2. Comparison of Spinal, Low-Dose Spinal and Epidural Anesthesia With Ropivacaine Plus Fentanyl for Transurethral Surgical Procedures

The aim of This study was to compare spinal, low-dose spinal, and epidural anesthesia using ropivacaine and fentanyl combinations for transurethral surgical procedures. Sixty patients with American Society of Anesthesiologists scores of I–III were allocated into three groups. After pre- loading with 5 mL/kg normal saline, patients in the spinal anesthesia group (Group S) received 15 mg of hyperbaric ropivacaine plus 25 μg of fentanyl intrathecally; patients in the epidural anesthesia group (Group E) received 112.5 mg of ropivacaine plus 25 μg of fentanyl epidurally via an epidural catheter; and patients in the low-dose spinal anesthesia group (Group L) received 10 mg of hyperbaric ropivacaine plus 25 μg of fentanyl intrathecally. Blood pressure, heart rate, peripheral oxygen saturation, time to onset of thoracic (T)-10 dermatome, two-segment sensorial block regression time, full recovery of sensorial block, maximum motor blockade levels, motor blockade regression time, additional analgesic administration, patient comfort, and complications were recorded. The time to the onset of T10 dermatome level was shortest in Group S and longest in Group E (p < 0.001). The sensorial blockade time and motor blockade regression time were shorted in Group L (p < 0.001). The two-segment sensorial block regression time in Group E exceeded that in the other groups. Additional analgesic administration was not needed in any group. No complications or adverse effects were observed in any patient. We conclude that all three anesthetic techniques may be used safely and are appropriate for transurethral surgical procedures. However, low-dose spinal anesthesia with ropivacaine plus fentanyl may be preferable in transurethral surgery because we reach an adequate sensorial level with less motor blockade.

4. Secondary Infection and Co-infection in COVID-19 Patients Receiving Tocilizumab

Objective: Tocilizumab (TCZ) is a recombinant humanized anti-IL-6 receptor monoclonal antibody that is beneficial in critically ill COVID-19 patients. However, the clinical efficacy and safety of immunosuppressants (including tocilizumab, sarilumab and anakinra) in COVID-19 patients are not yet known. These treatments may predispose patients to infection. The aim of this study was to find any connection between the use of tocilizumab and increased secondary bacterial infections. Materials and Methods: In this study, we conducted retrospective analyses of secondary bacterial infections in COVID-19 patients in the intensive care unit. This study included patients with laboratory-confirmed COVID-19 infection or clinically and radiologically confirmed COVID-19 infections who were admitted to the university hospital adult ICUs between March 2020 and January 2022. Demographic data, recent exposure and travel history, clinical symptoms or signs, laboratory findings, and comorbidities were recorded. Microbial cultures from tracheal aspirates, blood, and urine were obtained at admission and throughout the hospital stay. The patients who received tocilizumab treatment noted and analyzed for seconder infections. Blood cultures were taken at least 48 h after the first dose of tocilizumab. Results: We found that 80 patients (%37) had positive culture samples at admission, and most of these cases were admitted to the ICU from various hospital wards. The analyzed data showed that the tocilizumab group had a higher incidence of positive culture samples (%75 vs %35, p=0,0001). The results showed that culture of tocilizumab taken patients had more incidence with metisilin resistance S. aureus, Klebsiella spp., and Acinetobacter spp. (p=0,0001). Infection and mortality rates were much higher than those in the usual care group. Conclusion: Secondary infections and sepsis are major risk factors for mortality. The pathogens detected were drug resistant and had a lower chance of treatment. The benefit of tocilizumab treatment was lost in these patients because of secondary infections. Future studies are needed to help determine the risks of tocilizumab treatments.

7. Spontaneous Pneumothorax and Pneumomediastinum in Covid-19 Pneumonia

Fifteen to twelve percent of COVID-19 patients who had mild to severe semptoms were hospitalized. Most of hospitalized patients had respiratory failure and admitted to the intensive care unit with the diagnosis of acute respiratory distress syndrome (ARDS). Pneumothorax (PX) and pneumomediastinum (PM) which can be seen as travmatic or spontaneous in intensive care unit (ICU) patients have a higher incidence in COVID-19 patients. Patients that admitted to ICU between April 2020 and November 2021, were reviewed for development of pneumothorax, pneumomediastinum and subcutaneous emphysema retrospectively. There was 13 patients who were developed these conditions in the study period. Five of them was male and median age for all patients was 62. Eight of them were intubated and mechanically ventilated. Only two of them discharged from ICU. The most common reason for death was sepsis due to secondary infections. Patients followed with physical examination, chest XR and computerized tomography (CT) if needed. Acute deterioation, increased oxygen demand and crepitation in the physical examination should be alarming findings in COVID-19 patients for PX and PM. Early management of these symptoms can be life saving.

Objective: The benefit of medical interventions applied in terminal patients who have no hope of recovery and whose death is thought to be approaching is still a controversial issue. In this study, it was aimed to analyze the terminal stage patients hospitalized in the intensive care unit. Materials and Methods: The general characteristics and laboratory results of 61 end-stage patients admitted to the adult intensive care unit of university hospital between December 2016 and may 2017 were examined. Life expectancy, mortality rates, factors affecting mortality, and costs per patient were evaluated according to the palliative prognostic index (PPI) and palliative care admission score (PCAS) of the patients. Objective:Severe pneumonia is an important cause of mortality in coronavirus disease-2019 (COVID-19) infection. This study evaluated pulmonary gas exchange and mortality according to nasal cannula, high flow nasal cannula, non-invasive positive pressure ventilation and invasive mechanical ventilation used for treating hypoxemic respiratory failure in severe COVID-19 patients.Materials and Methods:Hundred and forty severe COVID-19 patients who were treated in Department of Anesthesiology Intensive Care Unit of Pamukkale University Hospital were reviewed retrospectively. Arterial blood gas results on the 1st, 3rd, 7th, 14th, and 21st days of admission were recorded.Results:The alveolo-arterial oxygen gradient of the patients receiving oxygen via nasal cannula was significantly lower than the patients receiving high-flow nasal cannula, non-invasive and invasive mechanical ventilation. When the other groups were compared, no significant difference was observed between the alveolo-arterial oxygen gradients of the patients who received high-flow nasal cannula, non-invasive and invasive mechanical ventilation support. We observed that the mortality of the patients who were admitted as intubated was significantly higher than the other groups.Conclusion:Invasive and non-invasive oxygen support resulted in similar alveolo-arterial oxygen gradients in severe COVID-19 patients at admission and at 3-week follow-up, but mortality was higher in intubated patients.

Methods: During the 6-year study period, using Centers for Disease Control and Prevention National Healthcare Safety Network (CDC-NHSN) definitions for device-associated health care-associated infection (DAHAI), we collected prospective data from861,284 patients hospitalized in INICC hospital ICUs for an aggregate of 3,506,562 days. Results: Although device use in INICC ICUs was similar to that reported from CDC-NHSN ICUs, DA-HAI rates were higher in the INICC ICUs: in the INICC medical-surgical ICUs, the pooled rate of central lineassociated bloodstream infection, 4.1 per 1,000 central line-days, was nearly 5-fold higher than the 0.8 per 1,000 central line-days reported from comparable US ICUs, the overall rate of ventilator-associated pneumonia was also higher, 13.1 versus 0.9 per 1,000 ventilator-days, as was the rate of catheterassociated urinary tract infection, 5.07 versus 1.7 per 1,000 catheter-days. From blood cultures samples, frequencies of resistance of Pseudomonas isolates to amikacin (29.87% vs 10%) and to imipenem (44.3% vs 26.1%), and of Klebsiella pneumoniae isolates to ceftazidime (73.2% vs 28.8%) and to imipenem (43.27% vs 12.8%) were also higher in the INICC ICUs compared with CDC-NHSN ICUs. Conclusions: Although DA-HAIs in INICC ICU patients continue to be higher than the rates reported in CDC-NSHN ICUs representing the developed world, we have observed a significant trend toward the reduction of DA-HAI rates in INICC ICUs as shown in each international report. Objective:It was aimed to report the Acute Physiology and Chronic Health Evaluation-II (APACHE-II) score, Sequential Organ Failure Assessment (SOFA) score, Glasgow coma scale (GCS), 4C mortality score and the coronavirus disease-2019 (COVID-19) Reporting and Data System (CO-RADS) in predicting the outcome of critically ill COVID-19 patients.Materials and Methods:Patients with laboratory-confirmed COVID-19 infection or clinical and radiological confirmed COVID-19 infection who were admitted to adult intensive care unit (ICU) were included. Clinical characteristics, outcomes, APACHE-II score, SOFA score, International Severe Acute Respiratory and Emerging Infections Consortium/World Health Organization 4C mortality score and CO-RADS classification were reported at admission.Results:Two hundred seventy six patients were included in this study. The mean age was higher in non-survivor patients. The most common cause of hospitalization was respiratory failure (67%). The common co-morbidities were hypertension (51.8%), cardiac disease (43.4%) and diabetes (33.6%). Organ failure was present in 61.5% of the patients. The mean APACHE-II, SOFA, GCS and 4C mortality scores were higher in non-survivor patients. 4C mortality and SOFA scores showed higher predictive accuracy for mortality with an area under the curve 0.736 and 0.706, respectively. 4C mortality had sensitivity of 78.9% and specificity of 58.1% whereas of SOFA had a sensitivity of 78.9% and a specificity of 53.3%.Conclusion:4C mortality and SOFA scores could be a predictors of mortality in COVID-19 patients in the ICU.

Objective: Technological advances increased prolonged life expectancy of the terminal patients, who had end-stage diseases. End-of-life care in intensive care units (ICU) has increased with the rise in admissions of terminal patients to ICU. Our aims in this study were to determine the prevalence of terminal patients, and to find the reasons for potentially inappropriate treatments in ICUs. Materials and Methods: It was nationwide, multicenter, point prevalence and observational study. All adult patients, who stayed more than 48 h in the ICU, were enrolled. All patients were recorded on an electronic case record form, consisting of data on patient demographics, treatments, family participation and mini survey for physicians. The study was conducted on October 15, 2018 with a follow-up for 30 days. Results: Of 1127 patients 286 (25%) ICU patients were diagnosed as terminal patients by ICU physicians depending on primary physician statement. Terminal patients relatives requests and physicians legal concerns reduced end-of-life care quality. Reports of neurological problems are increasing for the clinical presentation of coronavirus disease-2019 (COVID-19). The clinical presentation reported in this study seemed to be a combination of nonspecific complications of the systemic disease, inflammation of the cerebrovascular system, and the effects of a direct viral infection. Creutzfeldt-Jakob disease, a spongiform encephalopathy caused by prions, is characterized by a severe neurological destruction, which has an extremely high mortality. In this publication, we presented a patient who was admitted to the Pamukkale University Anesthesiology Intensive Care Units with the neurological findings that developed after the COVID-19 vaccine (CoronaVac, Sinovac Life Sciences, Beijing, China). The patient died due to the progressive neurological disorders. In cases where rapidly progressive neurological disorders are observed, Creutzfeldt-Jakob disease should be considered and the role of immunity-related conditions in the progression of the disease should be investigated.

15. Evaluation of the Prognosis of Cancer Patients Treated in Intensive Care Units

Objective:The number of cancer cases and mean life expectancy are increasing with improvements in diagnosis and treatment options. This increase creates the need for intensive care units (ICUs) for cancer patients. This study aimed to determine the risk factors associated with admission of cancer patients to the ICU, intensive care prognosis, and mortality rates and cost per patient.Materials and Methods:In this study, we analyzed the data of cancer patients who were followed up in Pamukkale University Faculty of Medicine, Anesthesiology ICU. The variables that were analyzed included intensive care prognosis, mortality rates, risk factors affecting mortality, and cost per patient. ICU patient’s folders, epicrisis reports, and follow-up documents of 251 patients with solid or systemic malignancies who were admitted to the ICU of Pamukkale University Faculty of Medicine, Anesthesiology Department for a period of two-years were analyzed.Results:The most common reason for admission to ICU for patients with solid or systemic malignancies was respiratory failure 63.34% (n=159) followed by sepsis 16.33% (n=41) and cardiac arrest 5.17% (n=13). According to the cancer staging of the patients, 58.5% (n=147) were classified as stage 4 and 16.3% (n=41) as stage 3. The cost per patient per day in the ICU ranged from 186.86 Turkish Liras (TL) to 4407.39 TL and the mean cost was 1628.49±524.12 TL. The mortality rate among these cancer patients was 89.2%.Conclusion:Most of the cancer patients died in the ICU with high medical expenses. When patients are evaluated for admission to the ICU, the patient’s primary physician and ICU physician should reach a consensus as to whether the patient is in a terminal condition or not and whether palliative care should be administered to them.

During the 6-year study using the Centers for Disease Control and Prevention's (CDC) U.S. National Healthcare Safety Network (NHSN) definitions for device-associated health care-associated infection (DA-HAI), we collected prospective data from 605,310 patients hospitalized in the INICC's ICUs for an aggregate of 3,338,396 days. Although device utilization in the INICC's ICUs was similar to that reported from ICUs in the U.S. in the CDC's NHSN, rates of device-associated nosocomial infection were higher in the ICUs of the INICC hospitals: the pooled rate of central line-associated bloodstream infection in the INICC's ICUs, 4.9 per 1,000 central line days, is nearly 5-fold higher than the 0.9 per 1,000 central line days reported from comparable U. S. ICUs. The overall rate of ventilator-associated pneumonia was also higher (16.8 vs 1.1 per 1,000 ventilator days) as was the rate of catheter-associated urinary tract infection (5.5 vs 1.3 per 1,000 catheter days). Frequencies of resistance of Pseudomonas isolates to amikacin (42.8% vs 10%) and imipenem (42.4% vs 26.1%) and Klebsiella pneumoniae isolates to ceftazidime (71.2% vs 28.8%) and imipenem (19.6% vs 12.8%) were also higher in the INICC's ICUs compared with the ICUs of the CDC's NHSN. Background . Embolization during cardiopulmonary bypass probably alters cerebral autoregulation. Therefore, using laser Doppler flowmetry we investigated the cerebral blood flow velocity changes in response to changes in arterial pressure, before and after embolization in a canine bypass model. Methods . After Institutional Animal Care and Use Committee approval, 8 anesthetized dogs had a laser Doppler flow probe positioned over the temporoparietal dura. During 37° C cardiopulmonary bypass, the cerebral blood flow velocity response to changing mean arterial pressure (40 to 85 mm Hg in random order) was assessed before and after systemic embolization of 100 mg of 97-μm latex microspheres. Results . Before embolization, cerebral blood flow velocity increased 39% as mean arterial pressure increased from 40 to 85 mm Hg. Following embolization, a 94% increase in cerebral blood flow velocity was demonstrated over the same mean arterial pressure range. The slopes of the curves relating cerebral blood flow velocity to mean arterial pressure were 0.21 ± 0.74 and 1.31 ± 0.87, before and after embolization ( p = 0.016) respectively. Conclusions . Regional cerebral blood flow autoregulation may be impaired by microembolization known to occur during cardiopulmonary bypass, increasing the dependence of cerebral blood flow on mean arterial pressure.

BACKGROUND: In our previous report on Turkish COVID-19 patients requiring intensive care, the 24 patients in a single ICU were elderly and mortality was high. We extended our analysis to include patients admitted to ten ICUs. OBJECTIVES: Report the demographics, clinical features, imaging findings, comorbidities, and outcomes in COVID-19 patients. DESIGN: Retrospective. SETTING: Intensive care unit. PATIENTS AND METHODS: The study includes patients with clinical and radiological confirmed or laboratory-confirmed COVID-19 infection who were admitted to ten ICUs between 15 March and 30 June 2020. MAIN OUTCOME MEASURES: Clinical outcomes, therapies, and death during hospitalization SAMPLE SIZE: 974, including 571 males (58%). Entrapment of coronary angioplasty hardware is one of the rare complications of percutaneous coronary artery interventions. We reported herein a case of 58-year-old man with an entrapped balloon catheter and guidewire within the right coronary artery during the application of a conventional balloon angioplasty for in-stent restenosis. Surgical removal of the entrapped balloon catheter and guidewire was performed successfully with a coronary artery bypass grafting to the affected vessel. The application of the balloon angioplasty for in-stent restenosis requires every caution against such type of complications. © 2007 by Blackwell Futura Publishing, Inc.

Cardiac herniation and torsion is a rare condition associated with a high mortality rate. We present an unusual case of sudden cardiogenic shock that was caused by torsion and herniation of the heart after an operation for a penetrating cardiac injury. The patient was successfully treated by urgent surgical intervention.

22. Preservation of pleural integrity in patients undergoing coronary artery bypass grafting: effect on postoperative bleeding and respiratory function

The purpose of this study was to evaluate the influence of preserved integrity of pleura on postoperative bleeding and respiratory function in patients undergoing coronary artery bypass grafting (CABG).Seventy-two CABG patients who received pedunculated IMA graft without opening the pleura (group of intact pleura, group IP) between July 2002 and September 2004 were matched to 72 CABG patients who received pedunculated IMA graft with opened pleura (group of opened pleura, group OP). To match the patients with IP and unique patients with OP, logistic regression was used to develop a propensity score. The C statistic for this model was 0.79. Patients with IP were matched to unique patients with OP with an identical 5-digit propensity score. If this could not be done, we proceeded to a 4-, 3-, 2-, or 1-digit match. Patients characteristics were well matched. There were no differences in preoperative and peroperative variables between the groups. The incidence of postoperative pleural effusion and thoracentesis were significantly lower in group IP than group OP (pleural effusion in 15.2 versus 30.5%; p = 0.029, thoracentesis in 5.5 versus 18.5%; p = 0.036). Other pulmonary complications such as prolonged ventilation, reintubation, pneumothorax, atelectasis, diaphragmatic paralysis were similar in both groups. Patients with IP had significantly lower blood loss (520 versus 870 ml; p0.001) and whole blood unit transfusion (26.3 versus 41.6%, p = 0.036). Also, intensive care unit and hospital stay were similar in both groups.Meticulous internal mammary artery harvesting and preservation of the pleural integrity significantly reduces postoperative bleeding and pleural effusion.

Background . The purpose of this study was to determine the feasibility of differential perfusion of the aortic arch and descending aorta during cardiopulmonary bypass using a cannula designed for aortic segmentation. Methods . Pigs weighing 57 kg (n = 8), underwent cardiopulmonary bypass using the dual lumen aortic cannula. An inflatable balloon separated proximal (aortic arch) and distal (descending aorta) ports. During differential perfusion, the aorta was segmented and the arch and descending aorta perfused differentially using parallel heat exchangers. Ability to independently control brain and body temperature, cardiopulmonary bypass flow rate and mean arterial blood pressure was determined. Results . During differential perfusion cerebral hypothermia (27°C) with systemic normothermia (38°C) was established in 23 minutes. Independent control of arch and descending aortic flow and mean arterial blood pressure was possible. Analysis of internal jugular venous O 2 saturation data indicated an increase in the ratio of cerebral O 2 supply to demand during differential perfusion. Conclusions . A cannulation system segmenting the aorta allows independent control of cerebral and systemic perfusion. This device could provide significant cerebral protection while maintaining the advantages of warm systemic cardiopulmonary bypass temperatures.

Background Cerebral embolization is a primary cause of cardiac surgical neurologic morbidity. During cardiopulmonary bypass (CPB), there are well-defined periods of embolic risk. In theory, cerebral embolization might be reduced by an increase in pump flow during these periods. The purpose of this study was to determine the CPB flow-embolization relation in a canine model. Methods Twenty mongrel dogs underwent CPB at 35 degrees C with alpha-stat management and a fentanyl-midazolam anesthetic. In each animal, CPB flow was adjusted to achieve a mean arterial pressure of 65-75 mmHg. During CPB, an embolic load of 1.2 x 10(5) 67 microm fluorescent microspheres was injected into the arterial inflow line. Before and after embolization, cerebral blood flow was determined using 15-microm microspheres. Tissue was taken from 12 brain regions and microspheres were recovered. The relation between pump flow and embolization/g of brain was determined. Results The mean arterial pressure at embolization was 67 +/-4 mmHg, and the range of pump flow was 0.9-3.5 l x min(-1)x m(-2). Cerebral blood flow was independent of pump flow. At lower pump flow, the percentage of that flow delivered to the brain increased. There was a strong inverse relation between pump flow and cerebral embolization (r = -0.708, P < 0.000 by Spearman rank order correlation). Conclusions Cerebral embolization is determined by the CPB flow. At an unchanged mean arterial pressure, as pump flow is reduced, a progressively greater proportion of that flow is delivered to the brain.

25. Tramadol Versus Low Dose Tramadol-paracetamol for Patient Controlled Analgesia During Spinal Vertebral Surgery

Pain intensity may be high in the postoperative period after spinal vertebral surgery. The aim of the study was to compare the effectiveness and cost of patient controlled analgesia (PCA) with tramadol versus low dose tramadol-paracetamol on postoperative pain. A total of 60 patients were randomly divided into two groups. One group received 1.5 mg/kg tramadol (Group T) while the other group received 0.75 mg/kg tramadol plus 1 g of paracetamol (Group P) intravenously via a PCA device immediately after surgery and the patients were transferred to a recovery room, Tramadol was continuously infused at a rate of 0.5 mL/h in both groups, at a dose of 10 mg/mL in Group T and 5 mg/mL in Group P. The bolus and infusion programs were adjusted to administer a 1 mL bolus dose of tramadol with a lock time of 10 minutes. In Group P, 1 g of paracetamol was injected intravenously every 6 hours. The four-point nausea scale, numeric rating scale for pain assessment, Ramsey sedation scale, blood pressure, heart rate, respiration rate, peripheral oxygen saturation values and side effects were recorded at 0, 15 and 30 minutes, and at 1, 2, 4, 6, 12, 18 and 24 hours. The time to reach an Aldrete score of 9 was also recorded. A cost analysis for both groups was performed. In Group P, the numeric rating scale scores were significantly lower than that in Group T at 0 and 15 minutes. The number of side effects, additional analgesic requirement and the total dose of tramadol were lower in Group P than in Group T. However, the total cost of postoperative analgesics was significantly higher in Group P than in Group T (p < 0.001). We conclude that PCA using tramadol-paracetamol could be used safely for postoperative pain relief after spinal vertebral surgery, although at a higher cost than with tramadol alone.

BACKGROUND: The aim of the current prospective study was to examine the prognostic value of APACHE II, APACHE III (Acute Physiology and Chronic Health Evakuation II and III) and SOFA (Sepsis-related Organ Failure Assessment) scores and platelet counts in septic and nonseptic patients hospitalized and treated in ICU units. METHODS: One hundred and twenty ICU patients were included in this study. Patients were grouped as septic survivors, septic nonsurvivors, nonseptic survivors and nonseptic nonsurvivors. The SOFA, APACHE II and III scores, and platelet counts were recorded at the admission in the ICU units and discharge.from the hospital. RESULTS: Patients were grouped as septic survivors (n=42), septic nonsurvivors (n=14), nonseptic survivors (n=48) and nonseptic nonsurvivors (n=16). The highest SOFA, APACHE II and III scores and the lowest platelet counts were observed in septic nonsurvivors when compared with the other patient groups. All score values and platelet counts were found to be significant in prediction of mortality in septic patients. CONCLUSIONS: According to our results obtained in septic and non-septic patients, we concluded that SOFA, APACHE II and III scores and platelet counts might be used in the prediction of mortality in septic patients.
