1. Community Activation to TrAnsform Local sYSTems (CATALYST): A Qualitative Study Protocol

Abstract: Community Health Workers, promotores, and navigators (henceforth, CHWs) emerged as critical members of the public health workforce addressing social, economic, and health inequities worsened by the COVID-19 pandemic. While there is increased appreciation for and utilization of CHW models, and recognition of the importance of tailoring and innovating these models during the pandemic, few studies have examined the processes of change by which CHW models operated during the COVID-19 pandemic and factors that facilitated or constrained CHW health equity efforts. This protocol paper describes and reflects on the research methodology used in our qualitative study focused on CHWs. The CATALYST study aims to examine the roles that CHWs served during the COVID-19 pandemic and facilitators and barriers related to CHW health equity strategies. This qualitative study incorporates the lived experiences of CHWs, low-income communities of color whom CHWs engaged, and institutional representatives and policymakers familiar with locally implemented CHW models during the pandemic. Through a community-based participatory research process, this study involves an abductive qualitative approach to data collection and analysis. We integrate community member expertise alongside CHW and health equity frameworks in designing the research questions and data collection process. Additionally, we use an analytic approach that combines inductive (drawn from qualitative data) and deductive codes (drawn from theoretical frameworks and practice-based evidence integrated through a participatory research process) and nimbly leverages flexible coding to address inductive themes and practice-based questions. Our collaborative process offers concrete strategies to develop qualitative research protocols with community partners, with evidence used to inform policy, programmatic, and relational changes to support and amplify CHW models to promote community health and health equity.

Abstract: El impacto que los enemigos naturales producen sobre las plagas de cultivos es un factor importante a tener en cuenta en el momento de evaluar los métodos de control a ser aplicados. En este contexto, la fauna propia de una región y su influencia sobre una plaga en particular son datos relevantes. Este articulo reporta la ocurrencia de parasitoides asociados de manera natural al gusano cogollero Spodoptera frugiperda (Smith, 1797), en el Paraguay, y su impacto porcentual, calculado en la base de cría de larvas del hospedero colectados durante el período 2015 - 2016 en los departamentos de Caaguazú, Alto Paraná, Canindeyú e Itapúa. 4. Design and simulation of a non-resonant antenna: waveguide slot antenna for 30 GHz

Abstract: The main contribution of this article is the design and simulation of a non-resonant slot antenna based on wave guides to operate in the range of microwaves. The design is done using MATLAB and the Chebyshev synthesis to determine the distribution of current in each element. Based on this information, the size and distribution of the elements are calculated. Different design conditions are simulated, such as: changes in the level from main lobe to secondary and dierent number of slots at one frequency. The slots are interleaved with respect to the axis of the waveguide and separated by a distance (d). To determine its dimensions, it is necessary to deduce the conductances of the array. A particular design case is compared with simulations from the software CST Microwave Studio. It is observed that radiation diagrams are very similar in both number and amplitude of the lobes, validating the tool used.

Abstract: The Santo Nino ore body at Pinos Altos Mine has been in production since 2009 using both open pit and underground mining methods. Between 2015 and 2017, an extensive study evaluated extraction strategy of the Santo Nino crown pillar. The Agnico Eagle project team evaluated geomechanical, operational, safety and economical risks before developing a comprehensive plan to extract the Santo Nino crown pillar by underground stoping. The geomechanical aspects of that study focused on interpreting in situ rock mass conditions and understanding the mechanical and hydrological/hydrogeological mechanisms influencing ground reaction during open pit and underground mine operations to make meaningful forward predictions for crown pillar extraction. Numerical modeling played a significant role in making forward predictions. To minimize uncertainty in those predictions, the model parameters were calibrated using measured pit slope deformations, back analysis of historical slope instabilities and underground observations of rock mass conditions. Ultimately, it was decided that long-hole stoping would be utilized for crown pillar extraction. The pit floor would be used as a drilling horizon, and the upper most level of the underground operations served as the mucking horizon. Various safety and operational risk mitigation strategies were developed and implemented; this included geomechanical monitoring using the in-pit radar system and numerous borehole extensometers installed underground to monitor ground deformations. The calibrated numerical model results provided valuable input for the design of the instrumentation program, and once the crown pillar came into production in late 2017, the instrumentation provided valuable data for model verification and stope design adjustments.

Abstract: Partiremos de una primera reflexión: la propuesta de Mediación Policial requiere el planteamiento de que estamos ante una forma de ser y querer hacer las cosas, así, las capacidades personales son importantes y necesarias pero se requerirá, además, de una capacitación, o sea se trata de una actitud que precisa de una aptitud. Hemos de ser conscientes de que el futuro próximo puede requerir especialización en dos tipos de Policía: una Policía experta, especializada en grupos organizados, ciberdelincuentes, terrorismo… y otra Policía dedicada a la solución de conflictos y al desarrollo del capital social; es opinión común que una política penal de “cero tolerancia” tiene profundas limitaciones. Pero lo que también parece cierto es que la ciudadanía hoy demanda una Policía que, en general, sea sensible ante la necesidad de considerar la labor policial comunitaria como una estrategia más dentro de un modelo de salud pública, una Policía moderada en sus actuaciones que sabe emplear la autoridad necesaria y no más, que tiene gran capacidad de trato con el ciudadano, tacto y capacidad de mediación en los conflictos. De esta forma, la Mediación Policial, en sentido estricto, es una técnica de solución de conflictos...

Abstract: Qualificació obtinguda: 9 L’edifici objecte d’aquest projecte es troba al Quartier Européen de la ciutat de Brussel·les. Amb vora d’un milió d’habitants, la capital de Bèlgica és alhora també la principal seu administrativa de la Unió Europea. Prèviament a l’establiment de totes les institucions europees, el barri era eminentment residencial, però durant la segona meitat del segle XX amb l’arribada de les primeres institucions europees, el plantejament urbanístic es va anar allunyant dels paradigmes tradicionals. Ha desaparegut la caixa del carrer, característica de la ciutat de Brussel·les i els edificis s’han anat allunyant de l’alineació de façana per generar grans espais previs d’accés a les diferents delegacions de govern i altres institucions administratives. La topografia actual del Chaussée d’Etterbeck, que ressegueix el camí d’una antiga riera, és bruscament inferior a la de les vies principals properes a l’emplaçament, com la Rue de la Loi i la Rue de Belliard. Aquest fet, juntament amb l’arribada del ferrocarril, han fet que els grans edificis institucionals lluitin per assolir una relació més directa amb les cotes superiors de les avingudes principals, col·locant-se sobre pedestals. Existeix certa reticència de la ciutadania cap a aquesta arquitectura moderna, que en l’intent de dotar mitjançant grans volumetries la nova identitat visual d’una emergent UE, ha desembocat en la creació d’un centre administratiu que molts consideren aïllat de la resta de la ciutat. Molts edificis, amb el seu caràcter objectual, s’han oblidat de respondre a la complexitat de cada emplaçament i a l’escala humana de l’arquitectura. És per això que el Quartier Européen té una gran deficiència d’espais exteriors de relació acotats entre edificis. Així, el solar ocupa un lloc on xoquen tres realitats ben diferents: la construcció tradicional de gra petit; l’eixample d’edificis de mitjana alçada i finalment els edificis objecte de gran escala. El projecte, a la confluència d’aquestes tres realitats, afronta el repte actuant com a element integrador: continua amb l’amplada i l’alçada dels edificis d’escala petita, però és capaç també de respondre a la gran escala amb la seva claredat i forma. En aquest projecte, es tractarà de respondre a la complexitat de l’emplaçament, així com a la diversitat del programa, tot donant la representativitat que requereix construir una ambaixada per un nou Estat d’Europa, i utilitzant un llenguatge arquitectònic llunyà als edificis objecte i a les grans pantalles de vidre i acer. El projecte pren com a punt de partida l’aprofitament de la topografia com a eina de segregació d’accessos i d’espais exteriors. L’edifici, sense oblidar la seva naturalesa institucional, resulta opac a l’exterior i es desplega cap a la obertura del seu pati interior. Es generen dos espais, que es conjuguen i relacionen amb un canvi de nivell: l’espai públic i l’espai privat. D’una banda al pati interior a cota -0,47 m s'entra des de l’antic camí SUD-NORD Chaussée d’Etterbeck, conté un gran porxo d’entrada amb l’accés al públic aliè (tramitació de visats, exposicions temporals, etc). Per altra banda, la plaça superior a cota +3,60 m del carrer Toulouse, de caràcter residencial, acollirà l’accés controlat de les oficines, de les delegacions oficials i de l’ús privat de l’ambaixador i del seu servei. Aquests dos espais previs a l’accés seran el punt central de treball del projecte. L’edifici, com una cinta, delimita l’espai propi de l’ambaixada alhora que estableix una relació amb la ciutat que l’acull. L’edifici, entès com un únic volum encarregat de cosir el teixit residencial amb l’escala de la ciutat, funciona com un edifici lineal amb diverses entrades (segons l’usuari), i amb l’ajut dels nuclis, s’estratifica en alçada. Així, els espais susceptibles d’adquirir un ús públic, es situen a la planta baixa a cota +0,00, amb el pati com a element de relació. L'edifici s'organitza en una planta baixa amb caràcter públic i obert sobre la que es genera un edifici amb tres zones clarament diferenciades: la zona de l'ambaixador, la zona de servei i les oficines. La zona oest, formant mitgera amb el barri residencial, conté les dependències més compartimentades; en planta primera l'accés de l'ambaixador i de les recepcions, així com una zona de seguretat; la zona de les recepcions (amb la seva cuina) en planta segona; i a partir de la planta tercera es desplega la zona de la vivenda oficial i del convidat. En planta tercera es troba l'apartament del convidat (amb accés independent) i la zona de nit de l'ambaixador, a la qual s'accedeix des de la següent planta. La planta quarta es composa per la zona de dia de l'ambaixador; i finalment, sota coberta hi trobem un petit estudi i un local d'instal·lacions. La part nord i est, conté el cos d’oficines, amb les zones d’atenció al públic en planta primera i les zones de treball intern en plantes segona i tercera. Entre aquestes dues zones abans esmentades, hi trobem la zona de servei, composta per la vivenda i una zona de menjador per al servei; i en planta segona una zona d'instal·lacions. La plaça superior a cota +3,60 m es destinarà a l’accés de l’ambaixador i de les delegacions oficials; la càrrega/descàrrega de mercaderies utilitzen un accés independent des del carrer residencial. La plaça, com a relació/confrontació simbòlica del teixit residencial amb l’escala de la ciutat de grans edificis del districte europeu, es genera amb el buit central, i es consolida amb la relació de "balcó de l'amabaixador" sobre l'espai públic. L’edifici, tot i ser de tipologia lineal, es formalitza a base de plecs i girs en les zones de canvi d'ús. Per la seva condició d'edifici lineal es pren la decisió de fer participar l'estructura del tancament de façana, eliminant-la de l'espai interior; no s'amaga, tot al contrari, aquesta ajuda a crear l'atmosfera interior. Aquesta tipologia estructural descansa sobre la planta baixa, que a nivell estructural s'entén com una base de formigó. Es tria com a punt de partida un mòdul d'1,20 generador de façana, el doble (2,40m) com a llum estructural. Tots els elements de l'edifici, des del paviment fins a les plaques de coberta, seguiran aquesta lògica modular. Com a resposta a la voluntat de tancar-se a l'exterior -creant així una sensació de seguretat-, i d'obrir-se a l'interior; hi ha dos tipus de façanes clarament diferenciades: La façana exterior (carrer) és opaca en la seva majoria, a excepció de petites zones. Aquesta façana permet: - Generar una sensació de seguretat. - Crear un "mural" de llum gràcies a les petites obertures. Aquest mural que durant el dia es veu a l'interior, generarà un efecte de llum molt atractiu a l'exterior durant la nit. La façana interior (pati) és vidriada, i es planteja com una doble pell, permetent d'aquesta manera: - Crear una façana contínua, amagant les singularitats de les diferents zones, que únicament tindran una repercussió a la façana interior - Establir relacions canviants entre interior i exterior segons l’acabat de cada pell i, per tant, trobar diferents nivells de privacitat - Aprofitar l’aire contingut en la càmera com a sistema passiu (utilitzant-lo com a preescalfament), per poder millorar el comportament climàtic general de l’edifici. Per últim, la coberta de l’edifici projectada com l’última façana del projecte, la qual serà present des dels gratacels d’oficines que envolten l’emplaçament, es formalitza –de la mateixa manera que la façana- a base de plecs i girs. A la coberta se li fan uns talls, per a generar les claraboies de les oficines i serveis per a permetre l'entrada de llum; generant així una coberta a dos nivells, el segon dels quals ofereix una continuïtat a tota la coberta Com a punt final, destacar que el projecte es centra en resoldre el conflicte existent entre l’arquitectura tradicional del teixit residencial i l’aparició de grans volums d’oficines. Sense voler renunciar per complet a cap dels dos llenguatges, acaba per buscar el compromís entre representativitat i funcionalitat.

Abstract: This paper presents the application of a new multi-purpose facility for electromagnetic tests to the measurement of the influence of radomes on the radiation of an antenna. The antenna under test is part of a project for mounting a small, low-cost, low-power electronic system in the pod of an unmanned aerial vehicle. The shape of the radome is restricted to the pod physical specifications, but several materials could be tested to assess its effect on the antenna, and to select the best fitted for this particular application. La mayoría de los especialistas están de acuerdo en que es una vía alternativa que puede contribuir a encontrar soluciones a problemas que por las vías tradicionales no pueden resolverse. En la actualidad existen distintos ámbitos de mediación: mediación familiar, civil, mercantil, educativa, intercultural, penal, internacional, policial, sanitaria...etc. El objetivo es formar en la mediación como un instrumento de conciliación y resolución de conflictos.

Abstract: FASANT is a computer tool for the analysis of antennas on-board satellites, ships, aircraft, and other complex bodies. The structure under analysis, which can be metallic or dielectric (with and/or without losses), must be modeled by plane and/or curved surfaces. The geometrical input files are in DXF format, and can be generated by the most commonly used computer-aided geometrical-design (CAGD) tools. The code can also be applied to the analysis of arrays and arbitrarily shaped reflectors. The kernel of the code is based on the uniform theory of diffraction (UTD). Special algorithms have been developed to speed up the ray-tracing computation for both flat and curved surfaces. FASANT can obtain far-field patterns, field levels at points near the structure, can calculate the mutual coupling between antennas or between array elements, and can show each ray-tracing mechanism.

Inicialmente, los alumnos que asistían al Laboratorio cumplían el requisito de haber realizado el máster o estar realizándolo en el momento, posteriormente fueron integrándose licenciados y estudiantes de licenciatura o grado vinculados con la mediación como Psicología y Derecho, también participaron otros alumnos que habían realizado másters sobre mediación. El Laboratorio se diseñó con dos objetivos, de una parte, ofrecer a los alumnos la posibilidad de poder ejercitar su formación de forma práctica y supervisada por los profesores del máster. De otra, atender a familias a las cuales la mediación pudiera serles útil para gestionar o resolver algún conflicto, para cubrir este objetivo se han realizado diversas acciones de coordinación con diversos servicios, eventualmente, interesados en colaborar. En cada curso escolar han participado una media de 15 alumnos, las sesiones se han venido realizando con una periodicidad quincenal, con una elevada regularidad en la asistencia a las sesiones, lo que ha sido un indicador de la satisfacción percibida, esta información ha sido confirmada por las evaluaciones realizadas por los alumnos. A partir del curso 2011-12, en colaboración con la Fundación Universitat Empresa se formaliza el postgrado Curso de especialización Laboratorio de familia, entrenamiento y formación en potenciación y mediación familiar con una duración de 20 ECTS, como consecuencia la oferta se limita a licenciados y diplomados. En la formación se incluye la participación en las sesiones presenciales, preparación de las mismas, puesta en común acerca del diagnóstico, pronóstico y estrategias a implementar, así como visión de vídeos y valoración de los mismos, lecturas recomendadas, estudio y elaboración de un trabajo final.

Abstract: A heuristic solver such as particle swarm optimization (PSO) is used for single snapshot direction of arrival (DOA) estimation of a set of signals impinging on a uniform linear array (ULA). A performance analysis for infinite signal-to-noise ratio (SNR) shows the validity of this approach and an example using signal and ULA parameters from a more realistic DOA problem is also included. Abstract: The application of a time-domain electromagnetic simulation code to a realistic and challenging problem like the high-resolution range profile (HRRP) generation of an inlet cavity model is presented in this paper. HRRPs can be used to accomplish noncooperative target identification (NCTI) of aircrafts by means of radar, and the database needed in this technique tends to be populated with predicted data obtained with software tools. Most codes employed with these purposes are frequency domain methods, which need multiple simulations at different adequately chosen frequencies, as well as some postprocessing, to get a radar signature. Instead, this paper focuses on finite difference time domain (FDTD) analysis of the response of a cavity, because of the relevance of these parts of an aircraft in the overall signature, and numerical results are compared with measurements performed by the authors in an anechoic chamber. This work also shows the advantages of using an electromagnetic code based on time domain for HRRP generation. The authors thank the members of the North Atlantic Treaty Organization SET-085 group on radar signature prediction of cavities on aircraft, vehicles and ships for their helpful discussions. Abstract: A new indoor facility for electromagnetic measurements has been developed and built at the Detectability and Electronic Warfare Laboratory, INTA, Spain. The system was designed to extend the previous capabilities so that not only monostatic but also bistatic radar cross section (RCS) tests could be easily conducted. Due to limited space, far-field radar cross sections in the system are restricted to small targets, but this layout will allow the researchers to investigate ReS near-field-to-far-field transformations. The system was also conceived to be versatile enough to carry out other tests, such as near-field antenna measurements, material-absorption measurements, and electromagnetic characterization of materials. The paper describes the novel concept behind the facility, and presents some preliminary measurement results on different applications. Abstract: This paper presents Radar Cross Section (RCS) measurements of different targets suitable for electromagnetic software comparison and validation. The targets have been designed, fabricated and measured at INTA to study different scattering mechanisms such as reflection and diffraction on curved surfaces and edges (truncated cone), reflection and diffraction on planar surfaces and straight wedges (triangular prism) or tip diffraction and travelling waves (conesphere). These measurements can be used as a valuable tool to validate and adjust the input parameters of electromagnetic prediction codes according to the requirements of the engineer.

Abstract: Adequate characterization of materials allows the engineer to select the best option for each application. Apart from mechanical or environmental characterization, last decades' rise in the exploitation of the electromagnetic spectrum has made increasingly important to understand and explain the behavior of materials also in that ambit. The electromagnetic properties of non-magnetic materials are governed by their intrinsic permittivity or dielectric constant and free-space measurements is one of the various methods employed to estimate this quantity at microwave frequencies. This paper proposes the application of Artificial Neural Networks (ANNs) to extract the dielectric constant of materials from the reflection coefficient obtained by free-space measurements. In this context, two kind of ANNs are examined: Multilayer Perceptron (MLP) and Radial Basis Function (RBF) networks. Simulated materials are utilized to train the networks with and without noise and performance is tested using an actual material sample measured by the authors in an anechoic chamber.

Abstract: Direction of arrival (DOA) estimation is a well known problem involved in different applications. The latest of these applications have greater constraints which make necessary new efforts in estimating the DOA with fewer snapshots. In that sense, this paper presents an iterative method based on particle swarm optimization that solves the problem with a single snapshot. The performance of this algorithm is analyzed and shown with numerical results under different scenarios. Conclusions are also provided. Preprint

Abstract: INSPEC Accession Number: 10499025 The scattering behavior and simulation of cavities are still being studied by several research groups. In this paper, a simplified jet-engine model has been manufactured and measured under different configurations, such as straight or curved blades, different fixed blades position, and free motion capability at adjustable speed. Abstract: Identification of aircrafts by means of radar when no cooperation exists (Non-Cooperative Target Identification, NCTI) tends to be based on simulations. To improve them, and hence the probability of correct identification, right values of permittivity and permeability need to be used. This paper describes a method for the estimation of the electromagnetic properties of materials as a part of the NCTI problem. Different heuristic optimization algorithms such as Genetic Algorithms (GA) and Particle Swarm Optimization (PSO), as well as other approaches like Artificial Neural Networks (ANN), are applied to the reflection coefficient obtained via free-space measurements in an anechoic chamber. Prior to the comparison with real samples, artificial synthetic materials are generated to test the performance of these bio-inspired algorithms.

Abstract: This study reports the findings of research aimed at determining the rate of prevalence of attention deficit hyperactivity disorder (ADHD) in children of school age.The epidemiological study was conducted using a community sample extracted by means of multi stage stratified sampling according to socio economic level and schooling and consisted in 1,141 children of both sexes of school age from the city of Maracaibo. The revised Conners scales were used to collect data.The estimated prevalence of ADHD was 7.19% and we also obtained 0.35% for the hyperactive type, 1.14% for the disattentional subtype, and 5.70% for the combined type. Contrary to what was expected, prevalence was higher for females. It was found that 7.45% of the general sample scored higher on the academic problems scales, whereas for the sample identified as having ADHD, academic problems were 50% and the comorbidity between ADHD and academic problems was confirmed.The estimates for prevalence found in this study are consistent with those reported in the literature, which suggests that ADHD is a valid diagnosis for Marabino children.

Abstract: Two different kinds of cookies were prepared: ``polvorón`` and ``pasta seca``, using commercial corn (Zea mays L.) starch and another one isolated from banana (Musa paradisiaca), as a technological alternative source for this fruit, which is sub-used. Starch level of the products were the same while processing conditions were as similar as possible. An affective test was carried out with people using a numbered hedonic scale. The results were analyzed with the t-student and non-parametric Wilcoxon statistical tests. ´´Polvorón´´ cookies did not show significant difference (p0.059) between the two starch sources. ´´Pastas secas`` cookies were significantly different; those elaborated with corn starch showed better characteristics than made with banana starch. Besides, a difference (paired comparison) test was used to evaluate some sensorial and textural parameters between ´´polvorones´´ and ´´pastas secas´´. In this case, c2 test showed differences in odor and flavor between starch sources for each type of product. The texture profile indicated a similarity between sources of starch for roughness, ruggedness and adhesiveness in ´´polvorones´´ and ´´pastas secas´´. Bananas starch may be a good substitute of corns starch in these food types.

Abstract: In epidemiological investigation valid, reliable means of diagnosing the illnesses studies are essential.To validate the self-questionnaire 'Alcoi 1995' for the diagnosis of migraine.We used the questionnaire Alcoi-1992 (which had been validated previously) to find the prevalence of migraine in homogeneous populations. After this, we selected the significant items to obtain a shorter, more specific questionnaire for diagnosis of migraine, which we called 'Alcoi 1995'. We selected a population of patients and healthy volunteers. The diagnosis of migraine (neurologist and self-questionnaire) was made on criteria of the International Headache Society (IHS, 1988).There were 71 persons included in the study, 40 patients (5 men and 35 women those average age was 39.7 +/- 14.4 years) and 31 healthy volunteers (15 men and 16 women with an average age of 36 +/- 14.4). The sensitivity, specificity, positive and negative predictive value, and the Kappa concordance index for migraine (100%, 100%, 100%, 100% and 1); migraine with no aura (75%, 100%, 100%, 86% and 0.79) and migraine with a typical aura (100%, 88%, 63%, 100% and 0.71) were calculated.Our results show a high degree of validity and reliability when the self-questionnaire is used for the diagnosis of migraine. It is a tool for use in epidemiological studies which is quick and easy to use, and is cheap.

Amongst the many reasons for this would seem to be the use of questionnaires for screening populations, absent or incorrect validation of these questionnaires and the use of impracticable diagnostic criteria.To determine the prevalence of migraine in a homogeneous population using the self-questionnaire 'Alcoi 1995' which had previously been validated for the diagnosis of migraine. A self-questionnaire was given to all workers in the same company. Diagnostic criteria of the International Headache Society (IHS, 1988) were used for diagnosis of migraine.The study included 140 persons (43.6% men and 56.4% women with an average age of 26.7 +/- 4.8 years). There was a prevalence of headache and migraine (IC 95%): 90.8% (75.01-100) and 24.3% (16.3-32.3) respectively. The sex prevalence of headache and migraine was 88.5% (88.3-88.8) and 11.5% (3.4-19.4) for men and 34.2% (21.5-47) for women respectively.There are a large number of studies of the prevalence of migraine with wide variability in the figures obtained (1-2 at 35%). These variations depend on the population studied, age and sex sampled, diagnostic criteria used and the method employed in the survey. After validation of the questionnaire for diagnosis of migraine, this should be used in homogeneous populations before use in broad population studies. Our findings support the use of a suitably validated questionnaire as a useful method for the diagnosis of migraine in epidemiological studies.

Abstract: Nowadays, quality of life is considered to be important both in clinical investigation and patient care. Migraine is a particularly disabling illness, with a pronounced effect on general health and on the ability to take part in social activities. We review the instruments used for measuring quality of life and the published studies in which the relationship between migraine and quality of life is analyzed.In spite of increasing interest in quality of life and the means by which this is measured, there are many problems involved in studying this correctly since there is no agreement as to its definition or how to measure it. In recent years, most of the studies carried out have assessed the socio-economic cost of headache. However, at present, studies tend to focus on the human cost of the quality of life when suffering from headache. Migraine patients have a poorer quality of life than the general population and other populations with chronic illnesses which have always been considered to be severely disabling.Research should be aimed at finding the specific and/or generic measures necessary to improve quality of life while suffering from headache. In the future, the aim of new treatments should be to eliminate the pain of the acute crisis and restore functional capacity, reduce the period of disability and improve quality of life.

Abstract: A group of universities not related with health sciences were interviewed using the questionnaire 'Alcoi 1992', based on the operational diagnostic criteria of the International Headache Society (IHS), to evaluate the prevalence of migraine. This questionnaire for the diagnosis of migraine was validated in 1993, showing a high sensibility and specificity for the diagnosis of migraine. Sensitivity, specificity, predictive value and chance-corrected agreement rate for the diagnosis of migraine was 100%, 94%, 90% (PVpos), 1008 (PVneg) and 0.71 respectively. The study group was composed by 96 students, men and women; the mean age of the group was 21 years old. The prevalence of headache, migraine, tensional headache and cluster headache was 95.4%; 7.58%; 12.2% and 1.52% cases respectively. The prevalence for male of headache, migraine, tensional headache and cluster headache was 92.5%, 12.5%, 15.9% and 2.5% cases respectively. The prevalence for female of headache, migraine, tensional headache and cluster headache was 100%; 0%; 5.5% and 0% cases respectively. The questionnaire appears like an useful, fast and easy method for the evaluation of diagnosis of migraine in populations groups.

Abstract: A group of medical students were interviewed using the 'Alcoi 1992' questionnaire based on the diagnostic criteria of the International Headache Society (IHS) in order to assess the prevalence and various characteristics of migraine. This questionnaire for headache diagnosis was approved in 1993, showing both high sensitivity and exactness in diagnosing migraine. Sensitivity, exactness, predictive value and agreement index of the questionnaire in migraine diagnosis were 100%, 94%, 90% (positive) or 100% (negative) and 0.71% respectively. The group was made up of 96 students of both sexes, with an average age of 23.3 years (standard deviation 1.87). A total of 92 students, 35 male and 57 female, reported headaches. The total prevalence rate for headache was 95.8%, the reliability interval (RI) being 76.7-114.9. In the case of female students, the prevalence rate for headache was 96.6% (RI 71.7-121.5) and in the case of male students it was 94.6% (RI 63-125.7). A total of 20 students, six male and 14 female, fulfilled the criteria for migraine. The overall prevalence rate for migraine was 20.8% (RI 11.9-29.7), the prevalence for females being 23.7% (RI 11.6-35.8) and that for males 16.2% (RI 3.7-28.7). The questionnaire would appear to be a useful, rapid and simple method in the assessment of migraine diagnostic.

Abstract: Migraine is a paroxysmic abnormality in which asymtomatic periods alternate with the appearance of attacks. Such attacks are the end result of a chain of events leading on to the acute clinical syndrome. Amongst those phenomena which occur in the days prior to the attack starting, the factors which bring such attacks on have been widely studied by a great number of researchers. Identifying these initiating factors is a fundamental preventive element and looking into the behavioural mechanisms of such factors could prove useful in clarifying the pathogenic mechanisms of migraine. In the present study we review most of the works which have sought to identify these factors concerning the development of attacks and to work out their behavioural patterns.

Abstract: Throughout the many studies carried out on patients suffering from migraine, we find that a series of diseases or abnormalities with cardiovascular etiologies may be associated with the risk of migraine. In some studies published so far there is a greater prevalence of distinct cardiovascular diseases among those patients suffering from migraine. The shortage of case-control studies which might otherwise definitely establish a relationship between migraine and other cardiovascular diseases leads us to cautiously interpret the meaning of these associations. We studied family and personal history of cardiovascular disease in a group of patients with migraine, and we carried out a case-control study to try to establish the greatest risk of suffering from these diseases by our patients. We found that family antecedents of migraine with the next of kin was a risk factor for getting migraine and that Raynaud's disease was associated with a greater risk of suffering with this pathology.

40. [Profile of the migrainous patients according to drug intakes]

Abstract: Migraine headache is a common, recurrent and often incapacitating disorder resulting in a substantial loss of work days and productivity, significant consumption of medicines, and considerable economic cost. We performed this study in order to determine the characteristic features of migrainous patients with regular or frequent intake of ergotamine preparations and non steroidal anti-inflammatory drugs (NSAIDs) and the existence of significant differences between both groups. 186 patients were included in the study, 36.6% suffering from migraine without aura and 63.4% from migraine with aura. 24.7% were males and 75.3% females. Mean age of the sample was 35.6 +/- 12.9 years. With regard to medication used for immediate relief of headache, 74.7% patients reported an intake of analgesics and 25.3% ergotamine preparations. In patients with analgesic intake the mean age was significantly lower than in patients with ergotamine for treatment of migraine. When clinic features of these two headache groups were examined, the duration of migraine attacks in the analgesic group was significantly higher (p = 0.0014) than in the ergotamine group, and the severity of migraine was lower (p0.05) than in this group. Among associated symptoms, nausea and vomiting were significantly less frequent (p = 0.024) in the analgesic group than in the group of patients with ergotamine intake.

Abstract: Knowing the epidemiology of migraine is important for a host of reasons. Migraine is one of the most prevalent types of headache. In addition, it constitutes a pathology which due to its cyclic, recurring and highly incapacitating characteristics has enormous repercussions on the quality of life of those affected; it is responsible for the loss of an important number of working days and of production; responsible too for the vast consumption of medicaments and consequently represents a considerable economic cost. An epidemiological study, however, of its pathology poses countless difficulties owing to its characteristics and its association with risk factors. In this present work we review the obstacles to carrying out migraine epidemiological studies and we also comment upon many such studies found in the literature. In each section we present the data obtained in such studies as were performed by our working group.

Abstract: The behavior of brain flow on acute stress has not been previously evaluated. Transcranial Doppler ultrasonography (TD) is a bloodless method to evaluate the speed of cerebral flow in the arteries of the Willis polygon. The present study was designed to analyze the changes which occur in the same during two situations of acute stress.The mean speed of cerebral flow (MS) and the pulsation rate (PR) of the right and left middle cerebral artery (MCA) were measured basally and following a mental stress test (calculation) and physical stress (cold test) by TD and through the temporal window. Twenty-five healthy volunteers (18 women and 8 men) with a mean age of 27.8 +/- 7.3 years were studied.In response to mental stress an increase was observed in MS in both the right MCA (10 +/- 8.6 cm/sec) and the left MCA (10.4 +/- 8.7 cm/sec) with a decrease in the MCA (0.14 +/- 0.23 in the right MCA, 0.14 +/- 0.18 cm/sec in the left MCA). In response to the cold test an increase in MS (7.3 +/- 7.5 cm/sec in the right MCA, 14.8 +/- 14.7 cm/sec in the left MCA) was also observed with a decrease in the PR (0.2 +/- 0.2 in the right MCA and 0.2 +/- 0.16 in the left MCA). No significant differences were observed in the changes induced in the right or left artery or in those induced by the mental or cold tests.These results suggest that the acute stress produces an increase in cerebral flow in the arteries of the Willis polygon.

Abstract: Introduction Headache is one of the commonest causes of consultation in neurology. There are many studies of the prevalence of migraine showing considerable variation in the results obtained. Objectives To find the prevalence of migraine, with and without an aura, by means of a randomized transverse study carried out in a previously selected rural population, using the validated questionnaire 'Alcoi-92'. To find the overall prevalence of migraine, specifically regarding age and sex and adjusted for the European population. Patients and methods A door-to-door study was made by random selection of 790 persons aged over 18, in three towns in the comarca del Comtat, Alacant, Spain. A self-questionnaire was sent to all persons studies and subsequently they were seen by a medical interviewer. Results We interviewed 548 persons (overall response rate 78%). The average age was 52.5 +/- 19.3 years. Sex distribution of the population interviewed showed a predominance of females (52.9%) as compared to males (47.1%). According to the type of headache: other types of headache 62.32%, persons with no previous history of headache 20.07%, amplified migraine 16.6%, typical migraine 12.6%, cluster type migraine 0.18%. Conclusions The overall prevalence of amplified migraine was 16.6%. According to diagnostic groups the frequency of migraine with aura was 2.9%, migraine without aura 9.7% and typical migraine 12.6%. A predominance of women was seen in all types of migraine. The frequency of migraine, adjusted for the European population was 19.6%. Prevalence during the past year, as a measure of activity of the disorder was 15.7%.

Abstract: INTRODUCTION Headache is one of the commonest reasons for consultation and it might be useful to know its characteristics in order to determine which patients may need medical attention. OBJECTIVES To carry out a randomized transverse study in a rural population using the validated questionnaire 'Alcoi-92', and to determine the clinical characteristics of the headaches detected. PATIENTS AND METHODS A door to door survey was done by randomized selection of 790 persons aged over 18 and living in the Comarca del Comtat. A validated self-questionnaire was sent and this was followed by an interview with a doctor. RESULTS Five hundred and forty-eight persons were interviewed. The standard prevalence of migraine is 19.6% with a sex incidence of 1:2.4 and an average age of 46.1 years. The average age of onset was 20.64 years. There were family histories of cerebrovascular accidents in 26.4% and of migraine in 57.1%. There were no differences in the use of toxic substances, of socio-economic status or of marital status. The commonest pre-existing pathology was arterial hypertension. The number of episodes of headache was one to seven per year in 50% of those with migraine, with an average duration of 4-24 hours, predominantly bilateral presentation and pulsatile in character with moderate repercussion in everyday activities. The commonest associated symptoms were photophobia and sonophobia. There were prodromal symptoms in 36.9%, most commonly confusion and photopsy. CONCLUSION The questionnaire 'Alcoi-92' has been shown to be a useful tool for definition of the clinical characteristics of migraine.

Abstract: La mediación es una disciplina joven y su uso es cada vez más extendido como una alternativa de otras soluciones para la resolución de conflictos. La mayoría de los especialistas están de acuerdo en que es una vía alternativa que puede contribuir a encontrar soluciones a problemas que por las vías tradicionales no pueden resolverse. En la actualidad existen distintos ámbitos de mediación: mediación familiar, civil , mercantil, educativa, intercultural, penal, internacional, policial, sanitaria...etc. El objetivo es formar en la mediación como un instrumento de conciliación y resolución de conflictos.

49. Informatización de la atención primaria. Experiencia de implantación en un área de salud mediante la metodología de gestión por procesos

Abstract: INTRODUCCIÓN: La gestión por procesos se ha revelado como una herramienta bastante útil de cara a los planteamientos de reingeniería y análisis organizativo de la realidad de los centros de atención primaria. La implantación de un proyecto de informatización integral de la atención primaria pasa por la incorporación de los principios básicos de la excelencia y la modificación en la definición de esta organización dentro del equipo de atención primaria(1-4). OBJETIVO: Presentar un proyecto de implantación de la informatización en un área de salud, dentro del plan de informatización integral de la atención primaria, Abucasis II, de la Agencia Valenciana de Salud (AVS), Sistema de Información Ambulatorio (SIA). DISEÑO: Metodología de la gestión por procesos(5,6). EMPLAZAMIENTO: Atención primaria. Centros de salud dependientes del Departamento 19, Alicante. PARTICIPANTES: Miembros de la Unidad de Calidad y formación (UCyF) de Atención Primaria, departamento 19. INTERVENCIONES: Tras analizar la situación real respecto al proceso de informatización de los distintos centros de salud y para garantizar la operatividad de la implantación del proyecto Abucasis II, se ha definido un organigrama funcional con 5 niveles de responsabilidad y con actividades específicas, desde el nivel 1 «coordinador del proyecto » hasta el nivel 5 «profesionales de los centros de salud», pasando por informático de departamento responsables de subprocesos, documentalistas, coordinadores de los centros y «másters». Siguiendo la metodología de gestión por procesos, se ha definido en primer lugar el mapa de procesos. Entre los procesos estratégicos (gestión) se han incluido el programa de atención al cliente (organigrama funcional, gestión de la información y comunicación); la voz del cliente (externointerno); el programa de formación; las guías y normas de actuación; la documentación apoyo (externa e interna de la UCyF), y la definición de objetivos (pactados con los coordinadores y con la dirección de atención primaria). En los procesos de soporte (estratégicos) se han incluido: la coordinación de docencia; el mantenimiento y la supervisión del proceso externo (en 3 ámbitos: a) hardware, software, aplicativo SIA-GAIA-RNV-SIP; b) dependientes de la oficina del SIA, y c) Bull, C&G, CATS, servicio de informática de la AVS); los procesos internos, como la gestión de la documentación interna (inventario, custodia, informes), la actualización de los usuarios y el registro, el almacén y el suministro (papel, reciclaje, mobiliario, accesorios hardware, redes), la logística y la implantación. En el proceso clave (operativos) se han incluido: la programación CRC y las agendas; la atención al cliente interno (comunicación horizontal, comunicación ascendente y descendente); la docencia; la utilización SIAMOS (cita previa, cita segundo nivel, gestoría externa, procesos administrativos repetidos); la utilización SIA-historia clínica (atención: médica, enfermería, trabajadores sociales y unidades de apoyo); la utilización de procesos administrativos repetidos (CLT/IT), el subproceso de traspaso de información (papel/informatizado,); así como el subproceso de identificación de propuestas de mejora según el sistema externo de soporte. MEDICIONES PRINCIPALES: Se ha analizado el cumplimiento del cronograma definido por la UCyF, desde su fase de implantación hasta la de producción, considerando las intervenciones de carácter técnico como las de formación, mantenimiento y registro de incidencias. RESULTADOS: La implantación del proyecto se realizó de forma progresiva y estructurada según el esquema propuesto y su cronograma. La implicación de todos los profesionales ha sido clave para conseguir la fase de despliegue global del circuito. A los 10 meses del inicio del proyecto se ha realizado un total de 51 cursos de formación en aula informática, con una asistencia del 30-40% de los profesionales de los distintos centros de salud del departamento, con diferentes porcentajes de asistencias por estamentos y nivel de desarrollo de la aplicación SIA. Se han realizado 92 intervenciones locales en centros informatizados y se ha identificado un total de 85 sugerencias de mejora. En la actualidad, la población atendida en centros SIA en producción es del 33,84% (Comunidad Valenciana, 30,01%), con 84.578 de un total de 249.967 habitantes. DISCUSIÓN Y CONCLUSIONES: El proceso desarrollado ha permitido mejorar el rendimiento y la efectividad de la implantación del proyecto en cuanto a los procesos estratégicos y de soporte. Las dificultades más importantes encontradas se han relacionado con los resultados del proceso clave «utilización de la herramienta por los profesionales».
