Abstract

Recombinant (genetically engineered) human deoxyribonuclease I (rhDNase) may improve lung function in patients with cystic fibrosis (CF). CF patients have high concentrations of DNA in their lungs that cause the formation of thick secretions. RhDNase breaks down the DNA in these secretions, and improves lung function. Out of 11 adult patients with cystic fibrosis, seven were treated with 10 milligrams (mg) of aerosolized rhDNase twice a day for six days followed by six days of treatment with placebo, an inactive substance. The four other patients were treated first with placebo followed by treatment with rhDNase. Treatment with rhDNase increased lung function in the morning and early evening significantly compared to treatment with placebo. Patients produced the same volume of sputum during treatment with rhDNase and treatment with placebo. Among 16 patients who were treated with different dosages of aerosolized rhDNase, higher dosages did not increase lung function more than lower dosages.

Abstract

The drug celecoxib appears to be an effective painkiller in patients with rheumatoid arthritis and is less likely than other arthritis drugs to damage the gastrointestinal tract. Celecoxib inhibits an enzyme called cyclooxygenase 2 (COX-2). Researchers randomly assigned 1,149 patients with rheumatoid arthritis to take one of several dosages of celecoxib, naproxen or a placebo, or inactive substance. Both celecoxib and naproxen caused reduced pain, but celecoxib was less likely to cause ulcers. Thirty-one percent of those who took naproxen had a gastrointestinal side effect compared to about 25% of the celecoxib users and 19% of the placebo users.

Abstract

Alpha 1-antitrypsin deficiency is an inherited disorder characterized by low blood and lung levels of alpha 1-antitrypsin, a protein and carbohydrate compound that inhibits proteolytic or protein-degrading enzymes. Alpha 1-antitrypsin protects against neutrophil elastase, a proteolytic enzyme capable of destroying the walls of the alveoli or air sacs of the lungs. A deficiency in alpha 1-antitrypsin results in the loss of anti-neutrophil-elastase protection of the lower respiratory tract and leads to the development of emphysema, the distention of the lungs by gas or air. The effectiveness of an aerosol preparation of purified human plasma alpha 1-antitrypsin in improving anti-neutrophil elastase defenses in the lower respiratory tract was assessed in 12 patients with a deficiency of alpha 1-antitrypsin. The aerosol preparation increased alpha 1-antitrypsin levels in the epithelial cells lining the lung from 0.28 micromole (uM) before therapy to 5.86 uM after therapy. Anti-neutrophil-elastase capacity was increased from 0.78 uM before therapy to 4.16 uM after therapy. Aerosolized alpha 1-antitrypsin was shown to diffuse from the lung epithelial cells to cells within the lung tissue and eventually to the bloodstream, and was not associated with any side effects. Thus, short-term use of aerosolized alpha 1-antitrypsin is both safe and effective in treating alpha 1-antitrypsin deficiency, and improves anti-neutrophil-elastase defenses in the lower respiratory tract. 47. Treatment of Osteoarthritis with a OnceDaily Dosing Regimen of Celecoxib A Randomized, Controlled Trial

Abstract

A previous study has shown celecoxib 100 mg b.i.d. to be comparably effective to naproxen 500 mg b.i.d. in treating osteoarthritis OA. The primary objective of this study was to compare the efficacy of a oncedaily regimen of celecoxib 200 mg q.d. to the 100 mg b.i.d. regimen in treating the signs and symptoms of OA. In this doubleblind, placebocontrolled, parallelgroup, multicenter study, 686 patients with OA of the knee in a flare state were enrolled. Patients were randomly assigned to receive celecoxib 100 mg b.i.d. N 231, celecoxib 200 mg q.d. N 223, or the placebo N 232 for 6 weeks. Arthritis assessments were performed at baseline and at weeks 2 and 6, or at early termination.
