Abstract

Microwave hyperthermia is an effective adjuvant treatment for breast cancer. Tumor of 1-2 cm size is generally treated by hyperthermia. Local hyperthermia enhances the effects of anticancer drugs when used with chemotherapy and or radiotherapy. In breast hyperthermia, uniform heating of tumor also damages skin and healthy tissues of the breast. The available applicators for microwave hyperthermia are costly and bulky. This article presents a simple, cost-effective, and compact dual-band slotted rectangular monopole antenna (SRMA). The proposed SRMA design is suitable for breast hyperthermia treatment (HT). It offers dual frequencies (f1 = 2.45 GHz and f2 =5.5 GHz) for HT. The dual-band frequencies are widely suitable for deep-seated and superficial breast tumors treatment. CST-MW tool is used for antenna and tri-layer breast model design. Electromagnetic (EM) and thermal solvers are used for coupled simulation. The performance of the SRMA is evaluated for a hyperthermia treatment duration of 40 min. Evaluated SAR is within the specified limit of IEEE C95.1 standard. The thermal simulation results states that temperature distribution inside the tissue is in safe limits. The proposed applicator is suitable for breast hyperthermia. It helps to reduce hotspots. Standard FR4 substrate is used for fabricating this compact antenna. It fulfils the requirements for oncological application in breast hyperthermia, as per the IEEE C95.1 standard. Abstract

Background: Head and neck cancer is the leading cause of male mortality due to cancer in India. Surgery, radiation alone or in combination has been the backbone of treatment strategies. Chemo-radiation has emerged as the standard of care in most types of head and neck cancer. This strategy has the advantage of maintaining both structure and functions, albeit with increased acute and delayed side effects. Radiation with hyperthermia can achieve the same objective without additional toxicities. Materials and Methods: A total of 56 patients were randomized to radiation therapy (RT) alone or RT-hyperthermia (RT-HT) arm. Twenty-six patients were included in RT alone arm and 28 patients in the RT-HT arm. Both groups were evenly matched for age, sex, and stage. Patients in both the arms received radiation to a dose of 66-70 Gy in 6.5-7 weeks. Patients in the study group received weekly HT. HT was started after impedance matching to last for 30 minutes. Results: Complete response was seen in 42.4% of RT alone group compare to 78.6% in the HT group. The difference was statistically significant ( < 0.05). Kaplan-Meir analysis of survival also showed a significant improvement in favor of RT-HT. No dose limiting thermal burns and excessive mucosal or thermal toxicity were recorded. Conclusion: Radiofrequency (RF) based heating and radical radiation of head and neck cancers is better than in RT alone group. HT should be considered as a valid option wherever the facility for HT is available. This report should infuse greater confidence in radiation Oncologists to practice HT as an adjuvant treatment modality.

Abstract

Purpose: To evaluate the feasibility and results of sildenafil used as radiosensitizer by causing penile vasodilatation and reducing tumor hypoxia in radical radiotherapy for penile cancer. Materials and Methods: Three men with histopathologic diagnosis of squamous carcinoma of penis were treated with radical external radiotherapy with concomitant sildenafil. Acute and late adverse events were noted according to RTOG toxicity criteria. Response to radiotherapy, as well as disease-free and overall survivals, was evaluated. Results: All three patients achieved tumescence, completed the treatment and achieved complete response. One patient had recurrence at 10 months and died of the disease after 23 months from starting the treatment. Other two patients are disease-free and alive at 48 and 53 months respectively. No excess acute or late adverse event was observed. Conclusion: Sildenafil may act as radiosensitizer by causing penile vasodilatation and reducing hypoxia. Larger controlled studies are required to validate the hypothesis.
