The objective of this study was to evaluate the cortical thickness and the volume of deep gray matter structures, measured from 3D T1-weighted gradient echo imaging, and white matter integrity, by diffusion tensor imaging (DTI) in patients with typical absence epilepsy (AE). Patients (n = 19) with typical childhood AE and juvenile AE, currently taking antiepileptic medication, were compared with control subjects (n = 19), matched for gender and age. 3D T1 magnetization-prepared rapid gradient echo-weighted imaging and DTI along 30 noncolinear directions were performed using a 1.5-T MR scanner. FreeSurfer was used to perform cortical volumetric reconstruction and segmentation of deep gray matter structures. Forty patients with high-risk hematologic malignancies, median age 9 years, underwent haploidentical-HSCT from April 2005 to April 2015. Seventeen patients were transplanted with CD3-depleted PBSCs by negative selection (TCD group) following a reduced-intensity conditioning regimen (RIC), and 23 patients received T-cell-replete PBSCs followed by post-transplantation cyclophosphamide (PT-Cy group) after myeloablative conditioning (n=16) or RIC (n=7). Outcomes are reported for the TCD and PT-Cy recipients, respectively. Engraftment was achieved in 88% versus 100%. Median time to neutrophils>500/μL was 10 days versus 15 days. Platelets>20 000/μL occurred at a median of 16 days versus 20 days, respectively. Transplant-related mortality (TRM) was 24% versus 26% at 1 year. The cumulative incidence (CI) of grade III−IV acute GvHD was 7% versus 5%, and chronic GvHD 9% versus 53% (P=0.029). Relapse at 2 years was 31% versus 24%. Actuarial overall survival rates at 2 years were 47% versus 48%. Causes of death were infections (n=3), sinusoidal obstructive syndrome (n=4), acute GvHD (n=2) and relapse (n=9). These results indicate that haploidentical-HSCT is feasible in Uruguay. The TRM rate is of concern and should be the focus of continuing attention. Chronic GvHD risk was higher in the PT-Cy approach, so modifications are justified.

International audience; Overall, our findings point to a role for 5-HT2B receptors to drive behavioral flexibility in response to the environmental challenges like social stress that appears to be mediated by microglia activity. These results provide new targets for the development of effective pharmacological treatments and diagnostic strategies.

We describe the normal dural enhancement patterns of the sellar region and determine whether the duramater is affected by pituitary macroadenomas. Dural enhancement appeared to be usually abnormal in 20 patients with pituitary macroadenoma compared with 20 control patients, mainly at the planum sphenoidale and carotid sulcus. However dural changes are subtle and their recognition requires knowledge of the normal enhancement patterns. Dural changes, reported in a variety of inflammatory and infectious dural diseases and after surgery, are not specific and may be also seen in pituitary macroadenomas.

In this work, a novel approach in post-earthquake structural damage estimation is investigated. The approach is formulated as a problem of both damage approximation and localization. The inter-story drift ratio and the global damage index of Park/Ang (DIG,PA) are the estimated damage indicators for each floor of the structure. Artificial neural networks (ANNs), random forests (RFs), support vector machines (SVMs) with linear and radial basis function (RBF) kernels and adaptive neuro-fuzzy inference systems (ANFISs) are tested to predict the seismic damage state of each floor of an 8-storey reinforced concrete (r/c) building subjected to 155 natural and artificially generated seismic accelerograms. The damage potential of the accelerograms is described by three seismic parameters extracted from the response of the structure. The set of seismic accelerograms is defined by combining two outlier detection techniques, isolation forests and Z-score, while the set of seismic parameters is confirmed by minimum redundancy maximum relevance (mRMR) feature selection algorithm. Optimization methods are used to fine-tune the performance of all networks. Results indicate RFs and ANNs among the models with optimal performances, reaching average correct classification rates of up to 96.87% and 91.87% with RFs, and 96.25% and 90.12% with ANNs, for DIG,PA and ISDR, respectively.

Introducción. Según las directrices del Ministerio de Salud de Chile, la lactancia materna exclusiva (LME) se debe suministrar como único alimento hasta los 6 meses de vida del lactante; sin embargo, las cifras de destete precoz han aumentado, influyendo en este aspecto motivos de índole social, familiar y personal de salud. Objetivo. Determinar los factores que inciden en la duración de la lactancia materna exclusiva hasta los seis meses en un centro de salud rural. Métodos. Estudio no experimental, descriptivo y de cohorte retrospectivo; se incluyeron en este estudio a 81 madres de hijos/as en edades entre 6 y 11 meses de edad, y se aplicó un instrumento formado por 11 preguntas cerradas y semiestructuradas. Resultados. Se encontró un 56,8% de destete precoz, siendo las razones más relevantes: hipogalactia (45,7%), indicación médica y enfermedad de la madre (15,2%), y motivos laborales (13%). La edad de las madres se correlacionó con el destete precoz (p=0,046). Conclusiones. Las madres menores de 26 años proporcionaron por menos tiempo lactancia materna exclusiva a sus hijos, siendo el foco de atención para las actividades educativas de fomento de LME. Background: Exclusive breastfeeding (EBF) must be provided as the only food until 6 months of life, according to the guidelines of the Ministry of Health of Chile. However the prevalence of early weaning has increased, influencing social, family and health aspects. Objective: To determine the factors that could affect the duration of exclusive breastfeeding until six months in a rural health center. Methods: A non-experimental descriptive and retrospective study cohort that included 81 mothers and their offspring aged 6 to 11 months old, applying an ad-hoc instrument consisting of 11 questions closed and semi-structured. Results: 56.7% of mothers had an early weaning, the most important reasons being hypogalactia (45.6%), health staffindication, ands mother's illness (15.2%), and work reasons (13%). Age of mother correlated with early weaning (p 0.046). Conclusions: Mothers under 26 years of age showed reduced time giving exclusive breastfeeding.

We explored the association between use of mobile phones and lymphoma risk in a case-control study.We conducted unconditional logistic regression analysis in 322 lymphoma cases and 446 population controls, adjusting by age, gender and education.Risk of lymphoma (all types; OR = 1.5; 95% CI 1.0 - 2.1), and chronic lymphocytic leukaemia (OR = 1.8; 95% CI 1.0 - 3.4) was elevated in subjects reporting use of mobile phones, but it decreased with duration of use, and years from first purchase.Our contradictory findings would not support the aetiological nature of the observed associations.

We explored the risk of lymphoma and its most prevalent subtypes associated with occupational contact with livestock, and whether risk was modified by age at first contact, in 2,348 incident lymphoma cases and 2,462 controls who participated in the EPILYMPH case-control study. A detailed occupational history was collected in cases and controls, including working in a livestock farm, species of livestock, its approximate number and circumstances of contact. For each disease outcome, and each type of livestock, odds ratios (OR) and their 95% confidence intervals (95% CI) were calculated using unconditional logistic regression, adjusting for age, gender, education and center. Lymphoma risk (all subtypes combined) was not increased amongst those exposed to contact with any livestock (OR = 1.0, 95% CI 0.8-1.2). Overall, we did not observe an association between occupational contact with livestock and risk of lymphoma (all types) and B-cell lymphoma. The risk of diffuse large B cell lyphoma (DLBCL) was significantly lower amongst subjects who started occupational contact with any species of livestock before or at age 12 (OR = 0.5, 95% CI 0.2-0.9), but not at older ages. A significant heterogeneity in risk of B cell lymphoma by age at first contact was detected for contact with cattle, poultry and swine. Contact with household pets has been suggested to be inversely associated with lymphoma risk. We tested the hypothesis in a case–control study of lymphoma in the Sardinia region of Italy. Cases were 326 patients, first diagnosed with lymphoma in 1999–2003. Controls were 464 population controls, frequency matched to cases by age, gender, and area of residence. In person interviews included self-reported household contact with pets and birds, type of pet(s), and age at starting contact. Frequent contact with birds was inversely associated with lymphoma, and particularly B-cell non-Hodgkin lymphoma (odds ratio [OR] = 0.6, 95% confidence interval [95% CI]: 0.4, 0.9). Contact with chickens accounted for this inverse association, which was strongest for first contact occurring at age ≤8 years (OR = 0.4, 95% CI: 0.2, 1.0). No association was observed when first contact occurred at age 9 or older. Contact with any pets was inversely associated with risk of diffuse large B-cell lymphoma (OR = 0.4, 95% CI: 0.2, 1.0), but not other lymphoma subtypes. Our results support the hypothesis that early-life exposure to pets, birds and particularly with chickens might be associated with a reduced risk of lymphoma.

Background: Ethylene oxide, a high-volume commodity, is an established human carcinogen, although the relevant epidemiologic evidence is limited. Methods: We explored the association between occupational exposure to ethylene oxide and risk of lymphoma in a case-control study, including 2347 lymphoma cases first diagnosed in 1998-2004 and 2463 controls, from 6 European countries. The diagnosis of lymphoma was based on the 2001 World Health Organization Classification of lymphoma. Occupational exposure to ethylene oxide was retrospectively assessed by industrial hygienists and occupational physicians based on detailed self-reported information. We modeled risk of lymphoma with unconditional logistic regression analysis as a function of various exposure measures, adjusting for age, sex, and participating center. Results: Thirty-one cases and 27 controls (1.2% of the total study population) were defined as ever having been exposed to ethylene oxide (odds ratio = 1.3 [95% confidence interval [CI] = 0.7-2.1]). Lymphoma risk showed a 4.3-fold increase associated with medium-high frequency of exposure to ethylene oxide (95% CI = 1.4-13). Among major subtypes, chronic lymphocytic leukemia was consistently associated with ethylene oxide exposure, related in a dose-response manner to probability, frequency, and duration of exposure, as well as to cumulative exposure and (less definitively) with exposure intensity. Conclusions: Our results add to the evidence that ethylene oxide is a human carcinogen. © 2010 by Lippincott Williams & Wilkins.

23. Características y costos directos de infecciones respiratorias agudas en niños de guarderías infantiles

Se evaluó mediante estudio descriptivo, características y costos directos de infecciones respiratorias en 140 niños que asisten a sala cuna y jardín infantil. Se consignó diagnóstico, exámenes, fármacos, kinesioterapia, ausentismo, licencias médicas y hospitalización. Se analizan 202 eventos respiratorios en 90 niños: 75 (37,1%) en 30 lactantes y 127 (62,8%) en 60 pre-escolares. Hubo 61,3% de infecciones respiratorias bajas en lactantes versus 39,4% en pre-escolares (p < 0,002). Existió indicación de antimicrobianos en 42,6% de las consultas. Se encontró mayor costo en lactantes respecto de consulta médica, kinesioterapia, exámenes de laboratorio y pago de licencias médicas o permisos (p < 0,001). El costo promedio total de una infección respiratoria alcanzó a $ 68.577 en lactantes y $ 28.130 en pre-escolares (p < 0,001). Con predominio de infecciones bajas en lactantes, elevado consumo de antimicrobianos y alto impacto económico, esta realidad amerita focalizar programas de intervención The clinical profile of respiratory infections in 140 children who attend a day care center was described in a prospective study and direct costs were estimated. The costs estimation included diagnostic procedures, laboratory exams, drugs, kinesitherapy, parents' absenteeism, medical certificates and hospitalization. Two hundred and two respiratory episodes in 90 (64.2%) children, 75 (37.1%) in 30 infants and 127 (62.8%) in 60 toddlers were analyzed. There were 61.3% lower respiratory infections in infants versus 39.4% in toddlers (p < 0.002). Antimicrobials were prescribed in 42.6% of the medical attendance. Infants represent major costs regarding medical consultations, kinesitherapy, laboratory tests and refunds for medical certificates or leave of absence (p < 0.001). The mean global cost of a respiratory infection was US $ 129,3 for infants and US $ 53,0 for toddlers (p < 0.001). This situation needs the development of an intervention program

24. Attitudes of registered nurses and student nurses to disabled children

- Abstract
The purpose of this study was to examine and compare the attitudes of registered nurses and student nurses towards disabled children. A convenience sample consisting of 99 registered nurses employed in paediatric hospitals and 189 student nurses was selected. The instrument used for data collection was the 'Attitudes Towards Disabled Persons' (ATDP) scale. Descriptive statistics and one-way analysis of variance were performed to test for significant differences. On the whole subjects' attitudes appeared to be poor. Student nurses had more positive attitudes in comparison to registered nurses; however, the difference was not statistically significant. There were significant differences between the different age groups and between student groups within different education settings. There were no significant differences in gender. Actions to improve subjects' attitudes towards disabled children are discussed and suggested.

Methods In 1998–2003, 2337 incident lymphoma cases and 2434 controls participated in the EPILYMPH case-control study in six European countries. A detailed occupational history was collected in cases and controls, including species of breeding animals, their approximate number, and circumstances of contact. We conducted a preliminary analysis on ever exposed to contact with breeding animals, and we stratified the analysis by age at first exposure, whether before or after 12. The OR and its 95% CI was calculated with unconditional logistic regression for all lymphomas, and its major subtypes, adjusting by age, gender, and education. Results Lymphoma risk (all subtypes combined) did not increase among those exposed to contact with breeding animals (OR = 1.0, 95% CI 0.8 to 1.2). Risk of DLBCL was significantly lower among subjects employed in poultry farms (OR = 0.6, 95% CI 0.4 to 1.0). This inverse association was observed among subjects who started exposure before or at age 12 (OR = 0.5, 95% CI 0.2 to 1.1), but not later. Conclusions Early occupational contact with poultry might be associated with a decrease in risk of specific lymphoma subtypes.

Objectives A medical history of allergy, and particularly asthma, has been associated with an inverse risk of non-Hodgkin’s lymphoma (NHL). As occupational exposure to specific organic dusts is a risk factor for asthma, we explored risk of lymphoma and its major subtypes in relation to organic dusts. Method In 1999–2004, 324 incident lymphoma cases and 464 population controls, frequency matched to cases by age and gender, were recruited among adult residents in Sardinia, Italy. Expert industrial hygienists assessed exposure to organic dust overall, and specific organic dusts. The odds ratio (OR) for lymphoma (all types) and its major subtypes, and its 95% confidence interval, was calculated using unconditional logistic regression. Results Exposure to organic dust in general was inversely associated with risk of lymphoma (all types) (OR = 0.7, 95% CI 0.4–1.2), with a declining trend by duration and level of exposure. The inverse association was apparently more pronounced for exposure to flour dust and wood dust, but not to natural or artificial textile fibres. A consistent inverse risk was observed for B-cell lymphoma (OR = 0.6, 95% CI 0.3–1.0), and it was likewise for its major subtypes, namely diffuse large cell lymphoma (DLBCL), follicular lymphoma (FL) and chronic lymphocytic leukaemia (CLL). Age Conclusions Although with interpretative limitations due to the small study size, our results suggest that exposure to flour dust and wood dust might contribute a reduction in risk of malignant lymphoma.

We report a patient with an iatrogenic vertebro-vertebral fistula responsible for a cerebellar syndrome and a cervical bruit. Endovascular treatment of the fistula was carried out using electrically detachable coils. An angiogram 5 months after treatment showed that the fistula had totally disappeared.

Objective This study aimed to evaluate the association between polypharmacy and delirium, the association of specific drug categories with delirium, and the differences in drug-delirium association between medical and surgical units and according to dementia diagnosis. Methods Data were collected during 2 waves of Delirium Day, a multicenter delirium prevalence study including patients (aged 65 years or older) admitted to acute and long-term care wards in Italy (2015-2016); in this study, only patients enrolled in acute hospital wards were selected (n = 4,133). Delirium was assessed according to score on the 4 "A's" Test. Prescriptions were classified by main drug categories; polypharmacy was defined as a prescription of drugs from 5 or more classes. Results Of 4,133 participants, 969 (23.4%) had delirium. The general prevalence of polypharmacy was higher in patients with delirium (67.6% vs 63.0%, P = .009) but varied according to clinical settings. After adjustment for confounders, polypharmacy was associated with delirium only in patients admitted to surgical units (OR = 2.9; 95% CI, 1.4-6.1). Insulin, antibiotics, antiepileptics, antipsychotics, and atypical antidepressants were associated with delirium, whereas statins and angiotensin receptor blockers exhibited an inverse association. A stronger association was seen between typical and atypical antipsychotics and delirium in subjects free from dementia compared to individuals with dementia (typical: OR = 4.31; 95% CI, 2.94-6.31 without dementia vs OR = 1.64; 95% CI, 1.19-2.26 with dementia; atypical: OR = 5.32; 95% CI, 3.44-8.22 without dementia vs OR = 1.74; 95% CI, 1.26-2.40 with dementia). The absence of antipsychotics among the prescribed drugs was inversely associated with delirium in the whole sample and in both of the hospital settings, but only in patients without dementia. Conclusions Polypharmacy is significantly associated with delirium only in surgical units, raising the issue of the relevance of medication review in different clinical settings. Specific drug classes are associated with delirium depending on the clinical setting and dementia diagnosis, suggesting the need to further explore this relationship.

To se osobito očituje kroz utjecaj transeuropskog romantičarskog pokreta i kroz strategije kulturnoga nacionalizma, što se nastoji pokazati analizom estetskih i ideoloških elemenata drame "Teuta" hrvatskog romantičarskog pisca Dimitrija Demetra i epa "Hiawatha" američkog romantičarskog pjesnika Henryja Wadswartha Longfellowa.

- Abstract
Background: The Basic Health Unit (Unidade Básica de Saúde - UBS, in Portuguese) is the first point of contact in the public healthcare system for people with epilepsy. Primary care professionals need to appropriately diagnose, treat, and refer, if necessary, to tertiary services., Objective: To evaluate the knowledge of UBS professionals on the management of patients with epilepsy in Rio de Janeiro., Methods: Online questionnaires were performed on the topic of epilepsy before and after exposure to classes taught by epileptologists., Results: A total of 66 doctors participated, 54.5% of whom were residents or trained in family medicine. The majority had from 1 to 3 years of practice. Insecurity prevailed in the management of pregnant women and the elderly. Around 59.1% of the participants referred patients with seizures without examinations. A total of 78% of the participants did not correctly classify seizure types, and 2/3 did not define drug-resistant epilepsy. Induction and broad-spectrum drugs were common. The therapeutic decision depended on availability in the basic health unit (UBS) (81.8%), dosage (60.6%), side effects (34.8%), and age (36.4%). Comorbidities and sex influenced 1/4 of the sample. For 23% of the participants, the type of crisis did not affect the choice. Regarding typical non-pharmacological options, 75% of the participants were aware of cannabidiol, 40.9% of surgery, 22.7% of ketogenic diet, and 22.8% of deep brain stimulation/vagus nerve stimulation (DBS/VNS). A total of 90.2% indicated the need for training., Conclusion: There are deficits in the knowledge of UBS professionals in the management of epilepsy. Introduction: The Ruche test is a visuospatial form of the Rey auditory verbal learning test (RAVLT), with initial evidence of utility in the diagnosis of temporal lobe epilepsy (TLE)-related memory disorders., Aims: To present the translation to Brazilian Portuguese and modification of the Ruche test (RUCHE-M) and compare the RUCHE-M and RAVLT performance between patients with right and left TLE., Methods: Twenty-five neuropsychologists participated in instrument adaptation. Thirty-seven patients with right ( n = 19) and left ( n = 18) TLE participated. Data were compared with the Mann-Whitney U test., Results: All specialists considered the final RUCHE-M to be adequate. The RUCHE-M forgetting speed index (FSI) score and several RAVLT scores differed significantly between patients with right and left TLE., Conclusion: The RUCHE-M showed limited utility for the assessment of visuospatial episodic memory in patients with TLE. The manipulation of memory binding as demonstrated by FSI score seems to be a promising paradigm for the assessment of right hippocampal function.

- Abstract
Background: The effects of the selective serotonin reuptake inhibitors (SSRIs), the first-line antidepressant treatment, have been proposed to be affected, at least in part, by the living environment. Since the quality of the environment depends not only on its objective features, but also on the subjective experience, we hypothesized that the latter plays a key role in determining SSRI treatment outcome., Methods: We chronically administered the SSRI fluoxetine to two groups of adult CD-1 male mice that reportedly show distinct subjective experiences of the environment measured as consistent and significantly different responses to the same emotional and social stimuli. These distinct socioemotional profiles were generated by rearing mice either in standard laboratory conditions (SN) or in a communal nest (CN) where three dams breed together their offspring, sharing caregiving behavior., Results: At adulthood, CN mice displayed higher levels of agonistic and anxiety-like behaviors than SN mice, indicating that they experience the environment as more socially challenging and potentially dangerous. We then administered fluoxetine, which increased offensive and anxious response in SN, while producing opposite effects in CN mice. BDNF regulation was modified by the treatment accordingly., Limitations: Subjective experience in mice was assessed as behavioral response to the environment., Conclusions: These results show that the subjective experience of the environment determines fluoxetine outcome. Objective: To illustrate the clinical features and management of pneumatocoele presenting with a trap-valve mechanism after cochlear implantation with lateral petrosectomy (LP) in comparison with literature., Introduction: Pneumatocoele is a rare complication of cochlear implant (CI) surgery, generally managed with conservative treatments., Methods: We describe a progressively increasing pneumatocoele with a trap-valve mechanism occurring one year after CI with LP, successfully managed with revision surgery. Literature review was performed on this topic., Results: Our case was the second ever reported in literature. It appeared consequently to forceful nose-blowing in a patient with refractory nasal polyposis. Unlike the pneumatocoeles reported after standard CI, we advised revision surgery as well as in the case of pneumatocoele after staged LP and CI. In both the patients subcutaneous air collection occurred as a late complication and an insufficient Eustachian tube closure was disclosed, which is considered a complication of LP itself. Surgical management was necessary in consideration of the risk of ascending infections associated with Eustachian tube patency in LP with CI., Conclusion: We consider surgical treatment mandatory in pneumatocoele after CI with LP, it is indeed the sentinel of the restoration of Eustachian tube patency and the potential prelude to major complications.

46. Assessing dental surgeons' understanding of bisphosphonates: Implications for patient health in oral surgery.

Objectives: This study aimed to analyze the knowledge of dental surgeons concerning the pharmacological effects of bisphosphonates (BP) and their impact on the health of patients undergoing oral surgery treatment., Materials and Methods: A quantitative study was conducted with professionals in the Cariri region of Ceará, Brazil. The data were collected using a semi-structured questionnaire to interview the dental surgeons who met the inclusion criteria. The data were analyzed by Chi-square and Fischer's Exact tests using the SPSS program version 22.0., Results: The results indicated that most interviewed subjects have no experience with the dental treatment of individuals under treatment with bisphosphonates. When a sample of the medication was presented, a significant number of the participants (65.3%) did not recognize the drug. Among those who recognized it, 66.5% did not know how to treat bisphosphonate-using patients in the clinical routine. However, 95.1% of the professionals who recognized the need to discontinue these drugs admitted contacting the prescriber to establish the best dental conduct. Our results also showed that recently graduated professionals better understood the need to stop BP use before invasive treatment, although they did not know the protocol., Conclusion: In conclusion, a better knowledge of the pharmacological effects of BP is crucial for conducting adequate anamnesis, requesting additional tests, and performing medical monitoring, which has a significant impact on the clinical practice of dental surgeons., Competing Interests: The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (© 2023 THE AUTHORS.)

47. Predictive factors for successful vagus nerve stimulation in patients with refractory epilepsy: real-life insights from a multicenter study.

- Abstract
Introduction: Vagus nerve stimulation (VNS) therapy is an established treatment for patients with drug-resistant epilepsy that reduces seizure frequency by at least 50% in approximately half of patients; however, the characteristics of the patients with the best response have not yet been identified. Thus, it is important to identify the profile of patients who would have the best response to guide early indications and better patient selection., Methods: This retrospective study evaluated vagus nerve stimulation (VNS) as an adjuvant therapy for patients with drug-resistant epilepsy from six epilepsy centers in Brazil. Data from 192 patients aged 2-66 years were analyzed, and all patients received at least 6 months of therapy to be included., Results: Included patients were aged 2-66 years (25.6 ± 14.3), 105 (54.7%) males and 87 (45.8%) females. Median follow-up interval was 5 years (range, 2005-2018). Overall, the response rate (≥50% seizure reduction) after VNS implantation was 65.6% (126/192 patients). Most patients had 50-90% seizure reduction (60.9%) and nine patients became seizure-free. There were no serious complications associated with VNS implantation. The rate of a ≥ 50% seizure reduction response was significantly higher in patients with no history of neurosurgery. The presence of focal without generalized seizures and focal discharges on interictal EEG was associated with better response. Overall, etiological predictors of a better VNS response profile were tumors while a worse response to VNS was related to the presence of vascular malformations and Lennox-Gastaut Syndrome., Discussion: We observed an association between a better response to VNS therapy no history of neurosurgery, focal interictal epileptiform activity, and focal seizure pattern. Additionally, it is important to highlight that age was not a determinant factor of the response, as children and adults had similar response rates. - Abstract
The proper maturation of emotional and sensory circuits requires fine-tuning of serotonin (5-HT) level during early postnatal development. Consistently, dysfunctions of the serotonergic system have been associated with neurodevelopmental psychiatric diseases, including autism spectrum disorders (ASD). However, the mechanisms underlying the developmental effects of 5-HT remain partially unknown, one obstacle being the action of 5-HT on different cell types. Here, we focused on microglia, which play a role in brain wiring refinement, and we investigated whether the control of these cells by 5-HT is relevant for neurodevelopment and spontaneous behaviors in mice. Since the main 5-HT sensor in microglia is the 5-HT
2B receptor subtype, we prevented 5-HT signaling specifically in microglia by conditional invalidation of the Htr2b gene in these cells. We observed that abrogating the serotonergic control of microglia during early postnatal development affects the phagolysosomal compartment of these cells and their proximity to dendritic spines and perturbs neuronal circuits maturation. Furthermore, this early ablation of microglial 5-HT2B receptors leads to adult hyperactivity in a novel environment and behavioral defects in sociability and flexibility. Importantly, we show that these behavioral alterations result from a developmental effect, since they are not observed when microglial Htr2b invalidation is induced later, at P30 onward. Thus, a primary alteration of 5-HT sensing in microglia, during a critical time window between birth and P30, is sufficient to impair social and flexibility skills. This link between 5-HT and microglia may explain the association between serotonergic dysfunctions and behavioral traits like impaired sociability and inadaptability to novelty, which are prominent in psychiatric disorders such as ASD., (© 2023. The hypercoagulable state of COVID-19 is resulting in an increasing number of unexpected venous and arterial thromboses in patients. We report a case of subacute dual coronary stent thrombosis in the setting of COVID-19 and we provide a brief review of current management recommendations.

- Abstract
Background: Lacosamide (LCM) is a third-generation anti-seizure drug approved in Europe and the United States, either as a monotherapy or adjunctive therapy, to treat partial-onset seizures in adults, adolescents, and children. In Brazil, LCM is licensed for treatment only in patients older than 16 years of age., Objective: To evaluate a cohort of children presenting with refractory epilepsy who received LCM as an add-on therapy and observe the response and tolerability to the LCM treatment., Methods: A retrospective cohort study conducted in a tertiary health care facility, which included 26 children, aged up to 16 years, who presented with refractory epilepsy and received LCM as an add-on treatment. The follow-up visits were scheduled every 3 months until 9 months of treatment with LCM., Results: After 3 months of LCM administration, in 73.1% of the children, there was a reduction of > 50% in the frequency of seizures, and this clinical improvement was maintained in most patients (73.9%) for the following 9 months. Mild (such as, somnolence and behavioral changes) or severe (seizure worsening) adverse effects were observed in two and three children respectively. Among responders to LCM, there was a higher prevalence of males, fewer concomitant anti-seizure drugs, and lower percentage of patients using sodium channel blockers., Conclusions: Lacosamide should be considered as an early treatment option in pediatric patients with refractory epilepsy, mainly focal seizures., Competing Interests: The authors have no conflict of interests to declare., (Academia Brasileira de Neurologia.
