- Abstract
Fairbanks Natural Gas (FNG) has been evaluating the need for a new liquefied natural gas (LNG) storage tank since 2012. The new tank is required to support expanded service of natural gas in the greater Fairbanks and North Pole areas of Interior Alaska. Because of the seismic demand, an at-grade mat foundation was selected for this tank over the typical pile supported foundations. LNG is typically stored at temperatures around -160°C and then warmed before passing into service lines. Traditionally, LNG storage tanks constructed on thawed ground include heater systems between the foundation and underlying soil to avoid the generation of permafrost and basal frost heave. In regions with permafrost, LNG storage tanks are generally supported on pile foundations with elevated pile caps that allow for a thermal break between the tanks and soil. The FNG LNG storage tank is one of the first LNG storage tanks constructed on permafrost soils that has been designed as an at-grade foundation. Because the permafrost soils only extend approximately 40 m deep and the site is adjacent to the Tanana River, the aggradation of permafrost would not be acceptable over a 75 year design life. The delicate balance of maintaining warm permafrost was evaluated, modeled, and designed to allow for the construction of an at-grade foundation system. The system balances the cold from the LNG within the storage tank to maintain the permafrost, but not to significantly lower the temperature of the permafrost. - Abstract
Theory: Burnout is prevalent among medical students and is correlated with negative feelings, behaviors, and outcomes. Empathy is a desired trait for medical students that has been correlated with reduced burnout. The concept of guilt is closely related to concern about the well-being of others; therefore, feelings of guilt may be associated with empathy. Excessive guilt poses an increased risk for internalized distress, symptoms such as anhedonia, and may be related to burnout. The relationship between pathogenic guilt and burnout in medical students is unknown., Hypothesis: We hypothesize that pathogenic guilt is present and related to both burnout and empathy in medical students., Methods: We conducted a cross-sectional survey study of all students in one medical school. Data were collected in February 2020. The Oldenburg Burnout Inventory (OBLI), Toronto Empathy Questionnaire (TEQ), and Interpersonal Guilt Questionaire-67 (IGQ-67) were used. A modified version of IGQ-67 was used to measure four subscales of pathogenic guilt: survival guilt, separation guilt, omnipotence guilt, and self-hate guilt. Data analyses for this study including screening, evaluation of assumptions, descriptive statistics, reliabilities, one-way ANOVA, and correlation coefficients, were conducted using SPSS version 26., Results: Of 300, 168 (56.0%) students participated in the study. Survival, omnipotence, and self-hate classes of pathogenic guilt were positively correlated with burnout. Empathy was correlated with two classes of pathogenic guilt: survival and omnipotence. Empathy was inversely related to burnout (disengagement)., Conclusions: Pathogenic guilt may be a contributor to burnout in medical students. Objectives/hypothesis: To determine whether Mallampati class correlates with Cormack-Lehane grade in obese adults, and investigate a novel airway trajectory measurement (ATM) to anticipate difficult laryngoscopy., Study Design: Retrospective cohort plus a pilot study., Methods: One hundred eighty-four nonobese and 160 obese adults underwent laryngoscopy. Spearman correlations, gamma coefficients (G), and Kendall's τ investigated body mass index (BMI):Mallampati, BMI: Cormack-Lehane, and Mallampati:Cormack-Lehane. A z test compared the two groups. Twenty-six volunteers had neck x-rays taken in the sniffing position to examine trajectories to the larynges (ATM)., Results: Positive predictive value of high Mallampati for difficult laryngoscopy was 8.57%. BMI did not correlate with Mallampati (r = 0.055 [nonobese], r = -0.056 [obese]) or Cormack-Lehane [r = -0.014 [nonobese], r = -0.022 [obese]). Among nonobese adults, gamma coefficients for BMI:Mallampati was 0.039 (P = .63), for BMI:Cormack-Lehane was 0.02 (P = .85), and for Mallampati:Cormack-Lehane was 0.43 (P = .004). Among obese adults, gamma coefficients for BMI:Mallampati was -0.127 (P = .16), for BMI:Cormack-Lehane was 0.014 (P = .88), and for Mallampati:Cormack-Lehane was 0.365 (P = .01). Kendall's τ were comparable to gamma coefficients in all analyses. When comparing gamma coefficients for Mallampati:Cormack-Lehane among the nonobese and obese, z = 0.04 (P = .98). In the ATM study, only Mallampati and upper lip bite test had a significant relationship (G = 1.00, P < .001)., Conclusions: Mallampati correlates poorly with Cormack-Lehane, regardless of BMI.
