BACKGROUND The safe and appropriate use of long-acting beta-agonists (LABAs) for the treatment of asthma has been widely debated. In two large clinical trials, investigators found a potential risk of serious asthma-related events associated with LABAs. This study was designed to evaluate the risk of administering the LABA salmeterol in combination with an inhaled glucocorticoid, fluticasone propionate. METHODS In this multicenter, randomized, double-blind trial, adolescent and adult patients (age, ≥12 years) with persistent asthma were assigned to receive either fluticasone with salmeterol or fluticasone alone for 26 weeks. All the patients had a history of a severe asthma exacerbation in the year before randomization but not during the previous month. Patients were excluded from the trial if they had a history of lifethreatening or unstable asthma. The primary safety end point was the first serious asthma-related event (death, endotracheal intubation, or hospitalization). Noninferiority of fluticasone–salmeterol to fluticasone alone was defined as an upper boundary of the 95% confidence interval for the risk of the primary safety end point of less than 2.0. The efficacy end point was the first severe asthma exacerbation. RESULTS Of 11,679 patients who were enrolled, 67 had 74 serious asthma-related events, with 36 events in 34 patients in the fluticasone–salmeterol group and 38 events in 33 patients in the fluticasone-only group. The hazard ratio for a serious asthmarelated event in the fluticasone–salmeterol group was 1.03 (95% confidence interval [CI], 0.64 to 1.66), and noninferiority was achieved (P = 0.003). There were no asthma-related deaths; 2 patients in the fluticasone-only group underwent asthmarelated intubation. The risk of a severe asthma exacerbation was 21% lower in the fluticasone–salmeterol group than in the fluticasone-only group (hazard ratio, 0.79; 95% CI, 0.70 to 0.89), with at least one severe asthma exacerbation occurring in 480 of 5834 patients (8%) in the fluticasone–salmeterol group, as compared with 597 of 5845 patients (10%) in the fluticasone-only group (P

- Abstract
The use of diffraction gratings to trap optical beams in substrates for optical interconnect applications has been proposed by many. We analyze the alignment and fabrication tolerances of such gratings to make them viable for such applications. The use of chirped circular gratings is proposed as a means of compensating for transverse-optical beam misalignments. Fabrication of 1-, 2-, and 4-microm-period gratings on glass substrates through ion-beam milling and their analysis are presented. High diffracted orders were confined to the substrates through total internal reflection.

- Abstract
The known problem of grating erasure in photorefractive-based holographic optical interconnects can lead to limitations in the density of the system. This problem is analyzed for an architecture that uses spatially multiplexed beams (to eliminate cross erasure) and angularly multiplexed input beams to record multiplexed gratings in a bismuth silicon oxide (BSO) crystal to interconnect three beams to an array of 8 x 8 (64) detectors. Drawbacks of the time-exposure technique, which compensates for erasure, are analyzed. A novel photorefractive-based interconnect architecture is proposed, which uses holograms and read beams that are segregated to minimize erasure during recording and reading.

15. Binary phase Fresnel lenses for generation of two-dimensional beam arrays.

- Abstract
Two-dimensional arrays of mutually coherent optical beams are needed in holographic interconnections for both recording and reading gratings. We analyze the use and limitations of binary phase Fresnel lenses to generate beams for these applications. Two known techniques of ion beam milling and thin film deposition are compared to fabricate such lens arrays in SiO(2) and Si(3)N(4). Each lens in the 8 x 8 arrays has a 1.2-mm square aperture with a focal length of 20 mm. Diffraction of a single argon-ion beam into an 8 x 8 array of highly uniform coherent focused beams (with 12-microm spot size) was achieved by the lenses with an efficiency of approximately 30% (41% theoretical limit).

Coherent holographic grating interconnect systems exhibit deleterious interference effects when multiple optical beams are fanned into a single beam with the use of multiplexed gratings. Elimination of such effects is possible with the use of arrays of incoherent sources for readout, temporal sequencing, random phase modulation of the readout beams, or allocated volume configurations. The merits and drawbacks of each alternative are analyzed.

- Abstract
Typical light-emitting diodes have a spectral width between 300 A and 500 A. This is sufficient to cause significant pulse broadening as a result of material dispersion in a long optical glass fiber. An experiment was performed to measure the magnitude of this effect in a fiber of 1-km length. The experiment measures the average intensity transfer function of the fiber.

18. Chromatic delay in light emitting diodes.

- Abstract
When the current through the junction of a light emitting diode (LED) is modulated, there is a delay between the modulation and the response of the optical signal to this modulation. We have recently observed that this delay is not the same at different parts of the LED's optical spectrum. This effect, the relative delay between spectral components, we call chromatic delay. This paper reports the results of measurements of chromatic delay for several LED's of various kinds. This effect amounts to delays of the order of 200-300 psec between half power points of the LED spectrum. Thus it establishes a lower limit on the rise time achievable from these devices.

20. Double-reverse-scatter interference in optical fiber communication systems.

- Abstract
The use of low-loss optical fibers as a medium for guided-wave communication systems has recently become feasible. The dominant source of loss in such a medium may well be scattering. When this is the case, a substantial amount of signal power may reach the receiver as the result of two back scatterings, i.e., double-reverse scattering. It is concluded that under practical conditions a surprisingly large amount of power can reach the receiver via this mechanism but that despite this fact the effect on system performance is nearly negligible in most cases. Since this is an intrachannel interference problem and not a noise problem, the effect can be reduced if necessary by means of a simple equalizer, which is described.
