Purpose Observational population-based research is a very suitable non-invasive method for studies in the vulnerable populations of pregnant women and children. Therefore, the PHARMO Perinatal Research Network (PPRN) was set up as a resource for life course perinatal and paediatric research by linking population-based data from existing registrations. Participants From 1999 to 2017, the PPRN captures approximately 542 900 pregnancies of 387 100 mothers (‘Pregnancy Cohort’). Additionally, mother–child linkage is currently available for a quarter of these pregnancies (‘Child Cohort’). The PPRN contains preconceptional information on maternal healthcare, as well as detailed pregnancy and neonatal data, extending into long-term follow-up and outcomes after birth for both mother and child up to nearly 20 years. Aims: Recent population-based data on drug utilization around pregnancy are lacking. This study aims to examine the prevalence of drug exposure in the Netherlands during the preconception, pregnancy and postpartum periods, with special emphasis on trends of potentially harmful medication over the years. Methods: A population-based study was conducted using records from the PHARMO Perinatal Research Network. From 1999 to 2017, the proportion of pregnancies during which women used any medication or potentially harmful medication was assessed, overall and stratified by timing of exposure relative to pregnancy and by the year of delivery. Results: Overall, 357 226 (73%) and 166 484 (34%) of 487 122 selected pregnancies were exposed to any and potentially harmful medication, respectively. Among these 487 122 pregnancies, preconception prevalence for use of potentially harmful medication was 43%, 24% during the first trimester, 19% during the second, 16% during the third, and 45% postpartum. A declining trend was observed for exposure to any medication, from 84% in 1999 to 68% in 2017. No clear changes were observed over time for the proportion of pregnancies exposed to potentially harmful medication. Conclusions: Our study shows that the use of potentially harmful medication was high over the last two decades. Although there was a declining trend over the years in overall medication use, during a steady one-third of pregnancies, women used potentially harmful medication. Our findings highlight the need for an increased sense of urgency among both healthcare providers and women of reproductive age regarding potential risks associated with pharmacological treatment during pregnancy.

Bacteria have evolved intricate secretion machineries for the successful delivery of large molecules across their cell envelopes. Such specialized secretion systems allow a variety of bacteria to thrive in specific host environments. In mycobacteria, type VII secretion systems (T7SSs) are dedicated protein transport machineries that fulfill diverse and crucial roles, ranging from metabolite uptake to immune evasion and subversion to conjugation. Since the discovery of mycobacterial T7SSs about 15 y ago, genetic, structural, and functional studies have provided insight into the roles and functioning of these secretion machineries. Here, we focus on recent advances in the elucidation of the structure and mechanism of mycobacterial T7SSs in protein secretion. As many of these systems are essential for mycobacterial growth or virulence, they provide opportunities for the development of novel therapies to combat a number of relevant mycobacterial diseases. Vitamin K-dependent matrix Gla protein (MGP) is a key inhibitor of vascular calcification (VC). MGP is synthesized by chondrocytes and vascular smooth muscle cells (VSMC) and the absence or inactivity of MGP results in excessive calcification of both growth plate and vasculature. Apart from its vitamin K dependency little is known about other factors that influence MGP metabolism. Phosphate, calcium and magnesium are involved in bone mineralization and play an important role in VC. In this review we provide a summary of the effect of phosphate, calcium, and magnesium on MGP metabolism. Elevated phosphate and calcium levels promote VC, in part by increasing the release of matrix vesicles (MV) that under the influence of calcium and phosphate become calcification competent. Phosphate and calcium simultaneously induce an upregulation of MGP protein and gene expression, which possibly inhibits calcification. Elevated phosphate levels did not change MGP protein levels in MV. On the contrary, elevated calcium concentrations caused a decrease of MGPloading in MV, which might in part explainthe calcifying effects of MV. Magnesium is a known inhibitor of VC. However, magnesium has been shown to have an inhibitory effect on MGP synthesis induced through downregulation of the calcium-sensing receptor and hereby causing a decrease in calcium induced MGP upregulation. There might also be stimulatory effect of magnesium on MGP in which the TRPM7 channel is involved. In conclusion there is a clear interaction between MGP and phosphate, calcium and magnesium. The upregulation of MGP by phosphate and calcium might be a cellular response that possibly results in the mitigation of VC.

Introduction: Patients with cirrhosis are at risk for adverse drug reactions (ADRs) due to altered pharmacokinetics and pharmacodynamics. We aimed to determine the prevalence of drug prescriptions and the potential safety of these prescriptions in a real-world cohort of patients with cirrhosis. Methods: This was a retrospective cohort study based on linked real-world data from the Out-patient Pharmacy Database and the Hospitalisation Database of the PHARMO Database Network. Patients with a diagnosis of cirrhosis between January 1998 and December 2015 were included. Follow-up ended when the patient underwent a liver transplant, died, transferred out of the database, or on 31 December 2015. Prescription data were derived from a community pharmacy database and were compared with our previously developed safety recommendations for 209 drugs. Results: In total, 5618 patients were included and followed for a median of 3 years (interquartile range [IQR] 1–7). In the first year after the diagnosis, patients used a median of nine drugs (IQR 5–14), with proton pump inhibitors (prevalence 53.9%), aldosterone antagonists (43.6%), and sulfonamide diuretics (41.3%) being the most commonly used drug groups. In deze studie is een antwoord gezocht op de vraag: Hoe kan CLIL het best worden vormgegeven in de middenbouw van de pilot tweetalig primair onderwijs (tpo)? Dit is enerzijds gedaan met behulp van een literatuuronderzoek en anderzijds door het interviewen van 17 participanten, verdeeld over vijf groepen: tpo-docenten, beleidsmakers, lerarenopleiders, onderzoekers en CLIL-experts uit het buitenland. Uit beide delen van het onderzoek blijkt dat er veel verschillen in mening over de betekenis en invulling van CLIL bestaan. Er zijn echter ook overeenkomsten gevonden. Zo moet er gelijke aandacht bestaan voor het content-gedeelte en het language-gedeelte en is het belangrijk duidelijke doelstellingen te formuleren voor de lessen. Vakken als wereldoriëntatie, creatieve vakken, wiskunde/rekenen en sportlessen lijken geschikt voor CLIL, hoewel het wellicht beter is projectmatig te werken. Het beoordelen van de L2-vaardigheid binnen de CLIL-les is goed, mits het op basis van de taaldoelen gebeurt en er correctieve feedbackmethodes voor gebruikt worden die de aandacht niet teveel wegnemen van de inhoud van de les. Het is raadzaam vaktermen in beide talen aan te bieden. Het lees- en schrijfonderwijs in het Engels kan het beste in groep 4/5 geïntroduceerd worden, mits er rekening wordt gehouden met de kinderen die er wellicht al eerder klaar voor zijn. CLIL is geschikt voor iedereen, er hoeft geen selectieprocedure te komen voor kinderen met leer-, lees- en/of taalproblemen of kinderen met een andere moedertaal. Ten slotte lijkt CLIL in de middenbouw nu al haalbaar te zijn in de praktijk. Aandachtspuntjes zijn: het tekort aan goed materiaal, een goede begeleiding van leerkrachten.

Dyslexie is een taalstoornis, met lees- en spelproblematiek als belangrijkste kenmerk. Uit onderzoeken is gebleken dat er een fonologisch kernprobleem aan ten grondslag ligt. Verder bestaat er een hoge comorbiditeit tussen dyslexie en andere ontwikkelingsstoornissen, waaronder de aandachtsstoornis ADHD. Daarnaast is in de afgelopen decennia duidelijk geworden dat dyslexie een genetisch aspect heeft; kinderen hebben een veel grotere kans dyslexie te ontwikkelen als één van hun ouders de taalstoornis heeft. Om te onderzoeken of kenmerken van het fonologische kernprobleem (en een eventuele comorbiditeit met aandachtsstoornis ADHD) ook al zichtbaar zijn bij kinderen die nog niet hebben leren lezen en schrijven, zijn 28 kinderen van 3,5 tot 4,5 jaar oud, met een risico op dyslexie, getest op hun fonologische vaardigheden, taalontwikkeling, aandacht, spelgedrag, sociaal-emotionele ontwikkeling en motorische ontwikkeling, en vergeleken met een even grote groep leeftijdsgenoten zonder risico op dyslexie. Uit het onderzoek bleek dat de risicokinderen slechter scoorden op de fonologische non-woord repetitietaak en de visuele aandachtstaak. Ook bleek er in de sociaal-emotionele ontwikkeling een trend te zijn dat de proefpersonen met een familiair risico op dyslexie iets liever alleen spelen dan de kinderen uit de controlegroep. Op de bredere taaltaken (vocabulaire en morfologie) scoorden alle kinderen echter even goed. Ook uit de data over spelgedrag en motoriek bleek geen verschil tussen beide groepen. Deze resultaten laten geen overtuigend bewijs zien voor een verminderde fonologische verwerking bij kinderen met een risico op dyslexie; het Snel Benoemen test namelijk ook de verwerkingssnelheid, het onderliggende kernprobleem van ADHD. Ook de lage score op de aandachtsmaat kan wijzen op kenmerken van ADHD. Wel ondersteunt dit laatste resultaat de claim dat aandachtsproblemen bij kinderen met een risico op dyslexie niet (alleen) veroorzaakt worden door een achterstand in de le

This study explored two factors that might have an impact on how participants perceive distance between objects in a visual scene: perceptual grouping and presentation mode (2D versus 3D). More specifically, we examined how these factors affect language production, asking if they cause speakers to include a redundant color attribute in their descriptions of objects. We expected speakers to use more redundant color attributes when distractor objects are perceptually close. Our findings revealed effects of perceptual grouping, with speakers indeed using color more often when all objects in a scene were in the same perceptual group as compared to when this was not the case. An effect of presentation mode (whether scenes were presented in 2D or in 3D) was only partially borne out by the data. Implications of our results for computational models of reference production are discussed.
