Unipolar depressive disorders in adolescence are common, lead to serious impairments and are often associated with comorbid disorders and a high risk for suicide. Thus, recognition and early treatment of depressive disorders are important. International and national treatment guidelines show that effective treatment approaches for prevention and acute therapy of depressive disorders are available. Based on current evidence and consensus-based guidelines, such as the Association of the Scientific Medical Societies in Germany (AWMF) S3 treatment guidelines for unipolar depressive disorders in children and adolescents, state of the art diagnostic procedures and treatment recommendations are proposed. Diagnostic procedures and differential diagnoses as well as differential indications and treatment planning are reported in detail. In the treatment section the focus is on cognitive behavioral therapy (CBT), which is currently the best evaluated psychological treatment form for depressive disorders in children and adolescents.

Der Tagungsband enthält das Programm und die Abstracts des 26. Symposiums der Fachgruppe Klinische Psychologie und Psychotherapie der Deutschen Gesellschaft für Psychologie, veranstaltet an der Universität Potsdam vom 1. bis 3. Mai 2008. Etwa 450 Kongressteilnehmer präsentieren den aktuellen Forschungs- und Wissensstand der Klinischen Psychologie und Psychotherapie in Deutschland. Grußworte halten die brandenburgische Ministerin für Arbeit, Soziales, Gesundheit und Familie, Dagmar Ziegler, die Präsidentin der Universität Potsdam, Prof. Dr.-Ing. Dr. Sabine Kunst, sowie Prof. Dr. Michael Linden als Vertreter der Deutschen Gesellschaft für Psychiatrie, Psychotherapie und Nervenheilkunde (DGPPN). Zu den Themenschwerpunkten des Kongresses gehören Einflussfaktoren auf die psychische Gesundheit Älterer, Impulsivität, Schlaf- und Traumforschung in der Klinischen Psychologie, Behandlung von Essstörungen, Wirksamkeitsstudien psychischer Störungen des Kindes- und Jugendalters, Angst und Depression, Behandlung von Kriegs- und Folteropfern, Risiko- und Schutzfaktoren der Kindesentwicklung sowie Adipositas im Kindes- und Jugendalter. Außer den Vorträgen gibt es eine Präsentation von etwa 150 Postern. Zum Programm der Tagung gehört ebenso die Verleihung des Klaus-Grawe-Awards for the Advancement of Innovative Research in Clinical Psychology and Psychotherapy an Prof. Dr. Timothy J. Strauman von der Duke University (USA), die Verleihung der Nachwuchswissenschaftler- und Posterpreise sowie ein Pre-conference Workshop für Doktorandinnen und Doktoranden der Klinischen Psychologie zum Thema "Verhaltens- und Molekulargenetik".

Theoretischer Hintergrund: Eine frühzeitige Gesundheitsförderung wird als wesentliches Bestimmungsstück zur Prävention körperlicher und psychischer Erkrankungen angesehen. Als ideales Setting für eine gezielte Gesundheitsförderung wird der Lebensraum Kita angesehen. Fragestellungen: Welche Aktivitäten zur Gesundheitsförderung gibt es bisher in den Brandenburger Kitas? Spielt hier die Bildungsnähe/-ferne der betreuten Kinder eine Rolle? In welchen Bereichen gibt es den größten Fort- und Weiterbildungsbedarf der Mitarbeiterinnen? Methode: Untersuchung einer Stichprobe von 269 Brandenburger Kitas mit insgesamt 21 653 betreuten Kindern. Erfassung struktureller Rahmenbedingungen, pädagogischer Konzepte, Aktivitäten zur Gesundheitsförderung, Partizipation und Fortbildungsbedarf. Ergebnisse: Es zeigte sich eine nachhaltige Umsetzung von Aktivitäten und große Kreativität. Dabei standen vor allem Angebote der Bereiche Bewegung und Ernährung im Zentrum. Psychosoziale Themen wurden seltener als Schwerpunkt benannt, wohingegen hierfür der größte Fortbildungbedarf (Ranking der Fortbildungsthemen: 1. Psychosoziale Entwicklung, 2. Psychische Probleme, 3. Stressbewältigung) vorlag.

Objectives: Prevalence rates and sex differences in depression, anxiety, and eating disorders and associations with recalled childrearing practices. Methods: Cross-sectional study based on self-report scales: Questionnaire of Recalled Parental Rearing Behavior (German version of EMBU), Beck Anxiety Inventory (BAI), Fragebogen zur Depressionsdiagnostik nach DSM-IV (German version of the Inventory to Diagnose Depression), and Eating Attitudes Test (EAT). 707 university entrants with an average age of 20 years were tested. Results: Point prevalence rates of 6.2 % for depression, 5.2 % for anxiety disorders, and 6.9 % for eating disorders. Higher rates in females for all of the three disorders. Significant associations of sociodemographic and biographical factors with the recalled parental rearing behavior and mental disorders was found. After controlling the impact of factors such as sex, parental divorce, or the occurrence of chronic physical disease the recalled parental rearing behavior proved to be a significant variable especially for depression (lack of warmth by the father, rejection/punishment and control/overprotection by the mother), but also for anxiety (control/overprotection by the father and rejection/punishment by the mother) and eating disorders (control/overprotection by the mother)

This paper describes the current findings concerning efficacy from randomized controlled trials of family-based interventions for children and adolescents with anxiety and depressive disorders. To date, parents have only been included in controlled trials of cognitive-behavioral interventions. Efficacy trials for anxiety disorders have only been carried out in 6- to 14-year olds, but have shown that younger children (7 to 10 years old) benefited when the family was involved. By contrast, the existing efficacy trials for depressive disorders have been limited to adolescents (13 to 18 years old), and have shown that family-based interventions are not superior to pure adolescent therapy

This editorial stresses the great importance of family-oriented assessment and psychotherapy for mental disorders in children and adolescents. Further systematic evaluation of family-based intervention programs and family- oriented assessment scales in controlled trials is suggested. This could decrease the existing discrepancy between clinical practice and empirical research in the field of child and adolescent psychotherapy

An outline of evidence-based guidelines for the assessment and treatment of depressive disorders in childhood and adolescence is presented. Depressive disorders in children and adolescents are marked by core symptoms similar to those seen in adults, although symptom expression varies greatly with developmental stage. These disorders are common, especially in adolescence, chronic, and recurrent, and are associated with comorbid conditions such as anxiety disorders, conduct disorders, and substance use disorders. Effective treatment approaches for the prevention of depressive disorders and the acute treatment of mild and moderate depressive disorders are available. The psychotherapeutic interventions of choice are currently cognitive behavioral therapy (CBT) and interpersonal therapy (IPT). The antidepressants of choice are currently selective serotonin reuptake inhibitors (SSRI). Especially on relapse prevention and the evaluation of the combination of psychotherapy with antidepressant medication further studies are necessary

Praxis der Kinderpsychologie und Kinderpsychiatrie 52 (2003) 6, S. 409-424, Die Arbeit gibt einen Überblick über aktuelle kognitiv-verhaltenstherapeutische Behandlungsansätze nicht dissozialer Schulverweigerung (Schulphobie und Schulangst) bei Kindern und Jugendlichen. Einleitend wird der Problembereich definiert sowie der Stand des Wissens zu Prävalenz, Verlauf und Risikofaktoren dargestellt. Den Schwerpunkt der Arbeit stellt die psychotherapeutische Behandlung von Schulphobie und Schulangst dar. Neben allgemeinen Behandlungsprinzipien wird auf die wichtigsten verhaltenstherapeutischen Techniken sowie auf kognitivverhaltenstherapeutische Behandlungsprogramme eingegangen. Es folgt eine ausführliche Beschreibung des Stands der Wirksamkeitsforschung. Schließlich wird praxisnah der Ablauf der Diagnostik, Therapieplanung und -durchführung dargestellt.(DIPF/Orig.), The study provides an overview of current cognitive-behavioral interventions to treat school-refusal in children and adolescents. First of all we define the problem area and describe the standard of knowledge about prevalence, course and risk factors. The main focus of our work is presented by psychotherapeutic treatments of school phobia and school anxiety. Apart from general treatment principles we give a survey of the most important behavioral therapeutic as well as cognitive-behavioral therapeutic treatments. In addition, we provide a detailed description of current research about efficacy. After all we characterize diagnostic, treatment planning and implementation.(DIPF/Orig.)
