Abstract

Abstract Aims Identifying children and/or adolescents who are at highest risk for developing chronic depression is of utmost importance, so that we can develop more effective and targeted interventions to attenuate the risk trajectory of depression. To address this, the objective of this study was to identify young people with persistent depressive symptoms across adolescence and young adulthood and examine the prospective associations between factors and persistent depressive symptoms in young people. Methods We used data from 6711 participants in the Avon Longitudinal Study of Parents and Children. Depressive symptoms were assessed at 12.5, 13.5, 16, 17.5, 21 and 22 years with the Short Mood and Feelings Questionnaire, and we further examined the influence of multiple biological, psychological and social factors in explaining chronic depressive symptoms. Results Using latent class growth analysis, we identified four trajectories of depressive symptoms: persistent high, persistent low, persistent moderate and increasing high. After applying several logistic regression models, we found that loneliness and feeling less connected at school were the most relevant factors for chronic course of depressive symptoms. Conclusions Our findings contribute with the identification of those children who are at highest risk for developing chronic depressive symptoms.

Abstract

Introduction Little is still known about the long-term impact of childhood and adolescent persistent depression and anxiety on adulthood. Objectives To investigate the impact of persistent anxiety, depression, and comorbid anxiety and depression across childhood and adolescence on the development of multiple adverse outcomes in young adulthood. Methods This study used data from 8,122 participants in the ALSPAC cohort, in the UK. The Development and Wellbeing Assessment (DAWBA) was administered to capture child anxiety and depression symptomatology. We focused on the DAWBA generalized anxiety and mood subscales at 8, 10 and 13 years, and created a measure of comorbid anxiety and depression symptoms (i.e., anxiety + depression) at each time point. Further, a range of mental and physical health, and functional problems were assessed at 24 years. Latent Class Growth Analyses was used to detect trajectories of anxiety, depression and comorbid anxiety and depression, separately; and logistic regression to examine how persistent anxiety, depression or both associated with adverse outcomes at 24 years. Results All three classes with persistent anxiety, depression or both associated with presenting any mental health and functional problem at 24 years. However, persistent high anxiety was not associated with any physical health problem at 24 years. Finally, high levels of comorbid anxiety and depression was the domain that exerted the greatest negative impact at 24 years. Conclusions Children and adolescents with comorbid anxiety and depressions are at highest risk for suffering from more adverse outcomes at 24 years, compared to those presenting anxiety and depression alone. Abstract

The vertical plane transverse emittance of accelerated electron bunches at the AWAKE experiment at CERN has been determined, using three different methods of data analysis. This is a proof-of-principle measurement using the existing AWAKE electron spectrometer to validate the measurement technique. Large values of the geometric emittance, compared to that of the injection beam, are observed ($\sim \SI{0.5}{\milli\metre\milli\radian}$ compared with $\sim \SI{0.08}{\milli\metre\milli\radian}$), which is in line with expectations of emittance growth arising from plasma density ramps and large injection beam bunch size. Future iterations of AWAKE are anticipated to operate in conditions where emittance growth is better controlled, and the effects of the imaging systems of the existing and future spectrometer designs on the ability to measure the emittance are discussed. Good performance of the instrument down to geometric emittances of approximately $\SI{1e-4}{\milli\metre\milli\radian}$ is required, which may be possible with improved electron optics and imaging., Comment: 20 pages, 9 figures

Abstract

Introduction and Objectives: Hepatocellular carcinoma (HCC) is the most frequent liver tumor and it occupies sixth place of the malignant neoplasms worldwide. In the last decades, In Mexico has been reported an increase of 95% in incidence and 14% in mortality, becoming the second most lethal neoplasm after pancreatic cancer. Few studies describe the HCC epidemiological behavior in our population. Objective: To describe the clinical and epidemiological features of HCC in patients from Veracruz city. Material and methods: A descriptive and retrospective study was done from a patient database with diagnosed HCC by imaging and, if required, with a biopsy confirmation between 2005 and 2021 in Veracruz. Central tendency and dispersion measures were done for the analysis. Results: 130 patients with HCC were studied, the mean diagnosis age was 66.9±11 (29-62 rate), with a female genre lead 1.2:1. Eighty-six patients (66.2%) had cirrhosis at the diagnosis, 32 (24.6%) were secondary to chronic alcohol consumption, 8 (6.2%) with hepatitis C infection, 20 (15.4%) MAFLD/ Cryptogenic and 3 (2.3%) with hepatitis B infection. The comorbidities reported were overweight in 58 patients (43.9%), obesity 19 (14.4%), high blood pressure 54 (40.2%) and diabetes mellitus 51 (38.6%). The most frequent biochemical disturbance was hyperbilirubinemia (1.27±1.26). The rest f biochemical features are described in table 1. Sixty-six patients (50%) were found with Child-Pugh A with a MELD score of 9.86±3.0. In this stage, we found 74 patients (56.9%) with Barcelona B, 80% had a lone tumor, the mean tumoral size was 7.13±3.7 cm. 69.2% of patients had a tumoral size greater than 5 cm at the diagnosis. Discussion: Our results show that the HCC behavior is similar to the reported in previous Mexican studies, predominating in patients with advanced liver injury and tumor outside criteria of local treatment at the diagnosis. The high frequency of comorbidities associated with metabolic syndrome is remarkable as one of the main risk factors with chronic C virus infection. Conclusion: HCC in Mexico has been increasing. It shows similar epidemiological features with the reported in other populations due to its relationship with metabolic risk. Early screening in high-risk patients results in greater resectability and survival.The authors declare that there is no conflict of interest.

Abstract

Introduction and Objectives: In Mexico, hepatocellular carcinoma (HC) represents >90% of primary hepatic tumors. The diagnosis is determined by imaging findings (CT or MR). A biopsy is necessary for specific situations. The staging method is the Barcelona classification (BCLC) which considers hepatic biomarkers, the tumor burden and the performance status. Treatment options include transplantation, liver resection (LR), radiofrequency ablation (RFA), transarterial embolization (TACE/TAE) and systemic treatment (ST). Nonetheless, there are few surveillance studies in Mexico. The study aims to describe the surveillance of HC subjects after different therapeutic approaches. Materials and Methods: Descriptive and retrospective study with a database including 130 patients diagnosed with HC by imaging findings (or biopsy if needed) between 2005 and 2021 in Veracruz. For statistical analysis, surveillance data was first summarized by descriptive statistics and 5-year-overall survival rates (5OS). Subsequently, a comparison of surveillance between therapeutic options was made by Log-Rank, Cox regression and χ2. We considered statistical significance at p0.05). Recurrence of the tumor was only observed in stage B: 5 cases after LR (29.4%), and 2 cases after RFA (22.2%), χ2=12.084 p=0.017. The mean time for recurrence detection was 19 months and five months for LR and RFA, respectively. Discussion: This analysis showed higher accumulated surveillance for patients with LR, followed by RFA. Furthermore, Log-Rank curve of RFA showed a pronounced inclination of surveillance around the 25th month, after which it reached a plateau and deceleration until the 75th month. However, five year-OSr seemed to be lower than in other studies. Conclusion: Our results suggest that LR is a feasible treatment alternative. In the meantime, RFA seems a worthy option when patients are not candidates for LR, showing promising results.The authors declare that there is no conflict of interest.

Abstract

Los bosques representan uno de los tipos ecosistemas terrestres más importantes, tanto por la biodiversidad que albergan como por los servicios ecosistémicos que proporcionan. Por eso es necesario estudiar y anticipar las amenazas que existen sobre ellos. El cambio climático es una de las mayores amenazas sobre estos ecosistemas y es preciso investigar cuáles serán sus efectos en el futuro. La creciente disponibilidad de datos (p.ej. los inventarios forestales), y un aumento de la capacidad analítica (p.ej. mediante nuevas metodologías de análisis numérico y una mayor capacidad computacional), están permitiendo mejoras sustanciales en las predicciones de los efectos del cambio climático sobre los bosques. En este trabajo se revisan algunas herramientas de modelización, usadas en combinación con datos provenientes de inventarios forestales, para predecir los efectos del cambio climático en la distribución y funcionamiento de distintas especies de árboles. Prestando una especial atención a los estudios que han utilizado el inventario forestal nacional español. Asimismo, discutimos cuales son los avances más relevantes derivados del uso de estas herramientas y que oportunidades, y retos, existen en el futuro de la modelización de los impactos del cambio climático en los bosques.

8. Frequency of virulence genes in mixed infections with Helicobacter pylori strains from a Mexican population

Abstract

Background: Helicobacter pylori (H. pylori) is associated with gastroduodenal diseases. Virulence of clinical isolates is related to clinical outcome. Moreover, with microdiversity studies in clinical isolates from a single patient, but from a different origin (antrum or corpus), it is possible to demonstrate that there are simultaneous mixed infections. Aims: To genotype H. pylori strains with multiplex PCR, according to their clinical virulence, and in this manner know the frequency of each genotype and relate it to clinical outcome in order to prevent the development of severe diseases. Methods: A total of 210 patients with gastric alterations were studied. Virulence classification of H. pylori strains was carried out with multiplex PCR and 127 strains were identified as H. pylori by PCR (glmM and cagE). Genotype and clinical outcome were evaluated with the Fisher's exact test. In addition, RAPD-PCR was performed as a fingerprinting method to analyze mixed infections. Results: The cagA, vacAs1, and vacAm1 genes were detected in all the clinical isolates. Strains were classified as: type i, 40.15% (51/127); type ii, 22.04% (28/127); and type iii, 28.4% (36/127), but two new different genotypes were also detected: (1) babA2+, cagA+, vacAs1+, 6.29% (8/127) and (2) babA2+, cagA-, vacAs2/m2+, 3.14% (4/127). The cagE gene was detected in type i strains. Conclusions: The Fisher's exact test did not support a significant association between clinical outcome and genotype. The main circulating genotypes in the Mexican population studied were: cagA+, vacAs1, and vacAm1. Multiplex PCR can be used as a screening test for H. pylori strains. Furthermore, the cagE gene is a good marker for identifying cag-PAI positive strains.

Antecedentes: La infección por Helicobacter pylori se asocia con gastritis. La variabilidad genética de las cepas tiene importancia clínica. Los estudios de microdiversidad en cepas aisladas de diferente sitio anatómico en un mismo paciente revelan la posibilidad de recuperar 2 o más cepas diferentes. Objetivos: Genotipificar por PCR multiplex según la virulencia de aislados clínicos de H. pylori para determinar la frecuencia de cada genotipo y relacionarlo con la evolución clínica con la finalidad de prevenir el desarrollo de enfermedades graves. Métodos: Se estudió a 210 pacientes con alteraciones gastroduodenales; 127 cepas fueron identificadas por PCR (glmM y cagE) y después clasificadas según su virulencia por PCR multiplex. Se hizo la prueba de Fisher para evaluar la relación entre genotipo y resultado clínico. La técnica de RAPD-PCR se empleó como método de huella genética y para analizar la presencia de infecciones mixtas. Resultados: cagA, vacAs1 y vacAm1 estuvieron presentes en todos los aislados clínicos. Las cepas se clasificaron como: tipo i, 40.15%(51/127); tipo ii, 22.04% (28/127), y triples positivas, 28.4% (36/127); se detectaron 2 genotipos nuevos: 1) babA2+, cagA+, vacAs1+, 6.29% (8/27), y 2) babA2+, cagA–, vacAs2/m2+, 3.14% (4/127). cagE se detectó en las cepas tipo ii. Conclusiones: La prueba de Fisher no mostró una asociación significativa entre el resultado clínico y el genotipo en la población estudiada. Los genotipos circulantes en la población mexicana fueron cagA+, vacAs1, vacAm1. La PCR multiplex puede usarse para genotipificar rápidamente las cepas de H. pylori. cagE es un buen marcador para identificar cepas cag-PAI+.

10. Viability and functional impact of probiotic and starter cultures in salami-type fermented meat products

Abstract

Salami, a well-known fermented meat product, is made from selected ground meat mixed with curing agents and spices. This work aimed to determine the viability of Lactiplantibacillus plantarum (as a starter), Lactobacillus acidophilus (probiotic microorganism), and their mixture during the fermentation and ripening of a salami-type product, evaluate the microbiological and physicochemical changes and assess the sensory acceptability of the final product. L. acidophilus has not been sufficiently explored as a probiotic in fermented meats, especially in terms of its effects on fermentation and sensory qualities. Salami-type products were formulated and fermented for 48 h at 32°C, and then ripening took place at 8°C for 13 days. pH, titratable acidity, Lactobacillus counts, and contaminating microbiota were analyzed during the process. Sensory evaluation was analyzed in the final products. The salami-type formulation served as an effective medium for growing microorganisms, with the populations of starter and probiotic cultures exceeding 108 CFU/g after fermentation and ripening for 15 days. The pH of the end products was ∼5.1, titratable acidity ∼2.5%, and aw ∼0.83. During fermentation and ripening, a significant reduction in total mesophilic aerobic bacteria (>7 logs), coliforms, and Staphylococcus aureus (>8-fold reductions) were observed. The sensory evaluation results indicate that the product’s attributes are not influenced by the type of bacteria used, as no significant difference was found (p > 0.05). The results show that L. acidophilus, Lactiplantibacillus plantarum, or their mixture can be used as a starter culture in fermented meat products. Using L. acidophilus, whether alone or in combination, is a viable option that preserves the characteristics of the fermented product and may enhance the benefits of probiotic consumption.

Abstract

UV-absorbing covers reduce the incidence of injurious insect pests and viruses in protected crops. In the present study, the effect of a UV-absorbing net (Bionet) on the spatio-temporal dynamics of the potato aphid on lettuce plants was evaluated. A field experiment was conducted during three seasons in two identical tunnels divided in four plots. A set of lettuce plants were artificially infested with Macrosiphum euphorbiae adults and the population was estimated by counting aphids on every plant over 7 to 9 weeks. Insect population grew exponentially but a significantly lower aphid density was present on plants grown under the UV-absorbing cover compared to a standard 50 mesh net. Similarly, in laboratory conditions, life table parameters were significantly reduced under the Bionet. Moreover, SADIE analysis showed that the spatial distribution of aphids was effectively limited under the UV-absorbing nets. Our results indicate that UV-absorbing nets should be considered as an important component of lettuce indoor cropping systems preventing pesticide applications and reducing the risk of spread of aphid-borne virus diseases.

Abstract

Background: Insulin resistance (IR) is associated with a higher risk of multiple diseases and its early detection would allow to minimize the associated risk; the presence of acanthosis nigricans (AN) it´s associated to the presence of IR. Objective: To evaluate the sensibility and specificity of AN to diagnose IR in a group of Chilean patients. Methods: We designed a cross-sectional study and it was included subjects that were attended at the Center for the Attention of Metabolic Diseases at the Faculty of Medicine, University of Chile. Sixty subjects (18-60 years age) were included. It was determined BMI and diagnosed AN and skin phototype; blood samples were taken and calculated the HOMA-IR. The normality of the variables where analyzed by Kolmogorov-Smirnov test. There were used χ² and the diagnostic concordance between AN and IR was determined using the Kappa index and Pearson´s correlation. Sensibility, specificity, positive and negative predictive value were calculated and accepted p < 0.05. Results: The IR diagnose was 67% and AN was 43%. The major proportion of subjects diagnosed as positive for IR were also positive for AN (84.6%). The sensibility of AN to find IR was an 84% and specificity was 100%. Positive and negative predictive values were 100% and 89% respectively. It was observed a positive association between BMI and HOMA-IR (r = 0.674; r² = 0.454; p < 0.001). Conclusion: To detect acanthosis nigricans in Chilean population may be effective for the early diagnose of insulin resistance and, therefore, reduce the associated cost of the late treatment of glucose metabolic disturbances.Introducción: La presencia de resistencia a la insulina (RI) se asocia a un mayor riesgo de padecer diversas enfermedades y su detección precoz permitiría minimizar el riesgo asociado; la presencia de acantosis nigricans (AN) se asocia a la presencia de RI. Objetivo: Evaluar la sensibilidad y especificidad de la AN para diagnosticar RI en un grupo de pacientes chilenos. Método: Se realizó un estudio transversal y fueron incluidos sujetos atendidos por el Centro para Atención de Enfermedades Metabólicas, Facultad de Medicina, Universidad de Chile. En sesenta sujetos (18-60 años) fueron determinados IMC, presencia de AN y fototipo de piel; muestras de plasma fueron tomadas para el cálculo del índice HOMA-IR. Fue analizada la normalidad de las variables con el test de Kolmogorov-Smirnov y utilizado χ² ; la concordancia diagnóstica entre AN y RI fue determinada con índice Kappa y correlación de Pearson. Fueron calculados sensibilidad, especificidad, valor predictivo positivo y negativo y aceptado un p < 0,05. Resultados: El diagnóstico de RI fue de 67% y AN 43%. La mayor proporción de los sujetos positivos para RI también lo fueron para AN. La sensibilidad de AN para el diagnóstico de RI fue de 84% y la especificidad 100%. Los valores predictivos positivo y negativo fueron 100% y 89% respectivamente. Existe una asociación positiva entre el IMC y el HOMA-IR (r = 0,674; r² = 0,454; p < 0,001). Conclusión: En población adulta chilena la presencia de acantosis nigricans puede ser efectivo para un diagnóstico temprano de resistencia insulínica con disminución del costo asociado al tratamiento de las alteraciones en el metabolismo de la glucosa.

Abstract

El objetivo del presente trabajo fue evaluar los costos de producción de vaquillas Holstein mediante la comparación de dos sistemas de alimentación, basados en ensilado de caña de azúcar (ECA) vs el sistema tradicional del rancho basado en ensilado de maíz (EM). En ambos casos, con suplementación a los animales. Se utilizaron 28 becerras posterior al destete, con una edad promedio de 80±16 días y un peso inicial de 79.5 ± 12.9 kg. El estadístico fue un análisis de varianza para un diseño en bloques al azar, en donde el peso fue el factor de bloqueo y se dividieron en tres grupos dentro de cada tratamiento. Cada 30 días se midió la ganancia diaria de peso GDP (kg), la condición corporal CC, la conversión alimenticia CA (kg), el consumo de forraje CF (kg), de suplemento CS (kg), costo de alimentación/ día y la determinación del costo de producción de los reemplazos ($). Las vaquillas tuvieron una GDP 0.666 y 0.743 kg/día, CC 3.1b y 3.7a, CA 7.4a y 9.8b, CF 3.2 y 5.3, CS 1.8 y 1.5, costo de alimentación/día 9.4a y 11.4b y costo de la vaquilla $6,986.82 y $8,034.39, para ECA y EM, respectivamente. La alimentación fue el rubro que mayor impacto tuvo en la obtención de vaquillas de reemplazo, con 67 a 71% de los costos totales y la etapa de mayor inversión en la producción de vaquillas correspondió a la fase del nacimiento al destete. La alimentación basada en ensilaje de caña de azúcar permitió un mejor comportamiento económico para la producción de vaquillas de reemplazo, comparado con el sistema tradicional con ensilado de maíz.

Abstract

Prostate cancer is very frequent, but secondary ureteral metastasis are extremely rare. We present a 55 year old man with a 2 month history of right flank pain and lower urinary tract symptoms. Prostatic specific antigen of 11.3 ng/mL. Computed tomography showed right hydroureteronephrosis, a developing urinoma and right iliac adenopathies. He underwent right ureteronephrectomy, iliac lymphadenectomy and prostate biopsy. Pathology revealed prostatic carcinoma infiltrating the ureteral muscularis propria, without mucosal involvement. There are 46 reported cases of prostate cancer with ureteral metastases. Ureteral metastasis are a rare cause of renal colic and need of a high index of suspicion.

Abstract

A precise characterization of the incoming proton bunch parameters is required to accurately simulate the self-modulation process in the Advanced Wakefield Experiment (AWAKE). This paper presents an analysis of the parameters of the incoming proton bunches used in the later stages of the AWAKE Run 1 data-taking period. The transverse structure of the bunch is observed at multiple positions along the beamline using scintillating or optical transition radiation screens. The parameters of a model that describes the bunch transverse dimensions and divergence are fitted to represent the observed data using Bayesian inference. The analysis is tested on simulated data and then applied to the experimental data.

Abstract

We present numerical simulations and experimental results of the self-modulation of a long proton bunch in a plasma with linear density gradients along the beam path. Simulation results agree with the experimental results reported in arXiv:2007.14894v2: with negative gradients, the charge of the modulated bunch is lower than with positive gradients. In addition, the bunch modulation frequency varies with gradient. Simulation results show that dephasing of the wakefields with respect to the relativistic protons along the plasma is the main cause for the loss of charge. The study of the modulation frequency reveals details about the evolution of the self-modulation process along the plasma. In particular for negative gradients, the modulation frequency across time-resolved images of the bunch indicates the position along the plasma where protons leave the wakefields. Abstract

We use a relativistic ionization front to provide various initial transverse wakefield amplitudes for the self-modulation of a long proton bunch in plasma. We show experimentally that, with sufficient initial amplitude ($\ge(4.1\pm0.4)$ MV/m), the phase of the modulation along the bunch is reproducible from event to event, with 3 to 7% (of 2$\pi$) rms variations all along the bunch. The phase is not reproducible for lower initial amplitudes. We observe the transition between these two regimes. Abstract

Plasma wakefield dynamics over timescales up to 800 ps, approximately 100 plasma periods, are studied experimentally at the Advanced Wakefield Experiment (AWAKE). The development of the longitudinal wakefield amplitude driven by a self-modulated proton bunch is measured using the external injection of witness electrons that sample the fields. In simulation, resonant excitation of the wakefield causes plasma electron trajectory crossing, resulting in the development of a potential outside the plasma boundary as electrons are transversely ejected. Trends consistent with the presence of this potential are experimentally measured and their dependence on wakefield amplitude are studied via seed laser timing scans and electron injection delay scans., Comment: 11 pages, 8 figures

Abstract

In 2017, AWAKE demonstrated the seeded self-modulation (SSM) of a 400 GeV proton beam from the Super Proton Synchrotron (SPS) at CERN. The angular distribution of the protons deflected due to SSM is a quantitative measure of the process, which agrees with simulations by the two-dimensional (axisymmetric) particle-in-cell code LCODE. Agreement is achieved for beam populations between $10^{11}$ and $3 \times 10^{11}$ particles, various plasma density gradients ($-20 \div 20\%$) and two plasma densities ($2\times 10^{14} \text{cm}^{-3}$ and $7 \times 10^{14} \text{cm}^{-3}$). The agreement is reached only in the case of a wide enough simulation box (at least five plasma wavelengths)., Comment: 8 pages, 9 figures, 1 table

We briefly compare in numerical simulations the relativistic ionization front and electron bunch seeding of the self-modulation of a relativistic proton bunch in plasma. Abstract

We report on the neutrino mass measurement result from the first four-week science run of the Karlsruhe Tritium Neutrino experiment KATRIN in spring 2019. Beta-decay electrons from a high-purity gaseous molecular tritium source are energy analyzed by a high-resolution MAC-E filter. A fit of the integrated electron spectrum over a narrow interval around the kinematic endpoint at 18.57 keV gives an effective neutrino mass square value of $(-1.0^{+0.9}_{-1.1})$ eV$^2$. From this we derive an upper limit of 1.1 eV (90$\%$ confidence level) on the absolute mass scale of neutrinos. This value coincides with the KATRIN sensitivity. It improves upon previous mass limits from kinematic measurements by almost a factor of two and provides model-independent input to cosmological studies of structure formation.

Abstract

The determination of the neutrino mass is one of the major challenges in astroparticle physics today. Direct neutrino mass experiments, based solely on the kinematics of beta-decay, provide a largely model-independent probe to the neutrino mass scale. The Karlsruhe Tritium Neutrino (KATRIN) experiment is designed to directly measure the effective electron antineutrino mass with a sensitivity of 0.2 eV 90% CL. In this work we report on the first operation of KATRIN with tritium which took place in 2018. During this commissioning phase of the tritium circulation system, excellent agreement of the theoretical prediction with the recorded spectra was found and stable conditions over a time period of 13 days could be established. These results are an essential prerequisite for the subsequent neutrino mass measurements with KATRIN in 2019.

Abstract

We show in experiments that a long, underdense, relativistic proton bunch propagating in plasma undergoes the oblique instability, which we observe as filamentation. We determine a threshold value for the ratio between the bunch transverse size and plasma skin depth for the instability to occur. At the threshold, the outcome of the experiment alternates between filamentation and self-modulation instability (evidenced by longitudinal modulation into microbunches). Time-resolved images of the bunch density distribution reveal that filamentation grows to an observable level late along the bunch, confirming the spatiotemporal nature of the instability. We provide a rough estimate of the amplitude of the magnetic field generated in the plasma by the instability and show that the associated magnetic energy increases with plasma density.

Abstract

Experimental results show that hosing of a long particle bunch in plasma can be induced by wakefields driven by a short, misaligned preceding bunch. Hosing develops in the plane of misalignment, selfmodulation in the perpendicular plane, at frequencies close to the plasma electron frequency, and are reproducible. Development of hosing depends on misalignment direction, its growth on misalignment extent and on proton bunch charge. Results have the main characteristics of a theoretical model, are relevant to other plasma -based accelerators and represent the first characterization of hosing.

Resumen: Objetivo: Demostrar que la variante no descrita en el gen PKD1 c.7292T>A, identificada en cuatro familias de la comarca de la Alpujarra de Granada, es la causante de la poliquistosis renal autosómica dominante (PQRAD). Esta variante consiste en una sustitución transversión de timina (T) por adenina (A) que a nivel de la proteína policistina 1 produce un cambio de leucina (Leu/L) por glutamina (Gln/Q) en la posición 2431 (p.Leu2431Gln). Método: Registramos variables sociodemográficas y clínicas a través de la realización de historias clínicas, árboles genealógicos, ecografías y estudios genéticos a individuos afectos y sanos pertenecientes a estas familias en el contexto del estudio de segregación. Resultados: Todos los individuos afectados portaban en heterocigosis la variante c.7292T>A, mientras que los individuos sanos no la portaron. En las familias estudiadas, el 62,9% eran mujeres. El diagnóstico de PQRAD se realizó a los 29,3 ± 15,82 años de edad, después de haber tenido el primer hijo en el 64,8%. Los motivos principales de diagnóstico de la enfermedad fueron antecedentes familiares y episodios de hematuria. El inicio de tratamiento renal sustitutivo (TRS) se produjo a la edad de 55,8 ± 7,62 años (rango 44-67), y el éxitus a los 63 ± 92,2 años (rango 48-76), siendo la causa desconocida, cardiovascular e insuficiencia renal las más frecuentes; la mediana de supervivencia renal se estableció a los 58,5 ± 0,77 años y la mediana de supervivencia del paciente a los 67 ± 3,54 años. No observamos diferencias en la supervivencia del riñón y del paciente según el sexo. De los pacientes fallecidos, el 52,2% necesitaron TRS y el 94,4% tenían algún grado de insuficiencia renal (IR). Conclusiones: La variante c.7292T>A en el gen PKD1 es responsable de la enfermedad y su distribución en la comarca de la Alpujarra de Granada sugiere un efecto fundador. En la PQRAD es necesario realizar estudios de segregación que ayuden a reclasificar variantes genéticas, en este caso de indeterminada a patogénica. Method: Sociodemographic and clinical variables were registered using clinical histories, genealogical trees, ultrasounds and genetic analysis to ADPKD and healthy individuals belonging to these families in the context of segregation study. Results: All PKD individuals carried the c.7292T>A variant in heterozygosis, whereas healthy ones did not. Among all ADPKD patients, 62.9% were women. ADPKD diagnosis was made at 29.3 ± 15.82 years, after having the first child in 64.8%. The main reasons for diagnosis were family history and hematuria episodes. The onset of renal replacement therapy (RRT) occurred at 55.8 ± 7.62 years (range 44-67), and death at 63 ± 92.2 years (range 48-76), being the cause unknown, cardiovascular and insufficiency kidney the most frequent; the median of renal survival was established at 58.5 ± 0.77 years and the median survival of patients at 67.2 ± 3.54 years. No differences in kidney and patient survivals were observed according to sex. Among deceased patients, 52.2% required RRT and 94.4% suffered from renal failure. Conclusions: The variant c.7292T>A in PKD1 gene is responsible for the disease, and its distribution in the Alpujarra region of Granada suggests a founder effect. In ADPKD it is necessary to perform segregation studies that help us to reclassify genetic variants, in this case from indeterminate to pathogenic.

29. Frecuencia y factores de riesgo asociados a la presencia de Chlamydia abortus, en rebaños ovinos en México

Abstract

El objetivo fue evaluar la frecuencia serológica individual y de rebaño, así como detectar los factores de riesgo de C. abortus, en siete estados donde se realiza la producción ovina en México. Se efectuó un estudio multifactorial, transversal y estratificado, con análisis de 5,321 muestras serológicas en 323 rebaños de 61 municipios. Se realizó ELISA con un kit comercial. Los factores de riesgo asociados con la enfermedad se determinaron mediante cuestionarios y análisis estadísticos con una prueba de Ji cuadrada con un intervalo de confianza del 95%. Del total de 5, 231 muestras de suero que fueron colectadas, 581 (10.92 %) dieron positivas a la prueba de ELISA. Los resultados por estado de sueros positivos fueron: Tlaxcala 13.08% (73/558); Sonora 12.45 % (102/819); Chihuahua 11.56 % (107/925); Hidalgo 11.34 % (97/855); Chiapas 10.15 % (60/591); Querétaro 9.69 % (79/815); Estado de México 7.09 % (63/758). La frecuencia de hatos seropositivos, fue de 43.34 % (140/323). Los resultados al ser agrupados por estado fueron los siguientes: Hidalgo 67.39 % (31/46), Querétaro 67.18 % (43/64); Sonora 40.92 % (19/47); Tlaxcala 33.33 % (12/36); Chiapas 31.57 % (12/38); Estado de México 25.45 % (14/55) y Chihuahua 24.32 % (9/37). Los principales factores de riesgo que favorecen la presencia del aborto enzoótico de las ovejas, fueron que estuviesen gestantes, que tuviesen una edad entre los 37 a los 48 meses y que el sistema de producción fuese intensivo. Estos estudios serológicos identificaron la presencia del aborto enzoótico de las ovejas en México, así como algunos de los factores de riesgo que inciden para favorecer su presencia.

Abstract

Currently, metastatic colon cancer is treated with monotherapeutic regimens such as folinic acid, fluorouracil, and oxaliplatin (FOLFOX), capecitabine and oxaliplatin (CapeOX), and leucovorin, fluorouracil, and irinotecan hydrochloride (FOLFIRI). Other treatments include biological therapies and immunotherapy with drugs such as bevacizumab, panitumumab, cetuximab, and pembrolizumab. After the research, it was found that some mutations make those treatments not as effective in all patients. In this bibliographic review, we investigated the pharmacogenetic explanations for how mutations in the genes coding for rat sarcoma virus (RAS) and rapidly accelerated fibrosarcoma (RAF) reduce the effectiveness of these treatments and allow the continued proliferation of tumors. Furthermore, we note that patients with mutations in the dihydropyrimidine dehydrogenase (DPDY) gene usually require lower doses of therapies such as 5-fluorouracyl (5-FU) and capecitabine to avoid severe adverse effects. Some other mutations in the thymidylate synthase gene (TSYM), methylenetetrahydrofolate reductase gene (MTHFR), and ATP binding cassette transporter B (ABCB1 and ABCB2) affect efficacy and security of the treatments. It is important to address the clinical implication of the oncologist in the study of gene mutations than can influence in the antitumoral response and safety of colon cancer treatments.

os inhibidores del punto de control inmunitario (IPCI) son un grupo farmacológico destacable dentro de la inmunoterapia oncológica. Los IPCI se administran en monoterapia o en terapia combinada junto a quimioterápicos como el cisplatino, mostrando excelentes resultados clínicos. Sin embargo, no están exentos de reacciones adversas como la lesión renal inducida por IPCI, mínimamente estudiada y que empeora el pronóstico del paciente. El objetivo del estudio fue la caracterización funcional del daño renal asociado a los IPCI (anti-CTLA-4 y anti-PD-1) y a la combinación de estos con cisplatino, mediante una batería de biomarcadores de lesión renal. En el estudio, se emplearon ratones macho C57BL/6 divididos en grupos experimentales. Cada grupo recibió, correspondientemente: anti-PD-1, anti-CTLA-4, cisplatino o la terapia combinada de cisplatino+anti-CTLA-4+anti-PD-1. El análisis de biomarcadores se realizó mediante técnicas colorimétricas y de inmunoensayo. Los datos fueron sometidos a análisis estadístico empleando el programa IBM SPSS® Statistics. Los biomarcadores plasmáticos (creatinina y urea) no mostraron lesión renal en ningún grupo de tratamiento, mientras que los biomarcadores urinarios (NGAL, KIM-1 y albúmina) aumentaron en los grupos de terapia combinada mostrando daño renal subclínico. Por ello, los biomarcadores urinarios podrían resultar ventajosos en el diagnóstico precoz del daño renal asociado a IPCI en pacientes oncológicos.

34. The Implementation of a Nutrition Intervention in Food Pantries: The Spirit of SWAP

Abstract

Background. Food insecurity, affecting approximately 10% of the U.S. population, with up to 40% or higher in some communities, is associated with higher rates of chronic conditions and inversely associated with diet quality. Nutrition interventions implemented at food pantries are an effective strategy to increase healthy food choices and improve health outcomes for people experiencing food and nutrition insecurity. Supporting Wellness at Pantries (SWAP), a stoplight nutrition ranking system, can facilitate healthy food procurement and distribution at pantries. Purpose. Guided by the RE-AIM Framework, this study assesses the implementation and outcomes of SWAP as nutritional guidance and institutional policy intervention, to increase procurement and distribution of healthy foods in pantries. Method. Mixed-methods evaluation included observations, process forms, and in-depth interviews. Food inventory assessments were conducted at baseline and 2-year follow-up. Results. Two large pantries in New Haven, Connecticut, collectively reaching more than 12,200 individuals yearly, implemented SWAP in 2019. Implementation was consistent prepandemic at both pantries. Due to COVID-mandated distribution changes, pantries adapted SWAP implementation during the pandemic while still maintaining the “spirit of SWAP.” One pantry increased the percentage of Green foods offered. Challenges to healthy food distribution are considered. Discussion. This study has implications for policy, systems, and environmental changes. It shows the potential for SWAP adoption at pantries, which can serve as a guide for continued healthy food procurement and advocacy. Maintaining the “spirit of SWAP” shows promising results for food pantries looking to implement nutrition interventions when standard practice may not be possible.

Abstract

Anaplastic large cell lymphoma (ALCL) is a rare type of non-Hodgkin lymphoma (NHL) originated from mature post thymic T cells. They represent 1–3% of NHL. Different subtypes have been described: Anaplastic lymphoma kinase (ALK)-negative ALCL, ALK-positive ALCL and breast implant-associated ALCL. ALK-positive ALCL affects mainly the young and has better prognosis. We present a case report of an adult woman with AKL-positive ALCL, diagnosed by endobronchial ultrasound-guided transbronchial needle aspirate (EBUS-TBNA).A 59-year-old women with no history of breast implants, was admitted for a four-month low back pain. Initially, the patient was treated for a spondyloarthropathy, but due to persistence of the symptoms, a lumbosacral MRI was performed, showing changes in morphology and signal intensity in the vertebral body of L3, along with edema and a paravertebral collection that affected the left psoas muscle, suggesting granulomatous spondylodiscitis. Chest CT-scan showed mild left pleural effusion, subcarinal and right hiliar adenomegalies. An EBUS-TBNA with ROSE (rapid on-site evaluation) was performed showing positive findings for malignancy, suggestive of hematolymphoid neoplasia. Pathology analysis showed an AKL-positive ALCL. Additionally, a biopsy of paravertebral tissue biopsy was obtained, which was consistent with the nodal sample. Chemotherapy was initiated with the CHOP protocol: cyclophosphamide, hydroxydaunorubicin, vincristine sulfate and prednisone.EBUS-TBNA is a minimally invasive and safe technique for obtaining mediastinal samples. Collaboration with a cytopathologist trained to perform ROSE improves the diagnostic performance. Abstract

Los inhibidores del punto de control inmunitario (ICI) constituyen el grupo inmunoterapéutico más relevante del ámbito oncológico, pero no están exentos de efectos adversos. Las lesiones renales, aunque raras, empeoran el pronóstico. Actualmente existen combinaciones de inmunoterapia y quimioterapia que mejoran la eficacia, pero aumentan el riesgo de sufrir efectos secundarios nefrotóxicos. El objetivo fue la evaluación histológica del daño renal asociado a los ICI anti-CTLA-4 y anti-PD-1 en combinación con cisplatino. Se diseñó un modelo experimental en ratones C57BL/6 tratados con la terapia combinada de cisplatino (10 mg/kg, dosis única) con anti-CTLA-4 o anti-PD-1 (10 o 15 mg/kg/día, durante 6 días). Se incluyeron grupos con las monoterapias de los fármacos y un grupo control. La evaluación del daño renal histológico se realizó de forma ciega siguiendo un protocolo de cuantificación de lesión tisular basado en una escala. Los datos fueron analizados con el programa estadístico IBM SPSS Statistics. Los resultados mostraron daño renal tubular, más pronunciado en la corteza externa, asociado al tratamiento con cisplatino, mientras que no hubo alteraciones estructurales en la monoterapia con ICI. El cotratamiento potenció el daño estructural renal del cisplatino. Esta potenciación fue más evidente en la región corticomedular, lo que sugiere que la combinación de fármacos provoca una lesión más profunda en el riñón.

Abstract

MicroRNAs (miRNAs) are highly conserved, small non-coding RNA molecules (∼21 nucleotides) that regulate numerous biological processes, including developmental timing, hematopoiesis, organogenesis, apoptosis, cell differentiation, and proliferation either by mRNA degradation or translation repression. Since eye physiology requires a perfect orchestration of complex regulatory networks, an altered expression of key regulatory molecules such as miRNAs potentially leads to numerous eye disorders. In recent years, comprehensive progress has been made in demonstrating the precise roles of miRNAs, emphasizing their potential use in diagnostic and therapeutic purposes of chronic human diseases. Thus, this review explicitly illustrates the regulatory roles of miRNAs in four common eye disorders, such as cataract, glaucoma, macular degeneration, and uveitis, and their application in disease management.

Abstract

Proper muscle contraction requires the assembly and maintenance of sarcomeres and myofibrils. Although the protein components of myofibrils are generally known, less is known about the mechanisms by which they individually function and together synergize for myofibril assembly and maintenance. For example, it is unclear how the disruption of actin filament (F-actin) regulatory proteins leads to the muscle weakness observed in myopathies. Here, we show that knockdown of Drosophila Tropomodulin (Tmod), results in several myopathy-related phenotypes, including reduction of muscle cell (myofiber) size, increased sarcomere length, disorganization and misorientation of myofibrils, ectopic F-actin accumulation, loss of tension-mediating proteins at the myotendinous junction, and misshaped and internalized nuclei. Our findings support and extend the tension-driven self-organizing myofibrillogenesis model. We show that, like its mammalian counterpart, Drosophila Tmod caps F-actin pointed-ends, and we propose that this activity is crucial for cellular processes in different locations within the myofiber that directly and indirectly contribute to the maintenance of muscle function. Our findings provide significant insights to the role of Tmod in muscle development, maintenance and disease.

Les preses suposen una estratègia per satisfer la demanda d’una població mundial creixent, abastint les nostres ciutats d’aigua i energia, i fins i tot serveixen per regular desastres com inundacions violentes. Tot i això, la seva construcció té un fort impacte mediambiental amb efectes immediats evidents, i d’altres que romanen subjacents i es manifesten a llarg termini, suposant una greu amenaça. Aquests problemes causats per les preses són presents arreu del món, i Catalunya no n’és una excepció. Per posar un exemple, la construcció de preses sovint comporta la destrucció de les planes d’inundació, fet que genera una desestabilització ecosistèmica que desencadena en un desastre ecològic. El pronòstic però, no és tan catastròfic com sembla. Actualment es poden prendre mesures i implementar estratègies per minimitzar els efectes negatius de les preses, i en aquest article revisem les més importants., Dams are a strategy to meet the demand of a growing world population, supplying our cities with water and energy, and they even serve to regulate disasters such as violent floods. Even so, its construction has a strong environmental impact with obvious immediate effects, and others that remain underlying and manifest themselves in the long term, posing a serious threat. These problems caused by dams are present around the world, and Catalonia is no exception. To give an example, the construction of dams usually involves the destruction of floodplains, which generates ecosystem destabilization that triggers an ecological disaster. Despite this, the prognosis is not as catastrophic as it seems. Currently we can take measures and implement strategies to minimize the negative effects of dams, and in this article we review the most important ones., Las presas suponen una estrategia para satisfacer la demanda de una población mundial creciente, abasteciendo nuestras ciudades de agua y energía, y hasta sirven para regular desastres como inundaciones violentas. Aun así, su construcción tiene un fuerte impacto medioambiental con efectos inmediatos evidentes, y otros que permanecen subyacentes y se manifiestan a largo plazo, suponiendo una grave amenaza. Estos problemas causados por las presas están presentes alrededor del mundo, y Cataluña no es una excepción. Por poner un ejemplo, las construcción de presas suele comportar la destrucción de las planas de inundación, hecho que genera una desestabilización ecosistémica que desencadena en un desastre ecológico. A pesar de esto, el pronóstico no es tan catastrófico como parece. Actualmente se pueden tomar medidas e implementar estrategias para minimizar los efectos negativos de las presas, y en este artículo revisamos las más importantes.
