Abstract

Exposure to hazardous substances in the workplace can result in injuries and fatalities. This study aimed to investigate the characteristics and trend of occupational exposures reported to the Dutch Poisons Information Centre and to investigate whether the COVID-19 pandemic had an impact on the trend. A retrospective analysis of all acute occupational exposures reported to the Dutch Poisons Information Centre between 1 January 2016 and 31 December 2022 was performed. Data on patient and exposure characteristics, symptoms and treatment recommendations were analyzed. Between 2016 and 2022, the Dutch Poisons Information Centre received 5,508 calls regarding acute occupational exposures. The annual number of calls on acute occupational exposures almost doubled over the years studied (from 475 in 2016 to 936 in 2022). During and after the COVID-19 pandemic (March 2020-December 2022), the number of calls stabilized, but the upward trend was not significantly affected. There were an estimated 0.20 calls per 1,000 human exposure calls per month (95 per cent confidence interval: −0.14; 0.53). Victims were often exposed through multiple routes, with inhalation being the most common route (44 per cent), followed by ocular (32 per cent) and dermal contact (30 per cent). Acids (1,138 exposures) and alkalis (912 exposures) were often involved. The Dutch Poisons Information Centre had information on 6,334 patients, although the total number of exposed patients was not known as some victims did not seek medical assistance, or were treated by healthcare professionals who did not consult our Centre. At the time of contact, 13 per cent (n = 795) of the patients reported no symptoms, 76 per cent (n = 4,805) reported mild to moderate symptoms and 3 per cent (n = 183) reported potentially severe symptoms. Information on symptoms was missing for 9 per cent (n = 551) of the patients. Hospital observation and treatment were recommended for 5 per cent (n = 325) of the patients. This study highlights the necessity for poisoning prevention strategies to reduce the number of work-related incidents involving hazardous substances. The continuing increase in the number of workplace incidents involving hazardous substances is of concern. A comprehensive and multidisciplinary approach should be taken to gain a full understanding of occupational exposure to hazardous substances and to identify risk factors. [ABSTRACT FROM AUTHOR]

14. Impact Of Country On Adverse Outcome In Acutely Intoxicated Patients In ICU: A Generalized Linear Mixed Models Analysis

Abstract

Our study aimed to investigate poor outcomes among ICU patients with acute poisoning, with a specific focus on discerning international variations. Additionally, we assessed the efficacy of Generalized Linear Mixed Models (GLMM) when compared to Generalized Linear Models (GLMs). We found that patient outcomes significantly diverge across countries, even after accounting for various factors. Notably, countries introduced as random effects within the GLMM exerted a substantial influence on patient outcomes, reflecting the influence of distinct healthcare systems, sociocultural factors, and resource availability. In terms of modelling, GLMM outperformed GLMs. The inclusion of laboratory results alongside patient characteristics notably improved the ability to discriminate poor outcomes. The most effective model incorporated a comprehensive set of parameters, encompassing physiological indicators, laboratory findings, and exposure types. Our analysis unveiled the complexity of the data, indicating that relying solely on physiological and laboratory parameters does not suffice to explain patient outcomes fully. Exposure types, as revealed in this study, significantly contribute to our understanding, aligning with prior research on the mortality implications of intoxication. However, while exposures remained relevant, their impact was comparatively modest. The study also highlighted challenges related to categorizing exposures due to methodological disparities. To tackle this challenge, we must explore and evaluate new exposure categories. Furthermore, concerns emerged regarding patients exposed to multiple substances, warranting the need for a refined approach to exposure categorization and prudent consideration of exposure dosages. Abstract

INTRODUCTION: The accidental ingestion of diluted household descaling products by infants is a phenomenon that poison control centers regularly encounter. Feeding infants with baby milk prepared with water from electric kettles still containing descaler is a common way of exposure. This study aimed to determine the risks related to ingestion of (diluted) descalers by infants.METHODS: pH measurements were performed using acetic acid and three different commercially available electric kettle descalers. The pH of different dilutions was measured in the absence or presence of baby milk powder. In addition, an overview was made of pH values of different electric kettle descalers as given by the product information of the manufacturer. Finally, a simple pharmacokinetic (PK) model was used to predict changes in blood pH in infants after ingestion of acetic acid, which is the most commonly used descaler.RESULTS: Several commercially available electric kettle descalers have a pH <2. Even after diluting such descalers up to 10 times the pH can remain low. The addition of milk powder increases the pH of descalers containing weaker acids, with a pH >1.5, while descalers with stronger acids and pH <1 show little pH increase after the addition of milk powder. Finally, a simple PBPK model for the ingestion of acetic acid predicted that the ingestion of larger amounts of acetic acid (>1000 mg) by an infant could result in relevant changes in blood pH.CONCLUSIONS: Commercially available electric kettle descaling products may pose a health risk to infants in case of accidental ingestion since the pH of some of these products can be very low, even when diluted 10-times or in the presence of baby milk powder. Abstract

Context Pregabalin poisoning is mostly benign, although coma and convulsions occasionally occur. Aim: To determine the dose‐toxicity relationship of pregabalin. Methods: Dose‐toxicity data of isolated pregabalin poisonings were collected from (1) a prospective study performed by the Dutch Poisons Information Centre (4 April 2014 to 4 October 2016) and from (2) case reports and case series reported in literature. Poisonings were graded using the Poisoning Severity Score (PSS) and the relationship between dose (mg kg−1) and PSS was evaluated. Results: In our study (n = 21 patients), the most commonly observed symptoms were drowsiness (62%), confusion (29%) and apathy (24%). PSS was none in three (14%), minor in 15 (71%), and moderate in three patients (14%). Most case series also reported a PSS of none to minor in the majority of poisonings (69‐100%). For 34 individual patients (21 from our study and 13 from literature), detailed data on dose and clinical course were available to examine the dose‐toxicity relationship. The median dose was significantly lower in the PSS none‐minor group ("benign") (8.6 mg kg−1, interquartile range (IQ25‐75) 5.0‐17.6 mg kg−1) than in the PSS moderate‐severe group ("significant toxicity") (46.7 mg kg−1, IQ25‐75 21.3‐64.3 mg kg−1); estimate of the median difference = 27.3 mg kg−1 (95% confidence interval (CI): 10‐48.6). Conclusions: In general, higher pregabalin doses result in more severe poisonings. Below 20 mg kg−1 the majority of patients (83%) only suffer from mild poisoning. However, large interindividual differences exist in pregabalin‐induced toxicity. Therefore, pre‐hospital triage should not only include pregabalin dose, but also underlying illnesses, co‐exposures and reported symptoms. [ABSTRACT FROM AUTHOR]
