Background: Reverse total shoulder arthroplasty was originally designed for older patients with rotator cuff arthropathy and produces good results. The main objective of this retrospective study was to compare the patients younger than 65 years vs. the older patients in terms of the complications of reverse total shoulder arthroplasty and the functional recovery., Methods: From January 2014 to January 2020, 566 patients who underwent the reverse total shoulder arthroplasty were divided into two groups (group A, ≥ 65 years, n = 506; group B, < 65 years, n = 60). The patients reported the quality of life using the patient-reported Western Ontario Osteoarthritis of the Shoulder index. The Constant score was obtained preoperatively and 3 months postoperatively. The complications and reoperations were compared. Statistical significance was set at P < 0.05., Results: Clinically relevant improvements were found in group A and B. There was a multivariate statistically-significant but not clinically relevant difference in the change over time between group A and B. The mean 12-month Western Ontario Osteoarthritis of the Shoulder indexes were 58 in group B and 71 in group A. The mean Constant scores were 44 in group B vs. 43 in group A. Compared to group A, group B had a non-significant odds ratio of 1.9, which did not reach the clinically relevant Western Ontario Osteoarthritis of the Shoulder index of group A., Conclusion: In patients younger than 65 years of age, RTSA seems to be a safe procedure in short term follow-up. After 1 year, we found no increased risk of complications, revision, or inferior outcomes compared to patients older than 65 years of age. Background: There is an important gap in knowledge about the effectiveness of nonoperative treatment (exercise) for patients with traumatic primary and recurrent anterior shoulder dislocations (ASDs)., Purpose/hypothesis: The purpose of this study was to assess the efficacy and safety of physical therapist-supervised, shoulder instability neuromuscular exercise (SINEX) versus self-managed, home-based, standard care shoulder exercise (HOMEX) in patients with traumatic ASDs. The hypothesis was that SINEX would have a larger effect and fewer adverse events compared with HOMEX., Study Design: Randomized controlled trial; Level of evidence, 2., Methods: A total of 56 participants with radiographically verified, trauma-initiated primary or recurrent ASDs and self-reported decreased shoulder function were randomized to 12 weeks of either SINEX or HOMEX. The SINEX program consisted of 7 exercises, individually progressing from basic (2 × 20 repetitions each day) to elite (2 × 10 repetitions, 3 times weekly). The HOMEX program included 5 shoulder exercises performed 3 times weekly (2 × 10 repetitions). The primary outcome was the Western Ontario Shoulder Instability Index (WOSI) score, ranging from 0 (best possible) to 2100. The between-group minimal clinically important difference at 12 weeks was 250 points. Secondary outcomes included WOSI subdomain scores, patient-reported ratings of kinesiophobia and pain, objective shoulder function, patient satisfaction, and number of adverse events., Results: The between-group mean difference in the WOSI total score at 12 weeks significantly favored SINEX over HOMEX (-228.1 [95% CI, -430.5 to -25.6]). SINEX was furthermore superior to HOMEX in most of the secondary outcomes (3/4 subdomains of the WOSI and pain level during the past 7 days as well as clinical signs of anterior shoulder instability). Also, although not statistically significant, less than half the proportion of the SINEX patients compared with the HOMEX patients (3/27 [11%] vs 6/24 [25%], respectively; P = .204) underwent or were referred for shoulder stabilizing surgery. Satisfaction with both exercise programs was high, and no serious adverse events were reported., Conclusion: Neuromuscular shoulder exercise (SINEX) was superior to standard care exercise (HOMEX) in patients with traumatic ASDs. Background: Anterior shoulder dislocation is a common injury and may have considerable impact on shoulder-related quality of life (QoL). If not warranted for initial stabilising surgery, patients are mostly left with little to no post-traumatic rehabilitation. This may be due to lack of evidence-based exercise programmes. In similar, high-impact injuries (e.g. anterior cruciate ligament tears in the knee) neuromuscular exercise has shown large success in improving physical function and QoL. Thus, the objective of this trial is to compare a nonoperative neuromuscular exercise shoulder programme with standard care in patients with traumatic anterior shoulder dislocations (TASD)., Methods/design: Randomised, assessor-blinded, controlled, multicentre trial. Eighty patients with a TASD will be recruited from three orthopaedic departments in Denmark. Patients with primary or recurrent anterior shoulder dislocations due to at least one traumatic event will be randomised to 12 weeks of either a standardised, individualised or physiotherapist-supervised neuromuscular shoulder exercise programme or standard care (self-managed shoulder exercise programme). Patients will be stratified according to injury status (primary or recurrent). Primary outcome will be change from baseline to 12 weeks in the patient-reported QoL outcome questionnaire, the Western Ontario Shoulder Instability Index (WOSI)., Discussion: This trial will be the first study to compare the efficacy and safety of two different nonoperative exercise treatment strategies for patients with TASD. Moreover, this is also the first study to investigate nonoperative treatment effects in patients with recurrent shoulder dislocations. Introduction: On 3 December, 1999, at 15.00 pm, hurricane Adam struck Esbjerg (Denmark). Wind velocity exceeded 33 m/s, the strongest hurricane ever recorded in Denmark during the last century. This article describes and analyses the orthopaedic impact of the hurricane on a local hospital in Esbjerg., Materials and Methods: We examined retrospectively all the patients treated at the Emergency Department from 3 pm on 3 December and for the following 24 hours. For comparison, we used the records of all patients treated at the Emergency Department in the same time interval on the first three Fridays in December from 1994 to 1998., Results: No hurricane-associated deaths were recorded, but the number of patients treated on 3 December was significantly higher than that recorded in the period 1994-1998 (159 versus 74, p < 0.001). The main diagnosis was similar to that of the previous years, but more people sought treatment at the Emergency Department. Sixty-eight percent of the recorded injuries were injuries to the upper and lower extremities and 22% to the head and neck., Discussion: Hurricanes can lead to substantial morbidity and mortality. Early warning is the most effective way of reducing death and injury. People should seek cover and follow the instructions given by the news media. Educational programmes for the medical staff of the Emergency Department should be made available.

Popliteal cysts or Baker's cysts in childhood is a rare disease usually found by the parents. The purpose of this study is to study the recurrence rate after primary surgical resection and conservative treatment of the cysts. We retrospectively found 20 children with Baker's cysts treated at Ribe Amts Hospitals during the period 1992-1998. Thirteen patients were treated conservatively and seven had a surgical resection of the cysts performed. Of the 13 conservatively treated patients the cysts was still present in six patients (46%). In seven patients the cysts had disappeared within nine months. In the surgically treated group the cysts recurred in three patients (43%). Surgical intervention for Baker's cysts in children should only be performed after thorough consideration, due to a high recurrence rate.

During six-month periods in 1981, 1986 and 1991 a prospective registration was made of all injuries caused by violence treated in the casualty ward of Esbjerg Hospital. The numbers of such contacts during these periods were 236 in 1981, 379 in 1986 and 509 in 1991. There were no differences between the three periods with respect to the following parameters: age, sex, civil status, place, types of lesions, need for further treatment and influence of alcohol. The total incidence rate rose between 1986 and 1991 from eight to nine per 1000 (not significant). The only significant increase was among females aged 15-19, where the incidence rose from 2.0 per 1000 in 1986 to 11.3 in 1991. Fourteen percent of the injured required hospitalization, occupying 1216 hospital bed days during the 1991 period.

The results of 42 patients with displaced proximal humeral fractures treated by conservative methods were reviewed. Follow-up evaluation included clinical and radiographic examination median two years after the trauma. The results were assessed using Neer's protocol. The patients were asked to assess the results. The patients assessed the results better than the Neer score (p < 0.001). The patients' response to non-operative treatment was satisfactory. It is concluded that the Neer classification of the results underestimates the degree of satisfaction achieved by the patients. It is concluded that displaced proximal humeral fractures can be treated satisfactorily by non-operative methods.

This randomized, double-blind, multi-centre study was undertaken to evaluate the efficacy and safety of treatment for 4 weeks with codeine plus paracetamol versus paracetamol in relieving chronic pain due to osteoarthritis of the hip. A total of 158 outclinic patients entered the study. Eighty-three patients (mean age 66 years) were treated with codeine 60 mg plus paracetamol 1 g 3 times daily, and 75 patients (mean age 67 years) with paracetamol 1 g 3 times daily. Ibuprofen 400 mg was prescribed as rescue medication. Due to an unexpected high rate of adverse drug reactions, the study was closed before the planned 400 patients had entered. Over weeks 1-4, 87%, 64%, 61% and 52% of patients in the codeine plus paracetamol group, and 38%, 31%, 22% and 29% of patients in the paracetamol group had one or more adverse drug reactions. Significantly more patients in the codeine plus paracetamol group had adverse drug reactions in each of the 4 weeks. Nausea, dizziness, vomiting and constipation were predominant adverse reactions in the codeine plus paracetamol group. During the first week of treatment, 30 patients (36%) in the codeine plus paracetamol group and 9 (12%) in the paracetamol group dropped out. As evaluated from patients completing the first week of treatment, the pain intensity during that week compared to their baseline pain was significantly lower in the codeine plus paracetamol group than in the paracetamol group. Moreover, during the first week the paracetamol group received rescue medicine significantly more frequently. In conclusion, when evaluated after 7 days of treatment, the daily addition of codeine 180 mg to paracetamol 3 g significantly reduced the intensity of chronic pain due to osteoarthritis of the hip joint. However, several adverse drug reactions, mainly of the gastrointestinal tract, and the larger number of patients withdrawing from treatment means that the addition of such doses of codeine cannot be recommended for longer-term treatment of chronic pain in elderly patients.

The value of a compression bandage applied for 4 days following ankle sprains was assessed in a comparative study. The effect of the treatment was evaluated by changes in an inflammatory score based on a combination of subjective (pain, function) and objective (swelling, limitation of movement, pain on passive movement) factors. The study included two consecutive series each of 50 patients treated with a compression bandage, and untreated. There was no significant difference in the reduction of the inflammatory score from start to the fourth day (0.5 less than p less than 0.6) or from start to the eighth day (0.1 less than p less than 0.2) between the group treated with compression bandage and the untreated group. Analysis of the individual factors included in the inflammatory score demonstrated no difference in the course of pain, function, swelling, or limitation of movement between the two groups.

In a prospective study, 150 patients with lesions of lateral ankle ligaments were treated with early mobilization without any fixation of the ankle. After 8 days, 67% were free from pain on ordinary walking and 81% had resumed work. After 1 month 90% were free from pain and 97% had resumed work. Sport was resumed by 70% of athletes after 1 month and 90% after 3 months. At the 1-year follow-up (n = 137), 18% were not fully recovered and experienced pain (14%) or functional instability (7%) or had not resumed sport at their normal level (7%). However, only 8% found the condition inconvenient. Athletes had an increased risk of residual symptoms (p less than 0.01) and residual symptoms occurred in 32% of top athletes after 1 year.

In a prospective, randomized trial, 104 consecutive patients with displaced femoral neck fractures were allocated either to fixation with a sliding screw plate or 4 ASIF cancellous bone screws. The patients were reexamined at fixed intervals to determine the time of union. The 2-year-cumulated rate of union was 64 per cent in the plate group and 84 per cent in the screw group.

We operated on a consecutive series of 30 closed Achilles tendon ruptures under local anesthesia and without the use of a tourniquet. We had no reruptures or major complications, and the results were comparable to other series of operatively treated achilles tendon ruptures.
