1. Evaluating quality neonatal care, call Centre service, tele-health and community engagement in reducing newborn morbidity and mortality in Bungoma county, Kenya

Abstract

Abstract Background Neonatal mortality is a major health burden in Bungoma County with the rate estimated at 31 per 1000 live births and is above the national average of 22 per 1000. Nonetheless, out of the nine sub county hospitals, only two are fairly equipped with necessary infrastructure and skilled personnel to manage neonatal complications such as prematurity, neonatal sepsis, neonatal jaundice, birth asphyxia and respiratory distress syndrome. Additionally, with more than 50% of neonates delivered without skilled attendance, in below par hygiene environments such as home and on the roadsides, with non-existent community based referral system, the situation is made worse. The study aims to evaluate the progress made by an intervention “Collaborative Newborn Support Project” geared towards reducing neonatal mortality rate by 30% between October 2015 and December 2018 in Bungoma County, Kenya. Methods/Design This intervention will take a quasi-experimental design approach with experimental and control sites. The project will involve pre- and post-intervention data collection with comparison group to assess intervention effects. The primary outcome will be the percentage reduction of neonatal mortality in Bungoma County. Secondary outcomes include; a) Percentage of mothers or care givers able to identify at least three danger signs in neonates in the project area, b) Proportion of neonates with complications referred to specialized neonatal centers, through the call center, c) Percentage of health providers in neonatal care units who adhere to expected neonatal standards of care (rapid and complete application of standard protocols), d) Percentage increase in neonates with severe complications in the specialized neonatal units and e) Percentage of neonates who stay in neonatal care units beyond 5 days. Discussion We outline implementation details of the ongoing ‘Collaborative Newborn Support Project’ in Bungoma County, Kenya. This includes strategies in the operations of the telehealth platform, call centre service, community engagement and measuring of the outputs and outcomes. The funding and ethical approvals have been obtained and the study commenced. Nevirapine and lamivudine given to mothers are transmitted to infants via breastfeeding in quantities sufficient to have biologic effects on the virus; this may lead to an increased risk of a breastfed infant's development of resistance to maternal antiretrovirals. The Kisumu Breastfeeding Study (KiBS), a single-arm open-label prevention of mother-to-child HIV transmission (PMTCT) trial, assessed the safety and efficacy of zidovudine, lamivudine, and either nevirapine or nelfinavir given to HIV-infected women from 34 wk gestation through 6 mo of breastfeeding. Here, we present findings from a KiBS trial secondary analysis that evaluated the emergence of maternal ARV-associated resistance among 32 HIV-infected breastfed infants.All infants in the cohort were tested for HIV infection using DNA PCR at multiple study visits during the 24 mo of the study, and plasma RNA viral load for all HIV-PCR-positive infants was evaluated retrospectively. Specimens from mothers and infants with viral load >1,000 copies/ml were tested for HIV drug resistance mutations. Overall, 32 infants were HIV infected by 24 mo of age, and of this group, 24 (75%) infants were HIV infected by 6 mo of age. Of the 24 infants infected by 6 mo, nine were born to mothers on a nelfinavir-based regimen, whereas the remaining 15 were born to mothers on a nevirapine-based regimen. All infants were also given single-dose nevirapine within 48 hours of birth. We detected genotypic resistance mutations in none of eight infants who were HIV-PCR positive by 2 wk of age (specimens from six infants were not amplifiable), for 30% (6/20) at 6 wk, 63% (14/22) positive at 14 wk, and 67% (16/24) at 6 mo post partum. Among the 16 infants with resistance mutations by 6 mo post partum, the common mutations were M184V and K103N, conferring resistance to lamivudine and nevirapine, respectively. Genotypic resistance was detected among 9/9 (100%) and 7/15 (47%) infected infants whose mothers were on nelfinavir and nevirapine, respectively. No mutations were detected among the eight infants infected after the breastfeeding period (age 6 mo).Emergence of HIV drug resistance mutations in HIV-infected infants occurred between 2 wk and 6 mo post partum, most likely because of exposure to maternal ARV drugs through breast milk. Our findings may impact the choice of regimen for ARV treatment of HIV-infected breastfeeding mothers and their infected infants.

5. Uncovering Reading Habits of University Students in Uganda: Does ICT Matter?

Abstract

This paper investigates how Information and Communication Technology (ICT) can improve reading habits among university students. We also investigated the influence of home culture, school culture and disposable income on reading habit. Our main objective was to assess the effect of ICT on the reading habit of particularly university students in Uganda. We adopted a qualitative phenomenological approach for the study in order to get an in-depth understanding of how the ICT phenomenon impacts reading habits. Participants from four universities in Uganda were interviewed. Major findings indicate that most of the students find the use of ICT a necessity that lends a helping hand in accessing reading sources. The school culture, home culture and disposable income also impact the reading habit of students and particularly have an influence on ICT and reading. We also found that most participants were initiated into reading by use of force at home. This study provides a unique contribution through empirical evidence that ICT indeed impacts one's reading habit and thereafter proposes policy implications that may improve the reading habit of students through ICT interventions.

Data quality control is one of the most time consuming activities within Research Infrastructures (RIs), especially when involving observational data and multiple data providers. In this work we report on our ongoing development of data rogues, a scalable approach to manage data quality issues for observational data within RIs. The motivation for this work started with the creation of the FLUXNET2015 dataset, which includes carbon, water, and energy fluxes plus micrometeorological and ancillary data measured in over 200 sites around the world. To create an uniform dataset, including derived data products, extensive work on data quality control was needed. The unpredictable nature of observational data quality issues makes the automation of data quality control inherently difficult. Developed based on this experience, the data rogues methodology allows for increased automation of quality control activities by systematically identifying, cataloging, and documenting implementations of solutions to data issues. We believe this methodology can be extended and applied to others domains and types of data, making the automation of data quality control a more tractable problem.

10. Staff Perception towards Cloud Computing Adoption at Universities in a Developing Country

Abstract

The paradigm shift ensuing from cloud computing diffusion has moved organizations from traditional datacentre hosting to the cloud. Many university staff and students in western developed countries have leveraged cloud computing to enhance and improve teaching, research and collaboration without need for on-campus presence. Universities in developing countries tend to lack basic ICT infrastructures; hence cloud computing, which offers a more cost-effective and flexible solution to data storage and transfer, might be examined in order to understand how their staff can benefit from cloud computing. This study investigates the perception of university staff in a developing country context on adoption of cloud computing as a tool to enhance access to ICT resources for their educational needs. Using the diffusion of innovation theory and other contextual variables relevant to the study location, this study analysed data from 251 respondents from 11 universities in Uganda using structural equation modelling. Results provided strong support for impact of socio-cultural factors and results demonstrability on staff intention to adopt cloud computing for teaching, research and collaboration. The results also indicated a statistical difference between male and female staff in their perception towards cloud computing adoption. Recommendations for further research and practical implications are discussed.

Abstract

This study investigates the gender equality barriers among maritime students in the Philippines, focusing on the influence of ethnicity and age on perceptions of these barriers. Utilizing a descriptive correlational method, the research involved 554 student respondents from the Merchant Marine Academy of Caraga, Inc. (MMACI). The study employed a questionnaire adapted from Fidan et al. (2020) to assess perceptions of structural and social barriers. The findings reveal significant gender disparities in the maritime education sector, with males dominating both BS Marine Engineering (BSMarE) and BS Marine Transportation (BSMT) programs. The study also highlights the interconnected nature of structural and social barriers, emphasizing the need for comprehensive interventions to promote inclusivity and equity. Additionally, the research identifies variations in perceptions of gender equality barriers based on ethnicity, particularly among BSMarE students. The study concludes by proposing an action plan to promote gender and development programs in the College of Maritime Education, emphasizing awareness, policy revisions, training, mentorship, and industry collaboration. Abstract

We present details from a recent test of Lorentz and CPT symmetry using hydrogen masers. We have placed a new limit on Lorentz and CPT violation of the proton in terms of a recent standard model extension by placing a bound on sidereal variation of the F = 1 Zeeman frequency in hydrogen. Here, the theoretical standard model extension is reviewed. The operating principles of the maser and the double resonance technique used to measure the Zeeman frequency are discussed. The characterization of systematic effects is described, and the method of data analysis is presented. Abstract

We use the dressed atom formalism to calculate the frequency shift in a hydrogen maser induced by applied radiation near the Zeeman frequency, and find excellent agreement with a previous calculation made in the bare atom basis. The maser oscillates on the Delta_F = 1, Delta_m_F = 0 hyperfine transition, while the applied field is swept through the F = 1, Delta_m_F = \pm 1 Zeeman resonance. We determine the effect of the applied field on the Zeeman levels using the dressed atom picture, and then calculate the maser frequency shift by coupling the dressed states to the microwave cavity. Qualitatively, the dressed-atom analysis gives a new and simpler physical interpretation of this double resonance process, which has applications in precision hydrogen Zeeman spectroscopy, e.g., in fundamental symmetry tests., Comment: 11 pages, 4 figures; submitted to PRA

22. Alleviation of Child Abuse as a Religious Duty

Abstract

In Kenya, cultural practices have idiomatically remained a big thorn in the flesh among the various pastoral communities of Kenya. These pastoralist regions have remained traditionally undeveloped areas; either by design or by default. Such areas have posed a major challenge to Sustainable Development Goals (SDGs) No. 5 globally, as well as in Kenya. This research article sets out to investigate the role of the Catholic Church in alleviating Child abuse in Kajiado County, which is directly addressing SDG No. 5, which requires the “Elimination of all harmful practices, such as child [abuse], early and forced marriages and female genital mutilation.” Specifically, the research article evaluates the Catholic Church's social teachings and activities which enhance the alleviation of child abuse in Kajiado County. Besides identifying the challenges faced by the Catholic Church, it equally assesses the strategies adopted by the said Church in order to overcome this enigmatic problem of child abuse. This research article is set on the premise that the failure to eradicate child abuse will lead to the continuous psychological, emotional, physical and poor developmental growth of children, hence failing to attain SDG No. 5. The research article is justified by the need to respond to the alarming reports by media, opinion leaders and members of the community concerning the prevalence of child abuse amongst the Maasai communities in Kenya. For good results, this research article has adopted a descriptive survey design in order to collect in-depth data from a diversified sample of respondents who represent the whole Kajiado County population. Simple random and purposive sampling methods were used to choose a sample size of a number of respondents. At the end of the research article it has been revealed that the Catholic Church has social teachings on caring, creating a safe environment for children and ably demands integrity from the assigned the care-givers. Although it was an uphill task and a major challenge in reporting and investigating cases of child abuse, due to some cultural restrictions, the Church has adopted measures such as forming the Diocesan children and youth departments, which helps in formulating policies and advises their respective Church committees on appropriate measures to be taken on child protection. It is finally recommended that the civil authorities and the Sub-County Children Department could protect victims of abuse through giving proper information on the children rights and also by speedily liaising with the authorities in arresting child abusers.

24. Phonological Processes Governing Borrowing from English to Lungu, Mambwe, and Namwanga Languages

Abstract

This article accounts for the phonological processes which govern borrowing from English to Lungu, Mambwe, and Namwanga (LuMaNa) languages. Guthrie (1948) classifies Lungu as M14, Mambwe as M15, and Namwanga as M22. Data were collected using elicitation, comparative, and document analysis methods. The paper employed Lexical Phonology and Morphology, and the comparative Bantu morphophonology theories. The study shows that LuMaNa languages use four major phonological processes to borrow from the English language, namely, epenthesis, sound substitution, metathesis or permutation, and vowel lengthening. Vowel epenthesis is used to create open syllables and break up consonant clusters during borrowing into LuMaNa. Consonant sound substitution is used to modify loanwords through devoicing. LuMaNa languages obey the following general devoicing rules; q/ð→t, q/ð→ f, z → s and defy the rest of the rules. Vowel sound substitution is achieved in LuMaNa through adaptation and nativisation of phonetic approximation. LuMaNa also borrow words through metathesis where two adjacent sounds reshuffle their positions to facilitate ease of articulation. Vowel length is used to borrow words from English to enable words to adjust and fit in the phonology of the target language. The study concludes that LuMaNa have similarities in the application of phonological processes during borrowing.

Abstract

This paper examines specific constructs for sustainable entrepreneurship as perceived in the Ugandan context using confirmatory factor analysis. This study is cross-sectional. Data were collected through a face to face survey of 384 small businesses in Kampala selected through stratified and simple random sampling. Data were analyzed through exploratory factor analysis (EFA), confirmatory factor analysis (CFA) and descriptive statistics using Statistical Package for Social Sciences (SPSS) version 23. The study revealed that the constructs for sustainable entrepreneurship as perceived in the Ugandan context are production management, people and skills, ecosystem management, stakeholder, finance, strategy, marketing and sales. This suggests that seven factors with eigenvalues greater than one were identified, accounting for 63.23% of the total variance explained in sustainable entrepreneurship. This study presents initial evidence on the constructs of sustainable entrepreneurship that apply to the local context from the perspective of the business owners as opposed to the experts in the field. Abstract

The aim of this article is to evaluate the perception of Pentecostal Christians in regard to the importance of the indigenous guidance and counselling among the Aembu indigenous society. In the latter, they socialized their children from birth to puberty. In a nutshell, emphasis is given to transitional rites of passage. Transitional rites of passage served as important tools of child socialization which was meant to instil moral values among the Aembu youths. In our contemporary society, many adolescents face moral issues which often call for intensive child socialization from home and church circles. In its theoretical framework, the article used the structural functional theory. Qualitative approach was applied as the determinant design. The article reveals that there are moral issues among the youth and that there are useful Aembu teachings and practices which were used in the Aembu indigenous society as socialization tools in order to instil moral values among youths to solve morality issues. The article concludes that there is an urgent need to put in place alternative rites of passage with the sole aim of coming up with effective child socialization programs.

PurposeThe purpose of this paper is to report results of a study carried out to establish the mediation effect of sustainability intention in the relationship between sustainability behavioral control and sustainable entrepreneurship.Design/methodology/approachThis study is cross sectional and correlational. Data were collected through a questionnaire survey of 384 small businesses. Data were analyzed through correlation coefficients and linear regression using Statistical Package for Social Sciences and the Medgraph program.FindingsThe results suggest that sustainability intention partially mediates the relationship between sustainability behavioral control and sustainable entrepreneurship. Results further indicate that sustainability behavioral control and sustainability intention are significant predictors of sustainable entrepreneurship.Originality/valueThis study provides an initial empirical evidence on the mediation effect of sustainability intention in the relationship between sustainability behavioral control and sustainable entrepreneurship. To the researcher's knowledge, no study had been conducted on such an interesting topic using evidence from a developing country such as Uganda.

Abstract

Background Neonatal and maternal mortality rates remain high in Kenya. Knowledge of neonatal danger signs may reduce delay in deciding to seek care. Evidence is emerging on the influential role of male partners in improving maternal and newborn health. This study analysed the factors that determine men’s and women’s knowledge and practices in postnatal and neonatal care-seeking, in order to inform design of future interventions. Methods A quantitative, cross-sectional study was undertaken in Bungoma County, Kenya. Women who had recently given birth (n = 348) and men whose wives had recently given birth (n = 82) completed questionnaires on knowledge and care-seeking practices relating to the postnatal period. Univariate and multivariate logistic regression analyses were performed to investigate associations with key maternal and newborn health outcomes. Results 51.2% of women and 50.0% of men knew at least one neonatal danger sign, however women knew more individual danger signs than men. In the univariate model, women’s knowledge of a least one neonatal danger sign was associated with attending antenatal care ≥4 times (OR 4.46, 95%CI 2.73–7.29, p Conclusion Knowledge of neonatal danger signs was low, however there was an association between knowledge of danger signs and increased healthcare service use, including male partner involvement in antenatal care. Future interventions should consider the extra costs of facility delivery and the barriers to men participating in antenatal and postnatal care.

Abstract

The following authors were omitted from the original version of this Data Descriptor: Markus Reichstein and Nicolas Vuichard. Both contributed to the code development and N. Vuichard contributed to the processing of the ERA-Interim data downscaling. Furthermore, the contribution of the co-author Frank Tiedemann was re-evaluated relative to the colleague Corinna Rebmann, both working at the same sites, and based on this re-evaluation a substitution in the co-author list is implemented (with Rebmann replacing Tiedemann). Finally, two affiliations were listed incorrectly and are corrected here (entries 190 and 193). The author list and affiliations have been amended to address these omissions in both the HTML and PDF versions.
