Objectives: The objective of this study was to present the follow-up findings obtained through computed tomography in skeletally immature French bulldogs previously diagnosed with humeral intracondylar fissures., Materials and Methods: Medical records from a surgical referral centre between January 2018 and September 2022 were evaluated for all dogs with a humeral condylar fracture where preoperative CT of both humeri was performed. French bulldogs between the age of 12 weeks and 12 months were identified and evaluated for the presence of contralateral humeral intracondylar fissure. The dogs with follow-up CT were evaluated for humeral intracondylar fissure resolution., Results: A total of 137 humeral condylar fractures were operated on within the study period, 58 of which were in French bulldogs. Twenty-seven French bulldogs had a contralateral humeral intracondylar fissure with 12 dogs matching the inclusion criteria. Follow-up CT demonstrated resolution of the humeral intracondylar fissure in 11 (91.7%) of 12 cases., Clinical Significance: Contralateral humeral intracondylar fissures are common in skeletally immature French bulldogs sustaining humeral condylar fractures and the majority of these fissures tend to fuse within the follow-up period. Objective:  To report the use of a Lateral Epicondylar Anatomical Plate for the management of humeral condylar fractures (HCF) in dogs., Study Design:  Medical records of dogs with HCF stabilized using the Lateral Epicondylar Anatomical Plate at six UK veterinary referral centres between April 2018 and February 2021 were reviewed. Long-term follow-up (>6 months) was obtained via owner questionnaire, which incorporated the Liverpool Osteoarthritis in Dogs clinical metrology instrument., Results:  Sixty-two HCF were treated in 61 dogs (44 lateral condylar fractures [LCF] and 18 intracondylar (T/Y) fractures [ICF]). Fifty-one dogs were Spaniels or Spaniel crossbreeds. Intraoperative contouring of the plate was required for one dog-a French Bulldog. Postoperative complications occurred in 14/42 LCF and 6/18 ICF; overall there were 14 minor, 8 major, and 2 catastrophic complications. On final follow-up imaging, there was evidence of partial or complete osseous continuity of the condylar part of the fracture 32/53 HCF (24/39 LCF and 8/14 ICF) and lateral epicondylar part of the fracture in 53/53 HCF (39/39 LCF and 14/14 ICF). At final reexamination, 20/28 dogs with LCF and 5/13 dogs with ICF were not lame and the remaining dogs demonstrated mild lameness. According to the owner questionnaire, 17/17 dogs with LCF and 8/10 dogs with ICF returned to full limb use and median Liverpool Osteoarthritis in Dogs scores were 2/52 for LCF and 6.5/52 for ICF., Conclusion:  The Lateral Epicondylar Anatomical Plate can be used successfully for the surgical stabilization of HCF in dogs., Competing Interests: M.B.W. is Clinical Research and Development Director at Fusion Implants. Objective:  The aim of this study was to report (1) the prevalence of contralateral humeral intracondylar GAP/HIF (HIF) in French Bulldogs with humeral condylar fractures (HCF) and (2) treatment and short- and long-term outcome in French Bulldogs with HCF., Materials and Methods:  Medical records of French Bulldogs treated surgically for HCF between January 2011 and November 2022 were reviewed. Perioperative imaging, surgical technique, complications, and long-term outcome were assessed., Results:  Eighty-nine dogs with HCF were included and 36/89 were diagnosed with HIF. Surgical repair was performed in all HCF and a prophylactic transcondylar screw was placed in 20/36 HIF. For prophylactic transcondylar screw placement, two dogs required immediate revision surgery. Complications occurred in 12/89 HCF, and there were major complications in 6/89 HCF. Long-term outcome evaluated by owner questionnaire ( n  = 27) was rated as being excellent in 18/27 dogs, good in 8/27 dogs, and fair in 1/27 dogs., Clinical Significance:  Forty percent of French Bulldogs with HCF had a contralateral HIF. HIF as a cause of HCF should be considered in the French Bulldog, and a computed tomography (CT) scan of both elbows may be advisable prior to HCF repair. Objective:  The aim of this study was to report postoperative complications and long-term outcomes following transcondylar screw placement for humeral intracondylar fissure (HIF)., Study Design:  It was a retrospective single-centre case series. Medical records (2018-2022) were reviewed for dogs with HIF treated with transcondylar screw placement. Data collected included signalment, concurrent orthopaedic disease, partial or complete HIF, surgeon, surgical approach, surgical technique and implant type, transcondylar screw angulation, postoperative complications and outcomes. Long-term outcome was assessed with owner questionnaire, orthopaedic examination and follow-up radiography. Statistical analysis was performed to identify risk factors predisposing to a complication or a poor outcome., Results:  Forty-seven dogs (57 elbows) met the inclusion criteria; long-term follow-up was available in 41 dogs (50 elbows). Minor and major medical complications were noted in seven and three elbows respectively. The total complication rate was 17.5%. Increasing age was significantly associated with a reduced risk of postoperative complications ( p  = 0.0051). No other risk factors were identified. Objective:  To report the inciting cause and configuration of humeral condylar fractures in French Bulldogs, and to identify risk factors for complications associated with their surgical stabilization, as well as their occurrence., Study Design:  Retrospective multicenter clinical cohort study., Animals:  French bulldogs ( n  = 136) with humeral condylar fracture., Methods:  Records from eight United Kingdom referral centers were reviewed for French bulldogs that underwent surgery for humeral condylar fracture between January 2017 and January 2021., Results:  Lateral fractures of the humeral condyle represented two-thirds of condylar fractures in this study (95 elbows; 69.8%); medial fractures of the humeral condyle and intracondylar Y or T fractures affected 22 (16.2%) and 19 (14%), respectively. A fall was the inciting cause for 60 elbows (45.6%), with stairs implicated in 27 (20.45%) elbows. Skeletal immaturity was significantly correlated with fracture. Complications affected 30 out of 132 dogs (136 elbows) (22%), of which 10 (33%) were major, and 20 (66%) were minor. Fixation method did not significantly impact complication rate for medial or lateral fractures of the humeral condyle ( p  = 0.87). Comminution increased the risk of complication for all humeral condylar fracture types ( p  = 0.02)., Clinical Relevance:  Fixation method was not found to significantly impact the complication rate of surgically managed fractures of the humeral condyle in French bulldogs. Aims: To use a farm-based survey to identify characteristics of the New Zealand dairy system associated with the risk of spontaneous humeral fracture in dairy heifers., Methods: A questionnaire was designed and made available in print and online to collect information from dairy farmers and/or veterinarians, across New Zealand, about the management and nutrition of cows from birth to first lactation. Data were collected from July 2019 to March 2020 from farms that either had recorded (case farms) or not recorded (control farms) cases of humeral fractures in dairy heifers., Results: A total of 68 completed questionnaires were returned, with 35 responses from case farms and 33 responses from control farms. Twenty-six responses (38%) were from the South Island (13 case farms and 13 control farms) and 38 responses (56%) were from the North Island (20 case farms and 18 control farms). For four questionnaires (6%) farm location was not given. Adjusting for the effect of age when calves accessed pasture, case farms had increased odds of having Holstein-Friesian Jersey crossbreed cows as the predominant breed (OR = 9.7; 95% CI = 3.1-36.0; p < 0.001). Adjusting for the effect of breed, allowing calves access to pasture a week later decreased the odds of being a case farm (OR = 0.68; 95% CI = 0.47-0.90; p = 0.006)., Conclusions: Cows being Holstein-Friesian Jersey crossbreed was identified as a possible risk factor associated with spontaneous humeral fracture in dairy heifers in New Zealand. Given the small sample size, the likely multifactorial aetiology for humeral fractures, and the non-randomised survey, this risk factor, and the possible association between age at turn out and herd production with humeral fractures, all require further investigation.

Objective: To compare the biomechanical properties of using a novel composite construct (AdhFix) to an interfragmentary Kirschner wire or a reconstruction plate as adjunctive epicondylar stabilization in simulated lateral unicondylar humeral fractures., Study Design: Cadaveric biomechanical assessment., Sample Population: Paired humeri harvested from skeletally mature dogs (14-41 kg), nine cadavers per group., Methods: Simulated lateral unicondylar humeral fractures were stabilized with a transcondylar 4.5 mm cortical screw placed in lag fashion. Adjunct fixations consisting of a novel composite incorporating 2.7 mm cortical screws on one side, and either a 2.7 mm reconstruction plate or a 1.6 mm Kirschner wire on the contralateral side, were tested within paired humeri. Repaired humeri were axially loaded to failure and construct stiffness, yield load, and ultimate load were obtained from the load-deformation curves., Results: In pairwise comparison, yield load was significantly higher for AdhFix group compared to the pin group, p = .016. No statistical significance was seen in the comparison between AdhFix group and the plate group, p = .25., Conclusion: Adhfix was mechanically superior to K-wires, and comparable to plate fixation, for adjunctive fixation in a lateral humeral condylar model. Our results support further investigation of the novel composite for adjunct fracture fixation in lateral humeral condylar fractures., Clinical Significance: The novel composite tested may be a viable alternative for adjunct fixation of humeral condylar fractures, a technique that circumvents plate contouring., (© 2023 The Authors. Case Histories: Three dogs and one cat sustained forelimb trauma and were presented to a university veterinary clinic (Liège, Belgium) and a private veterinary hospital (Beacouzé, France). All four animals were referred for surgery., Clinical Findings: Two dogs and the cat were ambulatory on admission but unable to bear weight on the affected limb. One dog was non-ambulatory and lacked voluntary movement and sensation in one forelimb. Salter-Harris type II fractures of the distal humerus were diagnosed by radiography in all cases; avulsion of the brachial plexus and pelvic fractures were also present in the non-ambulatory dog., Treatment and Outcome: All Salter-Harris type II fractures were stabilised by open reduction and internal fixation with cross pins. One minor complication (seroma) and three major complications (implant migration) developed after surgery. The pins were completely removed in one case and partially removed in two cases to resolve these complications. At the final follow-up examination (12-31 months after surgery), owners reported no lameness in three of the four cases and grade 2/5 left forelimb lameness in one case., Clinical Relevance: This type of fracture is rarely described in the literature; however, it should be included in the differential diagnoses of traumatic humeral fractures in growing dogs and cats. In this case series, we achieved fair-to-excellent short-term and long-term outcomes after osteosynthesis of Salter-Harris type II fractures by cross pinning.

Objective: To report the prevalence of presumed concurrent medial coronoid process fractures in a series of cases of humeral condylar fractures and evaluate factors influencing the presence of a suspected fracture of the medial coronoid process., Animals: 48 dogs and 7 cats, with a total of 57 humeral fractures., Methods: Medical records of dogs and cats diagnosed with a humeral condylar fracture with radiographs were reviewed between October 2013 and March 2022. Species, sex, neutered status, age, weight, and the nature of the trauma were noted. Radiographs were assessed for the configuration of humeral condylar fracture, the presence of a suspected fractured medial coronoid process (MCP), number of MCP fragments, nature of fracture, degree of radioulnar incongruity, soft tissue swelling, and elbow luxation/subluxation., Results: A presumed fracture of the MCP was seen in 26 of 57 cases. Comminution of the condylar fracture was the only parameter that had a positive effect on the presence of a possible fractured MCP. Body weight was significantly associated with size of the suspected fractured MCP. The presence of this fracture was not associated with the type of humeral condylar fracture. The size of the presumed fractured MCP fragment was positively correlated with body weight., Clinical Relevance: There was a high prevalence of presumed fractured MCPs in dogs with humeral condylar fractures (almost 50%) and even more so in animals with comminuted fractures. The consequences of suspected fractured MCP associated with humeral condylar fractures and whether dogs and cats would benefit from removal of the fragment remain unknown.

Objective:  The aim of this study was to compare the interfragmentary compressive force and area of compression generated by cortical screws inserted as either a lag screw or position screw in simulated lateral humeral condylar fractures., Study Design:   Ex vivo biomechanical study., Materials and Methods:  Thirteen pairs of cadaveric humeri from skeletally mature Merinos with simulated lateral humeral condylar fractures were used. Pressure sensitive film was inserted into the interfragmentary interface prior to fracture reduction with fragment forceps. A cortical screw was inserted as a lag screw or a position screw and tightened to 1.8Nm. Interfragmentary compression and area of compression were quantified and compared between the two treatments groups at three time points., Results:  After fracture reduction using fragment forceps (Time point 1: T1), there was no significant difference in interfragmentary compression and area of compression between the two treatments. A combination of fragment forceps and a cortical screw inserted as a lag screw (Time point 2: T2) produced significantly greater interfragmentary compression and area of compression compared with the same screw inserted as a positional screw. After removal of the fragment forceps, leaving only the cortical screw (Time point 3: T3), both the interfragmentary compression and area of compression remain significantly greater in the lag screw group., Conclusion:  Lag screws generate a greater force of compression and area of compression compared with position screws in this mature ovine humeral condylar fracture model., Competing Interests: None declared., (Thieme. French bulldogs (FBD) are reported to be at an increased risk of developing humeral condylar fractures (HCF). Spaniel breeds have been reported to be at increased risk of HCF due to a high prevalence of humeral intracondylar fissures (HIF), which can predispose to fracture. The aims of this retrospective, single center, observational study were twofold. First, to compare the signalment, fracture configuration, and etiology of populations of FBD and spaniel breeds presenting with HCF at a single hospital. Second, to evaluate the computed tomographic (CT) findings of the contralateral elbow joints of FBD and spaniel breeds presenting with HCF for concomitant pathology. FBDs were overrepresented for HCF, most commonly presenting when skeletally immature, with a median age of 3.7 months. Fractures of the lateral condyle were the most common configuration for both breed groups. Sclerosis at the fracture site was identified in 62% of FBD and 95% of spaniels undergoing CT scans. Humeral intracondylar fissures and intracondylar sclerosis were commonly observed in the contralateral elbows of FBD (44% and 50%, respectively) and spaniel breeds (58% and 95%, respectively). Medial coronoid pathology, elbow incongruency, and periarticular osteophytes were not identified in the contralateral elbows of FBD but were identified in 29%, 4%, and 20% of spaniels, respectively. Objective: To design a precontoured plating system for humeral condylar fracture repair in spaniels with humeral intracondylar fissure (HIF) and quantify accuracy of fit of the system on 3D printed bone models., Study Design: Experimental on 3D printed models., Methods: The plating system was based on morphometric data obtained from CT. Plates were applied to models and impression material was measured under the plates at points of interest to define plate bone gap (PBG). Data on locking screw interference on plate application were also obtained., Results: Eleven elbows models were plated. Statistical analysis to assess for any difference in PBG measurements between left and right humuri revealed two loci (3 and 17) under the lateral plate and two loci (3 and 17) under the medial plate. On cumulate portion analysis, 67% of lateral plate loci and 80% of medial plate loci had a PBG of ≤2 mm respectively. An incidence of 2.1% (3/143) screw interference occurred., Conclusion: Design of a contoured plating system for application to humeral models from spaniels with HIF and its application with objective measurement of PBG is described. The majority of the loci measured under both lateral and medial plates had a BPG of ≤2 mm and the incidence of locking screw interference on plate application was low., Clinical Significance: This novel system may have application for the stabilization of humeral condylar fractures in spaniels avoiding the need for plate contouring and minimizing the incidence of locking screw interference., (© 2022 American College of Veterinary Surgeons.)

Case History: Serum and liver samples from 35, 2-year-old dairy heifers that had fractured one or both humeri post-calving between July and December 2019 were submitted to a diagnostic laboratory for analysis. Serum samples were analysed for albumin, β-hydroxybutyrate (BHB), creatinine, Ca, Mg, phosphate, non-esterified fatty acids (NEFA), and serum Cu concentration. Liver samples were analysed for liver Cu concentration. Data were compared to published reference intervals. Data values for heifers that prior to fracture had grazed fodder beet were also compared to values for those that had grazed pasture., Clinical Findings: Sixty-nine percent of heifers with humeral fracture had serum creatinine concentrations below the lower value of the reference range (55-130 µmol/L). In 3/32 (9%) heifers, serum NEFA concentrations were increased above the reference value indicating body fat mobilisation (≥1.2 mmol/L for peri-partum cows) and in 20/35 (57%) heifers BHB serum concentrations were above the reference value indicating subclinical ketosis (≥1.1 mmol/L for peri-partum cows). In 24/35 (69%) heifers, liver Cu concentration was low (≤ 44 µmol/kg) or marginal (45-94 µmol/kg). The concentration of Cu in serum was low (≤ 4.5 µmol/L) in 2/33 (6%) heifers and marginal (4.6-7.9 µmol/L) in 5/33 (15%) heifers. There was moderate positive correlation between the logged concentrations of Cu in paired liver and serum samples, r (31)  = 0.43; (95% CI = 0.1-0.79; p = 0.014). One heifer had a serum phosphate concentration below the lower limit of the reference range (< 1.10 mmol/L). For all heifers, the concentrations of albumin, Ca, and Mg in serum were within the reference intervals (23-38 g/L, 2.00-2.60 mmol/L, and 0.49-1.15 mmol/L respectively). Over winter, 15/35 (43%) heifers grazed predominantly pasture, 14/35 (40%) grazed fodder beet and 6/35 (17%) had a mixed diet., Clinical Relevance: In some of these heifers with humeral fractures, there was evidence for protein and/or energy malnutrition in the form of elevated NEFA and BHB concentrations and low creatinine concentrations in serum. Liver Cu concentrations were also reduced in most affected heifers. However, the absence of a control group means it is not possible to determine if these are risk factors for fracture or features common to all periparturient heifers. Objective:  Our objectives were to report complications associated with stabilization of long-bone fractures in cats using a 3.5-mm titanium interlocking nail and to examine the influences of signalment, fracture type and fixation evaluations on the occurrence of complications., Study Design:  Retrospective clinical study., Material and Methods:  Medical and radiographic records of cats with long-bone fractures treated with an interlocking nail were reviewed. Data included age, sex, weight, cause of the fracture, fractured bone(s) and fracture type. Complications were classified as minor and major complications. Fisher's exact tests and logistic regression analysis were used to test whether certain variables of signalment and interlocking nail configuration had an effect on the occurrence of complications., Results:  Sixty-seven fractures of 67 cats were examined in this study. Forty-eight femora, sixteen tibiae and three humeri were included. Complications occurred in 11/67 fractures. Major complications occurred in 8/67 fractures and included screw breakage ( n  = 3), nail breakage ( n  = 2), nail bending ( n  = 1), screw loosening ( n  = 1), non-union (n = 1). Statistical analysis showed a significant difference between fracture types and the occurrence of major complications ( p  = 0.02)., Conclusion:  In conclusion, use of this commercially available standard 3.5-mm titanium interlocking nail for stabilization of comminuted and oblique humeral, femoral and tibial fractures in cats is feasible., Competing Interests: None declared., (Thieme. Outbreaks of humeral fractures in dairy cows have been reported in New Zealand for several years. Gross, histologic, and histomorphometric findings in the humerus from primiparous cows with spontaneous humeral fracture were compared to age-matched control cows. Affected cows had a complete nonarticular spiral fracture of the humerus. Histologically affected humeri had a thicker growth plate with abnormal architecture, thinner cortex with increased abnormal resorption, increased resorption in the distal humerus, decreased trabecular density, abnormal trabecular architecture, presence of growth arrest lines and woven bone formation. Histomorphometry showed reduction in bone volume, trabecular perimeter, and trabecular width. Cows grazed on fodder beet had thicker growth plates with an abnormal appearance compared with cows grazed on pasture, and cows with low/marginal liver copper concentration had more resorption cavities in the distal humerus and thinner cortical bone compared with cows with adequate liver copper concentration. Decreased trabecular density (OR = 249.5), abnormal cortical resorption (OR = 54.2), presence of woven bone formation in the proximal metaphysis (OR = 37.2), and the number of resorption cavities in the distal humerus were significantly associated with a high probability of fracture. Ribs had enlargement of the costochondral junction with fractures in different stages of healing. Histology of the ribs revealed abnormal growth plate appearance, presence of fracture lines, callus tissue, fibrosis, and microfractures. Cows with humeral fracture have osteoporosis due to decreased bone formation and increased bone resorption, likely associated with inadequate feed quality and perhaps copper deficiency leading to a reduction in bone strength and fracture.

Objective: To report the configuration, risk factors, fixation methods and complication rates after repair of humeral condylar fractures (HCF) in French bulldogs, and report the presence of humeral intracondylar fissures (HIF) in this population as a possible predisposing factor., Study Design: Retrospective clinical cohort study., Sample Population: Forty-four elbows., Methods: The medical records of dogs referred between January 2012 and December 2021 were searched for French bulldogs presenting with HCF. Signalment, fracture configuration, stabilization method and complication occurrence were obtained. Postoperative radiographs were assessed for implant positioning, and computed tomography (CT) scans were assessed for the presence and size of HIF in the contralateral elbow., Results: Lateral humeral condylar fractures represented 28/44 (63.6%) of HCF in French bulldogs. Repair with a transcondylar screw (TCS) and Kirschner-wire(s) (K-wire) were 7.62 times more likely to result in a major complication (95% CI: 1.43, 21.89; p = .01) compared to other methods. All incidences (7/7) of TCS migration were within the TCS + K-wire group. A HIF was identified in 18/31 (58.1%) dogs. Older animals were not significantly less likely to have a HIF than younger animals (p = .129)., Conclusions: Fracture stabilization with a TCS and K-wire(s) was associated with an increased risk of major complications and migration of the TCS. A HIF was present in the contralateral elbow of over half of the French bulldogs where CT was available., Clinical Significance: A HIF may be a predisposing factor of HCF in French bulldogs. Background: To investigate the incidence of implant-related complications when humeral unicondylar fractures in immature dogs are stabilised with a transcondylar bone screw and epicondylar Kirschner wire (K-wire)/pin., Methods: Records of cases treated by the author in this clinic over a 10-year period were screened for those involving humeral condylar fractures. These were then reduced to include those patients who were less than 7 months of age with unicondylar fractures and follow-up radiographs., Results: Thirty fractures in 29 dogs met the inclusion criteria. All of these had been stabilised with a transcondylar bone screw and epicondylar K-wire. Fracture healing was documented in all dogs. Surgical site infection was not recorded. Implant-related major complications were reported in four (13%) dogs and successfully treated by either implant removal or replacement. Short-term outcome was judged as excellent in 23 out of 30 and good in seven out of 30 of these fractures., Conclusion: The use of a transcondylar bone screw in combination with an epicondylar K-wire is an appropriate fixation method for humeral unicondylar fractures seen in puppies less than 7 months of age., (© 2022 British Veterinary Association.)

Case History: Medical records from a single veterinary teaching hospital in Giessen, Germany were retrospectively reviewed for dogs presenting with medial or lateral humeral condylar fractures (HCF) between 2007 and 2019. Data regarding surgical technique and complications were collected from the medical records. These were supplemented through information obtained from telephone interviews with referral veterinarians and from an owner questionnaire completed ≥12 months after surgical repair, which also provided data regarding the outcome., Clinical Findings: Eighty dogs with 85 HCF were identified: 13 (15.3%) HCF were medial (bilateral in two dogs) and 72 (84.7%) were lateral (bilateral in three dogs). French Bulldogs (n = 19/80; 23.8%) were most commonly affected. Patients were predominantly skeletally immature and light-weight, with a median age at the time of presentation of 3 (min 2, max 118) months and with a median body weight of 6.4 (min 1, max 46) kg. There were 38 female (47.5%) and 42 male (52.5%) dogs. Fractures developed most frequently secondary to minor trauma (67/77; 87.0%). Dogs were presented in 35/84 (41.7%) cases more than 24 hours after fracture occurrence., Treatment and Outcome: Surgical treatment was performed in 80/85 (94.1%) HCF. An open reduction and internal fixation approach was chosen in all cases. A transcondylar screw (TS) combined with a supracondylar (SC) K-wire (67/80; 83.8%) was the most frequently used fixation technique. Considering all fracture fixation methods, complications (26/80; 32.5%) were classed as minor in 10 (12.5%), major in 14 (17.5%) and catastrophic in two (2.5%) of the 80 surgically treated HCF. Long-term outcome was excellent in 68.6% (24/35 HCF) and very good in 22.9% (8/35 HCF) of the cases for which follow-up information was obtained. Additionally, owner information revealed that 85.7% of dogs (30/35 HCF) were free of lameness in the long-term., Clinical Relevance: This case series demonstrates that surgical repair of lateral and medial HCF with a TS and SC K-wire is a viable option to consider in skeletally immature and light-weight patients. Complications occur frequently after surgical fixation of HCF, but owners can expect a very good to excellent long-term outcome in the majority of cases.

Objectives: The objective of this study was to report outcome and postoperative complications following stabilisation of humeral condylar fractures in skeletally immature French bulldogs with a transcondylar screw combined with locking or hybrid locking plates., Materials and Methods: Medical records from one referral hospital were reviewed to identify skeletally immature French bulldogs with humeral condylar fractures treated with a transcondylar screw and epicondylar locking or hybrid locking plates crossing the distal humeral physis., Results: Forty-five fractures in 41 different dogs with a mean age of 4 months (range 3.5 to 5) were identified. Six cases had complications: two (4.4%) minor and four (8.9%) major. Short-term clinical outcome was excellent in 35 (77.8%), good in nine (20%) and poor in one (2.2%) case. Forty-one of 45 fractures reached radiographic union without further surgical intervention; the remaining four cases reached union following revision surgery. Long-term owner assessed outcome by telephone interview was graded as excellent in all available cases (26 of 41 dogs)., Clinical Significance: This study suggests that the stabilisation of humeral condylar fractures in skeletally immature French bulldogs with combinations of a transcondylar screw and epicondylar locking or hybrid locking plates crossing the distal humeral physis was safe and led comparable outcomes and complication rates to previous reports., (© 2022 British Small Animal Veterinary Association.)

Objectives: The aims of this study were to describe the type, presentation and prognostic factors of feline humeral fractures over a 10-year period and to compare three stabilisation systems for feline humeral diaphyseal fractures., Methods: In total, 101 cats with humeral fractures presenting to seven UK referral centres between 2009 and 2020 were reviewed. Data collected included signalment, weight at the time of surgery, fracture aetiology, preoperative presentation, fixation method, surgical details, perioperative management and follow-up examinations. Of these cases, 57 cats with humeral diaphyseal fractures stabilised using three different fixation methods were compared, with outcome parameters including the time to radiographic healing, time to function and complication rate., Results: The majority of the fractures were diaphyseal (71%), with only 10% condylar. Of the known causes of fracture, road traffic accidents (RTAs) were the most common. Neutered males were over-represented in having a fracture caused by an RTA ( P  = 0.001) and diaphyseal fractures were significantly more likely to result from an RTA ( P  = 0.01). Body weight had a positive correlation ( r  = 0.398) with time to radiographic healing and time to acceptable function ( r  = 0.315), and was significant ( P  = 0.014 and P  = 0.037, respectively). Of the 57 humeral diaphyseal fractures; 16 (28%) were stabilised using a plate-rod construct, 31 (54%) using external skeletal fixation and 10 (18%) using bone plating and screws only. Open diaphyseal fractures were associated with more minor complications ( P  = 0.048). There was a significant difference between fixation groups in terms of overall complication rate between groups ( P  = 0.012). There was no significant difference between fixation groups in time to radiographic union ( P  = 0.145) or time to acceptable function ( P  = 0.306)., Conclusions and Relevance: All three fixation systems were successful in healing a wide variety of humeral diaphyseal fractures. There was a significantly higher overall complication rate with external skeletal fixators compared with bone plating; however, the clinical impact of these is likely low.

The objectives of this retrospective study were 1) to characterize humeral fractures in raptors at a rehabilitation program, 2) to determine risk factors for closed and compound humeral fractures in the raptors, and 3) to investigate prognostic factors for humeral fractures in raptors. Of the 6017 birds of prey admitted over 15 years to the Université de Montréal raptor rehabilitation program, 461 birds presented with a humeral fracture. The highest occurrences of humeral fractures were documented in short-eared owls (33%; Asio flammeus ), Northern hawk-owls (24%; Surnia ulula ), long-eared owls (20%; Asio otus ), and Northern harriers (19%; Circus hudsonius ). Humeral fractures were more common in males, and species from the intermediate weight group (300 g to 1 kg) were more likely to have compound humeral fractures than birds from the lower and higher weight groups. Humeral fractures of the middle and distal third of the bone were diagnosed as compound more often than fractures of the proximal third. A correlation between the occurrences of humeral fractures and the relative average width of the humerus for each species was observed and suggests that species with slender humeri have an increased likelihood of compound humeral fractures. Following the triage criteria used at this rehabilitation program, surgical (119 birds) or nonsurgical (77 birds) treatments were attempted. Release rate for birds without concomitant conditions in which a treatment was attempted was 39.3% (59/150) and did not significantly differ between closed and compound fractures, which suggests that the triage criteria used for compound fractures in this program were adequate. Our findings emphasize that raptors should not be systematically euthanatized when presented with compound humeral fractures to a rehabilitation facility.

Objective: To report the incidence of medial epicondylar fissure fracture (MEFF) after medial-to-lateral transcondylar screw placement in dogs with humeral intracondylar fissure (HIF) and to identify risk factors for MEFF., Study Design: Retrospective study., Sample Population: Seventy-four client-owned dogs (88 elbows)., Methods: Medical records of dogs surgically treated for HIF, and postoperative imaging studies were reviewed for demographics, fracture characteristics, and repair techniques. The width of the transcondylar screw was expressed relative to the height of the condyle. Screw angle and degree of countersinking were recorded. Information from case records and follow-up radiographs were used to identify complications., Results: Medial epicondylar fissure fracture was identified in 10 elbows (11.4%) following medial-to-lateral transcondylar screw placement: 4 cases were detected intraoperatively, 2 on immediate postoperative radiographs, 1 during routine radiographic follow up, and 3 when radiographs were reviewed for this study. A larger relative screw size was found to increase the risk of MEFF (P = .004, OR = 1.5). Fifteen additional complications were identified in 13/80 elbows at a median of 6 weeks postoperatively (range 1-56 weeks). Screw loosening was the most frequent complication (n = 9) and was the only complication in dogs with MEFF (n = 3); MEFF tended to increase the risk of perioperative screw loosening (P = .06)., Conclusion: Medial epicondylar fissure fracture occurred in 10/88 elbows treated for HIF and was more common in elbows treated with a larger screw size relative to the height of the condyle., Clinical Significance: Placing transcondylar screws with a diameter inferior to 41% of the height of the condyle is recommended to avoid MEFF. The objective of this study was to compare the strength and stiffness of various fixation methods applied to the long bones of the rabbit forelimb. Twenty rabbit radius/ulna and 20 rabbit humeri were randomly assigned to 1 of 4 groups. Control bones remained intact, whereas all others were osteotomized to create fracture models that were fixated with locking plate and locking screws (LP), veterinary cuttable plate (VCP) with cortical screws, or external skeletal fixator constructs (ESF), and tested in 4-point bending until failure. Load/deformation curves were generated for each sample and used to calculate stiffness (slope of the curve) and strength (load to failure). Intact controls had greater strength and stiffness than any fixation techniques in the rabbit radius/ulna and humeri samples. Locking plate and VCP constructs had greater stiffness than ESF when applied to the radius, whereas locking plate constructs were stronger than VCP or ESF when applied to the humerus. Overall, the LP construct had characteristics most closely resembling those of the intact control regarding strength in the humerus. Objective:  The aim of this study was to investigate the prevalence, breed predisposition and fracture conformation of humeral condylar fractures (HCF) over a 10-year period. Results were compared with published studies emanating from the United Kingdom exploring effect of breed on HCF., Methods:  Data for all canine admissions to the Veterinary Teaching Hospital at Kansas State University were extracted for the period January 2010 to October 2020. Humeral fractures were recorded and further subclassified as medial, lateral and 'T'/'Y' condylar fractures. The associations between HCF and subtypes with breed were assessed using univariate logistic regression with a comparison group. Multivariable logistic regression was used to evaluate the effect of breed while accounting for dog age, sex and neuter status., Results:  Of the 44,952 canine patients seen during the study period, period prevalence (95% confidence interval [CI] for HCF was 0.26% [0.22, 0.31]). After adjustment for age and neuter status, French Bulldogs were 49 times more likely to be diagnosed with a HCF compared with the comparison breed group (odds ratio [OR], 49.0; 95% CI, 26.9-89.3). After adjustment for age and neuter status, Cocker Spaniels (OR, 42.8; 95% CI, 16.8-108.6), Boston Terriers (OR, 22.9; 95% CI, 11.0-47.9) and Brittany Spaniels (OR, 21.5; 95% CI, 7.3-63.1) had the next highest increase in HCF compared with the comparison group., Conclusion:  Based on a study population from the United States, French Bulldogs were 49 times more likely to be diagnosed with a HCF compared with the comparison breed group., Competing Interests: None declared., (Thieme. Objective:  French Bulldogs are predisposed to humeral condylar fractures. Computed tomography (CT) in English Springer Spaniel elbows has allowed identification of humeral intracondylar fissures (HIF), which can cause lameness and predispose to condylar fractures. This study aimed to evaluate CT characteristics of non-lame French Bulldog elbows, to determine the presence of underlying elbow disease., Study Design:  Retrospective evaluation of CT images from elbows of non-lame French Bulldogs from scans obtained for soft tissue or neurological evaluation was performed. Images were evaluated for the presence of HIF and other elbow abnormalities: condylar and supracondylar remodelling, humeral condylar sclerosis, elbow incongruency, medial coronoid process disease and elbow osteoarthritis. Descriptive statistics were performed., Results:  Computed tomographic scans from 37 dogs (74 elbows) were reviewed. No HIF were identified, although 28/74 elbows had an intracondylar sclerotic band. Medial coronoid process disease was noted in 16/74 elbows, and small osteophytes were present in 21/74 elbows, suggesting a degree of subclinical elbow disease. Widening of the caudolateral aspect of the radial incisure was identified (37/74) as a breed normal. Elbow incongruency was otherwise rare., Conclusion:  Non-lame French Bulldogs showed a low prevalence of occult elbow disease but a proportion had radial incisure widening. Objective:  Application of extended computed tomography scale (ECTS) reconstruction to diagnose metal implant failure has been described in a single case report. The purpose of this study was to compare the features and visibility of humeral transcondylar screw fractures in standard CT scale (SCTS) and ECTS images., Study Design:  Case series: CT images of dogs with fractured transcondylar screws were retrospectively reviewed and described in both SCTS and ECTS images., Results:  Five dogs with a total of six transcondylar screw failures (five right and one bilateral) were reviewed. All cases had an ongoing humeral intercondylar fissure with varying degrees of stress remodelling. The fracture was seen in all screws on ECTS images, however only in three implants on SCTS images. The measured fracture gap was larger in ECTS images in all cases (range: + 0.14 mm to + 0.28mm). The three smallest fracture gaps were not seen on SCTS images. A subtle hypoattenuating streak (artefact) was visible adjacent to the screw fracture in 5/6 of cases using SCTS images. All screw fractures occurred parallel and often slightly medial to the humeral intercondylar fissure., Conclusion:  Implant failure is only seen with larger fracture gaps in SCTS images, with 3/6 screw fractures not visible in SCTS compared with ECTS. Background: French bulldogs have previously been reported to be at an increased risk of developing humeral condylar fractures (HCF). Humeral intracondylar fissures (HIF) are thought to be a predisposing factor for HCF occurrence in skeletally mature dogs particularly when the fracture occurs during normal activity. We hypothesised that HIF may be a predisposing factor for HCF in French bulldogs., Methods: A retrospective case series of nine French bulldogs that had sustained HCF and where CT was performed prior to surgery., Results: Five of the fractured humeri had sclerosis adjacent to the fracture site. Six of the contralateral intact humeri were diagnosed with HIF on the basis of a partial (2/6) or complete (4/6) hypoattenuating area extending between the distal articular surface and the supratrochlear foramen in the sagittal plane, with adjacent sclerosis (6/6)., Conclusions: Recognition of sclerosis at the intracondylar fracture surface on CT can prompt suspicion that HIF preceded HCF. Objective:  The aim of this study was to report the functional outcome and complications following transcondylar screw and epicondylar plate fixation of skeletally mature dogs with fractures of the lateral aspect of the humeral condyle using a string of pearls, limited contact dynamic compression plate (LC-DCP) or locking compression plate (LCP)., Study Design:  This study was a retrospective review of clinical records, radiographic images and canine brief pain inventory evaluation with a supplementary non-validated questionnaire., Results:  Thirty-one fractures in 29 dogs were included. The majority of dogs were Spaniels (23/29). Thirty fractures had evidence of humeral intracondylar fissure. Sixteen fractures had string of pearls, eight LCP and seven LC-DCP. There were two minor complications and three major complications. Perioperative (median: 6 weeks, range: 6-12) outcome was considered fully functional for 24 fractures and acceptable for 6 fractures. One dog had an unacceptable outcome due to the failure of an LCP that healed following revision surgery. Fractures with radiographic follow-up had a persistent visible intracondylar fracture line in 20/26. Twenty-six of twenty-seven owners perceived an excellent outcome (median: 135 weeks, range: 25-246)., Conclusion:  Repair of fractures of the lateral aspect of the humeral condyle with a transcondylar screw and epicondylar plate has a good perioperative, mid- and long-term outcome with 10% major complication rate, despite incomplete or non-union of the intracondylar fracture in 20/26 elbows., Competing Interests: None declared., (Thieme. Objectives: To describe the surgical technique and complications for fluoroscopically guided transcondylar screw placement for humeral intracondylar fissure in dogs., Materials and Methods: A retrospective review was undertaken of cases from two hospitals where identical surgical technique was employed. Factors were analysed for any association with postoperative complications., Results: Sixty-two dogs (82 elbows) were reviewed for which the postoperative complication rate was 45%; a total of 15% of cases required revision surgery. Complications were more likely in cases operated on earlier in the case series and with increasing dog bodyweight. Objective: To compare the axial biomechanical properties of intracondylar humeral osteotomies fixed with 4.5 mm transcondylar positional or cortical lag screws., Study Design: Ex vivo study., Sample Population: Paired humeri from 21 canine cadavers., Materials and Methods: An intracondylar osteotomy was created on each humerus to mimic an intracondylar fissure. Paired humeri were randomly assigned to fixation with a positional or a lag screw. All specimens were radiographed postinstrumentation to document proper screw placement. Axial load was applied to the distal articular surface of the trochlea at a rate of 1 mm/s until a 40% decrease in load was measured. Specimens were assessed for mode of failure with visual inspection and radiographs., Results: Stiffness (1236.7 ± 181 N/mm vs. 1050.8 ± 265 N/mm), yield load (3284.3 ± 1703 N vs. 2071.1 ± 740 N), and maximum load (7378.0 ± 1288 vs. 5793.7 ± 2373 N) were greater in constructs fixed with a positional rather than a lag screw (p = .0008, .044, and .040, respectively)., Conclusion: In our model, mechanical properties were improved when the transcondylar osteotomy was stabilized with a 4.5 mm positional screw rather than a lag screw., Clinical Significance: This ex vivo study suggests that a transcondylar lag screw and positional screw are not biomechanically equal. Objective: To determine the influence of interlocking screw threads on the biomechanical properties of repaired canine humeral condylar fractures., Study Design: Ex vivo biomechanical study., Sample Population: Thirty-six humeral condyles., Methods: Simulated fractures of the lateral aspect of the humeral condyle were stabilized by a 3.5 mm interlocking thread screw (ITS) or 3.5 mm buttress thread screw placed in lag (BTS-L) or positional fashion (BTS-P) and axially loaded at a walk, trot, 2-mm displacement, and failure cycles. Compact flute drill bits (CFBs) were used for ITS constructs and standard flute drill bits (SFB) for BTS constructs. The effects of bit type on drilling parameters and screw type on screw insertion properties and fragment stability were assessed., Results: CFB produced a 6°C greater temperature increase (p = .042) and required 20 N higher torque (p = .003) than SFB. Insertional torque was greater for ITS than BTS-P (p = .001) and BTS-L (p = .001). Condylar fragment rotation at failure was lower in ITS (lsmean ± SE, 8.3° ± 1.9°) than BTS-L constructs (14.5° ± 2.3°, p = .011). ITS resisted greater loads (1503 ± 105 N) than BTS-P (1189 ± 99 N, p = .038) but not BTS-L (1249 ± 123 N, p = .121) constructs., Conclusion: Biomechanical performance of constructs was improved with ITS rather than BTS fixation., Clinical Significance: ITS can be considered for stabilization of humeral condylar fractures in adult dogs., (© 2021 American College of Veterinary Surgeons.)

The aim of this study was to identify possible predisposing anatomical factors associated with humeral fractures in birds of prey through comparison of specific anatomical features in different raptor species. An anatomical study of bone features in birds of prey was performed on 3 male subjects from 5 different species. The selected species included in this investigation were 3 diurnal species (the common buzzard [ Buteo buteo ], the peregrine falcon [ Falco peregrinus ], and the European honey-buzzard [ Pernis apivorus ]) and 2 nocturnal species (the barn owl [ Tyto alba ] and the tawny owl [ Strix aluco ]). Humeral bone samples were tomographically analyzed with a micro-macro-focus computed tomographic machine. Specific humeral anatomical points were selected (foramen pneumaticum and tuberculum dorsale for the proximal humerus; corpus humeri for the diaphyseal humerus; and above the condylus dorsalis for the distal humerus) to measure foramen pneumaticum diameter (in millimeters), cortical thickness (in millimeters), and trabeculae number and sizes (in millimeters). Apparent density, measured with the Hounsfield unit, was used to assess the degree of bone resistance. Statistical analysis was performed with a Spearman's correlation, and significance was set at P < .05. The differences among the observed bone volumes were highly significant ( P = .00). Trabeculae number and the humeral anatomical point measurements showed differences in all 5 avian species investigated. However, those differences were not significant, except at the condylus dorsalis ; in which, a significant interspecies difference ( P = .002) was recorded. Trabecular size, cortical thickness, bone density, and diameter of the foramen pneumaticum were all different in all raptor species; however, these variations were not significant. The study confirms the existence of humeral bone volume differences between diurnal and nocturnal species. Furthermore, the data suggest that the humeri of peregrine falcons and European honey-buzzards may be stronger than the humeri of common buzzards, tawny owls, and barn owls.

Objective:  The aim of this study was to investigate the effects of an induced incomplete ossification of the humeral condyle (IOHC) lesion on ex vivo canine humeral condylar biomechanics., Study Design:  Nine paired cadaveric elbows were collected from mature dogs weighing between 20 and 25 kg. Left and right limbs were randomized to IOHC or normal groups. Limbs were prepared for mechanical testing; ligaments were preserved and an IOHC lesion was created. Elbows were potted, positioned into a biomaterials testing system at an angle of 135 degrees and axially loaded to failure at a rate of 30 mm/minute., Results:  Induced IOHC lesions reduced peak load ( p  = 0.02) when compared with an intact humerus. There was no difference between stiffness ( p  = 0.36) of intact humeri or humeri with an induced IOHC lesion. An induced IOHC lesion increased ( p  = 0.012) the probability of intracondylar fracture under load., Conclusion:  Cadaveric humeri are weakened by the creation of an intracondylar osteotomy and fractures secondary to induced IOHC are similar to spontaneous humeral condylar fractures. Objective:  Successful stabilization of comminuted supracondylar humeral fractures is challenging, and biomechanical studies are scarce. This study compares double-plate (DB-PLATE) and linear external fixator with an intramedullary pin tie-in (ESF-IMP) fixation techniques in a cadaveric gap model. The hypothesis was the DB-PLATE construct would be stiffer, stronger and more resistant to repeated loading than the ESF-IMP construct in both cyclic and load-to-failure axial compression testing., Study Design:  A 2 cm ostectomy was performed on 10 pairs of canine cadaveric humeri proximal to the supratrochlear foramen. Stabilization was with DB-PLATE ( n  = 10) or ESF-IMP ( n  = 10). Cyclic testing was performed by applying a 200 N load at 2 Hz for 63,000 cycles. Axial compressive load to failure testing followed. Data analysed included dynamic stiffness, stiffness and yield load., Results:  No constructs failed during cyclic testing or lost stiffness over time. Mean dynamic stiffness over the final 100 cycles was greater for DB-PLATE compared with ESF-IMP. Mean stiffness of DB-PLATE in load-to-failure testing was not different than ESF-IMP. Yield load of DB-PLATE was higher than ESF-IMP., Conclusion:  Both DB-PLATE and ESF-IMP survived cyclic testing with no change in dynamic stiffness. DB-PLATE was stronger than ESF-IMP in load-to-failure testing, which may make this construct preferable when prolonged healing or poor patient compliance is anticipated. Objective: To describe medium-term functional outcome after nail osteosynthesis in feline traumatology and report clinically relevant recommendations for I-Loc angle-stable interlocking nail use in cats., Study Design: Prospective clinical study., Sample Population: Client-owned cats (n = 29)., Methods: Consecutive cases with femoral, tibial, or humeral fractures were included. Outcome measures included fracture and surgical procedure description, limb alignment, nail size vs body weight (BW), percentage of nail medullary canal (MC) fill, time to limb function at clinical union (CU), and complications. Descriptive statistics were reported and compared with historical data., Results: Bone distribution was 53.3% femora, 30% tibiae, and 16.7% humeri. There were six epimetaphyseal and 24 diaphyseal fractures. Overall, 67% of fractures were comminuted. Open reduction and minimally invasive techniques were used in 73% and 27% of cases, respectively. Seventeen I-Loc 3 (cat mean BW 4.4 ± 2.2 kg) and 13 I-Loc 4 (cat mean BW 5.2 ± 1.2 kg) nails were placed with mean MC fill of ≤50%. Average time to CU was 7.2 weeks. At CU, lameness had resolved or was mild in every cat, and all cats ultimately regained full limb function. No major complications were encountered., Conclusion: Because of improved CU times, excellent functional outcomes, and low complication rate, our results provide evidence that I-Loc nails are safe and effective for feline traumatology., Clinical Significance: The I-Loc may be advantageous for fixation of epimetaphyseal fractures. Humeral intracondylar fissure (HIF) was first described as incomplete ossification of the humeral condyle. It is now known that the fissure is a stress fracture in some dogs. The descriptive term HIF is therefore preferred. In young dogs an incomplete ossification cause may still be valid. Symptomatic HIF is treated surgically with a transcondylar implant. The aim is to alleviate lameness and avoid condylar fracture. Choosing an appropriate surgical approach and implant can reduce complications. Objectives: To report complications, clinical outcomes and CT-imaging outcomes of a surgical system designed for the management of humeral intracondylar fissures and humeral condylar fractures., Materials and Methods: Retrospective review of fracture healing from medical records, direct owner contact and an online data-submission service. Follow-up included CT scans and a calculated "bone-opacity continuity index" to quantify bone healing., Results: There was one major surgical complication and one major medical complication out of 34 fissure cases, and two major surgical and one major medical complication out of 14 fractures. Follow-up times ranged from 29 to 1268 days. Objectives: The aim of this study is to describe the treatment and outcome of humeral condylar fractures and humeral intracondylar fissures in cats with patellar fracture and dental anomaly syndrome (PADS) and to provide advice on how to manage these cases in practice., Methods: Data were collated on cats with PADS that were reported to have sustained humeral fractures or had fractures or fissures of the humerus identified on radiographs. The details of the fractures were recorded in addition to any treatment and outcome information., Results: Of the 207 cases reported with PADS, 18 cats (8.7%) were found to have humeral condylar fractures, none of which was known to have resulted from significant trauma. Where treatment occurred, it involved the placement of transcondylar positional or lag screws. In some cases additional implants, including supracondylar bone plates and screws or Kirschner wires (K-wires), were used. Follow-up data revealed that only two cats were euthanased owing to the presence of the humeral fractures, with at least eight achieving some degree of recovery of function., Conclusions and Relevance: These humeral fractures all have the characteristics of stress insufficiency fractures, being simple isolated fractures that are short oblique, with increased radio-density at the fracture line and occurring following minimal or no trauma. Humeral intracondylar fissures were identified in two cats and it is possible that some of the other fractures may have occurred secondary to pre-existing fissures. To our knowledge, no prior reports exist of fissures in cats that do not meet the criteria for PADS. Surgical repair primarily consisted of the placement of transcondylar lag or positional screws with, in some cases, adjunct implants such as bone plates and screws or K-wires. Though there were insufficient data to determine the prognosis for these fractures in the long term, unlike patellar fractures, many of these fractures will heal if treated appropriately.

Objective: To assess the influence of humeral stress fracture location on the time to return to racing and postinjury performance of thoroughbred racehorses., Study Design: Retrospective study (1992-2015)., Sample Population: Thoroughbred racehorses (n = 131) that presented for lameness with the sole diagnosis of humeral stress fractures in the lame limb, as determined by scintigraphy or radiology., Methods: Sex, fracture site, age, starts, earnings, and average earnings per race were examined for differences in pre-stress and post-stress fracture diagnosis for the entire population as well as individual stress fracture locations (general linear model, χ 2 tests). Pre-stress and post-stress fracture performance for the three stress fracture locations were assessed: (1) earnings pre-stress and post-stress fracture (Kruskal-Wallis one-way analysis of variance), (2) average earnings per start prefracture, and (3) average earnings per start postfracture (Wilcoxon signed-rank tests)., Results: Stress fracture locations were caudodistal (n = 36), craniodistal (n = 43), and caudoproximal (n = 52). One hundred ten of 131 horses raced postfracture, and 54 of 131 horses raced prefracture. Age at injury was 43.61 months caudodistal, 33.48 months caudoproximal, and 36.65 months craniodistal. Horses returned to race at a median of 244 days (range, 218-272). Postfracture earnings per start were greater for caudodistal vs caudoproximal (P = .04)., Conclusion: There were no differences in prefracture earnings or fracture site and sex or limb affected. Return-to-race time varied by location but not significantly. Differences in earnings preinjury and postinjury were not significant. Objective: To report the surgical management, complications, and short-term outcomes for dogs with humeral Y-T fractures (ie, fractures affecting the medial and lateral aspect of the humeral condyle extending from the articular surface) that had been approached bilaterally (medial and lateral) and stabilized with two locking compression plates (LCP) and to investigate risk factors for major complications in the short term., Study Design: Retrospective case series., Animals: Twenty-eight client-owned dogs., Methods: Medical records of 28 dogs that presented with 29 consecutive Y-T fractures that had been approached bilaterally, reduced, and stabilized by using a transcondylar screw (in lag or positional fashion) and two or three LCP were reviewed. All dogs had at least 6 weeks clinical follow-up, and in some cases, responses from the long-term owner-based University of Liverpool questionnaire for owners of dogs with mobility problems (Liverpool Osteoarthritis in Dogs) was obtained., Results: Twenty-eight dogs (29 fractures) presented with Y-T fractures and met the inclusion criteria. The fractures were reduced and stabilized with bilateral LCP and screws. The French bulldog (n = 7) was the most commonly represented breed. Age ranged from 4 months to 9.5 years (mean, 43.53 ± 34.63 months); seven (25%) dogs were considered skeletally immature. Twenty-seven (96.4%) dogs were considered to have had resolution of lameness at 6 weeks. Major complications were registered in two (7%) dogs; one dog required surgical revision, and one dog had catastrophic complications leading to amputation of the limb. The owner-based questionnaire was available for 18 (64%) dogs., Conclusion: Management of canine Y-T humeral fractures stabilized with bilateral LCP and transcondylar screws led to a low complication rate with a good to excellent long-term outcome according to responses on the client questionnaire. Objectives: To investigate the effect of breed as a risk factor associated with humeral condylar fracture in skeletally immature dogs in the UK., Materials and Methods: Retrospective study of dogs under 12 months of age that were presented with humeral condylar fracture to three specialist referral centres between 2015 and 2018. Data retrieved from medical records included breed, age, gender, neuter status, affected limb, fracture configuration and aetiology of the fracture. Breed population percentages were compared with those recorded by the UK Kennel Club., Results: Of the 115 dogs with 118 fractures, French bulldogs (41%) and English springer spaniels (15%) were overrepresented: humeral condylar fractures were more commonly diagnosed in French bulldogs (odds ratio = 5.86) and English springer spaniels (odds ratio = 5.66) compared with mixed-breed dogs. Lateral condylar fractures occurred in 70% of cases, with medial condylar fractures and Y/T fractures accounting for 9% and 21%, respectively. This study evaluated the feasibility, complications and long-term outcomes of using a cannulated drill system combined with intraoperative imaging to place a transcondylar screw for the management of canine humeral intracondylar fissure. Thirteen dogs were enrolled, with one dog undergoing staged bilateral surgery. No intraoperative complications occurred. Five minor (36%) and three major (21%) postoperative complications occurred, giving an overall complication rate of 57%. None of the screws placed penetrated the articular surface. The mean duration of surgery was 28 min (SD ±3.5) for dogs that developed a major complication versus 46 min (SD ±18.1) for those that did not (p=0.015). The duration of preoperative lameness was significantly shorter for cases which suffered a major complication (2 days; SD ±2.8) than those that did not (34 days; SD ±31.7, p=0.008). None of the variables assessed were significantly associated with minor complications. Median time from surgery to last follow-up was 5.8 years (range 3.5-8.5 years). Median Liverpool Osteoarthritis in Dogs questionnaire score at the final point of follow-up was 16 (range 7-27). Objective: To report the prevalence, configuration, risk factors, fixation methods and outcomes after repair of humeral condylar fractures (HCF) in dogs., Study Design: Retrospective nested cohort study., Sample Population: One hundred twelve dogs., Methods: Medical records of dogs referred between January 2010 and August 2018 were searched for HCF. Demographics, fracture configuration, repair, and complications were obtained from medical records. Radiographs were assessed for fracture reduction, implant positioning, and bone healing. Short-term radiographic and clinical outcomes, and long-term owner-assessed outcome was determined. Associations between these variables were statistically analyzed., Results: Dogs with HCF represented 112 of 43 325 (0.26%, 95% CI 0.22-0.31) referrals. French bulldogs and spaniel breeds were predisposed to HCF (P < .02). French bulldogs were 6.58 times (95% CI 1.62-26.7) more likely than other breeds to have a medial HCF (P = .008). Epicondylar plate fixation was associated with reduced complications compared with lag screws and Kirschner wires (P = .009). Lameness was scored as 1 of 5 (median) in the 85 dogs with initial follow-up (median 6 weeks) after HCF repair. Outcome was considered excellent in 26 of 31 dogs with long-term follow-up (median 36 months)., Conclusion: French bulldogs and spaniels were predisposed to HCF, and medial HCF were more common in French bulldogs. Epicondylar plate fixation was associated with reduced complications., Clinical Significance: French bulldogs are predisposed to HCF, including medial HCF. Objectives:  The aim of this study was to describe the use of locking compression plates (LCP) in Y-T humeral condyle fractures and to evaluate their clinical outcome., Methods:  This study involved a retrospective review, including clinical, radiographical and canine brief pain inventory outcome evaluation., Results:  Eighteen consecutive dogs met the inclusion criteria, and 15/18 were considered to have humeral intracondylar fissure. Twelve of 18 dogs had simple fractures, and the remaining six had comminuted fractures. Postoperative radiographs revealed accurate intracondylar reconstruction (articular step defect [ASD] < 1 mm) in 17/18 of patients. Short-term outcome was considered fully functional in 9/13 and acceptable in 3/13 patients. Complications were diagnosed in 2/13; infection in one with resolution after antibiotic treatment, and one case of implant failure. Nine of 18 owners provided postoperative questionnaire responses (median 25; range: 14-52 months) and 8/9 clients perceived the treatment to have resulted in an excellent overall outcome., Clinical Significance:  Repair of Y-T humeral fractures with LCP allowed for hybrid fixation and monocortical screw placement in distal fracture fragments. There was no significant ASD at the intracondylar fracture line in most cases. Objective: To describe a minimally invasive surgical technique for the treatment of comminuted humeral bicondylar (also known as T-Y) fractures in dogs and report outcome in 3 cases., Study Design: Surgical technique description and case report., Animals: Three consecutive dogs with traumatic comminuted humeral T-Y fractures., Methods: Dogs that presented with a traumatic humeral T-Y fracture were prospectively included. The condylar component of the fractures was reduced first by using a combination of percutaneous forceps and K-wires. A transcondylar screw was applied, and epicondylar pins were preplaced medially and laterally. The metaphyseal component of the fracture was then reduced and stabilized with the pins. Finally, a bone plate was applied percutaneously along the medial humeral surface. Intraoperative fluoroscopy was used throughout surgery in all cases. Postoperative alignment in the frontal and sagittal planes was compared to the contralateral limbs. Time to clinical union, clinical outcome, and complications were recorded., Results: Three dogs were included. Body weight ranged from 5.9 to 40 kg. Incomplete ossification of the humeral condyle was identified as the primary pathology leading to the fracture in 2 cases. All cases experienced uncomplicated healing with good to excellent functional recovery., Conclusion: The reported technique was associated with good outcomes without complications in 3 cases., Clinical Significance: This study provides evidence that minimally invasive plate osteosynthesis represents an alternative to open reduction and internal fixation in the treatment of comminuted humeral T-Y fractures., (© 2019 The American College of Veterinary Surgeons.)

Long bone fractures in racehorses may present as stress fractures which have a good prognosis, or complete fractures, which often result in a fatal outcome. In order to identify differences in modifiable management practices that may contribute to these outcomes, racing histories of horses with humeral or tibial fractures and of matched controls were examined. A retrospective case-control study of Australian Thoroughbred racehorses diagnosed with a fracture of the humerus or tibia by scintigraphy or at post-mortem between 2002 and 2016 was undertaken. Control horses were matched from the same race or trial on age and sex. Statistical analysis was performed using conditional logistic regression, χ 2 and Mann-Whitney U tests. More humeral fractures than tibial fractures were fatal (12/47, 26% vs. 3/35, 8.6%, P = 0.049). No differences in pre-injury racing histories were observed between cases and controls for humeral and tibial fractures. Both humeral and tibial fracture case horses were younger than the registered Thoroughbred racing population (P < 0.001), but horses sustaining humeral fractures were older than those with tibial fractures (3.3 ± 0.9 vs. 2.8 ± 0.8 years, P = 0.005) yet raced fewer times prior to the injury (0.5 ± 1.1 vs. 1.3 ± 1.7 races, P = 0.009). Horses with fatal humeral fractures were less likely to have raced than those with non-fatal humeral fractures (16.7% vs. 55.6%, P = 0.02). In conclusion, tibial and humeral fractures occur in young racehorses, and humeral fractures are more likely to be fatal in those with the least exposure to trialling and racing., (Copyright © 2019.
