Few studies investigate the influence of inflammatory bowel disease (IBD) on complications following total knee arthroplasty (TKA). Therefore, we compared complications and readmissions frequencies after TKA in patients with Crohn's disease (CD) and ulcerative colitis (UC) to patients without IBD.A large administrative claims database was used to identify patients who underwent primary TKAs from 2010 to 2019 and had a diagnosis of IBD before TKA. Patients were stratified into two groups: those with CD (n = 8,369) and those with UC (n = 11,347). These patients were compared a control of 1.3 million patients without an IBD diagnosis. Chi-square and unadjusted odds ratios (OR) with 95% confidence intervals (CI) were used to compare complication frequencies. Multivariable logistic regression was used to evaluate independent risk factors for 90-day complications.Compared to patients without IBD, patients with IBD were associated with higher unadjusted 90-day odds for Clostridium difficile infection (CDI) (CD: OR 2.81 [95% CI 2.17 to 3.63]; p 0.001; UC: OR 3.01 [95% CI 2.43 to 3.72]; p 0.001) and two-year periprosthetic joint infection (CD: OR 1.34 [95% CI 1.18 to 1.52]; p 0.001; UC: OR 1.26 [95% CI 1.13 to 1.41]; p 0.001). After controlling for risk factors like obesity, tobacco use, and diabetes, both types of IBD were associated with higher 90-day odds for CDI and PJI (p 0.001 for all).IBD is associated with higher 90-day postoperative CDI and PJI compared with patients without IBD. Providers should consider discussing these risks with patients who have a diagnosis of IBD.

Surgical site infections (SSIs) are among the most prevalent and devastating complications following lower extremity total joint arthroplasty (TJA). Strategies to reduce the rates can be divided into preoperative, perioperatives, and postoperative measures. A multicenter trial is underway to evaluate the efficacy of implementing a bundled care program for SSI prevention in lower extremity TJA including: (1) nasal decolonization; (2) surgical skin antisepsis; (3) antimicrobial incise draping; (4) temperature management; and (5) negative-pressure wound therapy for selected high-risk patients. The purposes of this systematic review were to provide a background and then to summarize the available evidence pertaining to each of these SSI-reduction strategies with special emphasis on total knee arthroplasty. A systematic review of the literature was conducted in accordance with the 2009 Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement guidelines. Five individual literature searches were performed to identify studies evaluating nasal decolonization temperature management, surgical skin antisepsis, antimicrobial incise draping, and negative-pressure wound therapy. The highest level of evidence reports was used in each product review, and if there were insufficient arthroplasty papers on the particular topic, then papers were further culled from the surgical specialties to form the basis for the review. There was sufficient literature to assess all of the various prophylactic and preventative techniques. All five products used in the bundled program were supported for use as prophylactic agents or for the direct reduction of SSIs in both level I and II studies. This systematic review showed that various pre-, intra-, and postoperative strategies are efficacious in decreasing the risks of SSIs following lower extremity TJA procedures. Thus, including them in the armamentarium for SSI-reduction strategies for hip and knee arthroplasty surgeons should decrease the incidence of infections. We expect that the combined use of these products in an upcoming study will support these findings and may further enhance the reduction of total knee arthroplasty SSIs in a synergistic manner.

Computed tomography (CT) scan–based three-dimensional (3D) modeling operative technology has been shown to improve upon results of manual total knee arthroplasties (TKAs). Although there are many reports on superior precision of this CT-based technology, there has been continuing interest regarding extended clinical outcomes. The purpose of this study was to compare their clinical outcomes with manual TKAs at approximately 3-year follow-up. Specifically, we analyzed: (1) survivorship, (2) functional outcomes, (3) complications, and (4) radiographic outcomes (i.e., alignment, progressive radiolucencies). A total of 210 patients receiving CT-based TKAs performed by a single surgeon at a single center between July 1, 2016, and February 16, 2018, were compared with 210 manual TKAs completed by the same surgeon immediately preceding implementation of the CT-based technology. Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) surveys were collected at ∼3 years postoperatively. Subgroup analyses of pain and physical function scores were performed. Follow-up radiographs were evaluated for alignment, loosening, and/or progressive radiolucencies. There was 100% survivorship at final follow-up. As evidence signifies that short-stay total knee arthroplasties (TKA) can be safe options, it is important to identify factors that contribute to early discharge. There is evidence that robotic-assisted TKAs may lead to shorter lengths of postoperative stays. However, this has not been evaluated as the primary outcome of interest in a large-scale population. The purpose of this study was to compare manual and robotic-assisted TKAs with regard to: (1) length of stay (LOS) and (2) discharge dispositions. TKAs performed between January 1, 2018, to March 31, 2019, were identified. After applying inclusion and exclusion criteria, a total of 10,296 patients were included: 5,993 in the manual and 4,303 in the robotic-assisted group. Length of stay, discharge dispositions, and Charlson comorbidity indices (CCIs) were recorded for all patients. Intra-articular injections prior to total hip arthroplasty (THA) have been associated with postoperative infections. The purpose of this study was to determine whether a temporal relationship exists between hip injections prior to THA and infection. Specifically, we asked (1) Do patients who receive hip injections within 3 months of THA have a higher incidence of prosthetic joint infections (PJIs) or surgical site infections (SSIs)? and (2) Do these patients incur higher 90-day costs? Patients with hip injections prior to THA were identified using a national database from 2010 to 2019. Three laterality-specific groups (injection 0 to 3 months, 3 to 6 months, and 6 to 12 months prior to THA)were compared with a matched cohort without prior injection (n=277,841). Primary outcomes included PJIs, SSIs, and costs. Patients who had injections within 3 months of THA had a higher incidence of PJIs at 90 days (5.1% vs 1.6%, P P P P P P ≥.46). Patients who receive hip injections within 3 months of undergoing primary THA are at increased risk for postoperative PJIs, SSIs, and higher costs. Aims Social determinants of health (SDOHs) may contribute to the total cost of care (TCOC) for patients undergoing total knee arthroplasty (TKA). The aim of this study was to investigate the association between demographic data, health status, and SDOHs on 30-day length of stay (LOS) and TCOC after this procedure. Methods Patients who underwent TKA between 1 January 2018 and 31 December 2019 were identified. A total of 234 patients with complete SDOH data were included. Data were drawn from the Chesapeake Regional Information System, the Centers for Disease Control social vulnerability index (SVI), the US Department of Agriculture, and institutional electronic medical records. The SVI identifies areas vulnerable to catastrophic events with four themed scores: socioeconomic status; household composition and disability; minority status and language; and housing and transportation. Food deserts were defined as neighbourhoods located one or ten miles from a grocery store in urban and rural areas, respectively. Multiple regression analyses were performed to determine associations with LOS and costs after controlling for various demographic parameters. Results Divorced status was significantly associated with an increased LOS (p = 0.043). Comorbidities significantly associated with an increased LOS included chronic obstructive pulmonary disease/asthma and congestive heart failure (p = 0.043 and p = 0.001, respectively). Communities with a higher density of tobacco stores were significantly associated with an increased LOS (p = 0.017). Comorbidities significantly associated with an increased TCOC included chronic obstructive pulmonary disease (p = 0.004), dementia (p = 0.048), and heart failure (p = 0.007). Increased TCOCs were significantly associated with patients who lived in food deserts (p = 0.001) and in areas with an increased density of tobacco stores (p = 0.023). Conclusion Divorced marital status was significantly associated with an increased LOS following TKA. Living in food deserts and in communities with more tobacco stores were significant risk factors for increased LOS and TCOC. Food access and ease of acquiring tobacco may both prove to be prognostic of outcome after TKA and an opportunity for intervention. Cite this article: Bone Joint J 2021;103-B(6 Supple A):113–118.

Background: The number of golfers aged ≥65 years has increased in recent years, and shoulder arthritis is prevalent in this age group. Guidelines for return to golf (RTG) after shoulder arthroplasty have not been fully established. Purpose: To review the data available in the current literature on RTG after shoulder arthroplasty. Study Design: Systematic review. Methods: A systematic review based on the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines was performed. Two independent reviewers searched PubMed, Embase, and the Cochrane Library using the terms “shoulder,”“arthroplasty,”“replacement,” and “golf.” The authors sought to include all studies investigating RTG after total shoulder arthroplasty (TSA), shoulder hemiarthroplasty (HA), and reverse shoulder arthroplasty (RSA). Outcomes of interest included indications for shoulder arthroplasty, surgical technique, rehabilitation protocol, amount of time between surgery and resumption of golf activity, and patient-reported outcome measures. Results: A total of 10 studies were included, 2 of which reported on golf performance after shoulder arthroplasty. The other 8 studies described return to sports after shoulder arthroplasty with golf-specific data for our analysis. Three studies that included patients who underwent TSA reported RTG rates ranging from 89% to 100% after mean follow-up periods of 5.1 to 8.4 months. Two studies included patients who underwent TSA and HA and reported RTG rates of 77% and 100% after mean intervals of 5.8 and 4.5 months, respectively. Two studies included patients who underwent RSA, with RTG rates of 50% and 79% after mean postoperative intervals of 5.3 and 6 months, respectively. One study included only patients undergoing HA, with an RTG rate of 54% and a mean RTG time of 6.5 months. Varying surgical procedures and baseline patient characteristics precluded our ability to draw conclusions regarding surgical technique, rehabilitation protocol, or patient-reported outcome measures among studies reporting these data. Conclusion: Most patients who undergo a shoulder arthroplasty procedure can expect to resume playing golf approximately 6 months after the index procedure. The rate of return may be lower after RSA and HA as compared with anatomic TSA. The data presented in our review can help physicians counsel patients who wish to continue golf participation after a shoulder arthroplasty procedure.

Background Newer generation cementless total knee arthroplasty (TKA) designs have provided stronger osteointegration between the implant and bone. Despite excellent survivorship and outcomes with cemented TKAs, areas of concern within the bone–cement interface remain a concern and necessitate studies on alternative constructs. This study assesses: (1) implant survivorship; (2) clinical outcomes; and (3) complications with radiographic outcomes at a 5-year minimum follow-up of cementless highly porous titanium-coated baseplates in TKAs. Methods Part of this study has been reported. We retrospectively reviewed a prospectively collected database at a single high-volume institution between July 1, 2013 and June 30, 2014 for patients who underwent a primary TKA using cementless highly porous titanium-coated baseplate implants. Patients were evaluated clinically at postoperative follow-up visits at a minimum of 5 years. To calculate the survivorship, Kaplan–Meier analysis was performed to determine all-cause, aseptic, and septic implant survivorship at each final follow-up for all patients. Results A total of 228 TKAs were performed and followed for a minimum of 5 years (range, 5–6 years). As of the latest follow-up, one case of septic loosening of the patellar button and one case of patellar dislodgment secondary to physical manipulation were recorded and revised. Overall, the cohort displayed implant survivorship of 99.5% at 5-year minimum follow-up. Improvements were seen in both Knee Society pain and function scores and were 37 points (range, 17–60 points) and 28 points (range, 15–47 points), respectively. The mean improvement in flexion was 17.8 (range, –20 to 40 degrees) and mean improvement in extension was –5.5 (range, –30 to 5 degrees). Discussion Cementless TKAs serve as strong alternative choice to cemented TKAs. Although cemented fixation is commonly known as the gold standard, results of this study confirm the findings of previous investigations on the survivorship of cementless TKA implants. Therefore, patients who undergo primary TKA with a cementless tritanium baseplate can expect excellent clinical outcomes at a 5-year minimum follow-up.

Modular dual mobility cups have been developed to potentially address postoperative hip instability, which can occur in nearly 20% of all revision total hip arthroplasty (THA) patients. By having a prosthetic construct that contains two points of articulation between the femoral head and liner and between the liner and shell, joint stability can be increased. The purpose of this study was to report on dual mobility cup survivorships, patient satisfaction outcomes, complications, and radiographic outcomes at a minimum 7-year follow-up. A high-volume academic surgeon performed a total of 143 consecutive dual mobility primary THAs on patients who had a minimum follow-up of 7 years (range, 7–8.5 years). The study cohort consisted of 77 females (54%) and 66 males (46%) who had a mean age of 65 years (range, 34–90 years). Aseptic, septic, and all-cause survivorship was determined by Kaplan-Meier analysis. Harris Hip Scores (HHS), postoperative complications, and radiographs were also assessed. No cup failures were observed. Overall, septic survivorship was 99.3% (95% confidence interval [CI]: 0.98–1.0) and all-cause survivorship was 98.6% (95% CI: 0.97–1.0). Two patients (1.4%) required revision surgery unrelated to the use of a modular dual mobility cup. Of these, one patient experienced femoral stem loosening and the other developed a periprosthetic infection that was treated with a two-stage revision. The mean total HHS was above 95 points at the most recent follow-up. Three patients (2.3%) experienced medical complications, including two deep vein thromboses and one for nonfatal pulmonary embolism. Radiographic evidence revealed incomplete seating of the metallic liner in one patient. Dual mobility cups were developed in an attempt to decrease the rate of instability following THA. The results from this study indicate that excellent clinical and patient-reported outcomes can be achieved at 7-year follow-up in patients who undergo THA with a dual mobility cup. Therefore, dual mobility cups appear to be an appropriate treatment option for primary THA.

Large-scale studies evaluating the effects of Parkinson's disease (PD) on primary total knee arthroplasty (TKA) are limited. The purpose of this study was to determine if PD patients undergoing primary TKA have increased: (1) medical complications; (2) implant-related complications; (3) readmission rates; and (4) costs. A query was performed using an administrative claims database. The study group consisted of all patients undergoing primary TKA who had a history of PD. Matched non-PD patients undergoing primary TKA served as a control group. The query yielded 72,326 patients (PD = 18,082; matching cohort = 54,244). Pearson's chi-square tests, logistic regression analyses, and Welch's t-tests were used to test for significance between the cohorts. Primary TKA patients who had PD were found to have greater incidences and odds of medical complications (4.21 vs. 1.24%; odds ratio [OR]: 3.50, 95% confidence interval [CI]: 3.15–3.89, p

Clinicians should exercise a high level of suspicion in at-risk patients (those who use corticosteroids, consume excessive alcohol, have sickle cell disease, etc.) in order to diagnose osteonecrosis of the femoral head in its earliest stage. ➢. Nonoperative treatment modalities have generally been ineffective at halting progression. Thus, nonoperative treatment is not appropriate in early stages when one is attempting to preserve the native joint, except potentially on rare occasions for small-sized, medially located lesions, which may heal without surgery. ➢. Joint-preserving procedures should be attempted in early-stage lesions to save the femoral head. ➢. Cell-based augmentation of joint-preserving procedures continues to show promising results, and thus should be considered as an ancillary treatment method that may improve clinical outcomes. ➢. The outcomes of total hip arthroplasty in the setting of osteonecrosis are excellent, with results similar to those in patients who have an underlying diagnosis of osteoarthritis.

The number of total knee arthroplasties (TKAs) performed in the United States has increased considerably in recent years, with a major contribution from younger patients. Maximizing survivorship of these implants has always been a point of emphasis. Early TKA designs with cementless fixation were associated with high rates of complications and implant failures. However, recent advances in cementless designs have shown excellent results. The decision to use cemented or cementless fixation for patients undergoing TKA is typically based on the surgeon's experience and preference. However, several patient characteristics must also be taken into account. The purpose of this review was to describe the clinical outcomes of studies in which a cementless TKA system was utilized for patients who (1) were less than 60 years of age, (2) were greater than 75 years of age, (3) were obese, (4) had rheumatoid arthritis, and (5) had osteonecrosis of the knee. Based on the studies included in this review, it appears that cementless fixation is a viable option for patients who have all of the above demographics.

Introduction: Opioid use disorder (OUD) patients have an increased risk of venous thromboembolism (VTE), readmissions, and higher costs following primary elective primary total joint arthroplasty, but these risks have not yet been clarified for other arthroplasty surgeries. Thus, the purpose of this study was to investigate whether OUD patients undergoing revision total knee arthroplasty (RTKA) have higher rates of: VTEs, readmissions, and costs of care. Materials and Methods: Patients who had a 90-day history of OUD prior to undergoing RTKA were identified and randomly matched to a comparison cohort in a 1:4 ratio by age, sex, Elixhauser-Comorbidity Index (ECI), diabetes mellitus, hyperlipidemia, hypertension, obesity, and tobacco use with a total of 16,851 patients collectively in both groups. The 90-day frequency and odds (OR) of developing VTE, deep vein thrombosis (DVTs), and PEs along with 90-day readmission rates and 90-day costs of care were analyzed. Recent studies have shown the prevalence of depressive disorders has increased within the United States. Studies investigating the impact of depressive disorders following primary THA are limited. Therefore, the purpose of this study was to determine whether patients with depressive disorders have higher rates of: 1) in-hospital lengths of stay (LOS); 2) readmission rates; 3) medical complications; and 4) implant-related complications.A retrospective query of the Humana claims database was performed. Patients undergoing primary THA with a history of depressive disorders were identified by International Classification of Disease, Ninth Revision (ICD-9), and Current Procedural Terminology (CPT) codes. Study group patients were matched to controls in a 1:5 ratio by age, sex, and comorbidities. The query yielded 67,245 patients with (n=11,255) and without (n=55,990) depressive disorders. Welch's t-tests were used to test for significance in LOS between the cohorts; whereas, logistics regression analyses were used for complications and readmissions. A p-value less than 0.003 was statistically significant.Patients with depressive disorders undergoing primary THA had significantly longer in-hospital LOS (6.59 days vs. 2.96 days, p0.0001). Additionally, patients with depressive disorders had higher incidence and odds of readmission rates (46.02 vs. 35.43%; OR: 1.55, p0.0001), medical complications (7.05 vs. 1.84%; OR: 4.04, p0.0001), and implant-related complications (5.76 vs. 2.75%; OR: 2.16, p0.0001) compared to patients without depressive disorders.After matching age, sex, and medical comorbidities, the results of the study demonstrate patients with depressive disorders have longer in-hospital LOS and increased rates of complications and readmission rates. The study is useful as it can allow orthopedic surgeons to properly counsel these patients of the potential complications which may arise following their procedure.

Introduction: Approximately half of dislocating total hip arthroplasties (THAs) demonstrate acetabular component position within traditional safe zones. It is unclear if postoperative functional acetabular position can be reliably improved by considering preoperative pelvic tilt. We investigated whether standing cup position targets could be more accurately achieved by considering preoperative standing pelvic tilt in addition to bone landmarks when planning for robot-assisted THA. Methods: We reviewed 146 THAs performed by a single surgeon using computed tomography-based 3-dimensional planning and robotic technology to guide acetabular reaming and component insertion. Planning for 73 consecutive cases started at 40° of inclination and 22° of anteversion relative to the supine functional plane and was adjusted to better match native hip anatomy. Planning for the next 73 cases was modified to consider standing pelvic position based on standing preoperative radiographs. We compared groups to determine the rate when cups were placed outside our standing targets of 15–30° anteversion and 35–50° inclination. Results: Cup position proved to be reliable in both groups, with 83% of cups in the anatomic planning cohort and 90% of cups in the functional planning cohort achieving standing targets for both anteversion and inclination ( p = 0.227). Variances were lower in the functional planning group: 9.4° versus 15.8° of inclination ( p = 0.079) and 18.3° versus 26.1° of anteversion ( p = 0.352). The range of functional positions was narrower in the functional planning group: 35.7–47.5° versus 31.8–54.9° of inclination and 16.7–35.0° versus 10.1–35.9° of anteversion. Discussion: Our results suggest enhanced planning that considers pelvic tilt, when coupled to a precision tool to achieve the plan, can reliably achieve target standing component positions. Considering preoperative functional pelvic position may improve postoperative functional acetabular component placement in THA, but the clinical benefit of this has yet to be confirmed.

Surgical-site infections (SSIs) are among the most difficult-to-manage complications after lower extremity total joint arthroplasty (TJA). While the rates of most implant-related complications have decreased over time due to improvements in prosthetic materials and surgical techniques, the incidence of periprosthetic joint infections (PJIs) continues to increase. They place a tremendous economic burden on healthcare systems that is projected to reach $1.8 billion by the year 2030. A number of perioperative infection mitigation strategies exist that are often implemented concurrently to minimize the risk of these complications. A multicenter randomized controlled trial is underway to evaluate the efficacy of a bundled care program for the prevention of PJIs in lower extremity TJA. This bundle includes five infection-reduction strategies that are used pre-, peri-, and postoperatively, including: (1) povidone-iodine skin preparation and nasal decolonization; (2) iodine-alcohol surgical prepping solution; (3) iodophor-impregnated incise drapes; (4) forced-air warming blankets; and (5) negative pressure wound therapy for select patients. The aim of this review is to describe these products and their appropriate usage, review the available literature evaluating their use, and compare them with other commercially available products. Based on the available literature, each of these strategies appear to be important components for SSI-prevention protocols. We believe that implementing all five of these mitigation strategies concurrently will lead to a synergistic effect for infection control following lower extremity TJA.

To determine the effectiveness of simulator training on basic arthroscopic skills utilizing a novel, low-cost arthroscopic triangulation training system.A randomized controlled trial of subjects without prior arthroscopy training was conducted, with participants randomized to receive either a fixed protocol of simulation training on a triangulation simulation model (30 minutes of training for 4 consecutive days), or no training. On Days 1 and 5, all participants were evaluated on 3 simulated arthroscopic tasks by an independent observer. Variables analyzed included how many times portals were changed, the time it took to complete the tasks, and the task completion rate.Arthrex Inc., Naples, FL.Thirty-six participants (92% male, average 28 ± 5 years) with no prior arthroscopy training were randomized into 2 groups, with 17 in the training group (T) and 19 in the no-training group (NT).On Day 1, there was no difference in rate of task completion between the T group and NT groups (41% versus 53%, p = 0.52). On Day 5, significantly more participants in the T group completed all tasks compared to the NT group (100% versus 63%, p = 0.008). Participants in the T group had significantly improved task completion times on Day 5 versus Day 1 (p0.05). Participants in the NT group had a significantly improved task completion time for Task 1 on Day 5 versus Day 1 (p = 0.037); no differences were found for Tasks 2 or 3. On Day 5, participants in the T group required significantly fewer portal changes compared to the NT group (2.35 ± 2.29 versus 6.95 ± 8.55, p = 0.039).Simulation training on a simple, low-cost arthroscopic triangulation training system resulted in an overall improvement in arthroscopic probing and triangulation skills within 1 week of training, with significantly decreased task completion times and increased efficiency of movement.

Background: No consensus is available regarding the optimal choice of bone graft material for bone tunnel augmentation in revision anterior cruciate ligament (ACL) surgery. Purpose: To compare the outcomes of different bone graft materials for staged revision ACL reconstruction. Study design: Systematic review. Methods: A systematic review using PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines was performed. PubMed, EMBASE, and the Cochrane Library were queried through use of the terms anterior cruciate ligament and revision to identify all studies reporting outcomes of bone tunnel grafting in 2-stage revision ACL reconstruction. Data extracted included indications for 2-stage surgery, surgical technique, graft material, time between surgeries, rehabilitation protocols, physical examination findings, patient-reported outcomes, and radiographic and histologic findings. Results: The analysis included 7 studies with a total of 234 patients. The primary outcome in 2 studies was graft incorporation (mean follow-up, 8.8 months), whereas the other 5 studies reported clinical outcomes with follow-up mean ± SD of 4.2 ± 2.1 years. The indication for bone grafting and between-stage protocol varied among studies. Autograft was used in 4 studies: iliac crest bone autograft (ICBG, n = 3) and tibial bone autograft (TBA, n = 1). In 2 studies, the authors investigated the outcomes of allograft: allograft bone matrix (ABM) and allograft bone chips (AC). Finally, 1 study compared ICBG to a synthetic bone substitute. Radiographic evaluation of bone graft integration after the first stage was reported in 4 studies, with an average duration of 4.9 months. In 4 studies, the authors reported the time interval between first and second surgeries, with an average of 6.1 months for ICBG compared with 8.7 months for allogenic and synthetic grafts. Revision ACL graft failure rates were reported by 5 studies, including 1 study with ABM (6.1%), 1 study with AC (8.3%), 1 study with TBA (0%), and 2 studies with ICBG (0% and 2%). Conclusion: The indications for staged ACL reconstruction and the rehabilitation protocol between stages need to be clearly established. The available data indicate that autograft for bone tunnel grafting in 2-stage ACL revision may be associated with a lower risk of revision ACL reconstruction graft failure compared with allograft bone.

Purpose To compare outcomes of hip arthroplasty in patients with and without a history of hip arthroscopy through a systematic review. Methods A comprehensive search of the PubMed (MEDLINE) and Cochrane Central databases was performed using combinations of the keywords “hip,” “arthroscopy,” “arthroscopic,” “arthroplasty,” “replacement,” and “conversion” in December 2017. Level I through III studies directly comparing outcomes of total or resurfacing hip arthroplasty between patients with and without a history of hip arthroscopy were included in this review if they reported at least 1 outcome measure. Results Seven retrospective case-control studies collectively evaluating arthroplasty outcomes of 235 patients (104 male and 131 female patients) with a history of hip arthroscopy and 374 matched controls met the inclusion criteria. The mean age in the arthroscopy and control groups was 47.2 years and 49.1 years, respectively. The mean follow-up period after arthroplasty was 3.2 years in the hip arthroscopy group and 3.3 years in the control group. The mean time between arthroscopy and arthroplasty was 1.8 years. A posterior approach was used in 83.6% of arthroplasties. No statistically significant differences were noted in intraoperative measures, postoperative complications, or revision rates, with the exception of 1 study that reported an increased operative time among controls. Most studies reported similar subjective outcomes between groups, with a single study noting worse postoperative findings for the Harris Hip Score, Forgotten Joint Score-12, visual analog scale pain score, and patient satisfaction in the prior hip arthroscopy group. Conclusions The current literature suggests that short-term and midterm outcomes of hip arthroplasty are comparable in patients with and without a history of hip arthroscopy. However, the available literature is limited given the small sample sizes and therefore greater potential for β error. Nevertheless, our findings may be useful for surgeons evaluating risks and prognoses in this patient population. Level of Evidence Level III, systematic review of Level III studies.

These include acetabular bone deficiency, limb length discrepancy (LLD), and abnormal femoral anteversion. Three-dimensional planning and robot-assisted (RA) bone preparation may simplify these complex procedures and make them more reproducible. The purpose of this study was to evaluate radiographic and clinical outcomes in a cohort of patients who had DDH and underwent an RA THA.We retrospectively analyzed 26 DDH patients who underwent RA THA by a single surgeon between 2013 and 2019. Their mean age was 54 years (range, 29 to 72 years) and mean follow up was approximately two years. Medical records were reviewed for demographics, clinical scores, Crowe classifications, and complications. There were thirteen Crowe I and seven Crowe II DDH hips, who were routinely managed with primary cementless implants. Two patients who had Crowe III and four patients who had Crowe IV DDH were also identified. All hips were reconstructed with cementless hemispherical acetabular components with or without the use of screws, but no acetabular augments or bulk allografts. Implants allowing control of femoral anteversion were selected in 23.1% of cases, including all six cases with Crowe III or IV dysplasia, and the need for these implants was uniformly identified using preoperative information about femoral version provided by the three-dimensional planning software. No patient was managed with a shortening femoral osteotomy. Postoperative radiographs were examined for LLD, center of rotation (COR), cup position (inclination and anteversion), and component osseous-integration.Mean radiographic LLD was 1.7mm (range, -9 to +14) in patients who had Crowe I DDH, and there was no clinical LLDs greater than 5mm observed. Although patients who had Crowe II and greater DDH had a mean radiographic LLD of -11.6mm (range, -26 to +2.2), again no clinical LLD greater than 5mm was observed other than one patient who had bilateral Crowe II DDH in whom 10mm of clinical lengthening was accepted at the index arthroplasty with the plan to match lengths when her contralateral THA was performed. There were no cases of dislocation or acetabular fixation failure. One patient who had a femoral deformity and an intra-osseous blade plate from a prior femoral osteotomy suffered a failure of femoral osseous-integration, resulting in revision. A 32-point increase in mean modified Harris Hip Score (mHHS) was found (p=0.002), from 48 points preoperatively to 80 points postoperatively.RA THA provides an excellent option for the arthroplasty surgeon to both preoperatively localize and characterize the acetabular deficiency, while providing a targeted, optimal, and secure placement of the components intraoperatively. Our results suggest favorable outcomes when compared to previous research on manual THA in DDH. Optimal wound closure techniques following total knee arthroplasty (TKA) have focused on enhancing healing potential, preventing infection, yielding satisfactory cosmesis, and allowing early ambulation and functionality. An appropriate layered closure and management of the TKA typically involves addressing the (1) deep fascial layer; (2) subdermal layer; (3) intradermal layer, including the subcuticular region; and (4) final application of a specific aseptic dressing, each of which are covered here in detail. This focused critical review of the literature discusses traditional techniques used in all layers of wound closure following TKA while introducing several emerging popular techniques. For example, absorbable barbed skin sutures and occlusive dressings have the potential to reduce operative time, limit the need for early postoperative visits, obviate the need for suture or staple removal, and safely promote patient communication via telemedicine. As novel wound closure techniques continue to emerge and traditional approaches are improved upon, future comparative studies will assist in elucidating the key advantages of various options. In an extremely important field that has tremendous variability, these efforts may enable the reaching of a classically elusive standard of care for these techniques.

Telehealth has recently been used more often in an attempt to protect practitioners and patients during the 2019 coronavirus infectious disease (COVID-19) crisis. Despite telehealth's existence, there was no prior need to fully realize its potential. Recently, technological innovations in orthopaedic surgery have assisted in making this modality more useful. However, it is important to continually educate the medical community regarding these technologies and their interplay to improve patient care. Therefore, our purpose is to provide information on telehealth by assessing: (1) steps the hospital/system are taking to reduce COVID-19 exposure for teams and patients; (2) new technologies allowing for the optimization of patient safety; and (3) use of telehealth for postoperative follow up. We will demonstrate that telehealth and its associated strategies can be used effectively to decrease COVID-19 exposure risks for both medical staff and patients during these rapidly changing and uncertain times.

Surgical-site infections (SSIs) are among the most difficult-to-manage complications after lower extremity total joint arthroplasty (TJA). While the rates of most implant-related complications have decreased over time due to improvements in prosthetic materials and surgical techniques, the incidence of periprosthetic joint infections (PJIs) continues to increase. They place a tremendous economic burden on healthcare systems that is projected to reach $1.8 billion by the year 2030. A number of perioperative infection mitigation strategies exist that are often implemented concurrently to minimize the risk of these complications. A multicenter randomized controlled trial is underway to evaluate the efficacy of a bundled care program for the prevention of PJIs in lower extremity TJA. This bundle includes five infection-reduction strategies that are used pre-, peri-, and postoperatively, including: (1) povidone-iodine skin preparation and nasal decolonization; (2) iodine-alcohol surgical prepping solution; (3) iodophor-impregnated incise drapes; (4) forced-air warming blankets; and (5) negative pressure wound therapy for select patients. The aim of this review is to describe these products and their appropriate usage, review the available literature evaluating their use, and compare them with other commercially available products. Based on the available literature, each of these strategies appear to be important components for SSI-prevention protocols. We believe that implementing all five of these mitigation strategies concurrently will lead to a synergistic effect for infection control following lower extremity TJA.

Advanced imaging used in robotic-assisted total knee arthroplasty (TKA), such as computed tomography (CT)-based three-dimensional (3D) planning, may provide an accurate means of implant sizing preoperatively. The purpose of this study was to examine preoperative CT-based implant planning accuracy for robotic-assisted TKA in patients who have (1) varus deformities, (2) valgus deformities, (3) neutral alignment, and (4) retained hardware. A total of 393 patients underwent a robotic-assisted TKA by a single surgeon received preoperative CT scans. The surgeon reviewed the CT-based model preoperatively and recorded the expected size of the components. The final implants used in each case were recorded and compared with the surgeon's preoperative plan. In all groups of patients, the surgeon's CT-based implant plan was within one size of the implant utilized 100% of the time for both the tibiae and femora. Overall, the surgeon was exactly matched in 319 (81%) and 315 (80%) cases for the femoral and tibial components, respectively. For the femoral component, the mean age for patients in whom the original plan was exactly matched was younger than those whose implants were upsized and older than patients those implants were downsized (p = 0.024). Other patient demographics and preoperative knee alignment were not associated with predictive accuracy for femoral or tibial components. Our results demonstrate how preoperative CT-based, 3D planning for robotic-assisted TKA is accurate to within one size of the components in every case (100%), and exactly matched in 80%. The results of this study are important because they demonstrate how CT-based preoperative implant planning for TKA is reliable and accurate across all native knee alignments and other patient-specific factors. In addition, they build on a previous study by the same single surgeon, demonstrating that predictive ability can improve over time. This may be important as we move toward more outpatient surgery with less ability for prostheses inventory at ambulatory sites.

The acetabular "safe zone" has recently been questioned as a reliable reference for predicting total hip arthroplasty impingement and instability as many dislocations occur within the described parameters. Recently, an improved understanding of spino-pelvic mechanics has provided surgeons useful information to both identify those at a higher risk of dislocation and, in some cases, allows altering component positioning to accommodate the patient's individual "functional" range of motion. The purpose of this study was to create a new patient-specific impingement-free zone by considering range of motion (ROM) to prosthetic impingement for both high flexion and extension poses, thus demarcating a zone that avoids both anterior and posterior impingement, thereby creating an objective approach to identifying a patient's ideal functional safe zone.A validated hip ROM three-dimensional simulator was utilized to create ROM-to-impingement curves for both high flexion as well as pivot and turn poses. The user imported a computerized tomography (CT) with a supine pelvic tilt (PT) value of zero and implant models (tapered wedge stem, 132° neck angle, 15° stem version, 36mm femoral head). Femur-to-pelvis relative motions were determined for three upright seated poses (femur flexed at 90° and 40° internal rotation, with 0°, 10°, and 20° posterior PT), one chair rise pose (femur flexed at 90° and 0° internal rotation, with the pelvis flexed anteriorly until the pelvis made contact with the femur), and three standing pivot and turn poses (femur set at 5° extension, and 35° external rotation, with 5° posterior PT, 0°, and 5° anterior PT). ROM-to-impingement curves for cup inclination versus anteversion were graphed and compared against the Lewinnek safe zone.The ROM-to-impingement curves provide an objective assessment of potential impingement sites as they relate to femoral rotation and pelvic tilt. The area between the stand and sit curves is the impingement-free area. A sitting erect pose with a simulated stiff spine (0° PT) yielded less impingement-free combinations of cup inclination and version than poses with greater than 0° posterior pelvic tilt.The results demonstrate that the acetabular target zone has a relatively small margin for error between the sitting and standing ROM curves to impingement. Importantly, anterior and posterior pelvic tilt can markedly increase the risk of impingement, potentially leading to posterior or anterior dislocations, respectively. This study highlights the importance of correctly identifying the patient-specific functional range of motion to execute optimal component positioning.

Dual mobility constructs for THA have been a tremendous advancement for hip arthroplasty surgeons, especially in scenarios where instability is a possibility. While some researchers have reported events of malseating with their use, the authors of the current study believe that this may be avoided by ensuring appropriate surgical technique. Therefore, the purpose of this study was to: (1) describe the surgical techniques that we employ to ensure that the liner is adequately seated; and (2) report the rates of malseating, dislocation, and aseptic loosening among our collective cohort of dual mobility THA patients.All patients who underwent THA with a dual mobility construct between January 1, 2010 and December 31, 2018 at four institutions were identified. Those who had less than two years of follow up were excluded. Outcomes of interest included radiographic evidence of liner malseating, aseptic loosening, and dislocation. A total of 1,826 patients who underwent THA with a dual mobility construct were identified. Among these patients, 504 had less than two years of follow up and were excluded from our analysis. The remaining 1,322 patients met our criteria including 941 primary THAs (71.2%) and 381 revision THAs (28.8%).After a minimum follow-up period of two years, there were only two cases of malseated liners (0.15%). Serial follow ups have demonstrated no movement or changes in the position of the liners over time for both patients. In addition, they have been shown to have normal serum metal ion levels and no clinical complaints after 5.3- and 7.1-year follow up. Seven of 1,322 patients (0.53%) experienced a dislocation. Aseptic loosening of the acetabular cup was diagnosed in one patient 3.4 years postoperatively. In three patients, femoral component loosening occurred after a mean follow-up period of 2.3 years, (1.3 to 3.1 years). Among the 941 primary cases, the incidence of liner malseating was 0.21%, as both patients who experienced this complication were in this subgroup. As stated above, these patients have demonstrated normal serum metal ion levels and no clinical or radiographic sequelae as a result of the liner malseating. The dislocation rate among primary cases was 0.21% (2 of 941). Aseptic loosening of the acetabular component occurred in two (0.21%) while one patient (0.1%) was found to have femoral component loosening at final follow up. Of the 381 revision THAs, there were no cases of liner malseating. Five revision THA patients (1.3%) experienced a dislocation over our study period. Two revision THA patients experienced aseptic loosening of the femoral component (0.79%) at final follow up.The results of this paper demonstrate that malseating is not a prevalent issue with dual mobility THA when appropriate surgical techniques are used. It is hoped that that this paper clarifies the techniques for implantation of these implants and that excellent results can be achieved when care is taken to ensure that liners are well-seated intraoperatively.

As bundle payments have begun focusing on orthopaedic procedures, particularly total knee arthroplasties (TKAs), surgeons and hospitals have evaluated methods for improving efficiency. Few studies have investigated the impact of novel, sealed-container and instrument-tray technology on turnover and costs. Therefore, the purpose of this study was to compare traditional and sealed container-sterilized TKA surgical trays by: 1) investigating the setup and clean-down time in the operating room (OR); 2) examining trays processing time in central sterile supply (CS); and 3) estimating OR and CS costs and waste produced.An interdisciplinary team determined points throughout a TKA tray single-case life cycle that could cause variations in turnover time. The times were recorded for two different TKA tray configurations. Process A utilized instruments housed in vendor trays that were "blue" wrap sterilized, while Process B employed optimized trays that were sealed container-sterilized. Times were recorded during preoperative setup and postoperative clean down in the OR and CS. Reductions in mean OR or CS times were used to estimate cost savings. Wastes were analyzed for each method. Statistical analyses using Student t-tests were used to determine statistical differences and a p-value of less than 0.05 was considered significant.Overall, the use of optimized trays and sealed sterilization containers reduced the turnover time by 57 minutes and the number of trays by a mean of three. OR and CS processing yearly savings were estimated to be $249,245. Waste disposal was an estimated 10,590 ounces and 450 ounces for traditional and sealed containers, respectively.Novel sealed sterilization containers demonstrated increased efficiency in the total turnover time of TKA trays. This is important for surgeons participating in bundle payments who perform surgery in a hospital and ambulatory surgery center. Reduced turnover time could potentially increase case load and decrease the need for extra instrumentation or loaner trays. This simple means of increasing efficiency could be used as a model for surgeons wishing to streamline surgical trays and reduce costs.

Malposition of THA implants can lead to many complications, some of which may necessitate reoperation. Thus, proper implant placement is critical for optimizing patient outcomes. In addition, intraoperative visual estimation of stem position has been shown to be unreliable. Therefore, the purpose of this study was to compare a surgeon's visual estimation of femoral version to the actual version captured using a three-dimensional robotic-arm assisted platform.A prospective study of 25 THAs performed by a single surgeon was performed. The mean version, as estimated by intraoperative visual assessment, was compared to that measured by the robotic-arm assisted technology software using a two-sided t-test. Outliers were evaluated for the following intervals: 1 to 5°, 6 to 10°, and greater than 10°. A separate analysis was performed for anteverted versus retroverted stems.The mean version, as estimated by intraoperative visual assessment, was 9.16 ± 4.02° (range, 3 to 18°) compared to 3.52 ± 8.66° (range, -12 to 19) as measured by the robotic-arm assisted software (P=0.005). The surgeon's estimates of broach version and those measured by the robotic-arm assisted software were identical in three cases (12%). The evaluation methods differed by 1 to 5° in six cases (24%), 6 to 10° in 10 cases (40%), and greater than 10° in six cases (24%). Larger differences between methods were noted for cases in which the stem was found to be in anteversion by the robotic-arm assisted software.Visual estimation of femoral implant version differed significantly from measurements captured by three-dimensional robotic-arm assisted imaging. This suggests that estimating stem position intraoperatively by eye is not reliable, even when done by an experienced surgeon. The use of robotic-arm assisted technology may be recommended for determining femoral stem version intraoperatively.

Robotic-assisted surgery was introduced to make various mechanical aspects of a total hip arthroplasty more reproducible. When paired with sophisticated three-dimensional preoperative planning, robotic surgery offers the promise that a surgeon might select and reliably achieve targets for component position to optimize hip center-of-rotation, acetabular anteversion and inclination, femoral offset, as well as limb length. This paper describes a patient-specific step-by-step approach to performing these procedures including taking into account pelvic tilt. It is hoped that these described techniques will further optimize robotic-assisted hip arthroplasty procedures.

Revision total hip arthroplasties (rTHAs) are typically more complicated than primaries and may have a greater economic burden. To date, the current economic model supporting these procedures has not been evaluated. Therefore, the purpose of this study was to determine the 10-year temporal changes in charges, reimbursement rates, and net losses in patients undergoing an rTHA utilizing a large, nationwide database.A query was performed from 2005 to 2014 to identify patients who underwent rTHA. Analyzed outcomes included trends in costs of care, reimbursement rates, and net losses per annum within the last 10 years. Charges are defined as those by the provider to the insurance company, whereas, reimbursements were those payments received from the insurance company. Net losses were calculated as the difference in charges from reimbursement rates.Total charges increased from $1,119,725,881 in 2005 to $2,066,842,547 in 2014. Total reimbursements increased from $287,461,852 in 2005 to $478,430,569 in 2014. Per patient charges increased 67.4% from 2005 to 2014 and from $51,963.18 in 2005 to $86,791.07 in 2014. There was an increase in reimbursement per patient of 61.4% from $12,450.70 in 2005 to $20,090.31 in 2014. Net losses increased 68.80%, from $39,512.48 to $66,700.76 from 2005 to 2014.This study indicates substantial increases in charges and reimbursements over a 10-year period for patients undergoing rTHA. However, reimbursement increased at a lower rate than that of charges, resulting in major net losses. This study highlights the need to reevaluate the economic models behind rTHA for longer-term sustainment.

Background: There are many treatment options for patients who have osteonecrosis of the femoral head (ONFH) and management strategies vary widely both among and within individual countries. Although many researchers have attempted to elucidate the optimal strategies for managing this disease, the lack of large-scale randomized control trials and the lack of agreement on disease staging have curtailed the development of clear-cut guidelines.Methods: The Association Research Circulation Osseous (ARCO) group sought to address three questions for the management of patients who have ONFH: (1) What imaging studies are most sensitive and specific for the diagnostic evaluation of patients who have ONFH?; (2)What is the best treatment strategy for preventing disease progression in patients who have pre-collapse lesions?; and (3) What is the best treatment strategy for patients who have post-collapse disease? The PICO (Patient, Intervention, Comparison, and Outcome) format was used to formulate the search strategy for each research question. A systematic review will be performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. ARCO participants have been allocated to three groups, each representing one of the PICO questions. After qualitative and quantitative analysis of the data extracted from studies pertaining to each of the three research questions, a set of evidence-based clinical practice guidelines will be proposed for the management of patients who have ONFH. Discussion: It is not always clear which treatment method is optimal for the management of ONFH. Thus, many surgeons have developed and performed various procedures based on various patient-specific factors. As there is no consensus on the optimal treatment for various stages of disease, it was clear that developing evidence-based clinical practice guidelines would provide more structure and uniformity to management of these patients. Therefore, the results of this systematic review will lead to the development guidelines that may improve patient-care strategies and result in better outcomes for patients who have ONFH.Systematic Review Registration: Registration with PROSPERO has been submitted and is pending.

With constantly evolving materials and techniques used in wound closure management for total knee arthroplasty (TKA), it is imperative that we continuously evaluate new modalities and techniques to optimize healing. This article provides a concise review of the current closure and wound management methods for each of the following three layers and dressing: 1) deep fascia layer, or arthrotomy; 2) subdermal layer or subcutaneous layer; 3) skin and subcuticular layer; and 4) dressing application. By introducing a comprehensive and systematic approach to TKA wounds, this report also incorporates newer modalities such as barbed sutures and adhesive dressings, which have increasingly replaced traditional suture and staple methods. Furthermore, we examine how various layers of modern wound closure compare to conventional methods while discussing both the clinical and economic impacts of each. Newer wound management methods, such as the adhesive and occlusive dressings with varying monofilament subcuticular sutures, can eliminate the need for staple and suture removal, increase the value of care provided, limit unnecessary postoperative visits, and potentially address wound issues by communicating safely with patients via telemedicine, an ever-relevant discussion in the era of coronavirus disease 2019 (COVID-19).

Unicompartmental knee arthroplasty (UKA) is a feasible alternative to total knee arthroplasty (TKA) for selected patients with severe single-compartment knee osteoarthritis. Robotic-assisted UKA (rUKA) has recently emerged as a complementary tool to ameliorate previous difficulties with manual UKA (mUKA). However, the influence of rUKA compared to mUKA on patient outcomes are still largely unknown.To compare outcomes of the manual technique and a single robotic-assisted system in patients undergoing unicompartmental knee arthroplasty.The PubMed-Medline was searched using terms: "robotic," "unicompartmental," "knee," and "arthroplasty" to identify all studies comparing outcomes of mUKA and rUKA. Data pertaining to the following outcomes were extracted: (1) studies comparing robotic-assisted UKA to manual UKA; (2) reports which only included the Mako UKA system (Mako Surgical Corporation, Fort Lauderdale, Florida) as the robotic-assisted system; and (3) studies which discussed implant survivorship, complications, early postoperative parameters, functional outcomes, or implant positioning. Review articles were excluded.A total of eight publications with 337 patients who underwent rUKA and 481 who underwent mUKA were included in our analysis. Two studies reported that early postoperative pain was decreased with rUKA compared to mUKA. In one study, pain levels in the first eight postoperative weeks were 54% lower in the rUKA group and in the other, pain scores based on the numeric rating scale were significantly lower in rUKA (2.5) compared to mUKA (4.2) upon discharge (p0.001). Furthermore, mean time to hospital discharge in this study was reduced with rUKA (42.5 ± 5.9 hours) compared to mUKA (71.1 ± 14.6 hours) (p0.001). One study demonstrated a significant improvement in range of motion at two-year follow up in the rUKA group (15°, range, 5° to 25°) compared to the mUKA group (10°, range 0° to 20°; p=0.04). One study reported that at three-month follow up, mean Knee Society scores were significantly better in the rUKA group (164; interquartile range [IQR] 131 to 178) compared the mUKA group (143; IQR 132 to 166; p=0.04). Three studies reported more accurate implant positioning with rUKA compared to mUKA. Among the five studies that reported implant survivorship, four studies (including a randomized control trial) found no difference between techniques in implant survival rate, while one retrospective analysis reported improved survivorship with mUKA.In conclusion, unicompartmental knee arthroplasty implants demonstrated comparable survivorship rates whether performed manually or with robotic assistance. However, compared to the manually performed procedure, robotic-assisted unicompartmental knee arthroplasty were found to offer benefits including shorter lengths of hospital stays, decreased postoperative pain scores, and improved functional outcomes.

Background: Guidelines for return to driving after anterior cruciate ligament reconstruction (ACLR) have not been established. Purpose: To review the literature pertaining to driving after ACLR and provide evidence-based guidelines to aid clinicians in counseling patients about driving after ACLR. Study Design: Systematic review; Level of evidence, 4. Methods: A systematic review was performed according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Two independent reviewers searched PubMed, EMBASE, and the Cochrane Library using the terms anterior cruciate ligament, ACL, drive, and driving. Studies reporting on functional recovery after ACLR were included when data regarding return to driving were reported. Results: Five studies were included. Two studies included patients who underwent right-sided ACLR. Of these, 1 study evaluated bone-patellar tendon-bone autograft and reported that brake response time (BRT) returned to normal approximately 4 to 6 weeks postoperatively. The other study found that BRT returned to normal 3 weeks after allograft ACLR, but 6 weeks elapsed after autograft ACLR before values were not significantly different than controls. One study reported that patients who underwent left-sided hamstring tendon autograft ACLR demonstrated BRTs similar to controls within 2 weeks, while those with right-sided ACLR had significantly slower BRTs until 6 weeks postoperatively. Another study including patients who underwent either right- or left-sided ACLR and employed a manual transmission simulator found that 4 to 6 weeks should elapse after ACLR with hamstring tendon autograft. Survey data from 1 study demonstrated that the mean time for patients to resume driving was 13 and 10 days after right- and left-sided ACLR, respectively. Conclusion: BRT returned to normal values approximately 4 to 6 weeks after right-sided ACLR and approximately 2 to 3 weeks after left-sided ACLR. According to 1 study in this review, ACLR laterality should be disregarded for patients who drive manual transmission automobiles, as a 4- to 6-week time period was required for driving ability to reach the level of healthy controls. Future studies should aim to elucidate the influence of graft choice and transmission type on return to driving after ACLR.

Because of the early follow-up positive outcomes with cementless fixation, continued evaluations need to be performed to ensure longer-term efficacy. Additionally, although many studies report on the results of femoral and tibial component fixation, few studies report specifically on patellar outcomes. Therefore, the purpose of this study was to report on the: (1) implant survivorship; (2) complications; and (3) radiographic outcomes in a large cohort of patients who received cementless total knee arthroplasties (TKAs), with particular attention to the patellar component. A total of 261 patients who underwent cementless TKA by a single, high-volume academic surgeon were studied. Patients had a mean age of 66 years and were distributed between 192 women (74%) and 69 men. All patients received the same cementless tibial, femoral, and patellar components. Mean follow-up period was 4.5 years (range, 4–5 years). Primary outcomes evaluated included all postoperative complications, with particular emphasis on the patellar component. Only one patellar loosened leading to a patellar aseptic loosening rate of 0.3% (1 of 261). The one patellar loosening was the component being dislodged after a manipulation under anesthesia (MUA) at 6 weeks. This was revised to a cemented component and the patient is doing well 4 years later. A second patient experienced a patellar tendon rupture, later surgically repaired. Another patient sustained a patella fracture that was managed nonoperatively. The fracture healed by 1 year and the patient continued to have an otherwise successful outcome, now at 2 years follow-up. No progressive radiolucencies, subsidence, or changes in initial postoperative axial alignment were observed at final follow-up. The results from this study highlight a 98% success rate at mean 4.5 years follow-up in a large cohort of patients with a diverse spread of demographic details. Specific to the patella, only one patient experienced an adverse event, which was managed nonoperatively. Therefore, based on this data, patellar fixation in cementless TKA can be considered a safe technique.

Manual total knee arthroplasty (TKA) has successfully treated end-stage knee osteoarthritis for several years. However, recent technological advancements have enabled surgeons to perform TKA with more accuracy and precision. Aligning the femoral and tibial components perpendicular to the mechanical axes of the femur and tibia is a fundamental principle for restoring knee kinematics and soft-tissue balance. Computer-assisted robotic TKA has proven its ability to fine tune lower leg alignment, component position, and soft-tissue balancing. Furthermore, robotic-assisted TKA (RATKA) offers the additional benefit of improving soft-tissue protection compared to manual techniques. Numerous systems have been developed in the advancement of technology in computer processing, and the number of robotic surgical systems is increasing as well. The three main categories of navigation systems can be classified as: image-based console navigation, imageless console navigation, and accelerometer-based handheld navigation systems. The purpose of this review was to describe emerging technologies for TKA. Specifically, we outline the available literature pertaining to each system with regards to their: (1) accuracy and precision of component alignment; (2) soft-tissue protection; (3) postoperative outcomes; and (4) other reported outcomes such as costs.

Although the use of cementless implants in total knee arthroplasty (TKA) has increased in recent years, there is still ongoing debate regarding the optimal method of fixation. The purpose of this review was to evaluate the evidence regarding cementless versus cemented total knee arthroplasty (TKA) with regard to: (1) all-cause survivorship and aseptic survivorship; and (2) patient-reported outcome measures (PROMs) of newer generation TKAs.A systematic review of all reports on cementless TKA published from January 2010 to February 2019 was performed. A total of 221 articles were evaluated and 39 studies met inclusion criteria for final analysis. Metrics evaluated included all-cause survivorship, aseptic survivorship, and Knee Society Scores (KSS).Modern cementless TKA provides excellent survivorship and patient-reported outcomes as compared to cemented designs.Recent studies have demonstrated that newer generation cementless TKAs provide similar functional outcomes and survivorship as compared to cemented TKA. However, additional prospective, randomized trials with long-term follow up are necessary to further compare the outcomes of cementless versus cemented TKA.

Component position and overall limb alignment following total knee arthroplasty (TKA) have been shown to influence implant survivorships and clinical outcomes. While most surgeons utilize standard x-ray imaging for preoperative joint assessments, computer tomography scans (CT), coupled with automated digital analyses have been shown to provide additional surgical and clinical benefits. However, to date, a postoperative CT measurement protocol has not been reported for robotic-arm assisted TKA (RATKA). Therefore, the purpose of this paper was to assess the validity of a novel, vector-based CT alignment measurement protocol. Specifically, we compared: 1) final versus planned component alignment and placement; 2) inter-observer reliability; and 3) intra-observer reliability.The CT-based technique utilized mathematical models to calculate prosthetic alignments from anatomical landmarks. To assess the models, 30 CT scans from multiple centers were collected on RATKA patients at six weeks postoperatively and analyzed using the proposed technique. Results were compared to the surgeons' preoperative plans for accuracy. Analyses were performed on the same protocol to determine inter-observer reliability. These analyses were repeated 30 days later to assess for intra-observer variability.The mean measurement errors compared between final versus planned component positions and alignments were: 0.79±1.48o varus in overall limb alignment (p=0.004); 0.34±1.20o varus (p=0.121); and 0.35±1.15o varus (p=0.17) for femoral and tibial varus/valgus alignment; 0.71±1.77o flexion (p=0.18) and 0.38±1.88o posterior (p=0.41) for femoral flexion and tibial slope. There was strong reproducibility between observers. Correlation analyses showed low variabilities, with slopes between 0.8 to 1.0 and all R0.8.As robotic technologies become widely available in orthopaedic surgery, it is critical to have tools, such as CT protocols, which can quantitatively verify operative decisions concerning limb alignment and component placement. This study described a novel, vector-based, CT alignment measurement protocol for RATKA which has not previously been defined. The method demonstrated excellent accuracy to plan and low intra- and inter-observer variability. This is a valuable analysis tool for RATKA studies where component accuracy is assessed using postoperative CT images.

Periprosthetic joint infections (PJIs) are among the most dreadful complications associated with total joint arthroplasty (TJA). Despite the numerous precautionary measures employed by surgeons to avoid their occurrence, the incidence of PJIs after total knee arthroplasty (TKA) and total hip arthroplasty (THA) remains between 2.0% and 2.4% (1). Because the management of PJIs typically entails two-stage revision arthroplasty, it poses a substantial burden on patients, surgeons, hospitals, and healthcare in general. As the baby boomer generation continues to age, the incidence of TJA is increasing, and as a result, PJIs are expected to increase concordantly.

Background: Various methods exist for managing the joint capsule during the Latarjet procedure. Repairing the capsule to the native glenoid rim results in an extra-articular bone block, while repairing it to the remnant coracoacromial ligament stump of the coracoid graft renders it intra-articular. The technique that optimizes patient outcomes is not well defined. Purpose: To compare the outcomes of intra-articular and extra-articular bone block techniques for the Latarjet procedure. Study Design: Systematic review; Level of evidence, 4. Methods: Using PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses) guidelines, we queried the PubMed, EMBASE, and the Cochrane Library for all studies reporting outcomes of the Latarjet procedure with a clearly defined method of capsular repair that rendered the coracoid graft intra-articular or extra-articular. The included levels of evidence and degree of heterogeneity in this study precluded meta-analysis. Outcomes of interest included preoperative variables, surgical technique, rehabilitation protocols, functional outcome assessments, recurrent instability, range of motion, and radiographic findings. Results: A total of 16 studies including 816 patients were included. A total of 8 studies employed an intra-articular bone block in 338 patients, while the other 8 employed an extra-articular technique in 478 patients. There was variation among studies in reference to baseline patient characteristics, surgical techniques, rehabilitation, methods for assessing patient outcomes, and follow-up times. Rates of postoperative instability were reported in 8 intra-articular (0%, 0%, 2.1%, 2.7%, 3.2%, 5%, 5.4%, 5.9%) and 7 extra-articular (0%, 0%, 1.2%, 2%, 3.9%, 6.3%, 14%) bone block studies. Postoperative osteoarthritis or progression of preoperative osteoarthritis was reported in 5 intra-articular bone block studies (0%, 5.6%, 23.5%, 23.5%, 25%) and 4 extra-articular bone block studies (0%, 1.9%, 5.2%, 8.6%). Conclusion: Varying capsular repair methods appeared to provide similar outcomes regarding stability. There was an apparent trend toward higher rates of post-traumatic arthritis among studies in which an intra-articular bone block technique was employed; however, it is possible that this was influenced by substantially different follow-up times between groups and other various sources of heterogeneity among the included studies. There were no studies in the literature directly comparing intra-articular and extra-articular bone block techniques. Large-scale randomized controlled trials or comparative studies are needed to draw stronger conclusions comparing the 2 techniques.

Background:When meniscal repair is performed during anterior cruciate ligament (ACL) reconstruction (ACLR), the effect of ACL graft type on meniscal repair outcomes is unclear.Hypothesis:The authors hypothesized that meniscal repairs would fail at the lowest rate when concomitant ACLR was performed with bone--patellar tendon--bone (BTB) autograft.Study Design:Cohort study; Level of evidence, 3.Methods:Patients who underwent meniscal repair at primary ACLR were identified from a longitudinal, prospective cohort. Meniscal repair failures, defined as any subsequent surgical procedure addressing the meniscus, were identified. A logistic regression model was built to assess the association of graft type, patient-specific factors, baseline Marx activity rating score, and meniscal repair location (medial or lateral) with repair failure at 6-year follow-up.Results:A total of 646 patients were included. Grafts used included BTB autograft (55.7%), soft tissue autograft (33.9%), and various allografts (10.4%). We identified 101 patients (15.6%) with a documented meniscal repair failure. Failure occurred in 74 of 420 (17.6%) isolated medial meniscal repairs, 15 of 187 (8%) isolated lateral meniscal repairs, and 12 of 39 (30.7%) of combined medial and lateral meniscal repairs. Meniscal repair failure occurred in 13.9% of patients with BTB autografts, 17.4% of patients with soft tissue autografts, and 19.4% of patients with allografts. The odds of failure within 6 years of index surgery were increased more than 2-fold with allograft versus BTB autograft (odds ratio = 2.34 [95% confidence interval, 1.12-4.92]; P = .02). There was a trend toward increased meniscal repair failures with soft tissue versus BTB autografts (odds ratio = 1.41 [95% confidence interval, 0.87-2.30]; P = .17). The odds of failure were 68% higher with medial versus lateral repairs ( P < .001). There was a significant relationship between baseline Marx activity level and the risk of subsequent meniscal repair failure; patients with either very low (0-1 points) or very high (15-16 points) baseline activity levels were at the highest risk ( P = .004).Conclusion:Meniscal repair location (medial vs lateral) and baseline activity level were the main drivers of meniscal repair outcomes. Graft type was ranked third, demonstrating that meniscal repairs performed with allograft were 2.3 times more likely to fail compared with BTB autograft. There was no significant difference in failure rates between BTB versus soft tissue autografts.Registration:NCT00463099 (ClinicalTrials.gov identifier).

The perioperative management of lower extremity total joint arthroplasty (TJA) patients is constantly evolving. Several innovations have been developed to facilitate rapid recovery after these procedures. Some of the outcomes that are targeted for improvement include reducing perioperative complications, decreasing length of hospital stay, promoting early functional recovery, and improving patient satisfaction. Technology has been developed to address these objectives in the preoperative, intraoperative, and postoperative periods. The development of mobile health platforms and telehealth systems for preoperative education has shown to improve patient participation compared to in-person presurgical classes. Intraoperative innovations with robotic-assisted arthroplasty may effectively reduce postoperative pain and length of hospital stay. Postoperative technology with wearable technology and mobile applications has allowed closer monitoring of patient outcomes and functionality. These innovations may reduce cost of home care, physical therapy, and other services while maintaining or even improving patient outcomes. All of these new technologies for preoperative, intraoperative, and postoperative care will be reviewed.
