Funding Acknowledgements Type of funding sources: None. Background The involvement of apolipoprotein B (apoB) in atherogeneis is well known because of its role in the metabolism of lipids. Elevated levels of apoB have been found not only to be correlated with clinical atherosclerosis, but also with sub-clinical forms. Cardiovascular risk can be estimated based on clinical risk scores. However, the main limitation of these risk score models is to estimate only traditional risk factors and not other important factors such as apoB. Purpose To assess the interactive effects of apoB in asymptomatic patients, and its link to cardiovascular risk. Methods This prospective observational study included 811 asymptomatic participants. According to the median apoB value (set at 130 mg/ dl) the study population was divided into two groups as follows: group 1 – 713 patients with low apoB values and group 2 – 98 patients with high apoB. Also, for each patient personal data, such as family history of premature cardiovascular disease, smoker status, personal history of heart failure or atherosclerotis were obtained. Regarding the subclinical atherosclerosis the carotid thickness of intima-media was measured. Results Patients with high apoB are more frequent males (p=0.008) with a mean age of 57.6 ± 11.9 years old (p Conclusions Apolipoprotein B level is correlated with subclinical atherosclerosis and worse outcomes expressed by a higher lipid profile and risk of diabetes melitus development. These findings highlighted that the determination of the apoB is therefore mandatory for a better stratification of cardiovascular risks in primary prevention of atherosclerosis.

Background Periodontal disease (PD) is an inflammatory condition that increases cardiovascular risk and favours development of acute coronary events; however, the association of periodontal pathogens and inflammatory markers in patients with acute coronary syndromes (ACS) has not been elucidated so far. Purpose the aim of this study was to evaluate the association between periodontal pathogens of the red and orange complex and inflammatory biomarkers in patients with ACS. Methods Sixteen patients with ACS and concomitant PD were enrolled in the present study. All patients underwent a complex dental examination, including DNA sampling collection from the periodontal pocket. The study population was divided into two groups according to the presence of germs from red, respectively orange complex: group 1 – 9 patients with germs from red complex (RC group) and group 2 – 7 patients with germs from red-orange complex (ROC group). Inflammatory status was assessed on the basis of interleukine 6 (IL6), endothelial/ intravascular adhesion molecules (VCAM/ICAM), P-selectin, metalloproteinase (MMP9), Albumin (Ab), Apolipoprotein B (ApoB), C reactive protein (CRP), alkaline phosphatase (AF) and sST2 and lymphocytes to monocytes ratio. Results Germs from red complex (Porphyromonas gingivalis, Tannerella forsythia and Treponema denticola) were more frequent and more expressed compared with those from orange complex (p=0.0008). There was no significant correlation between RC and RCO group and inflammatory markers; however, P-selectin level (185,4 ± 66,73 vs 89,62 ± 40,94 ng/ml, p=0.005) was higher in RC group, while AF level at baseline (47,83 ±12,53 vs 75,72 ±24,42 UI/l, p=0.006) and in day 7 (52,83 ± 6,122 vs 70,95 ± 9,71 UI/l, p =0.01) was more expreesd in ROC group. A significantly higher lymphocytes to monocytes ratio was identified in patients with ROC compared with those with RC only (3,2 ± 1,44 vs 10,94 ± 12,75, p=0.01). Conclusions in patients with ACS and PD, the presence of germs from red complex in the gingival pocket is associated with increased blood vulnerability. The severity of the PD, expressed by the presence of germs of the red complex, is associated with a higher risk of developing acute coronary events.

Background Periodontal disease (PD) is associated with increased inflammation, which can contribute to atheromatous plaque rupture and consequent acute coronary syndrome (ACS). Periostin (Pn) is a new biomarker associated with inflammation, being involved in the complex process of myocardial recovery after an ACS; however, the association between Pn, other inflammatory biomarkers and concomitant PD in patients with ACS has not been elucidated so far. Purpose the aim of this study was to investigate the link between serum levels of Pn, other systemic inflammatory biomarkers and their impact on the clinical evolution following an ACS. Methods this was an observational prospective study in which 92 patients with ACS and concomitant PD were enrolled. Patients were divided into two groups, according to the median value of Pn which was 30,63 ng/ ml: group 1 included 46 patients with low Pn level and group 2 included 46 patients with high Pn level. Inflammatory status was investigated using serum levels of C reactive protein (CRP), interleukin 6 (IL6), endothelial/ intravascular adhesion molecules (VCAM/ICAM), P-selectin, matrixmetalloproteases (MMP9), Albumin (Ab), Apolipoprotein B (ApoB), alkaline phosphatase (AF) and sST2. For Ab, CRP, and AF the values were recorded at baseline and on day 7. Results Patients with high Pn levels presented more frequent STEMI type myocardial infarction (p=0.0004), heart failure (p=0.002) and a longer duration of hospitalization (p=0.008) and stay in the intensive coronary care unit (p=0.004). They also expressed significantly higher levels of MMP 9 (201 ± 69,37pg/ml vs 132,7 ± 112,2 pg/ml, p = 0.003), Ab (4,08 ±0,34 mg/dl vs 3,08 ± 1,46 mg/dl p=0.02), and AF at baseline at day 7 (53,84 ± 16,2 vs 47,36 ± 16,36 UI/l p=0.022; 58,73 ± 29,91 vs 42,56 ± 16,42 UI/l, p=0.0001). Patients in group 2 had also higher levels of triglycerides (p=0.02) and LDL cholesterol (p=0.0008), as well as a higher amount of myocardial necrosis, as expressed by total creatine kinase (p=0.0004) and CK-MB (p=0.0096). However, serum levels of CRP at baseline (9,2 ± 12,87 mg/ dl vs 26,61 ± 25,59 mg/dl, p=0.0009) and at day 7 (19,25 ± 26,77 mg/dl vs 31,84 ± 29,4 mg/dl, p=0.043) were significantly lower in group 2. Conclusions In patients with ACS and PD, increased systemic levels of circulating Pn indicate a larger myocardial infarction and worse clinical outcomes, being also associated with increased levels of matrixmetalloproteases, involved in the myocardial repair.

Purpose We sought to perform a comparative analysis between plaques located at different levels within the coronary tree, to identify the differences in plaque composition, morphology, and vulnerability between the three major coronary branches. Methods We conducted a cross-sectional, observational study on 75 patients with stable coronary artery disease who underwent CCTA for assessment of coronary lesions that exhibited at least one vulnerable plaque (VP) in the coronary tree. After image acquisition, coronary plaque analysis was performed with the use of the Syngo.via Frontier (Siemens) software. Plaque analysis also included evaluation of presence of VM: low attenuation plaque – LAP; napkin ring sign – NRS; spotty calcifications – SC; positive remodeling – PR. VP were defined as lesions that presented at least 1 vulnerability marker (VM). In total, 90 coronary VPs located at the level of the left anterior descending (LAD; n = 30), circumflex (CXA; n = 30) and right coronary artery respectively (RCA; n = 30) were identified and analyzed. Results Lesions located in the RCA presented a significantly higher length (LAD - 18.67± 5.49 vs. CXA - 15.48 ±3.73 vs. RCA - 20.47 ± 5.97 mm, p = 0.001), a higher degree of stenosis (LAD - 57.77 ± 8.62 vs. CXA - 54.50 ± 11.25 vs. RCA - 59.63 ± 10.42 mm, p = 0.022), and were more voluminous (LAD - 187.9 ± 86.03 vs. CXA - 146.9 ± 102.4 vs. RCA - 248.1 ± 11.4 mm3, p = 0.0007) compared to those located in the LAD and CXA, but no difference was observed regarding the remodeling (p = 0.180) or eccentricity indexes (p = 0.423). Plaque composition was also significantly different according to plaque location: calcified volume (LAD - 44.07 ± 63.90 vs. CXA - 12.40 ± 19.65 vs. RCA - 33.69 ± 34.38 mm3, p = 0.002), non-calcified volume (LAD - 143.8 ± 76.02 vs. CXA - 134.5 ± 102.2 vs. RCA - 214.4 ± 99.67 mm3, p = 0.002), lipid rich volume (LAD - 14.95 ± 22.69 vs. CXA - 6.44 ± 13.42 vs. RCA -16.07 ± 15.74 mm3, p = 0.0005), fibrotic volume (LAD - 128.9 ± 66.10 vs. CXA - 128.1 ± 91.56 vs. RCA - 198.3 ± 92.34 mm3, p = 0.003). The highest number of VM per plaque was present in the LAD (LAD - 2.2 ± 0.8 vs. CXA - 1.6 ± 0.7 vs. RCA - 1.8 ± 0.6, p = 0.01), as well as highest rate of VPs (LAD – 80%, CXA – 46.6%, RCA – 70%, p = 0.01). No difference was registered between coronary arteries on the presence of SCs (p = 0.670), NRS (p = 0.455), PR (p = 0.833), but LAPs were more frequently located in the LAD (p = 0.0009). Conclusions Coronary plaques located in the RCA were more voluminous and exhibited a higher volume of lipid rich and non-calcified atheroma. However, compared to the RCA and CXA, the left anterior descending artery presented CPs with a more expressed degree of vulnerability, a higher number of vulnerability markers per plaque, and a higher incidence of LAP.

However, the evolution of CSS after implantation of different types of coronary stents is still under investigation. The aim of this study was to assess the evolution of the CSS along the coronary lesions following implantation of bioabsorbable vascular scaffolds (BVS), to determine the impact of BVS on coronary flow haemodynamics. Methods and results This was a single center prospective pilot study which enrolled 15 patients (aged 58.35 +/- 7.79 years, 13 males) with coronary artery disease undergoing BVS implantation in a major epicardial vessel. In all patients, angio CT scanning (Siemens Somatom Sensation scanner, Erlangen, Germany) was performed prior to the BVS implantation and repeated after 12 months. Lumen information was extracted from the vessels of interest and coronary rest hemodynamics, including CSS, were calculated using a computational fluid dynamics solver. All shear stress calculations were performed at baseline and repeated after 1 year. Average CSS was determined proximally, distally, and at the level of the minimal lumen area (MLA). Average CSS along the stented segment significantly decreased after BVS implantation, from 2.87 +/- 1.64 Pa at baseline to 1.9 +/- 0.49 at 1 year (p = 0.0001). Maximum CSS along the segment also exhibited a significant decrease, from 11.78 +/- 10.06 Pa to 6.35 +/- 3.08 Pa (p = 0.0009). Proximally to the MLA, CSS significantly decreased after BVS implantation, from 3.39 +/- 1.93 Pa at baseline to 1.91 +/- 0.68 Pa at 1 year (p Conclusions Implantation of BVS leads to a significant decrease of CSS after 1 year, especially within coronary segments located proximally to the stenosis. This underlines the role of BVS in re-establishing a physiological pattern of coronary flow in diseased coronary vessels. The feature (mentioned herein) is not commercially available. Due to regulatory reasons its future availability cannot be guaranteed.

PD leads to increased cardiovascular risk and favours development of acute coronary syndromes; however, the association between PD and markers of subclinical atherosclerosis has not been elucidated so far. Purpose To evaluate the interrelation between severity of PD, coronary calcium, subclinical atherosclerosis and plaque vulnerability in patients with unstable angina, who underwent coronary computed tomography angiography (CCTA). Methods Fifty-two patients with unstable angina were enrolled in the ATHERODENT clinical trial (NCT03395041). All patients underwent: (1) complex dental examination for assessment of periodontal status, expressed by the total periodontal index (PI) evaluating the following indices: gingival index, plaque index, tantrum index, furcation index, mobility, loss of attachment, pocket depth and papillary bleeding index, (2) CCTA for analysis of morphology, composition and vulnerability features of the culprit plaques causing myocardial schema. For each patient, coronary calcium score, body mass index, neck circumference, abdominal circumference and Intima-media thickness of the carotid artery were calculated. According to the median value of the total PI (set by 22) the study population was divided into two groups: group 1 included 26 patients with low PI (normal gum or gingivitis) and group 2 included 26 patients with high PI (periodontitis and severe PD). Results In patients with high PI the plaque volume (p = 0.019) and the non-calcified volume (p = 0.002) were more increased compared with patients with low PI. In patients with high risk features in the culprit coronary plaques (positive remodelling, low density atheroma, spotty calcification and napkin ring sign) the severity of PD was more expressed compared with those with low risk plaques (28.20+/-13.34 vs. 18.71+/-11.31, p = 0.001). From all PI indices, loss of gingival attachments (3.6 +/-2.91 vs. 1.66 +/- 1.8, p = 0.009) and papillary bleeding index (4.5 +/-3.06 vs. 2.04+/-1.96, p = 0.002) were significantly correlated with the plaque vulnerability. there was no significant correlation between PI and the markers of subclinical atherosclerosis expressed by the neck circumference, abdominal circumference and IMT of the carotid artery. However, total PI presented significantly correlation with the total coronary artery calcium score (r = 0.45, p = 0.0008). A higher calcium score was identified in patients with high PI compared with those with low PI (505.29 +/-478.64 vs. 93.82+/-233.0, p = 0.0001). Conclusions In patients with vulnerable coronary plaques, subclinical atherosclerosis is not correlated with PI. However, the severity of PD is directly associated with the total calcium score and with a more vulnerable phenotype of the atheromatous coronary plaques.

Funding Acknowledgements PlaqueImage Background The relationship between the degree of pulmonary hypertension (PH) and left ventricular performance in patients with systemic sclerosis is still a controversial issue in the literature. We aimed to conduct a comparative analysis of indexes characterizing left ventricular systolic and diastolic function, in two etiological types of pulmonary hypertension involving different pathophysiological mechanisms: PH caused by systemic sclerosis and PH caused by myocardial ischemia. Material and method We performed a prospective study on 83 patients (36 patients with documented PAH with a systolic pulmonary arterial pressure – sPAP of >35 mmHg and 47 subjects with normal sPAP), out of which group 1 – with systemic sclerosis (n = 48); group 2 – significant coronary artery disease - CAD (n = 35). Patients of each group were divided in two subgroups based on the diagnosis of PH: group 1A - subjects with scleroderma and associated PH (n = 20), group 1B - subjects with scleroderma without PH (n = 28), group 2A - ischemic patients with associated PH (n = 16) and subgroup 2B - patients with ischemic disease without PH (n = 19). Results Patients in group 1 presented a significantly higher number of female subjects (p = 0.001) and a higher mean age (p = 0.009) compared to group 2. Patients with associated PH presented a significantly lower left ventricular ejection fraction (LVEF) compared to those without PH within the ischemic group (p = 0.023). There was a significant inverse correlation between the sPAP and LVEF in ischemic patients (r=-0.52, p = 0.001) as well as for scleroderma patients without PH (r=-0.51, p = 0.04). Tissue Doppler analysis of the left ventricular function indicated a significant negative correlation between the septal E’ value versus the sPAP and lateral E’ value versus the sPAP (r=-0.49, p = 0.002; r=-0.43, p = 0,008). Conclusions Intrinsic myocardial damage plays an important role in left ventricular systolic function even in the absence of PAH. Scleroderma patients present a less pronounced deterioration of the LVEF in response to pulmonary hypertension, indicating that in this group, additional compensatory mechanisms could be involved in the complex response of myocardium to elevated pulmonary pressures.

Restoration and improvement of LV function highly depends on the magnitude of scar formation. Late gadolinium enhancement cardiac magnetic resonance (LGE-CMR) imaging has been validated for an accurate determination of the myocardial scar size and transmurality. The recovery of ST segment elevation is a liable marker of vessel patency following PCI. Purpose The aim of the study was to validate a new integrated score of ST segment elevation score (ISSTE) as a new predictor of the myocardial scar tissue size, in relation with increased inflammatory biomarkers, with the extent of myocardial fibrosis at one month, assessed with LGE-CMR, after STEMI. Methods We included 65 patients with STEMI who underwent urgent revascularization with PCI in the first 12 hours from the onset of symptoms. The ISSTE was determined by summing the ST segment elevation in all registered ECG leads at presentation (ISSTE-1) and at 2 hours (ISSTE-2) after primary PCI. Blood samples were also collected at baseline and day-5 for determination of serum hs-CRP levels. At 1-month follow-up all patients under LGE-CMR (1.5T scanner) for evaluation of the myocardial scar extent (volume, percentage, transmurality). Results ISSTE-2 was significantly correlated with day-5 hs-CRP serum levels (r = 0.546, 95%CI: 0.030-0.832, p = 0.037), although no significant correlations were noted with baseline hs-CRP levels (r= 0.238, p = 0.407). There were no significant correlations between ISSTE-1 score and the myocardial scar percentage (r = 0.241, p = 0.11) or high transmurality volume (r = 0.194, p = 0.21), while ISSTE-2 significantly correlated with myocardial scar mass (r = 0.406, 95%CI: 0.107-0.637 p = 0.007) and high transmurality volume (r = 0.344, 95%CI: 0.0320-0.596, p = 0.0273). The restoration of the ST segment, reflected by the difference between ISSTE-1 and ISSTE -2 is correlated with the infarct size mass (r = 0.336, 95%CI: 0.0307 -0.584, p = 0.027). Conclusion The magnitude of ST segment elevation determined at 2 hours after PCI was associated with the inflammatory response at day 5 after STEMI and it may serve as a predictor for the extent of the myocardial scar tissue determined with LGE-CMR at 1 month following STEMI. The ISSTE-1 score calculated at presentation does not reflect extent of the affected myocardial tissue following PCI.

Analysis of CSS after coronary stent placement is still lacking of large clinical studies. Purpose: The aim of this study was to: (1) test the feasibility of Coronary Computed Tomography Angiography (CCTA) for assessment of local hemodynamic and luminal changes after implantation of bioresorbable vascular scaffolds (BVS), and (2) to investigate CSS modifications after BVS implantation. Methods: We conducted a single center, prospective pilot study on 73 patients with coronary artery disease who underwent elective percutaneous coronary intervention (PCI); group 1 (n = 30) – patients with BVS and group 2 (n = 43) – patients with bare metal stent (BMS) implanted. CCTA scanning was performed in all patients prior PCI and at 12 months after PCI. The mean CSS at proximal and at distal part of the stented segment and at the level of the minimal lumen area (MLA) was calculated at baseline and at 12 months after BVS placement. However, not all vulnerable coronary plaques produce an ACS and the prediction power of various vulnerability features to predict an acute coronary event in a close future, has not been elucidated so far. Objective We aimed to use multi-slice computed tomography angiography (CTA) for assessment of morphological characteristics of culprit lesions producing an ACS in the next several months after CT assessment, in comparison with morphological characteristics of unstable coronary atherosclerotic plaques which did not trigger an ACS. Material and methods We analyzed 40 patients in whom CTA revealed presence of unstable coronary lesions, exhibiting at least one marker of vulnerability: napkin ring sign (NRS), spotty calcium (SC), positive remodeling (PR) or presence of low attenuation plaque (LAP), divided in 2 groups: group 1 - 20 patients who developed an ACS in the next 6 months following CTA examination, and group 2 – 20 patients matched for age, gender and risk factors, who did not present any cardiovascular event 6 month after CTA assessment. Post-processing of multi-slice CTA images was performed in order to assess morphological characteristics and CT-derived markers of atherosclerotic plaque instability. Results Similar mean values of plaque length (17.1 +/- 5.9 mm vs 16.9 +/- 3.4 mm; p = 0.6) and total atheroma volume (188.1 +/- 104.7 mm3vs 186.4 +/- 90.7 mm3; p = 0.8) were obtained for both groups. The mean number of vulnerability markers was 1.6 in group 1 vs 1.2 in group 2 (p = 0.07). However, atherosclerotic lesions in patients from group 1 presented significantly higher values of lipid-rich atheroma (9.8 +/- 10.8 mm3vs 2.6 +/- 1.0 mm3; p = 0.01) and remodeling index (1.14 +/- 0.3 in group 1 vs 0.89 +/- 0.19 in group 2, p = 0.04). At the same time, atheromatous plaques in patients who developed an ACS during the 6-months follow-up showed in a significantly higher proportion LAP (45% in group vs 10% in group 2, p = 0.03) and PR (15%in group 1 versus 5% in group 2, p = 0.04), but not NRS (30% vs 25%, p = ns) or SC (65% vs 40%, p = 0.2). Conclusions Atherosclerotic plaques producing an ACS exhibit a different phenotype than unstable plaques that remain silent. The CTA profile of atheromatous plaques producing an ACS includes the presence of low attenuation, positive remodeling, higher RI and lipid-rich atheroma. Presence of these features in high-risk coronary plaques identifies very high risk patients, who can benefit from adapted therapeutic strategy in order to prevent the development of an ACS.

Funding Acknowledgements Plaqueimage Background Atrial fibrillation (AF) is the most common supraventricular rhythm disturbance and pulmonary vein (PV) isolation has an important role in rhythm control treatment strategies of this disease. Various anatomical and clinical characteristics have been well established as predictors of the risk of recurrence following ablation procedures, but the role of epicardial fat tissue (EFT) in the recurrence of AF has not been elucidated so far. Purpose To investigate the influence of left atrial size and EFT volume in the recurrence of AF after pulmonary vein ablation, during a 6-month follow-up. Methods A total of 40 patients, 52.5% with paroxysmal and 47.5% with chronic AF underwent PV isolation using radiofrequency and cryoablation techniques. EFT was determined using cardiac computed tomography angiography (CCTA) associated with advanced image post-processing techniques. Results In patients who developed AF recurrence at 6 months after AF ablation, the volume of EFT and of left atrium were significantly larger than in the group who maintained sinus rhythm (202.5 ± 64.56 ml vs. 138 ± 55.74 ml, p = 0.01 for EFT, and 149.3 ± 4.66 ml vs. 90.63 ± 5.19 ml, p Conclusion Patients with AF recurrence after pulmonary vein ablation present significantly higher EFT or left atrial volumes compared to patients who maintained sinus rhythm. This indicates the inflammatory mediated response, usually accompanied by an increased amount of EFT, could be associated with the risk of AF recurrence following catheter ablation of the pulmonary veins.

Background: Lacrimal drainage system lymphomas are rare, accounting for less than 10% of lacrimal sac tumors. They often appear as a secondary involvement within the confines of systemic lymphoproliferative disorders, therefore detailed ophthalmological examination and auxiliary testing is necessary to have an accurate diagnosis. Case report: We present the case of a 72-year-old woman with a medical history of chronic lymphocytic leukemia and small lymphocytic lymphoma. She presented to the ophthalmology clinic with a painful, discharging swelling in the right lacrimal sac area. Magnetic resonance imaging revealed a mass in the right nasolacrimal duct. A right external dacryocystorhinostomy was performed. The histological findings led to a diagnosis of small lymphocytic lymphoma. Conclusions: Special attention is required in cases of known systemic hematological disorders that associate with nasolacrimal duct obstruction or epiphora. Quick recognition and a full history with multidisciplinary clinical and diagnostic workup are fundamental to plan the treatment.

Background: Myocardial revascularization procedures have undergone important developments over the last decades, which led to a major shift in current clinical practice and therapeutic guidelines across the world. Bare metal and drug-eluting stents present several limitations, all centered on the concept of disturbed coronary hemodynamics after implantation, which can be surpassed by bioresorbable vascular scaffolds (BVS). BVSs are fourth-generation stents used in coronary revascularization procedures, but despite all the promising initial results published on their efficiency, several clinical trials have reported unsatisfactory results, and the main explanation was accredited to improper implantation method. Shear stress is a central element of intravascular homeostasis; it controls vascular remodeling, as well as the development, progression, and destabilization of atheromatous plaques. This study aims to assess the role of in-stent flow hemodynamics (evaluated by computational determination of shear stress via coronary CT imaging) in predicting the clinical evolution following BVS implantation. Material and methods: This case-control observational study will include patients with BVSs implanted at least 12 months prior to randomization. Each patient will undergo a complete evaluation of the demographic and clinical characteristics, cardiovascular risk factors, and imaging acquisitions via coronary CT angiography, based on which the endothelial shear stress will be calculated before and after BVS implantation. Post-processing of CT imaging data will evaluate the shear stress and the composition of the coronary plaques along the entire coronary tree. The primary endpoint will be the major adverse cardiovascular events (MACE) in patients with altered vs. non-altered BVS-related shear stress, and the secondary endpoints will comprise evaluating the rate of progression of stent resorption and progression of shear stress alteration. Conclusions: The findings of the STAFF study can be extremely useful in clinical practice for providing an answer to a key question that is still under debate: why do BVSs fail and how can we prevent this?.

We present a case of a 51 year old multiple myeloma patient due to the complexity of his disease. After prior high dose therapy with cardiotoxic cytostatic drugs patient developed cardiac arrest due to a ventricular fibrillation. Cardiopulmonary resuscitation was successfully but the hematologist stopped the cytostatic treatment due to the high cardiac risk. The only way to control his myeloma was an autologous transplantation but this also had a very high cardiotoxic potential due to the high dose of Melphalan administered as conditioning treatment. We present the difficulties and the results of this complex case. It also arises the potential beneficial effects of the megadose of stem cells on the myocardium but this needs further studies.

Background: The elucidation of the genetic background of the myeloproliferative neoplasms completely changed the management of these disorders: the presence of the Philadelphia chromosome and/or the BCR-ABL oncogene is pathognomonic for chronic myeloid leukemia and identification of JAK2 gene mutations are useful in polycytemia vera (PV), essential thrombocytemia (ET) and myelofibrosis (PMF). The aim of this study was to investigate the role of molecular biology tests in the management of myeloproliferative neoplasms. Materials and methods: We tested the blood samples of 117 patients between April 2008 and February 2013 at the Molecular Biology of UMF Târgu Mureș using RQ-PCR (for M-BCR-ABL oncogene) and/or allele-specific PCR (for JAK2V617F mutation). Results: Thirty-two patients presented the M-BCR-ABL oncogene, 16 of them were regularly tested as a follow-up of the administered therapy: the majority of chronic phase patients presented decreasing or stable values, while in case of accelerated phase and blast phase the M-BCR-ABL values increased or remained at the same level. Twenty patients were identified with the JAK2V617F mutation: 8 patients with PV, 4 with ET, 3 with PMF, 4 with unclassifiable chronic myeloproliferative disease and 1 patient with chronic myelomonocytic leukemia. There was no case of concomitant occurance of both molecular markers. Conclusions: Molecular biology testing plays an important role in the management of myeloproliferative neoplasms: identification of the molecular markers confirms the final diagnosis, excluding secondary causes of abnormal blood count parameters. Regular monitoring of MBCR- ABL expression level is useful in the follow-up of therapeutic efficiency.

Objective: The aim of the study is to assess 24-hour blood pressure variability, circadian blood pressure profile and its relation to 24-hour blood pressure and cardiovascular risk factors in primary hypertension without any associated disease versus associated with a disease such as diabetes or chronic kidney disease. Methods: This observational study included 90 hypertensive patients, 49 with primary hypertension without associated disease and 41 patients with primary hypertension and diabetes or chronic kidney disease. Circadian blood pressure profile and 24-hour variability were assessed using ambulatory monitoring. Laboratory data regarding cardiovascular risk factors and demographic data were collected in a questionnaire. Results: The number of dipper patients was higher in the group without associated disease, but the difference was not statistically significant (p=0.27). In both groups a positive correlation was found between 24-hour systolic blood pressure variability and 24-hour systolic blood pressure (p=0.029) and was related to age (p=0.031). In the second group, systolic variability showed a positive correlation with serum triglycerides (p=0.006, r=0.416, CI: 0.1252 to 0.6422). Conclusion: Our findings suggest that systolic blood pressure variability is related to age, systolic blood pressure values and serum lipid levels. To prevent end organ damage in hypertension, the assessment of ambulatory monitoring derived 24-hour systolic blood pressure variability and its reduction may be at least as important as blood pressure lowering.

Introduction: Chronic myeloid leukemia (CML) is a clonal myeloproliferative disorder; the molecular hallmark of the disease is the BCR-ABL gene rearrangement, which usually occurs as the result of a reciprocal translocation between chromosomes 9 and 22. Tyrosine kinase inhibitors (TKI) were the first drugs that targeted the constitutively active BCR-ABL kinase and it have become the standard frontline therapy for CML. Monitoring the treatment of CML patients with detection of bcr-abl transcript levels with real time qualitative polymerase chain reaction (RQ-PCR) is essential in evaluating the therapeutic response. Material and method: At the Clinical Hematology and BMT Unit Tîrgu Mureș, between 2008-2011, we performed the molecular monitoring of bcr-abl transcript levels with RQ-PCR in 16 patients diagnosed with CML. Results: We have 11 patients on imatinib treatment who achieved major molecular response. One patient lost the complete molecular response after 5 years of treatment. Two patients in blast crisis underwent allogeneic hematopoietic stem cell transplantation from identical sibling donors. The first patient is in complete molecular remission after 4 years of the transplant with mild chronic GVHD. The other patient had an early relapse with treatment refractory disease and died from evolution of the disease. Three patients with advanced phases of the disease present increasing transcript levels. We performed the dose escalation, and for two of them the switch to the second generation of TKI. Conclusions: Regular molecular monitoring of individual patients with CML is clearly desirable. It allows for a reassessment of the therapeutic strategy in cases of rising levels of BCR-ABL as an early indication of loss of response.

Introduction: Plasmacytoid dendritic cell leukemia is a rare subtype of acute leukemia, which has recently been established as a distinct pathologic entity that typically follows a highly aggressive clinical course in adults. The aim of this report is to present a case of plasmacytoid dendritic cell leukemia due to its rarity and difficulty to recognize and diagnose it.Case report: We present a case of a 67 year-old man who presented multiple subcutaneous lesions on his face, neck, chest and upper extremities with reddish-brown, brown colour. In the bone marrow aspirate 83% of the blast cells were found. Immunophenotypically the blasts were positive for CD4, CD56, CD123 (high intensity), CD36, CD22, CD10 (10.42%), CD33, HLA-DR, CD7 (9.24%), CD38 (34.8%) and negative for CD13, CD64, CD14, CD16, CD15, CD11b, CD11c, CD3, CD5, CD2, CD8, CD19, CD20, CD34. The skin biopsy showed lymphohistiocytoid infiltration in the dermis. The patient was diagnosed with acute plasmacytoid dendritic cell leukemia and received polychemotherapy with rapid response of skin lesions and blastic infiltration of the bone marrow. After 3 courses of polychemotherapy the cutaneous lesions reappeared and multiplied. The blast infiltration in the bone marrow increased to 70%. A more aggressive polychemotherapy regimen was administered, but the patient presented serious complications (febrile neutropenia) and died in septic shock 8 months after the initiation of treatment.Conclusions: Immunophenotyping of blasts cells is indispensable in the diagnosis of plasmacytoid dendritic cell leukemia. The CD4+, CD56+, lin-, CD123 ++high, CD11c-, CD36+, HLA-DR+, CD34-, CD45+ low profile is highly suggestive for pDCL. The outcome of plasmacytoid dendritic cell leukemia is poor. Despite the high rate of initial response to treatment, early relapses occur and the patients die of disease progression.

During the past 10 years, the health of people in Eastern Europe and the former Soviet Union has undergone changes very different from the health patterns seen in their Western counterparts. Mortality from cardiovascular disease has been decreasing continuously in the USA and many Western European countries, but it has increased or remained unchanged in many of the states of Eastern Europe. Analysis of this phenomenon has been hindered by insufficient information. The International Registry of Acute Coronary Syndromes registry study in Transitional Countries (ISACS-TC) is both a retrospective - over a 1-year period - and prospective study which was designed in order to obtain data of patients with acute coronary syndromes (ACSs) in countries with economy in transition in Central and Eastern Europe, and herewith control and optimize internationally guideline recommended therapies in these countries. Adhesion to the project was given by 112 Collaborating Centres in 17 countries with economy in transition (Albania, Bosnia and Herzegovina, Belarius, Bulgaria, Croatia, Hungary, Kosovo, Latvia, Lithuania, Macedonia, Moldova, Montenegro, Romania, Russian Federation, Serbia, Slovakia, Slovenia, and Ukraine). A total of 47 cluster sites in 11 countries in Central and Eastern Europe are currently collaborating in ISACS-TC. The registry encourages optimal individualization of evidence-based therapies and the international patient body ensures good representation of multiple practice patterns. It may help to make an additional improvement in clinical outcomes of countries with economy in transition.

Introduction: We aimed to present a case of acute coronary syndrome with unexpected etiology complicated by syncope and arrhythmias, confirmed by imagistic examinations as cardiac parasitosis. Cardiac parasitic diseases are rare diseases, whose diagnosis and therapy should be adapted to each case. Imaging techniques allow precise diagnosis of cardiac echinococcosis, providing essential structural details on the damage degree of heart structures, allowing optimization of complex treatment in these cases. Case presentation: A 67-year old, obese and diabetic woman presented with cardiac syncope, arrhythmias and acute chest pain. Imagistic examinations excluded intracoronary thrombosis and confirmed a severe structural damage of myocardial tissue, consisting in replacement of the myocardial structure by many cysts caused by parasitic infestation with echinococcus multilocularis and echinoccocus granulosus originating from the liver. CT scan confirmed severe distruction of the left ventricular myocardium by policysts, that led to thinning of inferior and apical left ventricle wall without any possibility of surgical excision. Therefore a specific chemotherapy with albendazole was initiated. Follow up at 2 months indicated a favorable evolution, with serological decrease of echinococcal antibodies and reduction of cysts volume. Conclusion: In cases of angina and arrhythmias with non-atherosclerotic etiology, imaging techniques can diagnose the anatomopathological substrate of the disease and represent a valuable tool for the follow up.

We have investigated the effect of stem cell delivery on the release of hypoxia-inducible factor 1 alpha (HIF-1alpha) in peripheral circulation and myocardium in experimental myocardial ischemia. Closed-chest, reperfused myocardial infarction (MI) was created in domestic pigs. Porcine mesenchymal stem cells (MSCs) were cultured and delivered (9.8 +/- 1.2 x 10(6)) either percutaneously NOGA-guided transendocardially (Group IM) or intracoronary (Group IC) 22 +/- 4 days post-MI. Pigs without MSC delivery served as sham control (Group S). Plasma HIF-1alpha was measured at baseline, immediately post- and at follow-up (FUP; 2 h or 24 h) post-MSC delivery by ELISA kit. Myocardial HIF-1alpha expression of infarcted, normal myocardium, or border zone was determined by Western blot. Plasma level of HIF-1alpha increased immediately post-MI (from 278 +/- 127 to 631 +/- 375 pg/ml, p0.05). Cardiac delivery of MSCs elevated the plasma levels of HIF-1alpha significantly (p0.05) in groups IC and IM immediately post-MSC delivery, and returned to baseline level at FUP, without difference between the groups IC and IM. The myocardial tissue HIF-1alpha expression in the infarcted area was higher in Group IM than in Group IC or S (1,963 +/- 586 vs. 1,307 +/- 392 vs. 271 +/- 110 activity per square millimeter, respectively, p0.05), while the border zone contained similarly lower level of HIF-1alpha, but still significantly higher as compared with Group S. Trend towards increase in myocardial expression of HIF-1alpha was measured in Group IM at 24 h, in contrast to Group IC. In conclusion, both stem cell delivery modes increase the systemic and myocardial level of HIF-1alpha. Intramyocardial delivery of MSC seems to trigger the release of angiogenic HIF-1alpha more effectively than does intracoronary delivery.

Introduction: Anemia is a common complication of malignant lymphomas, which could be a direct consequence of the disease or secondary to the myelosupressive chemotherapy. The aim of this study was to assess the effect of erythropoietin to treat anemia. The main objectives were to demonstrate increases in hemoglobin levels and the existence of an association between symptom relief and treatment. Material and method: In the Clinical Hematology and BMT Unit Tîrgu Mureș we performed an analytical, observational study to assess the role of erythropoietin treatment in malignant lymphoma related anemia. This linear, retrospective study included 127 patients diagnosed and treated with malignant lymphoma between January 1st, 2007 and December 30, 2011. The 127 patients were divided into two groups: a group of patients (n = 88) who were treated with erythropoietin and the other group (n = 39) who did not receive this treatment. Patients included in the study received treatment with epoetin beta 40,000 IU/week. We followed the hemoglobin level and the symptomatology at baseline and after 4 weeks. Results: Patients who received treatment with erythropoietin had a 7.12 times higher possibility of being asymptomatic than patients who did not receive this treatment. The hemoglobin concentration of patients with erythropoietin treatment increased significantly (p Conclusion: Effective treatment of anemia is an important aim in the management of patients with malignant lymphomas, because it increases their hemoglobin concentration, decreases the need of transfusion and maintains an acceptable quality of life.

Introduction: Autologous haemopoietic stem cell transplantation (SCT) is an important treatment modality for patients with acute myeloid leukemia with low and intermediate risk disease. It has served advantages over allogenic transplantation, because it does not need a matched donor, there is no graft versus host disease, there are less complications and a faster immune reconstitution than in the allo-setting. The disadvantage is the lack of the graft versus leukaemia effect. Materials and methods: In the Bone Marrow Transplantation Unit Tîrgu Mureș 14 patients with acute myeloid leukemia received an autologous SCT. Mobilization of the stem cells was performed using chemotherapy and granulocytic colony stimulating factor. The conditioning regimen for SCT consists in monotherapy with busulfan (Bu) 16 mg/kg, BuCy: busulfan in combination with Cyclophosphamide (CY) 120 mg/ kg or BuMel: Busulfan in association with Melphalan (Mel) 140 mg/m2. Results: The median patient age was 36 years (range 20-55), 9 (64%) were males and 5 (36) were females and the median time interval from diagnosis to autologous SCT was 9 months (range 3-25). All the patients were transplanted successfully, all of them achieved a sustained neutrophil count (> 0.5 G/L), median time 11 days (9-15) and platelet count (> 20 G/L) median time 14 days (10-19) after transplantation. Conclusions: We conclude that autologous stem cell transplantation is an effective treatment in acute myeloid leukemia with the possibility of long survival, particularly in patients with standard risk disease

We present the possibilities of diagnosis correlating the pathological, immunophenotyping and clinical aspects of a rare case of T-cell lymphoma in a 23-year-old patient with leukemic transformation. In our consideration, it is very important to describe this case because in the literature there are very few cases presented and the treatment of this type of lymphoma does not present optimal results, the evolution of the patients being from three months to two years. The treatment modality that gives the possibility to prolong survival and cure is hematopoietic stem cell transplantation.

The aim of the present study was to investigate the changes in absolute myocardial blood flow (AMF) after intracoronary injections of mesenchymal SC (MSC) and compared to controls in closed-chest reperfused acute myocardial infarction (AMI) in pigs. Male MSCs, transiently transfected with Luciferase (Luc-MSC) were delivered $$ \left( {{9}.{7}\pm {1}.{2} \times {1}{0^{{6}}}} \right) $$ intracoronary in the open infarct-related artery one-week post-AMI in female pigs (group MSC), while saline was injected with the same injection rate in controls (group C). The AMF was measured immediately after, and 3, 12 and 24 h post-intracoronary Luc-MSC or saline injections. In vitro bioluminescence images and quantitative real-time TaqMan PCR measurements were performed to quantify the sex-mismatched MSCs. No difference between the groups was observed regarding the weight, heart rate, blood pressure and global ejection fraction 1-week post-AMI. The baseline AMF were similar in the groups (61.3 ± 15. vs 61.1 ± 12.0 ml/min). AMF was decreased significantly immediately after intracoronary MSC delivery (42.0 ± 12.4 vs 57.7 ± 15.7 ml/min p = 0.013), and remained low at 3 h (40.9 ± 13.4 vs 55.8 ± 4.9 ml/min, p = 0.004), 12 h (43.0 ± 3.7 vs 57.8 ± 5.4 ml/min, p = 0.001) with incomplete recovery at 24 h (47.2 ± 5.5 vs 62.1 ± 14.1 ml/min, p = 0.038) as compared to controls, respectively. In vitro bioluminescence displayed transfected Luc-MSCs along the proximal and mid part of the LAD, with limited number (295 ± 101 sry copied/million cardiac cells) of Y-chromosome-MSCs in the infarcted area. Intracoronary injection of SCs results in immediate decrease of AMF, with delayed recovery. The delivery of the SC into the injured myocardium might be hindered by the altered coronary pressure and flow conditions.

Aim: In a period of 4 years we created a regional registry on acute coronary syndromes on a target population of 1 million inhabitants, and we followed the evolution of all patients with acute coronary syndromes (ACS). During this period we performed an intense educational activity in order to increase the adherence to European guidelines regarding the treatment of ACS. Material and method: The registry included 10 hospitals in 2004 and 13 hospitals in the following years, serving a population of 1 million inhabitants. Data regarding clinical presentation, risk factors, therapy and evolution were collected, centralized and processed in the clinic of cardiology of Targu Murescedil. Results: The registry included a number of 3455 patients with ACS, from which 1457 acute myocardial infarction (AMI) and 1998 unstable angina (UA). Mortality rates in AMI in the territorial hospitals was: 19,8% in 2004 which decreased to 11.04% in 2007, in parallel with an increase in the number of cases sent to interventional center (from 20% in 2004 to 28.51% in 2007). Conclusions: After the experience in running a registry on ACS, we conclude that we succeeded to decrease the mortality rates in AMI by increasing the adherence to European guidelines and by increasing the rates of patients sent to interventional center.

Congenital heart diseases are the most common congenital malformations and account for about eight cases per 1000 births and are often associated with pulmonary arterial hypertension. Increased shear stress and the excess flow through the pulmonary vascular bed due to a systemic-to-pulmonary shunt lead to the development of pulmonary vascular disease and an increase in pulmonary vascular resistance. Without surgical repair approximately 30% of patients develop pulmonary vascular disease. Eisenmenger syndrome represents the extreme end of pulmonary arterial hypertension with congenital heart disease. We summarized the current therapeutic options for pulmonary arterial hypertension; conventional treatments including calcium channel blockers, anticoagulation, digitalis, diuretics, and new treatment: prostacyclin, bosentan, sildenafil, ambrisentan. Preliminary data of new therapies are encouraging with disease significantly improved natural history, but there is need for more evidence-based data.

The importance of peripheral arterial diseases is revealed by statistical data, which showed a frequency of arterial diseases equal to the one of stable angina Traditional treatment of these patients was the surgical bypass, but an alternative therapy in these cases is the interventional treatment. Arguments for this option could be: it is less expensive, it decreases the hospitalization time, it does not require general anesthesia, and complications are less frequent and less severe. Interventional treatment is recommended especially in cases of short, isolated lesions of great vessels. During the last years, indications for interventional treatment were expanded to more difficult and complex cases, due to the advances in technology of endoluminal treatment products and the recently introduced laser angioplasty technology.Among the interventional techniques we apply in our clinic, we mention: percutaneous transluminal angioplasty, intraarterial stent implantation, laser angioplasty and intraarterial thrombolysis, some of them peformed with IVUS control. In a period of 14 years we examined a total number of 6,532 patients suffering from peripheral arterial obstructive diseases. In the past 3 years, in our new laboratory of interventional cardiology, new procedures have been introduced in the algorithm of interventional treatment, such as laser angioplasty. Invasive diagnosis performed in the Cardiology Clinic consisted in aortography in 746 cases and peripheral arteriography in 2035 cases. Interventional treatment consisted in percutaneous transluminal angioplasty (1.240 procedures, 756 performed in the latest 3 years), intraarterial thrombolysis (214 procedures), laser angioplasty (since June 2002-130 procedures performed) and stent implant (since 2001-61 cases). Axillar approach was also introduced in 2001 and was applied in 23% of cases.We recorded a 76.5% success rate using only PTA for repermeabilisation of the lower limb arteries. Association of intraarterial thrombolysis increased the success rate to 91%. Using laser angioplasty and autoexpandable stent implantation, the success rate achieved was 100%.Interventional treatment is an important alternative in the complex algorithm of treatment in PAOB. Intraarterial stenting significantly improves the immediate and the long term results of interventional treatment, and autoexpandable stents with thermal memory show superior results. Laser angioplasty is an adjuvant method which significantly improves the results of interventional treatment. In all these methods, a very important role plays the good cooperation of the team formed by cardiologist, angiologist and vascular surgeon.

The importance of interventional procedures in the complex treatment of peripheral arterial diseases is continuously increasing. In the current practice of our clinic, association of balloon angioplasty, laser angioplasty and arterial stenting in reconstruction of iliac arteries led to superior results in the latest years, these methods 11:42 PM 11:42 PM 11:42 PM being proved as an alternative to surgical interventions. In this article, we present several cases in which current indications for laser angioplasty were extended to target occlusions located in the terminal abdominal aorta.106 consecutive primary iliac interventions were performed on 88 patients with iliac or aortoiliac obstructive diseases, in the period September 2001 - October 2005, at the University of Medicine and Pharmacy of Târgu-Mureş, Romania, Clinic of Cardiology. Five of these patients (4 males, 1 female) presented occlusions of terminal aorta, in whom interventional treatment (peripheral transluminal angioplasty, laser angioplasty and stenting) was performed. Three cases presented total occlusion of terminal aorta, without any visualization of iliac arteries, and 2 cases presented occlusion of one aortoiliac axis, starting from terminal aorta.In all cases, complete repermeabilisation of aortoiliac axes was achieved, without complications. In all patients we recorded a significant improvement of symptomatology, and arterial Doppler showed an increase of Doppler ankle/brachial index in average from 0.4 up to 0.95. No complications have been recorded so far.Extension of classical indications of interventional treatment for balloon and laser angioplasty to occlusions located in terminal aorta is possible when the procedure is performed by an experienced team. Interventional techniques, having a superior applicability in practice, good results, low complication rates, and decreasing the hospitalization times, could be applied in the future to a larger extent, targeting also aortic occlusions.

In the past years, diagnosis of myocardial diseases became less difficult. A major contribution in this direction has the recent development of diagnosis methods. One of the most recent diagnosis procedures is the endomyocardial biopsy, which manages directly through the histologic examination to establish a clear diagnosis in myocardial diseases. This method provides faithful morphological data about the pathological changes in the myocardium. The aim of our study was to underline the importance of endomyocardial biopsy in an accurate diagnosis of different cardiomyopathies. In the period of 12 years (1986-1998), in the Cardiovascular Department of the First Medical Clinic Târgu-Mureş, Romania, were admitted 137 patients suffering from different cardiomyopathies. We performed a number of more than 100 endomyocardial biopsies on 25 of them, obtaining in each case 3 to 5 samples of endomyocardial tissue from the right ventricle. In all cases the Cordis bioptome was introduced in the right ventricle via femoralis vein. The complications were minor: ventricular extrasystoles. In all cases, we succeeded to obtain enough tissue for the histopathological examination, from different areas of RV. Histologic examination showed in all cases characteristic aspects of different cardiomyopathies. helping to clarify the diagnosis. After performing Endomyocardial Biopsy, we record an important increase of dilated cardiomyopathy percentage of diagnosis, from 40% to 72%. In patients with dilated cardiomyopathy, we observed a good correlation between hemodynamical data (Ejection Fraction, Left Ventricle End-Diastolic Pressure, etc.) and the index of myocardial damage, especially the degree of interstitial fibrosis. Using this method it is possible to distinguish different types of cardiomyopathies, such as: primary dilated cardiomyopathy, eosinophilic cardiomyopathy, myocarditis, idiopathic hypertrophic cardiomyopathy. Because the complications of this procedure are insignificant, and because of its important benefit for an accurate diagnosis, we recommend to apply this method for different cardiomyopathies.

It is well known that in the evolution of cardiac diseases, thromboembolic accidents are quite frequent. In patients with acute peripheral ischaemia, surgical treatment is usually the procedure of choice, but in some circumstances some of these patients cannot benefit from the surgical treatment. Conservative treatment rarely leads to satisfactory results. An alternative of therapy is the thrombolysis, but administration of Streptokinase in systemic dose can lead to serious thromboembolic accidents and hemorrhage. Intra-arterial administration of the drug, in small doses, can eliminate this risk. The aim of this study was to demonstrate the efficacy of intra-arterial thrombolysis in patients with cardiac diseases complicated with peripheral arterial emboli.In the period 1984-1997, we performed a number of 84 intra-arterial thrombolyses. Sixty-one of them were performed in case of chronic or acute arterial obstruction of the inferior limbs, occurring in the evolution of chronic arterial disease, 14 in the patients with acute obstruction caused by emboli from cardiac cavities, and 9 in the patients with arterial embolism of other origin. We applied this procedure infusing Streptokinase intraarterially, continuously, directly and progressively in the thrombus, with a rate of 2-4000 unit/min, and a total dose more than 100,000-120,000 units. All patients had been consulted previously in a surgical unit and for different reasons surgical treatment was not indicated.We succeeded to repermeabilise the entire arterial route at all the patients (fact demonstrated by the clinical and Doppler examinations). We obtained complete primary repermeabilisation in 79% cases (11 cases), and repermeabilisation at 24 hours in all cases. We did not record major complications during this procedure.At the cases we studied (a small number), this method seems to be an alternative of therapy with good results in the restoration of obstructed arterial circulation. Intra-arterial administration of Streptokinase, in small doses, can eliminate the haemorrhagic and thromboembolic risks that appear in case of systemic administration of the drug.

It is well known in the literature that myocardial tissue is present in different degrees in pulmonary veins, but the eventual role of this myocardial tissue in pulmonary veins is not very clear at this moment. We presumed that one of the compensatory mechanisms in heart failure could be represented by the development of this myocardial tissue in the pulmonary veins. We studied the extension of this myocardial tissue in patients with dilated cardiomyopathy (DCM). We performed a histologic examination of the myocardial tissue from the pulmonary veins, in patients with heart failure in advanced stages. These patients died in the hospital and we obtained the pulmonary tissue for examinations during the autopsy. At the border between the media and the adventitia of the pulmonary vein, the myocardial tissue is present and is well developed. Some of the fibers have a sinuous course, others present a longitudinal aspect, especially in the external layers. In some sections bundles and bands of muscle fibers are present. In the sections where no myocardial tissue was shown, the smooth muscular fibers were very numerous. The necroptic histologic studies showed different degrees of hypertrophy of the myocardial tissue in the pulmonary veins, in the patients with heart failure in advanced stages. We observed that dilatation degree of cardiac cavities and decrease of the ejection fraction correlates with hypertrophy degree of the myocardial tissue in the pulmonary veins. This can be an argument for the compensatory role of this hypertrophy.
