32. Stochastic Analysis Of Storm-Surge Induced Infrastructure Losses In New York City

Abstract

Hurricanes are among the most catastrophic types of natural hazards, with the potential to cause serious losses in lives and property. While hurricanes rarely have a huge impact on the New York City area, they do have the potential to cause major damage to the city's transportation infrastructure. This research will deal with two main considerations--fragility curves and exceedance curves of vulnerable points in that infrastructure. The primary objective of this study is to provide a model for predicting future hurricane related storm surge patterns and for estimating possible levels of damage from future events in order to develop planning strategies to mitigate against possible damage. The first step is to describe the frequency of past storm surge events in New York City from 1920 to 2012 and determine a probability distribution for hurricane hazard about the maximum daily and yearly storm surges. The second step is to estimate potential probabilistic models by looking at the empirical data on storm surges in New York City. The last step is to concentrate on the reliability assessment for several infrastructures subjected to hurricane loading and storm surges. No significant studies have been conducted using the available empirical data on storm surge heights in New York City, despite the fact that since an observation station was installed in the Battery, New York in 1920, daily and yearly maximum water levels at that location have been documented by the National Oceanic and Atmospheric Administration (NOAA). Considering the available daily maximum sea water levels from 1920 to 2012 yields a total of 31,148 data points (2,394 days of maximum height data are unfortunately missing); 92 data points of maximum sea water levels are also available. This is the first study to utilize the nearly century's worth of empirical data obtained by the observation station at the Battery. Extensive goodness of fit testing (including the use of various probability papers) is performed on the empirical daily maximum sea water level data. It is concluded that the daily maximum sea water levels at the Battery from 1920 to 2012 follow closely a logistic distribution, with a mean value of 8.10 feet and a coefficient of variation (COV) of 9.63%. The methodology of analyzing the yearly maximum sea water levels is quite similar to that used for the daily sea water levels (and the analysis is performed independently). It is found that the yearly maximum sea water levels at the Battery from 1920 to 2012 follow closely a generalized extreme value (GEV) distribution with a mean value of 10.72 feet and a COV of 10.07%. Then, applying exact and asymptotic Extreme Value Theory, the parent GEV distribution is used to determine the probability distributions for maximum sea water levels over a range of different multi-year periods including 1, 10, 50, 100, 200, and 500 years. Finally, the total volume of flood-vulnerable infrastructure is generated and flood damage probabilities when related to the established probability distributions for sea water levels are considered. The flood vulnerabilities of different parts of the built infrastructure in New York City are studied; specifically, the subway system and the tunnel system. The concept of fragility curves is used to express these vulnerabilities. Conclusions and recommendations are provided for estimating losses probabilistically over different periods, retrofitting and strengthening the infrastructure to reduce future potential losses, and determining repair priorities. This is very useful for cost-benefit analysis.

Abstract

Background Iodine excess has been suggested as an exogenous risk factor of papillary thyroid cancer (PTC). We performed a systematic review and meta-analysis to assess the relationship between iodine exposure measured in various forms and PTC prevalence. Methods We searched MEDLINE, Embase, and the Cochrane Library for case-control studies on iodine and PTC published up to December 2015. Exposure to iodine was compared between PTC and control groups. Results From the 16 selected studies, the odds ratio (OR) for the overall effect size between high iodine exposure and PTC risk was 1.418 (95% confidence interval [CI] 1.054-1.909). Based on 7 studies conducted in high iodinated regions, a positive association between iodine exposure and PTC was observed (OR 2.200; 95% CI 1.389-3.483). Conclusion This study demonstrated a higher exposure to iodine in patients with PTC compared with controls, especially for patients from high iodinated regions. [ABSTRACT FROM AUTHOR]

39. Medication Adherence and the Risk of Cardiovascular Mortality and Hospitalization Among Patients With Newly Prescribed Antihypertensive Medications.

Abstract

The importance of adherence to antihypertensive treatments for the prevention of cardiovascular disease has not been well elucidated. This study evaluated the effect of antihypertensive medication adherence on specific cardiovascular disease mortality (ischemic heart disease [IHD], cerebral hemorrhage, and cerebral infarction). Our study used data from a 3% sample cohort that was randomly extracted from enrollees of Korean National Health Insurance. Study subjects were aged ≥20 years, were diagnosed with hypertension, and started newly prescribed antihypertensive medication in 2003 to 2004. Adherence to antihypertensive medication was estimated as the cumulative medication adherence. Subjects were divided into good (cumulative medication adherence, ≥80%), intermediate (cumulative medication adherence, 50%-80%), and poor (cumulative medication adherence, <50%) adherence groups. We used time-dependent Cox proportional hazards models to evaluate the association between medication adherence and health outcomes. Among 33 728 eligible subjects, 670 (1.99%) died of coronary heart disease or stroke during follow-up. Patients with poor medication adherence had worse mortality from IHD (hazard ratio, 1.64; 95% confidence interval, 1.16-2.31; P for trend=0.005), cerebral hemorrhage (hazard ratio, 2.19; 95% confidence interval, 1.28-3.77; P for trend=0.004), and cerebral infarction (hazard ratio, 1.92; 95% confidence interval, 1.25-2.96; P for trend=0.003) than those with good adherence. The estimated hazard ratios of hospitalization for cardiovascular disease were consistent with the mortality end point. Poor medication adherence was associated with higher mortality and a greater risk of hospitalization for specific cardiovascular diseases, emphasizing the importance of a monitoring system and strategies to improve medication adherence in clinical practice. Abstract

Introduction: Thyroid cancer incidence in Korea is the highest in the world and has recently increased steeply. However, factors contributing to this sudden increase have not been fully elucidated, and few studies have explored the postoperative prognosis. The Thyroid Cancer Longitudinal Study (T-CALOS) was initiated with three aims: (1) to identify factors predicting quality of life, recurrence, and incidence of other diseases after thyroid cancer treatments; (2) to investigate environmental exposure to radiation, toxicants and molecular factors in relation to tumour aggressiveness; and (3) to evaluate gene-environment interactions that increase thyroid cancer in comparison with healthy participants from a pool of nationwide population-based healthy examinees., Methods and Analysis: T-CALOS enrols patients with incident thyroid cancer from three general hospitals, Seoul National University Hospital, Seoul National University Bundang Hospital and National Medical Center, Korea. The study is an ongoing project expecting to investigate 5000 patients with thyroid cancer up until 2017. Healthy examinees with a normal thyroid confirmed by sonography have been enrolled at the Healthy Examination Center at Seoul National University Hospital. We are also performing individual matching using two nationwide databases that are open to the public. Follow-up information is obtained at patients' clinical visits and by linkage to the national database. For statistical analysis, we will use conditional logistic regression models and a Cox proportional hazard regression model. A number of stratifications and sensitivity analyses will be performed to confirm the results., Ethics and Dissemination: Based on a large sample size, a prospective study design, comprehensive data collection and biobank, T-CALOS has been independently peer-reviewed and approved by the three hospitals and two funding sources (National Research Foundation of Korea and Korean Foundation for Cancer Research). Abstract

Background: Cellular phones enable communication between healthcare providers and patients for prevention, diagnosis, and treatment of diseases. However, few studies have examined the user-friendliness or effectiveness of cellular phone-based medical informatics (CPBMI) for healthcare., Materials and Methods: This study investigated the use of CPBMI to identify its current status within the medical field, advantages and disadvantages, practicability, clinical effectiveness, costs, and cost-saving potential., Results: CPBMI was validated in terms of practicality and provision of medical benefits. It is critical to use CPBMI in accordance with the different features of each disease and condition. Use of CPBMI is expected to be especially useful for patients with chronic disease., Conclusions: We discussed the current status of the clinical use, benefits, and risks of CPBMI. CPBMI and information technology-based health management tools are anticipated to become useful and effective components of healthcare management in the future.
