BackgroundPrevious studies on emergency service utilization by refugees have been mixed, with some showing overuse and others showing underuse relative to host populations. Much of the previous literature focuses on refugees by continent of origin, which may mask important differences within immigrant groups. Limited research has investigated emergency medicine utilization for East African refugees in the United States.MethodsThe current study investigated differences in ED service utilization for East African refugees compared to local residents at an Emergency Trauma Center (Level 2) in Central Minnesota. From a convenience sample of 48 East African refugees and 116 local residents that presented to an emergency department (ED) in Central Minnesota, survey data was collected on self-reported reasons for presenting to ED; chart review data was collected on care received at the emergency department and discharge diagnoses.ResultsLogistic regressions showed that refugees were significantly (p < .05) more likely than local residents to self-report difficulties with navigating the primary care settings as a reason for presenting to the ED, and less likely to report needing specialized care. Refugees were also significantly less likely than local residents to call their primary care provider/clinic before presenting to the ED. During the ED visit, refugees were significantly more likely than local residents to receive symptomatic care and less likely to receive advanced imaging.ConclusionWe hope these results spur additional research on this unique population, particularly related to health literacy and health equity in an emergency medicine setting for East African refugees continuing to migrate to the United States at high rates.

The process of transition from pediatric to adult epilepsy care has received increased attention and emphasis in recent literature, particularly related to the assertion that effective transition is likely to lead to improved medical and psychosocial outcomes. However, the majority of current transition literature focuses on the structure of a transition program, with very little research providing relevant clinical data during the transition period and beyond. The current paper attempts to address this gap in the literature by providing pilot data on participants who engaged in the initial visit of a multidisciplinary transition-focused program housed in a level 4 epilepsy center in the Midwest. Pilot data are presented on 28 participants (36% female) who completed the initial transition appointment. All but one participant presented with a positive history for a neurobehavioral comorbidity, the most common of which included anxiety (61%), attention-deficit/hyperactivity disorder (ADHD; 39%) and depression (36%). Seventy-seven percent of participants further identified a current neurobehavioral comorbidity that was impacting their psychosocial functioning. Recommendations provided most frequently involved increased independence with epilepsy management (64%), increased independence with self-care/independent living (82%), psychological intervention (43%), and increased socialization (43%). A case example is also provided to further highlight program process and outcomes of the initial visit. Pilot results emphasize the value of multidisciplinary care involving psychosocial providers to facilitate a smooth transition between pediatric and adult healthcare.

Mother–child interactions can be emotionally charged, particularly when mothers experience difficulties with depression. Further, variation in the tone of such interactions has been linked to variation in youth functioning. Yet, little is known about the role of emotion regulatory processes during mother–child interactions. This study examined negative affective transactions between mothers and youth across 5-min conflict and fun-activity planning discussions, in relation to depressive symptoms and maternal and youth respiratory sinus arrhythmia (RSA). Maternal depressive symptoms predicted more negative affect. Maternal and youth RSA interacted with maternal depressive symptoms to predict both overall affect and the stability of affect across time. The beneficial effects of youth RSA were stronger, but maternal RSA weaker, in the face of elevated maternal depression.

Recent research has highlighted the potential role of attention bias for emotional stimuli as a possible cognitive risk factor for depression in youth. However, differences in youth emotion regulation or maternal affect may moderate the association between maternal and youth depression and youth attention biases. The current study investigated the relationship between maternal and youth depressive symptoms and youth (aged 11-17 years) attention bias for sad and happy faces in 59 mother-youth dyads, examining whether positive and negative maternal affect observed during structured interaction tasks or youth emotion regulation tendencies moderated associations between maternal and youth depression and attention biases. Youth suppression interacted with maternal and youth depression to predict sad attentional biases in youth, while maternal positive affect interacted with maternal depression to predict happy attention biases in youth.

Obsessive-compulsive disorder (OCD) is common in children with rates of 1% to 4% in children and adolescents. A prominent first-line treatment for pediatric OCD is cognitive-behavioral therapy (CBT) using exposure and response prevention (ERP). However, it has also been suggested that family therapy be used as an adjunct treatment in pediatric OCD cases, as family members often accommodate or contribute to the maintenance of OCD symptoms. The following case study reviews the successful treatment of a 7-year-old with comorbid OCD and oppositional defiant disorder, in which traditional CBT and ERP for OCD was integrated with family-based approaches utilizing a family systems theory framework.

Biopsychosocial conceptualizations of eating disorders (EDs) suggest the combination of an individual's emotional vulnerability and invalidating environment increases the likelihood of developing pervasive emotion dysregulation, and subsequent use of ED behaviors to regulate emotion (Haynos & Fruzetti, 2011; Safer, Telch, & Chen, 2009). The current study aimed to provide initial support for this model in adolescent EDs, through examining the interaction between an adolescent's emotional vulnerability, indexed by attentional biases for emotions, and an invalidating family environment. Specifically, we examined the ability of this interaction to discriminate youth with EDs from a comparison group of youth with chronic pain diagnoses, who were used to control for the presence of non-specific effects of having any illness. Fifty adolescent girls (25 with EDs and 25 with chronic pain) completed an emotional dot-probe task assessing attentional biases for emotional faces, and parents completed the Emotions as a Child Scale (Magai, 1996; Klimes-Dougan et al., 2007) to assess response to teen emotion. Results showed that teen angry attentional bias moderated the relationship between parental response to sadness and teen ED status: for teens with high attention bias towards angry faces, maladaptive parental response to sadness predicted increased odds of ED status versus chronic pain status.

Maternal depression is associated with problematic parenting and the development of emotional and behavior problems in children and adolescents. While emotional regulatory abilities are likely to influence emotional exchanges between parents and teens, surprisingly little is known about the role of emotion regulation during parent-child interactions, particularly in high-risk families. Respiratory sinus arrhythmia (RSA) has been widely linked to emotion regulatory abilities in recent research, and the current study investigated RSA and maternal depression in relation to dyadic flexibility, as well as mutuality of negative and positive affect displayed during three discussion tasks between 59 mother-adolescent pairs (age 11-17 years). Dyadic flexibility was predicted by the interaction of maternal depression, maternal RSA, and teen RSA, with higher maternal RSA predicting greater dyadic flexibility, particularly in highest risk dyads (i.e., elevated maternal depression and lower teen RSA). Teen RSA interacted with maternal depression to predict mutual negative affect, serving as a protective factor. Finally, maternal and teen RSA interacted to predict mutual positive affect, with maternal RSA buffering against low teen RSA to predict higher mutual positive affect. Results support the role of RSA in affectively laden interactions between parents and adolescents, particularly in the face of maternal depression.
