Abstract

Equimolar amounts of trichlorovinylsilane (9), lithium-tert-butyl, and 1,3- enynes were reacted to yield the corresponding isomeric silacyclobutanes and silacyclobutenes competitively. As reaction pathway the mixture 9/[sup.t.BuLi] is discussed to give a silene equivalent, [Cl.sub.2]Si=CHC[H.sub.2] [sup.t]Bu (10), yielding the four-membered ring silacycles by formal [2 + 2] cycloaddition reactions of 10 with the C=C double or the C[equivalent]C triple bond of the 1,3-enyne. The relative ratio of the products formed depends on the polarity of the multiple bonds in the enyne, which is mainly determined by the substituent pattern. Thus, from the organosubstituted 1,3-enynes [R.sup.1]C[equivalent]C-C([R.sup.2])=C[R.sup.3][R.sup.4] ([R.sup.1] = Me, Et, Si[Me.sub.3], Ph; [R.sup.2] = H, Me, Ph; [R.sup.3]=Me, OMe, Ph; [R.sup.4]=H; and [R.sup.1]C [equivalent]C-R' (R' = 1-cyclohexenyl, cyclohexanevinylidyne)) and 9/[sup.t.BuLi] the silacyclobutanes 12, 13, and 15 and the silacyclobutenes 14, 16-2A, and 27 (from 10 and 3-hexyne) are prepared in a one-step synthesis and isolated from the reaction mixtures. The silacyclobutanes and -butenes are thermally stable and can be distilled under vacuo up to temperatures of about 150[degrees]C without decomposition. The experimental results are confirmed qualitatively by semiempiric calculations at the AM-1 level and their analysis using FMO theory. The solid state structure of the silacyclobutene 19 ([C.sub.17][H.sub.30][Cl.sub.2] [Si.sub.2]) has been determined by single crystal X-ray diffractometry. 19 is orthorhombic, space group P[2.sub.1][2.sub.1][2.sub.1], a = 1030.94(2) pm, b = 1244.6(2) pm, c = 1605.5(3) pm, Z = 4. Abstract

Photoreflectance (PR) and photoluminescence (PL) are applied to study the band structure and PL thermal quenching for [Ga.sub.0.24][In.sub.0.76]Sb/GaSb quantum wells (QWs) of the varying widths. The thermal quenching of PL is described by a standard expression with two characteristic activation energies, which is attributed to an electron escape and a heavy-hole escape to the conductance and valence continua in GaSb barriers.

Abstract

Negative symptoms that do not improve following antipsychotic treatment represent a challenge for development of effective treatments. Few studies have been carried out so far, especially in first-episode schizophrenia patients, to clarify prevalence, correlates and impact of persistent negative symptoms (PNS) on short- and long-term outcome of the disease.All patients from EUFEST study for whom both baseline and 12-month assessments were available were included (N=345). PNS were defined as the presence of at least one negative symptom of moderate or higher severity, not confounded by depression or parkinsonism, at baseline and after 1 year of treatment. Patients with PNS were compared to those with at least one negative symptom of moderate or higher severity at the baseline, not persisting after 1 year, on demographic, clinical, neurocognitive, global functioning and quality of life measures.PNS not confounded by depression or parkinsonism were present in 6.7% of the sample. The symptom that more often persisted was blunted affect. Patients with PNS differed from those without PNS for a longer duration of untreated psychosis (DUP) and a more frequent discontinuation of study treatment; they also had a poorer psychopathological outcome and a worse global functioning after 1 year of treatment.The presence of PNS was associated to poorer improvement of all psychopathological dimensions and worse global functioning after 1 year of treatment. The longer DUP in subjects with PNS suggests that programs aimed at shortening DUP might reduce the prevalence of PNS and improve prognosis of schizophrenia. © 2012 Elsevier B.V. and ECNP.

Abstract

The population with schizophrenia is characterised by a leftward shift in handedness-sinistrality. However, findings are inconsistent in chronic patients, and familial sinistrality (FS), defined as the presence of left-handed close relatives, might contribute to the discrepancies. Therefore the aim of this study was to investigate the strength of manual lateralisation in patients with first episode schizophrenia, taking into account familial sinistrality. The Edinburgh Inventory (EI) allowed us to categorise 179 patients from the EUFEST study and 189 controls presenting "strong handedness'' (SH: EI absolute value between j81j and j100j) or "weak-handedness'' (WH: EI value between -80 and +80). The nominal logistic regression did not show an FS effect, but a nearly significant interaction between illness and FS (p = .07). There were fewer participants without FS presenting SH among patients (99/151: 65.6%) than among controls (134/164: 81.7%, p = .001). In contrast, the number of participants with FS presenting SH was similar between controls (68%) and patients (75%, p = .57). The presence of left-handed relatives (FS +) tended to reduce manual lateralisation, but only in controls. This supports the notion that reduced manual lateralisation in schizophrenia is related to the illness rather than to familial left-handedness.

Abstract

Granulocytopenia and agranulocytosis are severe side effects of clozapine therapy. Even if these side effects are detected early and if clozapine is discontinued, patients suffering from agranulocytosis are extremely endangered by infectious diseases for up to 3 to 4 weeks until hematologic recovery. Therefore, any treatment that reduces this critical time span would decrease the risks of clozapine treatment.The case of a patient in whom severe agranulocytosis developed after 7 weeks of clozapine treatment is presented.After clozapine discontinuation, treatment with granulocyte-macrophage colony-stimulating factor (GM-CSF), a glycoprotein that has been shown to stimulate the proliferation of precursor cells in the bone marrow and their differentiation into granulocytes and macrophages, was initiated. Under GM-CSF treatment, total granulocyte count rose from 63/cu mm to a value greater than 1500/cu mm within 5 days without complications or major side effects.This case report suggests that treatment with GM-CSF may lower the risks associated with clozapine-induced agranulocytosis and therefore may indirectly improve the safety of clozapine therapy.
