Aim. To describe demographic and clinical laboratory characteristics, concomitant diseases and drug therapy of outpatients with heart failure (HF) in the Russian Federation.Material and methods. An interim analysis of a prospective observational multicenter registry study of patients with chronic heart failure in the Russian Federation ("PRIORITET-CHF") was performed. The study included outpatients with HF followed by a general practitioner or cardiologist.Results. Data from 6255 patients were analyzed (31,3% of the study sample; median age, 65 years; men, 65%). HF with reduced ejection fraction (HFrEF) was diagnosed in 42,4%, HF with preserved EF — in 31,9%. In addition, 57,4% of patients were characterized by NYHA class II HF. The most common causes of HF were hypertension, coronary artery disease, and atrial fibrillation or flutter. Of the concomitant diseases, doctors most often reported chronic kidney disease (CKD) (43,2%), obesity (37,8%) and diabetes (26,7%) in HF.Conclusion. In the Russian Federation, among outpatients with HF, there was domination of men, HFrEF phenotype, NYHA class II. The relatively young mean age and frequent associations of HF with cardiovascular risk factors and diseases underscore the importance of timely prevention initiatives. The identified high proportion of patients with CKD requires special attention and separate analysis. Despite the relatively high prescription rate of certain classes of diseasemodifying therapy for HF, the prescription of optimal quadruple therapy and electrophysiological treatments for HFrEF is insufficient.

Heart failure (HF) is a global health problem. Despite advances in the development of effective treatments for patients with heart failure, morbidity and mortality from remain high, and the prognosis is poor. However, there is potential to improve outcomes in HF patients with current disease-modifying therapy. Planning needs and resources, assessing the effectiveness of care for HF patients in clinical practice requires high-quality epidemiological data. Previously performed Russian observational studies of HF were characterized by relatively small sample sizes, inclusion of patients in only one or a few regions, strict selection criteria, single-stage design, or short follow-up. The rationale and design of the all-Russian prospective observational multicenter registry study "PRIORITET-CHF", which included 20000 patients with HF throughout the Russian Federation, is presented. The main aims of the study are to (1) describe baseline clinical and demographic characteristics in outpatients with HF in the Russian Federation and (2) characterize routine therapy and evaluate the compliance of treatment for HF with reduced ejection fraction with current clinical guidelines.

Aim. To analyze newly diagnosed diseases and features of the post-COVID course in patients after a coronavirus disease 2019 (COVID-19) within 12-month follow-up.Material and methods. A total of 9364 consecutively hospitalized patients were included in the ACTIV registry. Enrollment of patients began on June 29, 2020, and was completed on March 30, 2021, corresponding to the first and second waves of the pandemic. According to the ACTIV registry, 18,1% of patients after COVID-19 had newly diagnosed diseases (NDDs) over the next 12 months. Hypertension (HTN), type 2 diabetes and coronary artery disease (CAD) prevailed in the NDD structure. Comparison of the age-standardized incidence of NDDs (HTN, CAD, diabetes) in the post-COVID period in the ACTIV registry with NDD incidence in 2019 according to Rosstat and the expected incidence of NDDs according to the EPOHA study revealed that HTN, diabetes, CAD in patients after COVID-19 were registered more often as follows: HTN by 7,0 and 4,4 times, diabetes by 7,3 and 8,8 times, CAD by 2,3 and 2,9 times, respectively. NDDs most often developed in patients aged 47 to 70 years. Aim. To investigate on post-COVID period in patients of the Eurasian region.Material and methods. A total of 9364 consecutively hospitalized patients were included in ACTIV registry. Enrollment of patients began on June 29, 2020, and was completed on March 30, 2021, corresponding to the first and second waves of the pandemic. Demographic, clinical, and laboratory data, computed tomography (CT) results, information about inhospital clinical course and complications of COVID-19 during hospitalization were extracted from electronic health records using a standardized data collection form. The design included follow-up telephone interviews with a standard questionnaire at 3, 6, and 12 months to examine the course of post-COVID period.Results. According to ACTIV register, 63% of patients after COVID-19 had new adverse symptoms or exacerbations of the existing symptoms lasting for up to 1 year. After hospital discharge, 79,8% of patients sought unscheduled medical attention in the first 3 months, 79,1% at 4-6 months, and 64,8% at 7-12 months. Readmission rate was 11,8% in the first 3 months, 10,9% at 4-6 months, and 10,1% at 7-12 months. The most common reasons for unscheduled treatment in the first 3 months were uncontrolled hypertension, decompensated type 2 diabetes, destabilization of coronary artery disease, gastrointestinal disease, AF episodes, exacerbation of asthma and chronic obstructive pulmonary disease, decompensated heart failure (HF). The 12-month mortality of COVID-19 survivors after the discharge was 3,08%. Multivariate analysis showed that independent risk factors for fatal outcome were age (direct correlation), the levels of hemoglobin (inverse correlation), oxygen saturation (inverse correlation), and aspartate aminotransferase (direct correlation), as well as class III-IV HF, prior stroke, cancer, inhospital acute kidney injury. Based on these identified risk factors, a nomogram was constructed to determine the 3-month mortality risk after discharge.Conclusion. Analysis of ACTIV register showed that end of the acute phase of COVID-19 does not imply a complete recovery.

Aim. To study the lipid profile in hospitalized patients with coronavirus disease 2019 (COVID-19) depending on the outcome of its acute phase according to the AKTIV international registry.Material and methods. The AKTIV registry included men and women over 18 years of age with a diagnosis of COVID-19, who were treated in a hospital. A total of 9364 patients were included in the registry, of which 623 patients were analyzed for levels of total cholesterol, low-density lipoprotein cholesterol (LDL-C) and triglycerides on days 1-2 of hospitalization. The level of high-density lipoprotein cholesterol (HDL-C) was calculated using the Friedewald equation.Results. We found that a decrease in LDL-C level was significantly associated with an unfavorable prognosis for hospitalized patients with COVID-19. This pattern persisted in both univariate and multivariate analyses. LDL-C levels in the final multivariate model had a significant relationship with the prognosis (an increase in the death risk by 1,7 times with a decrease per 1 mmol/l). Aim. To study the survival of patients with New York Heart Association (NYHA) class IIIV heart failure (HF) after a coronavirus disease 2019 (COVID-19) and compare it with a group of similar patients without COVID-19, both during and before the pandemic.Material and methods. A sample of 810 patients with HF of any class was created, who were treated in the HF center at the City Clinical Hospital № 38 in Nizhny Novgorod for the period from March 2019 to March 2020. Patients with NYHA class HF III-IV (n=263) were selected from this sample. The diagnosis was confirmed by echocardiography and chest radiography. At the time of discharge, the 6-minute walk test was 300 pg/ml. The group was analyzed from March 2019 to December 2021 with the definition of all-cause mortality depending on prior COVID-19.Results. During 520-day follow-up, 30,8% of NYHA class III-IV patients died, while 16,0% of patients underwent COVID-19, of which 79,25% of patients died during this period. Aim. To carry out comparative analysis of echocardiographic and electrocardiographic (ECG) data of survivors and deceased patients with COVID-19 (sub-analysis of the international register “Dynamics analysis of comorbidities in SARS-CoV-2 survivors”).Material and methods. The study presents the results of a sub-analysis of the international AKTIV registry, which was called AKTIV CARDIO. Data were collected from 9 medical centers in the Russian Federation. AKTIV CARDIO included 973 hospitalized patients, of which 50 patients died during hospitalization.Results. Comparative analysis of echocardiographic parameters revealed that 4 parameters differed in deceased patients compared to survivors: left ventricular ejection fraction (LVEF), right ventricular end diastolic dimension (RV EDD), right atrial (RA) short axis diameter and pulmonary artery systolic pressure (PASP). RA short axis diameter was higher in deceased patients compared with survivors (38,0 [36,0; 39,0] versus 35,0 [33,0; 38,0] mm, p=0,011). RV EDD was higher in deceased patients than in survivors (3,0 [29,0; 33,0] vs 28,0 [27,0; 32,0] mm, p=0,019). An online expert meeting held on November 17, 2021 reviewed the results of the randomized, double-blind, multinational, parallel-group EMPULSE trial, evaluating the clinical benefit and safety of the sodium-glucose co-transporter-2 inhibitor (SGLT2i) empagliflozin compared with placebo in patients hospitalized with acute decompensated heart failure (ADHF). Patients were included in the study regardless of ejection fraction (EF) and the presence of diabetes and randomized during hospitalization after stabilization. In addition, the EMPULSE trial used a composite result analyzed using a stratified benefit ratio — Win Ratio analysis. There is evidence of clinical benefit of empagliflozin in hospitalized patients with preserved and reduced LVEF, as well as in patients with newly diagnosed ADHF or with acute decompensation of chronic heart failure (CHF) compared with placebo, regardless of type 2 diabetes presence. The importance of the favorable results of the EMPULSE trial and its significance for clinical practice, which implies the early administration of empagliflozin for inpatients, is noted. A number of proposals have been adopted to accelerate the introduction of empagliflozin into clinical practice for patients with ADHF.

At an online meeting of experts held on May 14, 2021 additional research results on a sodium-glucose co-transporter-2 (SGLT2) inhibitor empagliflozin in patients with heart failure with reduced ejection fraction were considered. According to the data from the EMPEROR-Reduced international study, cardiovascular and renal effects of empagliflozin therapy in patients with and without type 2 diabetes (T2D) were analyzed. A number of proposals and recommendations was accepted regarding the further study of cardiovascular and renal effects of empagliflozin and its use in clinical practice in patients with heart failure, regardless of the T2D presence.

The agreement of experts of the Eurasian Association of Therapists (EAT) discusses pathogenesis and treatment of COVID-19. Modern data on the characteristics of cardiovascular, kidney, respiratory damage in SARS-infected CoV-2 are presented. The tactics of managing patients initially having cardiovascular diseases, diabetes mellitus, chronic obstructive pulmonary disease, bronchial asthma, chronic kidney disease are discussed in detail. The article presents data on drug interaction of drugs.

Abstract We describe how to reconstruct generalized scalar–tensor gravity (GSTG) theory, which admits exact solutions for a physical type of potentials. Our consideration deals with cosmological inflationary models based on GSTG with non-minimal coupling of a (non-canonical) scalar field to the Ricci scalar. The basis of proposed approach to the analysis of these models is an a priori specified relation between the Hubble parameter H and a function of a non-minimal coupling $$F=1+\delta F$$ F=1+δF , $$H\propto \sqrt{F}$$ H∝F . Deviations from Einstein gravity $$\delta F$$ δF induce corresponding deviations of the potential $$\delta V$$ δV from a constant value and modify the dynamics from a pure de Sitter exponential expansion. We analyze the models with exponential power-law evolution of the scale factor and we find the equations of influence of non-minimal coupling, choosing it in the special form, on the potential and kinetic energies. Such a consideration allows us to substitute the physical potential into the obtained equations and then to calculate the non-minimal coupling function and kinetic term that define the GSTG parameters. With this method, we reconstruct GSTG for the polynomial, exponential, Higgs, Higgs–Starobinsky and Coleman–Weinberg potentials. Special attention we pay to parameters of cosmological perturbations and prove the correspondence of the obtained solutions to observational data from Planck.

Aim. To study the clinical course specifics of coronavirus disease 2019 (COVID-19) and comorbid conditions in COVID-19 survivors 3, 6, 12 months after recovery in the Eurasian region according to the AKTIV register. Material and methods.The AKTIV register was created at the initiative of the Eurasian Association of Therapists. The AKTIV register is divided into 2 parts: AKTIV 1 and AKTIV 2. The AKTIV 1 register currently includes 6300 patients, while in AKTIV 2 — 2770. Patients diagnosed with COVID-19 receiving in- and outpatient treatment have been anonymously included on the registry. The following 7 countries participated in the register: Russian Federation, Republic of Armenia, Republic of Belarus, Republic of Kazakhstan, Kyrgyz Republic, Republic of Moldova, Republic of Uzbekistan. This closed multicenter register with two nonoverlapping branches (in- and outpatient branch) provides 6 visits: 3 in-person visits during the acute period and 3 telephone calls after 3, 6, 12 months. Subject recruitment lasted from June 29, 2020 to October 29, 2020. Register will end on October 29, 2022. A total of 9 fragmentary analyzes of the registry data are planned. This fragment of the study presents the results of the post-hospitalization period in COVID-19 survivors after 3 and 6 months. Results. According to the AKTIV register, patients after COVID-19 are characterized by long-term persistent symptoms and frequent seeking for unscheduled medical care, including rehospitalizations. The most common causes of unplanned medical care are uncontrolled hypertension (HTN) and chronic coronary artery disease (CAD) and/or decompensated type 2 diabetes (T2D). During 3- and 6-month follow-up after hospitalization, 5,6% and 6,4% of patients were diagnosed with other diseases, which were more often presented by HTN, T2D, and CAD. The mortality rate of patients in the post-hospitalization period was 1,9% in the first 3 months and 0,2% for 4-6 months. The highest mortality rate was observed in the first 3 months in the group of patients with class II-IV heart failure, as well as in patients with cardiovascular diseases and cancer. In the pattern of death causes in the post-hospitalization period, following cardiovascular causes prevailed (31,8%): acute coronary syndrome, stroke, acute heart failure. Conclusion. According to the AKTIV register, the health status of patients after COVID-19 in a serious challenge for healthcare system, which requires planning adequate health system capacity to provide care to patients with COVID-19 in both acute and post-hospitalization period.

The extent of the problem of heart failure determines the priority of the intention of developing and implementing an optimal model of medical care for this group of patients. The article describes the key components of the heart failure medical care (continuity, patient routing, educational activities) and provides examples of documents, protocols, checklists that can be used in real clinical practice by specialists in medical organizations in the regions of the Russian Federation.

An online meeting of experts held on November 6, 2020 describes the results of EMPA-REG OUTCOME and EMPEROR-Reduced trials on sodium-glucose co-transporter-2 (SGLT2) inhibitor empagliflozin. We analyzed cardiovascular and renal outcomes in patients with and without type 2 diabetes (T2D) receiving empagliflozin. A number of proposals and recommendations have been adopted regarding the further study of the cardiovascular and renal effects of empagliflozin and its practical use in patients with heart failure, regardless of the T2D presence.

The international AKTIV register presents a detailed description of out- and inpatients with COVID-19 in the Eurasian region. It was found that hospitalized patients had more comorbidities. In addition, these patients were older and there were more men than among outpatients. Among the traditional risk factors, obesity and hypertension had a significant negative effect on prognosis, which was more significant for patients 60 years of age and older. Among comorbidities, CVDs had the maximum negative effect on prognosis, and this effect was more significant for patients 60 years of age and older. Among other comorbidities, type 2 and 1 diabetes, chronic kidney disease, chronic obstructive pulmonary disease, cancer and anemia had a negative impact on the prognosis. This effect was also more significant (with the exception of type 1 diabetes) for patients 60 years and older. The death risk in patients with COVID-19 depended on the severity and type of multimorbidity. Clusters of diseases typical for deceased patients were identified and their impact on prognosis was determined. The most unfavorable was a cluster of 4 diseases, including hypertension, coronary artery disease, heart failure, and diabetes mellitus. The data obtained should be taken into account when planning measures for prevention (vaccination priority groups), treatment and rehabilitation of COVID-19 survivors.

The article is devoted to the analysis of research tools that are dominant in international relations forecasting. The study is based on quantitative description of 160 prognostic articles from leading journals on international relations for the period from 2006 to 2015. An innovative typology of prognostic studies is proposed and tested. The typology introduces a distinction between «weak prognoses» (probabilistic predictive statements that appear as extrapolations of deductive nomothetical theories) and «strong prognoses» («ideographic» predictions that are formulated as scenarios of possible future developments in specific situations and with specific sets of actors). The study shows that it is the weak prognoses that are the dominant type of forecasts in contemporary international studies. The dominance of the weak approach to forecasting remains total, despite the fact that it is almost two decades ago that its fundamental limitations were demonstrated and a “forward reasoning” approach suggested as an alternative. The methodology of Teaching, Research, and International Policy project was applied for a more detailed epistemological profiling of the field. It showed that academic forecasting in international relations is dominated by quantitative methods and positivist non-paradigmatic approaches. As to the traditional paradigms, it is liberalism that is the most common with Marxism being completely neglected. The described profile of the field follows the trends that are inherent in the discipline of international relations in general. The findings of the study can be interpreted from the perspective of possible tracks for the development of forecasting methods in the Russian school of international relations.

Abstract We investigate the models of cosmological inflation in generalized scalar–tensor gravity, which we consider as a source of deviation from de Sitter dynamics in the case of GR. Within the framework of the proposed approach, the exact equations of cosmological dynamics and parameters of cosmological perturbations are obtained.

Aim. To study the prevalence of carbohydrate metabolism disorders in patients with heart failure (HF) hospitalized in the city HF center.Material and methods. According to the local registry, the study sequentially included 183 patients (99 men and 84 women) hospitalized in the Nizhny Novgorod city HF center from September 1, 2019. The examination and treatment were carried out in accordance with the current clinical guidelines. In the first 48 hours after hospitalization, the concentration of the N-terminal pro-brain natriuretic peptide, soluble stimulating growth factor 2 (sST2), neutrophil gelatinase-associated lipocalin, cystatin C, blood creatinine was determined. The glomerular filtration rate was calculated using the СKDEPI equation. To assess the carbohydrate metabolism disorders, all patients were studied for fasting plasma glucose, glycated hemoglobin (HbA1c) and fructosamine. Statistical data processing was carried out using the R statistics package (R Core Team (2019)).Results. The incidence of carbohydrate metabolism disorders among patients withdecompensated HF was 75,89%, including previously diagnosed type 2 diabetes in 31,25%, newly diagnosed dysglycemia in 44,64% of patients. Less than one fourth of patients had normal parameters of carbohydrate metabolism according to HbA1c, fructosamine and fasting plasma glucose. The severity of carbohydrate metabolism disorders was significantly correlated with the severity of HF according to the following criteria: 6-minute walk test, HF functional class, sST2 level, and some parameters of cardiac remodeling. Among the criteria used for carbohydrate metabolism disorders, the HbA1c level was most closely associated with the criteria for HF severity.Conclusion. Carbohydrate metabolism disorders in HF patients are widespread and underdiagnosed during routine examination. The interrelation of carbohydrate metabolism parameters and indicators of HF severity is rationale for active detection of dysglycemia in these patients in order to potentially influence the prognosis.

The organizer of the registers “Dynamics analysis of comorbidities in SARSCoV-2 survivors” (AKTIV) and “Analysis of hospitalizations of comorbid patients infected during the second wave of SARS-CoV-2 outbreak” (AKTIV 2) is the Eurasian Association of Therapists (EAT). Currently, there are no clinical registries in the Eurasian region designed to collect and analyze information on long-term outcomes of COVID-19 survivors with comorbid conditions. The aim of the register is to assess the impact of a novel coronavirus infection on long-term course of chronic non-communicable diseases 3, 6, 12 months after recovery, as well as to obtain information on the effect of comorbidity on the severity of COVID-19. Analysis of hospitalized patients of a possible second wave is planned for register “AKTIV 2”. To achieve this goal, the register will include men and women over 18 years of age diagnosed with COVID-19 who are treated in a hospital or in outpatient basis. The register includes 25 centers in 5 federal districts of the Russian Federation, centers in the Republic of Armenia, the Republic of Kazakhstan, the Republic of Kyrgyzstan, the Republic of Belarus, the Republic of Moldova, and the Republic of Uzbekistan. The estimated capacity of the register is 5400 patients.

COVID-19 is a severe infection with high mortality. The concept of the disease has been shaped to a greater extent on the basis of large registers from the USA, Spain, Italy, and China. However, there is no information on the disease characteristics in Caucasian patients.Therefore, we created an international register with the estimated capacity of 5,000 patients — Dynamics Analysis of Comorbidities in SARS-CoV-2 Survivors (AKTIV SARS-CoV-2), which brought together professionals from the Russian Federation, Republic of Armenia, Republic of Kazakhstan, and Kyrgyz Republic. The article presents the first analysis of the register involving 1,003 patients. It was shown that the most significant difference of the Caucasian population was the higher effect of multimorbidity on the mortality risk vs other registers. More pronounced effect on mortality of such diseases as diabetes, obesity, hypertension, chronic kidney disease, and age over 60 years was also revealed.

The high and growing incidence and mortality of patients with heart failure (HF) should receive priority attention when developing an action plan to reduce cardiovascular mortality in the Russian Federation. The article provides an analysis of the implemented elements of HF care in 40 Russian regions (Northwestern, North Caucasian, Volga, Southern Federal districts), some of the best practices, as well as prospects for implementation of the cardiovascular risk management system.

Guidelines for the diagnosis and treatment of circulatory diseases in the context of the COVID-19 pandemic

Heart failure (HF) is one of the leading causes of adult mortality. Increased risk of death determines need for better understanding of the pathophysiological mechanisms, predictive risk stratification models and applicable methods to improve prognosis. One of the unfavorable prognostic factors may be an inadequate hemodynamic response to orthostatic stress. Orthostatic hypotension (OH) is known to be an independent predictor of many cardiovascular diseases, particularly HF, and death. Singlecenter study of HF population revealed that systolic blood pressure within 3-5 minutes after standing up may be a predictor of long-term unfavorable outcomes. Nevertheless, data about OH in patients with HF are limited and inconsistent due to heterogeneity of populations and different methodology in published studies. In this regard, a population-based study of the orthostatic response in patients with HF (stable and decompensated) is needed. The article describes the rationale and design of a multicenter prospective observational study aimed to assess the clinical and prognostic significance of orthostatic responses in HF patients.

Vaccination against pneumococcal infections by 13-valent conjugate vaccine (PCV13) can significantly reduce morbidity and mortality.The study has been aimed to evaluate the social and pharmacoeconomic aspects of PCV13 vaccination of 65-year-old patients with various risks of pneumococcal infection.Material and methods. Markov model with 5 and 15 years time horizon was used for the analysis from the position of the health care system.The analysis was carried out for 65-year-old citizens with low (absence of immunocompromized conditions and chronic diseases), moderate (patients with chronic diseases without immunodeficiency) and high (immunocompromized conditions) risk of pneumococcal infection as well as for the entire population of 65-year-old citizens, regardless of the risk level.In base-case assumption has been made that 1 dose of PCV13 should be administered for the patients from low and moderate risk groups and in the high-risk group 1 dose of PCV13 and in 8 weeks a dose of polysaccharide pneumococcal vaccine (PPV23) should be given.The treatment and physician visit costs have been calculated using CHI rates for St. Petersburg in 2018. Vaccination cost was calculated using the auction price to purchase PCV13 and PPV23 in 2018.Results. Vaccination of 1 cohort of 65-year-old citizens in Russian Federation within 5 years will result in prevention of 2200 deaths, 3900 cases of invasive pneumococcal diseases (IPD) and 48700 cases of community-acquired pneumonia. In 15 years prevention of about 4,3 thousand deaths, 6,6 thousand IPD and 101,1 thousand cases of CAP will be provided.Within 15-year horizon the cost-effectiveness ratio will be RUR 30,3, 82,4 and 410,0 thousand per QALY in high, moderate and low risk groups, respectively. Even if the time horizon is reduced to 5 years the PCV13 vaccination can be considered as an economically high-efficient intervention in moderate and high risk groups (cost-effectiveness ratio - RUR 279,2 and 221,7 thousand / QALY, respectively).In the 15-year-horizon noting the distribution of 65-yearolds by risk levels the cost-effectiveness ratio of PCV13 in population as a whole will be RUR 216,4 thousand / QALY. If moderate and high risk groups only are vaccinated, the average cost-effectiveness ratio will drop to RUR 67,6 thousand /QALY. At universal PCV13 vaccination of 65 years old in 5 year time horizon return of investment to the health care system budget will be 33.2% and at vaccination of persons with moderate and high risk return of investment will be 44.0%. With the assumption of vaccination during the planned physician visit (without additional visit) the return to the budget will be 46.8% and 60.9% for vaccination of all 65-year-olds and patients from the moderate and high risk groups, respectively.Conclusions. Vaccination of the 65-year-old persons against PCV13 pneumococcal infection in Russian Federation can be considered as a highly socially and economically effective intervention resulting in significant reduction of pneumococcal infection incidence and related mortality. The cost-effectiveness of vaccination is increasing along with the level of the risk. PCV13 vaccination of patients with moderate and high risk only provides a significant reduction in the burden for the health care budget in comparison with the vaccination of the entire population of 65-year-olds.

Aim. To conduct a comparative analysis of existing methods for measuring the angles of inclination of tooth and determining the torque of teeth with orthognathic occlusion.Materials and methods. Inclination angles were measured in 137 patients with physiological occlusion and the standard values of the torques of permanent teeth. Four methods of investigation were used to measure tooth torques. In the first method, the angle of the inclination formed by the occlusal plane and the conditional middle vertical of the tooth was measured by the protractor on the incisions of the plaster models of the dentition rows. The second method, which allows evaluating torques and teeth angulation at the same time, was carried out with the aid of the «Arco-zet» apparatus of «Scheu Dental GmbH». As the third method of research, the technique of analyzing images of cone-beam computed tomography was used, on which linear and angular reference points were applied. The fourth method of investigation was based on the results of scanning gypsum models with a laser scanner "ORAPIX" with the subsequent creation of a digital three-dimensional image and the formation of a virtual Set-Up model in the file "ORAPIX" 3Txer 2.5.0.Results. All methods of measuring the angles of teeth in the vestibular-lingual direction can be applied both in the applied and in the clinical aspect. Determining the torques of permanent teeth using the morphometry of gypsum models of dentition and instrumentation method is characterized by laboriousness, length of studies, and also the presence of errors in measurements. Methods for analyzing images of cone-beam computed tomography and using the virtual diagnostic SetUp model in the ORAPIX file 3Txer 2.5.0 are high-tech, precise, and reproducibility and interpretation of the results allows using the available angular and linear parameters for assessing the effectiveness of conducted orthodontic activities at all stages of treatment.Conclusion. The introduction into clinical orthodontics derived from research boundaries reference values and confidence intervals inclination angles of the upper teeth, the lower jaw, as well as tilt incline of teeth of antagonists will reduce the time required for the early stage diagnosis and increase the effectiveness of the test results of orthodontic treatment. Improvement of existing methods of diagnosis of dentoalveolar anomalies has led to the creation of high-tech, precision, computerized measurement methods, the introduction of which will allow not only minimizing errors associated with instrumental measurement techniques in the structures of the maxillofacial region, but, due to the predictability of orthodontic treatment, achieving optimal functional – aesthetic results.

The analysis is presented, of the three epidemiologic studies (EPOCHA-CHF, EPOCHA-Hospital-CHF, EPOCHA-Decompensation-CHF). In 16 years, prevalence of chronic heart failure (CHF) in Russia increased from 4,9% (1998) to 10,2% (2014), p=0,01. However, number of CHF patients with III-IV FC NYHA increased more dramatically: from 1,2% (1998) to 4,1% (2014), p=0,002. This happened due to significant increse of the age of patients selection from 64,0±11, 9 y.o. (1998) to 69,9±12,2 y.o. (2014), p=0,02; increase of the weight of etiological causes of ischemic heart disease and myocardial infarction. Overall mortality of the patients with any CHF is 6% per year. This value depends on low rate of prescription of RAAS blockers and beta-blockers during outpatient stage, with usage of low dosages of medications that does not control BP and heartrate. Patients with CHF decompensation led to hospitalization are significantly older than in general population (72,9±10,5 y.o.), and gender differences are for benefit of women, as in general population. 58,2% of hospitalized patients were admitted with noncontrolled hypertension, and in 70,5% there was heartrate higher than 80 bpm. Overall mortality of CHF patients was 25,1% (46,4% died in 1 year with the formed hypotension and 22,1% with retained level of BP). Hospital mortality was 6,8%. Mortality risks increased by repeated admittances for decompensation, absence of RAAS blockers and beta-blockers in management.

Article is devoted to the analysis of a problem of the attitude of patients and doctors to treatment of an illness from a position of medical and socio-psychological approaches on the example of the most widespread in population disease, which is very difficult to treat - an arterial hypertension. The article describes the features of the attitude to an illness of the men and women sick with an arterial hypertension. It is noted that despite the progress in medical technologies, the consciousness of the patient changes extremely slowly and he is still dependent on the doctor, waiting for his leading and guiding assistance. The possibility of purposeful impact of the doctor on development of a responsible position of the patient in relation to an illness and its treatment, and also ways of involvement of patients to adoption of the medical decision is discussed. As a result of empirical research it is established that the active attitude of the patient to an illness and its treatment (compliance with therapy) is formed when the doctor is ready to build with him dialogical, the subject - the subject relations.

Objective: To show the efficiency of dyslipidemia control in patients with coronary heart disease (CHD) and diabetes mellitus (DM) in real outpatient clinical practice and the possibilities of its correction in the long-term use of simvastatin (simvastatin forte 40 mg) in one of theNizhny Novgorod polyclinics.Subjects and methods. The efficiency of lipid profile control was analyzed in a sample from the entire dispensary group of patients with DM (n = 713). Patients at highest cardiovascular risk were selected from the dispensary group and included into a group of CHD and DM. There were a total of 461 (64.7 %) such patients. Forty-three patients were identified in this group, who were matched for baseline systolic and diastolic blood pressures, age, gender, and DM duration and were found to have significant hypercholesterolemia. Simvastatin forte 40 mg wasprescribed to these patients at their outpatient visit. During a year, the patient made 6 visits to his/her physician (5 visits at 6-werk intervals and the sixth visit by the end of the follow-up year). The dose of simvastatin was unchanged throughout the study.Results.In the entire dispensary group, 64.7 % of the patients had a concurrence of CHD and DM. This group of patients was at high risk for dyslipidemia. In this group, 2.8 % had a total cholesterol (TC) level of ≤ 3.5 mmol/l; 4.7 % had a low-density lipoprotein cholesterol (LDL-C) level of < 1.8 mmol/l. The total lipid profile was analyzed in only 6.3% of the patients from the entire dispensary group. 28.6 % of the patients with type 1 DM and 21.3% of those with CHD and type 2 DM took statins. Fifty-two week therapy with simvastatin forte 40 mg showed its high efficiency in real practice in decreasing the levels of atherogenic lipids and lipoproteins: TC by 27.6 % (p < 0.001), LDL-C by 36.9 % (p < 0.001), and triglycerides by 34.3 % (p < 0.001) with antiatherogenic high-density lipoprotein cholesterol elevation by 15.2 % (p < 0,001). Simvastatin forte therapy was well tolerated: the mean values of liver enzymes and the activity of creatine phosphokinase remained within the normal range. After one-year simvastatin forte therapy, the blood concentration of glucose decreased by 10.5%.Conclusion. Simvastatin forte (40 mg/day) demonstrated good hypolipidemic efficacy, tolerability, and high safety profile in patients with CHD and DM.

The history of thiazide diuretics’ use during the last 50 years is reviewed. The effects of thiazide diuretics on arterial hypertension treatment effectiveness, cardiovascular mortality and cardiovascular morbidity are discussed. In addition, their hemodynamic effects and safety profile are assessed.

Aim. To evaluate the pregnancy and delivery course, as well as fetal and newborn status, while achieving optimal blood pressure (BP) levels with different regimens of antihypertensive therapy (AHT) in pregnant women with chronic arterial hypertension (AH). Material and methods. Starting from Trimester I, pregnant women received AHT, with dose titration and optimal BP achievement. Group I received chronic beta-adrenoblocker therapy (BAB), Group II – was administered BAB in Trimesters I and III, and calcium antagonists (CA) in Trimester II. Group III received CA in Trimesters I-III. The control group (CG) did not receive any AHT. Results. Optimal BP levels were achieved more often in Group II. In Group III, inadequate BP control was linked to early gestosis. In CG, systolic and diastolic BP levels (SBB, DBP) were significantly higher. DBP was normalized more often than SBP. The minimal gestosis prevalence was observed in Group II (30%). Mean delivery terms in Group II were the latest, with the best newborn status by Apgar score and the lowest prevalence of delivery complications. The CG demonstrated the worst Apgar score parameters, due to hypoxia and ischemia of central nervous system in the newborns. Group I was characterised by prevalent respiratory distress syndrome in the newborns. Higher BP levels were generally associated with worse newborns’ functional status and lower mean body weight; however, in Group II, this association was not observed. Conclusion. BP reduction to the optimal levels was safe and the most effective in rotation-based AHT (BAB+AC+BAB) among pregnant women with chronic AH.
