1. Blood-based MASH diagnostic in candidates for bariatric surgery using mid-infrared spectroscopy: a European multicenter prospective study

Abstract

Abstract Metabolic dysfunction-associated steatotic liver disease (MASLD) is common in individuals with obesity. Sexual dimorphism is present in MASLD. A noninvasive test to diagnose the severity of the disease, in particular the presence of Metabolic dysfunction-associated steatohepatitis (MASH), is lacking. This European multicenter prospective study uses a blood test based on mid-infrared (MIR) metabolic fingerprinting of individuals with severe or morbid obesity to diagnose MASH. Three hundred eighty-two individuals with severe or morbid obesity undergoing bariatric surgery were enrolled prospectively. Liver biopsies were obtained during surgery and assessed centrally. An algorithm was defined to calculate a score from the recorded MIR spectrum and to establish a diagnostic threshold to classify patients with MASH. Among the women (n = 217), MASH was diagnosed in 14.3% of cases. For women, the performance in terms of AUC were 0.83 and 0.82 in the calibration and validation groups, respectively. For a threshold of 0.1817, sensitivities were 86% and 70%, specificities were 81% and 75%, PPV were 43% and 32%, NPV were 97% and 94% and ACC were 82% and 74% for the calibration and validation groups, respectively. For men (n = 78; MASH: 33.3%), the performance of the spectral model was poor. The metabolic fingerprint obtained by MIR spectroscopy can rule out MASH in women with severe or morbid obesity. Abstract

Background and aims: A highly sensitive and specific point-of-care method for diagnosing spontaneous bacterial peritonitis (SBP) is currently lacking. The objective of the present study is to evaluate the diagnostic value of a rapid, easy-to-use, mid-infrared fiber evanescent wave spectroscopy (MIR-FEWS) method for ruling out SBP. Patients and Methods: Cirrhotic patients (n = 256) at five centers in France were included for suspected SBP or for the scheduled evacuation of ascites fluid. The mid-infrared spectrum of 7 µL of an ascites fluid sample was recorded using a MIR-FEWS system. To define a model for the diagnosis of SBP, the patients were divided into a calibration group (n = 170) and a validation group (n = 86). Results: Most of the patients were male (71%). The mean age was 60.25 years. Alcohol-related liver disease was the most common cause of cirrhosis. SBP was observed in 18% of the patients. For the diagnosis of SBP in the calibration and validation groups, respectively, the model gave areas under the receiver operating characteristic curves of 0.87 and 0.89, sensitivities of 90% and 87%, specificities of 78% and 80%, positive predictive values of 48% and 50%, negative predictive values of 97% and 96%, positive likelihood ratio of 4.09 and 4.35, negative likelihood ratio of 0.13 and 0.16, Youden index of 0.68 and 0.67, and correct classification rates of 80% and 81%. Conclusion: The results of this proof-of-concept study show that MIR-FEWS is a highly sensitive diagnostic method for ruling out SBP. The method warrants further investigation.

Abstract

Background & Aims: There is an urgent medical need to develop non-invasive tests for non-alcoholic steatohepatitis (NASH). This study evaluates the diagnostic performance of an innovative model based on mid-infrared (MIR) spectroscopy for the diagnosis of NASH. Methods: Severely obese patients who underwent a bariatric procedure at the University Hospital of Nice, France (n = 395) were prospectively recruited. The clinico-biological characteristics were measured prior to surgery. Liver biopsies were collected during the surgical procedure and assessed by a pathologist. A training group (316 patients, NASH: 16.8%) and a validation group (79 patients, NASH: 16.5%) were randomly defined. MIR spectra were acquired by fiber evanescent wave spectroscopy, using chalcogenide glass fiber optic sensors and a spectrometer. This absorption spectroscopic technique delivers a spectrum that identifies the molecular composition of a sample, defining a patient's metabolic fingerprint. Results: The areas under the receiver operating curve (AUROC) for the diagnosis of NASH were 0.82 and 0.77 in the training and validation groups, respectively. The best threshold was 0.15, which was associated with a sensitivity of 0.75 and 0.69, and a specificity of 0.72 and 0.76. Negative predictive values of 0.94 and 0.93 and positive predictive values of 0.35 and 0.36, as well as correctly classified patient rates of 72% and 75% were obtained in the training and validation groups, respectively. A composite model using aspartate aminotransferase level, triglyceride level and waist circumference alongside the MIR spectra led to an increase in AUROC (0.88 and 0.84 for the training and validations groups, respectively). Conclusions: MIR spectroscopy provides good sensitivity and negative predictive values for NASH screening in patients with severe obesity. Lay summary: There is an urgent need for tools to non-invasively diagnose and monitor non-alcoholic steatohepatitis (NASH). This study evaluates the performance of a new tool for fast NASH diagnosis based on mid-infrared (MIR) spectroscopy. Using serum samples from severely obese patients who underwent a bariatric procedure, which enabled a concomitant liver biopsy to be performed, the MIR spectroscopy model performed well in screening patients for NASH compared to a traditional, histological diagnosis. Abstract

Prognostic tests are critical in the management of patients with cirrhosis and ascites. Biological tests or scores perform poorly in that situation. Mid-infrared fibre evanescent wave spectroscopy (MIR-FEWS) which allows for global serum metabolic profiling may provide more relevant information by measuring a wider range of metabolic parameters in serum. Here we present the accuracy of a MIR-FEWS based predictive model for the prognosis of 6 months survival in patients with ascites and cirrhosis.Patients with ascites were prospectively included and followed up for 6 months. MIR-FEWS spectra were measured in serum samples. The most informative spectral variables obtained by MIR-FEWS were selected by FADA algorithm and then used to build the MIR model. Accuracy of this model was assessed by ROC curves and 90%/10% Monte Carlo cross-validation. MIR model accuracy for 6 months survival was compared to that of the Child-Pugh and MELD scores.119 patients were included. The mean age was 57.36±13.70, the MELD score was 16.32±6.26, and the Child-Pugh score was 9.5±1.83. During follow-up, 23 patients died (20%). The MIR model had an AUROC for 6 months mortality of 0.90 (CI95: 0.88-0.91), the MELD 0.77 (CI95: 0.66-0.89) and Child-Pugh 0.76 (CI95: 0.66-0.88). Abstract

Chalcogenide glass fibers are promising photonic tools to develop Fiber Evanescent Wave Spectroscopy (FEWS) optical sensors working in the mid-infrared region. Numerous pioneering works have already been carried out showing their efficiency, especially for bio-medical applications. Nevertheless, this technology remains confined to academic studies at the laboratory scale because chalcogenide glass fibers are difficult to shape to produce reliable, sensitive and compact sensors. In this paper, a new method for designing and fabricating a compact and robust sensing head with a selenide glass fiber is described. Compact looped sensing heads with diameter equal to 2 mm were thus shaped. This represents an outstanding achievement considering the brittleness of such uncoated fibers. FEWS experiments were implemented using alcoholic solutions as target samples showing that the sensitivity is higher than with the routinely used classical fiber. It is also shown that the best compromise in term of sensitivity is to fabricate a sensing head including two full loops. From a mechanical point of view, the breaking loads of the loop shaped head are also much higher than with classical fiber. Finally, this achievement paves the way for the use of mid-infrared technology during in situ and even in vivo medical operations. Indeed, is is now possible to slide a chalcogenide glass fiber in the operating channel of a standard 2.8 mm diameter catheter.

Abstract

Objectives The aim of this study was to show the usefulness of a mid-infrared fibre evanescent wave spectroscopy point of care device in the identification of septic arthritis patients in a multicentre cohort, and to apply this technology to clinical practice among physicians. Methods SF samples from 402 patients enrolled in a multicentre cohort were frozen for analysis by mid-infrared fibre evanescent wave spectroscopy. The calibration cohort was divided into two groups of patients (septic arthritis and non-septic arthritis) and relevant spectral variables were used for logistic regression model. Model performances were tested on an independent set of 86 freshly obtained SF samples from patients enrolled in a single-centre acute arthritis cohort and spectroscopic analyses performed at the patient’s bedside. Results The model set-up, using frozen–thawed SFs, provided good performances, with area under the curve 0.95, sensitivity 0.90, specificity 0.90, positive predictive value 0.41 and negative predictive value 0.99. Performances obtained in the validation cohort were area under the curve 0.90, sensitivity 0.92, specificity 0.81, positive predictive value 0.46 and negative predictive value 0.98. The septic arthritis probability has been translated into a risk score from 0 to 4 according to septic risk. For a risk score of 0, the probability of identifying a septic patient is very low (negative predictive value of 1), whereas a risk score of 4 indicates very high risk of septic arthritis (positive predictive value of 1). Conclusion Mid-infrared fibre evanescent wave spectroscopy could distinguish septic from non-septic synovial arthritis fluids with good performances, and showed particular usefulness in ruling out septic arthritis. Abstract

International audience; In this work, spark plasma sintering is used to mold non conventional chalcogenide glasses of high refractive index at low temperature (andlt;400°C). This equipment, usually used for sintering refractory materials, is presented as efficient for both densification and high precision molding of IR transparent bulks and lenses of telluride glasses. Thermo-mechanical and optical characteristics of the selected Ge25Se10Te65 glass composition were investigated showing a refractive index of 3,12@10 µm and with however a limited resistance to crystallization. Mechanical milling of raw Ge, Se, Te elements leads to a major amorphous phase with the formation of a small proportion of GeTe crystals. Remaining GeTe crystals induce a fast crystallization rate during the sintering process leading to the opacity of the material. SPS flash moldings were then performed using melt quenched glass powders to produce complex lenses. It has been found that the critical parameter to reach optimal IR transparency is mainly the powder granulometry, which should be superior to 100 µm to prevent from MIE scatterings. The possibility of producing high refractive index infrared lenses has been demonstrated even with unstable glasses against crystallization. © 2021 Optical Society of America under the terms of the OSA Open Access Publishing Agreement

International audience; Background & aims:Prognostic tests are critical in the management of patients with cirrhosis and ascites. Biological tests or scores perform poorly in that situation. Mid-infrared fibre evanescent wave spectroscopy (MIR-FEWS) which allows for global serum metabolic profiling may provide more relevant information by measuring a wider range of metabolic parameters in serum. Here we present the accuracy of a MIR-FEWS based predictive model for the prognosis of 6 months survival in patients with ascites and cirrhosis.Methods: Patients with ascites were prospectively included and followed up for 6 months. MIR-FEWS spectra were measured in serum samples. The most informative spectral variables obtained by MIR-FEWS were selected by FADA algorithm and then used to build the MIR model. Accuracy of this model was assessed by ROC curves and 90%/10% Monte Carlo cross-validation. MIR model accuracy for 6 months survival was compared to that of the Child-Pugh and MELD scores.Results: 119 patients were included. The mean age was 57.36±13.70, the MELD score was 16.32±6.26, and the Child-Pugh score was 9.5±1.83. During follow-up, 23 patients died (20%). The MIR model had an AUROC for 6 months mortality of 0.90 (CI95: 0.88–0.91), the MELD 0.77 (CI95: 0.66–0.89) and Child-Pugh 0.76 (CI95: 0.66–0.88). Abstract

Chalcogenide glass fibers are promising photonic tools to develop Fiber Evanescent Wave Spectroscopy (FEWS) optical sensors working in the mid-infrared region. Numerous pioneering works have already been carried out showing their efficiency, especially for bio-medical applications. Nevertheless, this technology remains confined to academic studies at the laboratory scale because chalcogenide glass fibers are difficult to shape to produce reliable, sensitive and compact sensors. In this paper, a new method for designing and fabricating a compact and robust sensing head with a selenide glass fiber is described. Compact looped sensing heads with diameter equal to 2 mm were thus shaped. This represents an outstanding achievement considering the brittleness of such uncoated fibers. FEWS experiments were implemented using alcoholic solutions as target samples showing that the sensitivity is higher than with the routinely used classical fiber. It is also shown that the best compromise in term of sensitivity is to fabricate a sensing head including two full loops. From a mechanical point of view, the breaking loads of the loop shaped head are also much higher than with classical fiber. Finally, this achievement paves the way for the use of mid-infrared technology during in situ and even in vivo medical operations. Indeed, is is now possible to slide a chalcogenide glass fiber in the operating channel of a standard 2.8 mm diameter catheter.

Abstract

International audience; The present study relates to a new method for the synthesis of As2Se3 glass in a controlled atmosphere. The advantage of this technique is that it does not require sealing of the silica reaction container and therefore makes it likely to substitute the current industrial batch by batch synthesis which actually needs very expensive single-use sealed silica vessels. An experimental device has been developed for these purposes. It is equipped with a stirring mechanism to homogenize the molten bath. In order to avoid contamination by oxygen and moisture, the synthesis is carried out under argon flow (pressure of 1 bar). Material losses during synthesis can be reduced to less than 2% when temperature is progressively increased up to 430 °C. Bulk glass ingots are finally obtained according to a two-step annealing process. Their chemical composition is analyzed by EDS and shows a variation range of less than 0.2%. The excellent reproducibility of the given method is also confirmed by the refractive indexes, that do not differ for more than 1 * 10−3 from one another. Adverse absorption bands due to oxygen do not occur in the 8-12 μm spectral region when 1000 ppm of Mg is added. As no distilling operation has been carried out until now, the magnesium oxide partially keeps staying in the glass and leads to scattering losses at short wavelengths.

Abstract

Chalcogenide glass fibers are matchless devices to collect mi-infrared signal. Depending on the spectroscopic strategy, different kind of optical fibers have been developed during the past 10 years. The first fibers have been fabricated from selenide glasses to implement Fiber Evanescent Wave Spectroscopy (FEWS). It is an efficient way to collect optical spectra in situ, in real time and even, in the future, in vivo. Thanks to selenide glass fibers, it is possible to record such spectra on the mid-infrared range from 2 to 11 μm. This working window gives access to the fundamental vibration band of most of biological molecules and numerous multi-disciplinary works have been led in biology and medicine. New glasses, only based on tellurium, have been recently developed, initially in the frame of the Darwin mission led by the European Space Agency (ESA). These glasses transmit light further toward the farinfrared and permit to reach the absorption band of CO2 located at 15 μm as requested by the ESA. Moreover, these telluride glass fiber are also very interesting for FEWS and medical application. Indeed, they give access to the mid-infrared signal of biomolecules beyond 11 μm, where classical selenide glass fibers are blind. Alternatively, in order to fight against global warning, some optical fibers have been developed for the monitoring of the CO2 stored into geological storage area underground. These fibers were doped with Dy3+ which emits a broad fluorescent band embedding the CO2 absorption band at 4.3 μm. thus, these fibers are used both to transmit light and as secondary sources in the mid-infrared. To conclude, original microstructurated fibers have also been used for mid-infrared sensing. They exhibit a nice sensitivity compared to classical chalcogenide glass fibers.

Abstract

Fiber Evanescent Wave Spectroscopy (FEWS) is an efficient way to collect optical spectra in situ, in real time and even, hopefully, in vivo. Thanks to selenide glass fibers, it is possible to get such spectra over the whole mid-infrared range from 2 to 12 µm. This working window gives access to the fundamental vibration band of most of biological molecules. Moreover selenide glasses are stable and easy to handle, and it is possible to shape the fiber and create a tapered sensing head to drastically increase the sensitivity. Within the past decades, numerous multi-disciplinary studies have been conducted in collaboration with the City Hospital of Rennes. Clinical trials have provided very promising results in biology and medicine which have led to the creation in 2011 of the DIAFIR Company dedicated to the commercialization of fiber-based infrared biosensors. In addition, new glasses based on tellurium only have been recently developed, initially in the framework of the Darwin mission led by the European Space Agency (ESA). These glasses transmit light further into the far-infrared and could also be very useful for medical applications in the near future. Indeed, they permit to reach the vibrational bands of biomolecules laying from 12 to 16 µm where selenide glasses do not transmit light anymore. However, while Se is a very good glass former, telluride glasses tend to crystallize easily due to the metallic nature of Te bonds. Hence, further work is under way to stabilize the glass composition for fibers drawing and to lower the optical losses for improving their sensitivity as bio-sensors.

Abstract

International audience; Chalcogenide glasses are a matchless material as far as mid-infrared (IR) applications are concerned. They transmit light typically from 2 to 12 μm and even as far as 20 μm depending on their composition, and numerous glass compositions can be designed for optical fibers. One of the most promising applications of these fibers consists in implementing fiber evanescent wave spectroscopy, which enables detection of the mid-IR signature of most biomolecules. The principles of fiber evanescent wave spectroscopy are recalled together with the benefit of using selenide glass to carry out this spectroscopy. Then, two large-scale studies in recent years in medicine and food safety are exposed. To conclude, the future strategy is presented. It focuses on the development of rare earth-doped fibers used as mid-IR sources on one hand and tellurium-based glasses to shift the limit of detection toward longer wavelength on the other hand.

Abstract

Synchrotron radiation photoelectron spectra taken at 100 eV photon energy have been measured to characterize the interface reactions of the metal-insulator-semiconductor systems Cu/RbF/GaAs(100) and Cu/RbF/Ge(100). In comparision, similar sequences are studied on the Cu/GaAs(100) and Cu/Ge(100) interfaces without the RbF interlayer. After Cu-deposition of 1–4 A on RbF-covered (10–14 A) GaAs and Ge surfaces, shoulder peaks appear on both the Ga 3d and Ge 3d core levels. The shoulder peaks are shifted 1.1 and 1.4 eV to higher binding energy for the Ga 3d and Ge 3d levels, respectively. The Rb4p and F2p peak positions are slightly shifted between 0.25 and 0.5 eV. The broad second Ga 3d and Ge 3d peaks can be correlated to plasmon loss of electrons from these levels in a two-dimensional Rb metal-like layer formed at the Cu/RbF interface. The excitation energy of a Rb surface plasmon in a CuRbRbF system is calculated to be 1.3 eV.
