The integration of satellite and terrestrial networks has become inevitable in the next generations of communications networks due to emerging needs of ubiquitous connectivity of remote locations. New and existing services and critical infrastructures in remote locations in sea, on land and in space will be seamlessly connected through a diverse set of terrestrial and non-terrestrial communication technologies. However, the integration of terrestrial and non-terrestrial systems will open up both systems to unique security challenges that can arise due to the migration of security challenges from one to another. Similarly, security challenges can also arise due to the incompatibility of distinct systems or incoherence of security policies. The resulting security implications, thus, can be highly consequential due to the criticality of the infrastructures such as space stations, autonomous ships, and airplanes, for instance. Therefore, in this article we study existing security challenges in satellite-terrestrial communication systems and discuss potential solutions for those challenges. Furthermore, we provide important research directions to encourage future research on existing security gaps.

Background: Preterm birth is defined as live birth before 37 completed weeks of pregnancy, and it is a major problem worldwide. The molecular mechanisms that lead to onset of spontaneous preterm birth are incompletely understood. Prediction and evaluation of the risk of preterm birth is challenging as there is a lack of accurate biomarkers. In this study, our aim was to identify placental proteins that associate with spontaneous preterm birth. Methods: We analyzed the proteomes from placentas to identify proteins that associate with both gestational age and spontaneous labor. Next, rare and potentially damaging gene variants of the identified protein candidates were sought for from our whole exome sequencing data. Further experiments we performed on placental samples and placenta-associated cells to explore the location and function of the spontaneous preterm labor-associated proteins in placentas. Results: Exome sequencing data revealed rare damaging variants in SERPINA1 in families with recurrent spontaneous preterm deliveries. Protein and mRNA levels of alpha-1 antitrypsin/SERPINA1 from the maternal side of the placenta were downregulated in spontaneous preterm births. Alpha-1 antitrypsin was expressed by villous trophoblasts in the placenta, and immunoelectron microscopy showed localization in decidual fibrinoid deposits in association with specific extracellular proteins. siRNA knockdown in trophoblast-derived HTR8/SVneo cells revealed that SERPINA1 had a marked effect on regulation of the actin cytoskeleton pathway, Slit–Robo signaling, and extracellular matrix organization. Conclusions: Alpha-1 antitrypsin is a protease inhibitor. We propose that loss of the protease inhibition effects of alpha-1 antitrypsin renders structures critical to maintaining pregnancy susceptible to proteases and inflammatory activation. This may lead to spontaneous premature birth.

Remote health care is currently one of the most promising solutions to ensure a high level of treatment outcome, cost-efficiency and sustainability of the healthcare systems worldwide. Even though research on remote health care can be traced back to the early days of the Internet, the recent COVID-19 has necessitated further improvement in existing health care systems with invigorated research on remote health care technologies. In this article we delve into the state-of-the-art research in latest technologies and technological paradigms that play a vital role in enabling the next generation remote health care and assisted living. First the need of using the latest technological developments in the domain of remote health care is briefly discussed. Then the most important technologies and technological paradigms that are crucial in enabling remote health care and assisted living are emphasised. Henceforth, a detailed survey of existing technologies, potential challenges in those technologies, and possible solutions is conducted. Finally, missing research gaps and important future research directions in each enabling technology are brought forth to motivate further research in remote health care.

Circulating immature surfactant protein B (proSP-B) forms emerged as the most reliable lung-specific circulating marker for alveolar-capillary membrane (ACM) dysfunction and for the overall clinical status of heart failure (HF). Notably, in terms of HF hospitalization, immature SP-B overwhelms the prognostic role of other most frequently used clinical parameters such as those related to lung dysfunction. The strong prognostic value of circulating proSP-B in HF suggests more widespread and possible systemic effects. Thus, we assessed the plasma distribution of proSP-B evaluating whether it exists in a lipoprotein-bound form and its impact on lipoprotein structure and function. ProSP-B forms were detectable in high-density lipoprotein (HDL) only. To assess the impact of proSP-B on HDL, HDL from healthy subjects were enriched with proSP-B produced by a stably transfected CHO cell line that specifically expresses and releases the human proSP-B. After enrichment, HDL size and lipoprotein electrophoretic mobility, and protein composition did not show apparent differences. HDL antioxidant capacity (HOI), assessed as their ability to inhibit air-induced LDL oxidation, was impaired after proSP-B enrichment. HOI was also higher in HF patients with respect to age-matched control healthy subjects (p = 0.013). Circulating proSP-B, besides its potential role as a specific marker for ACM dysfunction in HF patients with diagnostic and prognostic value, binds to human HDL impairing their antioxidant capacity. These findings shed light on proSP-B as a molecule that contributes to the reduction of the defense against oxidative stress, a key mediator in the pathogenesis of HF.

5G enables the use of different types of services over the same physical infrastructure through the concepts and technologies of virtualization, softwarization, network slicing and cloud computing. Mobile Virtual Network Operators (MVNOs), using these concepts, provide an opportunity to share the same physical infrastructure among multiple operators. Each MVNO can have own distinct operating and support systems. However, the technologies used to enable such an environment have their own explicit security challenges and solutions. The integrated environment built upon these novel concepts and technologies, thus, will have complex security implications and requirements to be satisfied. In this vain, this article provides an overview of the security challenges and potential solutions for MVNOs.

Spontaneous preterm birth (SPTB) is the leading cause of neonatal death and morbidity worldwide. Both maternal and fetal genetic factors likely contribute to SPTB. We performed a genome-wide association study (GWAS) on a population of Finnish origin that included 247 infants with SPTB (gestational age [GA] < 36 weeks) and 419 term controls (GA 38–41 weeks). The strongest signal came within the gene encoding slit guidance ligand 2 (SLIT2; rs116461311, minor allele frequency 0.05, p = 1.6×10−6). Pathway analysis revealed the top-ranking pathway was axon guidance, which includes SLIT2. In 172 very preterm-born infants (GA <32 weeks), rs116461311 was clearly overrepresented (odds ratio 4.06, p = 1.55×10−7). SLIT2 variants were associated with SPTB in another European population that comprised 260 very preterm infants and 9,630 controls. To gain functional insight, we used immunohistochemistry to visualize SLIT2 and its receptor ROBO1 in placentas from spontaneous preterm and term births. Both SLIT2 and ROBO1 were located in villous and decidual trophoblasts of embryonic origin. Based on qRT-PCR, the mRNA levels of SLIT2 and ROBO1 were higher in the basal plate of SPTB placentas compared to those from term or elective preterm deliveries. In addition, in spontaneous term and preterm births, placental SLIT2 expression was correlated with variations in fetal growth. Knockdown of ROBO1 in trophoblast-derived HTR8/SVneo cells by siRNA indicated that it regulate expression of several pregnancy-specific beta-1-glycoprotein (PSG) genes and genes involved in inflammation. Our results show that the fetal SLIT2 variant and both SLIT2 and ROBO1 expression in placenta and trophoblast cells may be correlated with susceptibility to SPTB. SLIT2-ROBO1 signaling was linked with regulation of genes involved in inflammation, PSG genes, decidualization and fetal growth. We propose that this receptor-ligand couple is a component of the signaling network that promotes SPTB.

This paper presents a roadmap for the transition from current gadget-centric digital services towards a gadget-free services environment called the Naked world. The main idea of the Naked world is that all the services which are currently provided by gadgets will be provided by the infrastructure, thus no gadgets will be needed to use any kind of digital services. When a user in the Naked world intends to use a service, the infrastructure senses the user, the nearby intelligent surrounding launches an interactive user interface, performs identification through biometric identities, provides the service, and then closes the session when the user finishes the job. Therefore, the Naked world comprises highly intelligent and context-aware interactive environments. The vision of the Naked world is an evolution towards a user-friendly and ubiquitously available digital services, which is naturally bounded by the technological advancement. Henceforth, this paper presents the essential technologies and functional requirements along with the current and forthcoming novel technological concepts and challenges for the realization of the Naked world.

Aims: Low-density lipoprotein (LDL) particles cause atherosclerotic cardiovascular disease (ASCVD) through their retention, modification, and accumulation within the arterial intima. High plasma concentrations of LDL drive this disease, but LDL quality may also contribute. Here, we focused on the intrinsic propensity of LDL to aggregate upon modification. We examined whether inter-individual differences in this quality are linked with LDL lipid composition and coronary artery disease (CAD) death, and basic mechanisms for plaque growth and destabilization. Methods and results: We developed a novel, reproducible method to assess the susceptibility of LDL particles to aggregate during lipolysis induced ex vivo by human recombinant secretory sphingomyelinase. Among patients with an established CAD, we found that the presence of aggregation-prone LDL was predictive of future cardiovascular deaths, independently of conventional risk factors. Aggregation-prone LDL contained more sphingolipids and less phosphatidylcholines than did aggregation-resistant LDL. Three interventions in animal models to rationally alter LDL composition lowered its susceptibility to aggregate and slowed atherosclerosis. Similar compositional changes induced in humans by PCSK9 inhibition or healthy diet also lowered LDL aggregation susceptibility. Aggregated LDL in vitro activated macrophages and T cells, two key cell types involved in plaque progression and rupture. Conclusion: Our results identify the susceptibility of LDL to aggregate as a novel measurable and modifiable factor in the progression of human ASCVD.

We used large data sets that included the gestational duration to determine possible genetic associations. Methods: We performed a genomewide association study in a discovery set of samples obtained from 43,568 women of European ancestry using gestational duration as a continuous trait and term or preterm (<37 weeks) birth as a dichotomous outcome. We used samples from three Nordic data sets (involving a total of 8643 women) to test for replication of genomic loci that had significant genomewide association (P<5.0×10⁻⁸) or an association with suggestive significance (P<1.0×10⁻⁶) in the discovery set. Results: In the discovery and replication data sets, four loci (EBF1, EEFSEC, AGTR2, and WNT4) were significantly associated with gestational duration. Functional analysis showed that an implicated variant in WNT4 alters the binding of the estrogen receptor. The association between variants in ADCY5 and RAP2C and gestational duration had suggestive significance in the discovery set and significant evidence of association in the replication sets; these variants also showed genomewide significance in a joint analysis. Common variants in EBF1, EEFSEC, and AGTR2 showed association with preterm birth with genomewide significance. An analysis of mother–infant dyads suggested that these variants act at the level of the maternal genome. Conclusions: In this genomewide association study, we found that variants at the EBF1, EEFSEC, AGTR2, WNT4, ADCY5, and RAP2C loci were associated with gestational duration and variants at the EBF1, EEFSEC, and AGTR2 loci with preterm birth. Bronchopulmonary dysplasia (BPD), the main consequence of prematurity, has a significant heritability, but little is known about predisposing genes. The aim of this study was to identify gene loci predisposing infants to BPD. The initial genome-wide association study (GWAS) included 174 Finnish preterm infants of gestational age 24–30 weeks. Thereafter, the most promising single-nucleotide polymorphisms (SNPs) associated with BPD were genotyped in both Finnish (n = 555) and non-Finnish (n = 388) replication cohorts. Finally, plasma CRP levels from the first week of life and the risk of BPD were assessed. SNP rs11265269, flanking the CRP gene, showed the strongest signal in GWAS (odds ratio [OR] 3.2, p = 3.4 × 10⁻⁶). This association was nominally replicated in Finnish and French African populations. A number of other SNPs in the CRP region, including rs3093059, had nominal associations with BPD. During the first week of life the elevated plasma levels of CRP predicted the risk of BPD (OR 3.4, p = 2.9 × 10⁻⁴) and the SNP rs3093059 associated nominally with plasma CRP levels. Finally, SNP rs11265269 was identified as a risk factor of BPD (OR 1.8, p = 5.3 × 10⁻⁵), independently of the robust antenatal risk factors. As such, in BPD, a potential role for variants near CRP gene is proposed.

This thesis seeks to develop a robust semi-analytical performance prediction method for an advanced iterative receiver that processes spatially multiplexed signals that have propagated through frequency-selective receive correlated multiple-input multiple-output (MIMO) wireless communication channels. In a change of perspective, the proposed performance prediction methods are applied at the transmitter, which seeks to attain a target frame error rate (FER) either by adaptive power control or by adaptive modulation and coding (AMC). The performance prediction scheme utilises the statistical properties of the channel—namely noise variance, number of separable propagation paths and the eigenvalues of the receive correlation matrix—to predict the signal-to-interference-plus-noise ratio (SINR) at the output of a frequency domain soft interference cancellation minimum mean square error equaliser. The SINR distribution is used to derive the distribution of the variance of the log-likelihood ratios (LLRs) at the output of a soft symbol-to-bit demapper. Mutual information transfer charts establish a bijective relationship between the variance of the LLRs and mutual information. A 3rd Generation Partnership Project compliant turbo code is assumed. Since the decoder operates independently from the channel, its extrinsic information transfer (EXIT) charts can be simulated in advance. By utilising the approximate LLR variance distribution of the demapped equaliser output, it is possible to evaluate the probability of an intersection between an equaliser chart associated with a random channel realisation and a fixed decoder chart. This probability provides the FER. Since the proposed performance prediction method does not require any instantaneous channel state information, it can be applied at the transmitter side as a robust link adaptation scheme. In adaptive transmission power control, the modulation order and code rate are fixed. Each year in Finland, approximately 5.7% of infants are born preterm, i.e., before 37 completed weeks of gestation. Preterm birth is a major cause of mortality and several neonatal morbidities, especially the respiratory diseases. Infants born very preterm (<32 wk) are at higher risk of developing a chronic lung disease called bronchopulmonary dysplasia (BPD). The genetic factors predisposing to spontaneous preterm birth (SPTB) and BPD are incompletely known. The aims of this thesis project were to identify genetic factors that affect susceptibility to SPTB and BPD. Genetic case-control association studies were performed in mothers and infants of northern Finnish origin (SPTB study), or in multiple populations of very preterm infants of Finnish or European origin (BPD study). The candidate genes were selected based on their proposed roles in inflammation which is involved in both SPTB and BPD susceptibility. Additionally, the aim was to study the possible functional role of polymorphisms in the gene encoding surfactant protein B (SP-B) that have been shown previously to associate with pulmonary function. An association between Met31Thr polymorphisms in the gene encoding SP-D (SFTPD) and SPTB infants was found. The other collectin genes that were studied, encoding SP-A and mannose-binding lectin, did not associate with SPTB in mothers or infants. An intronic polymorphism in the gene encoding Kit ligand (KITLG) was associated with the risk of BPD in the northern Finnish and in the combined population that originated from Finland, Canada and Hungary. The role of KITLG in BPD was further supported by biomarker data, which showed higher concentrations of Kit ligand at the time of birth in infants that later developed BPD. The genes encoding interleukin 6 (IL-6), its receptors, IL-10, tumor necrosis factor alpha or glucocorticoid receptor did not associate with BPD susceptibility.
