Abstract

Introduction: We described a rare clinical case of osteoma associated with homolateral radicular cyst of the maxillary sinus. Observation: Imaging showed two different lesions in the right maxillary sinus. We performed a combined surgical approach to completely remove the lesions and used a plasma rich in growth factors membrane to repair dental roots. No relapse after a 2 years follow-up was observed. Commentaries: The simultaneous presence of two large lesions, a 23,7 mm osteoma and a 33,7 mm radicular cyst in the same maxillary sinus, has been rarely described in the literature. Although nasal endoscopy and imaging are mandatory to assess the diagnosis, the definitive diagnosis was obtained after histopathologic examination. A combined surgical approach allowed a complete removal of both lesions, ensuring, at same time, an optimal surgical field. Plasma rich in growth factors membrane due to its cohesive properties was particularly useful in improving bone neoformation and periodontal regeneration. Conclusion: Diagnostic assessment of maxillary lesions requires nasal endoscopy, imaging and histopathological examination. If these lesions are symptomatic, they should be completely removed and minimally invasive surgery is indicated. Plasma rich in growth factors membrane ensures a good postoperative recovery.

10. Unusual Localization of an Emergent Bacterium, Raoultella ornithinolytica

Abstract

Raoultella ornithinolytica is a bacterium belonging to the family Enterobacteriacae. It is a rare but emergent cause of human pathologies especially in immunocompromised patients. We described the first case in the literature of isolated external otitis sustained by Raoultella ornithinolytica in an immunocompetent host. A 54-year-old Caucasian man with a history of previous myringoplasty came to our attention reporting otalgia and otorrhea. We performed right ear swab for culture examination, meanwhile we started empirical therapy with topic administration of neomycin, without any clinical improvement. The cultural examination showed the presence of a Raoultella ornithinolytica infection. After ten days of treatment with oral ciprofloxacin and topic levofloxacin, there was the complete resolution of pain and inflammation. Raoultella ornithinolytica must be taken into consideration as an emergent cause of human infection, also in case of external otitis. Infection can be severe and can occur both in immunocompromised and in immunocompetent hosts. Culture test is mandatory to choose the proper therapy and avoid potential severe complications.

Abstract

L’osteoma osteoide è una lesione osteogenica benigna la cui localizzazione nel seno mascellare è piuttosto rara, appena 7 casi descritti negli ultimi 15 anni. Di più frequente riscontro, sono le cisti odontogene del mascellare. Il riscontro contestuale delle due patologie è un evento piuttosto raro. Descriviamo il caso di osteoma del mascellare associato a voluminosa cisti odontogena omolaterale a partenza dalla radice dell’11mo. La paziente (42a), giungeva alla nostra osservazione per ostruzione respiratoria nasale, cacosmia, rinorrea mucopurulenta ed epifora monolaterali. L’imaging evidenziava la presenza di entrambe le lesioni che occupavano interamente il seno mascellare destro. La paziente veniva indirizzata a chirurgia e si optava per un approccio endoscopico combinato con doppio accesso transorale e transnasale con exeresi delle lesioni e ricostruzione del pavimento del mascellare con ausilio di membrana con PRGF (plasma ricco di fattori di crescita). Nessuna complicanza peri e post-operatoria è stata osservata. Il caso in esame pone l’attenzione sull’approccio multidisciplinare alla patologia e sulla scelta di una tecnica chirurgica conservativa con utilizzo di PRGF.

Abstract

Myxomas are benign lesions which can arise in any anatomic location but are mainly found in the heart. Exceptionally they interest the head and neck district, where the most frequently involved sites are the mandible and the maxilla. There are only few described cases of myxomas of the larynx. Here we report a new case of a laryngeal myxoma of the left true vocal fold, the second occurred in a female patient, presenting with severe dyspnea, dysphonia and dysphagia. We describe this case for two reasons. First is because it presents intubation which was safely treated through careful planning. Ameticulous treatment planning by surgeon and anaesthetist was made mutually carried out prior to microlaryngoscopic excision in case di large vocal cord polyps causing severe airway obstruction. The second reason is because a laryngeal myxoma is a very rare lesion. We provide a review of the literature, focusing on possible risk factors, and discuss the differential diagnosis and the best treatment options in laryngeal myxoma.

23. Skull Base Fungal Osteomyelitis: A Case Report and Review of the Literature

Abstract

Skull base osteomyelitis (SBO) is an invasive infection refractory to therapy, closely linked with malignant otitis externa (MOE). It is characterized by a mild clinical presentation that can delay cross-sectional imaging considered as the key to revealing it. Skull base osteomyelitis typically affects elderly diabetics and immunocompromised patients (>70 years). It most commonly has an otogenic origin due to an extension of MOE. The prognosis can be very poor without the administration of adequate and timely therapy at an early disease stage. Nowadays, Pseudomonas aeruginosaremains the most common pathogen associated with SBO. Fungi are a rare cause of MOE. This report documents a rare case of otogenic SBO caused by Candida parapsilosisin a diabetic patient, with persistent otologic symptoms as clinical onset and resistance to medical treatment. Fungal MOE has more subtle symptoms and is more aggressive than its bacterial counterpart. When MOE is resistant to antibacterial drugs, this should raise the suspicion of a fungal etiology of MOE. The current guidelines do not exhaustively describe the diagnosis, antifungal drugs of choice, and optimum duration of treatment. The description of these rare clinical cases should help with the multidisciplinary management of this disease in order to optimize the diagnosis and therapeutic protocol.

Abstract

Although individuals categorize odors according to their pleasantness, experience may also influence odor perception-a phenomenon that partially explains why different populations perceive odors differently. Italy, which comprises 20 regions, is characterized by very different cultures. In the present study, we investigated for the first time how Italian regional differences can affect odor perception. 254 healthy volunteers coming from northern, central, southern Italy, and Sicily, one of the two major Italian islands, were recruited in Padua, Rome, Naples, and Syracuse, respectively. Olfactory function was tested with Sniffin' Sticks identification subtest. Subjects who had a score in the range within the mean identification value +/- 1 SD, in accordance with the age classes identified in the literature, were asked to judge the odor pleasantness of 20 substances. The hedonic tone of the odorants was categorized as pleasant, neutral, unpleasant, and very unpleasant. Some odorants were appreciated more in northern Italy than in the other parts of the country, whereas others were appreciated more in the south and in Sicily than in the north. Unpleasant odorants were judged less unpleasant in central Italy. Some odorants such as strawberry and vanilla were perceived similarly in all the regional areas. Our study indicates that in Italy, hedonic perception of odorants differs probably in relation with genetic, cultural, and environmental factors. Further investigation is needed to delve deeper into the factors that influence the quality odor perception amongst humans., Ačkoliv jednotlivci rozdělují pachy dle jejich libosti, zkušenost může také ovlivňovat vnímání pachů. Tak lze vysvětlit, proč různé populace různě vnímají pachy a vůně. Itálie, která se skládá z 20 regionů, je charakterizována velmi rozdílnými kulturami. Ve studii jsme zjišťovali, jak v různých italských regionech posuzují rozdíly v percepci pachů. 254 zdravých osob ze severní, centrální, jižní Itálie, a Sicílie bylo vyšetřeno v Padově, Římě, Neapoli a Syrakusách. Čich byl vyšetřen testem Sniffin´ Sticks (částí Identifikace). Osoby, které měly výsledek v rozsahu ± 1 SD jejich věkové kategorie, byly požádány, aby posuzovaly libost 20 pachů. Hedonický charakter pachů měly osoby zařadit do kategorií voní, neutrální, zapáchá, a výrazně zapáchá. Některé látky byly hodnoceny libě především v severní Itálii, jiné spíše v jižní. Nepříjemné látky byly posuzovány jako méně zapáchající ve střední Itálii. Některé z látek, jako jahoda a vanilka, byly obdobně vnímány ve všech regionech. Na základě naší studie se domníváme, že hédonická percepce se liší pravděpodobně ve vztahu ke genetickým, kulturním a environmentálním faktorům. Další studie jsou potřeba k objasnění faktorů, které přispívají k hodnocení kvality pachů a vůní.

Abstract

Recently, an increasing number of reports demonstrated that the bitter taste G-protein coupled receptors family T2R, expressed in ciliated epithelial cells of the respiratory tract, are able to detect bacterial products and to stimulate innate immune defense against pathogens. Most microbial agents, secretes chemical signals known as quorum-sensing molecules that regulate the expression of genes involved in biofilm formation, virulence and other vital processes for microorganisms. Among the quorum-sensing molecules, the AHLs produced by P. aeruginosa, activate the receptor for bitter T2R38 expressed in ciliated epithelial cells of the respiratory tract, whereas mutants of P. aeruginosa lacking the AHL are not able to activate it. The activation of the receptor results in an increase of the Ca2+ flow and the ciliary beat frequency, as well as stimulating the production of NO which acts as a bactericide against the pathogen. The Caucasian population express three common polymorphisms (Pro49Ala, Ala262Val, Val296Ile) for TAS2R38 that lead to two major haplotypes PAV and AVI. The expression of either haplotype gives respectively 2 forms of receptors ̶ functional or non-functional ̶ i.e. unable to respond to specific agonists such phenylthiocarbamide and propylthiouracil (PROP). The two haplotypes PAV and AVI segregate into two major phenotypic classes: the "functional", sensitive to bitter, are homo- or heterozygous for the allele PAV, the "non-functional", are homozygous for the allele AVI. The genetic variations of the receptor TAS2R38 that affect sensitivity to bitter taste can help determine individual differences in susceptibility to bacterial infections of the respiratory tract allowing to plan a “target therapy”. Cellular cultures from homozygous PAV/PAV individuals showed a more effective NO production, mucociliary clearance and bactericide effect than cultures from AVI/PAV or AVI/AVI individuals. As a consequence it is reasonable to assume that patients with genotype AVI/PAV or AVI/AVI are at greater risk of contracting infections from gram-negative, compared with homozygous PAV. Some authors have studied the correlation between genotype and microbiological results TAS2R38 tissue of respiratory mucosa. The result of this analysis proved to be very interesting, because it showed a significant difference in the frequency of non-functional (AVI) than functional (PAV) among patients whose cultures were positive for Gram-negative bacteria, including P. aeruginosa. The aim of the study was to characterize phenotypically the sensitivity to PROP and the receptor polymorphisms of TAS2R38, in patients with chronic or recurrent infections of the upper respiratory tract to identify high risk patients. The identification of high-risk individuals would allow to draw up protocols for specific follow-up and appropriate “target therapy”.

Background: Cystic fibrosis (CF) is a multisystem disease that involves the upper airways with chronic rhinosinusitis (CRS) causing nasal congestion, rhinorrhea, mouth breathing, facial pain, and olfactory dysfunction. Twelve percent to 71% of CF patients report smelling alterations with an impact on nutrition and quality of life. Objectives: The goal was to study olfaction performance in CF patients with CRS that worsens quality of life. Methods: A total of 121 subjects were enrolled in this study. Seventy-one had CF and underwent ear, nose, and throat evaluation with nasal endoscopy, sinonasal outcome test 22 (SNOT-22), visual analog scale (VAS), and "Sniffin' Sticks." Fifty subjects were age-matched with healthy controls. Results: All 71 CF patients were affected by CRS; 59 of 71 (83.1%) had CRS without nasal polyps and 12 of 71 (16.9%) had CRS with early nasal polyps. None of the 50 controls had CRS. Total SNOTT-22 mean values in the 71 CF patients were 38.10 ± 21.08 points. If considering only the 59 CF patients without nasal polyps, the SNOTT-22 mean value was 36.76 ± 21.52 points. Moreover, based on the VAS scores, the degree of nasal symptoms was classified as mild for facial pain, smell alteration, nasal discharge, and sneezing and resulted in moderate symptoms for nasal blockage and headache. Among the CF patients, 55 of 71 (76.5%) declared to be normosmic, while the smelling ability assessed by "Sniffin' Sticks" showed that only 4 of 71 (5.63%) were normosmic, 58 (81.69%) were hyposmic, and 9 (12.68%) were anosmic. In the controls, 41(82%) were normosmic, 9 (18%) were hyposmic, and none were reported to be anosmic (P <.001). Conclusions: We confirm that most CF patients have a relevant olfactory impairment, although only a low percentage declares such alteration.
