34. Examining the trimester-specific effects of low gestational weight gain on birthweight: the BOSHI study.

Abstract

Low gestational weight gain (GWG) is a known risk factor of low birthweight. Although studies have previously examined the associations between GWG and birthweight, the period-specific effects of low GWG in each trimester remain unclear. This study aimed to quantify the trimester-specific direct effects of low GWG in Japanese women on birthweight. Using perinatal data from a cohort study, we analyzed pregnant women delivered at an obstetrics/gynecology hospital between October 2006 and May 2010. We focused on women with a pre-pregnancy body mass index (BMI) below 25 kg/m2. The exposure was low GWG. The gestation period was subdivided into trimesters, and the direct effects of low trimester-specific GWG on birthweight were estimated using marginal structural models. These models were guided by a direct acyclic graph that incorporated potential confounders, including pre-pregnancy BMI, age, smoking during pregnancy, height, and parity. We analyzed 563 women and their families. The mean cumulative GWG by the end of the first, second, and third trimesters was 0.9, 6.2, and 10.7 kg, respectively. Approximately 14.0% of the women gained total weight below the range recommended by Japanese Ministry of Health, Labour and Welfare. The direct effects of low GWG on birthweight were 65.9 g (95% confidence interval: 11.4, 120.5), −195.4 g (−263.4, −127.4), and −188.8 g (−292.0, −85.5) for the first, second, and third trimesters, respectively. Insufficient weight gain in the second and third trimesters had a negative impact on birthweight after adjusting for pre-pregnancy BMI and other covariates. [ABSTRACT FROM AUTHOR]

Abstract

From 1890s I of Meiji era, Japanese architects introduced elements of their understanding of the Western architecture into modern Japanese architecture to reflect the contemporary Japanese architecture of the time. Due to the particular historical background, as opposed to the Japanese mainland, Taiwan was like a fresh unpainted piece of paper, which became the paradise to showcase all architects' talent. The Japanese Red Cross Society Taiwan branch government office building, a hospital and the Government-General of Formosa central institute were the work which has a good reputation most as a work of Onogi in the Taiwanese age. Sovereign's momentum was indicated by new construction of the Western style built of brick in those days. On the other hand, government office building construction in XinZhu, YinLan, TaoYuan, MiaoLi, NanTou, DouLiu’s government office building often depended on brick construction. This study looks into Taiwanese architects and their works in the Japanese colonial period(1895-1945), and examine their current status.

Abstract

This research added observation of bodily positions that people assume in daily life to environmental design for enabling independent elderly people to live healthily and for a long time in their homes. An evaluation survey was conducted recording the times when subjects assumed standing, sitting, and lying-down positions in a day and their level of health and activity. The results of the survey revealed that for independent elderly people, there was a striking gender difference in health consciousness as shown by their living patterns. This study confirmed the necessity of incorporating bodily positions in design guidelines.

Abstract

This study retrospectively compared the clinical pregnancy rate (PR) per cycle using Fallopian tube sperm perfusion (FSP) to that obtained by intrauterine insemination (IUI) in relation to the number of motile sperm inseminated. We studied 76 infertile women aged 25 to 38 years who had undergone 216 insemination cycles with sperm prepared by the Percoll method between July 1994 and June 1995. Infertility was either unexplained or due to male infertility. All women in this study demonstrated tubal patency and spontaneous ovulatory cycles. Insemination was performed during an unstimulated cycle. Each cycle was scheduled for either FSP(N=52) or IUI(N=164) treatment. FSP was performed using a tubal patency test catheter with 4 ml of sperm suspension. IUI was performed with 0.5 ml of the suspension. Our results did not show any significant difference in the PR between the two groups. These results indicated that the FSP techniqu did not improve the PR, as compared to that achieved with standard IUI. However, a tendency toward a higher PR in the FSP group wa obained. It is well worth trying before moving on to more invasive and costly approached, such as gamete intrafallopian transfer (GIFT) or in vitro fertilization and embryo transfer (IVF-ET).

43. Utility of laparoscopic uterine myomectomy as a treatment for infertility with no obvious cause except for uterine fibroids.

Abstract

Objectives: Uterine fibroids are capable of causing infertility, but there are no definite criteria for which laparoscopic uterine myomectomy (LM) is known to be beneficial. To investigate the usefulness of LM, we examined pregnancy rates in patients with infertility with no obvious cause except for the presence of uterine fibroids. Materials and Methods: We retrospectively reviewed the clinical records at Suzuki Memorial Hospital between June 2010 and August 2014. We found 60 eligible patients (LM group, 46; non-LM group, 14). The criteria for performing LM were a maximal fibroid diameter of 40 mm or more or the presence of >4 fibroids. Results: The duration of infertility before the first visit was significantly longer in the LM group; although there was no significant difference in the mean patient age and body mass index. Pregnancy was achieved in 45.7% of patients (21/46) in the LM group and 28.6% (4/14) in the non-LM group. There were no pregnancies in patients with >10 fibroids. The postoperative pregnancy rate in the LM group was comparable to previously reported pregnancy rates. Conclusions: Our criteria for performing LM in patients with no obvious cause for infertility except for uterine fibroids seem appropriate, especially when the fibroids are large and the number of fibroids is between 4 and 9. However, our results suggest that the effectiveness of LM is low in patients with 10 or more uterine fibroids. [ABSTRACT FROM AUTHOR]
