Objectives. To assess the reliability of data in electronic health records (EHRs) for measuring processes of care among primary care physicians (PCPs) and examine the relationship between these measures and clinical outcomes. Data Sources/Study Setting. EHR data from 15,370 patients with diabetes, 49,561 with hypertension, in a group practice serving four Northern California counties. Study Design/Methods. Exploratory factor analysis (EFA) and multilevel analyses of the relationships between processes of care variables and factor scales with control of hemoglobin Alc, blood pressure (BP), and low density lipoprotein (LDL) among patients with diabetes and BP among patients with hypertension. Principal Findings. Volume of e-messages, number of days to the third-next-available appointment, and team communication emerged as reliable factors of PCP processes of care in EFA (Cronbach's alpha = 0.73, 0.62, and 0.91). Volume of e-messages was associated with higher odds of LDL control (≤ 100) (OR = 1.13, p < .05) among patients with diabetes. Frequent in-person visits were associated with better BP (OR = 1.02, p < .01) and LDL control (OR = 1.01, p < .01) among patients with diabetes, and better BP control (OR = 1.04,< .01) among patients with hypertension. Conclusions. The EHR offers process of care measures which can augment patient-reported measures of patient-centeredness. Two of them are significantly associated with clinical outcomes. Future research should examine their association with additional outcomes. Key Words. In selected patients with systemic hypertension it may be difficult to ascertain whether left ventricular (LV) hypertrophy is a secondary end-organ consequence of long-term elevations in blood pressure or, alternatively, a manifestation of a coexistent primary hypertrophic cardiomyopathy. To address this issue and better characterize LV hypertrophy in systemic hypertension, 2-dimensional echocardiography was used to define the patterns of LV hypertrophy in 102 patients with sustained systemic hypertension and marked degrees of wall thickening. Patients ranged in age from 31 to 88 years (mean 61) and were predominantly female 58%); all were black. By selection, each patient had a maximal LV wall thickness of >l5 mm (range 16 to 29). Distribution of hypertrophy was judged to be symmetric (i.e., concentric) in most patients (67 of 102, 66%). However, a substantial proportion 3S patients, 34%) demonstrated nonuniform, asymmetric patterns of hypertrophy in which at least 1 segment of the LV wall was at least l.5 times the thickness of any other. in these 3S patients, the distribution of hypertrophy was similar to that characteristic of the morphologic spectrum of hypertrophic cardiomyopathy, with thickening of portions of both the ventricular septum and free wall in 16 patients, anterior and posterior ventricular septum alone in 11 patients and segmental involvement of only the anterior ventricular septum in S. Patients with asymmetric patterns of waR thickening did not differ from the patients with symmetric hypertrophy with regard to age, sex or clinical findings. Asymmetric LV hypertrophy appears to represent an important feature of the morphologic spectrum of severe hypertensive heart disease. Moreover, the diverse patterns of hypertrophy observed in hypertensive patients with marked LV wall thickening often cannot be distinguished definitively, on a morphologic basis alone, from those characteristic of hypertrophic cardiomyopathy. (Am J Cardiol 1990;6S:874-881), Enlargement (hypertrophy) of the left ventricle (LV, the chamber of the heart that pumps blood to the body) is frequently associated with chronic high blood pressure (hypertension). It can be either symmetric, with all parts of the LV wall enlarged equally, or asymmetric, where some parts are thicker than others. Asymmetric LV hypertrophy has been associated with hypertrophic cardiomyopathy, a condition of ventricular enlargement due to factors other than hypertension. When hypertensive patients exhibit extensive, asymmetric thickening of the ventricular wall, diagnosis of the primary cause is difficult. To better describe the patterns of LV wall thickening, echocardiography (ultrasound) studies were performed on a large (102), black, mostly female (58 percent) population of patients. Their average age was 61 years and all had had blood pressure greater than 140/90 for more than one year and marked LV thickening (more than 15 mm) without previous heart attack or heart disease. Results showed that 67 of the patients had symmetric LV wall thickening, while the remaining 35 had asymmetric hypertrophy. In many of these, the ventricular septum (the tissue wall between the two ventricles) and LV free wall, were thickest, and the ventricular morphology for these 35, in general, conformed to the pattern associated with hypertrophic cardiomyopathy. These patients did not differ from those with symmetric LV hypertrophy with regard to sex, age, blood pressure, or other variables measured. Thus, it appears that firm conclusions about the etiology of hypertensive heart disease cannot be drawn from observations about the symmetry or asymmetry of LV enlargement, and asymmetric LV hypertrophy is probably an important marker for severe hypertensive heart disease.
