Objective To screen the influencing factors for the poor prognosis in patients with ultra⁃ severe traumatic brain injury (TBI). Methods Total 133 patients with ultra⁃severe TBI [Glasgow Coma Scale (GCS) score 3-5] which admitted to the 909th Hospital of Joint Logistic Support Force; Dongnan Hospital of Xiamen University from January 2012 to June 2017 were included. All cases underwent hematoma removal with either standard craniotomy decompression or oversized craniotomy decompression, even combined with internal decompression. Glasgow Outcome Scale (GOS) was used to evaluate prognosis 6 months after surgery. Univariate and multivariate Logistic regression analyses were adopted to screen the influencing factors for poor prognosis in patients with ultra⁃severe TBI. Results Total 133 patients were divided into favourable prognosis group (GOS score ≥ 4, n = 12) and poor prognosis group (GOS score < 4, n = 121). Logistic regression analysis showed bilateral pupils dilated (OR = 1.779, 95%CI: 1.698-4.531; P = 0.004), postoperative high intracranial pressure (ICP; OR = 12.629, 95%CI: 2.313-68.949, P = 0.007) and volume of secondly cerebral infarction > 75 ml (OR = 2.147, 95%CI: 1.894-5.156; P = 0.009) were the risk factors for poor prognosis, while injury subtyping of isolated epidural hematoma was the protective factor for favourable prognosis (OR = 0.172, 95%CI: 0.032-0.915; P = 0.002). Conclusions GCS score is not reliable in solely predicting the prognosis of patients with ultra ⁃severe TBI. Preoperative bilateral pupil dilation, postoperative high ICP and volume of secondly cerebral infarction > 75 ml were risk factors for poor prognosis of patients with ultra⁃severe TBI, while injury subtyping of isolated epidural hematoma was the protective factor for favourable prognosis.

The printed circuit heat exchanger (PCHE) is one of the promising candidates for supercritical CO2 Brayton cycle, its performance is important to the safety and economy of the next generation nuclear technology. In this paper, an experimental study on heat transfer performance of supercritical CO2-water in zigzag mini-channels was conducted to explore the heat transfer characteristics in the pre-cooler channels of a supercritical CO2 Brayton cycle. The experimental pressure of the supercritical CO2 is 7.5-9.0 MPa, the temperature is 50-90 ℃, and the flow rate is 300-600 kg/(m2·s). The test section has a diameter of 2.0 mm, a pitch of 7.24 mm, and a bend angle of 40°. By arranging multiple temperature measurement points, the temperature distribution data along the complex mini channels were obtained. The influence of different operating conditions on the logarithmic mean temperature difference and overall heat transfer coefficient along the mini channels was studied, and the effects of overall method and segmented method on the calculation results were further evaluated. The results show that the overall heat transfer coefficient exhibits a relatively high level near the pseudo-critical region. Meanwhile, due to the pronounced enhancement effect of the buffer layer on the near-critical temperature, the peak value of the overall heat transfer coefficient corresponds to a slightly higher CO2 bulk temperature than the near-critical temperature. Under the given operating conditions, increasing the CO2 inlet temperature has a small effect on the overall heat transfer coefficient of the heat exchanger. With the increase of inlet pressure, the peak value of overall heat transfer coefficient decreases, the peak values of the overall heat transfer coefficient corresponding to 7.5, 8.0, and 9.0 MPa are 3 828.3, 3 466.0, and 3 281.8 W/(m2·℃) respectively. However, the overall trend gradually becomes less sensitive to the changes in bulk temperature and exhibits higher heat transfer capability in the gas-like region. Increasing the inlet mass flow rate can significantly improve the overall heat transfer capacity of the heat exchanger. When the inlet mass flux increases from 311.72 kg/(m2·s) to 495.37 kg/(m2·s), the maximum value of the overall heat transfer coefficient increases from 3 305 W/(m2·℃) to 4 193 W/(m2·℃), which is a 26.87% improvement. Besides, in the heat transfer conditions near the pseudocritical region, the deviation between the overall heat transfer coefficients calculated by the overall method and the segmented method is significant, while in the heat transfer conditions across the pseudocritical region, due to the compensating effect of the s-shaped logarithmic mean temperature difference, the deviation in the calculation results is significantly reduced.

Cutting-edge detection is a critical step in mechanized rice harvesting. Through visual cutting-edge detection, an algorithm can sense in real-time whether the rice harvesting process is along the cutting-edge, reducing loss and improving the efficiency of mechanized harvest. Although convolutional neural network-based models, which have strong local feature acquisition ability, have been widely used in rice production, these models involve large receptive fields only in the deep network. Besides, a self-attention-based Transformer can effectively provide global features to complement the disadvantages of CNNs. Hence, to quickly and accurately complete the task of cutting-edge detection in a complex rice harvesting environment, this article develops a Transformer Convolution Network (TCNet). This cutting-edge detection algorithm combines the Transformer with a CNN. Specifically, the Transformer realizes a patch embedding through a 3 × 3 convolution, and the output is employed as the input of the Transformer module. Additionally, the multi-head attention in the Transformer module undergoes dimensionality reduction to reduce overall network computation. In the Feed-forward network, a 7 × 7 convolution operation is used to realize the position-coding of different patches. Moreover, CNN uses depth-separable convolutions to extract local features from the images. The global features extracted by the Transformer and the local features extracted by the CNN are integrated into the fusion module. The test results demonstrated that TCNet could segment 97.88% of the Intersection over Union and 98.95% of the Accuracy in the unharvested region, and the number of parameters is only 10.796M. Cutting-edge detection is better than common lightweight backbone networks, achieving the detection effect of deep convolutional networks (ResNet-50) with fewer parameters. The proposed TCNet shows the advantages of a Transformer combined with a CNN and provides real-time and reliable reference information for the subsequent operation of rice harvesting. BackgroundMultistage, stepwise HIV testing and treatment procedures can result in lost opportunities to provide timely antiretroviral therapy (ART). Incomplete engagement of patients along the care cascade translates into high preventable mortality. We aimed to identify whether a structural intervention to streamline testing and linkage to HIV health care would improve testing completeness, ART initiation, and viral suppression and reduce mortality.MethodsWe did a cluster-randomised, controlled trial in 12 hospitals in Guangxi, China. All hospitals were required to be level 2A county general hospitals and ART delivery sites. We selected the 12 most similar hospitals in terms of structural characteristics, past patient caseloads, and testing procedures. Hospitals were randomly assigned (1:1) to either the One4All intervention or standard of care. Hospitals were randomised in a block design and stratified by the historical rate of testing completeness of the individual hospital during the first 6 months of 2013. We enrolled patients aged 18 years or older who were identified as HIV-reactive during screening in study hospitals, who sought inpatient or outpatient care in a study hospital, and who resided in the study catchment area. The One4All strategy incorporated rapid, point-of-care HIV screening and CD4 counts, and in-parallel viral load testing, to promote fast and complete diagnosis and staging and provide immediate ART to eligible patients. Participants in control hospitals received standard care services. All enrolled patients were assessed for the primary outcome, which was testing completeness within 30 days, defined as completion of three required tests and their post-test counselling. Safety assessments were hospital admissions for the first 90 days and deaths up to 12 months after enrolment. This trial is registered with ClinicalTrials.gov, number NCT02084316.FindingsBetween Feb 24 and Nov 25, 2014, we enrolled 478 patients (232 in One4All, 246 in standard of care). In the One4All group, 177 (76%) of 232 achieved testing completeness within 30 days versus 63 (26%) of 246 in the standard-of-care group (odds ratio 19·94, 95% CI 3·86-103·04, p=0·0004). Although no difference was observed between study groups in the number of hospital admissions at 90 days, by 12 months there were 65 deaths (28%) in the in the One4All group compared with 115 (47%) in the standard-of-care group (Cox proportional hazard ratio 0·44, 0·19-1·01, p=0·0531).InterpretationOur study provides strong evidence for the benefits of a patient-centred approach to streamlined HIV testing and treatment that could help China change the trajectory of its HIV epidemic, and help to achieve the goal of an end to AIDS.FundingUS National Institute on Drug Abuse Clinical Trials Network and China's National Health and Family Planning Commission.

The slot link mechanism can project uniform rotation to a simple harmonic motion (SHM), based on which, the device including two sliding table systems has been prepared. These two sliding table systems can be stacked up and down, and each of them can produce a tunable mechanical SHM using a stepper motor. When the upper sliding table system is attached to the lower one and both are turned on, the motion of the sliding table of the upper component is the composition of the two SHMs. With the help of a self-powered LED, a violet laser, and a movable phosphorescent foil, this mechanical device can visualize the composition of the SHMs in the same and mutual perpendicular directions including the beat phenomenon and Lissajous figures. In addition to demonstrating several typical compositions of two SHMs, this device can also highlight the vector characteristics of mechanical vibration and demonstrate the tilted Lissajous figures.
