Abstract

BACKGROUND: Trainees need to operate under appropriate supervision to become competent. Transit time flow measurement (TTFM) is useful in detecting intraoperative graft failures during coronary artery bypass grafting (CABG). This study aims to compare the intra- and postoperative outcomes, including TTFM, of isolated CABG performed by a single consultant versus trainees and to determine the relationship between the pulsatility index (PI) and postoperative outcomes. METHODS: This retrospective study included 155 isolated CABG cases. Intraoperative outcomes included duration of cardiopulmonary bypass (CPB) and aortic cross-clamp and TTFM for arterial and venous grafts. Postoperative outcomes included common postoperative complications and mortality. The odds ratios of postoperative complications and mortality for arterial and venous grafts with PI ≤3 relative to grafts with PI >3 were determined with multiple logistic regression. RESULTS: The duration of CPB and aortic cross-clamp was significantly shorter in the consultant’s group. TTFM for both arterial and venous grafts were similar and no significant differences in postoperative complications and mortality were detected between the two groups. Patients with arterial grafts with PI ≤3 were less likely to require an intra-aortic balloon pump (IABP) or be ventilated for a prolonged period. No significant differences in postoperative outcomes and mortality were detected between venous grafts with PI ≤3 and PI >3. CONCLUSIONS: Trainees can achieve good results in isolated CABG with appropriate case selection. Patients with arterial grafts with PI ≤3 have better postoperative outcomes.

Abstract

Unwanted vibration and noise from railroads have a significant negative impact on the environment, causing damage to roads, buildings, and other structures. To mitigate this condition, rail pads have been installed as dampers to lessen the impact of vibration and shock on the railway track. The rail pad is made of a polymeric substance having nonlinear properties. This research examined the dynamic stiffness of rail pads made of thermoplastic elastomers (TPEs). ANSYS software was used to estimate the impact of temperature, toe load, and frequency under dynamic loading. The three-dimensional (3D) finite element model (FE) was created based on hyperelastic theory. The dynamic stiffness of the interlayer decreases with increasing temperature. For the effect of peak load and frequency, both parameters were directly proportional to dynamic stiffness. An increase in either parameter results in a stiffening of the interlayer. Frequency has the least effect on the dynamic stiffness of the track bed compared to temperature and peak load, with the average percentage difference between high and low being 28.31%, 55.57%, and 21.9%, respectively. Abstract

Fragility curves express the seismic vulnerability of bridge piers for different damage states at various earthquake intensities. A fragility curve typically gives a physical understanding of repair costs and functionally levels of a bridge pier. Shape memory alloys (SMAs) provide a promising alternative material in reducing the failure probability of a bridge pier. This study develops a family of seismic fragility curves for reinforced concrete (RC) wall piers reinforced with three different types of SMA rebar in plastic hinge regions. An incremental dynamic analysis (IDA) using a total of 20 earthquake ground motions was performed on a SMA-RC wall pier to evaluate its seismic performance. The maximum drift recorded for each ground motion was taken as the seismic performance demand parameter of interest in this study. The probabilistic seismic demand model (PSDM) was used to generate fragility curves for each RC-SMA wall pier. The results show that the different mechanical properties and type of SMAs affect the performance of RC-SMA wall pier.

Abstract

The effect of connected structure on SMK Bukit Tinggi building was investigated based on their dynamic behaviour. The structure was constructed with four main buildings are interconnected. Ambient vibration testing (AVT) was used to predict the dynamic response and characteristics by using triaxial 1Hz seismometer and CityShark data logger. The AVT signals were analysed using GEOPSY software for Fourier Amplitude Spectral (FAS). All predominant frequencies (fo) and mode shapes were identified and verified with previous research finding using similar testing approach on the same building, but with different instrument of accelerometer sensor and ARTeMIS software processing tool. Five modes of fo were found from the FAS curves from this research, but only four fo were obtained from ARTeMIS analysis. The highest deviation percentage was indicated at the 5th mode of building frequency at 9.5 %, but 0 to 2.5% to the rest frequencies mode (1st to 4th frequencies mode). Ununiformed buildings response behaviour was believed to contribute, and initiate active progressive shear cracking on the slab panels, columns, beam and at the reentrant corners between laboratory and academic buildings of Building C. It worst when the buildings were struck by several earthquakes’ series. The unsynchronised oscillation between adjacent buildings to Building C had induced couple lateral and torsional deformations. The structural and non-structural damages mostly concentrated at the reentrant corners and mid-span of the building, which identical to the location of maximum deflection amplitudes. In conclusion, strict attention must be emphasized on the fo and mode shapes based on their dynamic response and characteristics analyses, which could be altered by the presence of connected structures.
