Contains fulltext : 249977.pdf (Publisher’s version ) (Closed access) BACKGROUND: It is important to evaluate whether a new treatment for heart failure with reduced ejection fraction (HFrEF) provides additive benefit to background foundational treatments. As such, we aimed to evaluate the efficacy and safety of empagliflozin in patients with HFrEF in addition to baseline treatment with specific doses and combinations of disease-modifying therapies. METHODS: We performed a post-hoc analysis of the EMPEROR-Reduced randomised, double-blind, parallel-group trial, which took place in 520 centres (hospitals and medical clinics) in 20 countries in Asia, Australia, Europe, North America, and South America. Patients with New York Heart Association (NYHA) classification II-IV with an ejection fraction of 40% or less were randomly assigned (1:1) to receive the addition of either oral empagliflozin 10 mg per day or placebo to background therapy. The primary composite outcome was cardiovascular death and heart failure hospitalisation; the secondary outcome was total heart failure hospital admissions. An extended composite outcome consisted of inpatient and outpatient HFrEF events was also evaluated. Outcomes were analysed according to background use of angiotensin-converting enzyme (ACE) inhibitors, angiotensin II receptor blockers (ARBs) or angiotensin receptor neprilysin inhibitors (ARNIs), as well as β blockers and mineralocorticoid receptor antagonists (MRAs) at less than 50% or 50% or more of target doses and in various combinations. This study is registered with ClinicalTrials.gov, NCT03057977. FINDINGS: In this post-hoc analysis of 3730 patients (mean age 66·8 years [SD 11·0], 893 [23·9%] women; 1863 [49·9%] in the empagliflozin group, 1867 [50·1%] in the placebo group) assessed between March 6, 2017, and May 28, 2020, empagliflozin reduced the risk of the primary outcome (361 in 1863 participants in the empagliflozin group and 462 of 1867 in the placebo group; HR 0·75 [95% CI 0·65-0·86]) regardless of background therapy or its target doses for ACE inhibitors or ARBs at doses of less than 50% of the target dose (HR 0·85 [0·69-1·06]) and for doses of 50% or more of the target dose (HR 0·67 [0·52-0·88]; p(interaction)=0·18). A similar result was seen for β blockers at doses of less than 50% of the target dose (HR 0·66 [0·54-0·80]) and for doses of 50% or more of the target dose (HR 0·81 [0·66-1·00]; p(interaction)=0·15). Empagliflozin also reduced the risk of the primary outcome irrespective of background use of triple therapy with an ACE inhibitor, ARB, or ARNI plus β blocker plus MRA (given combination HR 0·73 [0·61-0·88]; not given combination HR 0·76 [0·62-0·94]; p(interaction)=0·77). Similar patterns of benefit were observed for the secondary and extended composite outcomes. Empagliflozin was well tolerated and rates of hypotension, symptomatic hypotension, and hyperkalaemia were similar across all subgroups. INTERPRETATION: Empagliflozin reduced serious heart failure outcomes across doses and combinations of disease-modifying therapies for HFrEF. Clinically, these data suggest that empagliflozin might be considered as a foundational therapy in patients with HFrEF regardless of their existing background therapy. Recent results for the cross-correlation signal of a newly proposed balanced correlation receiver at 1.5 μm pointed towards a possible bypassing of the standard quantum limit for the receiver noise-temperature hν⁄k in cross-correlation by a factor of 4-6. The only radiation source strong enough for a clear hot-cold measurement was a heavily attenuated fiber-coupled superluminant LED (SLED), because a multi-mode fiber-coupled thermal halogen lamp was difficult to control in polarization due to its weakness when coupled to a single-mode fiber. This peculiarity left some doubts regarding a possible “strange” quantum-mechanical behavior of the signal light from the SLED. Here we want to present the concept for more convincing measurements using a true thermal signal source.

The current work presents a fiber coupling tip-tilt controller developed for a three-telescope experimental prototype of an Astronomical Fiber-Based Near-Infrared Heterodyne Interferometer. It is based on a commercial magneto-mechanical compact-disk laser-beam actuator on which the fiber-ferrule is mounted. The actuator is driven by a two-axis controller electronics board which was developed by us based on digital processing in a dsPIC33EP device with analog periphery, which reads the quad-photodiode signals amplified by 109, and drives the actuator with two high-current outputs. While this realizes the very fine and relatively fast (up to 100 Hz) fiber-position control in the telescope focus, as a basis to this, a relatively coarse and slow auto-guiding is given by an amateur guiding camera. During first optical bench testing we obtained an average coupled power increase of up to 50% under certain perturbations.

Inflammation is a critical component in carcinogenesis. The neutrophil-to-lymphocyte ratio (nlr) has been retrospectively studied as a biomarker of prognosis in metastatic colorectal cancer (mcrc). Compared with a low nlr, a high nlr is associated with worse prognosis. In the present study, we compared real-world survival for patients with mcrc based on their nlr group, and we assessed the utility of the nlr in determining first-line chemotherapy and metastasectomy benefit. In this retrospective and descriptive analysis of patients with mcrc undergoing first-line chemotherapy in a single centre, the last systemic absolute neutrophil and lymphocyte count before treatment was used for the nlr. A receiver operating characteristic curve was used to estimate the nlr cut-off value, dividing the patients into low and high nlr groups. Median overall survival (mos) was compared using Kaplan&ndash, Meier curves and the log-rank test. A multivariate analysis was performed using a Cox regression model. The 102 analyzed patients had a median follow-up of 15 months. Regardless of systemic therapy, approximately 20% of patients underwent metastasectomy. The nlr cut-off was established at 2.35, placing 45 patients in the low-risk group (nlr < 2.35) and 57 in the high-risk group (nlr &ge, 2.35). The Kaplan&ndash, Meier analysis showed a mos of 39.1 months in the low-risk group and 14.4 months in the high-risk group (p < 0.001). Multivariate Cox regression on the nlr estimated a hazard ratio of 3.08 (p = 0.01). Survival analysis in each risk subgroup, considering the history of metastasectomy, was also performed. In the low-risk group, mos was longer for patients undergoing metastasectomy than for those not undergoing the procedure (95.2 months vs. 22.6 months, p = 0.05). In the high-risk group, mos was not statistically different for patients undergoing or not undergoing metastasectomy (24.3 months vs. 12.7 months, p = 0.08). Our real-world data analysis of nlr in patients with mcrc confirmed that this biomarker is useful in predicting survival. It also suggests that nlr is an effective tool to choose first-line treatment and to predict the benefit of metastasectomy.

La colitis granulomatosa representa las particulares manifestaciones clínicas y radiológicas de la enfermedad de Crohn, con localización en el colon. Aunque puede localizarse en cualquier órgano del aparato digestivo, la afección del colon sigue en importancia a la ileocecal, con idénticas características anatomopatológicas. La radiología es un pilar Fundamental del diagnóstico al establecer signos directos e indirectos de las alteraciones macroscópicas, de su evolución y complicaciones. Presentamos nuestra experiencia en 53 casos de colitis granulomatosa, de los que en 25 existía además afectación ileal. Tres pacientes en fase precoz, presentaron contornos irregulares y pólipos inflamatorios, destacando la importancia del estudio del patrón mucoso. En fase no estenótica con signos evidentes de afectación granulomatosa, se observaron 42 casos con típico patrón en suelo de guijarros y 8 casos en fase no estenótica, con pérdida de relieve mucoso y de haustras. La presencia de fistulizaciones fue poco frecuente; solamente dos casos.

We present concept and first experimental lab results for a low-cost near-infrared heterodyne interferometer based on commercial 1.55μm fiber components with relative phase-stabilization between both telescopes. After a demonstration with 14”-telescopes, the concept should be upgradable to larger numbers of mid- or large-class telescopes. Given that the employed fiber phase stabilization scheme should enable the operation of long baselines, we discuss the applicability of this concept for long-baseline, high telescope number systems (scalability of the concept) and mid-infrared wavelengths. This could finally result in contributions to the design of the large infrared Planet Formation Imager which is being proposed currently.

Clinical alterations related to diving accidents are little known perhaps because of its low prevalence. They can be classified as dysbarism-related and non-dysbarism-related according to its relation with pressure changes. We present the case of a diver that showed bradypsychia and transitory aphasia after carrying out an immersion of 30 minutes in duration and 22 meters deep. A left sylvian cerebral infarct was demostrated in cranial computerized axial tomography. Treatment was carried out in a hyperbaric chamber with good clinical evolution. We review with regard to this case the different forms of clinical presentation of dysbarism-related accidents, as well as the usefulness of the hyperbaric chamber in their treatment.

La segunda parte de este artículo ofrece un resumen de una de las investigaciones realizadas por el GREDI y que ha permitido diagnosticar los elementos que configuran el concepto y la práctica de la ciudadanía en los jóvenes de 14 a 16 años en Barcelona. Se concluye señalando los elementos significativos que pueden encontrarse en las investigaciones presentadas y que permiten dibujar las líneas de avance para quienes deseen adentrarse en la investigación de esta área temática. ___________________________________________________________________________________________________________________________________________ Cette approche de l’étude de la citoyenneté interculturelle à partir de la recherche éducative débute par une synthèse des nouveaux concepts de citoyenneté élaborés par le GREDI au cours des recherches effectuées ces dernières années. L’auteur défend que c’est l’éducation qui débouchera sur cette nouvelle citoyenneté à la fois active et responsable, critique et interculturelle, appelée à s’exercer au niveau local mais ouverte également sur des horizons plus larges. Après une synthèse des principales recherches effectuées dans ce domaine, la deuxième partie de l’article est consacrée au compte-rendu d’un travail du GREDI ayant permis de diagnostiquer les éléments constituants la théorie et la pratique de la citoyenneté chez les jeunes Barcelonais âgés de 14 à 16 ans. Enfin, un relevé des aspects significatifs des travaux publiés récemment donne des orientations qui peuvent être utiles pour ceux qui souhaiteraient poursuivre des recherches dans ce domaine., This article examines the study of intercultural citizenship within educational research. In so doing, the article first summarises the new approaches to citizenship underpinning the research conducted by the GREDI (Intercultural Education Research Group) in recent years. A model is presented which incorporates, as key traits of the citizenship that we seek to develop through education, the fact of being both active and responsible, critical and intercultural; played out in the local sphere but open to wider levels. The article moves on to summarise some of the research undertaken in this field. The second part offers a précis of one of the studies carried out by the GREDI and which has enabled us to diagnose the elements, both conceptually and in practice, that make up the citizenship of adolescents aged between 14 and 16 years old in Barcelona. The article concludes by identifying the significant elements to be found in the research presented, and which allow the way forward to be mapped out for those seeking to conduct research in this field., En este artículo ofrecemos una aproximación al estudio de la ciudadanía intercultural desde la investigación educativa. Para ello, hemos resumido en primer lugar, los nuevos planteamientos de la ciudadanía que fundamentan la investigación realizada por el GREDI en estos últimos años. Cortical development disorders constitute a group of entities resulting from an interruption in the development of the central nervous system. Most of them derive from the stoppage of migration, but proliferation and organisation can also be affected. Heterotopia is the most frequent of all migration disorders.We describe three cases of familial periependymal heterotopia consisting in three sisters who, after having seizures in infancy, were submitted to a magnetic resonance (MR) scan that revealed isointense nodules in the cortical grey matter in all the MR sequences. They were diagnosed as having periependymal heterotopia. The existence of a history of early onset seizures in the family of the mother supported the diagnosis of familial heterotopia.Heterotopia is the most frequently occurring anomaly affecting cortical development and, of these, the periependymal form is the most common. Periependymal heterotopia may be determined by sex-linked inheritance (X chromosome). It is considered to be one of the most common congenital disorders in familial and early onset epilepsy. MR is the preferred diagnostic technique, since its high resolution allows it to identify and characterise heterotopias.

The purpose of this work was to compare the results obtained with a scitigraphy using 99mTc-HMPAO labeled leukocyte and a computed tomography (CT) in the diagnosis of vascular graft infection (VGI). We carried out 22 studies with both scintigraphy and CT in 20 patients with vascular grafts with VGI suspicion. One patient had two grafts, thus a total of 23 grafts were analyzed. In addition, 6 control studies were performed. The persistent increase in leukocyte uptake in the graft area was considered as evidence of VGI and the scintigraphy with HMPAO-99mTc labeled leukocytes. For the CT, the presence of perigraft fluid and/or gas collection in the graft bed was also considered to be evidence of VGI. The total number of VGI was 12. The VGI diagnosis was confirmed by culture in all cases. The sensitivity and specificity of the scintigraphy in the diagnosis of VGI was 100%, while the CT has a sensitivity of 58% and specificity of 91%. Five grafts presented pseudoaneurysm in distal aorto-bifemoral graft anastomosis. All theses cases were detected with CT and only one patient with pseudoaneurysms showed scintigraphic evidence of VGI. In addition, both techniques detected two soft tissue infections associated with cutaneous fistulae, one case of abdominal abscess and another of acute diverticulitis. The scintigraphy was also pathologic in one infection of the surgical wound while that the CT was negative. In conclusion, although both techniques are useful to evaluate patients with suspicion of VGI, the scintigraphy with 99m-Tc-HMPAO labeled leukocytes has been shown to be more sensitive than the CT in the assessment of patients with VGI suspicion.

Consultable des del TDX, Títol obtingut de la portada digitalitzada, All clinically relevant Enterobacteriaceae strains isolated between 1994 and 1996 without inducible chromosomal b-lactamase, that showed decreased susceptibility to broad-spectrum cephalosporins and/or aztreonam, were selected. The mechanism implicated in the decreased susceptibility was determined by analytical isoelectric focusing, PCR and/or sequenciation. The results obtained showed that the most frequent mechanism implicated in the decreased susceptibility to broad-spectrum cephalosporins and/or aztreonam in E. coli and K. oxytoca was the hyperproduction of the chromosomal b-lactamase, followed by the SHV-1 hyperproduction in E. coli and K. pneumoniae. In our hospital, the incidence of plasmid-mediated extended-spectrum b-lactamases (ESBLs) between 1994 and 1996 was low (0.14%). The b-lactamase that couldn't be identified by the techniques previously described, was characterised by conjugation studies, clonation, and sequenciation experiments, allowing us to describe a new b-lactamase, CTX-M-9, that was the most frequent ESBL detected in our laboratory. The CTX-M-type b-lactamases has been described in various species of the family Enterobacteriaceae in geographically and temporally widely distant areas, most of them being plasmid-encoded. To know more about the dissemination of these enzymes around the world, we decided to study the environment of blaCTX-M-9. This study suggested us that blaCTX-M-9 is contained in a new complex integron, In60. This integron has the common 5'-CS and 3'-CS conserved sequences of class 1 integron and two gene cassettes encoding for a dihydrofolate reductase (dfrA16) and aminoglycoside-adenylytransferase (aadA2). Downstream of the first sul1 gene is present the orf513. Following this region there is the blaCTX-M-9 and an orf3-like region showing both about 80% identity with blaKLUA-1 and orf3 of K. ascorbata, respectively. Percutaneous gastrostomy (PG) was performed on 18 patients and 11 patients were subjected to surgical gastrostomy (SG) in the HCU of Zaragoza from October 1990 to November 1991. Involvement of the high digestive tract was the most common finding: 11 patients had CA of the oesophagus and stomach, 2 suffered from extrinsic pressure of the oesophagus due to metastatic adenopathy of a lung and breast tumour and 9 patients had CA of the larynx with total dysphagia. Another less common indication in our series, although still of great importance, was a severe involvement of the CMS, which made oral nutrition impossible due to functional dysphagia. Due to the long-term nature of this pathology, feeding by nasogastric tube was not considered. This was the case of 4 patients. The duration of enteral nutritional in the patients' home was 112.8 +/- 61 days with PG and 37 +/- 26 days with SG (p: 0.007). The incidence of complications was 33% in PG and 73% in SG, and for both techniques, the most common complication was blockage of the tube. Percutaneous gastrostomy is a simple technique, with no surgical risks involved, but with the benefits of traditional surgical techniques which, despite the small number of cases recorded, has very few complications both with regard to placing and afterwards, and also enables the patient to receive nutrients earlier than would be the case with traditional surgical techniques during prolonged periods at the patient's home.

En este trabajo, se construyó y evaluó la respuestaelectroquímica de un electrodo de membran aheterogénea selectivo a cimetidina incorporando un complejo de par iónicocimetidina - ácidofosfotúngstico (CMT-AFT) como material electroactivo, dibutilftalato (DBP) comoplastificante y policloruro de vinilo (PVC) comosoporte. Se optimizaron los parámetros que afectan el funcionamiento del electrodo, tales como: composición de la membrana, concentración de la disolución interna y pH. El electrodo mostró una respuesta rápida y estable con un intervalo lineal desde 1,00x10-4 hasta 1,00x10-1 M encimetidina al serutilizadoen un rango de pH entre 2,73 – 5,00 unidades, unapendiente de 56,63 ± 0,06 mV pordécada y un límite de detección de 3,98x10-5 M a 25,00 ± 1,00 ° C. El electrodoexhibióunabuenaselectividad para la cimetidina con respecto a un número de ionesinorgánicoscomunes. El electrodo de membranafueaplicadoen la determinación de cimetidina en formulacione sfarmacéuticas mediante potenciometría directa obteniéndose resultados que no difieren significativamente con los declarados por las empresasfarmacéuticas.

Calcium-modified lead titanate extruded and isostatically pressed ceramics have been studied, for the first time, by depth profile Auger electron spectroscopy. Our results show the high potential of the technique to determine the composition of inter- and intragranular fracture surfaces. Two different mechanisms of lead enrichment of the fracture surfaces are observed. Finally, the results are compared with a model of preferential sputtering of the different oxides.
