COPD patients often present with malnutrition, sarcopenia, and osteoporosis with possible onset of cachexia, with an inadequate dietary intake and a poor quality of life. Moreover, diet plays a pivotal role in patients with COPD through three mechanisms: regulation of carbon dioxide produced/oxygen consumed, inflammation, and oxidative stress. A narrative review based on 99 eligible studies was performed to evaluate current evidence regarding optimum diet therapy for the management of COPD, and then a food pyramid was built accordingly. The food pyramid proposal will serve to guide energy and dietary intake in order to prevent and treat nutritionally related COPD complications and to manage progression and COPD-related symptoms. The nutrition pyramid described in our narrative review is hypothetical, even in light of several limitations of the present review; the main limitation is the fact that to date there are no randomized controlled trials in the literature clearly showing that improved nutrition, via the regulation of carbon dioxide produced/oxygen consumed, inflammation and oxidative stress, improves symptoms and/or progression of COPD. Even if this nutritional pyramid is hypothetical, we hope that it can serve the valuable purpose of helping researchers focus on the often-ignored possible connections between body composition, nutrition, and COPD.Keywords: COPD, nutrients, inflammation, fat free mass, antioxidants, gas exchanges

Secondary outcomes were 3) health-related quality of life, by the ShortForm36 (SF-36); 4) inflammation grade by C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR); and 5) body composition by dual-energy X-ray absorptiometry (DXA) measured at baseline and 30 days after treatment. Data showed significant effects of supplement intake for WOMAC (β=− 3.27, p< 0.0001), Lysholm (β=1.06, p=0.0003), CRP (β=− 0.13, p=0.006), ESR (β=− 3.09, p=0.004), physical activity (β=4.3, p=0.009) and fat-free mass (β=376.7, p=0.046). A significant VAS’s decrease over time was observed in both knees (left: β=− 0.08, p< 0.0001; right: β=− 0.07, p< 0.0001).Conclusion: The tested formulation seems to be effective and also free of side effects.Keywords: pain, knee osteoarthritis, dietary supplements, Phytosome®

Abstract

Background: A study has been performed in overweight and obese subjects to assess the effects of adiposity and inflammation indicators on dysmetabolic biomarkers via red cell distribution width (RDW) and mean corpuscular volume (MCV), taking into account pro-antioxidant balance. Methods: Data from 166 overweight subjects were analyzed by a path analysis model using structural equation modelling (SEM) to evaluate the direct and indirect pathway effects of adiposity, measured by body mass index (BMI) and waist circumference (WC), and inflammation status, measured by pro-antioxidant balance [reactive oxygen species (ROS)], lag-time and slope and C-reactive protein (CRP) values on dysmetabolic biomarkers, via RDW and MCV. Results: BMI was strongly linked to CRP and ROS levels. Moreover, there was a significant negative decrease of MCV (1.546 femtoliters) linked to BMI indirectly via high CRP levels. Furthermore, WC affected RDW, indicating a possible mediatory role for RDW in relation to the relationship between WC and homeostatic model assessment (HOMA), insulin and high density lipoprotein (HDL), respectively. This was evident by the elevated HOMA and insulin levels and the decreased levels of HDL. Finally, ROS-related markers did not affect directly RDW and MCV. Conclusion: The reported outcomes suggest that RDW might play a mediatory role in the relationship between WC and the dysmetabolic outcomes in overweight and obese individuals. CRP seems to modulate the linkage between BMI and MCV. This study provides the backbone structure for future scenarios and lays the foundation for further research on the role of RDW and MCV as suitable biomarkers for the assessment of cardiovascular disease (HDL-cholesterol), inflammatory bowels and insulin resistance.

Abstract

We compare bioimpedance analysis (BIA) with dual-energy X-ray absorptiometry (DXA) in the assessment of free fat mass (FFM), fat mass (FM) and percentage of body fat under different conditions in relation to age categories, hydration parameters, body mass index (BMI) and sarcopenia. A cross-sectional analysis of body composition was estimated by BIA and DXA in 379 hospitalized elderly patients. In addition, estimates of FFM, FM and percentage of body fat were investigated across different conditions. Paired t-tests, Bland-Altman plot and intraclass correlation coefficient analysis were used to compare methods. Data showed an underestimation of means (BIA versus DXA) of FFM (women: 0,97 kg, p0,01; men: 1,99 kg; p0,01), and an overestimation of both the FM (women: +1,11 kg; p0,01; men: +1,67 kg; p0,01) and percentage of body fat (women: +2,07 %, p0,01; men: +2,82 %, p0,01). BIA underestimated FFM and overestimated FM and percentage of body fat in patients from the age group of 75 to 85 years, in patients with a total body water content60%, in underweight and normal weight patients and in patients with sarcopenia (p0,01). The intraclass coefficient results were indicative of poor reproducibility between BIA and DXA for FFM (women: +0,197; men: +0,250) and FM (women: +0,141; men +0,144). BIA is a good alternative for estimation of FFM and FM only in overweight or obese patients or in patients with good hydration status. BIA, on the other hand, is not an accurate method for assessing FFM in sarcopenic patients.Мы сравнили биоимпедансный метод диагностики с двухэнергетической рентгеновской абсорбциометрией (ДЭХА) в оценке массы свободного жира, массы жира и доли жира в организме в различных условиях в зависимости от возрастных категорий, параметров гидратации, ИМТ и саркопении. Кросс-секционный анализ состава тела был оценен с помощью биоимпедансного метода и ДЭХА у 379 госпитализированных пациентов пожилого возраста. Для сравнения методов использовали парные t-критерии, график Бланда—Альтмана и коэффициент внутригрупповой корреляции. Данные показали недооценку значений (биоимпедансный метод диагностики по сравнению с ДЭХА) массы свободного жира (женщины: 0,97 кг, р0,01; мужчины: 1,99 кг; р0,01), а также завышенную оценку обоих показателей массы жира (женщины: +1,11 кг; р0,01; мужчины: +1,67 кг; р0,01) и процентного содержания телесного жира (женщины: +2,07%, р0,01; мужчины: +2,82%, р0,01). Биоимпедансный метод диагностики занизил показатели массы свободного жира и завысил показатели массы жира, а также процентное содержание жира в организме у пациентов 75–85 лет, у пациентов с общим содержанием воды в организме60%, у пациентов с недостаточной массой тела и нормальной массой, а также у пациентов с саркопенией (р0,01). Результаты внутриклассового коэффициента свидетельствуют о плохой воспроизводимости между биоимпедансным методом диагностики и ДЭХА для массы свободного жира (женщины: +0,197; мужчины: +0,250) и массы жира (женщины: +0,141; мужчины +0,144). С одной стороны, биоимпедансный метод является хорошей альтернативой для оценки массы свободного жира и массы жира только у пациентов с избыточной массой тела или ожирением, а также у пациентов с хорошим гидратационным статусом. С другой стороны, биоимпедансный метод диагностики не является точным методом оценки массы свободного жира у пациентов с саркопенией.

Abstract

A novel subtype of Neurodegeneration with Brain Iron Accumulation (NBIA) recently has been described: mitochondrial membrane protein-associated neurodegeneration (MPAN), caused by mutations of c19orf12 gene. We present phenotypic data and results of screening of C19orf12 in five unrelated NBIA families. Our data led to identify novel pathogenic mutations in C19orf12.

26. Short-term bone level observations associated with platform switching in immediately placed and restored single maxillary implants: a preliminary report.

Abstract

PURPOSE: The aim of this study was to evaluate the short-term bone level response around immediately placed and provisionally restored implants using a platform switching concept. MATERIALS AND METHODS: Twenty-two implants with a platform diameter of 5.5 mm were immediately placed in healthy maxillary sites in 22 patients. Resultant circumimplant spaces were filled with a mixture of bovine bone matrix and collagen. The implants were randomly divided into two equal groups: 11 implants connected with 3.8-mm-diameter abutments (test group) and 11 with 5.5-mm-diameter ones (control). Provisional crowns were adapted and adjusted for nonfunctional immediate placement on each implant and the final crowns were constructed 2 months later. Posttreatment assessments were carried out by an independent trained observer at the time of implant placement (baseline), at definitive prosthesis insertion, and every 6 months thereafter. These assessments included periapical radiographs, pocket probing depths (PPD), bleeding on probing (BOP), and modified Plaque Index (mPII) on both implants and first proximal teeth. An image analysis software application was used to compare the bone crestal heights at the mesial and distal aspects of the implants. RESULTS: The mean follow-up observation period was 25 months and all implants were judged to be successfully osseointegrated. In the test group, radiographic analysis showed an average bone reduction level of 0.30 mm (SD = 0.16 mm). This mean value was statistically significantly different (P < or = .005) from the average reduction in the control group (mean = 1.19 mm, SD = 0.35 mm). No differences between the two groups in PPD, BOP, or mPII were found. CONCLUSION: This preliminary study suggests that immediate single implant restorations in specific maxillary sites with subsequent platform switching may provide peri-implant alveolar bone-level stability. Abstract

In ottemperanza all’art. 280 del D.Lgs. 81/08 e s.m.i. l’ISPESL e l’Organo di Vigilanza territorialmente competente, rappresentano i destinatari dei Registri degli Esposti ad Agenti Biologici negli Ambienti di Lavoro: secondo la Normativa vigente,tali Registri vanno compilati dal datore di lavoro, con l’aiuto del Medico Competente, qualora le attività lavorative che si svolgono presso la propria azienda prevedano il contatto con microrganismi di Gruppo 3 e 4, secondo la classificazione degli agenti biologici operata dall’Art. 268 del D.Lgs. 81/08. I dati illustrati in questo studio sono stati rilevati dagli archivi dei registri pervenuti all’Unità Operativa Controllo e Vigilanza Sanitaria dello S.Pre.S.A.L. di Palermo negli anni 2007-2010. La rilevante esiguità del numero di registri pervenuti nel periodo di riferimento fa riflettere sulla notevole sottostima del numero di lavoratori esposti ad agenti biologici nel territorio della Provincia di Palermo, e offre uno spunto per intraprendere dei percorsi formativi da rivolgere ai Datori di Lavoro al fine di sensibilizzarli all’osservanza di tale adempimento.

Abstract

Characterizing the cytoarchitecture of mammalian central nervous system on a brain-wide scale is becoming a compelling need in neuroscience. For example, realistic modeling of brain activity requires the definition of quantitative features of large neuronal populations in the whole brain. Quantitative anatomical maps will also be crucial to classify the cytoarchtitectonic abnormalities associated with neuronal pathologies in a high reproducible and reliable manner. In this paper, we apply recent advances in optical microscopy and image analysis to characterize the spatial distribution of Purkinje cells (PCs) across the whole cerebellum. Light sheet microscopy was used to image with micron-scale resolution a fixed and cleared cerebellum of an L7-GFP transgenic mouse, in which all PCs are fluorescently labeled. A fast and scalable algorithm for fully automated cell identification was applied on the image to extract the position of all the fluorescent PCs. This vectorized representation of the cell population allows a thorough characterization of the complex three-dimensional distribution of the neurons, highlighting the presence of gaps inside the lamellar organization of PCs, whose density is believed to play a significant role in autism spectrum disorders. Furthermore, clustering analysis of the localized somata permits dividing the whole cerebellum in groups of PCs with high spatial correlation, suggesting new possibilities of anatomical partition. The quantitative approach presented here can be extended to study the distribution of different types of cell in many brain regions and across the whole encephalon, providing a robust base for building realistic computational models of the brain, and for unbiased morphological tissue screening in presence of pathologies and/or drug treatments.
