Abstract

Experts and international public health organisations stress the lack of surveillance systems for companion animal diseases and the need to implement such surveillance as a priority of the 'One Health' perspective. This paper presents the features of a system for the collection, analysis, interpretation and dissemination of data regarding the health status of pets in the Veneto region (Italy). The system involved the construction of a Web-based database containing the diagnoses of transmissible and non-transmissible diseases of dogs and cats made by veterinarians in their practices, hospitals, kennels and catteries. Each diagnosis constitutes a single record, also containing data on the identification of the individual animal and on several characteristics of epidemiological relevance. The World Health Organization (WHO) 10th revision of the International Classification of Diseases (ICD-10) for human diseases has been adapted to canine and feline diseases to standardise the diagnostic nomenclature. Software has been specifically created for online data entry and data management. The first results show that the main disorders were digestive (21%), dermatological (18%) and cardiovascular (11%) among 1,087 diagnostic records in dogs, and digestive (23%), dermatological (15%) and urinary (14%) among 289 diagnostic records in cats. The main causes of death are represented by cardiovascular (21%) and gastrointestinal (21%) diseases in dogs and by urinary (31%) disorders in cats. At present, no institutional surveillance system for companion animal health exists in Italy, and veterinarians joining this project and sharing the outcomes of their clinical activity are acting on a voluntary basis.

Abstract

Conexões de ferro fundido maleável preto foram galvanizadas em dezesseis diferentes banhos de zinco, cada qual composto por concentrações e combinações variadas de bismuto (0,06 e 0,1%), níquel (0,003 e 0,05%), estanho (0,3 e 0,5%) e alumínio (0,01 e 0,14%). Os revestimentos obtidos foram estudados através de medidas de espessura de camada, análise metalográfica utilizando microscopia ótica e microanálise utilizando espectrometria de energia dispersiva de raios-x (EDS). Obtiveram-se algumas variações na espessura e na morfologia do revestimento com as diferentes ligas, onde cada elemento químico interferiu de uma forma, contribuindo ou não para a formação de um revestimento menos espesso. O bismuto influenciou na fluidez do banho de zinco, o estanho na formação dos compostos intermetálicos e o alumínio através da formação do composto Fe2Al5 inibiu a reação do zinco com o ferro. Entretanto, o níquel não interferiu positivamente na reatividade, não participando então da redução do revestimento galvanizado. Os elementos adicionados ao banho de galvanização apresentaram-se efetivos na diminuição da espessura do revestimento proporcionando assim uma economia de zinco, sendo ainda uma boa alternativa para substituir o chumbo, um elemento químico considerado nocivo ao meio ambiente. Black malleable cast iron fittings were galvanized at sixteen different zinc baths, each bath was made with different concentrations and combinations of bismuth (0,06 - 0,10%), nickel (0,003 - 0,05%), tin (0,3 - 0,5%) and aluminium (0,01 - 0,14%). The coatings obtained were studied by thickness measurements, metallographic observation and scanning electron microscopy associated with energy-dispersive spectroscopy microanalysis. Some changes have been observed in the morphology of the galvanized coating with the different alloys, where each element played different role in reduction or not the zinc coating thickness. Bismuth improved bath fluidity, tin influenced on intermetallic compounds formation and aluminium, by the Fe2Al5 formation, inhibited the reaction between zinc and iron. On the other hand, nickel didn't alter the reactivity and didn't participate of the coating galvanized reduction. The elements added to the galvanizing bath were effective on coating thickness reduction providing a zinc saving and can be considered a very good alternative to substitute lead, a chemical element considered detrimental to the environment.

Abstract

Objective: We evaluated the efficacy of muscle-targeted nutritional support on the functional outcomes of multidisciplinary intensive rehabilitation treatment (MIRT) in patients with Parkinson disease (PD) or parkinsonism., Methods: We conducted a pragmatic, bicentric, randomized (1:1), assessor-blind controlled trial (Protein, Leucine and Vitamin D Enhancing Rehabilitation [PRO-LEADER]; April 2017 to January 2018) in cognitively intact patients with PD or parkinsonism and undergoing a 30-day MIRT. Patients (n = 150) received a standard hospital diet with or without a whey protein-based nutritional supplement enriched with leucine and vitamin D twice daily. The primary efficacy endpoint was the increase in the distance walked during a 6-minute walking test (6MWT). Secondary endpoints were changes in 4-meter walking speed, Timed Up and Go test (TUG), Berg balance scale, handgrip strength, Self-assessment Parkinson's Disease Disability Scale, body weight, and skeletal muscle mass (SMM)., Results: Nutritional support resulted in greater increase in the distance walked during 6MWT (mean 69.6 meters [95% confidence interval (CI) 60.7-78.6]) than no support (51.8 meters [95% CI 37.0-66.7]): center-adjusted mean difference, 18.1 meters (95% CI 0.9-35.3) ( p = 0.039). Further adjustment for changes in dopaminergic therapy and SMM yielded consistent results: mean difference, 18.0 meters (95% CI 0.7-35.2) ( p = 0.043). A meaningful effect was also found for the following secondary endpoints: 4-meter walking speed ( p = 0.032), TUG ( p = 0.046), SMM, and SMM index ( p = 0.029). Six patients discontinued the nutritional therapy due to mild side effects., Conclusion: The consumption of a whey protein-based nutritional formula enriched with leucine and vitamin D with MIRT improved lower extremity function and preserved muscle mass in patients with PD or parkinsonism.Clinicaltrials.gov IDENTIFIER: NCT03124277., Classification of Evidence: This study provides Class I evidence that for patients with parkinsonism undergoing intensive rehabilitation, a whey protein-based nutritional formula enriched with leucine and vitamin D increased distance walked on the 6MWT., (© 2019 American Academy of Neurology.)
