Abstract: Background and aims: Non-invasive methods accurately estimating hepatic venous pressure gradient (HVPG) are un unmet clinical need. Preliminary data suggested that graph analysis of dynamic contrast enhanced ultrasonography (DCE-US) of the liver using a “connectome” approach allows assessment of the liver microcirculatory derangement and mirrors the severity of portal hypertension (Amat-Roldan et al. Radiology 2015). The EC-funded prospective CLEVER study (FP7-IAPP-GA-2013-612273-CLEVER) is aimed at developing a novel automatized software based on DCE-US able to improve prognostication in cirrhosis. First extended results were developed with a Siemens Acuson Sequoia in Barcelona, showing optimal correlation with HVPG. Here we report the adaptation of this CLEVER software to DCE-US videos acquired with ESAOTE MyLab equipments in Bologna to predict HVPG in a population of patients with F≥3 hepatopathy. Method: Ten seconds long videoclip(s) of the right liver lobe were recorded in each patient producing one cycle of microbubble disruption and reperfusion during SonoVue infusion. A total of 90 videos from randomly selected 47 patients were utilized to optimize the autoselection algorithm of the computer among 5models based on platelet count and spleen diameter. Results: Applicability: the CLEVER software was technically able to provide portal pressure estimations from DCE-US in 41/90 videos corresponding to 28/47 patients (59.6%). The Spearman coefficient of correlation between CLEVER values and HVPG was r = 0.585 ( p < 0.001). The CLEVER software was then tested in a separate validation set of 17 technically successful patients, showing a correlation r = 0.701 ( p < 0.002). Conclusion: We developed and validated the DCE-US based CLEVER software which allows an automatic and quantitative non-invasive estimation of portal pressure in patients with CLD. Larger set of patients with precise subgrouping will help improving the non-invasive predictability of portal pressure by DCE-US.

Abstract: cAMP generated in response to β-adrenoceptor stimulation modulates excitation-contraction coupling via activation of PKA. It is now widely accepted that cAMP signalling is compartmentalised in the heart and that PDEs, the only cAMP degrading enzyme, play a central role in spatial control of signal propagation. We have previously demonstrated that PKA-I and PKA-II isoforms are selectively activated by spatially confined pools of cAMP leading to phosphorylation of specific downstream targets. It has been suggested that selective deregulation of cAMP/PKA signals in specific subcellular compartment may lead to cardiac pathology. Here we investigate cAMP signalling in an in vitro model of cardiomyocyte hypertrophy and explore the role of different PDE families in the control of the local cAMP response to β-adrenoceptor stimulation. We used a real-time FRET-based imaging approach to monitor cAMP dynamics in adult myocytes. Adenoviral vectors carrying RI or RII Epac were generated by fusing the N-terminus of the cAMP FRET reporter Epac-1 and the dimerisation-docking (DD) domain of the regulatory subunit of PKA-I or PKA-II. Adult rat cardiac myocytes expressing the cAMP reporters were treated overnight with 1μM norepinephrine (NE) as a model of in vitro cardiomyocyte hypertrophy. In control cells, upon stimulation with 100nM isoproterenol, we detected a significantly higher cAMP response in the PKA-II compartment compared to the PKA-I compartment. In NE-treated myocytes, the differences between these compartments were abolished. To investigate which PDE family may be responsible for these changes in cAMP compartmentalisation, experiments were performed in the presence of selective PDEs inhibitors. We found that when PDE2 was inhibited with BAY60-7550 in control cells the resulting increase in cAMP was significantly greater in the PKA-I than in the PKA-II compartment. In hypertrophic myocytes, PDE2 inhibition resulted in a much greater increase in cAMP in the PKA-II compartment compared to control cells. Similar results were found with selective PDE4 inhibition with rolipram. Selective inhibition of PDE3 with cilostamide showed a small but significant rise in cAMP response in the PKA-I compartment in control cells. This was not replicated in hypertrophic myocytes. Our findings indicate that spatial control of cAMP/PKA signalling is altered in this in vitro model of hypertrophy, suggesting that in hypertrophic myocytes mechanisms may be activated resulting in altered localisation and/or activity of PDEs. Further studies will be necessary to fully determine the mechanism responsible for the observed changes.

Abstract: Respiratory disorders in premature are outlined as one of the major causes of newborn morbidity. Therefore, estimation in utero of the risk that a particular newborn will present respiratory distress syndrome is critical in perinatal medicine. Fetal lung maturity assessment by non-invasive methods is an unsolved problem since 80's. There has been a significant effort reporting that ultrasound images carry information associated with changes of fetal lung microstructure but it required the lung-to-liver measurements to be calibrated, which limits clinical feasibility. In this work, we explored the association between image features only from a fetal lung computed by a previously reported probabilistic model and fetal lung maturity. Two studies validated image features obtained from different US equipments: 957 scans were correlated to gestational age (r>0.97); and 83 scans were modeled to estimate a normalized lung maturity index in 7 blind samples. Bootstrapping provided median r>0.82.

Abstract: Objectives Diagnosis of white matter damage by cranial ultrasound imaging is still subject to interobserver variability and has limited sensitivity for predicting abnormal neurodevelopment later in life. In this study, we evaluated the ability of a semiautomated method based on ultrasound texture analysis to identify patterns that correlate with the ultrasound diagnosis of white matter damage. Methods The study included 44 very preterm neonates born at a median gestational age of 29 weeks 3 days (range, 26 weeks-31 weeks 6 days). Patients underwent cranial ultrasound scans within 1 week of birth and between 14 and 31 days of life. Periventricular leukomalacia was diagnosed by experienced clinicians on the 14- to 31-day scan according to standard criteria. To perform the texture analysis, 4 regions of interest were delineated in stored images: left and right periventricular areas and choroid plexuses. A classification algorithm was developed on the basis of the best combination of texture coefficients to correlate with the clinical diagnosis, and the ability of this algorithm to predict a later diagnosis of periventricular leukomalacia on the first scan was evaluated using a leave-one-out cross-validation. Results Periventricular leukomalacia was diagnosed by the standard procedure in 14 of 44 neonates. The texture classification algorithm performed on the first scan could identify cases with a later diagnosis of periventricular leukomalacia with sensitivity of 100% and accuracy of 97.7%. Conclusions These data support the notion that semiautomated quantitative ultrasound analysis achieves early identification of changes in subclinical stages and warrant further investigation of the role of ultrasound texture analysis methods to improve early detection of neonatal brain damage.

Abstract: Cardiovascular diseases are one of the main causes of mortality in the world. For this reason it is important to develop techniques that allow for better understanding of cardiac remodelling prior to heart failure. In this line, Second Harmonic Imaging (SHI) provides a label-free microscopy approach to image cardiac fibers. In this work, we automated means for measuring average sarcomere length, intrasarcomeric A-band distance and sarcomere fiber density as such features have been repeatedly correlated with major cardiomyopathies in the literature (previously measured by manual means). We tested our approach on healthy cardiac tissue of rabbits and human samples and compared such results with manual measurements on the same images to validate our methods. We also measured morphometric differences in cardiac fibers from our animal model which undergoes an irregular heart development. We then conclude that our automated measurements enable reliable characterization of cardiac fiber tissues.

Abstract: Diagnosis of white matter damage by neonatal cranial ultrasound (CrUS) is subject to inter-observer variability and has a low sensitivity to detect late abnormal neurodevelopment in life. In the last decades there have been a significant effort reporting that statistical features of ultrasound images carry important information associated with changes of tissue microstructure. In this work we explored the ability of a semi-automated image processing method to associate ultrasound texture patterns with Neonatal Behavioral Assessment Scale (NBAS) performance in premature neonates. A total of ninety infants born at a median gestational age of 29 weeks were included. The infants underwent one CrUS scan performed at the same day that NBAS test. In this work, we developed a feature selection algorithm to identify combination of features that correlated to NBAS clusters. Our algorithm was then able to predict individual underscored NBAS clusters with accuracy higher than 80% in a “blind” sample.

Abstract: Polarization sensitive second harmonic generation (PSHG) provides additional information in intensity only SHG imaging. In particular, it offers the SHG effective orientation of the implicated SHG active structures. Assuming that those structures possess cylindrical symmetry, the supplementary contrast is based on the ratio of two non-vanishing, independent elements of the χ 2 tensor. This ratio is experimentally extracted by fitting pixel by pixel a theoretical model to the PSHG images and by finding the maximum frequency value or the mean of the consequent pixels' histogram. In the present study we show that the above χ 2 elements' ratio critically depends on the tilted-off the plane SHG active structures. We performed PSHG in different z-planes of a starch granule (presenting radially oriented amylopectin molecules, the SHG source in starch) and we found different pick values of the χ 2 elements' ratio histogram for each plane. By assuming a fixed value for the χ 2 elements' ratio, we present here a generalized three dimensional (3D) model that determines the 3D orientation of the SHG active structures.

Abstract: In this paper, we show that the neuronal fillopodia can detect and respond to the presence of focused femtosecond laser light at a distance, an effect that does not occur when CW illumination is used. Control and manipulation of axonal growth is of extreme importance in the field of neurosciences as it could lead to advances such as in-vivo nerve regeneration or arbitrary patterning of neural circuits.

Abstract: In this paper we will describe a new technique that allows, for the first time, an analytical determination of the complex electric fields of two different pulses which have the same central frequency. This is done by performing a spectrally resolved analysis, in the Fourier domain, of a collinear cross-correlation. Blind-MEFISTO has the additional advantages in that it enables a simple extraction of pulse information without the need of an iterative retrieval algorithm and without having some of the ambiguities that are present in other techniques.

Abstract: In this paper we will describe general methodology that allows the phase of an unknown pulse to be analytically obtained. This technique brings together time-frequency and interferometric techniques while at the same time it maintains robust error checking capabilities. Besides, it does not rely on the use of iterative retrieval algorithms and does rely on Fourier analysis. Our methodology only requires a simple collinear autocorrelator whose output is spectrally resolved as a function of delay. We call this technique Measurement of Electric Field by Interferometric Spectral Trace Observation (MEFISTO).

Abstract: We have investigated the characteristics of ultrashort pulses as they propagate through a microscope. We have observed the dispersive symmetry of the objective lens by characterising the pulses at the sample plane and showing how the amplitude and the phase increase as the pulses are scanned away from the centre of the lens. Importantly, since starch has a very large phase matching bandwidth, the technique outlined here can be applied to even shorter pulses (>15 fs) where the dispersive effects will be far more dramatic.

Abstract: Purpose: Myosin binding protein C is an accessory protein of vertebrate striated muscle thick filaments. Many studies have aimed to elucidate both its structure and function since mutations in the cardiac isoform (cMyBP-C) gene (MYBPC3) are known to be a leading cause of familial hypertrophic cardiomyopathy (HCM) affecting millions of people worldwide. There is strong evidence that phosphorylation of N-terminus M-domain of cMyBP-C is essential for normal cardiac function and that their dephosphorylation is associated with contractile dysfunction and HCM. However, the details of the underlying molecular-level ultrastructure are still poorly understood. The aim of this work is to provide details of the ultrastructure of thick filaments in different states of cMyBP-C phosphorylation. Methods: Two transgenic mouse strains expressing a constitutive phosphorylation or a non-phosphorylatable cMyBP-C have been compared to a homozygous knockout mouse for cMyBP-C and a wild-type (WT) control. We have carried out detailed transmission electron microscopy (TEM) in intact cardiac muscles, and electron tomography (ET) combined with sub-tomographic averaging. Results: TEM shows that cardiomyocyte ultrastructure is relatively well maintained under the total deletion of cMyBP-C from the heart and due to the expression of constitutive phosphorylation of cMyBP-C. However, the constitutive expression of non-PKA-phosphorylatable cMyBP-C leads to both sarcomeric disarray and mitochondria disruption. Longitudinal views of the average 3D reconstructions employing ET illustrate the surface density features of thick filaments. In the WT, two projection masses can be identified originating from cMyBP-C. Both the absence of cMyBP-C and the presence of constitutive non-phosphorylatable cMyBP-C seem to be associated with poorer structural preservation of the thick filaments. Conclusions: This study shows that cMyBP-C constitutive phosphorylation effectively preserves the intracellular arrangement of cardiac myocytes. However, non-PKA-phosphorylatable cMyBP-C leads to a dramatic overall decrease of tissue organization which is much more deleterious than the total deletion of cMyBP-C. This finding provides a structural explanation for cardiac dysfunction in this sample. This study shows that chemically fixed samples can be used for ET to reveal 3D structural information of thick filaments. Results obtained by using this methodology are in good agreement with previous studies using isolated filament approaches. However, in order to obtain higher resolution, advanced methods of sample processing will be required.

Abstract: Contraction in muscle occurs by the relative sliding of thick and thin filaments. The thick filaments are composed by myosin molecules, of which two main isoforms have been found in ventricles, the alpha- and beta-myosin heavy chain (MHC) homodimers. The expression of the two MHCs is developmentally regulated and controlled by numerous stimuli. Polarization Second Harmonic Generation (PSHG) enables the computation of the average helical pitch angle of peptides (AHPAP) of a number of proteins arranged in a fiber structure, like collagen or myosin. AHPAP of these proteins has been used to discriminate them in tissue. We also reported that AHPAP of cardiac tissue correlates consistently with aging of cardiac LV in rabbits and beta-MHC distribution. This study reports a molecular model based on public DNA sequences that directly explains the experimental differences of AHPAP of unlabeled histological sections of cardiac tissue expressing different MHC isoforms. We computed all second-order tensor elements from the 3D molecular distribution of peptides and its associated hyperpolarizability of the S2 subfragment of the rod domain of MHC for different isoforms, species and mutations: for human, MYH6 (alpha), MYH7 (beta) and E924K mutation; for mouse, MYH6 and MYH7; for rabbit, MYH7. Based on a reported biophysical model we computed the expected AHPAP for each isoform and species. We finally acquired PSHG images of transversal histological sections in three animal models: healthy mouse, which naturally expresses MYH6; a mouse fed a iodine-deficient diet containing 0.15% 6-propyl-2-thiouracil for 4 weeks which induces hyperthyroidism and subsequent shift to MYH7, and 1 year old rabbit which naturally expresses 95% of MYH7 of the heart myosin. We also computed the average AHPAP for the experimental data. We found agreement between experimental PSHG calculations and molecular model of 3D protein structure: for mouse, MYH6: 67,8° and 67,4°, MYH7: 71,3° and 71,2°; for rabbit MYH7: 73,3° and 72,5°. We then extended our results to 3D protein models of human isoforms and mutations: MYH6: 73,3°, MYH7: 63,7°, and E924K mutation: 66,0°. This study supports the hypothesis that PSHG can measure subtle differences in molecular structure of myosin that are available only through much more complex techniques (i.e. crystallography). We report a direct explanation for the experimental differences in PSHG signal. This suggests that PSHG can provide in-situ and in-vivo tissue information that might help to quantify the expression and spatial distribution of myosin isoforms and mutations in tissue.

Abstract: Background and aims: Ultrasound diagnosis of white matter damage (WMD) is still subject to variability, and methods based on quantitative ultrasound analysis are under investigation to improve diagnostic accuracy. The correlation between WMD and cerebellar changes in term and one-year old infants has previously been described. We tested the hypothesis that a semiautomated method for ultrasound texture analysis (AQUA) could detect patterns to predict the standard clinical diagnosis of WMD in preterm neonates. Methods: A cohort of 40 very preterm infants (< 32 weeks) underwent 2 cranial US scans within the first and after 2 weeks of life. WMD was diagnosed using standard criteria. Texture analysis was run in the cerebellum, manually delineated in a coronal plane through the anterior fontanelle. A classification algorithm based on texture coefficients was used to correlate to clinical diagnosis and its ability to predict in the first scan a later diagnosis of WMD was then evaluated. Results: Median gestational age was 29+4 weeks (26+0-31+6). WMD was diagnosed in 10/40 infants. Cerebellar texture analysis in the first scan could identify cases later diagnosed with PVL with a sensitivity of 100% and an accuracy of 95%, after adjustment for gestational age and birth weight. Conclusions: The cerebellum might contain useful information that improves the prediction of standard white matter damage diagnosis. These preliminary data support further investigation of the role of this brain structure in the neonatal diagnosis of WMD by means of semi-automated quantitative ultrasound analysis.

The pulses are characterized using a nonlinear microscope.

Abstract: Purpose: Raman spectroscopy allows molecular changes to be quantified in vivo from the tissues like the retina. Here we aimed to assess the metabolic changes in the retina of patients with multiple sclerosis (MS)., Methods: We built a Raman spectroscopy prototype by connecting a scanning laser ophthalmoscope to a spectrophotometer. We defined the spectra of 10 molecules participating on energy supply, axon biology, or synaptic damage, which have been shown to be altered in the brain of patients with MS: cytochrome C, flavin adenine dinucleotide (FAD), nicotinamide adenine dinucleotide (NADH), N-acetyl-aspartate (NAA), excitotoxicity, glutamate, amyloid β (Aβ), τ and α-synuclein (SNCA), phosphatidyl-ethanolamine, and phosphatidyl-choline. We studied these molecules in a prospective cohort of patients with MS, either in the chronic phase or during relapses of acute optic neuritis (AON)., Results: Significant changes to all these molecules were associated with age in healthy individuals. There was a significant decrease in NADH and a trend toward a decrease in NAA in patients with MS, as well as an increase in Aβ compared with healthy controls. Moreover, NADH and FAD increased over time in a longitudinal analysis of patients with MS, whereas Aβ diminished. In patients with acute retinal inflammation due to AON, there was a significant increase in FAD and a decrease in SNCA in the affected retina. Moreover, glutamate levels increased in the affected eyes after a 6-month follow-up., Conclusions: Alterations of molecules related to axonal degeneration are observed during neuroinflammation and show dynamic changes over time, suggesting progressive neurodegeneration.

Abstract: Purpose: To test whether graph analysis of vascular images obtained with hepatic dynamic contrast material-enhanced (DCE) ultrasonography (US) allows calculation of the degree of organization of the liver circulation and whether graph properties are correlated to the severity of portal hypertension., Materials and Methods: Institutional review board approval and written informed consent were obtained. Fifteen patients with liver cirrhosis (nine men; mean age ± standard deviation, 55 years ± 8) who underwent DCE US and hepatic venous pressure gradient (HVPG) measurement and four healthy subjects (two men and two women; mean age, 34 years ± 4) were included between January 2007 and December 2008. Individual graph models ("vascular connectomes") were computed on the basis of time series analysis of video sequences of DCE US examinations (conducted with the disruption-reperfusion technique). Graph analysis was performed, and the clustering coefficient C was calculated. Correlations between clustering coefficient and HVPG were assessed., Results: Healthy subjects had a high clustering coefficient of vascular connectome (C = 0.4447; interquartile range [IQR], 0.3864-0.4679), suggesting a highly organized hepatic vascular network. Conversely, patients with cirrhosis showed a low clustering coefficient, indicating disruption of normal anatomy (C = 0.0288; IQR, 0.0157-0.0861; P = .001 vs healthy subjects). The clustering coefficient decreased as HVPG increased, with a clustering coefficient of 0.0237 (IQR, 0.0066-0.0378) in patients with HVPG of at least 10 mm Hg versus 0.1180 (IQR, 0.0987-0.1414) in those with HVPG of less than 10 mm Hg (P = .006). The correlation between the best model derived from the distribution of the clustering coefficient (10 bins) of vascular connectome and HVPG had a Pearson correlation of 0.977 (root mean squared error, 1.57 mm Hg; P < .0001)., Conclusion: This pilot study demonstrates that graph modeling of vascular connectivity based on video processing of liver DCE US examinations and subsequent graph analysis enable calculation of personalized parameters that reflect the degree of organization of the hepatic microvascular network and are correlated to the severity of portal hypertension in cirrhosis., ((©) RSNA, 2015 Online supplemental material is available for this article.)

Abstract: Retinal tissue is damaged during inflammation in Multiple Sclerosis. We assessed molecular changes in inflamed murine retinal cultures by Raman spectroscopy. Partial Least Squares-Discriminant analysis (PLS-DA) was able to classify retina cultures as inflamed with high accuracy. Using Multivariate Curve Resolution (MCR) analysis, we deconvolved 6 molecular components suffering dynamic changes along inflammatory process. Those include the increase of immune mediators (Lipoxygenase, iNOS and TNFα), changes in molecules involved in energy production (Cytochrome C, phenylalanine and NADH/NAD+) and decrease of Phosphatidylcholine. Abstract: The purpose of the study was to evaluate the association between a quantitative texture analysis of early neonatal brain ultrasound images and later neurobehavior in preterm infants. A prospective cohort study including 120 preterm (<33 wk of gestational age) infants was performed. Cranial ultrasound images taken early after birth were analyzed in six regions of interest using software based on texture analysis. The resulting texture scores were correlated with the Neonatal Behavioural Assessment Scale (NBAS) at term-equivalent age. The ability of texture scores, in combination with clinical data and standard ultrasound findings, to predict the NBAS results was evaluated. Texture scores were significantly associated with all but one NBAS domain and better predicted NBAS results than clinical data and standard ultrasound findings. The best predictive value was obtained by combining texture scores with clinical information and ultrasound standard findings (area under the curve = 0.94). We conclude that texture analysis of neonatal cranial ultrasound-extracted quantitative features that correlate with later neurobehavior has a higher predictive value than the combination of clinical data with abnormalities in conventional cranial ultrasound., (Copyright © 2014 World Federation for Ultrasound in Medicine & Biology. 30. DWI and complex brain network analysis predicts vascular cognitive impairment in spontaneous hypertensive rats undergoing executive function tests.

Abstract: The identification of biomarkers of vascular cognitive impairment is urgent for its early diagnosis. The aim of this study was to detect and monitor changes in brain structure and connectivity, and to correlate them with the decline in executive function. We examined the feasibility of early diagnostic magnetic resonance imaging (MRI) to predict cognitive impairment before onset in an animal model of chronic hypertension: Spontaneously Hypertensive Rats. Cognitive performance was tested in an operant conditioning paradigm that evaluated learning, memory, and behavioral flexibility skills. Behavioral tests were coupled with longitudinal diffusion weighted imaging acquired with 126 diffusion gradient directions and 0.3 mm(3) isometric resolution at 10, 14, 18, 22, 26, and 40 weeks after birth. Diffusion weighted imaging was analyzed in two different ways, by regional characterization of diffusion tensor imaging (DTI) indices, and by assessing changes in structural brain network organization based on Q-Ball tractography. Already at the first evaluated times, DTI scalar maps revealed significant differences in many regions, suggesting loss of integrity in white and gray matter of spontaneously hypertensive rats when compared to normotensive control rats. In addition, graph theory analysis of the structural brain network demonstrated a significant decrease of hierarchical modularity, global and local efficacy, with predictive value as shown by regional three-fold cross validation study. Moreover, these decreases were significantly correlated with the behavioral performance deficits observed at subsequent time points, suggesting that the diffusion weighted imaging and connectivity studies can unravel neuroimaging alterations even overt signs of cognitive impairment become apparent.

Abstract: Objective: The purpose of this study was to assess whether abnormal cardiac function in human fetuses with intrauterine growth restriction (IUGR) is associated with ultrastructural differences in the cardiomyocyte sarcomere., Study Design: Nine severe early-onset IUGR fetuses and 9 normally grown fetuses (appropriate growth for gestational age) who died in the perinatal period were included prospectively. Cardiac function was assessed by echocardiography and levels of B-type natriuretic peptide and troponin-I. Heart sections were imaged by second harmonic generation microscopy, which allowed unstained visualization of cardiomyocyte's sarcomere length., Results: Echocardiographic and biochemical markers showed signs of severe cardiac dysfunction in IUGR fetuses. Second harmonic generation microscopy demonstrated a significantly shorter sarcomere length in IUGR as compared with appropriate growth for gestational age fetuses., Conclusion: IUGR is associated with changes in the cardiomyocyte contractile machinery in the form of shorter sarcomere length, which could help to explain the cardiac dysfunction previously documented in IUGR., (Copyright © 2014 Mosby, Inc. Abstract: Automatic quantification of cardiac muscle properties in tissue sections might provide important information related to different types of diseases. Second harmonic generation (SHG) imaging provides a stain-free microscopy approach to image cardiac fibers that, combined with our methodology of the automated measurement of the ultrastructure of muscle fibers, computes a reliable set of quantitative image features (sarcomere length, A-band length, thick-thin interaction length, and fiber orientation). We evaluated the performance of our methodology in computer-generated muscle fibers modeling some artifacts that are present during the image acquisition. Then, we also evaluated it by comparing it to manual measurements in SHG images from cardiac tissue of fetal and adult rabbits. The results showed a good performance of our methodology at high signal-to-noise ratio of 20 dB. We conclude that our automated measurements enable reliable characterization of cardiac fiber tissues to systematically study cardiac tissue in a wide range of conditions.

Abstract: Objectives: We tested the hypothesis whether a texture analysis (TA) algorithm applied to MRI brain images identified different patterns in small for gestational age (SGA) fetuses as compared with adequate for gestational age (AGA)., Study Design: MRI was performed on 83 SGA and 70 AGA at 37 weeks' GA. Texture features were quantified in the frontal lobe, basal ganglia, mesencephalon, cerebellum and cingulum. A classification algorithm based on discriminative models was used to correlate texture features with clinical diagnosis., Results: Region of interest delineation in all areas was achieved in 61 SGA (12 vasodilated) and 52 AGA; this was the sample for TA feature extraction which allowed classifying SGA from AGA with accuracies ranging from 90.9 to 98.9% in SGA versus AGA comparison and from 93.6 to 100% in vasodilated SGA versus AGA comparison., Conclusions: This study demonstrates that TA can detect brain differences in SGA fetuses. 34. Effect of molecular organization on the image histograms of polarization SHG microscopy.

Abstract: Based on its polarization dependency, second harmonic generation (PSHG) microscopy has been proven capable to structurally characterize molecular architectures in different biological samples. By exploiting this polarization dependency of the SHG signal in every pixel of the image, average quantitative structural information can be retrieved in the form of PSHG image histograms. In the present study we experimentally show how the PSHG image histograms can be affected by the organization of the SHG active molecules. Our experimental scenario grounds on two inherent properties of starch granules. Firstly, we take advantage of the radial organization of amylopectin molecules (the SHG source in starch) to attribute shifts of the image histograms to the existence of tilted off the plane molecules. Secondly, we use the property of starch to organize upon hydration to demonstrate that the degree of structural order at the molecular level affects the width of the PSHG image histograms. The shorter the width is the more organized the molecules in the sample are, resulting in a reliable method to measure order. The implication of this finding is crucial to the interpretation of PSHG images used for example in tissue diagnostics.

Abstract: Intrauterine growth restriction (IUGR) due to placental insufficiency affects 5-10% of all pregnancies and it is associated with a wide range of short- and long-term neurodevelopmental disorders. Prediction of neurodevelopmental outcomes in IUGR is among the clinical challenges of modern fetal medicine and pediatrics. In recent years several studies have used magnetic resonance imaging (MRI) to demonstrate differences in brain structure in IUGR subjects, but the ability to use MRI for individual predictive purposes in IUGR is limited. Recent research suggests that MRI in vivo access to brain connectivity might have the potential to help understanding cognitive and neurodevelopment processes. Specifically, MRI based connectomics is an emerging approach to extract information from MRI data that exhaustively maps inter-regional connectivity within the brain to build a graph model of its neural circuitry known as brain network. In the present study we used diffusion MRI based connectomics to obtain structural brain networks of a prospective cohort of one year old infants (32 controls and 24 IUGR) and analyze the existence of quantifiable brain reorganization of white matter circuitry in IUGR group by means of global and regional graph theory features of brain networks. Based on global and regional analyses of the brain network topology we demonstrated brain reorganization in IUGR infants at one year of age. Specifically, IUGR infants presented decreased global and local weighted efficiency, and a pattern of altered regional graph theory features. By means of binomial logistic regression, we also demonstrated that connectivity measures were associated with abnormal performance in later neurodevelopmental outcome as measured by Bayley Scale for Infant and Toddler Development, Third edition (BSID-III) at two years of age. Abstract: Background: Intrauterine growth restriction (IUGR) affects 5-10% of all newborns and is associated with a high risk of abnormal neurodevelopment. The timing and patterns of brain reorganization underlying IUGR are poorly documented. We developed a rabbit model of IUGR allowing neonatal neurobehavioral assessment and high resolution brain diffusion magnetic resonance imaging (MRI). The aim of the study was to describe the pattern and functional correlates of fetal brain reorganization induced by IUGR., Methodology/principal Findings: IUGR was induced in 10 New Zealand fetal rabbits by ligation of 40-50% of uteroplacental vessels in one horn at 25 days of gestation. Ten contralateral horn fetuses were used as controls. Cesarean section was performed at 30 days (term 31 days). At postnatal day +1, neonates were assessed by validated neurobehavioral tests including evaluation of tone, spontaneous locomotion, reflex motor activity, motor responses to olfactory stimuli, and coordination of suck and swallow. Subsequently, brains were collected and fixed and MRI was performed using a high resolution acquisition scheme. Global and regional (manual delineation and voxel based analysis) diffusion tensor imaging parameters were analyzed. IUGR was associated with significantly poorer neurobehavioral performance in most domains. Voxel based analysis revealed fractional anisotropy (FA) differences in multiple brain regions of gray and white matter, including frontal, insular, occipital and temporal cortex, hippocampus, putamen, thalamus, claustrum, medial septal nucleus, anterior commissure, internal capsule, fimbria of hippocampus, medial lemniscus and olfactory tract. Regional FA changes were correlated with poorer outcome in neurobehavioral tests., Conclusions: IUGR is associated with a complex pattern of brain reorganization already at birth, which may open opportunities for early intervention. Diffusion MRI can offer suitable imaging biomarkers to characterize and monitor brain reorganization due to fetal diseases.

To evaluate texture features, 957 left and right lung images in a 2D four-cardiac-chamber view plane were previously delineated from fetuses between 20 and 41 weeks of gestation. Quantification of lung texture was performed by the Automatic Quantitative Ultrasound Analysis (AQUA) software to extract image features. A standard learning approach composed of feature transformation and a regression model was used to evaluate the association between texture features and gestational age., Results: The association between weeks of gestation and fetal lung texture quantified by the AQUA software presented a Pearson correlation of 0.97. The association was not influenced by delineation parameters such as region of interest (ROI) localization, ROI size, right/left lung selected or sonographic parameters such as ultrasound equipment or transducer used., Conclusions: Fetal lung texture analysis measured by the AQUA software demonstrated a strong correlation with gestational age. Abstract: Objectives: Diagnosis of white matter damage by cranial ultrasound imaging is still subject to interobserver variability and has limited sensitivity for predicting abnormal neurodevelopment later in life. In this study, we evaluated the ability of a semiautomated method based on ultrasound texture analysis to identify patterns that correlate with the ultrasound diagnosis of white matter damage., Methods: The study included 44 very preterm neonates born at a median gestational age of 29 weeks 3 days (range, 26 weeks-31 weeks 6 days). Patients underwent cranial ultrasound scans within 1 week of birth and between 14 and 31 days of life. Periventricular leukomalacia was diagnosed by experienced clinicians on the 14- to 31-day scan according to standard criteria. To perform the texture analysis, 4 regions of interest were delineated in stored images: left and right periventricular areas and choroid plexuses. A classification algorithm was developed on the basis of the best combination of texture coefficients to correlate with the clinical diagnosis, and the ability of this algorithm to predict a later diagnosis of periventricular leukomalacia on the first scan was evaluated using a leave-one-out cross-validation., Results: Periventricular leukomalacia was diagnosed by the standard procedure in 14 of 44 neonates. The texture classification algorithm performed on the first scan could identify cases with a later diagnosis of periventricular leukomalacia with sensitivity of 100% and accuracy of 97.7%., Conclusions: These data support the notion that semiautomated quantitative ultrasound analysis achieves early identification of changes in subclinical stages and warrant further investigation of the role of ultrasound texture analysis methods to improve early detection of neonatal brain damage.

Abstract: In isolated congenital diaphragmatic hernia, prenatal prediction is made based on measurements of lung size and the presence of liver herniation into the thorax. A subset of fetuses likely to die in the postnatal period is eligible for fetal intervention that can promote lung growth. Rather than anatomical repair, this is now attempted by temporary fetal endoscopic tracheal occlusion (FETO). Herein we describe purpose-designed instruments that were developed thanks to a grant from the European Commission. The feasibility and safety of FETO have now been demonstrated in several active fetal surgery programs. The most frequent complication of the procedure is preterm premature rupture of the membranes, which is probably iatrogenic in nature. It does have an impact on gestational age at delivery and complicates balloon removal. FETO is associated with an apparent increase in survival compared with same severity controls, although this needs to be evaluated in a formal trial. The time has come to do so., (Copyright Â© 2011. Abstract: Pixel resolution polarization-sensitive second harmonic generation (PSHG) imaging has been recently shown as a promising imaging modality, by largely enhancing the capabilities of conventional intensity-based SHG microscopy. PSHG is able to obtain structural information from the elementary SHG active structures, which play an important role in many biological processes. Although the technique is of major interest, acquiring such information requires long offline processing, even with current computers. In this paper, we present an approach based on Fourier analysis of the anisotropy signature that allows processing the PSHG images in less than a second in standard single core computers. This represents a temporal improvement of several orders of magnitude compared to conventional fitting algorithms. This opens up the possibility for fast PSHG information with the subsequent benefit of potential use in medical applications.

The metabolic spectrum of N-acetyl-aspartate/choline, choline/creatine, inositol/choline and creatine/choline ratios in the left frontal lobe and the apparent diffusion coefficient from the right and left basal ganglia and frontal and occipital lobes, pyramidal tract and corpus callosum were analyzed and compared., Results: As compared with controls, SGA fetuses showed a significant increase in inositol/choline ratio (SGA, 0.57 vs. AGA, 0.25; P = 0.04) and significantly higher ADC values in the pyramidal tract (SGA, 119.87 x 10(-5) mm(2)/s vs. AGA, 105.11 x 10(-5) mm(2)/s; P = 0.04)., Conclusions: SGA fetuses with normal umbilical artery Doppler present microstructural and metabolic brain changes, suggesting the existence of an abnormal in-utero brain development in fetuses with this condition.

Abstract: In this work we show that a pulsed laser light placed at a distance is able to modulate the growth of axons of primary neuronal cell cultures. In our experiments continuous wave (CW), chopped CW and modelocked fs (FS) laser light was focused through a microscope objective to a point placed at a distance of about 15 microm from the growth cone. We found that CW light does not produce any significant influence on the axon growth. In contrast, when using pulsed light (chopped CW light or FS pulses), the beam was able to modify the trajectory of the axons, attracting approximately 45% of the observed cases to the beam spot. Such effect could possibly indicate the capacity of neurons to interpret the pulsating NIR light as the source of other nearby cells, resulting in extension of processes in the direction of the source., (Copyright 2009 Elsevier B.V. Abstract: In this paper we provide, for the first time to our knowledge, the effective orientation of the SHG source in cultured cortical neuronal processes in vitro. This is done by the use of the polarization sensitive second harmonic generation (PSHG) imaging microscopy technique. By performing a pixel-level resolution analysis we found that the SHG dipole source has a distribution of angles centered at thetae =33.96 degrees , with a bandwidth of Deltathetae = 12.85 degrees . This orientation can be related with the molecular geometry of the tubulin heterodimmer contained in microtubules.

Abstract: In this study, the second harmonic generation (SHG) response to polarization and subsequent data analysis is used to discriminate, in the same image, different SHG source architectures with pixel resolution. This is demonstrated in a mammalian tissue containing both skeletal muscle and fibrilar collagen. The SHG intensity variation with the input polarization (PSHG) is fitted pixel by pixel in the image using an algorithm based on a generalized biophysical model. The analysis provides the effective orientation, theta(e), of the different SHG active structures (harmonophores) at every pixel. This results in a new image in which collagen and muscle are clearly differentiated. In order to quantify the SHG response, the distribution of theta(e) for every harmonophore is obtained. We found that for collagen, the distribution was centered at theta(e) = 42.7 degrees with a full width at half maximum of theta = 5.9 degrees while for muscle theta(e) = 65.3 degrees , with theta = 7.7 degrees . By comparing these distributions, a quantitative measurement of the discrimination procedure is provided.

Abstract: The polarization dependence of second harmonic generation (SHG) microscopy is used to uncover structural information in different muscle cells in a living Caenorhabditis elegans (C. elegans) nematode. This is done by using a generalized biophysical model in which element ratios for the associated second-order nonlinear tensor and angular orientations for thick filaments are retrieved using a pixel-by-pixel fitting algorithm. As a result, multiple arbitrary orientations of thick filaments, at the pixel-resolution level, are revealed in the same image. The validity of our method is first corroborated in well-organized thick filaments such as the nonfibrilar body wall muscles. Next, a region of the nonstriated muscular cells of the pharynx is analyzed by showing different regions with homogenous orientations of thick filament as well as their radial distribution. As a result, different sets of the nonstriated muscle cell groups in the pharynx of this nematode were exposed. This methodology is presented as a filtering mechanism to uncover biological information unreachable by common intensity SHG microscopy. 46. Influence of distant femtosecond laser pulses on growth cone fillopodia.

Abstract: A 3 mW focused femtosecond laser spot at a distance (>15 mum) has shown to attract the fillopodia from growth cones of primary neuronal cell cultures (mice E15). The phenomenological behavior of fillopodia is studied under short durations (~40 min) and different laser light conditions. The analysis of the fillopodia movement showed that they become significantly attracted towards the focused femtosecond laser light. In contrast, the use of continuous wave under the same conditions did not generate the same effect, the results of which were indistinguishable from when there was no laser light present (control condition). These results suggest the possible existence of an optically-induced signaling mechanism in growth cones.

47. Simultaneous analytical characterisation of two ultrashort laser pulses using spectrally resolved interferometric correlations.

Abstract: In this paper we discuss in detail the underlying theory of a novel method that allows the characterizing of ultrashort laser pulses to be achieved in an analytical way. MEFISTO, (measuring the electric field by interferometric spectral trace observation) is based on a Fourier analysis of the information contained in a spectrally resolved interferometric correlation and can be applied to both situations: the characterization of an unknown pulse (MEFISTO) or to the simultaneous characterization of two different unknowns pulses (Blind-MEFISTO). The theoretical development and experimental practical implications are discussed in both situations.
