Abstract

We report on the first measurement of exclusive single-photon muoproduction on the proton by COMPASS using 160GeV/c polarised μ+ and μ− beams of the CERN SPS impinging on a liquid hydrogen target. We determine the dependence of the average of the measured μ+ and μ− cross sections for deeply virtual Compton scattering on the squared four-momentum transfer t from the initial to the final proton. The slope B of the t-dependence is fitted with a single exponential function, which yields B=(4.3±0.6stat|sys−0.3+0.1)(GeV/c)−2. This result can be converted into a transverse extension of partons in the proton, 〈r⊥2〉=(0.58±0.04stat|sys−0.02+0.01±0.04model)fm. For this measurement, the average virtuality of the photon mediating the interaction is 〈Q2〉=1.8(GeV/c)2 and the average value of the Bjorken variable is 〈xBj〉=0.056. Abstract

Abstract Single hadron azimuthal asymmetries of positive and negative hadrons produced in muon semi-inclusive deep inelastic scattering off longitudinally polarised deuterons are determined using the 2006 COMPASS data and also combined all deuteron COMPASS data. For each hadron charge, the dependence of the azimuthal asymmetry on the hadron azimuthal angle $$\phi $$ ϕ is obtained by means of a five-parameter fitting function that besides a $$\phi $$ ϕ -independent term includes four modulations predicted by theory: $$\sin \phi $$ sinϕ , $$\sin 2 \phi $$ sin2ϕ , $$\sin 3\phi $$ sin3ϕ and $$\cos \phi $$ cosϕ . The amplitudes of the five terms have been extracted, first, for the hadrons in the whole available kinematic region. In further fits, performed for hadrons from a restricted kinematic region, the $$\phi $$ ϕ -dependence is determined as a function of one of three variables (Bjorken-x, fractional energy of virtual photon taken by the outgoing hadron and hadron transverse momentum), while disregarding the others. Except the $$\phi $$ ϕ -independent term, all the modulation amplitudes are very small, and no clear kinematic dependence could be observed within experimental uncertainties.

Abstract

Abstract Background Depression is a commonly occurring disorder linked to diminished role functioning and quality of life. The development of treatments that overcome barriers to accessing treatment remains an important area of clinical research as most people delay or do not receive treatment at an appropriate time. The workplace is an ideal setting to roll-out an intervention, particularly given the substantial psychological benefits associated with remaining in the workforce. Mobile health (mhealth) interventions utilising smartphone applications (apps) offer novel solutions to disseminating evidence based programs, however few apps have undergone rigorous testing. The present study aims to evaluate the effectiveness of a smartphone app designed to treat depressive symptoms in workers. Methods The present study is a multicentre randomised controlled trial (RCT), comparing the effectiveness of the intervention to that of an attention control. The primary outcome measured will be reduced depressive symptoms at 3 months. Secondary outcomes such as wellbeing and work performance will also be measured. Employees from a range of industries will be recruited via a mixture of targeted social media advertising and Industry partners. Participants will be included if they present with likely current depression at baseline. Following baseline assessment (administered within the app), participants will be randomised to receive one of two versions of the Headgear application: 1) Intervention (a 30-day mental health intervention focusing on behavioural activation and mindfulness), or 2) attention control app (mood monitoring for 30 days). Participants will be blinded to their allocation. Analyses will be conducted within an intention to treat framework using mixed modelling. Discussion The results of this trial will provide valuable information about the effectiveness of mhealth interventions in the treatment of depressive symptoms in a workplace context. Trial registration The current trial is registered with the Australian and New Zealand Clinical Trials Registry (ACTRN12617000547347, Registration date: 19/04/2017).

Abstract

We present a new amplitude analysis of the ηπ D-wave in the reaction π−p→ηπ−p measured by COMPASS. Employing an analytical model based on the principles of the relativistic S-matrix, we find two resonances that can be identified with the a2(1320) and the excited a2′(1700), and perform a comprehensive analysis of their pole positions. For the mass and width of the a2 we find M=(1307±1±6) MeV and Γ=(112±1±8) MeV, and for the excited state a2′ we obtain M=(1720±10±60) MeV and Γ=(280±10±70) MeV, respectively.

Abstract

The Sivers function describes the correlation between the transverse spin of a nucleon and the transverse motion of its partons. For quarks, it was studied in previous measurements of the azimuthal asymmetry of hadrons produced in semi-inclusive deep inelastic scattering of leptons off transversely polarised nucleon targets, and it was found to be non-zero. In this letter the evaluation of the Sivers asymmetry for gluons is presented. The contribution of the photon–gluon fusion subprocess is enhanced by requiring two high transverse-momentum hadrons. The analysis method is based on a Monte Carlo simulation that includes three hard processes: photon–gluon fusion, QCD Compton scattering and the leading-order virtual-photon absorption process. The Sivers asymmetries of the three processes are simultaneously extracted using the LEPTO event generator and a neural network approach. The method is applied to samples of events containing at least two hadrons with large transverse momentum from the COMPASS data taken with a 160 GeV/c muon beam scattered off transversely polarised deuterons and protons. With a significance of about two standard deviations, a negative value is obtained for the gluon Sivers asymmetry. The result of a similar analysis for a Collins-like asymmetry for gluons is consistent with zero.

8. eHealth interventions for the prevention of depression and anxiety in the general population: a systematic review and meta-analysis

Abstract

Abstract Background Anxiety and depression are associated with a range of adverse outcomes and represent a large global burden to individuals and health care systems. Prevention programs are an important way to avert a proportion of the burden associated with such conditions both at a clinical and subclinical level. eHealth interventions provide an opportunity to offer accessible, acceptable, easily disseminated globally low-cost interventions on a wide scale. However, the efficacy of these programs remains unclear. The aim of this study is to review and evaluate the effects of eHealth prevention interventions for anxiety and depression. Method A systematic search was conducted on four relevant databases to identify randomized controlled trials of eHealth interventions aimed at the prevention of anxiety and depression in the general population published between 2000 and January 2016. The quality of studies was assessed and a meta-analysis was performed using pooled effect size estimates obtained from a random effects model. Results Ten trials were included in the systematic review and meta-analysis. All studies were of sufficient quality and utilized cognitive behavioural techniques. At post-treatment, the overall mean difference between the intervention and control groups was 0.25 (95% confidence internal: 0.09, 0.41; p = 0.003) for depression outcome studies and 0.31 (95% CI: 0.10, 0.52; p = 0.004) for anxiety outcome studies, indicating a small but positive effect of the eHealth interventions. The effect sizes for universal and indicated/selective interventions were similar (0.29 and 0.25 respectively). However, there was inadequate evidence to suggest that such interventions have an effect on long-term disorder incidence rates. Conclusions Evidence suggests that eHealth prevention interventions for anxiety and depression are associated with small but positive effects on symptom reduction. However, there is inadequate evidence on the medium to long-term effect of such interventions, and importantly, on the reduction of incidence of disorders. Further work to explore the impact of eHealth psychological interventions on long-term incidence rates.

Abstract

Eight proton transverse-spin-dependent azimuthal asymmetries are extracted in four regions of the photon virtuality Q2 from the COMPASS 2010 semi-inclusive hadron measurements in deep inelastic muon–nucleon scattering. These Q2 regions correspond to the four regions of the di-muon mass Q2 used in the ongoing analyses of the COMPASS Drell–Yan measurements, which allows for a future direct comparison of the nucleon transverse-momentum-dependent parton distribution functions extracted from these two alternative measurements. In addition, for the azimuthal asymmetries induced by the Sivers transverse-momentum-dependent parton distribution function various two-dimensional kinematic dependences are presented. The integrated Sivers asymmetries are found to be positive with an accuracy that appears to be sufficient to test the sign change of the Sivers function predicted by Quantum Chromodynamics.

Abstract

Final results are presented from the inclusive measurement of deep-inelastic polarised-muon scattering on longitudinally polarised deuterons using a 6LiD target. The data were taken at 160 GeV beam energy and the results are shown for the kinematic range 1(GeV/c)24GeV/c2 in the mass of the hadronic final state. The deuteron double-spin asymmetry A1d and the deuteron longitudinal-spin structure function g1d are presented in bins of x and Q2. Towards lowest accessible values of x, g1d decreases and becomes consistent with zero within uncertainties. The presented final g1d values together with the recently published final g1p values of COMPASS are used to again evaluate the Bjorken sum rule and perform the QCD fit to the g1 world data at next-to-leading order of the strong coupling constant. In both cases, changes in central values of the resulting numbers are well within statistical uncertainties. The flavour-singlet axial charge a0, which is identified in the MS‾ renormalisation scheme with the total contribution of quark helicities to the nucleon spin, is extracted at next-to-leading order accuracy from only the COMPASS deuteron data: a0(Q2=3(GeV/c)2)=0.32±0.02stat±0.04syst±0.05evol. Together with the recent results on the proton spin structure function g1p, the results on g1d constitute the COMPASS legacy on the measurements of g1 through inclusive spin-dependent deep inelastic scattering.

Abstract

Precise measurements of charged-kaon multiplicities in deep inelastic scattering were performed. The results are presented in three-dimensional bins of the Bjorken scaling variable x, the relative virtual-photon energy y, and the fraction z of the virtual-photon energy carried by the produced hadron. The data were obtained by the COMPASS Collaboration by scattering 160 GeV muons off an isoscalar 6LiD target. They cover the kinematic domain 1(GeV/c)25 GeV/c2 in the invariant mass of the hadronic system. The results from the sum of the z-integrated K+ and K− multiplicities at high x point to a value of the non-strange quark fragmentation function larger than obtained by the earlier DSS fit.

Abstract

Abstract Using a novel analysis technique, the gluon polarisation in the nucleon is re-evaluated using the longitudinal double-spin asymmetry measured in the cross section of semi-inclusive single-hadron muoproduction with photon virtuality $$Q^2>1~(\mathrm{GeV}/c)^2$$ Q 2 > 1 ( GeV / c ) 2 . The data were obtained by the COMPASS experiment at CERN using a 160 GeV/c polarised muon beam impinging on a polarised $$^6$$ 6 LiD target. By analysing the full range in hadron transverse momentum $$p_\mathrm{T}$$ p T , the different $$p_\mathrm{T}$$ p T -dependences of the underlying processes are separated using a neural-network approach. In the absence of pQCD calculations at next-to-leading order in the selected kinematic domain, the gluon polarisation $$\Delta g/g$$ Δ g / g is evaluated at leading order in pQCD at a hard scale of $$\mu ^2= \langle Q^2 \rangle = 3 (\mathrm{GeV}/c)^2$$ μ 2 = ⟨ Q 2 ⟩ = 3 ( GeV / c ) 2 . It is determined in three intervals of the nucleon momentum fraction carried by gluons, $$x_\mathrm{g}$$ x g , covering the range $$0.04 \!

Abstract

Since the birth of the first cloned animal, Dolly the sheep, concerns have been raised about potential long-term health consequences of cloning. Here the authors report on a cohort of 13 aged cloned sheep, including four created from the same cells as Dolly, and find they are healthy and seem to age normally.

Abstract

In the fragmentation of a transversely polarized quark several left–right asymmetries are possible for the hadrons in the jet. When only one unpolarized hadron is selected, it exhibits an azimuthal modulation known as the Collins effect. When a pair of oppositely charged hadrons is observed, three asymmetries can be considered, a di-hadron asymmetry and two single hadron asymmetries. In lepton deep inelastic scattering on transversely polarized nucleons all these asymmetries are coupled with the transversity distribution. From the high statistics COMPASS data on oppositely charged hadron-pair production we have investigated for the first time the dependence of these three asymmetries on the difference of the azimuthal angles of the two hadrons. The similarity of transversity induced single and di-hadron asymmetries is discussed. A new analysis of the data allows quantitative relationships to be established among them, providing for the first time strong experimental indication that the underlying fragmentation mechanisms are all driven by a common physical process.

Abstract

New results for the double spin asymmetry A1p and the proton longitudinal spin structure function g1p are presented. They were obtained by the COMPASS Collaboration using polarised 200 GeV muons scattered off a longitudinally polarised NH3 target. The data were collected in 2011 and complement those recorded in 2007 at 160 GeV, in particular at lower values of x. They improve the statistical precision of g1p(x) by about a factor of two in the region x≲0.02. A next-to-leading order QCD fit to the g1 world data is performed. It leads to a new determination of the quark spin contribution to the nucleon spin, ΔΣ, ranging from 0.26 to 0.36, and to a re-evaluation of the first moment of g1p. The uncertainty of ΔΣ is mostly due to the large uncertainty in the present determinations of the gluon helicity distribution. A new evaluation of the Bjorken sum rule based on the COMPASS results for the non-singlet structure function g1NS(x,Q2) yields as ratio of the axial and vector coupling constants |gA/gV|=1.22±0.05 (stat.)±0.10 (syst.), which validates the sum rule to an accuracy of about 9%.

Abstract

When designing playable media with non-game orientation, alternative play scenarios to gameplay scenarios must be accompanied by alternative mechanics to game mechanics. Problems of designing playable media with non-game orientation are stated as the problems of designing a platform for creative explorations and creative expressions. For such design problems, two requirements are articulated: 1) play state transitions must be dynamic in non-trivial ways in order to achieve a significant level of engagement, and 2) pathways for players’ experience from exploration to expression must be provided. The transformative pathway from creative exploration to creative expression is analogous to pathways for game players’ skill acquisition in gameplay. The paper first describes a concept of simulation-based interface, and then binds that concept with the concept of dynamic score. The former partially accounts for the first requirement, the latter the second requirement. The paper describes the prototype and realization of the two concepts’ binding. “Score” is here defined as a representation of cue organization through a transmodal abstraction. A simulation based interface is presented with swarm mechanics and its function as a dynamic score is demonstrated with an interactive musical composition and performance.

Abstract

Deregulation of interleukin-6 (IL-6) expression caused the synthesis and release of many inflammatory mediators. It is involved in chronic inflammation, autoimmune diseases, and malignancy. Stephania tetrandra S. Moore is a Chinese medicinal herb which has been used traditionary as a remedy for neuralgia and arthritis in China. To investigate the anti-inflammatory effects of S. tetrandra S. Moore in vitro and in vivo, its effects on the production of IL-6 and inflammatory mediators were analysed. When human monocytes/macrophages stimulated with silica were treated with 0.1–10 μg/ml S. tetranda S. Moore, the production of IL-6 was inhibited up to 50%. At these concentrations, it had no cytotoxicity effect on these cells. It also suppressed the production of IL-6 by alveolar macrophages stimulated with silica. In addition, it inhibited the release of superoxide anion and hydrogen peroxide from human monocytes/macrophages. To assess the anti-fibrosis effects of S. tetrandra S. Moore, its effects on in vivo experimental inflammatory models were evaluated. In the experimental silicosis model, IL-6 activities in the sera and in the culture supernatants of pulmonary fibroblasts were also inhibited by it. In vitro and in vivo treatment of S. tetrandra S. Moore reduced collagen production by rat lung fibroblasts and lung tissue. Also, S. tetrandra S. Moore reduced the levels of serum GOT and GPT in the rat cirrhosis model induced by CCL4, and it was effective in reducing hepatic fibrosis and nodular formation. Taken together, these data indicate that it has a potent anti-inflammatory and antifibrosis effect by reducing IL-6 production.

Abstract

In the quantum version of a Trojan-horse attack, photons are injected into the optical modules of a quantum key distribution system in an attempt to read information direct from the encoding devices. To stop the Trojan photons, the use of passive optical components has been suggested. However, to date, there is no quantitative bound that specifies such components in relation to the security of the system. Here, we turn the Trojan-horse attack into an information leakage problem. This allows us to quantify the system security and relate it to the specification of the optical elements. The analysis is supported by the experimental characterization, within the operation regime, of reflectivity and transmission of the optical components most relevant to security.

Abstract

Tumour necrosis factor-α (TNF-α) is a major proinflammatory cytokine inducing the synthesis and release of many inflammatory mediators. It is involved in immune regulation, autoim mune diseases, and inflammation. Our previous study demonstrated that acanthoic acid, (-)-pimara-9(11), 15-dien19-oic acid, a pimaradiene diterpene isolated from Acanth opanax koreanum, inhibited TNF-α production. To extend our understanding of inhibitory effects of acanthoic acid on TNF-α production, its effects on TNF-α gene expression was tested. Based on the results from RT-PCR and promoter analysis of TNF-α, it was found that acanthoic acid suppressed TNF-α gene expression. But the same concentration of acanthoic acid had no effect on IL-6 gene expression. Haptoglobin is an acute phase protein which is induced by TNF-α. When liver cells were treated with acanthoic acid, haptoglobin synthesis was blocked by acanthoic acid. These data confirmed that acanthoic acid inhibited gene expression and biological function of TNF-α.

Abstract

Quantum key distribution (QKD) uniquely allows the distribution of cryptographic keys with security verified by quantum mechanical limits. Both protocol execution and subsequent applications require the assistance of classical data communication channels. While using separate fibers is one option, it is economically more viable if data and quantum signals are simultaneously transmitted through a single fiber. However, noise-photon contamination arising from the intense data signal has severely restricted both the QKD distances and secure key rates. Here, we exploit a novel temporal-filtering effect for noise-photon rejection. This allows high-bit-rate QKD over fibers up to 90 km in length and populated with error-free bidirectional Gb/s data communications. With a high-bit rate and range sufficient for important information infrastructures, such as smart cities and 10-Gbit Ethernet, QKD is a significant step closer toward wide-scale deployment in fiber networks.

Abstract

The advent of sensitive enhanced culture (metaculturomic) and culture-independent DNA-based (metagenomic) methods has revealed a rich collection of microbial species that inhabit the human urinary tract. Known as the urinary microbiome, this community of microbes consists of hundreds of distinct species that range across the entire phylogenetic spectrum. This new knowledge clashes with standard clinical microbiology laboratory methods, established more than 60 years ago, that focus attention on a relatively small subset of universally acknowledged uropathogens. Increasing reports support the hypothesis that this focus is too narrow. Single uropathogen reports are common in women with recurrent urinary tract infection (UTI), although wider disruption of their urinary microbiome is likely. Typical “UTI” symptoms occur in patients with “no growth” reported from standard culture and sometimes antibiotics improve these symptoms. Metaculturomic and metagenomic methods have repeatedly detected fastidious, slow growing, and/or anaerobic microbes that are not detected by the standard test in urine samples of patients with lower urinary tract symptoms. Many of these microbes are also detected in serious non-urinary tract infections, providing evidence that they can be opportunistic pathogens. In this review, we present a set of poorly understood, emerging, and suspected uropathogens. The goal is to stimulate research into the biology of these microbes with a focus on their life as commensals and their transition into pathogens

Abstract

A dual thermistor radiosonde (DTR) comprising two (aluminium-coated and black) sensors with different emissivities was developed to correct the effects of solar radiation on temperature probes based on in situ radiation measurements. Herein, the DTR performance is characterised in terms of the uncertainty via a series of ground-based facilities and an intercomparison radiosounding test. The DTR characterisation procedure using laboratory facilities is as follows: individually calibrate the temperature of the thermistors in a climate chamber from −70 to 30 ∘C to evaluate the uncertainty of raw temperature measurement before radiation correction; test the effect of temperature on the resistance reading using radiosonde boards in the climate chamber from −70 to 20 ∘C to identify a potential source of errors owing to the boards, especially at cold temperatures; individually perform radiation tests on thermistors at room temperature to investigate the degree of heating of aluminium-coated and black sensors (the average ratio = 1 : 2.4) and use the result for obtaining unit-specific radiation correction formulas; and perform parameterisation of the radiation measurement and correction formulas with five representative pairs of sensors in terms of temperature, pressure, ventilation speed, and irradiance using an upper air simulator. These results are combined and applied to the DTR sounding test conducted in July 2021. Thereafter, the effective irradiance is measured using the temperature difference between the aluminium-coated and black sensors of the DTR. The measured irradiance is then used for the radiation correction of the DTR aluminium-coated sensor. The radiation-corrected temperature of the DTR is mostly consistent with that of a commercial radiosonde (Vaisala, RS41) within the expanded uncertainty (∼ 0.35 ∘C) of the DTR at the coverage factor k = 2. Furthermore, the components contributing to the uncertainty of the radiation measurement and correction are analysed. The DTR methodology can improve the accuracy of temperature measurement in the upper air within the framework of the traceability to the International System of Units.

Background/Aims: Conflicting results have been reported regarding the interaction between proton pump inhibitors (PPIs) and clopidogrel. We investigated whether concomitant PPI use influenced the risk of recurrence in patients with stroke and myocardial infarction (MI). Methods: This study used two databases for two different designs, the Korean National Health Insurance Service (NHIS) database for a self-controlled case series design, and the national sample cohort of the NHIS data base converted to the Observational Medical Outcomes Partnership-Common Data Model version for a cohort study based on large-scale propensity score matching. Results: In the PPI co-prescription group, recurrent hospitalization with stroke occurred in 17.6% of the 8201 patients with history of stroke, and recurrent MI occurred in 17.1% of the 1216 patients with history of MI within1 year. According to the self-controlled case series, the overall relative risk (RR) of recurrent stroke was 2.09 (95% confidence interval (CI); 1.83–2.38); the RR showed an increasing trend parallel to the time from the beginning of PPI co-prescription. In the cohort study, there was a higher incidence of recurrent stroke in the PPI co-prescription group (Hazard ratio (HR): 1.34, 95% CI: 1.01–1.76, p = 0.04). The overall RR of recurrent MI was 1.47 (95% CI; 1.02–2.11) in the self-controlled case series; however, there was no statistically significant difference in recurrent MI in the cohort study (HR:1.42, 95% CI:0.79–2.49, p = 0.23). The impact of individual PPIs on stroke and MI showed different patterns. Conclusions: A PPI co-prescription >4 weeks with clopidogrel was associated with hospitalization of recurrent stroke within 1 year of initial diagnosis; however, its association with recurrent MI remains inconclusive. 34. Health status of children left behind in rural areas of Sichuan Province of China: a cross-sectional study

Abstract

Abstract Background In recent decades, many workers from rural areas in China migrated to urban cities in search of a better livelihood. Due to the household registration policy and other financial barriers, more than 40 million of children were left behind in their rural home by their migrated parents in 2015. In this cross-sectional study, we aimed to investigate the influence of being left behind on these children’s physical and mental health. Methods A self-administered questionnaire was completed by participants about their demographic background and health status. Chi-square Test was conducted to investigate the influence. Results A total of 1662 participants responded and completed all the questions in the questionnaire. Significant differences existed between left-behind children group and non-left-behind children group on several health issues such as not going to school due to sickness (p = 0.080), completeness of the vaccination scheme (p = 0.036) and feeling of loneliness (p = 0.039). However, regarding symptoms like fever, cough or respiratory difficulties, diarrhea and twitch, as well as mental health problems like unhappiness and insomnia, no significant difference was found. Gender difference was also demonstrated showing that girls were more vulnerable than boys to certain symptoms and emotional problems. Conclusion This study indicated that both being left-behind and gender had an impact on the children’s health. It is necessary to further reform the household registration system to improve rights of equal access to employment, education and health resources for workers and their children from rural areas in China.

38. Therapeutic Potential of Sitafloxacin as a New Drug Candidate for Helicobacter Eradication in Korea: An In Vitro Culture-Based Study

Abstract

Background: Increased prevalence of antibiotic resistance to Helicobacter pylori (H. pylori) infection worldwide has driven the search for a new therapeutic candidate. Recently, sitafloxacin, a novel 4-quinolone agent, has emerged as a new therapeutic option for H. pylori eradication, in Japan. However, data on its efficacy for H. pylori eradication in Korea are limited. Therefore, we aimed to investigate the therapeutic potential of sitafloxacin as a first-line treatment for patients with Helicobacter infection through gastric tissue culture-based studies. Materials and Methods: We prospectively enrolled treatment-naïve patients with H. pylori infection who visited the Gil Medical Center between March 2015 and March 2018. After obtaining written informed consent from patients, a total of 121 H. pylori strains were collected. We tested the susceptibility of these strains to sitafloxacin, and other antibiotics for Helicobacter eradication, including clarithromycin (CLR), metronidazole (MTZ), amoxicillin (AMX), tetracycline (TET), levofloxacin (LEV), and ciprofloxacin (CIP) using the agar dilution technique. The minimum inhibitory concentration (MIC) of these antibiotics against H. pylori strains were determined. Results: None of the H. pylori strains obtained were resistant to sitafloxacin (MIC > 1, n = 0), while other conventional eradication drugs including CLR, MTZ, AMX, and TET showed 24.8% (n = 30), 30.6% (n = 37), 5.0% (n = 6), and 0.8% (n = 1) resistance, respectively. Compared to the resistance rates of other quinolones (LEV [36.4%, n = 44] and CIP [37.2%, n = 45]), sitafloxacin showed the best antibiotic performance against Helicobacter strains (0%, n = 0). Furthermore, sitafloxacin also inhibited the growth of 14 H. pylori strains (12.4%), which were resistant to both of clarithromycin, and metronidazole, and 27 strains (22.3%) with multidrug resistance. Conclusions: Sitafloxacin might be a new promising candidate for Helicobacter eradication where antibiotic resistance for Helicobacter is an emerging medical burden, such as in Korea.

Abstract

Vaccine-induced immune thrombotic thrombocytopenia (VITT) is a severe prothrombotic complication of adenoviral vaccines, including the ChAdOx1 nCoV-19 (Vaxzevria) vaccine. The putative mechanism involves formation of pathological anti–platelet factor 4 (PF4) antibodies that activate platelets via the low-affinity immunoglobulin G receptor FcγRIIa to drive thrombosis and thrombocytopenia. Functional assays are important for VITT diagnosis, as not all detectable anti-PF4 antibodies are pathogenic, and immunoassays have varying sensitivity. Combination of ligand binding of G protein–coupled receptors (protease-activated receptor-1) and immunoreceptor tyrosine–based activation motif–linked receptors (FcγRIIa) synergistically induce procoagulant platelet formation, which supports thrombin generation. Here, we describe a flow cytometry–based procoagulant platelet assay using cell death marker GSAO and P-selectin to diagnose VITT by exposing donor whole blood to patient plasma in the presence of a protease-activated receptor-1 agonist. Consecutive patients triaged for confirmatory functional VITT testing after screening using PF4/heparin ELISA were evaluated. In a development cohort of 47 patients with suspected VITT, plasma from ELISA-positive patients (n = 23), but not healthy donors (n = 32) or individuals exposed to the ChAdOx1 nCov-19 vaccine without VITT (n = 24), significantly increased the procoagulant platelet response. In a validation cohort of 99 VITT patients identified according to clinicopathologic adjudication, procoagulant flow cytometry identified 93% of VITT cases, including ELISA-negative and serotonin release assay–negative patients. The in vitro effect of intravenous immunoglobulin (IVIg) and fondaparinux trended with the clinical response seen in patients. Induction of FcγRIIa-dependent procoagulant response by patient plasma, suppressible by heparin and IVIg, is highly indicative of VITT, resulting in a sensitive and specific assay that has been adopted as part of a national diagnostic algorithm to identify vaccinated patients with platelet-activating antibodies.

In Korea, the rate of Helicobacter pylori (H. pylori) eradication has declined steadily as a result of increasing resistance to antibiotics, especially dual resistance to clarithromycin and metronidazole. However, microbiological culture data on drug-resistant H. pylori is lacking. This study evaluated the antimicrobial efficacy of candidate antibiotics against resistant H. pylori strains. Methods. Purpose We sought to investigate whether a deep learning-based algorithm to estimate the AI ECG-heart age using standard 12-lead ECGs predicted mortality and cardiovascular outcomes. Methods We trained and validated a deep neural network using raw ECG digital data from 425,051 12-lead ECGs acquired between January 2006 and December 2021. The network performed a holdout test using a separate set of 97,058 ECGs. Additionally, randomly age-sex matched patients with reduced and preserved ejection fraction (EF) were compared. Results The deep neural network was trained to estimate the AI ECG-heart age (the mean absolute error 5.8 ± 3.9 years and R squared of 0.7 (r=0.84, p Conclusion The biological heart age estimated by AI had a significant impact on mortality and MACE. Those data suggested that the AI ECG-heart age might facilitate the primary prevention and health care for cardiovascular outcomes.

45. Artificial intelligence-enhanced electrocardiography for predicting severity prognosis in patients with COVID-19

Background There were no artificial intelligence (AI) tools to predict the severity and prognosis of patients hospitalized with COVID-19 using initial electrocardiography (ECG). Purpose We assessed whether AI using 12-lead ECG could assist in the early determination of COVID-19 severity and predict prognosis of hospitalized patients. Methods Patients diagnosed with COVID-19 via polymerase chain reaction and admitted to our institution during February 2020–October 2021 were enrolled. We identified 642 patients (mean age, 56±19 years; 41.7% male) who underwent 12-lead ECGs within 2 days of admission for COVID-19. Patients were divided according to severity: mild-to-moderate illness (oxygen or low-flow oxygen therapy not required) and severe-to-critical illness (required high-flow oxygen, invasive mechanical ventilation, or extra corporeal membrane oxygenation). Results Severe-to-critical illness occurred in 243 (37.8%) patients, including 23 (3.6%) in-hospital mortalities. Experimental results using our developed AI enhanced ECG demonstrated better outcomes for mild-to-moderate illness based on an area under the curve (AUC), sensitivity, specificity, and F1 score of 0.71, 0.95, 0.70, and 0.81, respectively. The AUC for detecting severe-to-critical illness was 0.71 (95% confidence interval [CI]: 0.58– 0.82; sensitivity: 0.70; specificity: 0.35; F1 score: 0.35; and overall accuracy: 0.70). When analyzing individuals without COVID-19, the AI algorithm for identifying COVID-19 showed an AUC of 0.85 (95% CI: 0.82–0.88; sensitivity: 0.60; specificity: 0.84; F1 score: 0.70; overall accuracy: 0.85). Conclusion AI-enhanced ECG could reasonably predict severity in patients hospitalized for COVID-19, suggesting that AI-ECG assists in the early determination of COVID-19 severity, helping physicians improve patient management.

Funding Acknowledgements Type of funding sources: None. Background Apical hypertrophic cardiomyopathy (HCM) is considered to have a benign prognosis in terms of cardiovascular mortality. Sudden cardiac arrest and ventricular fibrillation (VF) in patients with apical HCM is rarely reported. Methods Between July 2001 and May 2021, 96 HCM patients who had undergone implantable cardioverter-defibrillator (ICD) implantation from three tertiary hospitals were reviewed. We evaluated the prevalence of aborted sudden cardiac death and/or documented VF in apical HCM and known risk markers for sudden cardiac death in those populations. Result Fifteen patients (15.6%) were apical HCM in total population. Among apical HCM, seven patients (40.0%) presented with aborted sudden cardiac death or documented VF, therefore received ICD implantation for the secondary prevention. All seven patients had at least one risk marker suggested either 2020 ACC/AHA guideline or 2014 ESC guideline. One patient had three risk factors and two patients had two risk factors and four patients had one risk factor. Late gadolinium enhancement (LGE) on cardiac magnetic resonance (CMR), personal history of unexplained syncope and non-sustained ventricular tachycardia (VT) were the common risk factors. Conclusions Clinical outcomes in patients with apical HCM are not always as benign as previously thought. ICD implantation should be considered in high-risk patients with apical HCM as the same manner with the septal type of HCM according to the current guideline recommendations.

The real-world using trend of ILR implantation and indication were studied using nationwide database. Purpose To assess the indication of ILR and diagnostic rate of meaningful arrhythmia Method We analyzed the 1,460 patients who underwent ILR implantation (male 62%, mean age 62±16) from 2008 to 2019 using Korea Health Insurance review and assessment service database. Result The embolic stroke of undetermined source (ESUS) group, syncope group and palpitation group were 619 (42.3%), 765 (52.3%), 76 (5.2%) as implantation indication, respectively. From 2016 to 2019, the ESUS group increased by 14 times and the syncope group by 4 times. Among patients with ESUS, 191 patients (31%) were diagnosed with atrial fibrillation within 1 year. Bradycardia needed pacing therapy was revealed in 28%, tachycardia in 6% and atrial fibrillation in 24% in syncope group. The predictors of atrial fibrillation in ESUS group were age (OR: 1.024 per year, CI: 1.006-1.042, p=0.0072) and heart failure (OR: 2.215, CI:1.357-3.615, p=0.0015). The mean time from ILR to pacing therapy was 3±2 months in syncope group and that to anticoagulation in ESUS group was 6±3 months. Discussion The use of ILR for evaluation of ESUS is increasing, with atrial fibrillation diagnostic rate of 30%. The time to treatment differed by group.

Introduction Obstructive sleep apnea (OSA) is associated with left atrial (LA) remodeling, which increases the thromboembolic risk of atrial fibrillation (AF) and recurrence of atrial fibrillation after catheter ablation of AF. This study aimed to assess the association between severity of OSA and LA remodeling. Method In 126 patients with AF who underwent catheter ablation for AF (male 78.4% male, 58.0 ± 10 years old, persistent AF 27%), LA voltage distribution were analyzed during atrial pacing. All the subjects underwent polysomnography for screening of OSA. We categorized the subjects into 4 groups based on the severity of OSA which was determined by the respiratory disturbance index (RDI; RDI 1,250 ng/l, presence of spontaneous echo contrast [SEC] and LA volume index >32 ml/m3) were evaluated. Result The LA low voltage area was significantly greater (p=0.007) and LA volume measured by cardiac magnetic resonance imaging (MRI) was significantly larger with the severity of OSA (p=0.004). The presence of SEC (3.2% vs. 13.6%; p=0.201) and LAA pulse wave velocity < 20cm/sec (3.2% vs. 9.6%; p=0.454) were higher in patients with moderate to severe OSA but it did not reach statistical significance. In the multivariate regression analysis, the total number of the stoke risk factors other than CHA2DS2-VASc was significantly associated with RDI (β=0.191; p=0.037). Conclusion The severity of OSA has significant relationship with stroke risk factors.

49. Burnout in healthcare workers in COVID-19-dedicated hospitals

Abstract

Background Considering the prolongation of the COVID-19 pandemic, the lack of studies on burnout, particularly in healthcare workers, needs to be addressed. This report aimed to identify the risk factors of burnout by comparing the level of burnout between nurses in general wards and those in COVID-19-dedicated wards in a national university hospital. Methods A survey based on the Korean version of Burnout Assessment Tool (BAT-K) was conducted on nurses between 10 January and 31 January 2022. The BAT-K consists of exhaustion, mental distance, cognitive impairment, emotional impairment and secondary symptoms. Results A total of 165 nurses, including 81 nurses from the COVID-19-dedicated ward, completed the questionnaire. The percentage of general-ward nurses with an emotional impairment score above the clinical cutoff was higher than that of COVID-19 ward nurses. General ward compared to the COVID-19 ward increased the risk of presenting with total-core symptoms. Two factors increased the risk regarding mental distance: short career length and underlying disease. Conclusions In contrast to previous studies, the risk of burnout in the COVID-19-ward nurses was lower than that of the general ward nurses. The risk regarding mental distance was correlated with short career length and presence of an underlying disease.
