Abstract

Hepatitis C virus (HCV) infection is one of the major global health problems, particularly for people who inject drugs (PWID). This study aimed to investigate the spontaneous clearance rate of HCV and associated factors with the clearance among PWID using an HCV core antigen (HCV-cAg) test. Blood samples were collected from 311 male PWID concentrated in a rehabilitation center in Haiphong,Vietnam, every 6 months for up to 18 months from 2007, and analyzed serologically and genetically. Of the 311, 240 (77.2%) were positive for anti-HCV antibody (HCV-Ab), and 219 (70.4%) for HCV-cAg,though three HCV-cAg positives were negative for HCV-Ab. Of the 219, 52.5% were co-infected with human immunodeficiency virus (HIV). The most prevalent HCV genotype was genotype 1 (45.7%),followed by genotypes 6 (40.6%), and 3 (7.6%). Spontaneous clearance of HCV-cAg was observed in 12 (5.5%) of 219 PWID during the observation period, and the clearance rate was 6.5 per 100 personyears (95% confidence interval, 3.5–11.3). HCV-cAg level was marginally higher in PWID co-infected with HIV than those without HIV (p = 0.054) and lower in PWID with HCV genotype 3 than those with other genotypes (p < 0.001). Abstract

Background: Diarrhoea is the second disease killer after respiratory diseases in children. Globally, there are nearly 1.7 billion cases of diarrheal disease every year. In developing countries, enteric bacterial pathogens are most common causes of morbidity and mortality in children especially under 5 years. Most of the studies done on enteric bacteria pathogens and HIV co-infection have focused on the children less than 5 years of age but not above. Objective: This study aimed at evaluating the distribution of common circulating enteric bacterial pathogens; Escherichia (E).coli, Shigella, and Salmonella among HIV infected (n=79) and uninfected (n=78) children aged 5-12 years from Dandora slums of Nairobi. Methods: This was analytic cross-sectional study of HIV positive children enrolled at Nyumbani Lea Toto HIV/AIDS outreach program in Dandora, while HIV negative are children from same area (preferably sibling). Stool samples were collected from consenting participants and sent to Microbiology laboratory in Kenya Medical Research institute for processing. The samples were cultured using differential media for enteric bacteria. Suspected isolates were further identified using conventional biochemical methods and serology. Multiplex PCR was done on E. coli isolates to detect virulence factors responsible for different E. coli pathotypes. Results: The overall prevalence of pathogenic Escherichia coli, Shigella and Salmonella was 44 (28%), 31 (19.7%) and 0 (0.0%) respectively. Enteroaggregative E.coli (43.2%) was the main E. coli pathotypes observed. Distribution of pathogenic E. coli in HIV infected and uninfected was 12.7% and 15.3%, respectively (p = 0.30), while that of Shigella was 6.4% and 13.4% (p = 0.03). Conclusion: From this study HIV infected children had less infestation of Shigella species as compare to HIV uninfected children, which could be due to constant treatment for any infections thus interferes with bacteria grow.

Abstract

Changes in the expression of arginase and their association with nitrosative stress were investigated using an asthmatic model previously established in NC/Nga mice with mite extract. Mite crude extract (100 [micro]g/day) from Dermatophagoides farinae was administered intranasally for 5 consecutive days (day 0-4), and a single challenge was performed on day 11. On day 12, upregulation of the mRNA expression of inducible types of nitric oxide synthase (iNOS) and increases in immunohistochemical staining for iNOS and nitrotyrosine were observed. However, the level of nitrite nitrate was unchanged. An increase in enzymatic activity, upregulation of mRNA expression, and immunostaining for arginase I was detected in the lung tissue and serum. Moreover, increases in both arginase I and II were revealed by immunoblotting. Goblet cell hyperplasia in bronchial epithelial cells and increasing collagen synthesis around the bronchus were also observed. These results suggested that an increase in arginase may lead to decreased availability of arginine for nitric oxide synthase and may contribute to the remodeling of the lung. Abstract

The prevalence of antiretroviral therapy (ART) resistance-associated mutations among HIV-1 strains in western Cameroon was evaluated by genotypically analyzing strains isolated from drug-naive individuals, extracting proviral DNA from 54 blood samples. It was established that primary mutations of the HIV-1 protease existed in drug-naive patients in rural western Cameroon, despite little use of protease inhibitors.

The presence of genotypic HIV-1 variants circulating in eastern Cameroon from blood samples of 57 HIV-1-infected individuals attending 3 local health centers in the bordering rural villages with Central African Republic (CAR) collected and analyzed phylogenetically is monitored. The study showed that the findings strongly suggest that this part of Cameroon is a potential hotspot of HIV-1 recombination.

49. Clinical outcomes of patients with chronic hepatitis C after generic direct-acting antiviral treatments in Vietnam: A retrospective analysis.

Abstract

Objective: To evaluate the outcomes of generic direct-acting antiviral treatments for chronic hepatitis C in Vietnam. Methods: The medical records of 522 patients (median 45 years; Female, 25.3%) with chronic hepatitis C treated at a tertiary hospital in northern Vietnam in 2016 were retrospectively reviewed. Results: Female patients were significantly older than male patients (median 52, IQR 41-59 vs. 43 years, IQR: 39-55; P<0.001). Among 522 patients, 49.4% were infected with hepatitis C virus (HCV) genotype 6, followed by 1a (19.0%), 1b (13.0%), and 3 (5.9%). Co-infection with hepatitis B virus or human immunodeficiency virus was noted in 5.8% and 3.1% of patients, respectively. Patients were treated with ledipasvir/sofosbuvir with or without ribavirin (70.9%), sofosbuvir/pegylated-interferon + ribavirin (13.2%), daclatasvir/ sofosbuvir with or without ribavirin (12.5%), or sofosbuvir/ribavirin (3.4%), and 96.4% (n=503) completed the direct-acting antiviral treatment. No patient discontinued treatment due to adverse event(s). A sustained virologic response 12 weeks after the end of the treatment (SVR12) was evaluated in 62.6% of patients. Overall sustained virologic response 12 weeks after the end of the treatment was 98.7% regardless of HCV genotypes or direct-acting antiviral regimens. The severity of liver stiffness was significantly decreased from 10.2 to 6.3 kilopascals measured by transient elastography by the treatment (P<0.001). Among patients who completed the direct-acting antiviral treatment, 17.7% returned for further follow-ups after SVR12. Conclusions: In Vietnam, the current generic direct-acting antiviral treatment for chronic hepatitis C was effective regardless of HCV genotypes and direct-acting antiviral regimens with the attenuation of liver stiffness. It is feasible to implement direct-acting antiviral treatment to cure chronic hepatitis C patients at any liver fibrosis stages in Vietnam. [ABSTRACT FROM AUTHOR]
