4. Levels of evidence and grades of recommendation supporting European society for medical oncology clinical practice guidelines.

Background: The European Society for Medical Oncology (ESMO) guidelines are among the most comprehensive and widely used clinical practice guidelines (CPGs) globally. However, the level of scientific evidence supporting ESMO CPG recommendations has not been systematically investigated. This study assessed ESMO CPG levels of evidence (LOE) and grades of recommendations (GOR), as well as their trends over time across various cancer settings. Methods: We manually extracted every recommendation with the Infectious Diseases Society of America (IDSA) classification from each CPG. We examined the distribution of LOE and GOR in all available ESMO CPG guidelines across different topics and cancer types. Results: Among the 1,823 recommendations in the current CPG, 30% were classified as LOE I, and 43% were classified as GOR A. Overall, there was a slight decrease in LOE I (-2%) and an increase in the proportion of GOR A (+1%) in the current CPG compared to previous versions. The proportion of GOR A recommendations based on higher levels of evidence such as randomized trials (LOE I-II) shows a decrease (71% vs. 63%, p = 0.009) while recommendations based on lower levels of evidence (LOE III-V) show an increase (29% vs. 37%, p = 0.01) between previous and current version. In the current versions, the highest proportion of LOE I (42%) was found in recommendations related to pharmacotherapy, while the highest proportion of GOR A recommendations was found in the areas of pathology (50%) and diagnostic (50%) recommendations. Significant variability in LOE I and GOR A recommendations and their changes over time was observed across different cancer types. Conclusion: One-third of the current ESMO CPG recommendations are supported by the highest level of evidence. More well-designed randomized clinical trials are needed to increase the proportion of LOE I and GOR A recommendations, ultimately leading to improved outcomes for cancer patients. Global change drives multiple facets of biodiversity including interaction diversity, which is fundamental for ecosystem functioning. However, studying trophic interactions is challenging in meta‐ecosystems, that is ecosystems connected by spatial flows of energy, materials and organisms across ecosystem boundaries. While analytical methods based on abundances of polyunsaturated fatty acids (PUFAs) and stable isotopes of amino acids (AAs) are being increasingly used, it has never been explored if both approaches could be: (i) combined in mixing models to enhance precision in dietary inference (ii) compared to disentangle transfers of various PUFAs and proteins in food webs in the wild.We explore the utility of analytical approaches based on PUFA abundances and AA isotopes to resolve resource transfers in a natural riparian food web. We focus on spiders and their potential prey from blue, green and brown sources to address three important and persisting methodological issues in food‐web ecology, namely whether (i) essential AA carbon isotopes can resolve protein origin from blue, green and brown resources, (ii) PUFA relative abundance and AA isotopes can be combined in a mixing model to provide higher precision estimates (i.e. narrower intervals) and (iii) combining the two approaches can unveil the coupling of protein and PUFA transfers in food webs.Our research demonstrates the power of AA isotopes and PUFAs to distinguish blue, green, and brown sources and their transfer up to consumers. We show that combining PUFA relative abundance and AA isotopes in a mixing model provides overall estimates similar to the individual estimates but significantly increases precision. In addition, we showcase how combining approaches unveil the coupling of protein and PUFA transfers. For instance, we show that most PUFAs are less concentrated from prey to predators, relative to proteins, highlighting uncoupling of PUFAs and protein transfer along food chains.We show for the first time the effectiveness of combining AA isotopes and PUFA abundances, particularly relevant for complex trophic interactions in a meta‐ecosystem context. Background: Influenza A(H3N2) viruses dominated early in the 2022–2023 influenza season in Europe, followed by higher circulation of influenza A(H1N1)pdm09 and B viruses. The VEBIS primary care network estimated the influenza vaccine effectiveness (VE) using a multicentre test‐negative study. Materials and Methods: Primary care practitioners collected information and specimens from patients consulting with acute respiratory infection. We measured VE against any influenza, influenza (sub)type and clade, by age group, by influenza vaccine target group and by time since vaccination, using logistic regression. Results: We included 38 058 patients, of which 3786 were influenza A(H3N2), 1548 influenza A(H1N1)pdm09 and 3275 influenza B cases. Against influenza A(H3N2), VE was 36% (95% CI: 25–45) among all ages and ranged between 30% and 52% by age group and target group. VE against influenza A(H3N2) clade 2b was 38% (95% CI: 25–49). Overall, VE against influenza A(H1N1)pdm09 was 46% (95% CI: 35–56) and ranged between 29% and 59% by age group and target group. VE against influenza A(H1N1)pdm09 clade 5a.2a was 56% (95% CI: 46–65) and 79% (95% CI: 64–88) against clade 5a.2a.1. VE against influenza B was 76% (95% CI: 70–81); overall, 84%, 72% and 71% were among 0–14‐year‐olds, 15–64‐year‐olds and those in the influenza vaccination target group, respectively. VE against influenza B with a position 197 mutation of the hemagglutinin (HA) gene was 79% (95% CI: 73–85) and 90% (95% CI: 85–94) without this mutation Conclusion: The 2022–2023 end‐of‐season results from the VEBIS network at primary care level showed high VE among children and against influenza B, with lower VE against influenza A(H1N1)pdm09 and A(H3N2). Ultrapure water with minimum contents of impurities is used for the preparation of steam in thermal power plants. More recently it has been found that the corrosion process is also influenced by sodium ions, chloride ions, and all forms of silicon in water. At higher temperatures and under high pressure the less soluble compounds of silicon are extracted, which form deposits on the walls of the boiler, the piping system and the turbine blades. Silicon is found in water in the form of different types (species) which are characterized by specific physical and chemical properties. Distinctions can be made between highly reactive species of ionic (silicate anions) and molecular forms (silicic acid) and relatively inert types (suspended, colloidal, and polymerized silicon). The determination of various forms of silicon in water is a complex analytical task. This paper covers relevant research in the field of silicon specification analysis. Maintaining the unchanged, original composition of silicon species during various stages of analysis (sample collection, storage, and conservation) has been given special attention. A large number of methods and procedures have been developed for the analysis of species of silicon, including chromatographic, spectroscopic and electrochemical techniques and combinations thereof. The techniques used for determining both the total amount and individual forms of silicon have been singled out. There is also an overview of the coupled techniques used most frequently in practice by using the methodology which involves preliminary separation of species and then individual specification. The paper offers an overview of analytical properties, advantages and disadvantages of the most representative analytical methods developed specifically for the analysis of silicon species in ultrapure water. The most important studies focusing on the silicon species in water have been highlighted and presented in detail. The determination of silicon content in water is of great importance because of various effects of silicon species (corrodibility and toxicity) and the selection of methods for the efficient removal of silicon from water.

Introduction In July and August 2021, the SARS-CoV-2 Delta variant dominated in Europe. Aim Using a multicentre test-negative study, we measured COVID-19 vaccine effectiveness (VE) against symptomatic infection. Methods Individuals with COVID-19 or acute respiratory symptoms at primary care/community level in 10 European countries were tested for SARS-CoV-2. We measured complete primary course overall VE by vaccine brand and by time since vaccination. Results Overall VE was 74% (95% CI: 69–79), 76% (95% CI: 71–80), 63% (95% CI: 48–75) and 63% (95% CI: 16–83) among those aged 30–44, 45–59, 60–74 and ≥ 75 years, respectively. VE among those aged 30–59 years was 78% (95% CI: 75–81), 66% (95% CI: 58–73), 91% (95% CI: 87–94) and 52% (95% CI: 40–61), for Comirnaty, Vaxzevria, Spikevax and COVID-19 Vaccine Janssen, respectively. VE among people 60 years and older was 67% (95% CI: 52–77), 65% (95% CI: 48–76) and 83% (95% CI: 64–92) for Comirnaty, Vaxzevria and Spikevax, respectively. Comirnaty VE among those aged 30–59 years was 87% (95% CI: 83–89) at 14–29 days and 65% (95% CI: 56–71%) at ≥ 90 days between vaccination and onset of symptoms. Conclusions VE against symptomatic infection with the SARS-CoV-2 Delta variant varied among brands, ranging from 52% to 91%. While some waning of the vaccine effect may be present (sample size limited this analysis to only Comirnaty), protection was 65% at 90 days or more between vaccination and onset.

Background: In 2021-2022, influenza A viruses dominated in Europe. The I-MOVE primary care network conducted a multicentre test-negative study to measure influenza vaccine effectiveness (VE). Methods: Primary care practitioners collected information on patients presenting with acute respiratory infection. Cases were influenza A(H3N2) or A(H1N1)pdm09 RT-PCR positive, and controls were influenza virus negative. We calculated VE using logistic regression, adjusting for study site, age, sex, onset date, and presence of chronic conditions. Results: Between week 40 2021 and week 20 2022, we included over 11 000 patients of whom 253 and 1595 were positive for influenza A(H1N1)pdm09 and A(H3N2), respectively. Overall VE against influenza A(H1N1)pdm09 was 75% (95% CI: 43-89) and 81% (95% CI: 45-93) among those aged 15-64 years. Overall VE against influenza A(H3N2) was 29% (95% CI: 12-42) and 25% (95% CI: -41 to 61), 33% (95% CI: 14-49), and 26% (95% CI: -22 to 55) among those aged 0-14, 15-64, and over 65 years, respectively. The A(H3N2) VE among the influenza vaccination target group was 20% (95% CI: -6 to 39). All 53 sequenced A(H1N1)pdm09 viruses belonged to clade 6B.1A.5a.1. Among 410 sequenced influenza A(H3N2) viruses, all but eight belonged to clade 3C.2a1b.2a.2. Discussion: Despite antigenic mismatch between vaccine and circulating strains for influenza A(H3N2) and A(H1N1)pdm09, 2021-2022 VE estimates against circulating influenza A(H1N1)pdm09 were the highest within the I-MOVE network since the 2009 influenza pandemic. VE against A(H3N2) was lower than A(H1N1)pdm09, but at least one in five individuals vaccinated against influenza were protected against presentation to primary care with laboratory-confirmed influenza. Having in mind that exposure to low levels of microcystin in drinking water represents the health risk for people, microcystins can be observed as potent xenobiotics. The consequences of exposure to natural products of the blue-green algae, the microcystins, are numerous. Among other things, there is a possibility of development of malignant disease of colon and liver. The occurrence of the tumor is, on one side, enabled by the active transportation of microcystin for achievement of high concentration intracellularly and, on the other side, by high affinity of microcystins for serine/threonine phosphatase PP1 and PP2A after which follows their inactivation. The system, which is responsible for the active transportation of microcystin, is a family of polypeptides for transportation of organic anions (OATP). Isoforms of these carriers are distributed in one tissue, like OATP1B1 and OATP1B3, which are specific for liver or many tissues, OATP1A2 carrier, present in the liver, kidney, brain and the small intestine. The consequences of high concentration of microcystin intracellularly, binding and the inhibitions of protein phosphatase, are numerous. For the occurrence of a tumor, the changes in the nucleus are important, which are related to the gene expression changes, changes in the system for damaged DNA repair and the changes in mitosis. Mediators of these changes have their role in protein activity regulation, signal transmission in the cell, damaged DNA repair, performance of the cell cycle, apoptosis, tumor suppression, cell proliferation and differentiation. Phosphatase inhibition is observed as the strategy of development of a new group of anticancer agents. As phosphatase inhibitors, microcystins and analogs are applied, possibly having the potential to express the difference in toxicity mechanism and detoxification mechanism between the healthy and the tumor cell of liver. High expression of OATP1B1 and OATP1B3 microcystin transporters in hepatocellular carcinoma would confirm selective impact of phosphatase inhibitors to the tumor cell.

Objectives: Literature data on the immunopathogenesis of neuroinvasive arboviral infections, particularly in the context of complex cytokine immune responses, are limited. The aim of this study was to compare cytokine expression patterns in the cerebrospinal fluid (CSF) and serum in human neuroinvasive arboviral infections caused by tick-borne encephalitis virus (TBEV), West Nile virus (WNV) and Toscana virus (TOSV). Methods: Antiviral cytokine response in the serum and CSF was analysed by using a multiplex bead- based assay for simultaneous quantification of 13 human cytokines (LEGENDplex Human Th cytokine panel, BioLegend, USA) on FACS Canto II flow cytometer (Beckton Dickinson, USA). The panel included cytokines associated with innate and early pro-inflammatory immune responses (TNF-α, IL-6), Th1 type immune response (IL-2, IFN-γ), Th2 immune response (IL-4, IL-5, IL-9 and IL-13), Th17 immune response (IL-17A, IL-17F, IL-21 and IL-22) and the key anti-inflammatory cytokine IL-10. Results: Analysis of cytokine expression pattern in the CSF and serum of patients with neuroinvasive infection with TBEV (32 patients), WNV (68 patients) and TOSV (3 patients) revealed partially similar patterns. U lipnju 2019. godine, Služba za epidemiologiju zaraznih bolesti Hrvatskog zavoda za javno zdravstvo je obaviještena o grupiranju oboljelih od krpeljnog meningoencefalitisa (KME) na području Gorskog kotara. Od šest oboljelih pacijenata, pet ih je konzumiralo nepasterizirano (sirovo) kozje mlijeko s obiteljske farme u razdoblju od dva tjedna prije pojave simptoma, dok je jedna oboljela osoba u anamnezi navela nedavni ugriz krpelja. Kako bi procijenili rizične faktore za infekciju proveli smo slučaj-kontrola studiju u kojoj je izabrano šest kontrolnih ispitanika među zdravim osobama, članovima obitelji ili stanovnicima sela u kojem je zabilježeno grupiranje oboljelih. Provedeno je epidemiološko anketiranje i virološko testiranje oboljelih osoba i kontrolnih ispitanika. Izračunali smo omjer šansi za pojedine rizične izloženosti. Uzorci seruma koza, konja i psa s obiteljske farme testirani su na prisutnost neutralizacijskih protutijela na virus KME (KMEV). Uzorci kozjeg mlijeka i 65 krpelja vrste Ixodes ricinus prikupljenih sa životinja testirani su na prisutnost KMEV RNA metodom RT-PCR. Oboljele su 4 osobe ženskog i 2 osobe muškog spola, a medijan dobi oboljelih osoba bio je 41 godinu (10-85 godina). Oboljele osobe imale su 25 (95% interval pouzdanosti 0.8-1410.2, p = 0.021) puta više šanse da konzumiraju sirovo kozje mlijeko u posljednja dva tjedna u odnosu na zdrave kontrole. Nitko od oboljelih niti kontrola nije bio cijepljen protiv KME. Virus KME nije dokazan u uzorcima mlijeka niti krpeljima, dok je serološko testiranje životinja s farme ukazalo na izloženost virusu: šest koza iz stada imale su neutralizacijska protutijela na KMEV. Rezultati istraživanja pokazali su da je izvor epidemije bila konzumacija nepasteriziranog kozjeg mlijeka s obiteljske farme. Nakon preporuke za prestanak konzumacije proizvoda od nepasteriziranog mlijeka, nije bilo novooboljelih osoba. Ova epidemija ukazuje na potrebu provođenja edukacije među proizvođačima i potrošačima o rizicima koje donosi konzumacija nepasteriziranih mliječnih proizvoda. Preporuča se dodatno testiranje ljudi, domaćih životinja i krpelja radi boljeg razumijevanja epidemiologije KME u Gorskom kotaru, te za ciljano donošenje preporuka o cijepljenju protiv KME.

Background: Tick-borne encephalitis (TBE) is the most important re-(emerging) arboviral zoonosis in Europe. Although up to 80% of TBE cases are asymptomatic or presented as mild febrile illness, 20-40% patients develop neuroinvasive disease (meningitis, meningoencephalitis, meningoencephalomyelitis). While clinical characteristics of TBE are well described, data on the antiviral cytokine response to TBEV are limited. Methods: A total of 32 serum and 16 cerebrospinal fluid (CSF) samples from 33 patients with TBE were included in the study. A multiplex bead-based assay for the simultaneous quantification of 13 human antiviral cytokines was used. The panel included cytokines associated with innate and early pro-inflammatory immune responses: TNF-α, IL-6, IL-2, IFN-γ ; Th2 immune response: IL-4, IL-5, IL-9, IL-13 ; Th17 immune response: IL-17A, IL-17F, IL-21, IL-22 and the key anti-inflammatory cytokine IL-10. Results: Neuroinvasive infection was documented in 29/87.8% patients (meningitis 18/54.5%, meningoencephalitis 11/33.3%), while 4/12.2% of patients presented with nonspecific febrile disease. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a novel coronavirus that emerged in Wuhan, China in December 2019. Coronavirus disease (COVID-19) spread rapidly and in March 2020, WHO declared a pandemic. With global attention on SARS-CoV-2, many other viral infections remain neglected in the shadow of COVID- 19. However, (re-)emerging arboviruses still represent an emerging public health threat. Arboviruses detected in humans in Croatia (acute infections or serologic evidence) are tick-borne encephalitis virus (TBEV), West Nile virus (WNV), Usutu virus (USUV), Toscana virus (TOSV), Tahyna virus (TAHV) and Bhanja virus (BHAV). During the 2020 transmission season, a total of 82 patients with neuroinvasive disease, 15 patients with non- specific febrile disease and 34 travelers returning from endemic areas were tested for arboviruses. TBEV was confirmed u 10 patients, while other neuroinvasive arboviruses were not detected. Clinical presentations were febrile headache, meningitis and meningoencephalitis. All patients recovered. Majority of them reported risk behaviors such as frequent visiting of forest areas and frequent tick bites. TBEV infections occurred from May to July in continental Croatian counties. Previous TBEV, WNV, USUV and TOSV infection (IgG antibodies) was detected in 1.2%, 4.9%, 2.4% and 1.2% of patients, respectively. Additionally, 2.4% of patients showed IgG antibodies to sandfly fever Naples virus. Imported Zika virus infection was documented in a traveler returning from Thailand. During the same period, a total of 188 asymptomatic persons were tested to determine the seroprevalence of neuroinvasive arboviruses. IgG prevalence was 1.6% to TBEV, 3.2% to WNV and 1.1% to USUV. Like in other European countries, arbovirus infections in Croatia in 2020 were, at least partly overshadowed by COVID-19 pandemic.

Koronavirusi (CoV) su velika obitelj RNA virusa koja uzrokuje bolesti dišnog sustava, a one se klinički mogu manifestirati u rasponu od obične prehlade do znatno težih oblika bolesti poput pneumonije praćene akutnim respiratornim sindromom (SARS ; eng. Severe Acute Respiratory Syndrom). COVID-19 je bolest koju uzrokuje novi SARS-CoV-2 koji se lako širi između ljudi, neovisno o prisutnosti simptoma u zaražene osobe. Prvi slučajevi ove bolesti zabilježeni su u Kini u provinciji Hubei sredinom prosinca 2019. godine. Prvi slučaj u Hrvatskoj zabilježen je 25. veljače 2020. godine. Restriktivne epidemiološke mjere uvedene su u ožujku 2020. godine kako bi se spriječilo i kontroliralo širenje bolesti. Postupno popuštanje mjera započelo je u svibnju 2020. godine što je dovelo do postupnog rasta broja zaraženih tijekom ljeta te značajno ubrzanog rasta novozaraženih koje je započelo u rujnu 2020. godine. Do sada je u Hrvatskoj laboratorijski potvrđen 356 181 slučaj zaraze uzrokovane SARS- CoV-2, a 8026 osoba je preminulo. Kako bismo procijenili stvarnu proširenost COVID-19 analizirali smo seroprevalenciju i prisutnost neutralizacijskih protutijela (NT) u uzorcima seruma opće populacije u Hrvatskoj nakon prvog (veljača - srpanj 2020. godine) odnosno drugog (kolovoz 2020. - veljača 2021. godine) pandemijskog vala. Početno serološko testiranje provedeno je komercijalnim imunoenzimskim (ELISA) testom (Vircell, Microbiologists, Granada, Španjolska), a za reaktivne uzorke kao potvrdni test korišten je test neutralizacije virusa (VNT ; eng. virus neutralization test). Zabilježena je statistički značajna razlika u rezultatima ukupne seroprevalencije između prvog (ELISA 2, 2% ; VNT 0, 2%) i drugog pandemijskog vala (ELISA 25, 1% ; VNT 18, 7%). Standardized and effective antibiotic therapy regimes in gynecology need to be based on the protocols prescribed by acknowledged specialists. Antibiotic therapy can have either prophylactic or therapeutic purposes. Antibiotics of the narrowest possible spectrum are recommended, as well as their shortest while most effective application. The choice is firstly determined by the effectiveness and secondly by the price. Preventive use of antibiotics and the doses in gynecological surgery depend on the type of surgery. If antibiotic therapy is prophylactic, first generation cephalosporin should always be used. Irrational antibiotic therapy, especially when using third generation cephalosporin, can result in strains of resistant hospital bacteria.

44. Typology of spatial ability tests and its implementation in architectural study entrance exams

- Abstract
Specialized spatial skills are necessary for success in various fields of STEM (science, technology, engineering, and mathematics) education. Technical disciplines are an academic field where the largest correlation with spatial skills has been noticed, and therefore spatial skills have been included in the entrance exams of study of architecture at the University of Banja Luka. Given that the scientific community has not reached consensus on what the spatial abilities are, there are various tests and tools used for its assessment, listed by factors that they measure. The paper will present typology of these factors and the variety of tests used for their assessment. This typology of tasks will be compared to the entrance exams held at the University of Banja Luka in the period 2005-2013. Also, the results of entrance exams will be compared with the student’s success in specific groups of subjects during the study period to see if there would be any correlation among them. Results indicate at the emergence of a new factor in assessing the ability of candidates to study architecture - ability of divergent thinking. This correlation of divergent thinking and spatial ability has also been a topic of the latest research in cognitive psychology.

Cilj /Aim Prikazati epidemiološku situaciju hepatitisa B i C u Hrvatskoj i nacrt Nacionalne strategije za prevenciju i kontrolu virusnih hepatitisa. Materijali i metode / Materials and methods Epidemiologija hepatitisa B i C te razvoj i struktura nacrta Nacionalne strategije za prevenciju i kontrolu virusnih hepatitisa prikazat će se deskriptivnom metodom temeljem pregleda literature, i objavljenih i neobjavljenih dokumenata i podataka. Rezultati i zaključak / Result and conclusion Hrvatska ima nisku prevalenciju infekcije virusom hepatitisa B (HBV) i hepatitisa C (HCV) u općoj populaciji. Prema rezultatima seroepidemioloških studija, oko 0, 2 % - 0, 7 % opće populacije ima kronični hepatitis B, a 0, 5 % - 0, 9 % anti-HCV protutijela. Osobe koje injektiraju droge čine glavnu rizičnu skupinu s prevalencijom od 29 % - 65 %. Procjenjuje se da oko 25.000 osoba ima kroničnu HBV infekciju, a oko 40.000 osoba je pozitivno na anti-HCV protutijela. Prema podacima registra zaraznih bolesti u zadnjih pet godina prosječno se prijavi godišnje 120 oboljelih od hepatitisa B i 180 oboljelih od hepatitisa C. Univerzalno cijepljenje djece protiv hepatitisa B je glavna preventivna mjera za eliminaciju ihepatitisa B (uvedeno u Program obveznog cijepljenja 1999.godine). Hrvatska ima učinkovitu terapiju za oboljele od virusnih hepatitisa, a direktno djelujući antivirusni lijekovi za liječenje hepatitisa C (DAA) dostupni su od 2015. godine. Virusni hepatitisi predstavljaju javnozdravstveni prioritet, i 2018. godine izrađen je nacrt Nacionalne strategije za borbu protiv hepatitisa, čiji je cilj smanjiti učinak bolesti na ljude i društvo do 2030. godine, u skladu s Globalnim ciljevima održivih razvoja i Globalnom zdravstvenom strategijom o virusnim hepatitisima Svjetske zdravstvene organizacije, kojom se želi smanjiti pobol od virusnih hepatitisa za 90 % i smrtnost za 65 %. Ciljevi strategije su: podizanje svijesti opće populacije i rizičnih populacija, praćenje odgovora zdravstvenog sektora, smanjenje novih infekcija i smrtnosti oboljelih. Uz postojeće sustavne i kontinuirane mjere prevencije, dijagnostike i liječenja, ovom sveobuhvatnom strategijom se utvrđuju prioriteti za daljnji napredak. Eliminacija hepatitisa B i C se čini dostižnim javnozdravstvenim ciljem u koji se Hrvatska može uklopiti, za što će trebati međusektorska suradnja i stalna politička podrška.

Kirurško liječenje deformiteta šaka bolesnika s buloznom epidermolizom, unatoč stalnomu napretku znanstvenih spoznaja, još je i danas obilježeno brojnim prijeporima. Cilj istraživanja je na temelju medicinske dokumentacije utvrditi karakteristike, specifičnosti i ishode svih bolesnika s buloznom epidermolizom koji su kirurški liječeni zbog deformiteta šake u Zagrebu u razdoblju od 1994. do 2009. godine. Zagrebački kirurški protokol rekonstrukcije (ZSRP) deformiteta šake u bolesnika s buloznom epidermolizom (EB) jedinstven je alat standardiziranih postupaka i jasno definiranog algoritma. Analizirani su značajke i ishodi u bolesnika s buloznom epidermolizom skupine bolesnika liječenih u razdoblju od 1994. do 2001. sa skupinom bolesnika koji su kirurški liječeni od 2002. do 2009. godine (s follow up- om od 2 godine poslije termina operacije) s obzirom na to da su od 2002. godine u kiruršku praksu uvedene nove smjernice s brojnim modifikacijama postupka (ZSRP). Primjena Zagrebačkog kirurškog protokola rekonstrukcije deformiteta šake u bolesnika s buloznom epidermolizom sa svojim inovativnim sastavnicama ima praktične prednosti u usporedbi s prethodno primjenjivanim kirurškim postupcima zbog svoje veće učinkovitosti uz redukciju ukupnih troškova liječenja. Uzimanjem presatka kože po metodi PRST postiže se brže cijeljenje donorskog područja, uz smanjenje rizika očekivanih komplikacija u usporedbi s klasičnim tehnikama uzimanja STSG presatka kože dermatomom ili elektrodermatomom. Prema našem dosadašnjem 20- godišnjem iskustvu primjerena kirurška korekcija deformiteta šaka u bolesnika s EB-om omogućava uspješno višekratno ponavljanje kirurškog postupka koji je indiciran kao posljedica očekivane progresije lokalnog statusa zbog prirode same bolesti.

48. Procjena intenziteta sezonske gripe u Hrvatskoj upotrebom metode epidemijskog kretanja

Cilj: Istražiti mogućnost standardiziranog praćenja sezonske gripe u skladu sa metodologijom Svjetske zdravstvene organizacije i Europskog centra za prevenciju i kontrolu bolesti putem utvrđivanja pragova za početak epidemije i njezine različite intenzitete (niski, srednji, visoki i vrlo visoki), prosječnog tjedna početka epidemije te prosječnog trajanja epidemije gripe. Materijali i metode: Korišteni su podaci tjednih agregiranih izvješća o broju oboljelih od bolesti slične gripi (eng. influenza like illness, ILI) u Hrvatskoj, koji se se prikupljaju od 40. tjedna tekuće do 20. tjedna naredne godine. Analizom su obuhvaćeni podaci o obolijevanju tijekom pet sezona gripe: 2013./2014., 2014./2015., 2015./2016., 2016./2017. i 2017./2018. godine, na temelju kojih su izračunate tjedne stope prijava kliničke gripe za svaku promatranu sezonu. Analiza tjednih stopa ILI prijava izvršena je primjenom metode epidemijskog kretanja (eng. Moving Epidemic Method, MEM). Rezultati i zaključak: Primjenom MEM metode utvrđena stopa epidemijskog praga iznosi 19, 68/100 000. Epidemijski prag označava stopu kod koje broj oboljelih od gripe na određenom prostoru prelazi okvire sporadičnog obolijevanja označavajući tako početak epidemije gripe. Epidemija gripe ima svoju krivulju koja se razlikuje od sezone do sezone, a ovisi o nizu čimbenika, od kojih su najvažniji vrsta i intenzitet cirkulirajućih virusa gripe, učinkovitosti cjepiva protiv sezonske gripe i udio osjetljive populacije. Izračunati pragovi za različite intenzitete epidemije iznose 80, 12/100 000 za srednji, 283, 67/100 000 za visoki i 496, 04/100 000 za vrlo visoki intenzitet epidemije. Prosječni tjedan početka epidemije je 4. epidemiološki tjedan, a prosječno trajanje epidemije je 10 tjedana, pri čemu je epidemijom gripe obuhvaćeno 92, 32% ukupnih ILI prijava tijekom sezone. U sezonama gripe koje su obuhvaćene ovim istraživanjem, tri (2014./2015., 2016./2017. i 2017./2018.) su bile srednjeg (stopa 256, 0, 208, 1 odnosno 212, 5/100 000), a dvije (2013./2014. i 2015./2016. ) niskog intenziteta (stopa 35, 4 odnosno 64, 5 /100 000). Prema trenutno raspoloživim podacima (stopa 272, 8/100 000, 5.tjedan) sezona 2018./2019., koja je još u tijeku, blizu je praga visokog intenziteta.

49. Trendovi mortaliteta i morbiditeta kroničnih virusnih hepatitisa u Hrvatskoj

Cilj Prikazati epidemiološku situaciju kroničnih virusnih hepatitisa B i C u Hrvatskoj. Materijali i metode Epidemiologija virusnih hepatitisa prikazat će se deskriptivnom metodom temeljem podataka registra za zarazne bolesti, javnozdravstvenog registra uzroka smrti Hrvatskog zavoda za javno zdravstvo (HZJZ). Rezultati i zaključak / Results and conclusion Hrvatska ima nisku prevalenciju infekcije virusom hepatitisa B (HBV) i hepatitisa C (HCV) u općoj populaciji. Prema rezultatima seroepidemioloških studija, oko 0, 2 % - 0, 7 % opće populacije ima kronični hepatitis B, a 0, 5 % - 0, 9 % anti-HCV protutijela, dok osobe s povećanim rizikom za zarazu imaju značajno višu prevalenciju ( osobe koje injektiraju droge čine glavnu rizičnu skupinu za infekciju HCV-om s prevalencijom od 29 % - 65 %). Procjenjuje se da oko 25.000 osoba ima kroničnu infekciju HBV, a oko 40.000 osoba nosioci su anti-HCV protutijela. Prema podacima registra zaraznih bolesti HZJZ-a u posljednjih pet godina prosječno se godišnje zabilježi 115 oboljelih od hepatitisa B i 181 oboljelih od hepatitisa C, što pokazuje stabilan trend pojavnosti hepatitisa B i C u Hrvatskoj. Prema podacima iz baze hospitalizacija HZJZ-a, u promatranom petogodišnjem razdoblju je zbog kroničnih virusnih hepatitisa B i C bilo prosječno godišnje 2.060 hospitalizacija u stacionarnom dijelu i dnevnim bolnicama. Prema podacima javnozdravstvenog registra uzroka smrti HZJZ-a u razdoblju 2009.-2018. godine prosječno je bilo godišnje zabilježeno 95 smrtnih ishoda od kroničnih virusnih hepatitisa B i C. U svrhu poboljšanja kvalitete podataka i razumijevanja epidemiološke situacije i rizičnih čimbenika povezanih s pojavnosti virusnih hepatitisa, za oboljele u 2018. godini prikupljeni su anketni upitnici za 62 % oboljelih od hepatitisa B i 21 % oboljelih od hepatitisa C, što je za prvu godinu dodatnog praćenja razmjerno dobar udio prikupljenih dodatnih epidemioloških i kliničkih podataka, no zahtijeva ulaganje dodatnog napora u poboljšanje praćenja virusnih hepatitisa.

Contemporary cancer chemotherapy need to be active against malignant cell (selectivity), to be based on the molecular biology of the cancer cell, to enhance immune response to cancer, and to stop development of resistance to drug. Research in this field is expensive, as well as the cost of newly discovered drugs. Targeted therapies are registered in the Republic of Serbia and their applications are controlled by the state authority. At the Oncology Institute of Vojvodina smaller number of patients was treated with targeted therapy than with classical chemotherapy. However, our costs for treating patients with monoclonal therapies are at the level of those worldwide.
