- Abstract
Background: Early mobilisation (EM) is an intervention that may improve the outcome of critically ill patients. There is limited data on EM in COVID-19 patients and its use during the first pandemic wave. Methods: This is a pre-planned subanalysis of the ESICM UNITE-COVID, an international multicenter observational study involving critically ill COVID-19 patients in the ICU between February 15th and May 15th, 2020. We analysed variables associated with the initiation of EM (within 72 h of ICU admission) and explored the impact of EM on mortality, ICU and hospital length of stay, as well as discharge location. Statistical analyses were done using (generalised) linear mixed-effect models and ANOVAs. Results: Mobilisation data from 4190 patients from 280 ICUs in 45 countries were analysed. 1114 (26.6%) of these patients received mobilisation within 72 h after ICU admission; 3076 (73.4%) did not. In our analysis of factors associated with EM, mechanical ventilation at admission (OR 0.29; 95% CI 0.25, 0.35; p = 0.001), higher age (OR 0.99; 95% CI 0.98, 1.00; p ≤ 0.001), pre-existing asthma (OR 0.84; 95% CI 0.73, 0.98; p = 0.028), and pre-existing kidney disease (OR 0.84; 95% CI 0.71, 0.99; p = 0.036) were negatively associated with the initiation of EM. EM was associated with a higher chance of being discharged home (OR 1.31; 95% CI 1.08, 1.58; p = 0.007) but was not associated with length of stay in ICU (adj. difference 0.91 days; 95% CI − 0.47, 1.37, p = 0.34) and hospital (adj. difference 1.4 days; 95% CI − 0.62, 2.35, p = 0.24) or mortality (OR 0.88; 95% CI 0.7, 1.09, p = 0.24) when adjusted for covariates. Conclusions: Our findings demonstrate that a quarter of COVID-19 patients received EM. There was no association found between EM in COVID-19 patients' ICU and hospital length of stay or mortality. However, EM in COVID-19 patients was associated with increased odds of being discharged home rather than to a care facility. Trial registration ClinicalTrials.gov: NCT04

Objective: To find out if non-invasive ventilation (NIV) as a ventilation modality increases the burden on nursing personnel and give suggestions how the presence of non-medical caregivers in an ICU with an extreme staff shortage can reduce nursing workload during mechanical ventilation. Background: Although the European Union offers good quality healthcare, there are middle-income countries in the alliance that suffer from nursing staff shortage. For example, Bulgaria needs two times more nurses than it has now to meet the needs of its healthcare system. This calls for strategies that reduce nursing workload as much as possible. There is a common perception that NIV is more time-consuming for nurses compared to invasive mechanical ventilation (IMV) but only a few studies discuss the matter and none of them are settled in the unique environment where medical and non-medical caregivers provide direct patient care as a team. Study design and methods: This is an observational study conducted in a specialised respiratory ICU with trained nurses, physiotherapists, and non-medical caregivers in a university hospital in Bulgaria. Ninety adult patients (43 on NIV and 47 on IMV) with acute respiratory failure that were on mechanical ventilation for at least five days were included. Nursing workload was measured via the Nursing Activities Score (NAS). Average and daily NAS were compared between groups. Then individual components of the score were analysed to determine which activities have the greatest impact on nursing workload. Results: Average (39.72 ± 6.35 vs 46.08 ± 5,66, p< 0.001) and daily NAS for the first five days of mechanical ventilation and was significantly higher in the IMV group. There was a substantial drop of nurse workload with time in both groups, more significant for the patients who ventilated invasively. It occurred on the second day in both groups (NIV: p=0.005, 95%CI: 0.88-4.52; IMV: (p<0,001, 95%CI: 2.72-7.03). NIV patients required more time for monitoring but less for hygiene, fluid administration and nutrition. IMV patients also required specific care for their artificial airway and took more time for lung function improvement procedures. Conclusion: In a setting where direct patient care is provided by both formally trained nurses and nonmedical caregivers IMV was associated with a higher nursing workload than NIV. Implications for research, policy, and practice: In a situation with an extreme staff shortage some of the non-medical nursing activities involving patients on NIV can be potentially transferred to trained nonmedical caregivers. What is already known about the topic? * According to the World Health Organization in 2018 there was a nurse shortage of around six million and by 2030 it will increase to 36 million. This negative trend is most prominent in low- and middle-income countries. * There is a common perception that NIV is more time-consuming for nurses compared to IMV but only a few research groups address this matter. * NIV may require additional time from nursing personnel mainly because of problems with mask fit, skin breakdown and maintenance of an optimal position in bed. NIV is more time-consuming in the first 48 h but then becomes much less demanding. Цианобактериите (синьо-зелените водорасли или Cyanophyta) са повсеместно разпространени микроорганизми, срещани във водни басейни, но също и на сушата. Необходими условия за тяхното експоненциално намножаване, познато като „цъфтеж на водата“, са топлина (около 23-24 °C ) и пряка слънчева светлина. Цианотоксините, продукт от техния метаболизъм, са токсични за животните и човека. Анатоксин-а, един от цианотоксините, е невро-мускулен блокер, предизвикващ спазми и мускулни крампи. При хронична експозиция той е рисков фактор за развитието на невродегенеративни заболявания като болест на Алцхаймер (БА) и Паркинсон (БП). The subject of the dissertation is the toxins from microalgae-cyanotoxins. Cyanobacteria (blue-green algae or Cyanophyta) are microorganisms found in water basins, but also on land. Necessary conditions for their exponential multiplication, known as "water blooms", are heat (around 23-24 °C) and direct sunlight. Cyanotoxins, a product of their metabolism, are toxic to animals and humans. Anatoxin, one of the cyanotoxins, is a neuromuscular blocker that causes spasms and muscle cramps. With chronic exposure, it is a risk factor for the development of neurodegenerative diseases such as Alzheimer's disease (AD) and Parkinson's disease (BP). In February 2018, the research network CYANOCOST (Cyanobacterial blooms and toxins in water resources: Occurrence, impacts and management) published an open letter to the European Commission amending Council Directive 98/83 / EC on the quality of water intended for human consumption. . As a result of blooming, some cyanobacteria (Microcystis, Anabaena, Planktothrix, etc.) produce toxins at concentrations that are high enough to poison and even kill animals and humans. According to Annex II of the Framework Directive 2000/60/EC, transformed into the Bulgarian legislation, for the characterization of surface water, at this stage, there are no acceptable limit values for cyanotoxins. Cyanotoxins include some of the strongest natural poisons, including those that could cause rapid death due to respiratory failure. The aim of this publication is an overview of toxic cyanobacteria and toxicity mechanisms.

In a previous study, the authors had predicted that the local green frog population had developed adaptations to live and breed in mixoligohaline waters, however the registered in 2010 mass migration of the adults into the sea near the Shablenska Tuzla lagoon was an isolated event, conditioned by the correlated impact of defined climatic factors. To support that hypothesis, the site was an object of five-year monitoring. Additionally, other three regions with similar topographic characteristics - the Black sea shores near Durankulak lake, Ezeretz lake and the Bolata bay - were investigated. During the period of the field surveys and according to data from fishers and biologists working in the region, no further mass migration of frogs in the sea waters was detected. An isolated observation of a specimen, swimming in the sea at Bolata bay (water with conductivity of 19.55 mS/cm), leads to the conclusion that the adult green frogs from the local population are able to inhabit the sea waters, but other factors like the sea surf prevent them to colonise the sea shores. Ce travail porte sur le thème de la transmission linguistique et plus spécifiquement sur le rôle de l’aîné allophone dans la transmission de la langue de scolarisation, dans le rapport entre famille et école. Nous nous demandons : comment l’aîné allophone entre dans la langue française ? Quels moyens didactiques utilisent les enseignantes de l’accueil (CIF, intégration, spécialisée) pour aider les élèves allophones dans l’apprentissage du français ? Comment l’aîné transmet la langue à sa famille ? Est-ce que la place comme aîné est importante ? Est-ce que la place de l’aîné allophone est prise en compte par les enseignants au sein de l’établissement ? A quelle condition devrait-elle être prise en compte par les enseignants ? Nous avons démarré notre travail en nous basant sur la recherche de Christiane Perregaux, suite à laquelle nous avons amené des éléments nouveaux en cherchant à avoir le regard des enseignantes de l’accueil. Voici brièvement les points clés de notre méthodologie : nous avons effectué des entretiens avec quatre enseignantes de l’accueil en utilisant une grille. Ces entretiens nous ont permises de créer des vignettes afin de faire des entretiens avec cinq élèves aînés allophones choisis parmi nos classes de stage. Puis, finalement un dernier entretien avec une ancienne élève allophone aînée. Les résultats de notre recherche mettent en avant le manque de prise en compte de la place de l’aîné au sein de l’école ainsi que ses différentes fonctions.

- Abstract
In December 2019, a new Coronavirus (SARS-CoV-2) emerged in China, causing the pandemic disease COVID-19. The clinical presentation is variable, but the predominant symptoms are those of the upper respiratory tract., Aim: The aim of the current study is to describe the incidence and type of the gastrointestinal injury (GI) in COVID-19, as well as their prognostic value., Materials and Methods: We conducted a coincidental search on this topic in PubMed, Web of Science and EMBASE. We also followed a group of 31 Bulgarian COVID-19 patients throughout the course of their disease and analyzed their symptoms (catarrhal and other) and outcome., Results: The publications concerning our survey followed a total of 1509 COVID-19 patients. In the Bulgarian cohort, only 14 from the 31 patients were laboratory-confirmed COVID-19 cases. Approximately 1/3 of the infected individuals presented with GI. In some patients this was the first, or only, symptom of the disease. It was also indicative of a more severe disease course., Conclusion: GI may be an important symptom and prognostic factor in COVID-19. Therefore, patients with acute gastrointestinal symptoms must be actively tested for SARS-CoV-2.
