Two reviewers independently assessed the methodological quality of the articles using specific study design-related quality assessment forms.ResultsTwenty-nine methodologically acceptable studies were selected. A total of 2611 short implants (lengths 5-9.5 mm) were analysed. An increase in implant length was associated with an increase in implant survival (from 93.1% to 98.6%). Heterogeneity between studies was explored by subgroup analyses. The cumulative estimated failure rate of studies performed in the maxilla was 0.010 implants/year, compared with 0.003 found in the studies in the mandible. For studies that also included smokers, the failure rate was 0.008 compared with 0.004 found in studies that excluded smokers. To assess in a randomized-clinical trial the influence of three augmentation techniques (chinbone with or without a Bio-Gide((R)) membrane and Bio-Oss((R)) with a Bio-Gide((R)) membrane) on the clinical and radiographic characteristics of hard and soft tissues around implants and adjacent teeth in the reconstructed maxillary anterior region, up to 1 year after functional loading.Ninety-three patients requesting single-tooth replacement and presenting with a horizontal (bucco-palatinal) bone deficiency were included. After augmentation, 93 ITI-Esthetic(Plus) implants were placed. Clinical variables, standardized photographs and radiographs were analysed to assess the impact on the levels of the marginal gingiva (MGL) and marginal bone (MBL) around implants and adjacent teeth, viz at pre-augmentation, pre-implantation (TPI) and 1 (T(1)) and 12 (T(12)) months after final crown placement.Implant survival was 97.8%. No significant differences were observed in the treatment outcomes of the three augmentation modalities. Combining the three modalities, a slight but significant increase in the implants approximal pocket depth was found between T(1) and T(12). Approximal bone loss at the implant between T(1) and T(12) was 0.14 +/- 0.76 mm (mesial) and 0.14 +/- 0.47 mm (distal); the approximal MGL slightly increased (mesial: 0.24 +/- 0.46 mm, distal: 0.25 +/- 0.66 mm), and the buccal MGL decreased (0.11 +/- 0.61 mm). Bone loss at the adjacent teeth, although minor, was significant between TPI and T(1). No correlations were observed in changes of MBL and MGL.None of the three applied augmentation technique procedures influenced the characteristics of the MGL and MBL or the implant survival of single-tooth replacements. Purpose: The aim of this study was to compare the crestal incision with the labial flap design when inserting a 2-stage implant system in a nonsubmerged manner. Patients and Methods: Ten consecutive edentulous patients with a severely resorbed mandible (Cawood Class V to VI) that resulted in reduced stability and insufficient retention of the lower denture were included. In all patients, 2 IMZ implants were inserted in a 1-stage procedure in the mandibular canine regions as part of an implant overdenture treatment. In 5 patients, the labial nap approach was used, and a crestal incision approach was used in the other 5 patients. Standardized evaluations were performed at 2, 6, and 12 weeks after implant placement and 12 months after placement of the new prosthesis. Results: In the first postoperative weeks, more hyperplasia occurred around the implants inserted by the crestal incision than the labial flap. However, after 1 year of function, no striking differences between the 2 groups were present with regard to the clinical and radiographic parameters. Conclusions: From this pilot study, it was concluded that both the crestal incision and the labial flap approach are reliable procedures for insertion of IMZ implants in a 1-stage procedure. However, because of the smaller risk of soft tissue overgrowth, there is a preference for the labial flap approach. (C) 2000 American Association of Oral and Maxillofacial Surgeons.

Purpose The aim was to evaluate the outcome of a short wide-body implant in the atrophic posterior jaw without a grafting procedure. Materials and Methods Patients treated with a tapered wide-body implant measuring 8 to 9 mm in width and 7 to 9 mm in length (Max implant; Southern Implants, Irene, South Africa) were recalled to scrutinize implant survival. Preoperative cone beam computed tomography images were analyzed to measure bone height in reference to the mandibular canal and sinus floor. Results There were 57 implants inserted in 18 men and 24 women after a 2-stage procedure and delayed loading. The mean follow-up was 15 months (SD, 10; range, 1-32 months), with 63.2% of the implants having at least 1 year of follow-up and 26.3% having at least 2 years' follow-up. Forty-six implants were inserted in the posterior maxilla and eleven in the mandible. Fifteen were placed in an extraction socket and forty-two in healed bone. Thirteen implants were supporting a single crown. Two implants failed, resulting in a survival rate of 96.5%, with rates of 90.9% and 97.8% for mandible and maxilla, respectively. This was not affected by gender, jaw, immediate or delayed placement, implant diameter and length, or the use of a bone substitute. The mean preoperative bone height was 7.21 mm in maxilla and 8.76 mm in mandible. In 41 cases implant length surpassed available bone height. Conclusions Despite the compromised bone condition and height, the survival rate of 96.5% is comparable to normal implants and, therefore, placing a wide-body implant may be an alternative to avoid grafting procedures. This is probably related to the enlarged implant surface area and the good primary stability.
