Background: Although neurological complications are well recognized in sickle cell disease (SCD), myelopathy has been rarely described. We present the first case report of longitudinally extensive myelitis (LETM) in SCD and review the differential diagnosis of myelopathy in these patients. Design and setting: case-oriented review. Methods: We report the case of a 29-year-old African-Brazilian man with SCD, who experienced a subacute flaccid paraparesis, with T2 sensory level and urinary retention. CSF analysis showed a lymphocytic pleocytosis and increased protein levels. MRI disclosed a longitudinally extensive spinal cord lesion, with a high T2/STIR signal extending from C2 to T12. Serum anti-aquaporin-4 antibody was negative. We searched Medline/ PubMed, Embase, Scopus, and Google Scholar databases for myelopathy in SCD patients. Results: Spinal cord compression by vertebral fractures, extramedullary hematopoietic tissue, and Salmonella epidural abscess have been reported in SCD. We found only three case reports of spinal cord infarction, which is unexpectedly infrequent compared to the prevalence of cerebral infarction in SCD. We found only one case report of varicella-zoster myelitis and no previous report of LETM in SCD patients. Conclusion: Specific and time-sensitive causes of myelopathy should be considered in SCD patients. In addition to compression and ischemia, LETM should be considered as a possible mechanism of spinal cord involvement in SCD.

Introdução: As doenças do aparelho circulatório (DAC) são responsáveis por 30% das mortes no Brasil, além de gerar incapacidades ealtos custos. Apesar disso, pouco se sabe sobre a relação entre a mortalidade por DAC e as condições de vida e assistência à saúde nos municípios brasileiros, especialmente em contextos regionais. Objetivo: investigar a associação de variáveis de saúde e sociodemográficas com a mortalidade por DAC em Minas Gerais (MG). Método: estudo ecológico a partir de dados secundários,tendo como unidade de análise 418 municípios de MG. A variável dependente foi a taxa de mortalidade por DAC e as variáveis de desfecho foram 32 indicadores de serviços de saúde e 22 indicadores demográficos e socioeconômicos. Realizou-se a análise bivariada dos dados através do coeficiente de correlação de Pearson, seguida da análise fatorial e, por fim, da regressão linear múltipla. Resultados: As DAC ocasionaram 30,9% da mortalidade total nos municípios analisados, observando-se associação positiva com número deconsultórios de clínica básica, população urbana e internações por DAC no SUS; e negativa comvalor médio por internaçãoe despesa total com profissionais da saúde. Conclusão: frente ao elevado número de municípios com pequena estrutura assistencial em MG, as ações de promoção à saúde e prevençãoparecem ser as mais adequadas visando a redução dastaxas de mortalidade por DAC. Também com esse objetivo, os casos mais graves e as complicações devem ser manejados em centros de referência, contando com recursos materiais e profissionais adequados.

Resumo Objetivo: Identificar os principais diagnósticos e características epidemiológicas dos pacientes atendidos em um serviço público de urgência oftalmológica. Métodos: Estudo transversal e retrospectivo, com análise de prontuários de 2834 pacientes atendimentos no período de julho a setembro de 2017, no serviço de Urgência Oftalmológica do Complexo Hospitalar Ouro Verde, Campinas, Brasil. As variáveis estudadas foram idade, gênero e diagnóstico. Resultados: Houve a prevalência do gênero masculino (52,6%) e da faixa etária de 30 a 59 anos (43,5%); 21,1% eram idosos. Os diagnósticos mais prevalentes foram conjuntivite infecciosa (23,9%), trauma ocular (15,7%) e doenças da superfície ocular (14,6%). Entre mulheres e crianças houve o predomínio de quadros infecciosos/inflamatórios; 83,6% dos traumas ocorreram em homens, sendo 62,2% devido a corpo estranho. Conclusão: Os diagnósticos mais prevalentes no serviço de urgência oftalmológica foram conjuntivite infecciosa e trauma ocular. Medidas de educação e prevenção são necessárias para reduzir morbidade e absenteísmo e evitar o uso inapropriado dos serviços especializados de emergência. Abstract Objective: To identify the most frequent diagnosis and epidemiological characteristics of patients attended in a public ophthalmic emergency service. Methods: This is a retrospective and transverse study based on 2834 patients' chart attended from July to September 2017 at the Ouro Verde Hospital Complex, Campinas, Brazil. The following variables were investigated: age, gender, and diagnosis. Results: Most patients were male (52.6%) and aged between 30 to 59 years (43.5%); 21.1% were elderly. The most frequent diagnostics were conjunctivitis (23.9%), ocular trauma (15.7%), and ocular surface disorders (14.6%). Infectious/inflammatory conditions predominated among children and female; 83.6% of traumas occurred in man, and 62.2% were due to a foreign body. Conclusion: The most prevalent diagnoses in the ophthalmologic emergency department were infectious conjunctivitis and ocular trauma. Education and prevention measures are necessary to reduce morbidity and absenteeism and to avoid inappropriate use of specialized emergency services.

O objetivo deste artigo é descrever a percepção sobre quedas dos idosos residentes na comunidade; mensurar a exposição desses indivíduos a fatores de risco domiciliares relacionados; e avaliar a influência do conhecimento sobre queda na adoção de medidas preventivas. O questionário FRAQ-Brasil foi aplicado a 473 idosos, juntamente com um questionário sobre exposição a 20 fatores de risco domiciliares. Associações entre as variáveis foram analisadas utilizando o teste qui-quadrado com intervalo de confiança de 95%. A idade variou de 60 a 95 anos, com média de 70,6 anos; os entrevistados foram principalmente do sexo feminino (58,4%), e renda de 2 salários mínimos (46,3%); a média de acertos no questionário FRAQ-Brasil foi de 19,5 em até 32 pontos e os idosos entrevistados estavam expostos, em média, a 7,8 fatores de risco domiciliares; 180 idosos disseram já terem recebido informações sobre queda. A maioria da população idosa apresenta pouco conhecimento sobre queda. Indivíduos com idade mais avançada e com maior conhecimento sobre queda estão expostos a menos fatores de risco domiciliares, possivelmente pela maior adoção de medidas preventivas através da modificação do ambiente doméstico. Objectives: to describe the awareness about falls among elderly people living in their households in communities, to measure the other risks that they are exposed to in their homes and to evaluate the influence that knowledge on falling brings in the adoption of preventative measures. Method: The FRAQ-Brazil questionnaire was used on 473 elderly people as well as a questionnaire on elderly people being exposed to 20 household risk factors. Associations between the variables were analyzed using the chi-squared test with a confidence interval of 95%. Results: The age range was between 60 and 95 years with the average being 70.6 years. The majority of those interviewed were female (58.4%) who were earning 2 minimum wages (46.3%). The average amount of correct answers given with the use of the FRAQ-Brazil questionnaire was 19.5 out of 32 points and the elderly participants were, on average, exposed to 7.8 household risk factors. 180 of them stated that they had already received information on falls. Conclusion: The majority of the elderly population displayed little knowledge on falls and were exposed to a variety of daily risk factors. Individuals who were more advanced in years and who had more knowledge on falls, were exposed to less household risk factors. This may well have been due to the adoption of preventative measures through changing domestic environment.

A Política Nacional de Saúde Integral da População Negra tem o objetivo de garantir a equidade na atenção à saúde para esse segmento populacional. Tal medida possui um caráter compensatório em virtude das discriminações raciais existentes ao longo da história do Brasil. A população negra apresenta maior vulnerabilidade social e econômica, o que reflete uma menor expectativa de vida e maior susceptibilidade a agravos. O objetivo do estudo é investigar o conhecimento da população negra acerca da política, seus potenciais benefícios e as dificuldades de acesso à saúde. Trata-se de uma pesquisa transversal, descritiva e quantitativa. Foram realizadas entrevistas estruturadas com 391 indivíduos negros, usuários do SUS, da cidade de Juiz de Fora. A amostra foi estratificada de acordo com raça (preto e pardo), renda e escolaridade. Cerca de 90% dos entrevistados relataram desconhecer a existência de uma política de saúde para a população negra e 53% declararam uma possível discriminação racial. Observou-se também associação positiva entre discriminação e menor escolaridade e renda. Apesar de desconhecerem a existência da PNSIPN, a maioria dos entrevistados aprovou seus objetivos, mesmo relatando a possibilidade de discriminação dela advinda. Around 90% of the sample reported not knowing the existence of a health policy for the black population, and 53% stated that this policy could enhance racial discrimination. Having completed primary education and lower income was positively associated with higher discrimination in health care. Most of the sample didn’t know the existence of the PNSIPN, but was in favor of its goals, despite the possibility that it can reinforce the legacy of Brazilian racial discrimination.
