The formation of the basis for personalized medicine of the future should be based on the prognostic and preventive principles of medicine. The article presents an overview of the information on the method and application of the CME (complex medical expert) analysis. The features of the method, the object domain of its application, its task and methodology are described. The scheme of the options used to increase the efficiency of the method is of interest. The strengths and the shortcomings of the CME analysis noted by experts in the field of its application are given. Results. The authors propose an innovative technology for using the CME model to develop personalized forecasting, preservation, development, and management of health. The advantages of the technology and the urgency of its application are revealed. The new technology is based on the provisions of the development of the theory of the essential adaptation. To ensure the health state, the level of functioning of body systems must be consistent with the optimality, namely with the response norms, which determine the individual phenotype. In the process of the organism adaptation to the conditions of the external environment, the central nervous system and the significant body system (the system with the maximum response norm), which determine the specific and nonspecific responses of the organism, respectively, will be especially active (it is a known fact). Discussion. Dominant systems can actively involve other body systems in the adaptation process by the principle of interaction with them, increasing or decreasing their activity. Taking into account the direction of interaction (inhibition, toning), this fact becomes important for understanding the pathogenesis of diseases, their targeted prevention, and treatment. A comprehensive record of the mechanisms of the formation of a specific and nonspecific reaction is used with the aim of identifying and effectively preventing functional disorders in the body. The CME technology of personalized forecasting, preservation, development and health management eliminates the main drawbacks of CME analysis. On the other hand, the CME analysis technology helps to expand the range of its application, depending on the opportunities and threats of the environment. Conclusion. The presented approach will make it possible to optimally use the human potential to maintain a high quality of life and active longevity.

Recommendations for health resort treatment are given according to the conditions of patient’s health, the extent of illness and transportation problems. A smile is an important part of the attractiveness and beauty of a modern person [1]. Today, more than ever, it’s important to be a well-groomed and healthy person, in whom “...everything should be fine...” as the classic said. The vast majority of patients tend to lead a healthy lifestyle, which includes dental health as well [3]. Health of periodontal tissues directly depends not only on the proper oral hygiene carried out by patients at home, but also on an individually selected set of therapeutic measures performed by the dentist [4, 5]. Thus, dentists and related specialists should provide highly professional medical care in the treatment of inflammatory diseases of periodontal tissues to achieve maximum results, stop the inflammatory process and restore the periodontium as a whole [6, 7].

Aims: to assess the quality of drinking water using microbiological indicators and to assess epidemic risks of water-born infections in urban settings of the Arkhangelsk region. Methods: Secondary data on standard sanitary and hygienic assessments of the quality of centralized drinking water supply in Arkhangelsk, Severodvinsk, Novodvinsk, Kotlas and Koryazhma for 2006-2016 were used. Assessment of the microbial risk of water-born infections was performed. Results: All towns except Severodvinsk had moderate risk of water-born infections through centralized drinking water supply system. Arkhangelsk had 56 points while Kotlas, Novodvinsk and Koryazhma had 37, 30 and 19 points, respectively. Severodvinsk had low risk of water-born infections (5 points). Proportions of samples with abnormal concentrations of general and thermotolerant coliform bacteria in Arkhangelsk, Kotlas, Novodvinsk after water treatment varied between 16 % and 27 % while in Korazhma the corresponding proportions varied between 21 % and 53 % Irregular water supply have contributed with 18 % and 14 % in Arkhangelsk and Kotlas, respectively. Conclusions: The presence of general and thermotolerant coliform bacteria in drinking water detected immediately after water treatment and in water supply network are the main contributors to the epidemic risk of water-born infections in towns of the Arkhangelsk region. Irregularity of water supply was additional factor linked to microbial contamination in Arkhangelsk and Kotlas.

In perimenopausal women, the frequency of thyroid diseases and metabolic syndrome (MS) increases. In this work, the effect of non-drug correction programs for menopausal disorders on the structural and functional state of the thyroid gland in patients with MS was evaluated. For that, five groups of women (330 people total) aged 45–50 years with climacteric syndrome (CS) against the background of MS were examined. For the index group and the experimental groups the following was used: basic treatment, exercise therapy, drinking balneotherapy, additional oral intake of multivitamins and minerals, as well as physical therapy — vibrotherapy, chromotherapy, music therapy, aromatherapy, aeroionotherapy in various combinations. In the control group, only basic treatment was used. The levels of thyroid-stimulating hormone (TSH) and free thyroxine (free T4) in serum were determined, a sonographic study of the thyroid gland was performed. According to the study results, the use of correction programs using physical therapy methods improved the functioning parameters of the pituitary-thyroid system in the surveyed patients. In women with moderate-severity CS, the best results were obtained while simultaneously using all the mentioned above physiotherapy methods: during the transition period the TSH decreased by 6.5%, during the postmenopausal period — by 5.6%, the level of free T4 increased by 6.5% and 6,6% respectively (р < 0.05, applied to both parameters). It can be concluded that non-drug programs, including physical therapy, have a protective effect on the thyroid gland function. In case of moderate-severity climacteric syndrome, the program with simultaneous use of vibrotherapy, chromotherapy, music therapy, aromatherapy, aeroionotherapy has priority.

The inability to use hormone replacement therapy in women requires the search for effective non-pharmacological methods for the correction of menopausal disorders. Purpose of work. To assess the effect of non-drug programs for the correction of age-related disorders in women with metabolic syndrome (MS) on the level of sex hormones. Of the 330 women 45-50 years old with menopausal syndrome (CS) and MS, 5 groups were formed. Applied: basic treatment, physiotherapy, balneotherapy, multivitamins and minerals, physiotherapy (vibration therapy, chromotherapy, melotherapy, aromatherapy, aeroionotherapy) in various combinations. The levels of follicle-stimulating (FSH) and luteinizing (LH) hormones, estradiol were determined. In patients with mild CS, when using physiotherapy, after 3 months of treatment, FSH, LH decreased, estradiol increased. In patients with moderate KS, the best results were achieved using a full set of physiotherapeutic factors: after 6 months, FSH decreased by 19,3% (p0,0001) and LH by 23,9% (p0,0001), estradiol increased by 22,3% (p=0,0084). Comprehensive non-drug correction has a positive effect on the functioning of the hypothalamic-pituitary system in patients with CS and MS. In moderate-grade CS, a program with the use of vibration therapy, chromotherapy, melotherapy, aromatherapy and aeroionotherapy is a priority.Невозможность применения гормональной терапии у женщин требует поиска эффективных немедикаментозных методов коррекции климактерических нарушений. Цель работы — оценка влияния нелекарственных программ коррекции возрастных нарушений у женщин с метаболическим синдромом (МС) на уровень половых гормонов. Из 330 женщин 45–50 лет с климактерическим синдромом (КС) и МС сформировано пять групп. Применяли базовое лечение, лечебную физкультуру, бальнеотерапию, поливитамины и минералы, физиотерапию (вибротерапию, хромотерапию, мелотерапию, ароматерапию, аэроионотерапию) в различных сочетаниях. Определяли уровень фолликулостимулирующего (ФСГ) и лютеинизирующего (ЛГ) гормонов, эстрадиола. У пациенток с КС легкой степени при использовании физиотерапии уже через 3 мес лечения снизился уровень ФСГ, ЛГ, увеличилось содержание эстрадиола. У пациенток с КС средней степени лучшие результаты были достигнуты при применении полного набора физиотерапевтических факторов: через 6 мес снизился уровень ФСГ на 19,3% (р0,0001) и ЛГ на 23,9% (р0,0001), увеличилось содержание эстрадиола на 22,3% (р=0,0084). Комплексная нелекарственная коррекция оказывает положительное влияние на функционирование гипоталамо-гипофизарной системы у пациенток с КС и МС. При КС средней степени приоритетна программа с примением вибро-, хромо-, мело-, арома- и аэроионотерапии.

In the article, an overview of the information on the method and application technology of SWOT (Strengths, Weaknesses Opportunities, Threats) analysis is presented. The specific characters of the method, the object domain of its application, the task, the methodology are disclosed. An interesting idea is the scheme of the options used to achieve more complete benefit from the method. The strengths and the shortcomings of the SWOT analysis noted by experts in the field of its application are given. Further on, the authors propose an innovative technology for using the SWOT model to develop personalized forecasting, preservation, development, and management of health, based on the provisions of the functional direction of genetics - phenomics. Arbitrary and involuntary movements were studied from the position of the new chaos-self-organization theory. Analysis of organization of tappingrams and tremorograms within the framework of N. A. Bernstein "repetition without repetitions" hypothesis leads to the Eskov-Zinchenko effect. In this case, there is no statistical stability for samples of parameters of electromyograms of any movements obtained in a row, i.e. fj (xi)≠fj+1(xi) with the probability p≥0.97. In this paper, the basic problem of motion physiology, stochastic instability establishment mechanisms, is solved in the analysis of muscle electromyogram parameters under conditions of permanent static force. Statistical instability of electromyograms within the framework of the Eskov-Zinchenko effect is proven.

Aim of the study. To assess the effectiveness of the physical rehabilitation in the first year in recipients with donor kidney. Material and Methods. The article presents an assessment of the functional state and adaptive body reserves using hardware-software complex (HSC) “Sources of Health” for patients before and after the kidney transplantation. The results of the examination and treatment of 57 recipients (of the mean age - 35±9.65) donor kidney at different times of the postoperative period. Depending on the physical rehabilitation program there were selected 2 groups of patients: in Group I the physical rehabilitation was carried out during the year, in Group II - only in the first week after surgery for the prevention of early postoperative complications. Group III combined 30 relatively healthy people (15 women, 15 men) without the need for an organ transplant and of a mean age of 33.7 ± 8.7 years, keeping a normal lifestyle, not engaged in the regular recreational physical exercises. The functional state and adaptive body reserves were evaluated with HSC “Sources of Health” at 1, 3, 6 and 12 months after surgery. Results. The physical rehabilitation with a personalized approach for one year after the kidney transplantation can significantly improve functional and adaptive body reserves in donor kidney recipients. Conclusion. Patients are shown to need for the physical rehabilitation at least for a year after the kidney transplantation in order to improve the functional state and adaptive body reserves.

Ovarian failure is accompanied by a decrease in sexual function, which plays quite an important rolefor modern women. Purpose: to evaluate the impact of complex non-drug programs for correcting climacteric disorders on the sexual function of women with metabolic syndrome. We examined 330 women aged 45-50 with a climacteric syndrome of mild and moderate severity against the background of the metabolic syndrome. Patients were formed into 5 groups. All patients received standard treatment.Women of the main group and comparison groups were treated with exercise therapy, drinking balneotherapy, additional oral intake of multivitamins and minerals, and preformed physical factors in various combinations: vibrotherapy, full-spectrum and selective chromotherapy, melody therapy, aromatherapy, aeroionotherapy. In the control group, only standard treatment was used. A dynamic evaluation of the Sexual Function Index wasperformed. With the simultaneous use of vibration therapy, chromotherapy, melody therapy, aeroionotherapy, aromatherapy, the index of female sexual function increases as much as 26% in patients with menopausal syndrome of mild degree and 20% in patients with moderate menopausal syndrome. Thus, the complex non-drug correction of menopausal disorders with simultaneous application of all specified physiotherapeutic can improve the quality of life of a woman with a metabolic syndrome due to a positive effect on its sexual component.Угасание работы яичников сопровождается снижением сексуальной функции, которая для современной женщины имеет немаловажное значение. Цель исследования — оценка влияния комплексных нелекарственных программ коррекции климактерических расстройств на сексуальную функцию женщин с метаболическим синдромом. Обследованы 330 женщин 45–50 лет с климактерическим синдромом легкой и средней степени тяжести на фоне метаболического синдрома, которые были разделены на пять групп. Стандартное лечение получали все пациентки. Женщины основной группы и групп сравнения применяли лечебную физкультуру, питьевую бальнеотерапию, дополнительный пероральный прием поливитаминов и минералов, а также преформированные физические факторы в различных сочетаниях — вибротерапию, полноспектровую и селективную хромотерапию, мело-, арома- и аэроионотерапию. В контрольной группе использовали только стандартное лечение. Оценивали индекс женской сексуальной функции. При одновременном использовании вибро-, хромо-, мело-, аэроионо-, ароматерапии индекс увеличивался максимально: на 26% — у пациенток с климактерическим синдромом легкой степени, на 20% — у пациенток с климактерическим синдромом средней степени. Таким образом, комплексная нелекарственная коррекция климактерических расстройств с физиотерапией улучшает качество жизни женщины с метаболическим синдромом за счет положительного влияния на ее сексуальную составляющую.

Background:Menopause cause many symptoms, which may impair quality of life the women. The study aims to determine the effectiveness of mixed herbal medicinal extracts on menopausal symptoms.Method:A randomized, triple-blind, placebo-controlled clinical trial was conducted on 120 peri-menopausal women for 12 weeks. All participants took herbal extracts drops orally daily. They were randomly allocated into four groups: placebo (C), A (250 mg chamomile, 30 mg fennel, 15 mg saffron), B (1000 mg chamomile, 120 mg fennel, 60 mg saffron), and D (500 mg chamomile, 60 mg fennel, 30 mg saffron).They completed a demographic and MRS questionnaire at week 0, 6.12.Result:108 participants successfully completed the assessment. No significant differences in demographic variables were seen between the four groups. The median MRS score was significantly reduced in group B from 27.5±3 to 7.5±5. In group D, it was significantly reduced from 29±12 to 22±9.25The effect size in this study was 0.92.Conclusion:Study showed significant improvement in menopausal symptoms especially in group B.

The problem of ensuring long-term well-being and high quality of life for older women is becoming the most important goal of modern medicine. The initial presence of metabolic syndrome complicates the course of menopause; and menopausal hormone therapy cannot always be used. The urgency for finding the effective ways of non-drug correction of menopausal disorders in this category of patients is obvious.To comparatively evaluate the effectiveness of complex non-drug programs in the correction of psychoemotional disorders of menopause in patients with metabolic syndrome.A total of 330 women aged 45-50 years with menopausal syndrome in the presence of metabolic syndrome were examined. The examinees were randomized into 5 groups. All the patients received conventional treatment. A study and comparison groups used exercise therapy, drank mineral water, and additionally took multivitamins and minerals. The physiotherapeutic component of therapeutic complexes was represented by various combinations of factors (vibrotherapy, chromotherapy, melotherapy, aromatherapy, and aeroionotherapy). The control group received conventional treatment only. The degree of severity of psychoemotional disorders was estimated before and 3 and 6 months after treatment via the modified Kupperman menopausal index (MMI), the Spielberger-Khanin Trait Anxiety Inventory, and the Hamilton Depression Rating Scale.Psychoemotional symptoms were most significantly reduced (1.5-fold) after 6 months of treatment in patients with mild disorders when the complexes, including vibrotherapy, were applied. In patients with moderate disorders, the simultaneous use of vibrotherapy, chromotherapy, melotherapy, aromatherapy, and aeroionotherapy promoted the maximum regression of psychoemotional symptoms (by 47.3%) (p0.05). The application of the conventional approach was observed to aggravate psychoemotional disorders.Complex non-drug programs, including physiotherapy, effectively relieve perimenopausal psychoemotional disorders in patients with metabolic syndrome. Vibrotherapy substantially improves the results of treatment. Moderate psychoemotional disorders need the complex program to be applied simultaneously with vibrotherapy, chromotherapy, melotherapy, aromatherapy, and aeroionotherapy.Обоснование. Проблема обеспечения долговременного благополучия и высокого качества жизни женщин старшего возраста становится важнейшей целью современной медицины. Исходное наличие метаболического синдрома осложняет течение климактерия, а использование менопаузальной гормональной терапии не всегда возможно. Очевидна актуальность поиска эффективных способов немедикаментозной коррекции климактерических расстройств у данной категории пациенток. Цель исследования - провести сравнительную оценку эффективности комплексных нелекарственных программ в коррекции психоэмоциональных климактерических расстройств у пациенток с метаболическим синдромом. Материал и методы. Обследованы 330 женщин в возрасте 45-50 лет с климактерическим синдромом на фоне метаболического синдрома. Исследуемые были рандомизированы на 5 групп. У всех пациенток применялось стандартное лечение. У основной группы и групп сравнения применялись лечебная физкультура, прием внутрь минеральной воды, дополнительный пероральный прием поливитаминов и минералов. Физиотерапевтический компонент лечебных комплексов был представлен различными комбинациями факторов (вибротерапия, хромотерапия, мелотерапия, ароматерапия, аэроионотерапия). В группе контроля применялось только стандартное лечение. Оценку степени выраженности психоэмоциональных нарушений проводили до терапии, через 3 и 6 мес лечения по модифицированному менопаузальному индексу Купермана, показателю 'тревожность' по тесту Спилбергера-Ханина, тяжести депрессивных расстройств по шкале Гамильтона. Результаты. Психоэмоциональная симптоматика наиболее значимо снизилась (в 1,5 раза) через 6 мес лечения у пациенток с расстройствами легкой степени при применении комплексов, включающих вибротерапию. У пациенток с расстройствами средней степени тяжести максимальной регрессии психоэмоциональных симптомов - на 47,3% (p0,05) - способствовало одновременное применение вибротерапии, хромотерапии, мелотерапии, ароматерапии и аэроионотерапии. При стандартном подходе наблюдалось усугубление психоэмоциональных расстройств. Заключение. Комплексные нелекарственные программы, включающие физиотерапию, эффективно купируют психоэмоциональные расстройства в перименопаузе у пациенток с метаболическим синдромом. Использование вибротерапии значимо улучшает результаты лечения. При психоэмоциональных расстройствах средней степени тяжести преимущество имеет комплексная программа с одновременным применением вибротерапии, хромотерапии, мелотерапии, ароматерапии и аэроионотерапии.

The present paper concerns the investigation of surface layers in manganese steels subjected to an electron-beam treatment.
