Item does not contain fulltext The objective of this randomized clinical trial was to compare the clinical and technical results of digital preoperative planning for primary total hip arthroplasties with analogue planning. Two hundred and ten total hip arthroplasties were randomized. All plans were constructed on standardized radiographs by the surgeon who performed the arthroplasty the next day. The main outcome was accuracy of the preoperative plan. Secondary outcomes were operation time and a radiographic assessment of the arthroplasty. Digital preoperative plans were more accurate in planning the cup (P < .05) and scored higher on the postoperative radiologic assessment of cemented cup (P = .03) and stem (P < .01) components. None of the other comparisons reached statistical significance. We conclude that digital plans slightly outperform analogue plans.

Item does not contain fulltext, The function and survival time of unicompartmental knee prostheses for patients with severe gonarthrosis have been improved the past few years by developments in their design, the instrumentarium and the surgical technique. A medial unicompartmental knee prosthesis may be indicated in patients with arthrosis of the medial tibiofemoral compartment. The prerequisites are an intact anterior cruciate ligament, an intact lateral compartment, a correctable varus axis and sufficient flexion in the knee. Contraindications are inflammatory arthropathies and a recent episode of septic arthritis. Relative contraindications are: old age, excess body weight, patellofemoral arthrosis and chondrocalcinosis. A unicompartmental knee prosthesis can be placed via a small parapatellar incision. The postoperative recovery is more rapid than following the classical open approach, while the knee function after 5 years is comparable. The knee function also seems better following a medial unicompartmental knee prosthesis than after valgusising tibial head osteotomy. The latter is still preferred for young active patients. Medial unicompartmental knee prostheses fail in 6-8% of patients. Revision to a total knee prosthesis is then the treatment of choice. In the long term, unicompartmental prostheses with a mobile bearing become loose less often than comparable prostheses with a fixed bearing.

6. First clinical experience with a growing endoprosthesis, a limb saving procedure in children

Abstract

Purpose of the study: To perform the first clinical study with an extendable endoprosthesis that can be extended non-invasively for children with a malignant bone tumor in the leg. Method: A 14-year old boy had an osteosarcoma at the distal metaphysis of the femur. The patient was first treated successfully by chemotherapy (cisplatin, doxorubicin, ifosfamide and high dose methotrexate). The operation followed a few weeks after chemotherapy courses. Enough muscle tissue could be preserved. Resection was followed by reconstruction with a new extendable modular endoprosthetic system. The growing endoprosthesis is powered magnetically. An electromagnet, rotating outside the leg, produces a magnetic field that causes rotation of a small permanent magnet in the prosthesis. The magnet drives a motion screw via a gearbox. When the motion screw rotates the inner and outer tube of the prosthesis are forced apart. Results: Seven months after the operation there was a leg length discrepancy of 20 mm. The first extension was performed and resulted in 2 mm growth. Anaesthesia was not necessary. X-rays prior to and after the lengthening procedure were taken to demonstrate the increase in length of the endoprosthesis. Six weeks later the second extension of 5 mm was performed successfully. Further extensions of 5 mm will be repeated every one or two months to adjust the discrepancy in length of both legs. Conclusions: The first clinical experience with a growing endoprosthesis seems to be successful. The patient regained almost normal functioning of his leg. Future extensions will prevent a leg length difference.

Abstract

In this study, we followed-up the family with bilateral hereditary micro-epiphyseal dysplasia (BHMED) originally described by Elsbach [1959: J Bone Joint Surg [Br] 41-B:514–523]. Clinical re-examination of all available family members resulted in further delineation of the clinical and radiological phenotype, which is distinct from common multiple epiphyseal dysplasia (MED). Linkage analysis excluded EDM1, EDM2, and EDM3 as candidate genes. Linkage and mutation analysis of matrilin-3 (MATN-3) revealed a new pathogenic mutation confirming that BHMED is indeed a distinct disease entity among MED and MED-like disorders. © 2003 Wiley-Liss, Inc.
