in 21% and 10% of the appropriate and inappropriate groups respectively. Kaplan-Meier shows that unlike the inappropriate group (3.5%), 96% of the events in the appropriate group occur within the first 200 days. Patients in the appropriate group were at increased adjusted risk of MACE (HR 2.55, 95% CI (1.59- 4.08), p<0.001) on long-term follow up.Conclusion:Patients who do not meet appropriateness criteria for troponin testing have minimal events in the first 200 days of follow up. Patients who are tested appropriately are at higher risk of MACE upon 540 days follow up.
